Relatamos o caso de um adolescente encaminhado com o diagnóstico de hipertensão pulmonar. A investigação não invasiva detectou comunicação interatrial seio venoso com sinais de hipertensão pulmonar. No estudo hemodinâmico o diagnóstico foi confirmado, sendo também notada compressão esquerda pelo tronco pulmonar. O paciente foi submetido à oclusão cirúrgica da comunicação interatrial e à plastia redutora do tronco pulmonar. Dois anos após o procedimento, o paciente encontra-se bem, com sinais clínicos e ecocardiográficos de hipertensão pulmonar discreta e sem evidências, também pelo ecocardiograma, de obstrução do tronco da artéria coronária esquerda.

We report the case of an adolescent referred with initial diagnosis of pulmonary hypertension. Non-invasive investigation disclosed a sinus venous atrial septal defect with pulmonary hypertension. The hemodynamic study confirmed diagnosis, and also showed extrinsic compression of left main coronary artery by pulmonary trunk. Surgical closure of the defect in addition to pulmonary trunk plasty were undertaken. Two years after the surgery the patient is well, with clinical signs of mild pulmonary hypertension, and showing no evidence - also on echocardiogram - of left coronary artery trunk obstruction.

Apresenta-se a história e a evolução de uma paciente no qual o seio coronariano foi lesado e, imediatamente, reconstruído através do anel valvar mitral. O acidente cirúrgico aconteceu durante a instrumentação para remoção de uma prótese biológica degenerada. Uma quase completa lesão da artéria coronária circunflexa foi a primeira impressão. Um calibrador coronariano foi distalmente detectado dentro do átrio direito, mostrando tratar-se de uma lesão do seio coronariano. A correção cirúrgica foi realizada por sutura contínua com fio de polipropileno 7-0 através do anel valvar. Uma angiografia, realizada após 14 meses, mostrou apenas discreto estreitamento do seio coronariano no local da sutura, observando-se a fase venosa de uma arteriografia seletiva da artéria coronária descendente anterior. Os autores acreditam que a simples ligadura ou a reconstrução do seio coronariano, na condição iatrogênica apresentada, é motivo para especulações.

We report a case and evolution of a patient in whom the coronary sinus was damaged and reconstructed through the mitral annulus. The surgical accident happened during a degenerated biological valve prosthesis instrumentation. An almost complete left circumflex coronary artery division was our first hypothesis. A coronary probe introduced through the vascular injury was distally detected inside the right atrium confirming a coronary sinus lesion. The surgical correction was performed using running polypropylene 7-0 sutures through the mitral ring. After fourteen months, a coronary sinus angiographic image was taking observing the venous phase of selective left coronary arteriography. The image demonstrated an acceptable narrowing of the local venous repair. In our opinion the single coronary sinus division or its repair, in this iatrogenic situation, is a matter of speculation.

We describe the case of a 40-day-old female patient with a history of breathlessness since birth who was referred to our hospital for surgical correction of common arterial trunk. The invasive investigation disclosed a Fallot¢s tetralogy anatomy associated with an anomalous origin of the left pulmonary artery from the ascending aorta. Immediately after diagnosis, the patient underwent a successful total surgical correction of the defect, including simultaneous anastomosis of the left pulmonary artery to the pulmonary trunk.

Objetivos: Investigar a proteção miocárdica proporcionada pela infusão anterógrada intermitente, durante 60 minutos, de solução cardioplégica sangüínea a 37°C, em corações normais isolados de coelhos. Material e Métodos: Foram estudados 32 coelhos da raça Nova Zelândia, divididos em grupos, Experimental e Controle. O estudo foi realizado em 2 fases: Fase I: Estudo metabólico após isquemia sem reperfusão, Fase II: Estudo metabólico e funcional após reperfusão. A reperfusão foi realizada utilizando-se um sistema de perfusão parabiótica. Determinou-se o glicogênio miocárdico e a respiração mitocondrial no miocárdio ventricular, imediatamente ao final do período de infusão intermitente da solução cardioplégica (Fase I) e após reperfusão sangüínea (Fase II), durante a qual analisou-se, também, a função ventricular esquerda (dP/dtmax). Resultados: Detectou-se queda significativa nos níveis de glicogênio miocárdico de 58% em relação ao controle, ao final do período de infusão (Fase I). Na fase com reperfusão (Fase II), as diferenças no glicogênio miocárdico entre o grupo experimental e seu controle não foram significativas. A análise da respiração mitocondrial não mostrou diferenças significativas entre os grupos experimentais e seus controles, quer seja nas fases com ou sem reperfusão. Os valores da dP/dtmax, na Fase II foram de 903,39 ± 113,46 mmHg/s e 1.043 ± 256,94 mmHg/s, para o grupo experimental e controle, respectivamente. A diferença entre os valores não foi significativa. Conclusões: A infusão anterógrada intermitente a cada 20 minutos, durante 60 minutos, de solução cardioplégica sangüínea a 37°C, mostrou-se um método eficaz de proteção miocárdica em corações normais de coelhos.

Purpose: To study the degree of myocardial protection provided by intermittent infusion of normothermic blood cardioplegia during 60 minutes in normal rabbit hearts. Methods: The study was conducted on 32 New Zeland rabbits. There were 2 stages in this study: Stage I: Metabolic study after ischemia without reperfusion, and Stage II: Metabolic and functional study after reperfusion. The reperfusion was performed with a parabiotic system of perfusion. The glycogen concentration and mitochondrial respiration of the ventricular myocardium immediately after the intermittent infusion period of cardioplegia (Stage I) and after reperfusion (Stage II). During reperfusion the assessment of left ventricular function (dP/dtmax) was realized. Results: There was significant fall in myocardial glycogen levels of 58% in comparison with controls at the end of the intermittent infusion period of cardioplegia (Stage I). Glycogen level differences were not significant after reperfusion (Stage II). The differences in the results of the mitochondrial respiration were not significant in both Stages. The values of dP/dtmax, in Stage II were 903.39 ± 113.46 mmHg/s and 1.043 ± 256.94 mmHg/s, for experimental and control groups respectively. The difference was not significant. Conclusions: The intermittent anterograde normothermic blood cardioplegia infusion every 20 minutes during 60 minutes is an efficient method of myocardial protection in normal rabbit hearts.

Descreve-se modificação técnica da operação de Bentall e DeBono, para substituição completa da aorta ascendente e valva aórtica. Consiste na passagem de pontos separados em U, sucessivamente, no anel de fixação da prótese e no anel da valva aórtica. A posição subvalvar do anel protético, assim obtida, facilita o reimplante dos óstios coronários, sobretudo quando se encontram pouco deslocados, distalmente, como nos pequenos aneurismas. Além disso, a anastomose proximal tubo-aórtica, resulta mais segura. Quinze pacientes portadores de aneurisma da aorta ascendente foram operados por essa técnica: 14 com próteses separadas e 1 com tubo valvado. Com uma exceção, nos demais foi possível fazer o reimplante direto dos óstios coronários na prótese tubular. Ocorreram 2 óbitos hospitalares não relacionados à técnica. Os outros 13 pacientes foram seguidos por períodos variáveis de 72 dias a 109 meses, não se constatando qualquer disfunção da prótese valvar em avaliações clínicas e ecocardiográficas. Destes, 2 faleceram após 6 e 40 meses, de causa ignorada e dissecção de aneurisma toracoabdominal, respectivamente.

A techinical modification of the Bentall and DeBono operation for treatment of ascending aorta aneurysm is presented. The subvalvular insertion of the prosthesis sewing ring is accomplished, passing interrupted U sutures, consecutively in the sewing ring and in the native aortic valve annulus. As a result, direct reimplatation of the coronary arteries to the graft is simplified, particularly when the coronary ostia presents little or no cephalad displacement, as is usually the case in small aneurysms. In addition, it provides a more secure proximal anastomosis. Fifteen patients were operated on by this technique. In all but one, the coronary ostia were reattached to the vascular prosthesis. There were two hospital deaths not related to the technique. The 13 survivors were followed from 72 days to 109 months. No dysfunction of the valvular prosthesis was observed at periodic clinical and ecocardiographic evaluations. However, two late deaths occurred: one, of unknown cause, after 6 months; the other, after 40 months, related to a new surgery for treatment of a dissecting thoracoabdominal aneurysm.

A contratura isquêmica do miocárdio (CIM) tem sido muito utilizada na investigação da fisiopatologia da isquemia global do coração. Este trabalho descreve um modelo experimental adequado ao estudo da CIM, em ratos. Foram empregados 8 animais da raça Wistar, anestesiados com éter sulfúrico. O coração, removido rapidamente por toracotomia, foi imerso em solução de NaCl 0,9% a 37°C. Por meio de um cateter-balão de látex introduzido na cavidade ventricular esquerda, insuflado sob pressão de 20 mmHg e conectado a um eletrofisiógrafo, registrou-se o início da CIM (TCI) e sua intensidade (IC). A dP/dT do fenômeno foi calculada. Definiu-se uma elevação de 5 mmHg na linha de base do traçado como indicativa do início da contratura. O ponto de inflexão da curva representou a intensidade da contratura (IC). A dP/dT foi calculada. Os resultados (média ± desvio padrão) obtidos foram semelhantes aos da literatura: TCI (min) = 15,5 ± 0,59; IC (mmHg) = 62,0 ± 5,81; dP/dT (mmHg/min) = 14,2 ± 3,05.

Ischemic contracture of the heart (ICH) has been largely employed for the investigation of global heart ischemia physiopathology. We herein describe a simple ICH experimental model. Eight Wistar rats, anesthetized with sulfuric ether, were studied. The heart was rapidly removed by thoracotomy, and then immersed in a 37°C saline bath. A latex balloon catheter placed in the left ventricle and distended to 20 mmHg was used to detect ICH onset (ICHO) and intensity (ICHI). A 5 mmHg baseline elevation pepresented ICHO. ICHI was represented by peak balloon pressure. ICH dP/dT was calculated. The results (mean ± standard deviation) were similar to the literature: ICHO (min) = 15.5 ± 0.59; ICHI (mmHg) = 62.0 ± 5.81; dP/dT (mmHg/min) = 14.2 ± 3.05.

Estudaram-se a demanda metabólica e a distribuição do fluxo coronariano na presença de fibrilação ventricular (FV), durante a reperfusão pós-cardioplegia. Foram colocados 15 suínos em circulação extracorpórea e submetidos a parada cardíaca cardioplégica sangüínea anterógrada hipotérmica intermitente, durante uma hora, seguida por reperfusão miocárdica controlada. Os animais foram divididos em três grupos (n=5), conforme estivessem em assistolia (Grupo 1) ou em FV de curta (Grupo 2) ou longa duração (Grupo 3), durante os dez primeiros minutos de reperfusão. Os valores do consumo miocárdico de oxigênio (MVO2), em ml O2/min/g (média ± erro padrão) durante a reperfusão foram de 1,325 ± 0,144 (Grupo 1); 2,472 ± 0,208 (Grupo 2) e 2,469 ± 0,228 (Grupo 3). A diferença entre o MVO2 dos corações em assistolia e o dos corações em FV, quer de curta ou longa duração, foi significante (p<0,001). A relação entre os fluxos sangüíneos endo e epicárdico, bem como o fluxo sangüíneo coronário global (ml/mim/100g) foram semelhentes nos 3 grupos. Os valores dessa última variável, em ml/mim/100g, corresponderam a, respectivamente, 169,3 ± 11,7; 185,0 ± 15,7 e 179,9 ± 13,2. Os resultados demonstram que a auto-regulação coronária está alterada durante a fase inicial de reperfusão pós criocardioplegia, pois a perfusão miocárdica não aumentou em resposta à elevação do consumo de oxigênio imposta pela FV. Essa constatação, de grande interesse clínico, sugere que a ocorrência de FV durante a fase inicial da reperfusão possa contribuir para o desenvolvimento de lesões teciduais em corações cujo fluxo coronário já esteja previamente comprometido, por obstrução coronária, distensão ou hipertrofia ventricular.

Metabolic coronary blood flow recruitment and distribution were studied in the setting of ventricular fibrillation (VF) during post-cardioplegia reperfusion. Fifteen pigs were placed on cardiopulmonary bypass and subjected to one hour of intermittent cold blood antegrade cardioplegia, followed by controlled myocardial reperfusion. According to the electromechanical status during the first ten minutes of reperfusion, the animals were allocated into 3 groups (n=5). Whereas Group 1 remained asystolic, the other two groups developed short (Group 2) or long duration VF (Group 3). Myocardial oxygen consumption (MVO2), in m/O2 /min/g (mean ± standard error) during reperfusion was 1.325 ± 0.144 (Group 1); 2.472 ± 0.208 (Group 2) and, 2.469 ± 0.228 (Group 3). MVO2 difference between asystolic and both short and long duration VF hearts was significant (p<0.001). Groups 1.2 and 3 endo-epi ratio and global coronary blood flow rate (169.3 ± 11.7; 185.0 ± 15.7 and 179.9 ± 13.2 ml/min/100g; respectively) were similar. These results demonstrate that coronary blood flow autoregulation was impaired because there was no increase in myocardial perfusion in response to the increase oxygen consumption imposed by VF during the first ten minutes of post cryocardioplegia reperfusion. This finding is of great clinical importance because it suggests that VF can potentially aggravate reperfusion lesions in formerly compromised hearts, i.e., those with coronary artery obstruction, ventricular distension or ventricular hypertrophy.

Testou-se uma nova emulsão de perfluorocarbonos (OxygentMR, Alliance Pharmaceutical, San Diego, CA 92121, EUA) em circulação extracorpórea (CEC) com hipotermia de 32ºC e hematócrito de 12%. Estudaram-se 42 cães, 12 dos quais não receberam a droga e serviram de controle (Grupo 1), enquanto os demais constituíram 3 grupos de 10 animais cada, tratados com doses de Oxygent de 1,5 ml/kg (Grupo 2), 3 ml/kg (Grupo 3) e 6 ml/kg (Grupo 4) as quais geraram fluorocritos de, respectivamente, 1 %, 2% e 3%. Foram analisadas variáveis do metabolismo do oxigênio (O2) em 6 diferentes fluxos de perfusão (Q), ordenados ao acaso. Reaqueceram-se os cães, interrompeu-se a CEC e acompanharam-se os animais por 1 hora. Diferenças intergrupos foram analisadas pelo teste das médias dos quadrados mínimos e pelo teste de Duncan, considerando-se significantes os valores de p< 0,05. Embora algo maiores no Grupo 4, a pressão parcial bem como o conteúdo arterial de oxigênio (CaO2) foram estatisticamente semelhantes ao grupo controle. A pressão parcial de O2 no sangue venoso misto (PvO2) do Grupo 4, em função do Q, mostrou-se significativamente maior (p < 0,03) que a do grupo controle, conquanto isto não tenha se repetido ao ser analisada em função da oferta tecidual de O2 . Também não houve diferença quanto ao consumo de O2 total e dissolvido, nem quanto aos gradientes sistêmico e miocárdico de lactato, porém a maior dose da emulsão (Grupo 4) expressa efeito dose-dependente benéfico, ainda que de natureza especulativa, do Oxygent sobre o metabolismo do oxigênio.

A novel perfluorocarbon emulsion (OxygentTM - Alliance Pharmaceutical Corp., San Diego, CA 92121, USA) was tested during hypothermic (32ºC) cardio pulmonary bypass (CPB) under severe hemodilution (Hto = 12%). Forty-two mongrel dogs were allocated to 4 groups. Group 1 (n=12) was not given Oxygent and served as control whereas Groups 2, 3, and 4 (10 animals each) were given a bolus dose of the emulsion (1.5,3.0, and 6.0 ml/kg, respectively) at the onset of CPB. O2 metabolism variables were then studied at 6 randomized pump flow rates (Q) ranging from 0.25 to 3.0 l/min/m². After rewarming CPB was discontinued and the animals were followed-up for one hour and then killed by IV KGI injection. Differences between groups were evaluated by the least squares means and Duncan's tests and considered significant for p<0.05. Fluorocrit values remained stable on CPB Group 2 -1 %, Group 3 - 2%, and Group 4 - 3%). Although somewhat elevated, Group 4 arterial blood O2 partial pressure and content (CaO2) didn't reach statistical significance versus control. Group 4 mixed venous blood O2 partial pressure (PvO2) regressed against Q was significantly higher (p < 0.03) than control, but there was no statistical difference when this variable was regressed against O2 delivery rate. Groups were similar regarding total as well as dissolved O2 consumption and systemic and transmyocardial lactate gradients. A trend towards lower mortality rates was noticed in Groups 3 (20%) and 4 (10%) versus control (41.6%). Group 4 trend towards both reduced mortality and higher CaO2 as well as its significant PvO2 rise against Q suggest a beneficial, yet speculative, Oxygent dose dependent effect on O2 metabolism.

As anastomoses sistêmico-pulmonares continuam sendo um importante procedimento no tratamento e na preparação dos cardiópatas cianóticos, com hipoplasia das artérias pulmonares, para a correção total. Todas têm vantagens e desvantagens e os resultados dependerão de idade e peso dos pacientes e da complexidade da cardiopatia. O presente estudo relata a experiência com sete casos de anastomoses mamária-artéria pulmonar, realizadas através de toracotomia direita (5) e esquerda (2), em pacientes portadores de tetralogía de Fallot (quatro femininos e três masculinos). A idade variou de dois a 63 meses (m = 18,4), com peso médio de 7,9 kg. Seis apresentavam graus variados de hipoplasia pulmonar. Houve dois óbitos pós-operatórios (1º e 2º dias), devidos a trombose da artéria mamária, no local de seu clampleamento. Dois pacientes foram submetidos a correção total (um mês e três anos após): no 1º (menina de 24 meses, 10 kg de peso), a anastomose foi feita pelas más condições gerais, apesar do bom tamanho das pulmonares. No 2º (menino de 15 meses, 8,1 kg, no qual uma operação de Blalock-Taussig clássica foi feita e trombosou no 3º mês de vida), foi possível observar o progressivo desenvolvimento das artérias pulmonares, por estudo hemodinâmico. Em ambos, o funcionamento da anastomose era perfeito e a correção foi bem sucedida. Três outros pacientes aguardam o momento oportuno para a correção final. Apesar do reduzido número e do curto tempo de observação, acredita-se que a anastomose mamáriapulmonar possa ser outra opção paliativa para os portadores de cardiopatias congênitas cianogênicas, com hipoplasia das artérias pulmonares e que os melhores resultados devam ser obtidos nos pacientes maiores, fora de situações de emergência e com pequenas artérias pulmonares.

The systemic-pulmonary shunts are an important procedure to treat and prepare, for the final correction, cyanotic patients with hypoplastic pulmonary arteries. All of them have advantages and disadvantages, and the results will depend on the age and weight of the patients and the complexity of their heart disease. The present study reports seven internal mammary-pulmonary anastomosis, made through right (2) or left (5) thoracotomies, in patients with Tetralogy of Fallot and hypoplastic pulmonary arteries, from 2 to 63 months of age (m = 18,4) and mean weight of 7.9 kg. There were two deaths (1st and 2nd days), due to thrombosis of the internal mammary, where it was clamped. Two patients went to total correction (1 month and 3 years later). In the first case, the shunt was made due to the general conditions of the child, despite the good size of the pulmonary arteries. In the second case (a 15 months old boy, in whom a classical Blalock-Taussig shunt was made and thrombosed in the 3rd month of life) was possible to follow the progressive enlargement of the pulmonary arteries. In both the shunts were well functioning and the total correction was successful. Three patients are waiting for their best time for final procedures. Despite the small number and the short period of observations, the authors do believe that the internal mammary-pulmonary artery anastomisis is palliative option for these patients. The best results are obtained among the eldest children that are not in an emergency situation and not involving small pulmonary arteries.
