ABSTRACT The various early life factors have been associated with appetite behaviors in children. Furthermore, a relationship between these behaviors and childhood body weight development has been observed. Therefore, it is of utmost relevance to investigate how early life factors may be related to appetite behaviors at a young age. In order to achieve this objective, research was conducted using the Medline (Pubmed) and ScienceDirect databases between the months of January and June 2022. According to the literature analysis conducted, it was understood that maternal characteristics during pregnancy, child's characteristics at birth, and dietary habits during the first year of life have been linked to various appetite behaviors. There is a greater tendency for "food approach" behaviors to be associated with a higher risk of childhood overweight, which may extend into adulthood. Evaluating appetite behaviors in childhood and their potential association with early life factors is of great importance in understanding the best strategies for planning or modifying future interventions effectively and purposefully, to promote healthier eating behaviors and, consequently, the health and well-being of individuals.

RESUMO Os vários fatores de início de vida têm vindo a ser associados aos comportamentos relacionados com o apetite em crianças. Para além disso, tem-se verificado uma relação entre estes comportamentos e o desenvolvimento do peso corporal na infância. Como tal, torna-se de extrema relevância estudar como os fatores de início de vida se podem relacionar com os comportamentos relacionados com o apetite em idades precoces. De forma a alcançar este objetivo, desenvolveu-se uma pesquisa nas bases de dados Medline (Pubmed) e Science Direct de janeiro a junho de 2022. De acordo com a análise da literatura efetuada compreendeu-se que as caraterísticas da mulher durante a gravidez, as caraterísticas da criança ao nascimento e os hábitos alimentares durante o primeiro ano de vida têm sido associados aos variados comportamentos relacionados com o apetite. Verifica-se uma maior tendência para os comportamentos de “aproximação à comida” se associarem com um maior risco de excesso de peso na infância, podendo perlongar-se para a vida adulta. A avaliação dos comportamentos relacionados com o apetite na infância e a sua possível associação com os fatores de início de vida são de extrema importância para compreender quais as melhores estratégias para planear ou modificar intervenções futuras de forma eficaz e orientada, para uma melhor promoção de comportamentos alimentares saudáveis e consequentemente da saúde e bem-estar dos indivíduos.

ABSTRACT Challenges remain to improve the nutritional status of populations and ensure sustainable production and consumption patterns. The Sustainable and Healthy Eating Behaviors scale assesses healthy and sustainable eating behaviors in Polish young adults. To this day, there is no tool assessing these behaviors, having a more holistic approach, in young adult university students. Therefore, this study aimed to translate and cross-culturally adapt the Sustainable and Healthy Eating Behaviors scale in this population. After obtaining the portuguese version of the scale through translation and back translation, 30 young adults participated in a semistructured interview with the direct application of a preliminary version of the scale for cross-cultural adaptation. Then, the scale's statements were adjusted, considering problems of understanding by university students and suggestions made by a panel of experts. By obtaining the final version of the scale in the Portuguese, it will be possible to characterize the healthy and sustainable eating behaviors in Portuguese university students and to assess the impact of food education projects and food-relation policies in this population.

RESUMO Permanecem desafios para melhorar o estado nutricional das populações e garantir padrões de produção e consumo sustentáveis. A escala Sustainable and Healthy Eating Behaviors avalia comportamentos alimentares saudáveis e sustentáveis em jovens adultos polacos. No nosso melhor conhecimento, em Portugal, desconhece-se a existência de um instrumento que avalie estes comportamentos, de forma holística, em jovens adultos universitários. Como tal, o objetivo do estudo consistiu em traduzir e adaptar transculturalmente esta escala para esta população. Após obtenção da versão portuguesa da escala, através da tradução e retrotradução, procedeu-se à sua adaptação transcultural, com a participação de 30 jovens adultos, numa entrevista semiestruturada, com a aplicação direta da versão preliminar da escala. Depois, foram realizadas adaptações às afirmações da escala, que tinham suscitado problemas de compreensão por parte dos universitários e como resultado das sugestões feitas pelo painel de especialistas. Obteve-se a versão final da escala em língua portuguesa, pelo que será possível caracterizar os comportamentos alimentares saudáveis e sustentáveis em universitários portugueses e avaliar o impacto de projetos de educação alimentar e de políticas alimentares nesta população.

RESUMO: O uso da resistência de plantas atua intervindo na relação herbívoro-hospedeiro, através de fatores morfológicos, físicos ou químicos da planta. O objetivo deste trabalho foi avaliar a oviposição e o consumo foliar de Chrysodeixis includens (Walker [1858]) em genótipos de soja, através de testes com e sem escolha, correlacionando-os com os fatores, densidade e tamanho de tricomas. Os experimentos foram realizados em laboratório (25 ± 2 °C; UR= 70 ± 10%; fotoperíodo 14h) utilizando cinco cultivares (BRS 391, BRS 6203 RR, BMX Valente RR, Tec Irga 6070 RR, BMX Icone Ipro) e duas linhagens (PELBR 10-6000 e PELBR 10-6049). Os tricomas encontrados foram tectores filiformes e glandulares multicelulares claviformes. A densidade de tricomas glandulares nos estágios V2 e V5, foi maior em BRS 6203 RR e BRS 391, respectivamente. A maior densidade de tricomas glandulares foi observada em V5 e a maior densidade de tricomas tectores em V2. As menores densidades e os menores tamanhos de tricomas nos estágios V2 e V5 foram observados em PELBR 10-6049. O tamanho de tricomas tectores e o número de ovos não diferiram entre as cultivares. O consumo foliar foi menor para BMX Icone Ipro e Tec Irga 6070 RR. A densidade de tricomas influência o comportamento de consumo e oviposição de C. includens. ABSTRACT: The use of plant resistance acts by intervening in the herbivore-host relationship, through morphological, physical or chemical factors of the plant. This study evaluated the oviposition and foliar consumption of Chrysodeixis includens (Walker [1858]) in soybean genotypes, in free- and no-choice tests, correlating them with the factors, density and size of trichomes. The experiments were carried out in laboratory (25 ± 2 °C; RH= 70 ± 10%; photoperiod 14h) using five cultivars (BRS 391, BRS 6203 RR, BMX Valente RR, Tec Irga 6070 RR, BMX Icone Ipro) and two isolines (PELBR 10-6000 and PELBR 10-6049). The trichomes reported were filiform tectors and claviform multicellular glandular. The density of glandular trichomes, in stages V2 and V5, was higher on BRS 6203 RR and BRS 391, respectively. The higher density of glandular trichomes was observed in V5 and a higher density of tector trichomes in V2. The lowest densities and the smallest sizes of trichomes in V2 and V5 stages was observed on PELBR 10-6049. The size of tector trichomes and the number of eggs did not differ among the cultivars. Abstract Open access (OA) publishing provides free online access to research articles without subscription fees. In Brazil, absence of financial support from academic institutions and limited government policies pose challenges to OA publication. Here, we used data from the Web of Science and Scopus to compare with global trends in journal accessibility and scientific quality metrics. Brazilian authors publish more OA articles, particularly in Global South journals. While OA correlates with quality for global authors, it had no impact on Brazilian science. To maximize impact, Brazilian authors should prioritize Q1 journals regardless of OA status. High-impact or Global North journal publication seems more relevant for Brazilian science than OA. Our findings indicate that the present open access policy has been ineffective to improve the impact of Brazilian science, providing insights to guide the formulation of scientific public policies. ABSTRACT Purpose: To evaluate the profile of graduates of the Postgraduate Program (PGP) in Cardiology of a public federal university, according to sociodemographic factors and professional trajectory. Methods: The variables were collected from databases from the observed institution and digital platforms. The analysis of differences between the various levels of degrees was carried out in three cohorts: the entire historical series (graduates from 1978–2021), the first 20 years (1978–1997) and the second 20 years (1998–2018). Results: The results demonstrated that most students from the PGP completed a PhD and are men over 30 years old, they came from public universities and the Southeast region. In the first 20 years, significant differences were observed in the distribution of masters and doctors working professionally at the institution analyzed, as well as in the age of the students. In the 20 years of the second half, there were differences between masters and PhD working professionally in the institution itself, as they came from private universities, they are women and PhD. Conclusions: The changes in the profile of masters and PhD that graduated from this PGP in cardiology reflect transformations that occurred in the job market and academy over the decades. Resumo Introdução: Acredita-se que a obesidade desempenhe um papel na desregulação da ritmicidade cardíaca em crianças obesas, mas esse é um campo de investigação ainda pouco explorado. O objetivo deste trabalho foi avaliar o impacto do sobrepeso e da obesidade na ritmicidade cardiovascular circadiana e ultradiana de crianças pré-púberes, em comparação com crianças com peso normal. Métodos: Realizamos um estudo transversal com 316 crianças, acompanhadas na coorte de nascimentos Geração XXI (Portugal). Foram medidos dados antropométricos e a pressão arterial ambulatorial de 24 horas, e os perfis foram examinados com uma análise de Fourier para ritmos circadianos e ultradianos de pressão arterial (PA) e frequência cardíaca (FC). Resultados: Crianças com sobrepeso/obesidade apresentaram mais frequentemente um padrão de PA não-dipper em comparação com crianças com peso normal (31,5% vs. 21,6%; p = 0,047). A prevalência da pressão arterial média (PAM) de 24 horas e da ritmicidade da FC de 8 horas foi significativamente menor em crianças obesas (79,3% vs. 88,0%; p = 0,038 e 33,3% vs. 45,2%; p = 0,031, respectivamente). A prevalência das restantes ritmicidades da PAM e da FC foi semelhante em ambos os grupos. Não foram encontradas diferenças nos valores medianos das amplitudes e acrofases dos ritmos de PAM e FC. Abstract Introduction: Obesity is thought to play a role in the disruption of cardiac rhythmicity in obese children, but this is mostly an unexplored field of investigation. We aimed to evaluate the impact of overweight and obesity on circadian and ultradian cardiovascular rhythmicity of prepubertal children, in comparison with normal weight counterparts. Methods: We performed a cross sectional study of 316 children, followed in the birth cohort Generation XXI (Portugal). Anthropometrics and 24-hour ambulatory blood pressure were measured and profiles were examined with Fourier analysis for circadian and ultradian blood pressure (BP) and heart rate (HR) rhythms. Results: Overweight/obese children presented more frequently a non-dipping BP pattern than normal weight counterparts (31.5% vs. 21.6%, p = 0.047). The prevalence of 24-hour mean arterial pressure (MAP) and 8-hour HR rhythmicity was significantly lower in obese children (79.3% vs. 88.0%, p = 0.038 and 33.3% vs. 45.2%, p = 0.031, respectively). The prevalence of the remaining MAP and HR rhythmicity was similar in both groups. No differences were found in the median values of amplitudes and acrophases of MAP and HR rhythms. Discussion: The alterations found in rhythmicity suggest that circadian and ultradian rhythmicity analysis might be sensitive in detecting early cardiovascular dysregulations, but future studies are needed to reinforce our findings and to better understand their long-term implications. Abstract By focusing the discussion on human dignity and built spaces, this article is divided into two parts. In the first one, we present a philosophical analysis of the bases of the method and of digital tools, seeking to show how René Descartes’ metaphysical premises have been transformed into tools that submerge individualities and homogenize urban aesthetics. In the second part, we analyze neuroscientific research related to the human capacity to decide. We conclude that the built environment is an active element in the formation of such capacity. ABSTRACT INTRODUCTION: Food insecurity is characterized by any lack of guaranteed regular access to safe and adequate nutritional food, which may lead to inadequate eating habits and higher risk of obesity. OBJECTIVES: To characterize and study the associations between eating habits, anthropometric status and Food insecurity in a population group with social support for food purchases. METHODOLOGY: Cross-sectional study, which applied a survey including sociodemographic characteristics, anthropometric status, eating habits, Food insecurity and food products acquired through social support to the representatives of each household benefited by the “+ Alimentação” social support axis, part of the “Social +” program of the Municipality of Gondomar. RESULTS: From a total of 58 participants 70.7% (n = 41) were females and 58.6% (n = 34) lived alone. From the household representatives, 63.7% (n = 35) had overweight or obesity and 77.6% (n = 45) had moderate adherence to healthy eating habits, with 35.8% (n = 19) in a state of severe Food insecurity. Higher Body Mass Index was associated with higher Food insecurity (r = 0.380, p = 0.006) and older age was associated with healthier eating habits (r = 0.348, p = 0.007). CONCLUSIONS: More than half of the sample presented a moderate adherence to healthy eating habits. Higher Body Mass Index was associated with higher level of Food insecurity among individuals with social support for food purchases.

RESUMO INTRODUÇÃO: A insegurança alimentar caracteriza-se por qualquer falta de garantia do acesso regular a alimentos seguros e de valor nutricional adequado, podendo levar a hábitos alimentares inadequados e a um maior risco de obesidade. OBJETIVOS: Caracterizar e estudar as associações entre hábitos alimentares, estado estaturo-ponderal e insegurança alimentar num grupo populacional com auxílio económico para a compra de alimentos. METODOLOGIA: Estudo observacional de desenho transversal com aplicação de um questionário incluindo características sociodemográficas, estado estaturo-ponderal, hábitos alimentares, insegurança alimentar e géneros alimentícios adquiridos através do apoio social a cada representante dos agregados familiares beneficiários do apoio “+ Alimentação”, inserido no programa “Social +” do Município de Gondomar. RESULTADOS: De um total de 58 participantes, 70,7% (n = 41) era do sexo feminino e 58,6% (n = 34) vivia sozinho. Dos representantes dos agregados, 63,7% (n = 35) apresentavam excesso de peso e 77,6% (n = 45) uma adesão moderada a hábitos alimentares saudáveis, com 35,8% (n = 19) em contexto de insegurança alimentar grave. Um Índice de Massa Corporal mais elevado associou-se a maior insegurança alimentar (r = 0,380; p = 0,006) e participantes mais velhos apresentavam hábitos alimentares mais saudáveis (r = 0,348; p = 0,007). CONCLUSÕES: Mais de metade da amostra apresentou uma adesão moderada a hábitos alimentares saudáveis. Um Índice de Massa Corporal mais elevado associou-se a maior nível de insegurança alimentar nos beneficiários de apoio social para a compra de alimentos.

ABSTRACT INTRODUCTION: To reduce the incidence and prevalence of diseases related to eating habits, the School Diet Optimization Project was implemented, which aims to contribute to a school food supply in line with the premises of healthy eating. OBJECTIVES: To compare qualitatively and quantitatively the school food supply related to the lunch meal considering direct and concession management models. METHODOLOGY: Observational cross-sectional study. The School Diet Optimization Project, implemented in 34 public schools in the district of Viana do Castelo, aims to guarantee the offer of varied, normoenergetic, and nutritionally balanced meals that guarantee hygiene and food safety criteria. Data corresponding to one month of each school, between 2014 and 2019, was analyzed. RESULTS: School canteens under direct management fulfilled a higher percentage (82%), on a median, of qualitative criteria of the School Diet Optimization Project, compared to those under concession management (p = 0,022). All canteens with direct management complied with the required frequency of offering fish rich in omega 3 (ω3). These canteens used a greater amount of fat and saturated fat compared to those under concession management (p = 0,030 and 0,040, respectively). The opposite occurred in fiber supply (p = 0,016). CONCLUSIONS: Considering the qualitative assessment, the school canteens under direct management met the School Diet Optimization Project criteria better and offered fish rich in ω3 with greater frequency than those under concession management. Quantitatively, canteens under concession offered meals with lower amounts of fat and saturated fat and higher amounts of fiber.

RESUMO INTRODUÇÃO: De forma a contribuir para a diminuição da incidência e prevalência das doenças relacionadas com os hábitos alimentares, implementou-se o Projeto de Otimização das Dietas Escolares que pretende contribuir para uma oferta alimentar escolar alinhada com as premissas de uma alimentação saudável. OBJETIVOS: Comparar qualitativa e quantitativamente a oferta alimentar escolar referente à refeição do almoço considerando os modelos de gestão direta e concessionada. METODOLOGIA: Estudo observacional de desenho transversal. O Projeto de Otimização das Dietas Escolares, implementado em 34 escolas do ensino público do distrito de Viana do Castelo, tem como objetivo garantir a oferta de refeições variadas, normoenergéticas, nutricionalmente equilibradas e que garantam os critérios de higiene e segurança alimentar. Analisaram-se dados correspondentes a uma avaliação mensal de cada escola, entre 2014 e 2019. RESULTADOS: Os refeitórios com gestão direta cumpriram maior percentagem (82%), em mediana, de critérios qualitativos do Projeto de Otimização das Dietas Escolares, relativamente aos de gestão concessionada (p = 0,022). Todos os refeitórios com gestão direta cumpriram a periodicidade de oferta de peixe rico em ómega-3 (ω3) exigida. Estes utilizaram uma maior quantidade de gordura e gordura saturada comparativamente aos de gestão concessionada (p = 0,030 e 0,040, respetivamente). O contrário ocorreu na oferta de fibra (p = 0,016). CONCLUSÕES: Considerando a avaliação qualitativa, os refeitórios com gestão direta cumpriram melhor os critérios do Projeto de Otimização das Dietas Escolares e ofereceram peixe rico em ω3 com maior periodicidade, relativamente aos com gestão concessionada. ABSTRACT School plays an important role in health promotion regarding to healthy lifestyles and eating habits, with the closure of these establishments, due to the 2nd imposed lockdown by the COVID-19 pandemic, it became pertinent to assess the impact on the lifestyles of children/adolescents and study their associated factors. An online questionnaire was applied (to legal guardians of preschool/1st cycle students and 2nd/3rd cycle students) collecting sociodemographic information, anthropometric data, health status, lifestyles, and changes due the 2nd lockdown. UniANCOVA was used to identify the factors associated with the changes. This study was carried out in a Group of Schools in the Norther region of Portugal and had a total of 494 students (median age of 11, ranging from 3 to 16 years old, 51.0% female, 49.2% 3rd cycle, 35.2% overweight). Around 57.0%, 24.0% and 31.4% of the participants comply with the sleep, physical activity and screen time recommendations, respectively. More than 70.0% of students included non-recommended foods in snacks and dinner. Due to the lockdown, 93.0% reported at least one negative change and the most frequent was the increasing in daily screen time (63.8%). The most reported positive change was consuming more meals with the family (66.5%). Respondents who reported better self-perception of eating were those with the most positive lifestyle changes due to the pandemic. On the other hand, being between the 15th and 85th percentile of Body Mass Index was associated with more negative changes.

RESUMO A escola desempenha um papel essencial na promoção de estilos de vida e hábitos alimentares saudáveis. Com o encerramento destes estabelecimentos, devido ao 2.º confinamento imposto pela pandemia COVID-19, tornou-se pertinente avaliar o seu impacto nos estilos de vida das crianças/adolescentes e estudar os respetivos fatores associados. Aplicou-se um questionário online (aos encarregados de educação dos alunos da Educação Pré-Escolar/1.º Ciclo e aos alunos do 2.º/3.º Ciclo) recolhendo informação sociodemográfica, antropométrica, de estado da saúde, estilos de vida e respetivas alterações provocadas pelo 2.º confinamento. Recorreu-se à UniANCOVA para identificar os fatores associados às alterações encontradas. Este estudo realizou-se num Agrupamento de Escolas do Norte de Portugal em 494 alunos (idade média de 11 anos, variando entre os 3 e os 16 anos, 51,0% sexo feminino, 49,2% 3.º Ciclo, 35,2% excesso de peso). Do total, 57,0% cumpre as recomendações das horas de sono, 24,0% do tempo de atividade física e 31,4% de tempo de ecrã. Mais de 70,0% dos alunos incluiu alimentos não recomendáveis aos lanches e ao jantar. Devido ao confinamento, 93,0% relatou pelo menos uma alteração negativa, sendo a mais frequente o aumento do tempo de ecrã diário (63,8%). A alteração positiva mais relatada foi a realização de mais refeições em família (66,5%). Os inquiridos que reportaram uma melhor autoperceção da sua alimentação foram os que tiveram mais alterações positivas dos estilos de vida devido à pandemia. Por outro lado, estar entre o percentil 15 e 85 de Índice de Massa Corporal associou-se a mais alterações negativas.

ABSTRACT Objective To describe and compare the clinical characteristics and outcomes of patients admitted to intensive care units during the first and second waves of the COVID-19 pandemic. Methods In this retrospective single-center cohort study, data were retrieved from the Epimed Monitor System; all adult patients admitted to the intensive care unit between March 4, 2020, and October 1, 2021, were included in the study. We compared the clinical characteristics and outcomes of patients admitted to the intensive care unit of a quaternary private hospital in São Paulo, Brazil, during the first (May 1, 2020, to August 31, 2020) and second (March 1, 2021, to June 30, 2021) waves of the COVID-19 pandemic. Results In total, 1,427 patients with COVID-19 were admitted to the intensive care unit during the first (421 patients) and second (1,006 patients) waves. Compared with the first wave group [median (IQR)], the second wave group was younger [57 (46-70) versus 67 (52-80) years; p<0.001], had a lower SAPS 3 Score [45 (42-52) versus 49 (43-57); p<0.001], lower SOFA Score on intensive care unit admission [3 (1-6) versus 4 (2-6); p=0.018], lower Charlson Comorbidity Index [0 (0-1) versus 1 (0-2); p<0.001], and were less frequently frail (10.4% versus 18.1%; p<0.001). The second wave group used more noninvasive ventilation (81.3% versus 53.4%; p<0.001) and high-flow nasal cannula (63.2% versus 23.0%; p<0.001) during their intensive care unit stay. The intensive care unit (11.3% versus 10.5%; p=0.696) and in-hospital mortality (12.3% versus 12.1%; p=0.998) rates did not differ between both waves. Conclusion In the first and second waves, patients with severe COVID-19 exhibited similar mortality rates and need for invasive organ support, despite the second wave group being younger and less severely ill at the time of intensive care unit admission. ABSTRACT INTRODUCTION: Food consumption outside home is an important health determinant for population in general and for students, who resort frequently to public catering in the areas surrounding educational establishments. OBJECTIVES: To characterize the food offer included in the student menus in the Portuguese catering establishments. METHODOLOGY: This is a cross-sectional observational study was design in the Portuguese public catering establishments. It was collected information about the composition of foods and beverages that compose the Student Menu. RESULTS: 138 student menus were analyzed with an average of 3.4 options per menu (standard deviation = 3). Only 10% include soup and none of them include fruit. The vegetarian option is rare (11%) and only 20% include the fish option, contrasting with 35% that include the white meat option and 73% that include the red meat option. In 68% of the menus french fries are served, as a side dish. Regarding to drinks, 31% of the menus include sugary drinks and 42% include water. Allergen information is presented only in 3.6% of the menus. CONCLUSIONS: The Students Menus that were analyzed reflect the inadequacy of lack of variety of food supply, highlighting the lack of fruit and vegetables and vegetarian options and the excess of red meat and soft drinks. It is relevant to adopt national strategies that promote healthier eating habits in the young population, considering the existing offer in the proximity of educational establishments.

RESUMO INTRODUÇÃO: O consumo alimentar fora de casa é um importante determinante da saúde da população no geral e dos estudantes em particular, que recorrem com frequência à restauração pública existente nas imediações dos estabelecimentos de ensino. OBJETIVOS: Caraterizar a oferta alimentar incluída nos menus para estudantes em estabelecimentos de restauração pública portugueses. METODOLOGIA: Trata-se de um estudo observacional transversal realizado em estabelecimentos de restauração pública portugueses. Recolheram-se informações sobre a composição de alimentos e bebidas que compõem os menus para estudantes. RESULTADOS: Analisaram-se 138 menus com uma média de 3,4 opções por menu (desvio-padrão = 3). Apenas 10% dos menus incluem sopa e nenhum inclui fruta. A opção vegetariana é escassa (11%) e apenas 20% dos menus oferecem pescado, contrastando com 35% de opções de carne branca e 73% de carne vermelha. Em 68% das opções são servidas batatas fritas como acompanhamento. Relativamente à bebida, 31% dos menus incluem bebidas açucaradas e 42% incluem água. Os alergénios são apresentados somente em 3,6% dos menus. CONCLUSÕES: Os menus para estudantes analisados refletem que a oferta alimentar é desadequada e pouco variada, destacando-se a falta de hortofrutícolas e opções vegetarianas e o excesso de carne vermelha e bebidas açucaradas. É relevante adotar estratégias nacionais que promovam hábitos alimentares mais saudáveis na população jovem, considerando a oferta existente na proximidade dos estabelecimentos de ensino.

Abstract Background: The burden of COVID-19 is reported to be less significant in pediatric patients, with lower morbidity and mortality. Our center secured daily telephone follow-up for pediatric COVID-19 outpatients during the isolation period of the pandemic. The aim of this study was to describe COVID-19 manifestations in the pediatric population. Methods: This was a retrospective cohort study of pediatric COVID-19 patients conducted between March 2020 and September 2021. Pediatric patients with positive SARS-CoV-2 nucleic acid amplification test performed in Emergency Department, hospitalization, or pre-surgery setting in our center, and patients followed in regular appointments in our center with positive SARS-CoV-2 test performed elsewhere were included. Cases of multisystem inflammatory syndrome in children were excluded. Demographic data, medical history, and data regarding disease course and severity were collected. Results: Two hundred and forty-two patients were included, with a median (range) age of 48 (10-144) months. Approximately one third of patients (36.0%) had at least one chronic condition, and 142 had had a prior contact with an infected relative. At diagnosis, the large majority of patients (83.9%) were symptomatic, with fever, cough, rhinorrhea, and nasal congestion as main complaints. A total of 19.8% of patients were initially hospitalized. Among the 194 patients discharged, 22 required reassessment, eight of whom needed hospitalization. Most of these (91.7%) had mild disease or were asymptomatic, while 5.8% fulfilled criteria for moderate disease and 2.5% for severe disease. Two patients required admission to the Intensive Care Unit, one of whom died. Conclusions: In this pediatric cohort, COVID-19 presented mostly as a mild disease, with non-specific and multiple symptoms. The close follow-up conducted at our center enabled a better understanding of the disease and minimizing unnecessary emergency visits and hospitalizations, while maintaining adequate patient follow-up.

Resumo Introdução: O impacto da COVID-19 tem sido menos significativo em doentes pediátricos, apresentando menor morbilidade e mortalidade. O nosso centro assegurou um acompanhamento telefónico diário para doentes COVID-19 pediátricos não hospitalizados durante o período de isolamento. O objetivo deste estudo foi descrever as manifestações de COVID-19 na população pediátrica. Métodos: Este foi um estudo de coorte retrospetivo de doentes COVID-19 pediátricos conduzido entre março de 2020 e setembro de 2021. Foram incluídos doentes pediátricos com teste PCR positivo para SARS-CoV-2 efetuado no nosso centro em contexto de Serviço de Urgência, hospitalização ou pré-cirurgia e doentes seguidos em consulta no nosso centro com teste positivo para SARS-CoV-2 realizado noutra instituição. Foram excluídos casos de síndrome inflamatória multissistémica em crianças. Foram recolhidos dados demográficos, antecedentes clínicos e informação sobre a evolução e gravidade da doença. Resultados: Foram incluídos 242 doentes no estudo, com uma mediana de 48 (10-144) meses de idade. Aproximadamente um terço (36.0%) tinha pelo menos uma doença crónica e 142 tinham tido contacto prévio com um familiar infetado. No momento do diagnóstico, a grande maioria (83,9%) dos doentes estava sintomática, sendo os sintomas mais frequentes febre, tosse, rinorreia e obstrução nasal. Um total de 19,8% dos doentes foram inicialmente hospitalizados. Dos 194 doentes que tiveram alta, 22 necessitaram de reobservação e oito destes foram internados. A maioria dos doentes teve doença ligeira ou assintomática (91,7%), com 5,8% a apresentar doença moderada e 2,5% doença grave. Dois doentes necessitaram de admissão na Unidade de Cuidados Intensivos, um dos quais faleceu. Conclusões: Nesta coorte pediátrica, a COVID-19 apresentou-se maioritariamente como uma doença ligeira, com sintomas múltiplos e inespecíficos. A monitorização destes doentes permitiu um melhor conhecimento da infeção, minimizar idas desnecessárias ao Serviço de Urgência e hospitalizações e um adequado seguimento dos mesmos.

ABSTRACT Continuous demographic changes confront us with an increasingly elderly population with growing needs for nutritional care, both from a health care perspective and from meals’ provision, which is often supplied by food services. The nutritional and food quality is a major determinant of the seniors’ health and satisfaction, and the menu is one of the most important management tools. To promote an adequate food offer, it is essential to have guidelines, that must be updated and continuously improved. The involvement of nutritionists in this process enables peer review and greater adaptation to reality, allowing to obtain tools that help in the decision-making process and sustain the developed work. This work aims to explore the perceptions and contributions of nutritionists on the document “Proposta de Ferramenta de Avaliação Qualitativa de Ementas destinadas a Idosos”, with a view to an update proposal. A qualitative methodology was adopted by conducting focus groups with a convenience sample of 38 nutritionists. Results suggest that, although the tool is considered useful, it needs to be updated to better adapt to the targeted age group and to the practical context. From the analysis of specific criteria, participants identify difficulties to comply with some of them (e.g. equitable offering of cooking methods, non-repetition of fruit, offering of oilseeds, egg based meals and whole wheat pasta or wholegrain rice), sharing the reasons why. They also suggest the need to include criteria for texture modified foods and they reflect on possible improvements in the tool application. In conclusion, the discussion promoted breeds important contributions for a possible update of the tool, in a perspective of greater adjustment to the age group it is destined to and inclusion of criteria on emerging topics.

RESUMO As contínuas alterações demográficas colocam-nos perante uma população cada vez mais envelhecida e com necessidades crescentes de cuidados alimentares, tanto numa ótica de cuidados de saúde, como de fornecimento de refeições, necessidade frequentemente suprida por serviços de alimentação coletiva. A qualidade nutricional e alimentar da oferta assume-se como um determinante major na saúde e satisfação dos idosos, sendo o plano de ementas uma das mais importantes ferramentas de gestão na área. Para que seja promovida uma oferta alimentar adequada, é essencial a existência de orientações que a balizem e que sejam alvo de atualização e melhoria contínua. A envolvência de nutricionistas no processo possibilita a revisão por pares e uma maior adaptação à realidade, permitindo obter ferramentas que auxiliem na tomada de decisão e na sustentação do trabalho desenvolvido. O presente estudo tem como objetivo explorar a perceção e os contributos de nutricionistas sobre o documento “Proposta de Ferramenta de Avaliação Qualitativa de Ementas destinadas a Idosos”, visando uma proposta de atualização. Foi adotada uma metodologia qualitativa através da realização de grupos focais a partir de uma amostra por conveniência, constituída por 38 nutricionistas. Os resultados sugerem que, apesar de ser considerada útil, a ferramenta carece de atualização para uma maior adaptação à faixa etária e à realidade prática. Da análise de critérios específicos, os participantes identificam vários de difícil cumprimento (ex. oferta equitativa de métodos de confeção, não repetição de fruta, oferta de oleaginosas, refeições de ovo e de massa ou arroz integral), partilhando os motivos para tal. Sugerem ainda a necessidade de inclusão de critérios para alimentação de textura modificada e refletem acerca de possíveis melhorias na própria dinâmica de aplicação da ferramenta. Em suma, a discussão promovida deixa importantes contributos para uma possível atualização da ferramenta, numa ótica de maior ajuste à faixa etária e de inclusão de critérios sobre tópicos emergentes.
