A cardiomiopatia hipertrófica (CMH) é uma forma de doença do músculo cardíaco de causa genética, caracterizada pela hipertrofia das paredes ventriculares. O diagnóstico requer detecção por métodos de imagem (Ecocardiograma ou Ressonância Magnética Cardíaca) de qualquer segmento da parede do ventrículo esquerdo com espessura > 15 mm, sem outra causa provável. A análise genética permite identificar mutações de genes codificantes de diferentes estruturas do sarcômero responsáveis pelo desenvolvimento da CMH em cerca de 60% dos casos, permitindo o rastreio de familiares e aconselhamento genético, como parte importante do manejo dos pacientes e familiares. Vários conceitos sobre a CMH foram recentemente revistos, incluindo sua prevalência de 1 em 250 indivíduos, não sendo, portanto, uma doença rara, mas subdiagnosticada. A vasta maioria dos pacientes é assintomática. Naqueles sintomáticos, a obstrução do trato de saída do ventrículo esquerdo (OTSVE) é o principal distúrbio responsável pelos sintomas, devendo-se investigar a sua presença em todos os casos. Naqueles em que o ecocardiograma em repouso ou com Manobra de Valsalva não detecta gradiente intraventricular significativo (> 30 mmHg), devem ser submetidos à ecocardiografia com esforço físico para detecção da OTSVE. Pacientes com sintomas limitantes e grave OTSVE, refratários ao uso de betabloqueadores e verapamil, devem receber terapias de redução septal ou uso de novas drogas inibidoras da miosina cardíaca. Por fim, os pacientes adequadamente identificados com risco aumentado de morta súbita podem receber medida profilática com implante de cardiodesfibrilador implantável (CDI).
La miocardiopatía hipertrófica (MCH) es una forma de enfermedad cardíaca de origen genético, caracterizada por el engrosamiento de las paredes ventriculares. El diagnóstico requiere la detección mediante métodos de imagen (Ecocardiograma o Resonancia Magnética Cardíaca) que muestren algún segmento de la pared ventricular izquierda con un grosor > 15 mm, sin otra causa probable. El análisis genético permite identificar mutaciones en genes que codifican diferentes estructuras del sarcómero responsables del desarrollo de la MCH en aproximadamente el 60% de los casos, lo que permite el tamizaje de familiares y el asesoramiento genético, como parte importante del manejo de pacientes y familiares. Varios conceptos sobre la MCH han sido revisados recientemente, incluida su prevalencia de 1 entre 250 individuos, por lo tanto, no es una enfermedad rara, sino subdiagnosticada. La gran mayoría de los pacientes son asintomáticos. En los casos sintomáticos, la obstrucción del tracto de salida ventricular izquierdo (TSVI) es el trastorno principal responsable de los síntomas, y su presencia debe investigarse en todos los casos. En aquellos en los que el ecocardiograma en reposo o la maniobra de Valsalva no detecta un gradiente intraventricular significativo (> 30 mmHg), deben someterse a ecocardiografía de esfuerzo para detectar la obstrucción del TSVI. Los pacientes con síntomas limitantes y obstrucción grave del TSVI, refractarios al uso de betabloqueantes y verapamilo, deben recibir terapias de reducción septal o usar nuevos medicamentos inhibidores de la miosina cardíaca. Finalmente, los pacientes adecuadamente identificados con un riesgo aumentado de muerte súbita pueden recibir medidas profilácticas con el implante de un cardioversor-desfibrilador implantable (CDI).
Hypertrophic cardiomyopathy (HCM) is a form of genetically caused heart muscle disease, characterized by the thickening of the ventricular walls. Diagnosis requires detection through imaging methods (Echocardiogram or Cardiac Magnetic Resonance) showing any segment of the left ventricular wall with a thickness > 15 mm, without any other probable cause. Genetic analysis allows the identification of mutations in genes encoding different structures of the sarcomere responsible for the development of HCM in about 60% of cases, enabling screening of family members and genetic counseling, as an important part of patient and family management. Several concepts about HCM have recently been reviewed, including its prevalence of 1 in 250 individuals, hence not a rare but rather underdiagnosed disease. The vast majority of patients are asymptomatic. In symptomatic cases, obstruction of the left ventricular outflow tract (LVOT) is the primary disorder responsible for symptoms, and its presence should be investigated in all cases. In those where resting echocardiogram or Valsalva maneuver does not detect significant intraventricular gradient (> 30 mmHg), they should undergo stress echocardiography to detect LVOT obstruction. Patients with limiting symptoms and severe LVOT obstruction, refractory to beta-blockers and verapamil, should receive septal reduction therapies or use new drugs inhibiting cardiac myosin. Finally, appropriately identified patients at increased risk of sudden death may receive prophylactic measure with implantable cardioverter-defibrillator (ICD) implantation.

ABSTRACT Harvest delay can alter the physiological and sanitary quality of sorghum seeds due to the longer exposure time of seeds to adverse environmental conditions in the field. In this study, we investigated the impact of delayed Sorghum sp. harvest on the physiological and sanitary quality of seeds in four hybrids grown in the tropical soil of the Brazilian Cerrado region. Plants from four Sorghum sp. hybrids (IPA SUDAN 4202, BRS 658, BRS 810, and BRS 373) were grown and submitted to four seed harvest periods (0, 7, 14, and 21 days after stage 9 - seed physiological maturity). The treatments were arranged in a completely randomized block design in a 4 × 4 factorial, with four replicates. At each harvest period, the thousand-seed weight, water content, first count of the germination test, germination, seedling emergence, emergence speed index, seedling dry matter, accelerated aging, electrical conductivity, and sanitary quality of seeds were determined. Seeds of higher physiological and sanitary quality of the IPA SUDAN 4202 and BRS 658 sorghum hybrids can be obtained when the harvest is carried out up to 7 days after the physiological maturity stage. However, the delay in seed harvesting at 14 and 21 days after physiological maturity results in a higher incidence of Colletotrichum sp., Curvularia sp., Rhizoctonia sp., and Alternaria sp. on the seeds of these hybrids. Abstract Background Patients with psoriatic arthritis (PsA) experience reduced physical function and impaired quality of life. Better patient-reported functional outcomes are found when lower disease activity is achieved. Objectives To evaluate the variation of physical function by HAQ-DI over time in PsA patients treated with standard therapy in a real-life setting: to verify predictors of achieving a minimum clinically important difference (MCID) in function by HAQ-DI (ΔHAQ-DI ≤ − 0.35) and to measure the impact of achieving REM/LDA on long-term function by HAQ-DI. Methods This is a longitudinal analysis of a real-life retrospective cohort. Data from PsA patients with at least 4 years of follow-up in the PsA clinic from 2011 to 2019 were extracted from electronic medical records. The variations of physical function by HAQ-DI and disease activity by DAPSA over time were calculated. A multivariate hierarchical regression model was applied to verify predictors of MCID in HAQ-DI. A comparison of HAQ-DI variation between patients with DAPSA REM, LDA, moderate and high disease activity was made using the generalized estimating equation model (GEE), adjusted by Bonferroni test. The Spearman correlation method was applied to verify the correlation of ΔDAPSA and ΔHAQ-DI over time. Statistical analysis was performed in SPSS program version 21.0. Results Seventy-three patients were included in the analysis. Physical function measured by HAQ-DI was determined by PsA disease activity measured by DAPSA (p < 0.000). A moderate and statistically significant correlation between ΔDAPSA and ΔHAQ-DI was observed (rs = 0.60; p < 0.001). Only patients in DAPSA REM demonstrated a constant decline in HAQ-DI scores during the follow-up. White ethnicity and older age at baseline were predictors for not achieving MCID in HAQ-DI [RR 0.33 (0.16–0.6795% CI p = 0.002) and RR 0.96 (0.93–0.9895% CI p < 0.000), respectively, while higher scores of HAQ-DI at baseline were predictors of achieving MCID [RR 1.71 (1.12–2.6095%CI p = 0.013)]. Conclusion In PsA, patients who maintained DAPSA REM/LDA over time had better long-term functional outcomes. Higher HAQ-DI scores at baseline, non-white ethnicity and younger age were predictors for achieving a clinical meaningful improvement of HAQ-DI. ABSTRACT Flooding periods directly interfere with the availability of oxygen in the soil, affecting the aerobic processes of the plants. In this study, we aimed to: (i) characterize the effects of water excess soil on cassava growth; (ii) identify management practices that reduce the impact of water excess on cassava yield; and (iii) to know the crop potential yield for the study region in order to quantify the yield loss caused by water excess. The effects of water excess were evaluated through experiments in pots and in the field. The physiological and morphological response of cassava plants under water excess in a controllable environment indicated greater damage to cassava after exposure of 96 and 120 hours of water excess. Every 24 hours of water excess, about 20% of the plants did not emerge and, after 96 hours of excess, the emergence was null. The use of raised seedbeds was a management practice that ensured 62.6% of plant establishment, allowing cassava yields of up to 16.9 Mg ha-1. The potential yield to Santa Maria in the 2018/2019 crop year was 43.9 Mg ha-1 of cassava roots. Resumo Realizou-se este estudo com o objetivo de caracterizar a produção e a digestão anaeróbia de dejetos de suínos em terminação submetidos a restrição alimentar ou suplementados com ractopamina ou cromo. Os dejetos foram provenientes de 50 suínos machos castrados, em fase de terminação, com ± 154 dias de idade, com peso inicial de 99,0 ± 4,4 kg e final de 117,2 ± 5,8 kg. As dietas experimentais foram: controle (dieta convencional); restrição qualitativa (redução de 7,5% de energia líquida em relação à dieta controle); restrição quantitativa (redução de 15% no fornecimento de ração); cromo (0,8 mg); e ractopamina (10 ppm). Os dados foram submetidos à análise de variância por meio do delineamento em blocos ao acaso, no qual as semanas de análises foram consideradas como blocos (cofator). Não foram observadas diferenças entre as dietas nas produções de dejetos na matéria natural (MN), matéria seca (MS), matéria mineral (MM) e matéria orgânica (MO). Animais que receberam a dieta controle apresentaram o maior coeficiente de resíduo e não houve diferença entre as demais dietas. Não foram observadas diferenças entre as dietas para sólidos totais, pH e nitrogênio total dos afluentes e efluentes. O maior rendimento de biogás (574 mL g-1 de SV adicionados) foi obtido com os digestores abastecidos com dejetos de animais alimentados com dieta qualitativamente restrita. Abstract The aim of this study was to characterize the production and anaerobic digestion of manure from finishing pigs subjected to feed restriction or supplemented with ractopamine or chromium (Cr). The waste came from 50 barrows in the finishing phase, aged ± 154 days, with a starting weight of 99.0 ± 4.4 kg and a final weight of 117.2 ± 5.8 kg. The experimental diets were as follows: control (conventional diet), qualitative restriction (7.5% reduction in net energy compared to the control diet), quantitative restriction (15% reduction in feed supply), Cr (0.8 mg), and ractopamine (10 ppm). The data were subjected to an analysis of variance using a randomized block design, in which the weeks of analysis were considered blocks (cofactors). There were no differences in manure production between the diets regarding natural matter (NM), dry matter (DM), mineral matter (MM), or organic matter (OM). Animals fed the control diet had the highest residue coefficient, and there was no difference among the other diets. No differences were observed among the diets regarding total solids, pH, or total nitrogen in the tributaries or effluents. The highest biogas yield (574 mL g-1) of added volatile solids (VS) was obtained in the digesters supplied with manure from animals fed a qualitatively restricted diet. Resumo Realizou-se este estudo com o objetivo de avaliar fontes de minerais e de níveis do óleo de alecrim na dieta de poedeiras comerciais sobre as características qualitativas e quantitativas dos dejetos gerados. Os dejetos foram provenientes de 288 poedeiras comerciais da linhagem HyLine Brown (poedeiras semipesadas) com 30 semanas de idade, durante o período de 112 dias (4 ciclos de 28 dias). Foi utilizado delineamento inteiramente casualizado com esquema fatorial 2x3 com medidas repetidas no tempo (16 repetições), sendo duas fontes minerais (inorgânica e orgânica) e três níveis de óleo de alecrim (0, 100 e 200 mg kg-1). Semanalmente os dejetos foram coletados, pesados e realizados os cálculos de produção de dejetos, coeficiente de resíduo (Cr) e as análises de sólidos totais (ST), sólidos voláteis (SV), pH, macro e microminerais. As fontes de minerais afetaram as características qualitativas dos dejetos. Os níveis de óleo de alecrim afetaram as características quantitativas e qualitativas do dejeto gerado. Concluiu-se que dietas contendo mineral orgânico resultam em dejetos com menor poder poluente por apresentar menores valores de ST, pH, N total e P total. O óleo de alecrim aumenta a produção de dejetos na MN e aumenta os valores de ST e SV nos dejetos das poedeiras causando maior impacto ambiental. Abstract This study aimed to evaluate the effects of mineral sources and rosemary oil levels in the diet of commercial layers on the qualitative and quantitative characteristics of the waste generated. Manure was obtained from 288 commercial laying hens of the HyLine Brown lineage (semi-heavy layers) at 30 weeks of age over a period of 112 d (four cycles of 28 d). A completely randomized design was used with a 2×3 factorial scheme with repeated measures over time (16 repetitions), with two mineral sources (inorganic and organic) and three levels of rosemary oil (0, 100, and 200 mg kg-1). The waste was collected weekly, weighed, and the calculations of waste production, residue coefficient (RC) were performed. Also analyses of total solids (TS), volatile solids (VS), pH, macro-, and micro-minerals were performed. Mineral sources affect the quality characteristics of the manure. The amount of rosemary oil affected the quantitative and qualitative characteristics of the waste generated. It was concluded that diets containing organic minerals resulted in waste with less polluting power, as it had a lower TS, pH, total N, and total P values. RESUMO: O objetivo deste estudo foi avaliar a estabilidade no processo de digestão anaeróbia e a produção de biogás dos dejetos de poedeiras alimentadas com dietas contendo diferentes fontes de minerais e níveis de óleo de alecrim. Os dejetos foram provenientes de 288 poedeiras semipesadas, com 30 semanas de idade, colhidos por 112 dias. Utilizou-se o delineamento inteiramente casualizado com esquema fatorial 2x3, com medidas repetidas no tempo, sendo as dietas com duas fontes minerais (inorgânica e orgânica) e três níveis de óleo de alecrim (0, 100 e 200 mg kg-1). Foram utilizados 12 biodigestores contínuos (7,5 kg), operados com tempo de retenção hidráulica de 30 dias, com cargas diárias com 5% de sólidos totais, por 90 dias. Foram realizadas análises semanais do afluente e efluente mensurando-se os valores de sólidos totais (ST), sólidos voláteis (SV), pH, nitrogênio amoniacal (N amoniacal), alcalinidade parcial (AP), alcalinidade intermediária (AI) e a relação AP:AI; quinzenalmente análises das concentrações de nitrogênio total (N) e de fósforo total (P) e diariamente mensuradas as produções de biogás para calcular o potencial de produção de biogás do dejeto. Dejetos de poedeiras alimentadas com minerais orgânicos apresentam maior potencial de produção de biogás, maiores reduções de ST e SV em relação àquelas alimentadas com minerais inorgânicos. A adição de óleo de alecrim na dieta de poedeiras não oferece riscos de falência para o processo de digestão anaeróbia. ABSTRACT: This study evaluated the stability of anaerobic digestion and biogas production from the manure of laying hens fed diets containing different mineral sources and rosemary oil concentrations. Manure was obtained from 288 semi-heavy laying hens, 30 weeks old, and collected for 112 days. A completely randomized design was used in a 2×3 factorial scheme with repeated measures, considering two mineral sources (inorganic and organic) and three concentrations of rosemary oil (0, 100, and 200 mg kg-1). Twelve continuous bench digesters (7.5 kg) were operated with a hydraulic retention time of 30 days and daily loads of 5% total solids, for 90 days. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Resumo A vacina covid-19 confere proteção direta, reduz as taxas de transmissão do vírus e de novas variantes. No Brasil, estão liberadas para a população pediátrica as vacinas Pfizer/BioNTech e a CoronaVac, ambas seguras, eficazes e imunogênicas. Pacientes pediátricos com síndrome nefrótica e covid-19 têm curso clínico regular sem complicações relacionadas ao uso de esteroides ou vacinas. Esses pacientes, com ou sem imunossupressão, não apresentam maior risco de covid-19 grave e o tratamento com esteroides é seguro. Os pacientes com doença renal crônica têm covid-19 mais leve, sem necessidade de hospitalização. A resposta vacinal pode ser reduzida e/ou a duração dos anticorpos pós-vacinação pode ser menor do que na população geral. Entretanto, a vacina covid-19 está recomendada, considerando o risco de exposição. Acredita-se que pacientes com síndrome hemolítico-urêmica teriam maior risco de covid-19 grave. A vacina é recomendada, embora dados específicos sobre segurança e eficácia da vacina covid-19 sejam limitados. Há concordância que os benefícios da imunidade induzida superam quaisquer riscos da imunização. A vacina covid-19 é recomendada para crianças e adolescentes candidatos ao transplante renal ou já transplantados. Esses pacientes têm resposta imunológica reduzida após a vacina, entretanto ela é recomendada porque os benefícios superam qualquer risco dessa vacinação. A recomendação atual no Brasil é a vacina de tecnologia RNA mensageiro. ABSTRACT The covid-19 vaccine confers direct protection and reduces transmission rates of the virus and new variants. Vaccines from Pfizer/BioNTech and CoronaVac have been cleared for children in Brazil. They are safe, effective, and immunogenic. There are no known complications associated with the use of steroids or vaccines in pediatric patients with covid-19 and nephrotic syndrome. With or without immunosuppression, these patients are not at increased risk of severe covid-19, and steroids are safe for them. A milder form of covid-19 occurs in patients with chronic kidney disease without the need for hospitalization. The vaccine response may be reduced and/or the duration of antibodies after vaccination may be shorter than in the general population. However, considering risk of exposure, vaccination against covid-19 is recommended. It is believed that patients with hemolytic-uremic syndrome are at higher risk of severe covid-19. Vaccination is recommended, although specific data on the safety and efficacy of the covid-19 vaccine are limited. There is agreement that the benefits of induced immunity outweigh the risks of immunization. Vaccination against covid-19 is recommended for children and adolescents needing kidney transplantation or who have undergone transplantation. These patients present decreased immune response after vaccination, but immunization is recommended because the benefits outweigh the risks of vaccination. Current recommendations in Brazil stipulate the use of the messenger RNA vaccine. Serão analisadas ações judiciais contra uma operadora da saúde suplementar em Belo Horizonte, entre os anos de 2010 e 2017. Analisaramse proces¬sos judiciais por meio de técnica de análise documental. As variáveis foram relativas à natureza do processo judicial, ao perfil dos beneficiários e às características das demandas. A Regressão de Poisson foi utilizada na avaliação de impacto e relevância das variáveis selecionadas, e o software R versão 3.6.1 para os testes de significância. No período de 2010 a 2017, foram movidas 6.090 ações. As principais causas são questões contratuais, negativa de procedimento, órtese/prótese e exames. Planos anteriores à ‘Lei dos Planos de Saúde’ correspondem a 3% da carteira e 37,4% da judicialização. Este estudo demonstrou que a possibilidade de judicializar é maior entre clientes masculinos, contratos individuais, planos assistidos em rede ampla, sem coparticipação. A judicialização é mais acessível a cidadãos de maior condição econômica. Questões contratuais evidenciam frágil regulação. Adequada regulamentação reduz o desequilíbrio entre clientes e operadoras. A Agência Nacional de Saúde Suplementar precisa exercer sua função reguladora.

This study showed that the possibility of taking legal action is greater among male clients, individual contracts, plans assisted in a wide network, without co-participation. Judicialization is more accessible to citizens of higher economic status. Contractual issues show fragile regulation. Adequate regulation reduces the imbalance between customers and operators. The National Supplementary Health Agency needs to exercise its regulatory function.

Abstract Cancer is the leading cause of death. Melanoma skin cancer originates in melanocytes and represents 80% of the deaths associated with skin cancer. Vinblastine (VIN) is a chemotherapeutic agent used in the treatment of cancer through disrupting mitotic spindle and tumor development. Curcumin (CUR), a compound extracted from the rhizomes of the Curcuma longa plant, has beneficial effects in preventing the development and progression of cancer while modulating the immune response and oxidative stress. The expression of purinergic receptors, ecto-enzymes, and adenosine can modulate the inflammatory responses in cancer. The activity of enzymes, the markers of cell damage in oxidative stress, the generation of reactive oxygen species (ROS), and the activities of ecto-enzymes in the melanoma cell line were investigated. The human melanoma cell line was treated with curcumin (40 µM), vinblastine (VIN) (20 nM), or a combination of both for 24h. Oxidative stress enzymes and byproducts were measured and compared against the activity of ecto-enzymes. There was a marked increase in ROS production in all groups, but an increase in protein carbonylation was only detected in the VIN group. CUR had an inhibitory effect on extracellular ADP hydrolysis, as evidenced by a significant decrease in ADP substrate removal. VIN possibly increased adenosine formation, as demonstrated by an increase in ADP substrate removal. VIN (alone or in combination with CUR) reduced the activity of ADA, thus increasing the concentration of adenosine in the tumor microenvironment. CUR increased ROS generation in melanoma cells and disrupted the purinergic signaling cascade. Therefore, it may be a promising adjuvant therapy for melanoma, a cancer with a high incidence and lethality.

Abstract In this study we demonstrated the inactivation of E. coli, C. difficile, meshophilic bacteria and Salmonella spp. through Pulsed Electric Field (PEF) technology. First, a simulation using Finite Element Method Magnetics Mathematical Modeling was used to obtain the electric field values to be used in the electro-pasteurization tunnel. Then, practical tests were carried out by using an industrial scale apparatus set with the following parameters: pulse 20 s – 40 s, 40, 80 and 450 kV, radio frequency of 350 kHz and treadmill speed at 10 m/min. The results from practical tests shows a complete elimination of all microrganisms, thus proving that PEF technology significantly contributes to food safety and can be used on an industrial scale.
