Resumo Um macho de Pteronura brasiliensis (Carnivora: Mustelidae) foi encontrado morto às margens do Rio Negro, no Pantanal do estado de Mato Grosso do Sul, município de Aquidauana. Dois carrapatos encontrados aderidos à sua pele foram identificados, morfologicamente, como uma ninfa de segundo ínstar de Ornithodoros rostratus (Argasidae) e um macho de Amblyomma sculptum (Ixodidae). Para complementar a identificação morfológica, esses espécimes de carrapatos foram submetidos à extração de DNA e testados por meio de ensaios de PCR, para confirmar a identidade molecular dos espécimes. Além disso, os extraídos dos carrapatos foram testados e revelaram-se negativos nos ensaios de PCR para todos os patógenos testados. Também foram examinados 30 lotes, compostos por 174 indivíduos de O. rostratus, depositados na Coleção Acari do Instituto Butantan, e encontramos material de quatro estados brasileiros, incluindo-se um lote contendo 2 machos e 2 fêmeas de Aquidauana, estado de Mato Grosso do Sul (MS), coletado do solo. Este foi, portanto, o primeiro registro de O. rostratus parasitando P. brasiliensis e o primeiro registro de localidade (Aquidauana). Da mesma forma, A. sculptum é comumente encontrado no Pantanal e relatado aqui, pela segunda vez, parasitando a ariranha, hospedeiro pouco estudado em relação a ectoparasitas. Abstract A male of Pteronura brasiliensis (Carnivora: Mustelidae) was found dead on the banks of the Rio Negro, in the Pantanal wetlands of Mato Grosso do Sul state, Aquidauana municipality. Two ticks found attached to its skin were morphologically identified as a second-instar nymph of Ornithodoros rostratus (Argasidae) and a male of Amblyomma sculptum (Ixodidae). In order to complement the morphological identification, these tick specimens were subjected to DNA extraction, and tested using PCR assays to confirm the molecular identity the specimens. Also, the tick DNA samples were tested and were negative in the PCR assays for all the pathogens tested. We also examined 30 batches, consisting of 174 individuals of O. rostratus deposited in the Acari Collection of the Butantan Institute, and we found material from four Brazilian states, including one batch containing 2 males and 2 females from Aquidauana, of Mato Grosso do Sul state, collected from the soil. This was therefore the first record of O. rostratus parasitizing P. brasiliensis and the first locality record (Aquidauana). Fatores associados às atitudes acerca da morte em estudantes internos de Medicina

Resumo Introdução: O curso de Medicina constitui uma jornada de formação profissional exigente, acentuando-se nos dois últimos anos, no período do internato. Ocorre a exposição a inúmeros fatores estressantes, como o contato com a morte. O morrer envolve aspectos biológicos, sociais, culturais, legais, religiosos e históricos, mas, na medicina, é visto como ato de falha médica por causa da ausência de treinamento para manejá-lo. Objetivo: Este estudo teve como objetivo analisar os fatores associados às atitudes perante a morte em estudantes internos de Medicina. Método: Trata-se de estudo transversal analítico com abordagem quantitativa. A pesquisa foi realizada por meio de questionários aplicados de modo digital a estudantes internos de Medicina de todo país. Utilizaram-se os seguintes instrumentos: um questionário sociodemográfico e de aspectos relacionados à vivência com a morte e a Escala de Avaliação do Perfil de Atitudes acerca da Morte (EAPAM). Resultado: A amostra investigada foi de 171 internos de Medicina, sendo 74,9% do sexo feminino, 45% da religião católica e idade média de 24 anos. No fator medo da morte, houve maior escore nos estudantes de religião católica (p = 0,0395) e nos que não se consideraram preparados para lidar com a morte (p = 0,0010). No fator evitar a morte, maior escore no estado civil casado (p = 0,0147) e nos estudantes que não se consideraram preparados para lidar com a morte (p = 0,0020). Na aceitação por escape, maior escore nos estudantes de religião protestante (p = 0,0270), nos que referiram ter atendido paciente que faleceu (p = 0,0030) e nos que afirmaram que não se consideram empáticos (p = 0,0261). Na aceitação neutra, maior escore nos estudantes adeptos de outras religiões (p = 0,0296) e nos que afirmaram ter orientação sexual homoafetiva (p = 0,0398). Abstract Introduction: The medicine degree is a demanding journey of professional training, intensified in the final two years during the internship period. Students are exposed to numerous stressful factors, the most impactful of which is contact with death. Dying involves biological, social, cultural, legal, religious, and historical aspects, but when it comes to medicine, it is considered negligence, a failure caused by the lack of training. Objective: To analyze the factors associated with the frame of mind of medical interns towards death. Methods: This is an analytical cross-sectional study with a quantitative approach. The research was conducted through questionnaires applied digitally to medicine interns from all over Brazil. The instruments used were a sociodemographic questionnaire and aspects related to living with death and, the other, the Profile Assessment Scale of Attitudes About Death (EAPAM). Results: The study sample consisted of 171 medical interns, of which 74.9% were female, 45% were Catholic, and had an average age of 24 years old. In the “fear of death” domain, there was a higher score among Catholic students (p=0.0395), who also did not consider themselves well-prepared to deal with death. For “death avoidance”, there was a higher score among married students (p=0.0147), also those who did not consider themselves prepared to deal with death (p=0.0020). For “escape acceptance”, there was a higher score among Protestant students (p=0.0270), those who reported having attended a patient who died (p=0.0030), and those who stated that they did not consider themselves empathetic (p=0.0261). For “neutral acceptance”, there was a higher score among students adhering to other religions (p=0.0296) and those who claimed to have a homosexual orientation (p=0.0398). Abstract Accurate measurement of the residual stress in welded joints is still a major challenge for both welding quality control and non-destructive testing. One of the most recent and innovative magnetic techniques for this purpose, known as Magnetic Barkhausen Noise (MBN), is based on the reorganization of magnetic domains (regions with uniform magnetic orientations) in the presence of a varying magnetic field in the studied ferromagnetic material. A significant difficulty in using this method for the analysis of welded joints lies in evaluating of the contribution that each altered material property induces in the resulting noise signal. Therefore, the objective of this study was to correlate the MBN signal with the typical changes that occur in the weld bead and its specific regions during the welding process applied to ASTM A36 steel. ABSTRACT Purpose: To evaluate patient characteristics and factors associated with surgical resection in patients with Crohn’s disease (CD). Methods: An analysis was performed on data from 295 patients with CD in follow-up from 2001 to 2018. Medical record data comprised age, gender, location, behavior and duration of the CD, smoking, and extraintestinal manifestation. Patients were divided into two groups according to the presence or absence of surgical resection. Results: Out of the 295 patients with CD, 155 underwent surgical resection (53.2% male, mean age: 43.88 ± 14.35 years). The main indications for surgery were stenosis (44.5%), clinical intractability (15.5%), and intra-abdominal fistulas (15.5%). Smoking (p < 0.001), longer CD duration (p < 0.0001), ileo-colonic location (p = 0.003), stenosing behavior (p < 0.0001), and fistulizing behavior (p < 0.0001) were significantly associated with surgical resection. Initial use of biological was significantly more frequent in the group of patients without surgical resection (p < 0.001). Conclusions: Patients with CD still frequently need surgical treatment. Smoking (current or past), longer disease time, stenosing and fistulizing behavior, and ileo-colonic localization in CD patients were associated with a higher risk of surgery. RESUMO A Atenção Primária à Saúde (APS) é a base do Sistema Único de Saúde (SUS). Fortalecê-la exige permanente avaliação de seus processos e resultados. A Rede de Pesquisa em APS (Rede APS) promove intenso debate sobre o fortalecimento do modelo de APS de base territorial, comunitária e ordenadora da Rede de Atenção à Saúde no SUS. Este ensaio tem o objetivo de apresentar um histórico das iniciativas de monitoramento e avaliação da APS brasileira e a proposição de uma agenda de pesquisa implicada com a consolidação do SUS e o fortalecimento da Estratégia Saúde da Família (ESF). É resultante da reflexão crítica acerca da produção de conhecimento sobre APS no Brasil, fruto do debate em torno da agenda política e estratégica da Rede APS com participação de atores setoriais. A agenda está estruturada em oito eixos centrais, considerando o modelo de APS a ser integrado às redes de atenção, gestão e financiamento, gestão do trabalho, saúde digital, infraestrutura, atenção integral à saúde, e monitoramento e avaliação. ABSTRACT Primary Health Care (PHC) is the base of the Unified Health System (SUS). Strengthening it requires constant evaluation of its processes and outcomes. The PHC Research Network (Rede APS) is promoting an intense debate on strengthening the PHC model, which is territorialized, community-based, and organizes the Health Care Network in the SUS. This essay aims to present a history of initiatives to monitor and evaluate Brazilian PHC and to propose a research agenda involved in consolidating the SUS and strengthening the Family Health Strategy (ESF). RESUMO RACIONAL: A endometriose profunda infiltrativa (EP) pode afetar órgãos abdominais e pélvicos, tais como o intestino e a bexiga, necessitando de tratamento para aliviar os sintomas. OBJETIVOS: Estudar e investigar resultados clínicos e cirúrgicos em pacientes com diagnóstico de EP envolvendo o intestino, visando analisar a eficácia dos tratamentos cirúrgicos. MÉTODOS: Foram incluídos todos os casos atendidos de janeiro de 2021 a julho de 2023, com foco em pacientes com 18 anos ou mais com a doença acometendo o intestino. Foram excluídos pacientes sem comprometimento intestinal e aqueles com menos de seis meses de acompanhamento pós-operatório. O envolvimento intestinal foi definido como invasão direta da parede intestinal ou necessidade de lise de aderências para ressecção completa. Os desfechos primários foram lise de aderências, raspagem retal e excisão discóide (grupo não colectomia), e ressecção segmentar (grupo colectomia), juntamente com complicações cirúrgicas como fístulas, monitoradas por até 30 dias. RESULTADOS: Das 169 pacientes com EP tratadas cirurgicamente, 76 preencheram os critérios de inclusão. Não foi indicado colectomia em 50 (65,7%) pacientes, enquanto 26 (34,2%) foram submetidos à retossigmoidectomia (RTS). A diarreia durante a menstruação foi o sintoma mais prevalente no grupo RTS (19,2% vs. 6,0%, p<0,001). Os resultados cirúrgicos indicaram tempos operatórios e internações hospitalares mais longos para o grupo de ressecção segmentar, respectivamente, 186,5 vs. 104 min (p<0,001) e 4 vs. 2 dias, (p<0,001). As complicações graves (Clavien-Dindo ≥3) tiveram prevalência global de 6 (7,9%) casos, sem diferença entre os grupos. Não houve mortalidade relatada. Lesões mais graves e sintomas específicos como disquezia e sangramento retal foram associados a maior probabilidade de indicação de RTS. A regressão bayesiana destacou a diarreia próxima à menstruação como um forte preditor de ressecção segmentar. CONCLUSÕES: Em pacientes com endometriose envolvendo os intestinos, sintomas como disquezia, sangramento retal e diarreia relacionada ao período menstrual predizem a indicação de RTS. No entanto, as taxas de complicações graves não diferiram significativamente entre o grupo de ressecção segmentar e o grupo sem colectomia. ABSTRACT BACKGROUND: Deep penetrating endometriosis (DE) can affect abdominal and pelvic organs like the bowel and bladder, requiring treatment to alleviate symptoms. AIMS: To study and investigate clinical and surgical outcomes in patients diagnosed with DE involving the intestines, aiming to analyze the effectiveness of surgical treatments. METHODS: All cases treated from January 2021 to July 2023 were included, focusing on patients aged 18 years or older with the disease affecting the intestines. Patients without intestinal involvement and those with less than six months of post-surgery follow-up were excluded. Intestinal involvement was defined as direct invasion of the intestinal wall or requiring adhesion lysis for complete resection. Primary outcomes were adhesion lysis, rectal shaving, disc excision (no-colectomy group), and segmental resection (colectomy group) along with surgical complications like anastomotic leak and fistulas, monitored for up to 30 days. RESULTS: Out of 169 patients with DE surgically treated, 76 met the inclusion criteria. No colectomy treatment was selected for 50 (65.7%) patients, while 26 (34.2%) underwent rectosigmoidectomy (RTS). Diarrhea during menstruation was the most prevalent symptom in the RTS group (19.2 vs. 6%, p<0.001). Surgical outcomes indicated longer operative times and hospital stays for the segmental resection group, respectively 186.5 vs. 104 min (p<0.001) and 4 vs. 2 days, (p<0.001). Severe complications (Clavien-Dindo ≥3) had an overall prevalence of 6 (7.9%) cases, without any difference between the groups. There was no mortality reported. Larger lesions and specific symptoms like dyschezia and rectal bleeding were associated with a higher likelihood of RTS. Bayesian regression highlighted diarrhea close to menstruation as a strong predictor of segmental resection. CONCLUSIONS: In patients with DE involving the intestines, symptoms such as dyschezia, rectal bleeding, and menstrual period-related diarrhea predict RTS. However, severe complication rates did not differ significantly between the segmental resection group and no-colectomy group. ABSTRACT The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. ABSTRACT The factors and their levels influencing the seed germination process are well-known for most traditional forest species. But, for Jacaranda micrantha, a native and aesthetically appealing species, some factors influencing the germination process are not well described. So, we evaluated the effect of different light conditions and soaking on the germination of seeds. Four light intensity treatments (5, 25, 50, and 100%) and four soaking times were tested (0, 12, 24 and 48 hours). In both procedures, a completely randomized design was applied, with four treatments of four replicates containing 25 seeds each. Daily evaluations were performed to determine the germination percentage and the germination speed index (IVG). Seeds of J. micrantha presented a better germinative potential in lower light intensities, but more abnormal seedlings were observed at very low-intensity light (5%). Abstract Dynamic probing light (DPL) is available among a wide range of dynamic penetrometers. This equipment can be used in locations where conventional sounding equipment may have limited access. Here, the shear wave velocity (Vs) determination by the downhole seismic technique is incorporated to the DPL to develop a hybrid test, the seismic DPL test, for the preliminary characterization of tropical soils. The corresponding equipment, test procedure and data interpretation of the seismic DPL (S-DPL) test are presented and discussed. S-DPL test campaigns were carried out at two experimental research sites where crosshole seismic tests were conducted to provide reference Vs profiles. The Vs profiles determined with the S-DPL test are in accordance with those determined with the crosshole test for both sites. ABSTRACT Primary Health Care (PHC) is the base of the Unified Health System (SUS). Strengthening it requires constant evaluation of its processes and outcomes. The PHC Research Network (Rede APS) is promoting an intense debate on strengthening the PHC model, which is territorialized, community-based, and organizes the Health Care Network in the SUS. This essay aims to present a history of initiatives to monitor and evaluate Brazilian PHC and to propose a research agenda involved in consolidating the SUS and strengthening the Family Health Strategy (ESF). It is the result of a critical reflection on the production of knowledge on PHC in Brazil, the fruit of a debate on the political and strategic agenda of the PHC Network with the participation of sectoral actors. The agenda is structured around eight central axes, considering the PHC model to be integrated into care networks, management and financing, work management, digital health, infrastructure, comprehensive health care, and monitoring and evaluation. The PHC Research Network advocates universal coverage by a qualified ESF and the construction of a National Monitoring and Evaluation Policy to guide PHC management and care in the SUS.
RESUMO A Atenção Primária à Saúde (APS) é a base do Sistema Único de Saúde (SUS). Fortalecê-la exige permanente avaliação de seus processos e resultados. A Rede de Pesquisa em APS (Rede APS) promove intenso debate sobre o fortalecimento do modelo de APS de base territorial, comunitária e ordenadora da Rede de Atenção à Saúde no SUS. Este ensaio tem o objetivo de apresentar um histórico das iniciativas de monitoramento e avaliação da APS brasileira e a proposição de uma agenda de pesquisa implicada com a consolidação do SUS e o fortalecimento da Estratégia Saúde da Família (ESF). É resultante da reflexão crítica acerca da produção de conhecimento sobre APS no Brasil, fruto do debate em torno da agenda política e estratégica da Rede APS com participação de atores setoriais. A agenda está estruturada em oito eixos centrais, considerando o modelo de APS a ser integrado às redes de atenção, gestão e financiamento, gestão do trabalho, saúde digital, infraestrutura, atenção integral à saúde, e monitoramento e avaliação. A Rede APS defende a universalização da cobertura por uma ESF qualificada e a construção de uma Política Nacional de Monitoramento e Avaliação para orientar a gestão e o cuidado da APS no SUS.

Esto como resultado de la reflexión crítica acerca de la producción de conocimiento sobre APS en Brasil, fruto del debate en relación con la agenda política y estratégica de la Rede APS con la participación de actores sectoriales. Objectives: To present evidence-based guidelines for clinical practice regarding religiosity and spirituality in mental health care in Brazil. Methods: A systematic review was conducted to identify potentially eligible articles indexed in the PubMed, PsycINFO, SciELO, LILACS, and Cochrane databases. A summary of recommendations and their levels of evidence was produced in accordance with Oxford Centre for Evidence-Based Medicine guidelines. Results: The systematic review identified 6,609 articles, 41 of which satisfied all inclusion criteria. Taking a spiritual history was found to be an essential part of a compassionate and culturally sensitive approach to care. It represents a way of obtaining relevant information about the patient’s religiosity/spirituality, potential conflicts that could impact treatment adherence, and improve patient satisfaction. Consistent evidence shows that reported perceptual experiences are unreliable for differentiating between anomalous experiences and psychopathology. Negative symptoms, cognitive and behavioral disorganization, and functional impairment are more helpful for distinguishing pathological and non-pathological anomalous experiences. Conclusion: Considering the importance of religiosity/spirituality for many patients, a spiritual history should be routinely included in mental health care. Anomalous experiences are highly prevalent, requiring a sensitive and evidence-based approach to differential diagnosis.
