ABSTRACT The aim of this study was to characterise the lignocellulosic fractions of pruning waste from tree species showing potential for furniture production in Acaraú, Ceará. The experiment was conducted in a completely randomised, split-plot design, with the plots corresponding to two water regimes (irrigated for one year and irrigated for three years) and the sub-plots to eight tree species, with three replications. The chemical attributes were determined after seven years. Except for the insoluble lignin content, there was a significant effect from the different species in terms of the chemical properties of the wood. The results for the lignocellulosic fractions show the possibility of developing technological ways of adding value to these waste products, especially considering their lignin content. The different species also showed potential for use in producing cellulose pulp, with the exception of Handroanthus impetiginosus due to its high ash content. The results of this study offer new perspectives for future research into the use of forestry waste. Introdução: As doenças cardiovasculares (DCV) continuam a ser a principal causa de mortalidade em todo o mundo. A adesão ao tratamento medicamentoso é uma questão importante no manejo das DCV crônicas, influenciando diretamente os resultados e os custos com saúde. Objetivos: Esta revisão sistemática, apoiada pela Sociedade Brasileira de Cardiologia, avalia o impacto da baixa adesão aos medicamentos cardiovasculares em desfechos clínicos críticos, como morte e eventos cardiovasculares. Métodos: Foi realizada uma busca abrangente em quatro bases de dados, incluindo Medline, Embase, Lilacs e Cochrane Library. A revisão incluiu revisões sistemáticas com meta-análises que relataram estimativas de risco para a adesão aos medicamentos cardiovasculares. Foram selecionadas quatro revisões sistemáticas, cada uma incorporando estudos observacionais. Resultados: Os principais achados indicam que um aumento na adesão aos medicamentos reduz significativamente o risco de eventos cardiovasculares, acidente vascular cerebral (AVC) e morte por todas as causas. Especificamente, uma melhoria de 20% na adesão a medicamentos antihipertensivos, hipolipemiantes e outros medicamentos cardiovasculares correlacionou-se com reduções nos eventos cardiovasculares de 7%, 10% e 9%, respectivamente; AVC de 17%, 13% e 18%; e morte de 12%, 9% e 10%. A certeza das evidências foi moderada, sugerindo que esses efeitos provavelmente estão presentes. Esses achados enfatizam a importância de melhorar a adesão ao tratamento medicamentoso para aprimorar os resultados clínicos no manejo das DCV. Conclusões: As evidências demonstraram reduções em desfechos duros tanto na prevenção primária quanto secundária através do controle de condições como hipertensão e concentrações elevadas de colesterol LDL, bem como os benefícios da terapia antiplaquetária em doenças ateroscleróticas. No entanto, são necessários estudos adicionais para elucidar melhor a relação entre a adesão aos medicamentos cardiovasculares e a melhoria dos desfechos clínicos críticos.

Background: Cardiovascular diseases (CVD) remain the leading cause of mortality worldwide. Medication adherence is an important issue in managing chronic CVD, directly influencing outcomes and healthcare costs. Objectives: This systematic review, supported by the Brazilian Society of Cardiology, evaluates the impact of poor adherence to cardiovascular medications on critical clinical outcomes such as death and cardiovascular events. Methods: A comprehensive search was conducted across four databases, including Medline, Embase, Lilacs, and the Cochrane Library. The review included systematic reviews with meta-analyses that reported risk estimates for adherence to cardiovascular medications. Four systematic reviews, each incorporating observational studies, were selected. Results: The principal findings indicate that an increase in adherence to medications significantly reduces the risk of cardiovascular events, stroke, and all-cause death. Specifically, a 20% improvement in adherence to antihypertensive, lipid-lowering, and other cardiovascular medications correlated with reductions in cardiovascular events by 7%, 10%, and 9%, respectively; stroke by 17%, 13%, and 18%; and death by 12%, 9%, and 10%. The certainty of the evidence was moderate, suggesting that these effects are likely present. These findings emphasize the importance of enhancing medication adherence to improve clinical outcomes in CVD management. Conclusions: Evidence has demonstrated reductions in hard endpoints in both primary and secondary prevention through the control of conditions such as hypertension and elevated LDL cholesterol concentrations, as well as the benefits of antiplatelet therapy in atherosclerotic disease. However, additional studies are needed to better elucidate the relationship between adherence to cardiovascular medications and the improvement of critical clinical outcomes.

Resumo O estudo analisa o acesso ao cuidado em saúde de mulheres encarceradas no estado do Ceará, rastreando transtornos mentais comuns. Estudo analítico, transversal, de natureza quantitativa, desenvolvido na única penitenciária feminina do Ceará. Participaram 90 detentas, todas com alguma das seguintes questões de saúde: gestantes, puérperas, portadoras de hipertensão arterial sistêmica, diabetes mellitus, tuberculose, sífilis, hepatite B ou HIV/Aids. Os dados foram coletados por entrevista estruturada. Evidenciou-se acesso limitado à atenção à saúde das detentas, violando direitos básicos sob tutela do Estado. Foram constatadas limitações de exames de rastreamento de doenças nas presidiárias na ocasião de seu acesso ao cárcere, principalmente as que não se encontravam grávidas no momento do encarceramento. Houve diferenças entre as distintas condições de saúde analisadas, com prioridade da atenção às gestantes e puérperas. Entre as detentas, 68,24% apresentavam risco de transtornos mentais comuns (SRQ > 7). Houve correlação positiva entre idade e saúde mental (p = 0,0002). Embora exista legislação pertinente de garantia de acesso à saúde no cárcere, o sistema prisional está despreparado para atender às necessidades de portadoras de comorbidades, gestantes e puérperas.

Resumo A estomatite química é um tipo de lesão traumática causada pelo contato de substâncias químicas e medicamentos com os tecidos bucais, resultando em danos clinicamente significativos. Nesse contexto, é relatado o caso de um paciente do sexo masculino com 14 anos, portador de doença renal crónica, em terapia renal substitutiva (diálise peritoneal) e uso de carbonato de cálcio via cápsulas orais para tratamento. Devido a complicações da diálise peritoneal, o paciente apresentou um quadro de peritonite bacteriana, necessitando de internamento em unidade de terapia intensiva e alteração do modo de terapia dialítica para hemodiálise. Durante o período de internamento, foi observado o desenvolvimento de lesões orais significativas devido à administração inadequada de carbonato de cálcio, interferindo na evolução do estado geral de saúde do paciente e exigindo a assistência de uma equipa odontológica para diagnóstico e tratamento. O paciente foi diagnosticado com estomatite química. Durante o exame clínico foi possível observar a presença de lesões ulceradas e hemorrágicas localizadas no lábio, mucosas jugais, palato duro e palato mole. A fotobiomodulação a laser foi utilizada como tratamento associada ao corticosteróide. Cada região recebeu dez pontos de 1 J, de forma que foram fornecidos 60 J de energia por dia/sessão. No total, foram fornecidos 480 J de energia com um tempo total de irradiação de 4800 s. O paciente relatou melhora considerável dos sintomas álgicos após a primeira sessão de fotobiomodulação a laser. Abstract Chemical stomatitis is a type of traumatic injury caused by contact of chemical substances and drugs with oral tissues, resulting in clinically significant damage. This case reports a 14-year-old male patient with chronic kidney disease on renal replacement therapy (peritoneal dialysis) and using calcium carbonate via oral capsules for treatment. Due to peritoneal dialysis complications, the patient developed bacterial peritonitis, requiring intensive care unit admission and alteration of his dialysis therapy mode to hemodialysis. During the hospitalization period, significant oral lesions developed due to inadequate calcium carbonate administration, interfering with the evolution of the patient’s general health and requiring the assistance of a dental team for diagnosis and treatment. The patient was diagnosed with chemical stomatitis. During the clinical examination, ulcerated, bleeding lesions were observed on the lip, jugal mucous membranes, hard palate, and soft palate. Laser photobiomodulation was used as a treatment associated with a topical corticosteroid. Each region received ten 1 J/point, and 60 J of energy were delivered per day/session. In total, 480 J of energy were delivered with a total irradiation time of 4800 s. The patient reported considerable improvement in pain symptoms after the first laser-photobiomodulation session. After 8 sessions, the patient had improved, and laser photobiomodulation proved effective for treating oral lesions.

Resumo Fundamento: As diretrizes da Sociedade Europeia de Cardiologia recomendam um nível de colesterol LDL (LDL-C) < 55 mg/dL para pacientes com doença cardiovascular estabelecida. Embora a fórmula de Friedewald ainda seja amplamente utilizada para estimar o LDL-C, a fórmula mais recente de Martin-Hopkins mostrou maior precisão. Objetivos: Nosso objetivo foi avaliar: A) a proporção de pacientes que atingiram a meta de LDL-C e as terapias utilizadas em um centro terciário; B) o impacto da utilização do método de Martin-Hopkins em vez do método de Friedewald na proporção de pacientes controlados. Métodos: Estudo transversal monocêntrico, incluindo pacientes consecutivos pós-infarto do miocárdio, acompanhados por 20 cardiologistas, em um hospital terciário. Os dados foram coletados retrospectivamente de consultas clínicas realizadas após abril de 2022. Para cada paciente, os níveis de LDL-C e o atingimento das metas foram estimados a partir de um perfil lipídico ambulatorial, utilizando as fórmulas de Friedewald e Martin-Hopkins. Um valor-p bicaudal < 0,05 foi considerado estatisticamente significativo para todos os testes. Resultados: Foram incluídos 400 pacientes (com 67 ± 13 anos, 77% do sexo masculino). Utilizando a fórmula de Friedewald, a mediana de LDL-C sob terapia foi de 64 (50-81) mg/dL, e 31% tinham LDL-C dentro da meta. Estatinas de alta intensidade foram usadas em 64% dos pacientes, 37% estavam em uso de ezetimiba e 0,5% estavam em uso de inibidores de PCSK9. A terapia combinada de estatina de alta intensidade + ezetimiba foi utilizada em 102 pacientes (26%). A aplicação do método de Martin-Hopkins reclassificaria um total de 31 pacientes (7,8%). Entre aqueles considerados controlados pela fórmula de Friedewald, 27 (21,6%) teriam LDL-C estimado por Martin-Hopkins acima da meta. Conclusões: Menos de um terço dos pacientes pós-infarto do miocárdio apresentaram LDL-C dentro da meta. Abstract Background: The European Society of Cardiology guidelines recommend an LDL-cholesterol (LDL-C) < 55 mg/dL for patients with established cardiovascular disease. While the Friedewald equation to estimate LDL-C is still widely used, the newer Martin-Hopkins equation has shown greater accuracy. Objectives: We aimed to assess: A) the proportion of patients reaching LDL-C goal and the therapies used in a tertiary center; B) the impact of using the Martin-Hopkins method instead of Friedewald’s on the proportion of controlled patients. Methods: A single-center cross-sectional study including consecutive post-myocardial infarction patients followed by 20 cardiologists in a tertiary hospital. Data was collected retrospectively from clinical appointments that took place after April 2022. For each patient, the LDL-C levels and attainment of goals were estimated from an ambulatory lipid profile using both Friedewald and Martin-Hopkins equations. A two-tailed p-value of < 0.05 was considered statistically significant for all tests. Results: Overall, 400 patients were included (aged 67 ± 13 years, 77% male). Using Friedewald’s equation, the median LDL-C under therapy was 64 (50-81) mg/dL, and 31% had LDL-C within goals. High-intensity statins were used in 64% of patients, 37% were on ezetimibe, and 0.5% were under PCSK9 inhibitors. Combination therapy of high-intensity statin + ezetimibe was used in 102 patients (26%). Applying the Martin-Hopkins method would reclassify a total of 31 patients (7.8%). Among those deemed controlled by Friedewald’s equation, 27 (21.6%) would have a Martin-Hopkins’ LDL-C above goals. Conclusions: Less than one-third of post-myocardial infarction patients had LDL-C within the goal. Applying the Martin-Hopkins equation would reclassify one-fifth of presumably controlled patients into the non-controlled group. RESUMO: A enxertia cutânea é uma técnica cirúrgica simples e bastante útil para o reparo de feridas, principalmente, quando há dificuldade em realizar a síntese direta ou outras técnicas reconstrutivas. O laser de baixa intensidade (LBI) já foi utilizado com sucesso em casos em que o enxerto foi aplicado em feridas logo após a sua criação. No entanto, a LBI ainda carece de padronizações, mediante estudos conflitantes sobre seus resultados. Desta forma, este trabalho objetivou comparar a eficácia da LBI de onda contínua com onda pulsada na estimulação da cicatrização de enxertos cutâneos, aplicados em feridas recém-criadas em coelhos. Para tanto, foram utilizados 31 coelhos distribuídos em: grupo controle (GC, n = 10), grupo laser onda contínua (GLC; n = 10) e grupo laser onda pulsada (GLP; n = 11). O manejo pós operatório foi semelhante entre todos os grupos, com exceção da aplicação do laser, conforme o grupo indicado. Os grupos GLC e GLP apresentaram evolução semelhante, com processo cicatricial satisfatório do enxerto, ao contrário do grupo GC. Tais resultados se mantiveram na avaliação microscópica, pois os grupos tratados com laser apresentaram menor inflamação, melhor integração à área receptora, maior reepitelização e colagenização. Não houve diferença na densidade vascular entre os grupos de tratamento na análise imuno-histoquímica. A ferida recém-criada não é capaz de fornecer nutrição para o enxerto cutâneo nos coelhos, mas a LBI é uma terapia adjuvante eficiente em estimular a cicatrização entre a área receptora e o enxerto, promovendo integração total. Entretanto, embora não houve diferença estatística na forma de emissão de luz, pulsada ou contínua, os pacientes que receberam LBI em onda contínua apresentam evolução microscópica superior. ABSTRACT: Skin grafting is a simple and very useful surgical technique for wound repair, especially where there is difficulty in performing direct synthesis or other reconstructive techniques. low-intensity laser (LIL) has already been used successfully in cases where the graft was applied to wounds shortly after its creation. However, LIL still lacks standardization due to conflicting studies on its results. Thus, this study compared the effectiveness of continuous wave LIL with pulsed wave LIL in stimulating the healing of skin grafts, applied to newly created wounds in rabbits. For this purpose, 31 rabbits were distributed into: control group (CG, n = 10), continuous wave laser group (GLC; n = 10) and pulsed wave laser group (GLP; n = 11). Postoperative management was similar between all groups, with the exception of laser application, as indicated by the group. The GLC and GLP groups showed similar evolution, with a satisfactory healing process of the graft, unlike the GC group. These results were maintained in the microscopic evaluation, as the groups treated with laser showed less inflammation, better integration with the receptor area, greater re-epithelialization and collagenization. There was no difference in vascular density between the treatment groups on immunohistochemical analysis. The newly created wound was not able to provide nutrition for the skin graft in rabbits, but LIL is an efficient adjuvant therapy in stimulating healing between the recipient area and the graft, promoting full integration. Abstract The incidence of premature birth has increased worldwide, unequally distributed by race/ethnicity. Racism generates economic inequalities, educational disparities, and differential access to health care, which increases the risk of preterm birth. Thus, this study aimed to evaluate the factors associated with preterm birth and racial and ethnic disparities in premature birth among pregnant women attending prenatal care at the Brazilian Unified Health System health units in the urban area of Santo Antônio de Jesus, Bahia, Brazil. This study used data from 938 pregnant women aged between 18 to 45 years within the NISAMI prospective cohort. Premature birth prevalence was 11.8%, with a higher prevalence among black than non-black women (12.9% versus 6.0%, respectively). Maternal age between 18 and 24 years was the only factor associated with premature birth. A higher risk of premature birth was found among black women than non-black women (RR 3.22; 95%CI 1.42-7.32). These results reveal the existence of racial and social inequalities in the occurrence of premature birth.

O racismo gera desigualdades econômicas, disparidades educacionais e acesso diferenciado à saúde, o que aumenta o risco de parto prematuro. Assim, este estudo teve como objetivo avaliar os fatores associados à prematuridade e disparidades raciais e étnicas no parto prematuro entre gestantes atendidas durante o pré-natal em unidades de saúde do Sistema Único de Saúde na zona urbana de Santo Antônio de Jesus, Bahia, Brasil. Este estudo utilizou dados de 938 mulheres grávidas com idade entre 18 e 45 anos dentro da coorte prospectiva do NISAMI. A prevalência de prematuridade foi de 11,8%, sendo maior entre as negras do que entre as não negras (12,9% versus 6,0%, respectivamente). A idade materna entre 18 e 24 anos foi o único fator associado ao parto prematuro. Foi encontrado maior risco de prematuridade entre as mulheres negras do que entre as não negras (RR 3,22; IC95% 1,42-7,32). Esses resultados revelam a existência de desigualdades raciais e sociais na ocorrência do parto prematuro.

ABSTRACT Soil pH and base saturation are important parameters for evaluating the effects of acidity on plant growth. However, the relationship between soil pH and base saturation (V%) has been poorly studied in Brazil. The aim of this study was to evaluate the influence on the cation exchange capacity (CEC) on this relationship for soils of Mato Grosso do Sul, Brazil, using a database consisting of 30,244 analyses of soils collected from 0–0.2- and 0.2–0.4-m depths. The CEC at pH 7 of analyzed soils varied from 3.4 to 40.5 cmolc·dm-3 and were grouped into four classes (< 6, 6–10, 10–20, and > 20 cmolc·dm-3) for each soil layer. The V% versus pH relationship was CEC dependent for both soil layers. For 0–0.2 m, V% was directly related to CEC (< 6, 6–10, 10–20, and > 20 cmolc·dm-3) and reached 50, 55, 60, and 74% at pH 5 and 66, 73, 81, and 88% at pH 6, respectively. Predicting V% in relation to pH was also affected by CEC, with a decrease in the correlation of p = 0.82 for CEC < 6 cmolc·dm-3 to p = 0.64 for CEC > 20 cmolc·dm-3. The decrease in this correlation was related to low clay content (17%) of soils with CEC < 6 cmolc·dm-3, which provided low buffering capacity. For soils with CEC > 20 cmolc·dm-3 and higher clay content (45%), greater buffering capacity was observed associated with kaolinite. RESUMO JUSTIFICATIVA E OBJETIVOS: A disfunção temporomandibular (DTM) possui sintomas complexos que envolvem a região orofacial, como a otalgia. Considerando os difíceis diagnósticos diferenciais para associar sintomas otológicos com DTM. O objetivo deste estudo foi verificar o diagnóstico de DTM em pacientes com otalgia. MÉTODOS: Este é um estudo transversal e descritivo, com uma avaliação de 75 pacientes diagnosticados com otalgia. O questionário de triagem da Academia Europeia de Disfunções Craniomandibulares (EACD) foi aplicado inicialmente, e aqueles que responderam afirmativamente a pelo menos uma questão foram avaliados pelo Critério Diagnóstico de Pesquisa para Disfunções Temporomandibulares (DC/TMD), com amostra final de 50 pacientes. Os dados foram tabulados e por meio da ANOVA foi verificado se havia diferença estatística entre os subtipos de DTM classificados pelo DC/TMD, considerando intervalos de confiança com 95% de significância. RESULTADOS: A idade média dos participantes foi de 39,9±14,1 anos. Além disso, predominou-se o sexo feminino (76%) (p<0,0001), com maior percentual em todos os subtipos avaliados, destacando-se a artralgia (82%) e a dor miofascial com limitação de abertura (81,8%), seguida da dor miofascial (74%) e deslocamento de disco com redução (72,7%). Quando observada a distribuição dos subtipos de DTM entre os sexos, notou-se predominância de dor miofascial no sexo masculino (75%) e feminino (68%), mas não foi observada significância estatística nessa variável e nas demais estudadas. CONCLUSÃO: Os pacientes com otalgia manifestaram mais de um dos subtipos de DTM, sendo o subtipo de dor miofascial o mais prevalente entre os participantes do estudo. ABSTRACT BACKGROUND AND OBJECTIVES: Temporomandibular disorder (TMD) has complex symptoms that involve the orofacial region, such as otalgia. Considering the difficult differential diagnoses for associating otological symptoms with TMD. The aim of this study was to verify the diagnosis of TMD in patients with otalgia. METHODS: This is a cross-sectional and descriptive study, where 75 patients diagnosed with otalgia were evaluated. The European Academy of Craniomandibular Disorders’s (EACD) screening questionnaire was initially applied, and those who answered affirmatively to at least one question were evaluated by the Research Diagnostic Criteria for Temporomandibular Disorders (DC/TMD), with a final sample of 50 patients. Data were tabulated and ANOVA verified whether there was a statistical difference between TMD subtypes classified by DC/TMD, considering confidence intervals with 95% significance. RESULTS: The mean age of the participants was 39.9±14.1 years, with a predominance of females (76%) (p<0.0001). Among those diagnosed with TMD, females also predominated, with a higher percentage in all evaluated subtypes, with emphasis on arthralgia (82%) and myofascial pain with limited opening (81.8%), followed by myofascial pain (74%) and disc displacement with reduction (72.7%). When observing the distribution of TMD subtypes between genders, there was a predominance of myofascial pain in males (75%) and females (68%), but no statistical significance was observed in this variable and in the others studied. CONCLUSION: Patients with otalgia had one or more TMD subtypes, and the myofascial subtype TMD was the most prevalent among study participants. Resumo Este artigo visa a compreender a história da escolarização no Brasil, na segunda metade do século XIX, especificamente nos aspectos do ensino da leitura e da escrita para crianças, a partir da análise de um impresso escolar escrito por João Vicente Martins, em 1854. Na Cartilha de leitura repentina, ou plágio do Método Castilho, Martins lançou-se no mercado editorial na tentativa de aproximar os ideários de dois precursores do debate acerca dos métodos para o ensino da língua materna: o francês Joseph Jacotot e o português António Feliciano Castilho. Tomando como fonte, sobretudo a imprensa periódica e as produções de Castilho e Martins, a análise empreendida se organizou a partir de dois principais aspectos: a biografia desse último autor e a materialidade de sua cartilha. Abstract Based on an analysis of a school textbook produced by Joo Vicente Martins in 1854, this article aims to understand the history of schooling in Brazil in the second half of the nineteenth century, notably in the elements of teaching reading and writing to children. Martins launched himself into the publishing market with Cartilha de Leitura repentina, ou plágio do Método Castilho, in an attempt to bring together the ideals of two important forerunners of the debate about methods for teaching the mother tongue engaged with the universalization of elementary education: the Frenchman Joseph Jacotot and the Portuguese António Feliciano Castilho.
