Abstract INTRODUCTION: Leishmaniasis, a disease caused by a parasite endemic to large areas of tropical and subtropical countries, is a growing public health problem. METHODS: Male BALB/c mice were infected with Leishmania amazonensis and treated with extracts isolated from Annona mucosa. RESULTS: Treated groups had significantly reduced footpad swelling. The group treated intraperitoneally with hexane extract showed footpad swelling similar to groups treated with Pentamidine® and Glucantime®. Groups treated with dichloromethane extract and hexane extract presented the recovering phenotype associated with reduced parasite levels. CONCLUSIONS: Extracts of A. mucosa are promising sources of novel antileishmanial compounds.

Leishmaniose e doença de Chagas, causadas pelos parasitas kinetoplastideos Leishmania spp e Trypanosoma cruzi, respectivamente, estão entre as doenças parasitárias mais importantes, afetando milhões de pessoas e consideradas dentro do grupo mais relevante de doenças tropicais negligenciadas. A principal alternativa para controlar essas parasitoses é a quimioterapia. No entanto, os atuais tratamentos quimioterápicos estão longe de serem satisfatórios. Esta revisão delineia o entendimento atual de diferentes fármacos contra leishmaniose e doença de Chagas e seus mecanismos de ação. As abordagens recentes na área de terapias anti-Leishmania e tripanocida também são enumerados, assim como a busca por novas drogas.

Leishmaniasis and Chagas disease, caused by the kinetoplastid parasites Leishmania spp and Trypanosoma cruzi, respectively, are among the most important parasitic diseases, affecting millions of people and considered to be within the most relevant group of neglected tropical diseases. Chemotherapy is the main alternative to control such parasites, nevertheless, current treatments are far from satisfactory. This review outlines the current understanding on different drugs against leishmaniasis and Chagas disease and their mechanism of action. Recent approaches in the area of anti-leishmanial and trypanocidal therapies are also enumerated, as well as the search for new drugs.

Piper is a notable genus among Piperaceae due to their secondary metabolites such as lignans, amides, esters and long chain fatty acids used as anti-herbivore defenses with comparable effects of pyrethroids, that holds a promise in insect control, including malaria vectors such as Anopheles darlingi, the main vector in the North of Brazil. Methanolic extracts of Piper tuberculatum Jacq., Piperaceae, and P. alatabaccum Trel. & Yunck., Piperaceae, and some isolated compounds, i.e, 3,4,5-trimetoxy-dihydrocinamic acid, dihydropiplartine; piplartine, piplartine-dihydropiplartine and 5,5',7-trimetoxy-3',4'-metilenodioxiflavone were tested as larvicides against A. darlingi. The Lethal Concentrations (LC50 and LC90) of methanolic extracts were 194 and 333 ppm for P. tuberculatum and 235 and 401 ppm for P. alatabacum, respectively. Isolated compounds had lower LC values, e.g. the LC50 and LC90 of the piplartine-dihidropiplartine isolated from both plant species was 40 and 79 ppm, respectively.

The Piper species chemistry has been widely investigated and the phytochemical analyses have led to the isolation of a number of active compounds like alkaloids, terpenes and flavones among others. The aim of this study was to evaluate the leishmanicidal activity of 2-[1-hydroxy-3-phenyl-(Z,2E)-2-propenylidene]-4-methyl-4-cyclopentene-1,3-dione (DCPC), a cyclopentenedione derivative isolated from the roots of Piper carniconnectivum C. DC., Piperaceae. Leishmanicidal activity against Leishmania amazonensis promastigotes was assessed, and the risk to host cell was assessed by measuring the cytotoxicity to peritoneal macrophages from BALB/c mice in vitro. L. amazonensis promastigotes and host macrophages were cultured in the presence of 100, 50, 25, 12.5 and 6 µg/mL of the cyclopentenedione derivative for up to 96 h. At the end of this period, the inhibitory concentrations (IC50) were compared with those from untreated cultures. The IC50 for promastigotes was 4.4 µg/mL after 96 h of treatment with the derivative. The 50% cytotoxic concentration (CC50) against murine peritoneal macrophages was 129 µg/mL. These results indicate that DCPC is a promising molecule for the development of leishmanicidal drugs.

O presente trabalho descreve a atividade do extrato etanólico (EE) dos frutos de Combretum leprosum, do triterpeno 3β, 6β, 16β-triidroxilup-20(29)-eno (1) e seus derivados sintéticos (1a-1d), sobre promastigotas de Leishmaniaamazonensis. O EE apresentou atividade leishmanicida e o valor de IC50 foi de 24,8 µg mL-1. Já o triterpeno 3β, 6β, 16β-trihidroxilup-20(29)-eno (1), na concentração de 5,0 µg mL-1, apresentou uma potente ação inibitória sobre a proliferação das promastigotas (IC50 = 3,3 µg mL-1). Entre os derivados sintéticos, apenas 1b e 1d apresentaram atividade contra as promastigotas (IC50 = 3,48 µg mL-1e 5,8 µg mL-1, respectivamente). Por outro lado, o derivado sintético 1a não apresentou atividade sobre as promastigotas de L. amazonensis. O EE, (1) e os derivados sintéticos 1a-1d não apresentaram efeito citotóxico sobre macrófagos peritoneais de camundongos. Estes resultados fornecem evidencias de que o extrato etanólico e o lupano isolado de C. leprosum possui atividade contra promastigotas de L. amazonensis, podendo ser utilizados como ferramentas no estudo de novas drogas leishmanicidas.

This paper describes the activity of the ethanolic extract (EE), obtained from the fruits of Combretum leprosum, the triterpene 3β, 6β, 16β-trihydroxylup-20(29)-ene (1) and its synthetic derivatives 1a-1d on Leishmania amazonensis promastigotes. The EE displayed leishmanicidal activity and the IC50 was 24.8 mg mL-1. However, the triterpene 3β, 6β, 16β-trihydroxylup-20(29)-ene (1), at a concentration of 5.0 mg mL-1, showed a potent inhibitory activity on promastigotes proliferation (IC50 = 3.3 mg mL-1). Among the synthetic derivatives, only (1b) and (1d) were active against promastigotes (IC50 = 3.48 mg mL-1and 5.8 mg mL-1, respectively). Moreover, the synthetic derivative 1a showed no activity on promastigotes of L. amazonensis. EE, (1) and the synthetic derivatives 1a-1d showed no cytotoxic effect on mice peritoneal macrophages. These results provide evidence that the ethanolic extract and the lupane isolated from C. leprosum was active against promastigotes of L. amazonensis, and may be used as a tool in the studies of new antileishmanial drugs.

A atividade leishmanicida do ácido 3,4,5-trimetoxi-dihidrocinâmico (TMPP) isolado do extrato hidroalcoólico de frutos de Piper turbeculatum Jacq. amazônica foi testado em ensaios in vitro utilizando formas promastigotas de Leishmania amazonensis. O TMPP foi utilizado em culturas de L. amazonensis nas concentrações de 1600 a 6,25 µg/mL. A viabilidade celular das formas promastigotas foi observada em 24, 48, 72 e 96 h para cálculo da CI50. O TMPP apresentou efeito leishmanicida dose dependente para as formas promastigotas de L. amazonensis apresentando CI50 de 145 µg/mL.

Leishmanicidal activity of the 3-(3,4,5-trimethoxyphenyl) propanoic acid (TMPP) isolated from EtOH extracts of the Amazonian Piper turbeculatum Jacq. fruits was evaluated in vitro using Leishmania amazonensis promastigotes. The TMPP was assayed at concentrations of 1600 to 6.25 µg/mL for 24, 48, 72 and 96 h. Promastigotes viability was analyzed and the IC50 of TMPP was 145 µg/mL.

A quimioterapia é o único procedimento farmacológico validado para a terapêutica da leishmaniose e da malária, consideradas doenças negligenciadas para o desenvolvimento de fármacos pelas indústrias farmacêuticas. Com a não renovação medicamentosa, o surgimento de resistência, os efeitos colaterais e o longo período de tratamento indicam a necessidade do desenvolvimento de novos e mais eficientes fármacos. A Floresta Amazônica é a região com a maior biodiversidade do planeta, com uma riqueza de animais e plantas produtoras de moléculas com atividades biológicas relevantes para o desenvolvimento e exploração biotecnológica. Algumas destas moléculas, obtidas de extratos vegetais e de venenos de anuros, apresentam atividade leishmanicida e plasmodicida, o que demonstra o potencial desta biodiversidade para a investigação de novas drogas. A moderna abordagem na pesquisa de novos fármacos envolve a associação de química combinatória, high-throughput screening, bioinformática, interação molecular, cristalografia e o estudo dinâmico de toxicidade sistêmica e celular, que atualmente no Brasil, estão distribuídas em poucos grupos acadêmicos sem a devida associação industrial. Esta deficiência, agregada ao excesso de regulamentação para o acesso ao material biológico, sobretudo, proveniente de unidades de conservação, populações tradicionais e nações indígenas, é um importante entrave para o desenvolvimento deste tipo de pesquisa. A associação de grupos de pesquisa do Brasil, estimulados por políticas governamentais de financiamento acadêmico e industrial, são essenciais para a superação destas dificuldades, de forma que nos próximos anos possam surgir novos produtos para terapia de doenças negligenciadas oriundas da biodiversidade amazônica.

Chemotherapy is the only validated therapy for the treatment for the neglected diseases leishmaniasis and malaria. However, the emergence of drug resistance, collateral effects and long-term treatment encourage the development of new and more efficient drugs. The Amazon tropical forest includes the richest areas of biodiversity in the world, including a great number of microbes, plant and animal species that produce a source of interesting biologically active molecules. Several of these molecules, obtained from plant extracts and frog venom have leishmanicidal and plasmodicidal activity, highlighting the potential of this biodiversity for the development of new drugs. In research, modern approaches in new drug development are carried out using combinatorial chemistry, high-throughput screening, bioinformatics, molecular interaction, crystallography and dynamic studies of cellular and systemic toxicity. In Brazil, these techniques are mainly present in only a few academic groups with no efficient connection to industry. The problem associated with over-regulation for accessing the biological material in restricted areas, local populations and indigenous areas places major barriers in the path of research and development of new drugs. Thus, the association of academic research groups in Brazil, encouraged and supported by government and industry, is essential to overcome these major barriers related to the development of new products for treatment of neglected diseases from Amazonian biodiversity in future years.
