RESUMO A rinite ocupacional é uma patologia inflamatória da mucosa nasal caracterizada por sintomas nasais intermitentes ou persistentes e/ou limitação variável do fluxo aéreo nasal e/ou hipersecreção nasal devido a causas atribuíveis a um ambiente de trabalho específico. Pode ser classificada em alérgica (IgE-mediada ou não IgE-mediada) ou não alérgica. A rinite ocupacional alérgica é habitualmente a expressão clínica inaugural da sensibilização das vias aéreas a um alergénio presente no ambiente laboral e pode preceder o aparecimento da asma ocupacional. Embora o diagnóstico seja essencialmente clínico, pelos aspetos médico-legais envolvidos, torna-se importante identificar os agentes etiológicos, sendo a prova de provocação nasal o gold-standard diagnóstico; no entanto, quando não está disponível, poderá ser feita uma avaliação no local de trabalho. A opção terapêutica mais eficaz consiste na evicção da exposição. Neste artigo reunimos a evidência científica atual relativa à rinite ocupacional, partindo de um caso clínico para contextualização do problema.

ABSTRACT Occupational rhinitis is an inflammatory disease of the nasal mucosa, characterized by intermittent or persistent nasal symptoms and/or variable limitation of nasal airflow and/or nasal hypersecretion due to causes attributable to a specific work environment. It can be classified as allergic (IgE-mediated or non-IgE-mediated) or non-allergic. Allergic occupational rhinitis is usually the inaugural clinical expression of airway sensitization to an allergen present in the workplace and may precede the onset of occupational asthma. Although the diagnosis is essentially clinical, due to the medical-legal aspects involved, it is important to identify the etiological agents, being the nasal provocation test considered the gold-standard of diagnosis; however, when not available, a workplace assessment can be made. The most effective therapeutic option is to avoid exposure. In this article we gather the current scientific evidence related to occupational rhinitis, starting from a clinical case to contextualize the problem.

OBJECTIVE: To examine the association between diabetes-related lower-extremity amputation (LEA) and ethnicity, age, source of payment, geographic location, diabetes severity, and health condition in adults with diabetes mellitus type 2 living in border and non-border counties in Texas, United States of America, and to assess intra-border region geographic differences in post-LEA treatment. METHODS: This correlational study was based on secondary data from the 2003 Texas Inpatient Hospital Discharge Data. The sample consisted of individuals 45 years of age and older with type 2 diabetes who had undergone a nontraumatic LEA (n = 5 865). Descriptive statistics and logistic regression analyses were applied. RESULTS: The following characteristics were predictors of LEA: being Hispanic or African American, male, > 55 years old, and a Medicare or Medicaid user, and living in a border county. Persons with moderate diabetes and those who suffered from cardiovascular disease or stroke also had higher odds of undergoing an LEA. Post-LEA occupational therapy was significantly less prevalent among border residents (9.5%) than non-border residents (15.3%) (P < 0.001). CONCLUSION: Understanding the factors that influence diabetes-related LEA may lead to early detection and effective treatment of this disabling consequence of diabetes along the U.S.-Mexico border.

OBJETIVO: Analizar la asociación entre las amputaciones de extremidades inferiores (AEI) relacionadas con la diabetes y el grupo étnico, la edad, la procedencia del pago, la ubicación geográfica, la gravedad de la diabetes y el estado de salud de los adultos que padecen diabetes tipo 2 residentes en los condados fronterizos y no fronterizos de Texas (Estados Unidos de América), y evaluar la diferencias geográficas dentro de la zona fronteriza en cuanto al tratamiento posterior a la amputación. MÉTODOS: Este estudio correlacional se basó en datos secundarios procedentes de la información de egreso de pacientes hospitalizados en Texas durante el año 2003. La muestra estuvo integrada por personas de 45 años o mayores con diabetes tipo 2, que habían sido sometidas a la amputación no traumática de una extremidad inferior (n = 5 865). Se aplicaron estadística descriptiva y análisis de regresión logística. RESULTADOS: Las siguientes características constituyeron factores predictivos de AEI: ser hispano o afroestadounidense, hombre, de 55 años o mayor, beneficiario de Medicare o Medicaid, y residente en un condado fronterizo. Las personas con diabetes moderada que padecían enfermedades cardiovasculares o habían sufrido un accidente cerebrovascular también tenían una mayor probabilidad de ser sometidas a una AEI. La terapia ocupacional posterior a la amputación fue significativamente menos prevalente entre los residentes fronterizos (9,5%) que entre los no fronterizos (15,3%) (P < 0,001). CONCLUSIONES: La comprensión de los factores que influyen en las AEI relacionadas con la diabetes puede conducir a la detección temprana y el tratamiento eficaz de esta secuela discapacitante en la zona fronteriza entre los Estados Unidos y México.

Três ensaios comparativos foram feitos com o objetivo de aperfeiçoar a sensibilidade do diagnóstico da infecção pela Bordetella bronchiseptica a partir de suabes nasais de leitões. O experimento inicial comparou a recuperação de B. bronchiseptica a partir de suabes, simultaneamente inoculados com B. bronchiseptica e algumas bactérias interferentes, imersos em três formulações para transporte (meio Amies com carvão, caldo tripticaseína de soja e tampão de fosfatos segundo Soerensen suplementado com 5% de soro fetal bovino) e submetidos a diferentes temperaturas (10ºC e 27ºC) e períodos de incubação (24, 72 e 120 horas). O experimento subseqüente comparou três meios seletivos (ágar MacConkey, meio seletivo G20G modificado e o meio de ceftiofur) em relação às suas capacidades de recuperação a partir de amostras clínicas. O último experimento comparou a reação em cadeia pela polimerase aos três meios seletivos. No primeiro experimento, a recuperação de B. bronchiseptica nos sistemas de transporte foi melhor a 27ºC e as três formulações tiveram boas performances nesta temperatura, mas o conjunto das análises qualitativa e quantitativa apontou para o uso preferencial do meio Amies para o transporte de suabes nasais. O segundo experimento indicou que o ágar MacConkey e o G20G modificado mostraram resultados similares e foram superiores ao meio de ceftiofur. No experimento final, a técnica de reação em cadeia pela polimerase apresentou uma capacidade de detecção da B. bronchiseptica superior a do cultivo.

Three comparative assays were performed seeking to improve the sensitivity of the diagnosis of Bordetella bronchiseptica infection analyzing swine nasal swabs. An initial assay compared the recovery of B. bronchiseptica from swabs simultaneously inoculated with B. bronchiseptica and some interfering bacteria, immersed into three transport formulations (Amies with charcoal, trypticase soy broth and phosphate buffer according to Soerensen supplemented with 5% of bovine fetal serum) and submitted to different temperatures (10ºC and 27ºC) and periods of incubation (24, 72 and 120 hours). A subsequent assay compared three selective media (MacConkey agar, modified selective medium G20G and a ceftiofur medium) for their recovery capabilities from clinical specimens. One last assay compared the polymerase chain reaction to the three selective media. In the first assay, the recovery of B. bronchiseptica from transport systems was better at 27ºC and the three formulations had good performances at this temperature, but the collection of qualitative and quantitative analysis indicated the advantage of Amies medium for nasal swabs transportation. The second assay indicated that MacConkey agar and modified G20G had similar results and were superior to the ceftiofur medium. In the final assay, polymerase chain reaction presented superior capability of B. bronchiseptica detection to culture procedures.

We report a 21 years old woman that was admitted in the emergency room due to a extensive chest trauma caused by a gunshot. The patient was subjected to an emergency thoracotomy. A right lung lower and middle lobectomy, partial hepatectomy, diaphragmatic suture and mastectomy were performed. A primary chest closure was done, using a Marlex prosthesis covered by a cutaneous flap. During the postoperative period, the patient had a surgical wound infection, that was treated conservatively and was discharged 42 days after surgery

Es nuestro objetivo comunicar un caso clínico del Hospital Clínico Herminda Martin, en el cual se realizó una reparación torácica con material protésico en un tiempo como consecuencia de un traumatismo penetrante torácico por arma de fuego con pérdida de pared, con buenos resultados
