RESUMO Introdução: Doenças pulmonares são comuns em pacientes com doença renal em estágio terminal (DRET), dificultando o diagnóstico diferencial com COVID-19. Este estudo descreve achados de tomografia computadorizada de tórax (TC) em pacientes com DRET em terapia renal substitutiva (TRS) hospitalizados com suspeita de COVID-19. Métodos: Indivíduos maiores de 18 anos com DRET, encaminhados ao pronto-socorro com suspeita de COVID-19 foram incluídos. Dados clínicos e epidemiológicos foram extraídos de registros eletrônicos de saúde. A TC foi classificada como típica, indeterminada, atípica, negativa. Comparamos achados tomográficos de pacientes com COVID-19 positivos e negativos. Resultados: Recrutamos 109 pacientes (62,3% COVID-19-positivos) entre março e dezembro de 2020, idade média de 60 ± 12,5 anos, 43% mulheres. A etiologia mais comum da DRET foi diabetes. Tempo médio em diálise foi 36 meses, intervalo interquartil = 12–84. A lesão pulmonar mais comum foi opacidades em vidro fosco. O padrão típico de TC foi mais comum em pacientes com COVID-19 (40 (61%) vs. 0 (0%) em pacientes sem COVID-19, p < 0,001). Sensibilidade 60,61% (40/66), especificidade 100% (40/40). Valores preditivos positivos e negativos foram 100% e 62,3%, respectivamente. Padrão atípico de TC foi mais frequente em pacientes COVID-19-negativos (9 (14%) vs. 24 (56%) em COVID-19-positivos, p < 0,001), enquanto padrão indeterminado foi semelhante em ambos os grupos (13 (20%) vs. 6 (14%), p = 0,606), e padrão negativo foi mais comum em pacientes COVID-19-negativos (4 (6%) vs. 12 (28%), p = 0,002). ABSTRACT Introduction: Lung diseases are common in patients with end stage kidney disease (ESKD), making differential diagnosis with COVID-19 a challenge. This study describes pulmonary chest tomography (CT) findings in hospitalized ESKD patients on renal replacement therapy (RRT) with clinical suspicion of COVID-19. Methods: ESKD individuals referred to emergency department older than 18 years with clinical suspicion of COVID-19 were recruited. Epidemiological baseline clinical information was extracted from electronic health records. Pulmonary CT was classified as typical, indeterminate, atypical or negative. We then compared the CT findings of positive and negative COVID-19 patients. Results: We recruited 109 patients (62.3% COVID-19-positive) between March and December 2020, mean age 60 ± 12.5 years, 43% female. The most common etiology of ESKD was diabetes. Median time on dialysis was 36 months, interquartile range = 12–84. The most common pulmonary lesion on CT was ground glass opacities. Typical CT pattern was more common in COVID-19 patients (40 (61%) vs 0 (0%) in non-COVID-19 patients, p < 0.001). Sensitivity was 60.61% (40/66) and specificity was 100% (40/40). Positive predictive value and negative predictive value were 100% and 62.3%, respectively. RESUMO Objetivo Verificar a influência das habilidades intelectuais-cognitivas verbais na percepção de fala no ruído, em idosos com perda auditiva sensorioneural, considerando a escolaridade, a idade e o grau da perda auditiva. Método Participaram 36 idosos entre 60 e 89 anos com perda auditiva sensorioneural bilateral, que após avaliação psicológica por meio do Wechsler Intelligence Scale for Adults (WAIS III), foram divididos em (GI) 24 idosos sem alteração cognitiva e (GII) 12 idosos com risco de alteração cognitiva. Foram submetidos à avaliação otorrinolaringológica, entrevista audiológica, audiometria tonal liminar e a avaliação da percepção de fala no ruído por meio do Hearing in Noise Test (HINT-Brasil). O teste estatístico U de Mann-Whitney comparou os resultados entre os grupos, e a correlação de Spearman verificou as variáveis idade, grau da perda auditiva e nível de escolaridade. Resultados Não houve diferença entre os grupos na habilidade de percepção de fala no ruído, exceto na condição ruído à esquerda, no qual o GII apresentou melhor desempenho no HINT-Brasil. ABSTRACT Purpose To verify the influence of verbal intellectual-cognitive skills on speech perception in noise, in elderly with sensorineural hearing loss, considering education, age, and degree of hearing loss. Methods 36 elderly between 60 and 89 years old with bilateral sensorineural hearing loss participated in the study. After psychological assessment using the Wechsler Intelligence Scale for Adults (WAIS-III), they were grouped into (GI) 24 elderly without cognitive alteration and (GII) 12 elderly with risk of cognitive alteration. They underwent otorhinolaryngological assessment, audiological interview, pure tone audiometry, and assessment of speech perception in noise using the Hearing in Noise Test (HINT-Brazil). A eficácia da capacitação foi verificada a partir da comparação do desempenho dos enfermeiros, nos questionários pré e pós-capacitação, e a avaliação do Cybertutor foi investigada por meio de um formulário com perguntas fechadas e abertas. Resultados: O desempenho no questionário pré-capacitação demonstrou a necessidade da capacitação em saúde auditiva infantil. Houve diferença no desempenho após a capacitação em todos os domínios avaliados e no escore total. Conclusão: A eficácia do cybertutor na capacitação de enfermeiros na área de Saúde Auditiva Infantil pôde ser comprovada, uma vez que houve melhora no desempenho em todos os domínios avaliados.

Resumo: Introdução: Este trabalho aborda a utilização do mecanismo de feedback na avaliação formativa durante a formação médica, visto que esse instrumento é de grande importância para o estímulo dessa formação que se pauta pelas metodologias ativas de ensino. O estudo tem como objetivos caracterizar a aplicação dessa ferramenta e identificar possíveis falhas na utilização dela no cotidiano do módulo Pinesc, no curso de Medicina da Universidade Anhanguera (Uniderp). Método: Trata-se de uma pesquisa quantitativa e seccional que abrange alunos do primeiro ao oitavo semestre inseridos no módulo longitudinal Pinesc. Na coleta dos dados, utilizou-se um questionário com perguntas relativas à avaliação formativa que é realizada pelos preceptores, o qual é repassado aos acadêmicos de acordo com o cálculo de amostragem. O feedback é a atividade central da avaliação formativa e possibilita o desenvolvimento da capacidade reflexiva e autoavaliativa e de habilidades. Contudo, a utilização do feedback ainda encontra obstáculos principalmente pela dificuldade de os avaliadores elencarem os pontos negativos e as facilidades dos avaliados. Resultado: Neste estudo, foram relatadas algumas falhas e pontos positivos no processo de aplicação desse instrumento. Por conta disso, elencam-se e sugerem-se aperfeiçoamentos para uma melhor utilização do feedback e um aproveitamento acadêmico mais favorável. Conclusão: Como base da avaliação formativa, o feedback ainda demonstra possuir muitas fragilidades quanto à sua forma e aplicação.

Abstract: Introduction: This present study assesses the use of the feedback mechanism in the formative assessment during medical training, considering this instrument is of great importance for this training stimulation, which is based on active teaching methodologies. The study aims to characterize the application of this tool and to identify possible failures in its application in the daily routine of the PINESC module in the Anhanguera University (Uniderp) medical course. Method: This is a quantitative, sectional research, including students from the first to the eighth semesters of the medical course, attending the PINESC longitudinal module. In the data collection, a questionnaire was used with questions related to the formative assessment performed by the preceptors, which is reported to the students according to the sample calculation. The feedback is the core activity of the formative assessment and allows the development of reflective and self-evaluation capacities and the development of skills. However, the use of feedback still faces barriers, mainly due to the difficulty of the evaluators in listing negative points and capabilities of the evaluated individuals. Result: In this study, some shortcomings and positive points were reported regarding the application process of this tool, listing and suggesting improvements for a better use of feedback and thus a more favorable academic achievement. Conclusion: In conclusion, as the basis of formative evaluation, the feedback still shows many weaknesses regarding its form and application.

Abstract Background: Human herpesviruses (HHVs) are responsible for a significant number of clinical manifestations in systemic lupus erythematous (SLE) patients. The aim of this study was to determine the frequency of active HHV infections in SLE patients and correlating them with disease activity. Methods: Serum samples were collected from 71 SLE patients and their DNAs were extracted and analyzed to detect HHV-DNA viruses using the nucleic acid amplification technique. Results: Fifteen out of the 71 (21.1%) patients tested positive for the HHV-DNA virus. Of them, 11/15 HHV-DNA-positive patients (73.3%) had SLE activity index (SLEDAI - Systemic Lupus Erythematosus Disease Activity Index) ≥8 (p = 0.0001). Active HCMV infection was the mostly frequently observed infection, occurring in 6/15 patients (40%). The frequencies of other active viral infections were 22% for HSV-1, 16.7% for HHV-7, and 5.5% for HSV-2. Viral coinfection (two or more viruses detected in the same sample) occurred in three patients (16.7%). Active HHV infections in SLE patients are more frequent in those with active SLE (≥8), who is at high risk of HHV reactivation and HCMV disease. Conclusion: Viral surveillance is important to identify active HHV infections that can cause clinical symptoms and other complication in SLE patients.

Resumo Objetivando determinar o perfil de sensibilidade às drogas antimicrobianas de bactérias presentes em carpas, foi utilizado o teste de sensibilidade de difusão em disco Kirb Bauer modificado. Foram avaliados 60 isolados bacterianos, analisados e encontrados os seguintes gêneros: Staphylococcus spp. (27), Streptococcus spp. (03), Aeromonas spp. (15), Proteus spp. (04), Acinetobacter spp. (05), Pseudomonas spp. (01) e Enterobacteriaceae (05). O perfil de sensibilidade observado foi de: amicacina (35%), amoxicilina (65%), apramicina (32%), ceftiofur (37%), doxicilina (32%), enrofloxacina (15%), josamicina (75%), lincomicina (37%), nitrofurantoina (60%), ácido nalidíxico (32%), novobiocina (82%), penicilina (70%) e sulfozotrim (40%). O índice de resistência múltipla às drogas antimicrobianas médio variou de 0,33 para Streptococcus spp até 0,71 para bactérias da família Enterobacteriaceae. Dessa forma, o maior perfil de resistência às drogas antimicrobianas testadas foi observado para novobiocina, enquanto que o menor foi observado para a enrofloxacina. Os isolados bacterianos obtidos de carpas apresentaram resistência múltipla às drogas testadas, sendo três isolados resistentes a todos os antimicrobianos testados.

Abstract The present work has the purpose to determine the susceptibility pattern to antimicrobial drugs of bacteria isolates from carps, using Kirb Bauer modified disk diffusion test. We analyzed 60 bacterial isolates from these groups: Staphylococcus spp. (27), Streptococcus spp. (03), Aeromonas spp. (15), Proteus spp. (04), Acinetobacter spp. (05), Pseudomonas spp. (01) and Enterobacteriaceae (05). The percentual of susceptibility was: amikacine (35%), amoxyciline (65%), apramycin (32%), ceftiofur (37%), doxycyline (32%), enrofloxacin (15%), josamycin (75%), lyncomicin (37%), nitrofurantoin (60%), nalidixic acid (32%), novobiocin (82%), penicillin (70%) and trimethoprim: sulfamethoxazole (40%). The multiple resistance ratios to antimicrobial drugs were from 0.33 to Streptococcus spp. to 0.71 Enterobacteriaceae bacteria. Thus, the highest resistance profile of the antimicrobial drugs tested was observed for novobiocin, while the lowest was observed for enrofloxacin. Bacterial isolates from carp showed multiple resistance to the drugs tested, with three isolates resistant to all tested antimicrobials.

RESUMO Introdução Pesquisas voltadas para as populações com baixas exposições a solventes, ou exposições dentro dos níveis de tolerância permitidos em âmbito ocupacional ainda são restritas. Objetivo Caracterizar o perfil auditivo de frentistas de postos de combustíveis. Métodos Estudo transversal, constituído por dois grupos, pareados por gênero e idade: Grupo Controle - 23 indivíduos sem exposição a ruído ou agentes químicos; Grupo Experimental - 21 frentistas de postos de combustíveis. Foi realizada avaliação audiológica, composta por audiometria tonal liminar, logoaudiometria e medidas de imitância acústica. A análise estatística utilizou cálculo de média, desvio padrão, valor mínimo e máximo, teste Qui-quadrado e coeficiente de correlação de Pearson. O nível de significância adotado foi de 5%. Resultados Foram verificadas alterações auditivas nos frentistas, caracterizadas por comprometimento no sistema auditivo periférico, sugerindo ação tóxica da exposição a combustíveis. Houve correlação entre idade e tempo de exposição a solvente. Na comparação entre os grupos, o reflexo acústico demonstrou maior número de alterações no grupo experimental, com diferença para os reflexos acústicos ipsilaterais da orelha direita e contralaterais da orelha esquerda. Conclusão Não houve diferença entre os grupos para os limiares auditivos, porém, a diferença verificada nos reflexos acústicos ipsilaterais e contralaterais no grupo experimental sugere comprometimento retrococlear. Diante das evidências observadas neste estudo, considera-se relevante incluir a pesquisa do reflexo acústico na avaliação auditiva dos frentistas, bem como a integração desta categoria profissional aos programas de prevenção de perda auditiva.

ABSTRACT Introduction Researches into populations with low solvent exposures, or exposures within tolerance levels allowed in the occupational field are still restricted. Purpose To characterize the hearing profile of gas station attendants. Methods Cross-sectional study, constituted of two groups, matched by gender and age: Control Group - 23 subjects without exposure to noise or chemicals; Experimental Group - 21 gas station attendants. An audiological evaluation was performed, composed by pure tone audiometry, speech audiometry and acoustic impedance tests. The statistical analysis used average calculation, standard deviation, minimum and maximum value; Chi-square Test and Pearson’s correlation coefficient. The significance level was 5%. Results Hearing loss was verified in gas station attendants characterized by impairment of the peripheral auditory system, suggesting toxic effects of exposure to fuels. There was a correlation between age and solvent exposure time. Comparing the groups, the acoustic reflex showed more alterations in the experimental group, with a difference for the ipsilateral acoustic reflexes of the right ear and contralateral ones of the left ear. Conclusion There was no difference between the groups for the hearing thresholds; however, the difference observed in the ipsilateral and contralateral acoustic reflexes in the experimental group suggests retrocochlear impairment. Due to the evidence observed in this study, it is considered relevant to include the acoustic reflex research in the auditory evaluation of the gas station attendants, as well as the integration of this professional category into hearing loss prevention programs.

RESUMO Objetivo Verificar os hábitos e atitudes de jovens em idade escolar com relação ao uso de estéreos pessoais e avaliar o nível de conhecimento sobre os efeitos nocivos da exposição à música amplificada na audição, antes e após uma palestra educativa. Métodos Participaram 58 estudantes do ensino fundamental e médio, entre 10 e 17 anos de idade. Foi realizada palestra educativa, mediante aula expositiva dialogada, utilizando recursos audiovisuais. Para avaliação foram aplicados dois questionários no formato online, antes e após 15 dias da palestra. Realizou-se, também, análise descritiva. Resultados A maioria dos escolares relatou fazer uso de estéreos pessoais com fone de ouvido, acima da intensidade média do equipamento e por período inferior a três horas diárias. A queixa mais frequente foi o zumbido. Após a palestra educativa, houve diferença no conhecimento sobre o conceito de perda auditiva e de que música em intensidade elevada, em momentos de lazer, pode causar perda auditiva. Também houve diferença na intensidade utilizada para ouvir música com fone de ouvido, que foi reduzida. Conclusão Grande parte dos escolares possuía conhecimento prévio a respeito dos prejuízos da música amplificada, para a audição e, ainda assim, alguns referiram hábitos inadequados. A palestra educativa revelou mudanças significativas em apenas alguns aspectos investigados, evidenciando a necessidade de estruturar ações contínuas na prevenção de perdas auditivas nessa população. A inclusão da escola e da família nas ações educativas e a continuidade dessas ações são fundamentais para mudanças efetivas de hábitos e atitudes das crianças e adolescentes.

ABSTRACT Purpose To verify the habits and attitudes of school-age youths regarding the use of personal music players, and assess their knowledge level about the harmful effects to the hearing of exposure to amplified music before and after an educational lecture. Methods A total of 58 elementary and middle school students aged 10 to 17 years participated in the study. We presented an educational lecture using an expository dialogue format with audiovisual resources. To evaluate the students, we applied two online questionnaires before and 15 days after the lecture. Descriptive analysis was also performed. Results Most students reported using personal music players with earphones above the mean intensity of the equipment and for fewer than 3 hours a day. The most frequent complaint was tinnitus. After the educational lecture, there were differences in knowledge about the concept of hearing loss and the fact that high-intensity music in moments of leisure may cause hearing loss. There was also a difference in the intensity used to listen to music with earphones, which decreased. Conclusion Most students had prior knowledge of the harmful effect of amplified music on hearing and even then, some indicated inadequate habits. The educational lecture showed significant changes in only some of the aspects investigated, reflecting a need to structure continuous actions towards hearing loss prevention in this population. The inclusion of the school and the family in educational actions and the continuity of these actions are fundamental to effective changes in habits and attitudes among children and adolescents.

OBJETIVO: analisar as habilidades comunicativas verbais e não-verbais de crianças autistas. MÉTODOS: foram selecionadas seis crianças com o diagnóstico de atraso de linguagem secundário a autismo, submetidas à terapia fonoaudiológica em uma clínica escola de uma universidade privada da cidade do Recife. As crianças foram observadas em duas sessões de terapia, que foram gravadas em fita VHS para posterior análise e discussão. A análise foi baseada no uso do protocolo de observação pertencente ao ABFW, que contempla os meios e funções comunicativas. RESULTADOS: em detrimento dos meios vocais e verbais, o meio gestual apareceu com maior frequência nos atos comunicativos. Os gestos, mesmo constituindo uma forma de comunicação não verbal, demonstraram, muitas vezes, expressar intenções dos sujeitos. Com relação às funções comunicativas, pôde-se concluir que houve uma grande variedade das mesmas, porém, entre as vinte funções investigadas, apenas poucas se destacaram. Entre elas, apareceram, predominantemente, as funções não-focalizadas, protesto, exploratória e reativa. CONCLUSÃO: foi possível verificar que as crianças investigadas utilizam formas funcionais de comunicação nos diferentes contextos situacionais, entre as habilidades verbais e não-verbais, aspectos considerados essenciais para a proposição de recursos na intervenção fonoaudiológica.

PURPOSE: to evaluate the verbal and non-verbal communicative abilities in autist children. METHODS: six children was selected with the diagnostic of language delay secondary to autism, submitted to speech therapy in a school-clinic of a University private from Recife. The children were monitored in therapy sessions, and these was stored in VHS tape for posterior analysis and discussion. The analysis was based on the use of ABFW observation protocol, which analyses the communicative means and functions. RESULTS: among the acts, the gestures appeared with major predominance among the analyzed subjects. Regarding the communicative functions, we observed with major predominance: non-focused, protest, explorer and reactive. CONCLUSION: It was possible to verify that the studied children used functional communication forms in different situational contexts, verbal and non-verbal skills, aspects considered essential to the proposition of resources in speech therapy intervention.

Paciente admitida com quadro de engasgo seguido de dispnéia, cianose e perda de consciência, recuperada após aspiração de vias aéreas. Evoluiu com agravamento do quadro clínico até início de tratamento de suporte, quando apresentou rápido restabelecimento. Radiografia de tórax do dia da internação demonstrou infiltrado difuso bilateral e à tomografia observou-se imagem em vidro fosco difuso. Objetiva-se demonstrar alterações tomográficas nesta síndrome.

A choking patient was admitted with dyspnea, cyanosis, and loss of consciousness, which initially improved after suction of the airways. The patient presented worsening of the clinical parameters until the introduction of supportive treatment, after which she rapidly recovered. Chest x-ray taken on day of hospitalization showed bilateral diffuse alveolar filling pattern, and computed tomography showed ground glass areas. The aim of this paper is to show the radiological features of this syndrome.
