RESUMO: O ensino de anatomia vem se modificando ao longo dos anos, introduzindo métodos tecnológicos, principalmente após o período pandêmico. Questiona-se o uso exclusivo da tecnologia em comparação aos métodos tradicionais de dissecação e aulas teóricas presenciais no que diz respeito à aquisição de conhecimento. Este artigo analisou a percepção de conhecimento de alunos de diversos semestres do curso de Medicina Veterinária, comparando alunos que tiveram aulas apenas online com aqueles que tiveram aulas presenciais de Anatomia Descritiva dos Animais Domésticos. Para tanto, questões foram enviadas por meio de formulário, com preenchimento voluntário e anônimo, e posteriormente analisadas estatisticamente. Os resultados indicam que 11,5% dos alunos que cursaram a disciplina remotamente falharam na aplicação dos conhecimentos em outras disciplinas, enquanto nenhum aluno do ensino presencial relatou o mesmo. A maioria dos alunos, tanto dos grupos remotos quanto dos presenciais, lembram-se do assunto, mas revisam o conteúdo com frequência. Em relação ao conhecimento teórico, o grupo online (80,8%) compreendeu melhor à medida que o curso avançava, comparado a 59,6% do grupo presencial. A ligação entre Anatomia e outras disciplinas foi mais destacada pelo grupo presencial (30,8%) em comparação ao online (15,4%). Ambos os grupos concordam que o contacto físico com peças anatômicas é essencial, mas alguns consideram possível ter um primeiro contacto com imagens ou fotografias 3D. Baseado nesse estudo preliminar, conclui-se que as interações iniciais com a área de Anatomia devem incluir técnicas pedagógicas diversificadas com comunicação empática. A utilização de recursos tecnológicos modernos é um aliado do processo de ensino-aprendizagem, e após a abstração dos conteúdos é possível o uso singular da tecnologia na manutenção e atualização do conhecimento anatômico. ABSTRACT: Anatomy teaching has been changing over the years, introducing technological methods especially after the pandemic period. An ongoing debate revolves around whether exclusive reliance on technology for anatomy classes, as opposed to traditional methods and face-to-face instruction, enhances knowledge acquisition. This study analyzed the knowledge perception of students from various semesters throughout the Veterinary Medicine course, comparing students who only had online classes with those who had face-to-face classes in Gross Anatomy of Domestic Animals. For this purpose, a questionnaire was distributed for voluntary and anonymous completion, and the responses were subsequently analyzed statistically. The results indicated that 11.5% of the students who took Anatomy remotely failed to apply the knowledge in other subjects, while no face-to-face learning student reported the same. Most students, both remote and face-to-face groups, remember the subject but review the content frequently. Regarding theoretical knowledge, the online group (80.8%) understood better as the course progressed, compared to 59.6% of the face-to-face group. The connection between Anatomy and other subjects was more highlighted by the face-to-face group (30.8%) compared to the online group (15.4%). Both groups agree that physical contact with anatomical specimens is essential; however, some believe that initial exposure through 3D images or photographs is feasible. We concluded based on this preliminary study that the initial interactions with the Anatomy field should include diversified pedagogical techniques with empathetic communication. The use of modern technological resources are allies in the teaching-learning process. Furthermore, after abstracting the contents, technology alone can effectively maintain and update anatomical knowledge. Resumo Objetivo: Avaliar o Rácio de gânglios linfáticos (RGL) como preditor de sobrevida e recidiva de cancro da laringe com metastização cervical (pN+). Desenho do estudo: Estudo retrospetivo, que incluiu doentes submetidos a laringectomia total associada a esvaziamento ganglionar cervical bilateral, entre outubro/2010 e abril/2023. Resultados: Foram incluídos 73 doentes. Entre os doentes pN+ (n=37;50,7%), aqueles com evidência de recidiva tumoral (n=16;43,2%) apresentaram uma mediana de RGL superior (0,159 versus 0.060), p=0,007. Após análise da curva ROC, o RGL>0,063 indicou recidiva tumoral (sensibilidade:87%; especificidade:48%). Para além disso, o RGL>0,063 manteve-se como um preditor independente de recidiva tumoral, após análise multivariada ajustada para potenciais fatores de confundimento (idade, extensão extraganglionar, margem de resseção e estadio patológico T),(HR=6,58; p=0,049). Por último, um RGL elevado (>0,063) esteve associado à morte específica por cancro em doentes pN+ (OR=8,64; p=0,008). Conclusão: Este estudo indica que doentes com RGL>0,063 têm maior probabilidade de doença agressiva. Assim, o RGL é um parâmetro prognóstico relevante, podendo ser útil para complementar o estadiamento TNM no cancro da laringe pN+.

Abstract Objectives: To evaluate Lymph node ratio (LNR) as a predictor for survival and recurrence of laryngeal cancer with metastatic lymph nodes (pN+). Study design: This retrospective study included patients who underwent primary laryngectomy combined with bilateral neck dissection, between October/2010 and April/2023. Results: A total of 73 patients were included. Among pN+ patients (n=37;50.7%), those with disease recurrence (n=16;43.2%) exhibited a higher median LNR (0.159 versus 0.060), p=0.007. ROC curve analysis showed that LNR>0.063 predicted recurrence (sensitivity 87%;specificity 48%). Moreover, multivariate analysis confirmed LNR>0.063 as an independent predictor of disease recurrence, after controlling for potential confounders, namely age, extracapsular spread, resection margin and pathological T category, (HR=6.58; p=0.049). Finally, high LNR (>0.063) was associated with cancer-specific death (OR=8.64; p=0.008) in pN+ patients. Conclusion: Our findings highlight that patients with LNR>0.063 are more likely to have aggressive disease, suggesting LNR as a significant prognostic factor complementing TNM staging in pN+ laryngeal cancer.

Resumo Introdução: Os Neandertais, nossos parentes extintos, exibiam uma morfologia nasal distinta em comparação com o Homo sapiens. Objetivos: Este estudo explora essas diferenças através de uma análise comparativa de medidas nasocranianas. Material e Métodos: Dados sobre as características nasais do Homo Neanderthalensis e Homo Heilderbergensis foram obtidos a partir da literatura paleoantropológica existente, com foco em 16 fósseis. Uma amostra paralela de 38 de crânios de Homo Sapiens foi analisada retrospetivamente mediante tomografia computadorizada (TC), com obtenção de medições relativas à região nasal. Resultados: Os Hominídeos exibiram cavidades nasais significativamente maiores, com distâncias basion-nasion e basion-prosthion aumentadas, dimensões intercaninas maiores e maior largura e altura nasal em comparação com os humanos modernos. A largura coanal e o índice nasal não diferiram significativamente. Conclusões: Esses achados sugerem adaptações funcionais na morfologia nasal neandertal, potencialmente auxiliando na termorregulação em ambientes frios. Pesquisas adicionais são necessárias para compreender totalmente o significado evolutivo dessas adaptações.

Abstract Introduction: Neanderthals, our extinct relatives, exhibited distinct nasal morphology compared to Homo sapiens. Objectives: This study explores these differences through a comparative analysis of cranionasal measurements. Material and Methods: Data on Neanderthal nasal features were obtained from existing paleoanthropological literature, focusing on 16 hominid fossils. A parallel sample of 38 modern human CT scans was retrospectively reviewed, with measurements focusing on nasal region. Results: Neanderthals displayed significantly larger nasal cavities with increased basion-nasion and basion-prosthion distances, wider intercanine dimensions, and larger nasal breadth and height compared to modern humans. Choanal width and nasal index did not differ significantly. Conclusions: These findings suggest functional adaptations in Neanderthal nasal morphology, potentially aiding in thermoregulation within cold environments. Further research is needed to fully understand the evolutionary significance of these adaptations.

Resumo A leucemia linfoblástica aguda (LLA) é a causa mais comum de neoplasia em idade pediátrica. Excluindo os tumores musculoesqueléticos, a LLA é uma das neoplasias mais frequentemente associadas a sintomas osteomusculares na apresentação inicial. Muitas das crianças com LLA são inicialmente referenciadas à Ortopedia, pelo que é necessário um elevado grau de suspeição para evitar o atraso do diagnóstico. Os autores relatam dois casos de crianças com diagnóstico presumível de infeção osteoarticular cuja evolução clínica levou ao diagnóstico de doença linfoproliferativa.

Abstract Acute lymphoblastic leukemia (ALL) is the most common cause of malignancy in pediatric age. With the exception of musculoskeletal tumors, ALL is one of the malignancies most commonly associated with musculoskeletal symptoms at initial presentation. Some children with ALL are initially referred to Orthopedics for diagnostic evaluation, and a high index of clinical suspicion is required to avoid diagnostic delay. The authors report two cases of children initially diagnosed with a presumed osteoarticular infection, but whose clinical evolution led to the diagnosis of lymphoproliferative disease.

Abstract Introduction: Nurses' intervention in the process of parental grief within primary healthcare reflects a fundamental health need, supporting couples in the experience of transition following the death of a child. It is crucial to value the functional and emotional balance of the family, as well as the grieving experience and associated feelings. Goals: To map the scientific evidence on nursing interventions within primary healthcare aimed at couples grieving the death of a child. Methods: A Scoping Review conducted following the PRISMA-ScR methodology and PCC mnemonic: population, concept, and context. Searches were performed in PubMed, CINAHL Complete, B-On, Science Direct, and RCAAP databases. Studies published in Portuguese, Spanish, and English from 2018 to 2023 were included. Identified studies were organized using Rayyan®, and their relevance was analyzed by two independent reviewers. Out of 1021 available articles, seven were included in the review. Results: Four themes emerged from the analysis: grief manifestations (denial, anger, bargaining, depression, acceptance); attitudes and behaviors promoting adaptive grief (religion, rituals, end-of-life care at home, redefining life goals, time occupation, self-care); obstacles to adaptive grief (insufficient professional support and behaviors of others); and support networks for parental grief (formal and informal). Final considerations: The grief experiences of couples allowed mapping of Family Health Nursing interventions aimed at couples grieving the death of a child, highlighting their priority, as well as the need for professional training in this area.

Resumo Introdução: A intervenção dos enfermeiros no processo de luto parental, em cuidados de saúde primários, espelha uma necessidade de saúde basilar, assistindo na vivência de um processo de transição pelo casal. Torna-se crucial a valorização do equilíbrio funcional e emocional da família, bem como a experiência de luto e os sentimentos vivenciados. Objetivo: Mapear a evidência científica sobre as intervenções de enfermagem, em cuidados de saúde primários, dirigidas ao casal enlutado pela morte de um filho. Metodologia: Scoping Review realizada de acordo com a metodologia PRISMA-ScR e a mnemónica PCC: população, conceito e contexto. Pesquisa realizada nas bases de dados PubMed, CINAHL Complete, B-On, Science Direct e RCAAP. Estudos publicados em português, espanhol e inglês foram incluídos, com o limite temporal 2018-2023. Os estudos identificados foram organizados no Rayyan®, e a sua relevância, analisada por dois revisores independentes. Dos 1021 artigos disponíveis, sete foram incluídos na revisão. Resultados: Da análise emergiram quatro temas: manifestações de luto (negação, raiva, negociação, depressão, aceitação); atitudes e comportamentos promotores de luto adaptativo (religião, rituais, fim de vida no domicílio, redefinição de objetivos de vida, ocupação do tempo, autocuidado); obstáculos ao luto adaptativo (apoio dos profissionais insuficiente e comportamentos de outras pessoas) e redes de apoio no luto parental (formal e informal). Considerações finais: As experiências de luto vivenciadas pelo casal, permitiram mapear as intervenções de Enfermagem de Saúde Familiar dirigidas ao casal pela morte do filho, as quais emergem como prioritárias, bem como a formação profissional na área.

Abstract Acupuncture is a therapy that is over 2000 years old, having recently become popular as a practice in Western culture. This therapy consists of inserting fine needles to alleviate symptoms such as pain or nausea. This article focuses on a pneumothorax secondary to acupuncture case. A 20-year-old man came to the Emergency Department with pleuritic pain in the right scapular region. The pain appeared after an acupuncture session performed for low back pain. On physical examination, a eupneic patient was observed, but with some difficulty in completing sentences, with O2 saturation of 98% in room air, with a slight decrease in vesicular murmur on auscultation of the upper right hemithorax and hypertympanism on percussion of the same area, with notorious pain facies, pain on right intercostal palpation and two dorsal punctiform wounds compatible with the insertion of needles. Chest X-ray revealed a right pneumothorax, so a chest drain was placed. After resolution, the user was discharged with indication for control in primary health care.

Resumo A acupuntura é uma terapia com mais de 2000 anos, tendo-se popularizado, recentemente, como prática na cultura ocidental. Esta terapêutica consiste na inserção de agulhas finas para minorar sintomas como dor ou náusea. O presente artigo centra-se num caso de pneumotórax secundário à acupuntura. Um homem de 20 anos recorreu ao Serviço de Urgência com dor pleurítica na região escapular direita. A dor surgiu após sessão de acupuntura realizada por lombodorsalgia. No exame objetivo observou-se um doente eupneico, mas com dificuldade em completar frases, com saturação de O2 de 98% a ar ambiente, com diminuição discreta do murmúrio vesicular à auscultação do hemitórax direito superior e hipertimpanismo na percussão da mesma área, com notória fácies de dor, dor à palpação intercostal direita e duas feridas punctiformes dorsais compatíveis com a inserção das agulhas. A radiografia de tórax revelou um pneumotórax à direita, tendo sido colocado um dreno torácico. Após resolução, o utente teve alta com indicação para controlo nos cuidados de saúde primários.

Abstract Uterine niche is a recess located in the anterior wall of the uterine isthmus, over the cesarean section scar. This condition is often asymptomatic, but it can be associated with symptoms such as abnormal uterine bleeding, pelvic pain and secondary infertility. Endovaginal ultrasound is the first-line diagnostic method. Treatment includes hormonal methods or surgery, and surgical treatment seems to be the most effective in resolving symptoms. We describe the case of a 37-year-old woman, with an obstetric history of a pregnancy with cesarean delivery, who complained of pelvic pain and menstrual irregularity. An endovaginal ultrasound was requested, which reported the presence of a “… echogenic oval formation with well-defined limits measuring 28x17 mm in the topography of the cesarean scar.” After a clinical discussion with a doctor specializing in Gynecology, the patient was referred to a hospital consultation in this specialty for investigation. The diagnosis of uterine niche was confirmed, and the treatment consisted of the placement of an intrauterine system.

Resumo Nicho uterino é um recesso localizado na parede anterior do istmo uterino, sobre a cicatriz de cesariana. Frequentemente esta condição é assintomática, mas pode estar associada a sintomas como hemorragia uterina anómala, dor pélvica e infertilidade secundária. A ecografia endovaginal é o método diagnóstico de primeira linha. O tratamento inclui métodos hormonais ou cirurgia, sendo que o tratamento cirúrgico parece ser o mais eficaz na resolução dos sintomas. Descrevemos o caso de uma mulher de 37 anos, com antecedentes obstétricos de uma gestação com parto por cesariana que apresentava queixas de dor pélvica e irregularidade menstrual. Foi solicitada ecografia endovaginal que relatou a presença de uma “…formação ovalada ecogénica de limites bem definidos com 28x17 mm na topografia da cicatriz de cesariana.” Após discussão clínica com médico especialista em Ginecologia, a doente foi encaminhada para consulta hospitalar desta especialidade para investigação, tendo sido confirmado o diagnóstico de nicho uterino e cujo tratamento consistiu na colocação de sistema intrauterino.

Abstract Introduction: The negotiation process between parents, children, and nurses during hospitalization must be structured for a successful interaction. However, there is limited scientific evidence on parents' perspectives. Objective: Understand the negotiation process that takes place between nurses and parents in the Paediatric Ward. Methods: A qualitative descriptive study was conducted with 11 parents using semi-structured interviews. The data was analysed using content analysis techniques by Bardin (2016). Results: Five domains emerged from the discourse and perspectives of parents regarding care negotiation. The structuring conditions and knowledge of the child and family serve as foundations to support the negotiation process. During the negotiation itself, two roles converge: the parental role, involving the assumption and adaptation to the parental role, and the role of the nurses. Some behaviours promote negotiation, aimed at making the process more natural. However, some behaviours on the part of the nurses could hinder negotiation. Post-discharge support is acknowledged as an essential extra support once the child returns home. Conclusion: The findings provide clarification of the roles of the different participants in care negotiation, with potential contributions to the implementation of actions that improve the negotiating process in a more sustained and intentional manner.

Resumen Introducción: La negociación de los cuidados es un aspecto clave para el éxito de la interacción entre padres, hijos y personal de enfermería durante la hospitalización. Sin embargo, hay poca evidencia científica sobre las perspectivas de los padres en el proceso de negociación. Objetivo: Comprender la negociación de los cuidados entre enfermeras y padres en el servicio de Pediatría Médica. Métodos: Estudio de enfoque cualitativo de tipo descriptivo, que contó con la participación de 11 padres. La recolección de datos se llevó a cabo a través de entrevistas semiestructuradas y los datos fueron analizados mediante la técnica de análisis de contenido según Bardin (2016). Resultados: De los discursos y perspectivas de los padres sobre la negociación de cuidados, surgieron cinco dominios. Las condiciones estructurantes y el conocimiento del niño y la familia se presentan como pilares para sustentar el proceso negociador. En la negociación propiamente dicha, convergen dos roles: el parental, con la asunción y adaptación a la condición del rol parental, y el rol de los enfermeros. Hay comportamientos que favorecen la negociación, que tienden hacia la naturalidad del proceso de negociar; sin embargo, también existen comportamientos por parte de los enfermeros que perjudican la negociación. El apoyo después del alta es reconocido como un soporte importante después del regreso del niño a casa. Conclusión: Los hallazgos permiten aclarar los roles de los diferentes intervinientes en la negociación de cuidados, con el potencial de contribuir a la implementación de acciones favorecedoras del proceso negociador de manera más sostenida e intencional.

Resumo Introdução: A negociação de cuidados assume uma dimensão estruturante para o sucesso da interação entre pais, criança e enfermeiros durante a hospitalização. Contudo, constata-se uma escassa evidência científica sobre as perspetivas de pais relativamente ao processo negocial. Objetivo: Compreender a negociação de cuidados desenvolvida entre enfermeiros e pais no serviço de Pediatria Médica. Métodos: Estudo de abordagem qualitativa de tipo descritivo, que contou com a participação de 11 pais. A recolha de dados realizou-se através de entrevistas semiestruturadas e os dados analisados com recurso à técnica de análise de conteúdo segundo Bardin (2016). Resultados: Dos discursos e perspetivas dos pais sobre a negociação de cuidados, emergiram cinco domínios. As condições estruturantes, e o conhecimento da criança e família, são alicerces para sustentar o processo negocial. Na negociação propriamente dita, confluem dois papéis, o parental, com a assunção e a adaptação à condição do papel parental, e o papel dos enfermeiros. Há comportamentos promotores da negociação, que tendem para a naturalidade do processo de negociar, não obstante, existirem também comportamentos por parte dos enfermeiros que inibem a negociação. O apoio após a alta é reconhecido como um importante suporte após o regresso da criança a casa. Conclusão: Os achados permitem uma clarificação dos papéis dos diferentes intervenientes na negociação de cuidados, com potencial para contribuir para a implementação de ações favorecedoras do processo negocial de uma forma mais sustentada e intencional.

Resumo Objetivo: O herpes genital (HG) é uma das infeções sexualmente transmissíveis (IST) mais comuns. As infeções genitais pelo vírus herpes simples tipo 1 (HSV-1) têm aumentado, com uma mudança no sentido da sua predominância em muitos países desenvolvidos. O objetivo deste estudo foi caracterizar a população com HG diagnosticada no Centro Hospitalar Universitário de Lisboa Central, Lisboa, Portugal. Métodos: Foi realizada uma análise retrospetiva dos diagnósticos de HG confirmados laboratorialmente entre 2017 e 2021. O diagnóstico foi estabelecido por PCR em tempo real, em amostras colhidas por zaragatoa de lesões mucocutâneas suspeitas. Foram revistos os registos clínicos dos doentes incluídos e analisados os dados de interesse. Resultados: No período de estudo, 239 doentes foram diagnosticados com HG, dos quais 76.6% (n = 183) foram causados pelo HSV tipo 2 (HSV-2). A maioria dos doentes eram do sexo masculino (68%; n = 163), com idade média de 35.7 anos. Em comparação com o grupo diagnosticado com HSV-2, a idade média foi significativamente menor no grupo HSV-1 (28.6 anos vs. 37.9; p < 0.001) e a proporção de doentes com manifestação clínica inicial de HG foi significativamente maior neste último (67.8% vs. 30%; p < 0.001). A infeção concomitante pelo vírus da imunodeficiência humana foi detetada em 17.6% dos indivíduos, sendo significativamente mais prevalente entre homens e no grupo HSV-2 (p = 0.018). Conclusões: O HSV-2 foi a causa mais comum de HG durante todo o estudo. Ainda assim, à semelhança de outros estudos europeus, os doentes com HSV-1 eram mais jovens e a proporção de infeção inicial neste grupo foi significativamente maior.

Abstract Objective: Genital herpes (GH) is one of the most common sexual transmitted infections. Herpes simplex virus type 1 (HSV-1) genital infections have been increasing, with a shift toward its predominance in many developed countries. The aim of this study was to characterize the population with GH diagnosed in Centro Hospitalar Universitário de Lisboa Central, Lisbon, Portugal. Methods: A retrospective analysis of all laboratory-confirmed diagnosis of GH between 2017 and 2021 was conducted. The diagnosis was established by real-time polymerase chain reaction, in samples collected by swabbing suspicious mucocutaneous lesions. Medical records of included patients were reviewed and data of interest analyzed. Results: During the studied period, a total of 239 patients were diagnosed with GH, from which 76.6% (n = 183) were caused by HSV type 2 (HSV-2). Most patients were men (68%; n = 163), with a mean age of 35.7 years. Compared to the group diagnosed with HSV-2, the mean age was significantly lower in the HSV-1 group (28.6 years vs. 37.9; p < 0.001) and the proportion of patients with first clinical manifestations of GH was significantly higher in the latest (67.8% vs. 30%; p < 0.001). Concomitant infection with human immunodeficiency virus was detected in 17.6% of the subjects, being significantly more prevalent among men and within the HSV-2 group (p = 0.018). Conclusions: HSV-2 remained the most common cause of GH throughout the study. Even so, similar to other European studies, HSV-1 patients were younger and the proportion of initial infection in this group was significantly higher.

Abstract Objectives: Intraoperative bleeding may compromise the results of functional endoscopic sinus surgery (FESS). Tranexamic acid (TXA) and hot saline irrigations (HSI) are potentially useful in reducing blood loss during FESS. To date, no review comprehensively compared outcomes between TXA and HSI. The objective was to bring an insight between these two modalities in FESS. Study design: Systematic review and meta-analysis Material and Methods: The research was performed across PUBMED and Cochrane collaboration databases. A meta-analysis was performed to allow the comparison between TXA and HSI outcomes in FESS. Results: Twelve papers met the eligibility criteria for meta-analysis. HSI presented a more significant reduction in volumetric blood loss compared to TXA (p=0.01). No statistically significant difference was found between HSI and TXA considering surgical field quality (p = 0.99) and duration of surgery (p=0.24). Conclusions: This study suggests similar efficacy of HSI and TXA in improving operative conditions in FESS, although with lower volumetric blood loss in HSI.

O ácido tranexâmico (ATX) e a irrigação com soro quente (ISQ) demonstraram ser úteis na redução da perda sanguínea na CENS. O objetivo desta revisão foi o de comparar os resultados destas duas modalidades na CENS. Desenho do estudo: Revisão sistemática e meta-análise Material e Métodos: A investigação incluiu as bases de dados da PUBMED e Cochrane. Foi efetuada uma meta-análise que explorou os resultados do ATX e da ISQ na CENS. Resultados: Doze trabalhos cumpriram os critérios de elegibilidade para meta-análise. A ISQ apresentou uma redução mais significativa na perda sanguínea comparativamente ao ATX (p=0,01). Não foi encontrada diferença estatisticamente significativa entre ATX e ISQ quanto à qualidade do campo cirúrgico (p = 0,99) e à duração da cirurgia (p=0,24). Conclusões: Este estudo sugere eficácia semelhante entre ATX e ISQ na melhoraria das condições operatórias na CENS, com vantagem da ISQ na perda sanguínea volumétrica.

Abstract Introduction: Fenestrated and branched stent graft technology has come a long way over the past few years, enabling the treatment of complex juxtarenal aneurysms, thoracoabdominal aneurysms, and arch pathologies. Many innovations have been developed, namely device and delivery optimization and technical tricks. These concepts have proven to work well when there is sufficient time to plan and manufacture a custom-made device for the patient. However, this is different in urgent or emergent cases. Using parallel graft techniques or off-the-shelf stent grafts may be efficient in urgent situations, but it is also associated with selection limitations. Recent publications have demonstrated similar mid-term technical and clinical results between physician-modified endografts (PMEGs) and customized devices. The authors aim to describe their institutional series of PMEGs. Methods: The clinical files of all patients undergoing PMEGs were consulted, and demographic data as surgery outcomes were collected. Technical success: creating the intended number of fenestrations, target vessel catheterization, and patency. Procedural success: technical success with adequate aneurysm exclusion and without endoleak in the final angiography. 30-day complications and mortality were also evaluated. Technical and procedural success were assessed, as well as morbidity and mortality. Results: Between December 2020 and December 2022, 3 patients underwent PMEGs. The indications were a juxtarenal aortic aneurysm, a type V thoracoabdominal aneurysm and a persistent type 1a endoleak. All patients were symptomatic, with one stable rupture. All cases were performed with technical and procedural success and no morbidity and mortality at 30 days. Conclusion: Stent graft modification is a valuable and valid tool in emergencies and should be a vascular surgeon's trump card when dealing with complex aortic pathologies. Nevertheless, due to the absence of long-term evidence, it should be reserved for acute patients unfit for open repair and in aneurysms with unfavorable anatomy for an off-the-shelf device.

However, the authors are not aware of the literature on treatments in human whey, made available by pasteurization holder, freeze-drying, spray drying, and high hydrostatic pressure. In this context, the present study applied treatments to human whey and evaluated their lipid and immunological quality. Among the results, a different formation in the triacylglycerol profile was evidenced after the application of spray drying and high hydrostatic pressure, while pasteurization and freeze-drying presented ion intensity close to the control human whey. In addition, pasteurization proved to be adequate for lipid nutritional quality and transformation factor-β1 (TGF-β1) concentration, while an increase in interleukin-10 (IL-10) levels was promoted, between 73 and 80%, after freeze-drying, spray drying, and high hydrostatic pressure. Through the principal component analysis, it is noteworthy that the processes presented divergences in terms of the effects caused, with similarity only between pasteurization and freeze-drying in the composition of fatty acids. However, it was observed that all processes were able to maintain the nutrients. RESUMO Objetivo: Avaliar a qualidade de vida e o nível de estresse relacionada à função visual após a cirurgia de catarata pediátrica em um hospital público brasileiro. Métodos: Estudo prospectivo em crianças de seis a 14 anos submetidas à cirurgia de catarata. A Escala de Stresse Infantil e o Questionário de Função Visual em Crianças foram usados para avaliar o nível de estresse e a qualidade de vida, respectivamente. Ambos os instrumentos foram aplicados por duas psicólogas antes e após a cirurgia. O exame oftalmológico foi realizado por dois oftalmologistas. Os dados coletados no pré e pós-operatório foram comparados. Resultados: Vinte e três crianças (32 olhos) foram incluídas no estudo, nove delas apresentavam catarata bilateral. A média de idade na cirurgia foi de 9,65±2,26 (6 a 14) anos. Um mês após a cirurgia, o equivalente esférico foi de -0,90 ± 1,66D e a acuidade visual corrigida a distância foi de 0,13 ± 0,10 (0-0,3) LogMAR em casos bilaterais e 0,50 ± 0,39 (0-1,3) LogMAR em casos unilaterais (p<0.01). De acordo com a Escala de Stresse Infantil, 77,7% dos casos de catarata bilaterais, e 57,1% dos casos unilaterais mantiveram o nível de estresse e 34,7% das crianças melhoraram o nível de estresse. A análise do Questionário de Função Visual em Crianças foi baseada em pontuações para saúde geral, saúde geral da visão, competência, personalidade e tratamento. ABSTRACT Purpose: To evaluate the quality of life and stress level related to visual function following pediatric cataract surgery in a Brazilian public hospital. Methods: This prospective study analyzed children aged 6-14 years old who underwent cataract surgery. The Childhood Stress Scale and Children’s Visual Function Questionnaire (CVFQ) were used to assess stress levels and quality of life, respectively. Both instruments were applied by two psychologists before and after the surgery. Eye examination was performed by two ophthalmologists. Preoperative and postoperative data were compared. Results: In total, 23 children (32 eyes) were enrolled in the study, of which 9 had bilateral cataracts. The average age group at the time of surgery was 9.65 ± 2.26 (6-14) years old. One month after the surgery, the spherical equivalent was -0.90 ± 1.66D, and the corrected distance visual acuity was 0.13 ± 0.10 (0-0.3) LogMAR in bilateral cases and 0.50 ± 0.39 (0-1.3) LogMAR in unilateral cases (p<0.01). According to the Childhood Stress Scale, 77.7% of the bilateral cases and 57.1% of the unilateral cases had stable stress levels, and 34.7% of the children improved their stress level. The analysis of the CVFQ was based on scores for general health, general vision health, competence, personality, and treatment. After cataract surgery, 78.2% of the patients had improved or maintained CVFQ scores in the general health domain; 82.6%, general vision health; 95.6%, competence; 56.5%, personality; and 78.2%, treatment. Abstract This article aims to analyze the consumption of healthy food consumption markers, according to racial groups of Brazilian women interviewed in the 2019 National Health Survey (NHS). This work was a cross-sectional study with data from 45,148 white and black women, aged ≥ 20 years. The variables used were the consumption of fruits, vegetables and legumes, beans, and fish. The association between color/race and the dietary intake indicators was tested using crude Poisson regression and adjusted to estimate prevalence ratios and 95% confidence intervals (95%CI). The prevalence of the consumption of fruits and vegetables was statistically higher among white women, while fish and beans was higher among black women. After adjusting for socioeconomic and demographic variables, it was found that black women remained only less likely to consume fruit (PR = 0.91; 95% CI: 0.88-0.95) and only more likely to consume beans (PR = 1.07; 95% CI: 1.04-1.10) than whites. There were racial inequalities for the consumption of healthy foods among Brazilian women, indicating that color/race defined a dietary pattern for black women that put them in vulnerable conditions in terms of fruit consumption.

Resumo O objetivo do artigo é analisar o consumo de alimentos marcadores de uma alimentação saudável, segundo os grupos raciais de mulheres brasileiras entrevistadas na Pesquisa Nacional de Saúde 2019. Estudo transversal com dados de 45.148 mulheres brancas e negras de ≥ 20 anos de idade. Os marcadores utilizados foram o consumo de frutas, verduras e legumes e feijão e peixe. A associação de cor/raça com os indicadores de consumo alimentar foi testada por regressão de Poisson bruta e ajustada para estimar razões de prevalência e intervalo de confiança de 95%. A prevalência do consumo de frutas, verduras e legumes foi estatisticamente maior nas brancas, e o de peixes e feijão foi maior nas negras. Após o ajuste pelas variáveis socioeconômicas e demográficas, verificou-se que mulheres negras permaneceram apresentando menores chances apenas de consumir frutas (RP = 0,91; IC95%: 0,88-0,95) e maior somente de consumir feijão (RP = 1,07; IC95%: 1,04-1,10) do que as brancas. Verificaram-se desigualdades raciais para o consumo de alimentos saudáveis entre mulheres brasileiras, indicando que a cor/raça definiu um padrão alimentar para as mulheres negras que as colocam em condições vulneráveis em grande parte do consumo de frutas.

ABSTRACT Objective: To evaluate glycemic control according to the number of daily basal rates (BRs) in type 1 diabetes patients using continuous subcutaneous insulin infusion (CSII). Subjects and methods: Cross-sectional study of patients treated with an open-loop CSII for at least 6 months and using a flash glucose monitoring system. Patients were divided into 2 groups: group 1 (G1) and group 2 (G2), with ≤4 and >4 BRs/24h, respectively. The groups were compared regarding HbA1c, time in range (TIR), time above range (TAR), time below range (TBR), glucose management indicator (GMI), glucose variability and data related to hypoglycemia. Regression models were performed. Results: The study included 99 patients (n = 55 in G1; n = 44 in G2). Median (Interquartile range) overall age was 30 (17) years, with 19.5 (48) and 51 (77) months of CSII use, respectively. The median number of different BRs was 3 (2) for G1 and 6 (2) for G2. There were no differences concerning age, sex, educational stage, weight, and insulin analog used. G2 had longer disease duration, longer CSII use, and higher total basal daily dose/kg. No significant differences regarding HbA1c, median glucose, GMI, TIR, TAR, and CV were found. G2 patients had more hypoglycemia, more asymptomatic hypoglycemia, and higher TBR. After adjusting for potential confounders, G1 maintained a lower risk of asymptomatic hypoglycemia. Conclusion: Programming open-loop CSII devices with more than 4 BRs does not improve metabolic control.
