RESUMO O desastre-crime do petróleo em 2019 é tido como o maior desastre ocorrido no litoral do Brasil. Considerada a primeira linha de enfrentamento de desastres no território, a gestão municipal de saúde é fundamental no processo de gestão de risco de desastre. Objetivou-se analisar a atuação de uma secretaria municipal de saúde da região metropolitana do Recife, Pernambuco, em relação ao derramamento do petróleo ocorrido em 2019. Foi realizado um estudo de caso do tipo descritivo com abordagem qualitativa. Foi realizada Análise de Conteúdo, com utilização do software IRaMuTeQ, das entrevistas com gestores da saúde, cujos resultados revelaram quatro categorias: gestão municipal na atenção com o mar; governança de risco de desastre e suas ausências; sindemia da covid-19 e impactos na reabilitação e recuperação das comunidades afetadas pelo desastre do petróleo; o que fazer se um desastre voltar a acontecer. ABSTRACT The 2019 oil disaster-crime is considered the largest disaster on the Brazilian coast. Given the frontline in disaster response, the municipal health secretariat is crucial in the disaster risk management process. This study aimed to analyze the performance of a municipal health secretariat in the metropolitan region of Recife, Pernambuco, regarding the 2019 oil spill, adopting a descriptive, qualitative case study. Abstract The aim of this study was to analyze the expression of mast cell markers toluidine blue, c-kit, and tryptase and presence of mononuclear inflammatory cells in oral lichen planus (OLP) and oral lichenoid lesions related to dental amalgam. Nineteen specimens of OLP, OLLC, and healthy oral mucosa were selected. Mononuclear inflammatory cells were analyzed. Histochemical and immunohistochemical analyses were performed using toluidine blue, anti-c-kit and anti-tryptase reagents, and the results were quantified in areas A and B of connective tissue. Mast cells of all OLP and OLLC samples were positive for toluidine blue, c-kit, and tryptase. The density of toluidine blue+, c-kit+ and tryptase+ mast cells was higher in tissue with OLP and OLLC compared with healthy controls (p < 0.05). No difference was noted in mast cells density between OLP and OLLC (p > 0.05). The density of tryptase+ mast cells was higher in the subepithelial region (area A) than the region below it (Area B) in OLLC (p = 0.047). The mononuclear inflammatory cell density was higher in OLLC compared to OLP, but without statistical significance (p > 0.05). A positive statistical correlation was found between mononuclear immune cells and density of c-kit+ and tryptase+ mast cells in OLP (r = 0.943 and r = 0.886, respectively). Our data demonstrate that the etiopathogenesis process of OLP and OLLC modulates the expansion and degranulation of mast cells; mast cells density, however, was similar between OLP and OLLC. ABSTRACT This study aimed to analyze the actions taken by health management to deal with the criminal disaster of an oil spill off the coast of Pernambuco in 2019. It is a case study investigating the health sector’s actions in four municipalities in Pernambuco. Data was collected through interviews with 16 health department managers (municipal and state), using a semi-structured script with questions about performance, impacts, actions taken, difficulties, challenges, and lessons learned. The Collective Subject Discourse analysis revealed four thematic axes: a) managers’ perception of the impacts of the crime disaster; b) actions taken in the process of risk management of the disaster crime by the health sector; c) difficulties faced in the process of mitigating the disaster; d) evaluation of management actions during the disaster crime and future preparation. Weaknesses in health management were identified in disaster mitigation actions due to the need for more preparation of health services and professionals. The lack of instruments and coordination between the federated entities intensified the difficulties, generating calamity in the territory. This points to the need to build instruments and protocols to guide such actions in these situations so that activities can be carried out more efficiently and effectively.
RESUMO Este estudo objetivou analisar as ações desenvolvidas pela gestão de saúde no enfrentamento do desastre-crime de derramamento de petróleo na costa pernambucana em 2019. Trata-se de estudo de caso que investigou a atuação do setor saúde em quatro municípios de Pernambuco. A coleta de dados foi mediante entrevistas com 16 gestores de secretarias de saúde (municipal e estadual), utilizando roteiro semiestruturado com perguntas sobre atuação, impactos, ações desenvolvidas, dificuldades, desafios e lições aprendidas. A análise do Discurso do Sujeito Coletivo evidenciou quatro eixos temáticos: a) percepção dos gestores sobre os impactos do desastre-crime; b) ações desenvolvidas no processo de gestão de risco do desastre-crime pelo setor saúde; c) dificuldades enfrentadas no processo de mitigação do desastre; d) avaliação das ações da gestão durante o desastre-crime e preparação futura. Identificaram-se fragilidades da gestão em saúde nas ações de mitigação do desastre, resultado da falta de preparo dos serviços e dos profissionais de saúde. A falta de instrumentos e a desarticulação entre os entes federados intensificaram as dificuldades, gerando situações de calamidade no território. Isso aponta para a necessidade de construção de instrumentos e protocolos que guiem tais ações nessas situações, para que as atividades sejam desenvolvidas com mais eficiência e eficácia.

Abstract We investigated the Biogran on bone repair and metabolism at several time-intervals upon grafting into rat tibia artificial defects. The biomaterial was thoroughly characterized in vitro, and its dissolution behavior upon immersion was assessed in simulated body fluid (SBF) solution for 1, 3, 7, 14, and 30 days. Biogran was also assessed by in vitro hydroxyapatite formation in SBF solution, which is a marker for bioactive behavior. In vivo, distal and proximal bone defects were performed in the Wistar rat's right tibia and filled according to the experimental groups: I) negative control, no filling; II) positive control, 10 mg of autogenous bone; and III) 10 mg of Biogran. Animals were euthanized at 1, 2, 3, 4, 7, and 10 weeks. Bone neoformation was analyzed using histomorphometry (proximal defect), and local levels of bone morphogenetic protein 2 (BMP-2) were measured using the ELISA assay (distal defect). In vitro, the Biogran sample showed a fast dissolution rate within the first 7 days, parallel to the formation of the hydroxyapatite layer. In vivo, the sample was progressively resorbed at a higher rate within the first month until it became almost absent at week 10th. The sample presented similar or higher bone neoformation concerning the autogenous bone. BMP-2 levels were sustained in the Biogran group (around 200 pg/mg) and detected until the last experimental time with a significant difference compared with the controls. Resumo O semiárido brasileiro é o novo destino do capital global em energias renováveis, diante da crise ambiental. Esse fato traz como consequência a expansão das bases da acumulação de capital: extração de mais-valor da força de trabalho, apossamento de terras dos camponeses e concentração fundiária. Para discutir essa problemática, os objetivos do presente texto são: a) apontar as novas particularidades da questão agrária ante a expansão dos parques eólicos; b) analisar o processo de desapossamento de terras dos agricultores; e c) apresentar a relação dos contratos de arrendamento com a renda da terra. Para isso, realizamos uma revisão de literatura sobre os temas da privatização dos bens comuns, em particular das terras para a produção de energia eólica, buscando explicar como a privatização sobre bens comuns, particularmente a força dos ventos, implica estratégias legais de redução de direitos e de serviços básicos a direitos exclusivos de propriedade privada. A “acumulação por despossessão”, conceito cunhado por David Harvey (2011) e aplicado por Mariana Traldi (2019) na análise da expansão da energia eólica no Brasil, implica apropriação pelo capital de extensas áreas a pretexto de se assegurar o “desenvolvimento sustentável”, o combate à “crise hídrica” ou uma “agenda verde”. Abstract The leasing of land for wind power production is the subject analyzed in this work, precisely the control exercised by multinationals of the exploration rights from peasants’ lands. Taking in consideration that the semi-arid region of Brazil is the new destination for the global capital in renewable energies, given the environmental crises, the research presents its relevance for the study of wind farms. The main problem is that this expansion copies the foundations of the primitive accumulation of capital: extraction of surplus value from the labor force, land grabbing of peasants and land concentration. The goals go from pointing out the new singularities of the agrarian situation in face of the expansion of wind farms to analyze the process of dispossession of land from farmers and to show the link between lease contracts and land profit. Abstract Introduction The voice and hearing can be affected to different degrees by aging, which can cause communication difficulties for elderly people. Vocal production requires effective temporal auditory processing at central levels within the nervous system, which can be compromised by the aging process. Objective To analyze the correlation between voice and temporal auditory processing in older adults. Materials and Methods A total of 40 elderly people aged 60 years or older were subdivided into 2 groups according to the presence or absence of vocal symptoms measured by the Voice Symptom Scale. All of the participants were submitted to auditory temporal tests, vocal self-assessment, and acoustic and perceptual auditory analyses of voice. Results Most of the subjects assessed had decreased voice intensity and normal variability in terms of vocal quality. The performance was normal in the Pitch Pattern Sequence test and altered in the Random Gap Detection test. In the Masking Period Pattern test, the detection thresholds for the target signal were increased in the presence of masking in different temporal target signal positions. Only pitch differed between the two groups. There were differences between the genders regarding frequency, shimmer, the overall severity of the alteration, and roughness. There was a correlation regarding temporal resolution ability and the overall severity of the alteration and roughness of the voice. RESUMO Objetivos: Analisar a percepção dos enfermeiros sobre os métodos de trabalho em enfermagem no contexto hospitalar. Métodos: Estudo descritivo com abordagem qualitativa, realizado num hospital do norte de Portugal, com participação de 17 enfermeiros. Como instrumento de coleta de dados, recorreu se à entrevista semiestruturada. Os dados, recolhidos entre maio e junho de 2023, passaram por análise de conteúdo, com suporte do software Atlas.ti. Resultados: Emergiram três áreas temáticas: “Métodos de trabalho dos enfermeiros em contexto hospitalar”, destacando-se a concepção e componentes dos métodos de trabalho e os métodos em uso; “Implementação dos métodos de trabalho”, salientando-se fatores influenciadores e desafios à implementação; e “Impacto dos métodos de trabalho dos enfermeiros para os pacientes, enfermeiros e instituições”. ABSTRACT Objectives: To analyze nurses’ perspectives on nurses’ work methods in the hospital context. Methods: A descriptive study with a qualitative approach was conducted in a hospital in northern Portugal, involving 17 nurses. Semi-structured interviews were used for data collection. Data collected between May and June 2023 underwent content analysis, supported by Atlas.ti software. Results: Three thematic areas emerged: “Nurses’ work methods in a hospital context,” highlighting the conception and components of work methods and the methods in use; “Implementation of nurses’ work methods,” emphasizing influencing factors and challenges to implementation; and “Impact of nurses’ work methods on patients, nurses, and institutions.” Final Considerations: Nurses’ work methods constitute the structure of nursing care. The Behavior of Probable Sleep Bruxism, Anxiety and Need for Orthodontic Treatment in Adolescents Bruxism

ABSTRACT Objective: To determine the prevalence of the need for orthodontic treatment, anxiety, and probable sleep bruxism and its association in adolescents. Material and Methods: A cross-sectional study was conducted with 294 adolescents aged between 11 and 16 years. Orthodontic treatment need was determined using the Dental Health Component of the Index of Orthodontic Treatment Needs (IOTN-DHC). Anxiety symptoms were assessed using the Brazilian version of the Multidimensional Anxiety Scale for Children (MASC). The probable sleep bruxism was identified based on the American Academy of Sleep Medicine (AASM) questionnaire. Descriptive data analyses were performed. Simple logistic regression models were applied between each independent variable and the outcome (anxiety score). Variables with p<0.20 in the individual (raw) analyses were studied in a multiple logistic regression model, with p≤0.05 remaining in the final model. Based on the regression models, the prevalence ratios were estimated with the respective 95% confidence intervals. Results: 68.7% of the adolescents had probable sleep bruxism, and 35.4% had a moderate or severe normative need for orthodontic treatment. Adolescents aged up to 12 years (OR=1.82; CI: 1.10-3.02), females (OR=2.67; CI: 1.64-4.34), and with a moderate or severe need for orthodontic treatment according to the IOTN-DHC (OR=1.76; CI: 1.06-2.90), are more likely to have a higher anxiety score. The prevalence of adolescents with a moderate or severe need for normative orthodontic treatment by the IOTN-DHC is 35.4% (95%CI: 29.9-40.8%), while the perceived need for treatment by the IOTN-AC is 14.0% (95%CI: 10.0-17.9%). Adolescents with a high degree of anxiety were more likely to have probable sleep bruxism (OR=3.64; CI: 1.06-12.50). Conclusion: Female adolescents up to 12 years of age and with a moderate or severe need for orthodontic treatment are more likely to have higher levels of anxiety; adolescents with a high degree of anxiety are more likely to have probable sleep bruxism. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. Abstract Background: Medications currently used to treat pain are frequently associated with serious adverse effects and rapid development of tolerance. Thus, there is a need to develop more effective, and safer medicines for the population. Blocking NMDA receptors (NMDAR) has shown to be a promising target for the development of new drugs. That statement is due to NMDAR activation and glutamate release in the spinal cord which affects chronic pain modulation. Therefore, the aim of this study was to evaluate the possible spinal antinociceptive activity of PnTx4(5-5) toxin. The peptide is purified from the venom of the spider P. nigriventer and its affinity for NMDAR and sodium channels Nav1.2-1.6 has already been established. Methods: We compared its effect and safety with MK-801 (NMDAR antagonist) and evaluated its influence on glutamate and reactive oxygen species (ROS) levels in CSF. PnTx4(5-5) was administered intrathecally in the Formalin test and co-administered with NMDA in the Spontaneous pain test. After three minutes of observation, mice cerebrospinal fluid was collected to measure glutamate and ROS levels. Results: The spider peptide inhibited nociception as post-treatment in the inflammatory phase of the Formalin test. Furthermore, it inhibited spontaneous nociception induced by NMDA, being more potent and effective than MK-801 in both models tested. A glutamate rise level in the CSF of mice was significantly reduced by the toxin, but ROS increase was not affected. The animals’ motor skills were not affected by the tested doses of NMDAR inhibitors. Conclusion: In conclusion, the results suggest PnTx4(5-5) may mediate its antinociceptive effect in the spinal cord not only by inhibiting postsynaptic receptors but probably also by acting on autoreceptors. This effect does not affect the motricity of mice at the highest dose tested, which suggests that it has therapeutic potential and safety for use as a painkiller. RESUMO Este estudo objetivou analisar as ações desenvolvidas pela gestão de saúde no enfrentamento do desastre-crime de derramamento de petróleo na costa pernambucana em 2019. Trata-se de estudo de caso que investigou a atuação do setor saúde em quatro municípios de Pernambuco. A coleta de dados foi mediante entrevistas com 16 gestores de secretarias de saúde (municipal e estadual), utilizando roteiro semiestruturado com perguntas sobre atuação, impactos, ações desenvolvidas, dificuldades, desafios e lições aprendidas. A análise do Discurso do Sujeito Coletivo evidenciou quatro eixos temáticos: a) percepção dos gestores sobre os impactos do desastre-crime; b) ações desenvolvidas no processo de gestão de risco do desastre-crime pelo setor saúde; c) dificuldades enfrentadas no processo de mitigação do desastre; d) avaliação das ações da gestão durante o desastre-crime e preparação futura. Identificaram-se fragilidades da gestão em saúde nas ações de mitigação do desastre, resultado da falta de preparo dos serviços e dos profissionais de saúde. A falta de instrumentos e a desarticulação entre os entes federados intensificaram as dificuldades, gerando situações de calamidade no território. ABSTRACT This study aimed to analyze the actions taken by health management to deal with the criminal disaster of an oil spill off the coast of Pernambuco in 2019. It is a case study investigating the health sector’s actions in four municipalities in Pernambuco. Data was collected through interviews with 16 health department managers (municipal and state), using a semi-structured script with questions about performance, impacts, actions taken, difficulties, challenges, and lessons learned. The Collective Subject Discourse analysis revealed four thematic axes: a) managers’ perception of the impacts of the crime disaster; b) actions taken in the process of risk management of the disaster crime by the health sector; c) difficulties faced in the process of mitigating the disaster; d) evaluation of management actions during the disaster crime and future preparation. Weaknesses in health management were identified in disaster mitigation actions due to the need for more preparation of health services and professionals. The lack of instruments and coordination between the federated entities intensified the difficulties, generating calamity in the territory. Resumo Fundamento As complicações cardiovasculares da COVID-19 são aspectos importantes da patogênese e do prognóstico da doença. Evidências do papel prognóstico da troponina e da lesão miocárdica em pacientes hospitalizados com COVID-19 na América Latina são ainda escassos. Objetivos Avaliar a lesão miocárdica como preditor independente de mortalidade hospitalar e suporte ventilatório mecânico em pacientes hospitalizados, do registro brasileiro de COVID-19. Métodos Este estudo coorte é um subestudo do registro brasileiro de COVID-19, conduzido em 31 hospitais brasileiros de 17 cidades, de março a setembro de 2020. Os desfechos primários incluíram mortalidade hospitalar e suporte ventilatório mecânico invasivo. Os modelos para os desfechos primários foram estimados por regressão de Poisson com variância robusta, com significância estatística de p<0,05. Resultados Dos 2925 pacientes [idade mediana de 60 anos (48-71), 57,1%], 27,3% apresentaram lesão miocárdica. A proporção de pacientes com comorbidades foi maior nos pacientes com lesão miocárdica [mediana 2 (1-2) vs. 1 (0-20)]. Os pacientes com lesão miocárdica apresentaram maiores valores medianos de peptídeo natriurético cerebral, lactato desidrogenase, creatina fosfoquinase, N-terminal do pró-peptídeo natriurético tipo B e proteína C reativa em comparação a pacientes sem lesão miocárdica. Como fatores independentes, proteína C reativa e contagem de plaquetas foram relacionados com o risco de morte, e neutrófilos e contagem de plaquetas foram relacionados ao risco de suporte ventilatório mecânico invasivo. Os pacientes com níveis elevados de troponina apresentaram um maior risco de morte (RR 2,03, IC95% 1,60-2,58) e suporte ventilatório mecânico (RR 1,87;IC95% 1,57-2,23), em comparação àqueles com níveis de troponina normais. Abstract Background Cardiovascular complications of COVID-19 are important aspects of the disease’s pathogenesis and prognosis. Evidence on the prognostic role of troponin and myocardial injury in Latin American hospitalized COVID-19 patients is still scarce. Objectives To evaluate myocardial injury as independent predictor of in-hospital mortality and invasive mechanical ventilation support in hospitalized patients, from the Brazilian COVID-19 Registry. Methods This cohort study is a substudy of the Brazilian COVID-19 Registry, conducted in 31 Brazilian hospitals of 17 cities, March-September 2020. Primary outcomes included in-hospital mortality and invasive mechanical ventilation support. Models for the primary outcomes were estimated by Poisson regression with robust variance, with statistical significance of p<0.05. Results Of 2,925 patients (median age of 60 years [48-71], 57.1% men), 27.3% presented myocardial injury. The proportion of patients with comorbidities was higher among patients with cardiac injury (median 2 [1-2] vs. 1 [0-2]). Patients with myocardial injury had higher median levels of brain natriuretic peptide, lactate dehydrogenase, creatine phosphokinase, N-terminal pro-brain natriuretic peptide, and C-reactive protein than patients without myocardial injury. As independent predictors, C-reactive protein and platelet counts were related to the risk of death, and neutrophils and platelet counts were related to the risk of invasive mechanical ventilation support. Patients with high troponin levels presented a higher risk of death (RR 2.03, 95% CI 1.60-2.58) and invasive mechanical ventilation support (RR 1.87, 95% CI 1.57-2.23), when compared to those with normal troponin levels. Conclusion Cardiac injury was an independent predictor of in-hospital mortality and the need for invasive mechanical ventilation support in hospitalized COVID-19 patients. Resumo Objetivos: construir e validar vídeos educativos como parte do Programa LISA Down- Letramento e Inovação em Saúde para Adolescentes com Síndrome de Down. Métodos: trata-se de estudo metodológico de construção e validação de vídeos educativos fundamentados no letramento em saúde arquitetados para adolescentes com síndrome de Down, atendidos por uma Associação de Pais e Amigos dos Excepcionais (APAE), localizada no Nordeste brasileiro. As etapas da construção e validação dos vídeos foram: pré-produção, produção e pósprodução. Os storyboards foram elaborados de acordo com os fundamentos do letramento em saúde e validados por sete juízes especialistas. Na sequência os storyboards foram transformados em vídeos (produção) e estes foram avaliados por 13 adolescentes com Síndrome de Down (pós-produção). Resultados: os storyboards totalizaram 248 cenas, variando de 39 a 67 cenas cada um, foram aprovados pelos juízes com percentuais de concordância que variaram de 94% a 100%. Os vídeos gravados foram aprovados pelos adolescentes com síndrome de Down, com percentuais de concordância que variaram de 79,17% a 83,33%. Conclusão: os vídeos educativos foram validados e bem avaliados e, portanto, podem ser utilizados entre adolescentes em espaços sociais nos quais o foco da atenção sejam os adolescentes com síndrome de Down.
