Resumo Este estudo avaliou o impacto do pré-alargamento do canal usando uma lima K de tamanho #25 na incidência de fraturas de três instrumentos reciprocantes após múltiplos usos. Após um glide path realizado com uma lima K de tamanho #15, os instrumentos (Reciproc R25, Reciproc Blue R25, e WaveOne Gold Primary) foram inicialmente usados em 936 canais radiculares. Depois, para o segundo e terceiros usos (933 e 927 canais, respectivamente), um pré-alargamento usando uma lima K de tamanho #25 no comprimento de trabalho foi realizado. Radiografias periapicais avaliaram a localização e o comprimento dos fragmentos, e os testes qui-quadrado compararam as frequências observadas e esperadas (α = 5%). Abstract This study assessed the impact of pre-enlarging the root canal using a size 25 K-file on the fracture incidence of three reciprocating instruments after multiple uses. After a glide path performed with a size 15 K-file, the instruments (Reciproc R25, Reciproc Blue R25, and WaveOne Gold Primary) were initially used in 936 root canals. Abstract This study compared the effects of different final irrigation protocols on the mechanical properties and structural integrity of root dentine. One-hundred eight teeth were instrumented and irrigated with sodium hypochlorite (NaOCl) using conventional irrigation (CI). Teeth were distributed into four groups based on final irrigation protocols: Control Group (CG): 17%EDTA/CI + H2O; G1: 2.5%NaOCl/Passive Ultrasonic Irrigation (PUI) + EDTA/PUI + NaOCl/PUI (60s each); G2: EDTA/PUI + NaOCl/PUI (30s each); G3: EDTA/PUI + NaOCl/PUI + H2O/PUI + CHX/PUI (30s each). Four tests were conducted: three-point flexural strength test, Scanning Electron Microscopy analysis, microhardness assessment, and the push-out bond strength (POBS) of the filling material to the root dentine. Data were analyzed using ANOVA and Tukey tests (Flexural strength), and Student t-test (Microhardness). Erosion scores and POBS were analyzed by Kruskal-Wallis and Dunn tests (α = 0.05). Results indicated no significant differences in flexural strength (p > 0.05) among groups. CG exhibited the lowest erosion scores in the cervical third. In the middle third, CG had lower scores than G1 and G2, while in the apical third, CG had lower scores than G1 (p < 0.05). Microhardness values decreased following the protocols (p < 0.05), except for the CG (p > 0.05). G2 displayed higher POBS values in the middle and apical thirds (p < 0.05). The protocols did not significantly influence the flexural strength of root dentine. However, they did affect microhardness and promoted greater erosion. Resumo Este estudo avaliou os instrumentos Reciproc R25 e Pro-R 25 sem uso, após um primeiro e um segundo uso em retratamento endodôntico com perfilômetro 3D por interferometria de luz sem contato, microscópio eletrônico de varredura (MEV) e testes de fadiga cíclica. Vinte dentes unirradiculares foram instrumentados com Reciproc R25 e obturados com guta-percha e cimento endodôntico. Um perfilômetro 3D com uma objetiva 20x usando o software Mx ™ foi usado para avaliar as superfícies da lâmina de corte do Reciproc e Pro-R (n = 5 por grupo) na condição sem uso, após o primeiro e após um segundo uso em procedimentos de retratamento. Após retratamento, o MEV foi usado para avaliar as características topográficas dos instrumentos utilizados. Testes de fadiga cíclica foram realizados para comparar instrumentos novos com instrumentos usados. O teste One-way ANOVA seguido pelo teste de Tukey foi usado para comparar os instrumentos testados nos diferentes estágios. Para comparar os diferentes instrumentos e para avaliação de fadiga cílcica, foi utilizado o Student t-test. O Reciproc sem uso apresentou maior tempo de fratura do que os instrumentos Pro-R (p<0,05), e apenas o Pro-R apresentou diferenças entre instrumentos novos e usados (p<0,05). Os procedimentos de retratamento com Reciproc e Pro-R não alteraram a topografia da superfície dos instrumentos. Reciproc apresentou maior resistência à fadiga cíclica em comparação com o Pro-R.

Abstract This study evaluated the Reciproc R25 and Pro-R 25 instruments in unused condition, after one and a second use in endodontic retreatment employing a noncontact 3D light interferometer profiler, scanning electron microscopy (SEM) and cyclic fatigue tests. Twenty single-root teeth were instrumented with Reciproc R25 and filled with gutta-percha and sealer. A 3D profiler with a 20x objective using the Mx™ software was used to evaluate the cutting blade surfaces of Reciproc R25 and Pro-R 25 (n=5 per group) in unused condition, after the first and second uses in retreatment procedures. After retreatment, SEM was used to evaluate the topographic features of the used instruments. Cyclic fatigue tests were performed to compare new to used instruments. One-way ANOVA followed by Tukey test was used to compare the tested instruments before and after the first and second uses. Student t-test was used to compare the different instruments and for cyclic fatigue evaluation. No significant differences were observed in the cutting blade surfaces of Reciproc and Pro-R before and after one and two uses (p>0.05). Reciproc without use showed higher Sa and Sq when compared to Pro-R without use (p<0.05). No differences were observed between Reciproc and Pro-R after one and two uses (p>0.05). New and unused Reciproc showed longer time to fracture than Pro-R instruments (p<0.05), and only Pro-R showed differences between new and used instruments (p<0.05). Retreatment procedures with Reciproc and Pro-R did not change the surface topography of instruments. Reciproc had greater resistance to cyclic fatigue compared with Pro-R.

Abstract This study evaluated the cytotoxicity, the antimicrobial and physicochemical properties of root canal sealers incorporated with phytotherapic Uncaria tomentosa (UT). Unmodified AH Plus (Dentsply, DeTrey, Germany) and MTA Fillapex (Angelus, Londrina, Brazil) were used as controls. UT was incorporated into AH Plus and MTA Fillapex, at concentrations of 2% and 5% of the total weight of these sealers (w/w). Flowability, setting time, and solubility were evaluated following ISO requirements. The pH values were measured at periods of 12, 24, 48 hours, and 7 days. The antimicrobial activity of the sealers against Enterococcus faecalis was analyzed by both direct contact tests in freshly prepared sealers, and after 7 days. The cytotoxicity of the samples was evaluated by the MTT assay, to check Balb/c 3T3 cell viability. The statistical analysis was performed by one-way ANOVA and Tukey’s test (p < 0.05). The incorporation of UT was associated with a decrease in flow, for both sealers, an increase in AH Plus setting time, increase in MTA Fillapex pH values, and solubility (after 14 days), for both sealers (p < 0.05). Regarding the antibacterial evaluation, bacterial reduction was reported after incorporation of UT into both AH Plus and MTA Fillapex, up to 7 days after handling of the material (P<0.05). UT incorporation decreased the cytotoxic effects of both AH Plus and MTA Fillapex sealers in a way directly proportional to their respective concentrations (p < 0.05). In conclusion, UT can be added to both sealers to reduce their cytotoxicity, and improve their antibacterial effects, without compromising their original physicochemical properties.

Abstract: Accumulated hard tissue debris (AHTD) in root canal irregularities may negatively impact adequate root canal disinfection. In light of this, the efficacy of passive ultrasonic irrigation (PUI) to reduce AHTD has been largely studied in in vitro studies, which have adopted different analytic methods of varying accuracy to determine the extent of AHTD more correctly. Therefore, the aim of this study was to compare how well PUI and non-activated irrigation (NAI) systems perform in reducing AHTD during final irrigation protocols, based exclusively on studies whose analyses used microCT scanning. A systematic search of the studies published up to April 2020 was performed using MeSH terms and free terms, in the following databases: PubMed, Scopus, Web of Science, BVS (Lilacs and BBO) and Embase. The inclusion criteria consisted of laboratory studies that evaluated the amount of AHTD, and compared PUI with NAI protocols using microCT analysis. The risk of bias in the selected studies was assessed critically by two reviewers. A meta-analysis was performed using the RevMan software program (P<0.05), and included studies providing the standardized mean difference (SMD), using a fixed effect model, and adopting a confidence interval of 95%. In all, 3495 studies were identified, three of which met the inclusion criteria. All three were considered as having a low risk of bias. The meta-analysis comparing the ability of PUI and NAI protocols to remove hard tissue debris showed a higher percentage of AHTD reduction (P<0.01) for PUI, with a confidence interval of 1.41 [0.79, 2.02]. The heterogeneity among the studies was 82% (I2). Considering the limitations of the present study, this systematic review and meta-analysis showed that PUI was more effective than NAI in removing hard tissue debris, based exclusively on studies that used microCT scanning to provide a more precise analysis of the two techniques used. The findings presented in the present study reinforce the concept that PUI can increase residue removal and improve the cleanliness of the root canal in endodontic treatments.

Abstrac The aim of the present study was to evaluate the cyclic fatigue, torsional resistance and surface roughness of Reciproc R25 instruments in four different situations, namely as new instruments and as instruments tested after clinical preparation of one, two or three maxillary molars with four root canals. The total time required to perform each root canal preparation was recorded. Cyclic fatigue resistance was determined by the time to fracture using a customized testing device (n = 10 per group). The torsional test evaluated the torque and angle of rotation to failure according to ISO 3630-1 (n = 10 per group). The roughness of the working parts of new and used instruments was evaluated with a profilometer (n = 5 per group). Statistical analysis was performed using one-way ANOVA and Tukey’s test. The level of significance was set at 5%. No fractures or deformations were observed after clinical use. Higher preparation time was needed during the third use of the instruments for all root canals (p < 0.05). There were no significant differences among the groups in regard to either cyclic fatigue or torsional resistance (p > 0.05). Regarding the roughness measurements, groove depth was higher on new and one- versus two- or three-maxillary-molar-prepared instruments (p < 0.05). It can be concluded that the clinical use of Reciproc instruments increased preparation time and decreased surface roughness. However, clinical use did not affect the cyclic fatigue or torsional resistance of the Reciproc instruments.

Abstract The aim of this randomized clinical trial was to compare the occurrence and intensity of postoperative pain and analgesic intake after root canal treatment, using different root canal sealers. Sixty single-rooted teeth diagnosed with asymptomatic necrosis and apical periodontitis were randomly assigned to 3 experimental groups (n=20), according to the root canal sealer: AH Plus, Endofill or MTA Fillapex. Endodontic treatment was performed in two sessions, and calcium hydroxide was used as the intracanal dressing. Patients were instructed to record pain intensity as none, slight, moderate and severe. Scores from 1 to 4 were attributed to each level of pain after 24 h, 48 h and 7 days. The need for analgesic intake was also recorded. Differences in the incidence of postoperative pain and the need for an analgesic were analyzed using the chi-square test. Differences in pain intensity after treatment were analyzed using the ordinal (linear) chi-square test. No significant differences were detected among the groups in terms of either incidence or intensity of postoperative pain, or need for analgesic intake, at any timepoint (p>0.05). No pain was reported after 7 days. AH Plus, Endofill and MTA Fillapex used for filling root canals resulted in the same rate of postoperative pain and need for analgesic medication.

Abstract The objective of this study was to assess the influence of chlorhexidine (liquid and gel) and zinc oxide in calcium hydroxide (CH) pastes on root pH in simulated external resorption. One hundred human anterior teeth with a single root canal were selected. After decoronation and root canal instrumentation, the specimens were divided into 4 experimental groups and 1 control group (without intracanal paste): CH + saline (CH+S), CH + 2% chlorhexidine liquid (CH+ CHX), CH + 2% chlorhexidine gel (CH+ CHXg), and CH + 2% chlorhexidine gel + zinc oxide (CH+ CHXg+ZnO). pH was measured using a microelectrode at 3 and 24 h, and 1, 2, 3, and 4 weeks after inserting intracanal pastes. Data were analyzed statistically using an ANOVA and Tukey’s test (p < 0.05). The CH+CHXg+ZnO group had the highest pH values throughout (p<0.05). The CH+S and CH+ CHX groups had the highest pH values after 1 week and the CH+ CHXg group after 2 weeks. CH+ CHXg maintained the highest pH until the fourth week compared with CH+ CHX (p < 0.05). The control group remained at a neutral pH at all evaluated times. It can be concluded that chlorhexidine solution or gel maintained the alkaline pH of CH, and chlorhexidine gel allowed a slower decrease in pH over time. CH+ CHXg+ZnO showed the highest pH values and was an effective intracanal medication for maintaining alkaline root pH in the area of resorption.

Resumo Esse estudo investigou a resistência de união de dois cimentos endodônticos experimentais à base de MTA e butiletilenoglicol dissalicilato: MTAe e MTAe. Os materiais de referência utilizados para comparação foram os cimentos endodônticos MTA Fillapex e AH Plus. Vinte incisivos superiores humanos foram selecionados e um slice dentinário de 1 mm de espessura foi obtido do terço cervical de cada raiz. Na superfície coronária de cada slice, quatro orifícios com 0,9 mm de diâmetro foram confeccionados através da dentina. Uma irrigação padronizada foi realizada e os orifícios foram preenchidos com um dos quatro cimentos endodônticos avaliados: AH Plus, MTA Fillapex, MTAe, e MTAe-HA. Os slices preenchidos foram armazenados em solução PBS (pH 7,2) durante 7 dias a 37°C. O ensaio de push-out foi realizado por meio de um dispositivo com 0,7 mm de diâmetro. A carga foi aplicada com a velocidade de 0,5 mm/min até a obtenção de deslocamento do material obturador. Os resultados foram expressos em MPa. O teste de Kruskal-Wallis foi aplicado para avaliar o efeito da resistência de união de cada cimento endodôntico. O teste de Mann-Whitney com correção de Bonferroni foi utilizado para isolamento das diferenças. O erro do tipo-alfa foi fixado em 0,05. Diferenças significantes entre os valores de medianas obtidos pelos materiais foram observados (p<0,001). O AH Plus demonstrou os maiores valores de resistência de união (p<0,001). Em contraste, o MTA Fillapex apresentou a menor resistência de união entre todos os cimentos testados (p<0,001). Os cimentos experimentais demonstraram valores intermediários, com ausência de diferenças estatísticas entre si (p>0,05). Em conclusão, os cimentos endodônticos experimentais à base de MTA e butiletilenoglicol dissalicilato apresentaram resultados adequados de resistência de união quando comparados ao MTA Fillapex.

Abstract This study investigated the bond strength of two experimental root canal sealers based on MTA and butyl ethylene glycol disalicylate: MTAe and MTAe-HA. The reference materials used for comparison were AH Plus and MTA Fillapex. Twenty human upper incisors were selected and one 1 mm slice was obtained from the cervical third of each root. On the coronal surface of each slice, four 0.9 mm wide holes were drilled through the dentine. Standardized irrigation was performed and holes were filled with one of the four tested sealers: AH Plus, MTA Fillapex, MTAe, and MTAe-HA. The filled slices were stored in a PBS solution (pH 7.2) for 7 days at 37 °C. A push-out assessment was performed with a 0.7 mm plunger tip. Load was applied at a crosshead speed of 0.5 mm/min until sealer displacement. The results were expressed in MPa. The Kruskal-Wallis test was applied to assess the effect of each sealer on the push-out bond strength. Mann-Whitney with Bonferroni correction was used to isolate the differences. The alpha-type error was set at 0.05. Significant differences among medians values obtained by materials were observed (p<0.001). AH Plus displayed the highest value of bond strength (p<0.001). In contrast, MTA Fillapex presented the lowest bond strength among all tested sealers (p<0.001). Experimental sealers showed intermediary bond strength values, with no statistical differences between them (p>0.05). In conclusion, experimental root canal sealers presented suitable bond strength outcomes when compared to MTA Fillapex.

Resumo O objetivo do presente estudo foi avaliar a fadiga cíclica de dois instrumentos finalizadores anatômicos: XP-Endo Finisher e XP-Clean. O padrão de rugosidade e a micro dureza dos instrumentos também foram avaliados. Os instrumentos foram submetidos à resistência à fadiga cíclica, medindo o tempo de fratura em um canal artificial de aço inoxidável com um ângulo de 60 ° e um raio de curvatura de 5 mm. A superfície de fratura de todos os fragmentos foi examinada com um microscópio eletrônico de varredura. A rugosidade dos instrumentos foi quantificada usando um perfilômetro e o teste de micro dureza foi realizado usando um testador de dureza Vickers. Os resultados foram analisados estatisticamente usando o teste t de student em um nível de significância de p<0,05. A análise Weibull também foi realizada. XP-Endo Finisher apresentou vida de fadiga cíclica significativamente mais longa do que os instrumentos XP-Clean (p<0,05). XP-Endo Finisher foi capaz de suportar 1000% mais ciclos para fratura quando comparado aos instrumentos XP-Clean. A inspeção visual em microscópio eletrônico de varredura das superfícies de fratura revelou características fractográficas da fratura dúctil em todos os instrumentos testados. Não foi observada deformação plástica no eixo helicoidal dos instrumentos fraturados. Quando o tempo para a fratura foi comparado entre os instrumentos, o XP-Endo Finisher durou mais do que o XP-Clean com uma probabilidade de 99,9%. Os instrumentos XP-Endo Finisher também exibiram uma rugosidade significativamente menor do que os instrumentos XP-Clean (p<0,05). Não foram observadas diferenças na micro dureza entre os arquivos (p>0,05). Pode-se concluir que os instrumentos XP-Endo Finisher apresentaram desempenho melhorado em comparação com os instrumentos XP-Clean, demonstrando maior resistência à fadiga cíclica e menor rugosidade.

Abstract The aim of the present study was to evaluate the cyclic fatigue of two anatomic finishing files: XP-Endo Finisher and XP-Clean. Roughness pattern and the micro-hardness of the files were also assessed. Instruments were subjected to cyclic fatigue resistance measuring the time to fracture in an artificial stainless-steel canal with a 60° angle and a 5-mm radius of curvature. The fracture surface of all fragments was examined with a scanning electron microscope. The roughness of the working parts was quantified by using a profilometer and the micro-hardness test was carried out using a Vickers hardness tester. Results were statistically analyzed using a student´s t-test at a significance level of P < 0.05. Weibull analysis was also performed. XP-Endo Finisher presented significantly longer cyclic fatigue life than XP-Clean instruments (P<0.05). XP-Endo Finisher was able to withstand 1000% more cycles to fracture when compared to XP-Clean instruments. SEM visual inspection of the fracture surfaces revealed fractographic characteristics of ductile fracture in all tested instruments; wide-ranging forms of dimples were identified and no plastic deformation in the helical shaft of the fractured instruments was observed. When mean life was compared XP-Endo Finisher lasted longer than XP-Clean with a probability of 99.9%. XP-Endo Finisher instruments also exhibited significantly lower roughness than XP-Clean instruments (P<0.05). No differences in the micro-hardness was observed between the files (P>0.05). It can be concluded that XP-Endo Finisher instruments showed improved performance when compared with XP-Clean instruments, demonstrating higher cyclic fatigue resistance and lower roughness.

Abstract This study analyzed the influence of different retreatment protocols on amount of remaining filling material and amount of new sealer after endodontic retreatment. Forty mandibular molars with curved mesial roots were prepared with ProTaper Universal system, and filled with AH Plus sealer mixed with 0.1% rhodamine B and gutta-percha. After 7 days, the specimens were randomized according to the retreatment protocol (n = 10): ProTaper Retreatment System (PTR); PTR+Orange Oil (PTR+OO); PTR+Passive Ultrasonic Irrigation (PTR+PUI). No retreatment was performed in the control group (CG). After retreatment, the root canals were filled with AH Plus mixed with 0.1% fluorescein and gutta-percha. Samples were evaluated under confocal laser scanning microscopy and analyzed using Image J software. Data were analyzed using Kruskal-Wallis and Dunn tests (p < 0.05). Regarding presence of residual filling, the Kruskal-Wallis test indicated no differences among the different retreatment techniques in the perimeter and the isthmus analyses (p > 0.05); however, PTR+PUI was associated with a lesser amount of residual filling material in the canal area analysis (p < 0.05). In evaluating the new filling, the perimeter analysis showed a lesser amount of new endodontic sealer in the PTR group (p < 0.05). Moreover, the PTR+PUI group presented a significantly greater amount of new endodontic sealer in the canal area analysis (p < 0.05). There was no difference among groups in the isthmus analysis (p > 0.05). It can be concluded that PTR associated to PUI yielded better results in removing root canal filling material from the canal area. However, none of the protocols resulted in root walls completely free of remnants.

Resumo O presente estudo comparou o efeito da utilização de uma irrigação intermediária com água destilada usando a irrigação convencional, a microcânula EndoVac ou o sistema Self-Adjusting File (SAF) na prevenção de formação da smear-layer química (SLQ). Trinta pré-molares humanos foram utilizados. Os canais foram preparados com sistema Reciproc e irrigados com NaOCl a 5,25%. Após o preparo químico mecânico, as amostras foram divididas em 3 grupos (n=10) de acordo com o protocolo de irrigação intermediária com água destilada utilizado: irrigação convencional, a microcânula EndoVac ou SAF. Utilizou-se uma lavagem final com solução de clorexidina a 2% e a microscopia electrónica de varredura foi utilizada para avaliar a eficácia dos protocolos. Dois avaliadores calibrados atribuíram escores de acordo com a presença ou ausência de SLQ nas paredes do canal radicular nos terços coronal, médio e apical, como a seguir: (1) sem SLQ; (2) pequenas quantidades de SLQ; (3) SLQ moderada e (4) muita SLQ. As diferenças entre protocolos foram analisadas com testes de Kruskal-Wallis e Mann-Whitney U. Os testes Friedman e Wilcoxon foram utilizados para comparação entre cada terço do canal radicular. SAF resultou em menor formação de SLQ quando comparado com a irrigação convencional e a microcânula EndoVac (p<0,05). Quando os terços dos canais radiculares foram analisados, os grupos irrigação convencional e microcânula EndoVac apresentaram menor formação de SLQ nos terços coronal e médio em relação ao terço apical (p<0,05). No grupo SAF, não houve diferença entre os terços (p>0,05). Dentro dos resultados do presente estudo, pode-se concluir que um fluxo intermediário de água destilada, administrado pelo sistema SAF resultou em melhor redução da formação de SLQ durante o preparo químico mecânico.

Abstract This study compared the effect of intermediate flush with distilled water delivered by conventional irrigation, EndoVac microcannula or Self-Adjusting File (SAF) system in the prevention of chemical smear layer (CSL) formation. Thirty human premolars were used. Canals were prepared with Reciproc system and 5.25% NaOCl. After chemomechanical preparation, samples were divided in 3 groups (n=10) according to the intermediate irrigation protocol with distilled water using: conventional irrigation, EndoVac microcannula or SAF. A final flush with 2% chlorhexidine solution was used and scanning electron microscopy was performed to assess protocol effectiveness. Two calibrated evaluators attributed scores according the presence or absence of CSL on the surface of the root canal walls at the coronal, middle and apical thirds, as follows: (1) no CSL; (2) small amounts of CSL; (3) moderate CSL; and (4) heavy CSL. Differences between protocols were analyzed with Kruskal-Wallis and Mann-Whitney U tests. Friedman and Wilcoxon signed rank tests were used for comparison between each root canal third. SAF resulted in less formation of CSL when compared with the conventional irrigation and EndoVac microcannula (p<0.05). When root canal thirds were analyzed, conventional irrigation and EndoVac groups showed less CSL formation at coronal and middle thirds in comparison to the apical third (p<0.05). In SAF group, there was no difference among the thirds (p>0.05). It may be concluded that an intermediate flush of distilled water, delivered by the SAF system resulted in a better reduction of CSL formation during chemomechanical preparation.

Resumo O objetivo deste estudo clínico foi avaliar a relação entre a demora na procura por tratamento e o desenvolvimento de reabsorções inflamatórias externas em dentes luxados com ápice fechado. A amostra desta pesquisa foi constituída por 67 pacientes que apresentaram 133 dentes traumatizados, dos quais 56 foram acometidos por luxação extrusiva, 69 por luxação lateral e 8 por luxação intrusiva, proservados por no mínimo 24 meses. Foi registrado o tempo decorrido entre a data do trauma e o primeiro atendimento. Verificou-se a ocorrência de necrose pulpar e reabsorções inflamatórias externas em cada tipo de traumatismo. O Teste Exato de Fisher foi aplicado para verificar a influência da data do atendimento inicial e o desenvolvimento de reabsorções inflamatórias externas em cada tipo de luxação e o teste de Odds Ratio foi aplicado para verificar o risco do desenvolvimento de reabsorções inflamatórias externas em função da demora na procura por tratamento. Os resultados revelaram que a necrose pulpar foi verificada em 105 dentes luxados (78,9%) e a reabsorção inflamatória externa foi verificada em 17.8% das luxações extrusivas (10), sendo 15,9% das luxações laterais (11) e 25% das luxações intrusivas (2). A demora na procura por tratamento influenciou o desenvolvimento de reabsorções infamatórias externas nas luxações laterais (p=0,0174). Os pacientes que procuraram tratamento após 45 dias da data do trauma apresentaram 3,4 vezes mais chance de desenvolver reabsorção inflamatória externa do que os pacientes que procuraram tratamento logo após o traumatismo. Conclui-se que o tratamento tardio após a ocorrência do traumatismo pode prejudicar o prognóstico e favorecer o desenvolvimento de reabsorções inflamatórias externas em dentes luxados com ápice fechado.

Abstract This clinical study aimed to evaluate the relationship of the delay between dental trauma and the initial attendance to the development of external inflammatory root resorption in permanent teeth affected by severe luxation. Sixty-seven patients, aged between 11 and 56 years, presenting 133 injured teeth with closed apex (56 extrusive luxation, 69 lateral luxation and 8 intrusive luxation) were followed up for a minimum of 24 months. The time elapsed between dental trauma and the initial attendance was recorded. The presence of necrotic pulp and external inflammatory resorption for each type of trauma was verified. Fisher’s exact test was used to verify the influence of the initial attendance date at the Dental Trauma Center and the development of inflammatory resorption (p<0.05). The odds ratio was used to check the risk of developing external inflammatory resorption due to delay in seeking treatment. Pulp necrosis was observed in 105 teeth (78.9%) and external inflammatory resorption was detected in 17.8% cases of extrusive luxation (10 teeth), 15.9% of lateral luxation (11 teeth) and 25% of intrusive luxation (2 teeth). For lateral luxation, extended delay between the date of injury and initial attendance influenced the occurrence of external inflammatory resorption (p=0.0174). Patients who began treatment 45 days after the injury were 3.4 times more likely to develop external inflammatory resorption than patients who sought treatment after the trauma. Treatment late after the occurrence of dental trauma can impair the prognosis and result in the development of external inflammatory resorption in luxated teeth.

Resumo O objetivo do presente estudo foi avaliar os efeitos citotóxicos de cinco cimentos endodônticos (AH Plus, Endomethasone N, EndoSequence BC, MTA Fillapex e Pulp Canal Sealer EWT) utilizando um modelo de cultura celular tridimensional (3D). Utilizou-se um modelo convencional de cultura de células bidimensionais (2D) como técnica de referência para comparação. Os fibroblastos Balb/c 3T3 foram cultivados em culturas de células bidimensionais convencionais e em modelos de cultura de células tridimensionais de colagéno de cauda de rato do tipo I. Em seguida, ambas as culturas de células foram incubadas com eluções dos cimentos endodônticos recém manipulados, durante 24 h. A viabilidade celular foi medida pelo ensaio de MTT. Os dados foram analisados estatisticamente utilizando ANOVA e o teste de Tukey com nível de significância de p<0,05. Todos os cimentos testados exibiram efeitos citotóxicos. Contudo, o efeito citotóxico foi dependente do modelo de cultura e do cimento testado. Os cimentos apresentaram maior citotoxicidade no modelo 2D do que no modelo 3D (p<0,05). Em ambas as condições, a EndoSequence BC apresentou a menor citotoxicidade (p<0,05). MTA Fillapex foi mais citotóxico do que os outros cimentos endodônticos testados (p<0,05), com exceção do AH Plus no modelo de cultura de células 2D (p>0,05). Endomethasone N e EWT mostraram efeitos citotóxicos mais baixos do que AH Plus no modelo de cultura de células 2D (p<0,05); entretanto, não houve diferenças estatísticas entre esses cimentos no modelo de cultura de células 3D. Pode concluir-se que a citotoxicidade foi maior na cultura de células 2D em comparação com a cultura de células 3D. EndoSequence BC selante exibiu a maior citocompatibilidade e MTA Fillapex a menor citocompatibilidade.

Abstract The aim of the present study was to evaluate the cytotoxic effects of five endodontic sealers (AH Plus, Endomethasone N, EndoSequence BC, MTA Fillapex and Pulp Canal Sealer EWT) using a three-dimensional (3D) cell culture model. A conventional bi-dimensional (2D) cell culture model was used as reference technique for comparison. Balb/c 3T3 fibroblasts were cultured in conventional bi-dimensional cell culture and in rat-tail collagen type I three-dimensional cell culture models. Then, both cell cultures were incubated with elutes of freshly mixed endodontic sealers for 24 h. Cell viability was measured by the methyl-thiazol-diphenyltetrazolium assay (MTT). Data were statistically analyzed using ANOVA and the Tukey test at a significance level of p<0.05. All tested sealers exhibited cytotoxic effects; however, cytotoxic effect was culture model- and sealer-dependent. Sealers showed higher cytotoxicity in 2D than in 3D cell culture model (p<0.05). In both conditions, EndoSequence BC showed the lowest cytotoxicity (p<0.05). MTA Fillapex was much more cytotoxic than the other tested endodontic sealers (p<0.05), with the exception of AH Plus in the 2D cell culture model (p>0.05). Endomethasone N and Pulp Canal Sealer EWT showed lower cytotoxic effects than AH Plus in 2D cell culture model (p<0.05); however no statistical differences was observed among these sealers in 3D cell culture model. It may be concluded that cytotoxicity was higher in 2D cell culture compared to 3D cell culture. EndoSequence BC sealer exhibited the highest cytocompatibility and MTA Fillapex the lowest cytocompatibility.

Resumo O objetivo deste estudo foi avaliar a relação de causa-efeito entre o preparo do canal radicular com o sistema ProTaper Universal (PTU) e a formação de defeitos dentinários usando a análise por micro-tomografia computadorizada (micro-CT). Quarenta raízes mesiais de molares inferiores com a configuração tipo II de Vertucci foram escaneadas com resolução isotrópica de 14.16 μm. A amostra foi distribuída em um grupo experimental (n = 30) e um grupo controle (n = 10), e os canais mesiais foram preparados com o sistema PTU até a lima F2. As amostras do grupo experimental foram escaneadas e as imagens de secção transversal das raízes mesiais, antes e após o preparo, foram analisadas para identificar a presença de defeitos dentinários. No grupo controle, os dentes foram seccionados perpendicularmente em relação ao longo eixo da raiz em fatias de 1 mm de espessura (n = 80) e examinados ao microscópio óptico. Uma vez detectado um defeito dentinário, a fatia foi escaneada através da micro-CT. No grupo experimental, defeitos dentinários foram observados em 4.828 secções transversais (24,04%). Em todas as imagens de secção transversal, os defeitos dentinários após os procedimentos experimentais já estavam presentes na secção pré-operatória correspondente. No grupo controle, 13 das 80 fatias (16,25%) apresentaram pelo menos um defeito dentinário visualizado através do microscópio, o qual foi identificado em um escaneamento posterior. A micro-CT mostrou confiabilidade com a microscopia óptica para detecção e visualização dos defeitos dentinários, permitindo acompanhar o tecido dentinário antes e depois do preparo do canal. O preparo do canal radicular com o sistema PTU não induziu a formação de novos defeitos dentinários.

Abstract This study aimed to evaluate the cause-effect relationship between canal preparation with ProTaper Universal (PTU) system and dentinal defects formation using micro-computed tomography (micro-CT) analysis. Forty mesial canals of mandibular molars with a type II Vertucci's canal configuration were scanned at an isotropic resolution of 14.16 µm. The sample was assigned to an experimental (n = 30) and a control (n = 10) groups, and the mesial canals were prepared with PTU system up to F2 instrument. The specimens from the experimental group were scanned and the cross-section images of the mesial roots, before and after preparation, were screened to identify the presence of dentinal defects. In the control group, the specimens were sectioned perpendicularly to the long axis of the root into 1-mm-thick slices (n = 80) and examined under optical microscope. Once a dentinal defect was detected, the slice was scanned through micro-CT. In the experimental group, dentinal micro-cracks were observed in 4,828 slices (24.04%). In all cross-section images, dentinal defects identified in the postoperative images were already present in the corresponding preoperative image. In the control group, 13 out of 80 slices (16.25%) had at least one dentinal defect visualized under stereomicroscopy, which was identified after a further micro-CT scanning. Micro-CT showed reliability as similar as optical microscopy in detecting dentinal defects, adding the possibility of tracking the dentinal tissue, before and after canal preparation, and providing a clear visualization of micro-cracks. Root canal preparation with PTU system did not induce the formation of new dentinal defects.
