Cryptococcosis is a systemic mycosis usually affecting immunodeficient individuals. In contrast, immunologically competent patients are rarely affected. Dissemination of cryptococcosis usually involves the central nervous system, manifesting as meningitis or meningoencephalitis. Prostatic lesions are not commonly found. A case of prostate cryptococcal infection is presented and cases of prostatic cryptococcosis in normal and immunocompromised hosts are reviewed. A fifty-year-old HIV-negative man with urinary retention and renal insufficiency underwent prostatectomy due to massive enlargement of the organ. Prostate histopathologic examination revealed encapsulated yeast-like structures. After 30 days, the patient's clinical manifestations worsened, with headache, neck stiffness, bradypsychia, vomiting and fever. Direct microscopy of the patient's urine with China ink preparations showed capsulated yeasts, and positive culture yielded Cryptococcus neoformans. This fungus was later isolated from cerebrospinal fluid and blood cultures, demonstrating thus its dissemination. The patient was discharged after 27 days in hospital and, despite a regimen of amphotericin B, he died four months later. This case points to cryptococcosis as a possible cause of prostatic disease and reinforces the importance of communication between the medical team and pathology and microbiology laboratories aiming at a more accurate diagnosis and successful treatment.

Trezentos e cinquenta pacientes sintomáticos respiratórios admitidos no Hospial Raphael de Paula Souza, sem diagnóstico etiológico definitivo, foram triados pela técnica de imunodifusão dupla (IDD) para aspergilose. Quando a IDD foi positiva, informações adicionais como histórico e exames radiológicos foram requisitados e, quando possível, espécimes clínicos foram processados para exames micológicos e histopatológicos. Linhas de precipitação específicas para o antígeno de A. fumigatus foram encontradas em 29(8,3%) dos 350 soros de pacientes testados. Dezenove (65,5% dos 29 pacientes cm sorologia positiva foram reconhecidos como tendo boa fúngica pelos achados radiológicos em dois ou por ambos em oito pacientes. Este estudo prospectivo de dois anos mostrou que aspergilose pulmonar é um problema considerável entre pacientes admitidos em um hospital para sintomáticos respiratórios, especialmente aqueles com cavidades pulmonares ou bronquiectasias.

Double immunodiffusion (DID) was used as a screening test for the diagnosis of aspergillosis. Three hundred and fifty patients were tested, all of them referred from a specialized chest disease hospital and without a definitive etiological diagnosis. When DID was positive addtional information such as clinical history and radiographic findings were requested and also surgical specimens were obtained whenever possible. Specific precipitin hamds for Aspergillus fumigatus antigen were found in 29 (8.3%) of 350 patients sera. Nineteen (65.5%) of the 29 patients with positive serology were recognized as having a fungus ball by X-rays signs in 17 or by pathological examination in 2 or by both in 8 patients. This two-year prospective study has shown that pulmonary aspergillos is a considerable problem among patiens admitted to a Chest Diseases Hospital, especially in those with pulmonary cavities or bronchiectasis.

Acentuando a necessidade da continuação das pesquisas terapêuticas em pacientes com paracoccidioidose (blastomicose sul-americana), os autores apresentam 23 pacientes submetidos a diferentes esquemas terapêuticos, sendo 14 virgens de tratamento e 9 com uso prévio de uma ou mais drogas. A associação sulfametoxazol + trimetoprim ( SMZ + TMP) foi empregada em 5 pacientes virgens de tratamento e 9 sulfa-resistentes. Outros sete foram submetidos ao esquema clássico com sulfadoxina. Os pacientes que não responderam aos dois esquemas anteriores, com exceção de dois casos inicialmente graves, receberam anfotericina B. A avaliação clínica, radiológica, micológica e sorológica a longo prazo não demonstrou vantagens no emprego de SMZ + TMP em substituição aos sulfamídicos, nos pacientes virgens de tratamento. Entretanto, a associação SMZ + TMP parece ser uma opção válida nos casos sulfa-resistentes, onde teria primazia, considerando-se a toxicidade e necessidade de controle em regime hospitalar da anfotericina B. Ressaltam ainda a boa tolerância clínica e laboratorial da associação SMZ + TMP em cursos terapêuticos prolongados de até 2 anos, quando empregadas em baixas doses de manutenção.

Three different therapeutic schedules for paracoccidioidomycosis were studied. Twenty three thoroughly investigated patients were submitted to long terrn treatment courses using either sulfadoxine, or sulfamethoxazol + thrimethoprim (SMZ + TMP) or Anfotericin B. Clinicai, roentgenographic, mycological and serologic evaluations were extensively performed. Particular attention to possible hematopoietic abnormalities due to long term, low-losage SMZ + TMP therapy was given. Data presented suggest that there seems to be no advantage in using SMZ + TMP in previously untreated patients, as compared to sulphadoxine alone. Otherwise sulpha-resistant patients may be successfully treated with the association SMZ + TMP. No side effects of clinicai significance were observed. Emphasis in given to good tolerance observed in patients submitted to as much as two years low-dosage therapeutic courses with the association SMZ + TMP.
