Resumen Introducción: Frente al COVID-19 se adoptaron medidas de restricción social que influyeron en los hábitos de vida de la población, aumentando el sedentarismo. Considerando la alta complejidad y la dedicación exigida por la carrera de Medicina, se torna relevante investigar el efecto pandémico sobre la práctica de ejercicio físico y la conducta sedentaria de estos estudiantes. Objetivos: Identificar los cambios en la actividad física y en la conducta sedentaria auto-reportados por los estudiantes de Medicina de la ciudad de Salvador, Bahía, Brasil, antes y durante el auto-confinamiento impuesto por la pandemia de COVID-19. Métodos: Estudio longitudinal, prospectivo, cuantitativo, realizado con estudiantes de medicina regularmente matriculados, mayores de 18 años. Resultados: Se incluyeron 268 estudiantes de medicina, predominantemente mujeres (65,7%), blancas (50,0%) y morenas (38,8%), de 24,2 ± 5,5 años de edad, solteras (90,7%), en el ciclo clínico (59,0%), en instituciones privadas (78,4%). No se identificaron diferencias estadísticamente significativas en el nivel de actividad física y el tiempo dedicado a actividades moderadas y vigorosas. Sin embargo, hubo una reducción en el tiempo de caminata (p<0,00001) y un aumento en la conducta sedentaria (p=0,001) durante la pandemia COVID-19, siendo mayor el impacto entre las mujeres (p=0,0009). Conclusión: Las repercusiones de la pandemia COVID-19 se manifestaron en el aumento de la conducta sedentaria entre los estudiantes de medicina, especialmente entre las mujeres, además de la reducción del tiempo de actividad dedicado a caminar. Son necesarios estudios longitudinales para analizar las consecuencias a medio y largo plazo de este cambio en los hábitos de vida saludables de los estudiantes de medicina. ABSTRACT Introduction: In order to face COVID-19, social restriction measures were adopted that influenced the population's living habits, increasing sedentary lifestyle. Considering the high complexity and dedication required by the Medicine course, it becomes relevant to investigate the effect of the pandemic on the practice of physical exercise and sedentary behavior of these students. Objective: Identify changes in physical activity and sedentary behavior self-reported by medical students in the city of Salvador, Bahia, before and during the self-confinement imposed by the COVID-19 pandemic. Methods: Longitudinal, prospective, quantitative study carried out with regularly enrolled medical students over 18 years of age. A virtual, structured, anonymous, self-completed questionnaire was applied, containing sociodemographic and academic aspects. In addition, the International Physical Activity Questionnaire (IPAQ) was applied in its short version, considering the period before and during the pandemic. Through this, the level of physical activity, sedentary behavior and time spent walking and performing moderate and vigorous activities were measured. Results: 268 medical students were included, predominantly women (65.7%), white (50.0%) and mixed race (38.8%), aged 24.2 ± 5.5 years, single (90.7%), attending the clinical cycle (59.0%), in private institutions (78.4%). No statistically significant differences were identified in the level of physical activity and in the time spent with moderate and vigorous activities. However, there was a reduction in walking time (p<0.00001) and an increase in sedentary behavior (p=0.001) during the COVID-19 pandemic, with the greatest impact among women (p=0.0009). Conclusion: The repercussions of the COVID-19 pandemic have been shown to increase the sedentary behavior of medical students, especially among women, and to reduce activity time spent with walking. Resumo Considerando a instituição da Rede de Atenção à Pessoa com Deficiência (RCPD) no Brasil, o estudo analisa a distribuição espacial e a tendência temporal da implantação de serviços especializados que receberam apoio financeiro nos primeiros oito anos dessa política. Realizamos um estudo ecológico com base nos dados do Cadastro Nacional de Estabelecimentos de Saúde no período de abril/2012 a março/2020. Uma regressão joinpoint foi usada para análise de tendência temporal e mapas temáticos foram produzidos para análise espacial de modalidades de reabilitação e tipos de serviços. Os serviços mais disponíveis foram reabilitação física e intelectual. As regiões Sudeste e Nordeste apresentaram maior concentração de serviços especializados. Apesar do menor número de serviços, houve crescimento médio anual entre 9,6% e 41,3%. Esse achado indica aumento de serviços especializados para pessoas com deficiência no período analisado, mas ainda se verificam lacunas assistenciais nas macrorregiões do Brasil.

RESUMO Na região Amazônica, o óleo de andiroba (Carapa guianensis) é um dos produtos mais utilizados pela população local devido às suas propriedades medicinais. Anteriormente, nosso grupo descreveu a atividade leishmanicida de três frações do óleo de andiroba ricas em limonóides. Este trabalho objetivou avaliar a atividade antioxidante e alterações ultraestruturais em Leishmania amazonensis tratadas com essas frações ricas em limonóides do óleo de andiroba: 6α,11β-diacetoxigedunina; 11β-hidroxigedunina; e uma fração com ambas 6α,11β-diacetoxigedunina + 11β-hidroxigedunina. A atividade antioxidante foi avaliada pelo método DPPH, e alterações ultraestruturais foram avaliadas por microscopia eletrônica de transmissão. A fração com ambas 6α,11β-diacetoxigedunina + 11β-hidroxigedunina exibiu a melhor atividade antioxidante (IC50 = 330,35 ± 8,60 µg mL-1; p = 0.0022) comparado com a fração 6α,11β-diacetoxigedunina. ABSTRACT In the Amazon region, andiroba (Carapa guianensis) oil is among the most used products by the local population due to its medicinal properties. Previously, our group described the leishmanicidal activity of three limonoid-rich fractions from andiroba oil. This work aimed to evaluate the antioxidant activity and ultrastructural changes in Leishmania amazonensis treated with these limonoid-rich fractions from andiroba oil: 6α,11β-diacetoxygedunin; 11β-hydroxygedunin; and a fraction with both 6α,11β-diacetoxygedunin +11β-hydroxygedunin. The antioxidant activity was evaluated by the DPPH method and ultrastructural changes were evaluated by transmission electron microscopy. The fraction with both 6α,11β-diacetoxygedunin + 11β-hydroxygedunin displayed the best antioxidant activity (IC50 330.35 ± 8.60 µg mL-1; p=0.0022) compared to the 6α,11β-diacetoxygedunin fraction. Abstract Hemophagocytic lymphohistiocytosis (HLH) is a rare genetic hyperinflammatory syndrome that occurs early in life. Macrophage activation syndrome (MAS) usually refers to a secondary form of HLH associated with autoimmunity, although there are other causes of secondary HLH, such as infections and malignancy. In this article, we reviewed the concepts, epidemiology, clinical and laboratory features, diagnosis, differential diagnosis, prognosis, and treatment of HLH and MAS. Resumo Antecedentes Encefalites autoimunes (EAIs) são um grupo de doenças inflamatórias caracterizadas pela presença de anticorpos contra antígenos neuronais e gliais, que ocasionam sintomas psiquiátricos subagudos, queixas de memória e distúrbios anormais do movimento. A maioria dos pacientes é jovem, e o atraso no tratamento está associado a pior prognóstico. Objetivo Com o apoio da Academia Brasileira de Neurologia (ABN) e da Sociedade Brasileira de Neurologia Infantil (SBNI), desenvolvemos um consenso sobre o diagnóstico e o tratamento da EAIs no Brasil utilizando a metodologia Delphi. Métodos Um total de 25 especialistas, incluindo neurologistas e neurologistas infantis, foram convidados a participar. Resultados Os especialistas concordaram que os pacientes com critérios de possíveis EAIs devem ser submetidos ao rastreio de anticorpos antineuronais no soro e no líquido cefalorraquidiano (LCR) por meio das técnicas de ensaio baseado em tecidos (tissue-based assay, TBA, em inglês) e ensaio baseado em células (cell-based assay, CBA, em inglês). O tratamento de segunda linha inclui rituximabe e ciclofosfamida. Bortezomib e tocilizumab são opções de tratamento de terceira linha. A maioria das crises epilépticas nas EAIs são sintomáticas, e os fármacos anticrise podem ser desmamadas após a fase aguda. Abstract Background Autoimmune encephalitis (AIE) is a group of inflammatory diseases characterized by the presence of antibodies against neuronal and glial antigens, leading to subacute psychiatric symptoms, memory complaints, and movement disorders. The patients are predominantly young, and delays in treatment are associated with worse prognosis. Objective With the support of the Brazilian Academy of Neurology (Academia Brasileira de Neurologia, ABN) and the Brazilian Society of Child Neurology (Sociedade Brasileira de Neurologia Infantil, SBNI), a consensus on the diagnosis and treatment of AIE in Brazil was developed using the Delphi method. Methods A total of 25 panelists, including adult and child neurologists, participated in the study. Results The panelists agreed that patients fulfilling criteria for possible AIE should be screened for antineuronal antibodies in the serum and cerebrospinal fluid (CSF) using the tissue-based assay (TBA) and cell-based assay (CBA) techniques. Children should also be screened for anti-myelin oligodendrocyte glucoprotein antibodies (anti-MOG). Treatment should be started within the first 4 weeks of symptoms. The first-line option is methylprednisolone plus intravenous immunoglobulin (IVIG) or plasmapheresis, the second-line includes rituximab and/or cyclophosphamide, while third-line treatment options are bortezomib and tocilizumab. Most seizures in AIE are symptomatic, and antiseizure medications may be weaned after the acute stage. Abstract Objective The present study aimed to investigate FOXO3a deregulation in Uterine Smooth Muscle Tumors (USMT) and its potential association with cancer development and prognosis. Methods The authors analyzed gene and protein expression profiles of FOXO3a in 56 uterine Leiomyosarcomas (LMS), 119 leiomyomas (comprising conventional and unusual leiomyomas), and 20 Myometrium (MM) samples. The authors used techniques such as Immunohistochemistry (IHC), FISH/CISH, and qRT-PCR for the present analyses. Additionally, the authors conducted an in-silico analysis to understand the interaction network involving FOXO3a and its correlated genes. Results This investigation revealed distinct expression patterns of the FOXO3a gene and protein, including both normal and phosphorylated forms. Expression levels were notably elevated in LMS, and Unusual Leiomyomas (ULM) compared to conventional Leiomyomas (LM) and Myometrium (MM) samples. This upregulation was significantly associated with metastasis and Overall Survival (OS) in LMS patients. Intriguingly, FOXO3a deregulation did not seem to be influenced by EGF/HER-2 signaling, as there were minimal levels of EGF and VEGF expression detected, and HER-2 and EGFR were negative in the analyzed samples. In the examination of miRNAs, the authors observed upregulation of miR-96-5p and miR-155-5p, which are known negative regulators of FOXO3a, in LMS samples. Conversely, the tumor suppressor miR-let7c-5p was downregulated. Resumo Introdução A doença de Parkinson (DP) é a segunda no grupo das doenças neurodegenerativas crônicas progressivas, com alta prevalência e incidência anual. Portanto, faz-se necessário atualizar as opções terapêutico-ocupacionais, vigentes ou em desenvolvimento, para essa patologia. Objetivo Mapear as práticas e abordagens de terapeutas ocupacionais conduzidas por meio de intervenções terapêuticas na DP. Método Revisão conduzida em atenção ao “Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist”, baseada na metodologia do Joanna Briggs Institute. A pergunta de pesquisa foi elaborada a partir do acrônimo PCC (Paciente, Conceito e Contexto), cujos termos principais e secundários foram consultados no DeCS/MeSH. As buscas foram realizadas em cinco plataformas científicas entre 2011 e 2020. Os pesquisadores cegos foram comparados pelo grau de concordância aferido pelo coeficiente kappa de Cohen. Abstract Introduction Parkinson’s disease (PD) ranks second among progressive chronic neurodegenerative diseases, with high prevalence and annual incidence. Therefore, it is necessary to update the current and developing occupational therapy options for this pathology. Objective To map the practices and approaches of occupational therapists conducted through therapeutic interventions in PD. Method Review conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist, based on the methodology of the Joanna Briggs Institute. The research question was formulated using the PCC (Patient, Concept, and Context) acronym, with primary and secondary terms consulted in the DeCS/MeSH multilingual thesaurus. Searches were conducted on five scientific databases between 2011 and 2020. Blind researchers were compared by the degree of agreement measured by Cohen’s kappa coefficient. Research in outpatient environments is the primary setting for knowledge production in the field. Abstract Bladder cancer is the tenth most frequently diagnosed cancer globally. Classification of high- or low-grade tumors is based on cytological differentiation and is an important prognostic factor. LncRNAs regulate gene expression and play critical roles in the occurrence and development of cancer, however, there are few reports on their diagnostic value and co-expression levels with genes, which may be useful as specific biomarkers for prognosis and therapy in bladder cancer. Thus, we performed a marker lesion study to investigate whether gene/lncRNA expression in urothelial carcinoma tissues may be useful in differentiating low-grade and high-grade tumors. RT-qPCR was used to evaluate the expression of the JHDM1D gene and the lncRNAs CTD-2132N18.2, SBF2-AS1, RP11-977B10.2, CTD-2510F5.4, and RP11-363E7.4 in 20 histologically diagnosed high-grade and 10 low-grade tumors. A protein-to-protein interaction network between genes associated with JHDM1D gene was constructed using STRING website. The results showed a moderate (positive) correlation between CTD-2510F5.4 and CTD2132N18.2. Resumo Objetivos: investigar a associação do estado nutricional com deficiência de iodo urinário em gestantes atendidas em Unidades Básicas de Saúde do município de São Luís - MA. Métodos: estudo transversal, realizado com 261 gestantes. Foram coletadas amostras de urina para análise de determinação do iodo urinário, considerou-se deficiência o valor <150 µg/L e adequado, valor ≥150 µg/L. Foram aferidas as medidas de peso e altura no momento da coleta e investigação do peso pré-gestacional para cálculo do IMC. As variáveis contínuas foram expressas em média, desvio padrão, mediana, percentil 25 e 75 e as categóricas em frequência simples e relativa e em percentual. Análises multivariadas investigaram associação entre IMC pré-gestacional e atual e UIC (Concentração urinária de iodo). Resultados: a média do IMC pré-gestacional foi 24,3 ± 4,5 kg/m2 e do IMC atual 27,2 ± 6,5 kg/m2. A mediana de concentração de iodo urinário foi de 181,3 µ/L (113,2-271,7) e 40,6% da amostra possuía deficiência de iodo. Abstract Objectives: to investigate the association of nutritional status with urinary iodine deficiency in pregnant women attending the Basic Health Units in the city of São Luís - Maranhão. Methods: cross-sectional study carried out with 261 pregnant women. Urine samples were collected for analysis of determination of urinary iodine. Abstract Systemic autoinflammatory diseases (SAIDs) arise from dysregulated innate immune system activity, which leads to systemic inflammation. These disorders, encompassing a diverse array of genetic defects classified as inborn errors of immunity, are significant diagnostic challenges due to their genetic heterogeneity and varied clinical presentations. Although recent advances in genetic sequencing have facilitated pathogenic gene discovery, approximately 40% of SAIDs patients lack molecular diagnoses. SAIDs have distinct clinical phenotypes, and targeted therapeutic approaches are needed. This review aims to underscore the complexity and clinical significance of SAIDs, focusing on prototypical disorders grouped according to their pathophysiology as follows: (i) inflammasomopathies, characterized by excessive activation of inflammasomes, which induces notable IL-1β release; (ii) relopathies, which are monogenic disorders characterized by dysregulation within the NF-κB signaling pathway; (iii) IL-18/IL-36 signaling pathway defect-induced SAIDs, autoinflammatory conditions defined by a dysregulated balance of IL-18/IL-36 cytokine signaling, leading to uncontrolled inflammation and tissue damage, mainly in the skin; (iv) type I interferonopathies, a diverse group of disorders characterized by uncontrolled production of type I interferons (IFNs), notably interferon α, β, and ε; (v) anti-inflammatory signaling pathway impairment-induced SAIDs, a spectrum of conditions characterized by IL-10 and TGFβ anti-inflammatory pathway disruption; and (vi) miscellaneous and polygenic SAIDs. The latter group includes VEXAS syndrome, chronic recurrent multifocal osteomyelitis/chronic nonbacterial osteomyelitis, Schnitzler syndrome, and Still’s disease, among others, illustrating the heterogeneity of SAIDs and the difficulty in creating a comprehensive classification. A partir da trajetória de Marianna, mina, escrava de Jacinto de Araujo, criado de D. João VI que regressou com o monarca para Portugal, procuro compreender os caminhos percorridos por cativos que ali desembarcaram em companhia de seus senhores. Abstract: This paper examines the circumstances under which slavery was discussed in Portugal, upon Dom João VI’s return on April 26, 1821. By drawing on the trajectory of the enslaved Marianna, Mina, owned by Jacinto de Araujo, Dom João VI’s servant, who returned with the monarch to Portugal, I seek to understand the paths taken by captives who landed there with their masters. From Marianna’s experience I ponder why the king granted permission to masters returned from Brazil to remain with their enslaved in Portugal in contempt of the Decrees of September 19, 1761 and March 10, 1800. Furthermore, I reflect upon what the royal authorities understood about slavery and freedom in a kingdom that had just become a constitutional monarchy with an anti-slavery legislation dating back to the late 1760s. The pheromones 9-ODA ((E)-9-oxodec-2-enoic acid) and 9-HDA ((E)-9-hydroxydec-2-enoic acid), produced by the mandibular gland of the queen, have various functions within the colony, including inhibition of ovarian development of workers, attraction of swarms, and stabilization of the cluster group. An example of reaction of the colony to adverse conditions is absconding, which is defined as mass exit of all of the adult individuals, leaving even brood and food. In this study, the pheromones 9-ODA and 9-HDA were determined in virgin and normal mated, egg-laying European Carniolan and Africanized queens, as well as in mated queens that were in absconding colonies, using liquid chromatography with UV detection developed methodology. Absconding was induced by maintaining free-flying five-standard-Langstroth-frame colonies in a chamber artificially heated to 45 ºC. The obtained results showed that the amount of 9-ODA in Africanized queens (6.56 µg bee-1) is very low compared with European queens. However, large amounts of 9-HDA were found in the queens of absconding Africanized swarms (107.4 µg bee-1).
