RESUMO Este trabalho pretende verificar o efeito imediato e em médio prazo na voz e na laringe de dois idosos submetidos à terapia vocal intensiva com progressão de intensidade e frequência vocais e de duração do tempo de fonação. Dois idosos (um homem, 79 anos e uma mulher, 82 anos) com queixa vocal e características de presbilaringe, realizaram 12 sessões de terapia vocal intensiva com progressão de intensidade e frequência da voz e duração do tempo de fonação, durante três semanas. Para analisar o efeito terapêutico foram realizadas avaliações perceptivoauditivas e acústicas da voz, de tempo máximo de fonação (TMF), de autorreferência da qualidade de vida em voz e do comportamento laríngeo nos momentos pré, imediatamente após e um mês depois do processo de terapia vocal. Os valores da maioria das medidas resultantes dessas avaliações indicaram mudanças positivas imediatamente após a terapia vocal para os idosos. Observou-se redução das medidas perceptivoauditivas de desvio da qualidade vocal e diminuição das medidas de perturbação e ruído do sinal acústico, o que indica melhora na voz. Houve elevação da frequência fundamental e, aumento do TMF, além de autorreferência de melhor qualidade de vida em voz. A avaliação das imagens laríngeas não mostrou diferença consistente. Após um mês do término da terapia alguns parâmetros pioraram em relação ao momento pós-imediato, mesmo assim, permaneceram melhores em relação ao momento pré-terapia. Desta forma os resultados da proposta terapêutica são promissores e seus efeitos devem ser pesquisados em estudos clínicos controlados para verificar sua eficácia em idosos.

ABSTRACT This study aims to verify the immediate and medium-term effects of an intensive voice therapy, with progression of vocal intensity and frequency and phonation time, on the voice and larynx of two elderly. A 79-year-old male and an 82-year-old female with vocal complaints and presbylarynx characteristics underwent 12 sessions of intensive voice therapy, with progression of voice intensity and frequency and phonation time, for three weeks. To analyze the therapeutic effects, the following assessments were performed pre-, post-immediately, and one month after voice therapy: auditory-perceptual analyses of the voice, acoustic analysis, and evaluation of maximum phonation time (MPT), self-reference voice-related quality of life, and laryngeal behavior. Most results of these measurements indicated positive changes immediately after voice therapy. There was reduction in the measures of vocal quality deviation, perturbation, and harmonics-to-noise pre-, post-immediately, and one month after voice therapy, which indicates vocal improvement. There was increase in fundamental frequency, maximum phonation time and self-reference voice-related quality of life. Assessment of the laryngeal images showed no consistent difference. One month after voice therapy, worsening of some results was observed compared with the post-immediate assessment, but improvements were maintained in relation to the initial evaluation. The results of this therapeutic proposal are promising, and their effects should be investigated in controlled clinical trials to verify their efficacy.

RESUMO Objetivo Comparar parâmetros vocais e acústicos de crianças infectadas e não infectadas pelo HIV (Vírus da Imunodeficiência Humana). Método Amostras vocais foram submetidas às análises perceptivo-auditiva e acústica. Foram analisadas amostras da vogal sustentada /ε/ e da fala encadeada de 74 crianças entre seis e 12 anos incompletos, divididas em dois grupos: 37 crianças infectadas pelo HIV (GHIV) e 37 crianças não infectadas pelo HIV (Grupo Controle=GC), pareadas por idade e gênero, e sem avaliação vocal prévia; todas as crianças eram pré-púberes, pelos Critérios de Tanner (MS, 2014). As crianças tiveram suas vozes gravadas e analisadas pelo programa VoxMetria 5.1, no período de 2014 a 2015. A análise perceptivo-auditiva avaliou o grau geral da qualidade vocal e foi realizada utilizando-se a escala analógico-visual (EAV) de 100 pontos, transformada em escala numérica de 4 pontos (0- ausência de desvio e 4- desvio intenso). A avaliação acústica constou da análise da distribuição da amostra vocal no Diagrama de Desvio Fonatório (DDF). A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa em Seres Humanos, sob o número 122.746. Resultados Na análise perceptivo-auditiva, a maioria das crianças de ambos os grupos foi avaliada como apresentando vozes sem desvio, grau 0 na escala numérica. Na análise acústica, não houve diferença entre os grupos na distribuição das amostras no DDF, com a maioria das amostras na área de normalidade, no primeiro quadrante, com distribuição de densidade ampliada e forma vertical. Conclusão Crianças infectadas pelo HIV apresentaram vozes semelhantes às de crianças sem a doença, tanto do ponto de vista auditivo quanto acústico.

ABSTRACT Purpose To compare vocal and acoustic parameters of HIV-infected children and non-HIV-infected children. Methods Vocal samples were submitted to auditory-perceptual and acoustic analysis. Samples of the sustained vowel /ε/ and continuous speech of 74 children between 6 and incomplete 12 years old were analyzed, divided into two groups: 37 HIV-infected children (GHIV) and 37 non-HIV-infected children, the control group (CG), age and gender-matched and without previous vocal evaluation; they were all prepubescent by the Tanner Stages (MS, 2014). The children had their voices recorded and analyzed by VoxMetria 5.1, in the period between 2014 and 2015. The auditory-perceptual analysis assessed the overall degree of the vocal quality and was performed using a 100-point visual analogue scale, transformed into a 4 point numerical scale (0 = no vocal deviation and 4 = severe vocal deviation). The acoustic evaluation was based on the analysis of the vocal sample distribution in the Phonatory Deviation Diagram (PDD). The research was approved by the Ethics Committee under the number 122.746. Results In the auditory-perceptual analysis, most children of both groups were evaluated as with no vocal deviation. No difference between the groups was found in the acoustic analysis using the PDD; most voice samples were within the normality area, in the quadrant 1, with a spread density distribution and a vertical shape. Conclusion HIV-infected children presented similar vocal quality to children without the illness, both for the perceptual-auditory and acoustic evaluation.

RESUMO Este estudo teve como tema avaliar o efeito do sistema de campo livre dinâmico, em um período do dia, nos sintomas vocais e na qualidade vocal de uma professora em sala de aula. Relato de caso clínico de uma professora de 44 anos, que atuava na função há 23 anos, e lecionava na mesma sala no período matutino e vespertino. A professora utilizou um sistema de amplificação em campo livre dinâmico durante um dos períodos do dia por aproximadamente três meses. Foi realizada avaliação diária da ocorrência dos sintomas no trato vocal, e da intensidade deles antes e após a aula; análise perceptivo-auditiva e acústica da voz antes e após o período de observação, e aplicação de um questionário de autopercepção sobre o uso do sistema de campo livre dinâmico, ao final do período de observação. Os dados foram analisados descritivamente e por meio do teste de Wilcoxon (p=0,05). Observou-se diminuição da intensidade do sintoma "secura" ao final da aula, e menor ocorrência de sintomas vocais com a utilização do sistema de campo livre dinâmico, além de melhora em parâmetros perceptivo-auditivos e acústicos da voz após o período de observação, e do relato de pontos positivos pela professora quanto ao uso de sistema de campo livre dinâmico. No caso estudado, o uso do sistema de campo livre dinâmico, em um dos períodos do dia, foi efetivo na diminuição dos sintomas e na melhora na qualidade vocal perceptivo-auditiva e acústica.

ABSTRACT The theme of this study was to evaluate the effect of the dynamic soundfield system in one period of the day, the teacher's vocal symptoms and voice quality in the classroom. A case relate of a teacher of 44 years who served in the role for 23 years, and taught in the same room in the morning and afternoon. The teacher used the dynamic soundfield system during one of the periods of the day for about three months. Daily evaluation was carried out of the occurrence of symptoms in the vocal tract, and their intensity before and after school; perceptual and acoustic analysis of voice before and after the observation period, and applying a self-perception questionnaire about the use of dynamic open field system, the end of the observation period. The data were analyzed descriptive and by the Wilcoxon test (p=0.05). There was a decrease in the intensity of the symptom "dryness" to the end of class, and lower incidence of vocal symptoms with the use of dynamic soundfield system, as well as improvement in perceptual and acoustic parameters of the voice after a period of observation and reporting of positives by the teacher on the use of dynamic soundfield system. In the case study, the use of dynamic soundfield system, one of the periods of the day, was effective in reducing symptoms and improvement in perceptual voice quality and acoustics.

RESUMO Objetivo: Determinar os valores de corte dos diferentes graus de rugosidade e soprosidade vocal em uma escala visual analógica, a partir de uma escala numérica. Métodos: Foram selecionadas 150 vozes, de indivíduos de ambos os gêneros, com qualidade variando desde a ausência de rugosidade e de soprosidade até a presença desses parâmetros em grau intenso. A avaliação perceptivo-auditiva foi realizada por quatro fonoaudiólogas especialistas em voz, com a utilização da escala visual analógica de 100 mm e da escala numérica de quatro pontos. A concordância intra e interjuízes foi verificada por meio do Coeficiente de Correlação Interclasse. Os valores de corte foram obtidos com base nos valores de sensibilidade, especificidade e eficiência. Resultados: A concordância intra e interjuízes foi excelente em ambas as escalas. Os valores de corte definiram as faixas de distribuição, encontrando-se para a rugosidade: grau 0 até 8,5 mm; grau 1 de 8,5 a 28,5 mm; grau 2 de 28,5 a 59,5 mm; e grau 3 a partir de 59,5 mm. Para a soprosidade encontrou-se: grau 0 até 8,5 mm; grau 1 de 8,5 a 33,5 mm; grau 2 de 33,5 a 52,5 mm; e grau 3 a partir de 52,5 mm. Conclusão: A partir dos valores de corte encontrados para a escala visual analógica foi possível concluir que o grau 0 (zero) da escala numérica, que representa a ausência do parâmetro, corresponde a uma pequena faixa de pontuação de presença do parâmetro na escala visual analógica. Além disso, o grau 3 da escala numérica correspondeu a uma extensa faixa da escala visual analógica.

ABSTRACT Purpose: To determine the cut-off values of different degrees of vocal roughness and breathiness in an analogical visual scale, from a numerical scale. Methods: We selected 150 voices from both genders, with quality ranging from neutrality to intense deviations, and predominance of breathiness or roughness. The auditory-perceptual assessment through two scales: 100-mm analogical visual scales and 4-point numerical scale. Intra and interrater reliability of auditory-perceptual ratings was analyzed using the Intraclass Correlation Coefficient. The cut-off values for graduation of roughness and breathiness voices were determinate using the analysis of the Receiver Operating Characteristic (ROC) curve, which is based on the sensitivity, specificity and efficiency. Results: The intra and interrater reliability of auditory-perceptual ratings was excellent on both scales. The cut-off values (mm) in the analogical visual scale, from the numeric scale, indicated a roughness distribution: 0-8.5 absence of the parameter; 8.5- 28.5 mild presence; 28.5- 59.5 moderate and 59.5- 100 intense. For breathiness, the distribution was: 0-8.5 absence of the parameter; 8.5-33.5 for mild presence; 33.5-52.5 moderate and 52.5-100 intense. Conclusion: The cut-off values found for the analogical visual scale conclude that the numerical scale zero, which represents the absence of parameter, corresponds to a small range of scores in the analogical visual scale. Furthermore, the third degree of the numerical scale corresponded to a wide range of the analogical visual scale.

Resumo: Este trabalho tem como objetivo apresentar revisão integrativa de literatura sobre a aplicabilidade e o resultado do uso da eletroestimulação na prática clínica fonoaudiológica. Foram seguidos os preceitos do Cochrane Handbook, que envolveu a formulação da questão a ser investigada, localização e seleção dos estudos e avaliação crítica dos artigos. Foram utilizadas as bases de dados Medical Literature Analysis and Retrieval Sistem on-line (Medline), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), PubMed e Web of Science/ISI. Os descritores utilizados foram: "estimulação elétrica nervosa transcutânea", "estimulação elétrica", "disfagia", "transtornos de deglutição", "disfonia", "distúrbios da voz", "treinamento da voz" e "terapia por estimulação elétrica" em inglês, português e espanhol e suas combinações, no período entre 2003 e 2013. Os estudos analisados demonstraram que a eletroestimulação traz benefícios na reabilitação de pacientes na clínica fonoaudiológica, mas a metodologia utilizada nos estudos foi divergente e a população estudada muito heterogênea o que dificulta sua utilização clínica pelos profissionais da área. A eletroestimulação traz benefícios na reabilitação fonoaudiológica, porém novos estudos devem ser realizados utilizando uma amostra mais homogênea e descrevendo metodologia e técnicas fonoaudiológicas utilizadas nos procedimentos, a fim de comprovar seus resultados e viabilizar seu uso pelos profissionais da área.

Abstract: The purpose of this study is to present the integrative literature review: about the applying result of the electrical stimulation use in the speech therapy clinic. The methodology used followed the concepts of the Cochrane Handbook involved question formulation related to the topic of investigation, identification and selection of the studies, and a critical evaluation of the selected articles. Selection of articles data bases of Medical Literature Analysis and Retrieval System on-line(Medline), Latin American Literature and the Caribe in Health Science, PubMed and Web of Science/ISI were used. The descriptors used were "transcutaneous nervous electrical stimulation", "dysphagia", "swallowing disorder", "dysphonia", "voice disorder" AND "speech language" in English, Portuguese and Spanish and its combinations, from 2003 to 2013. The electrical stimulation brings benefits the rehabilitation in speech therapy, but the methodology used in the studies was divergent and the studied population was very heterogeneous and it makes difficult its clinical use by the professionals of the area. The electrical stimulation brings benefits the rehabilitation in speech therapy clinic. But the new studies should be carried out using a more homogeneous sample and describing the methodology and the speech therapy techniques used into procedures, in order to prove its results and make its use feasible to the professionals of the area.

OBJETIVO: conhecer a auto-imagem, dificuldades e presença de sintomas negativos após o canto em coralistas amadores com diferentes classificações vocais, idades e experiência. MÉTODO: cento e vinte e cinco cantores responderam a um questionário que abordou dados de identificação, informações sobre a auto-imagem da voz cantada, dificuldades apresentadas no canto e ocorrência ou não de sintomas vocais após o canto. RESULTADOS: a comparação por naipes evidenciou maior dificuldade na emissão de sons agudos para os contraltos e baixos, maior dificuldade na emissão graves para os sopranos, maior dificuldade na transição de registro grave para agudo para os baixos, e maior cansaço vocal para os contraltos. Na divisão por idade, tanto adultos jovens e adultos referiram maior soprosidade do que os idosos. Os adultos referiram melhor intensidade vocal do que adultos jovens. Os adultos jovens consideraram seu timbre de voz adequado mais frequentemente do que adultos. Em relação à experiência, os cantores menos experientes referiram percepção de voz rouca em maior número do que os cantores mais experientes, que referiram apresentar intensidade adequada durante o canto em maior número do que os demais. Cantores menos experientes referiram maior ocorrência de rouquidão após o canto do que cantores mais experientes. CONCLUSÃO: as dificuldades encontradas no canto estão atreladas ao naipe, e não dependem da idade e nem da experiência. Sintomas estão relacionados ao naipe e ao grau de experiência com o canto. A auto-imagem vocal negativa também está relacionada ao naipe e ao nível de experiência, sendo que a auto-imagem positiva é mais comum em cantores experientes.

PURPOSE: to evaluate vocal self-perception, difficulties and presence of negative symptoms after singing of amateur choir singers of different vocal classifications, age and experience. METHOD: one hundred and twenty five singers answered a questionnaire containing identification data, information about self-perception of the singing voice, difficulties with singing and negative symptoms after singing. RESULTS: the comparison considering vocal classification evidenced greater difficulties with high notes for altos and basses, greater difficulty regarding the transition to high notes for basses and greater vocal fatigue for altos. Comparing the singers by age, both adults and young adults referred more breathiness than the elderly. The adults referred better vocal intensity than the young adults. The young adults referred better timbre than adults. Regarding the experience, the less experienced singers reported self-perception of hoarseness and presence of hoarseness after singing in greater number than the experienced singers. CONCLUSION: the difficulties with singing are connected to the vocal classification and do not depend on age or experience. Vocal symptoms are related to the vocal classification and to the experience with singing. Negative self-perception is also related the vocal classification and to the experience with singing, and positive self-perception was more reported by experienced singers.

Estudar os distúrbios da mastigação e deglutição na Disfunção Temporomandibular (DTM). OBJETIVO: Investigar as disfunções de mastigação e deglutição na DTM em grau grave, com indicação cirúrgica. MATERIAIS E MÉTODOS: O estudo clínico e experimental verificou dez indivíduos com DTM, submetidos à videofluoroscopia da deglutição. Verificou-se ausência de dentes posteriores, dor em músculos da mastigação e substituições de alimentos, privilegiando-se consistências pastosas. A avaliação das fases oral e faríngea abordou os seguintes aspectos: lado de início e lado preferencial para mastigação, escape prematuro, permanência de resíduos alimentares na cavidade oral e recessos faríngeos, número de deglutições necessárias, presença de penetração laríngea e/ou aspiração traqueal. RESULTADOS: Durante a mastigação e fase oral, observaram-se movimentos compensatórios de língua à mastigação (n = 7; 70%), escape prematuro (n = 4; 40%), resíduos na cavidade após a deglutição (n = 5; 50%) e número excessivo de deglutições (n = 5; 50%). Na fase faríngea, observou-se permanência de resíduos em valécula (n = 6; 60%), resíduos em seios piriformes (n = 4; 40%); penetração laríngea (n = 1; 10%) e aspiração traqueal (n = 4; 40%). CONCLUSÃO: Pacientes com DTM podem apresentar alterações nos padrões de mastigação e deglutição, com penetração laríngea e/ou aspiração traqueal. O estudo indica a necessidade de avaliação multidisciplinar devido à presença de disfagia em pacientes com DTM.

To study mastication and swallowing disorders in patients with temporomandibular disorders (TMD). OBJECTIVE: To investigate mastication and swallowing disorders in patients with severe TMD referred to surgery. MATERIALS AND METHODS: Clinical and experimental study involving ten individuals with TMD submitted to deglutition videofluoroscopy. These patients did not have posterior teeth, mastication pain and food replacement in favor of pasty consistence food. The assessment of the oral and pharyngeal phases approached the following aspects: side of onset and preferential side for chewing, premature escape, remains of food residues in the oral cavity or in the pharyngeal recesses, number of necessary swallowing efforts, laryngeal penetration and/or tracheal aspiration. RESULTS: During mastication and the oral phase we observed tongue compensatory movements upon chewing (n = 7; 70%), premature escape (n = 4; 40%), food remains in the cavity after swallowing (n = 5; 50%) and an excessive number of deglutition efforts (n = 5; 50%). On the pharyngeal phase we observed food remains in the valleculae (n = 6; 60%), in the pyriform sinuses (n = 4; 40%); laryngeal penetration (n = 1; 10%) and tracheal aspiration (n = 4; 40%). CONCLUSION: TMD patients may have alterations in their chewing and swallowing patterns, with laryngeal penetration and/or tracheal aspiration. The study indicates the need for a multidisciplinary assessment because of dysphagia in TMD patients.

OBJETIVO: verificar hábitos prejudiciais à voz referidos por crianças disfônicas e por seus respectivos pais e mães e compará-los com dados obtidos de um grupo controle, constituído por crianças sem alterações vocais e seus respectivos pais e mães. MÉTODO: investigou-se 28 crianças disfônicas, com idade entre 6 e 12 anos e seus pais (Grupo de Estudo - GE) e 22 crianças sem alterações vocais, com a mesma faixa etária, e seus respectivos pais (Grupo Controle - GC). As vozes foram classificadas em "alteradas" e "não alteradas" por meio de análise perceptivo-auditiva com amostra da fala espontânea.Todos responderam a um questionário sobre fatores prejudiciais à voz. Para análise dos resultados foram aplicados os testes de comparação de duas proporções (p<0,05). RESULTADOS: verificou-se que as crianças do GE apresentaram significantemente mais os hábitos: falar com esforço, falar sem descansar e imitar vozes. Os pais do GE relataram significantemente mais hábitos de: pigarrear, gritar, falar junto com os outros, além de viver em ambiente de fumantes. As mães do GE apresentaram significantemente mais hábitos de: falar com esforço, falar em ambiente ruidoso e falar muito rápido. Viver em ambiente familiar ruidoso foi significantemente mais relatado por crianças, pais e mães do GE quando comparados aos do GC. CONCLUSÃO: independentemente de fatores que possam justificar a disfonia, crianças disfônicas e seus pais e mães relataram maior ocorrência de hábitos prejudiciais à voz e viver em ambiente familiar ruidoso do que crianças sem alterações vocais e seus respectivos pais e mães.

PURPOSE: to investigate harmful vocal habits referred by dysphonic children and their parents and compare them with data gathered from a control group made up by children with no vocal alterations and their parents. METHOD: twenty-eight dysphonic children, with ages between 6 and 12 year old and their parents, (Study Group - SG) along with 22 children with no vocal alterations, from the same age group, and their parents (Control Group - CG) were investigated. The voices were classified as "healthy" and "unhealthy", depending on the result obtained from the percentage analysis of a spontaneous speech sample. Subjects answered a questionnaire about harmful vocal habits. Tests for comparing two proportions (p <0.05) were used in order to analyze the results. RESULTS: SG children showed a significantly higher number of habits such as speaking with effort, speaking without resting and imitating voices. SG fathers showed a significantly higher number of habits such as clearing the throat, shouting, speaking at the same time as others, in addition to living in smokers' environment. SG mothers significantly showed more habits such as speaking with effort, speaking in noisy environments and speaking too quickly. Living in noisy familiar environments was significantly reported by children, fathers and mothers of SG when compared to those of the CG. CONCLUSION: despite the factors that explain dysphonia, dysphonic children as well as their fathers and mothers reported a higher incidence of harmful vocal habits and living in noisy familiar environments than children with no vocal alterations and their parents.

OBJETIVO: avaliar a relação do grau de escolaridade, faixa etária e a profissão das mães com a oferta de chupeta e mamadeira a seus filhos MÉTODO: participaram deste estudo 190 mães de crianças entre 0 e 10 anos de idade, com idades de 20 a 45 anos que responderam a um questionário com questões fechadas a respeito de profissão, nível de escolaridade e uso de chupeta e mamadeira de seus filhos RESULTADOS: não foi observada diferença significante entre o grau de escolaridade quanto ao uso de chupeta (p= 0,915). O uso de mamadeira foi significantemente maior nos filhos de mães com ensino superior (p= 0,0124). Não houve diferença significante quanto à faixa etária das mães e a presença desses hábitos nos filhos (chupeta: p=0,427; mamadeira: p=0,427); e, entre mães que trabalham em casa e que trabalham fora de casa (chupeta: p=0,061; mamadeira: p=0,798). Não houve diferença entre mães que trabalham na área da saúde e em outras áreas (chupeta: p=0,339; mamadeira: p=0,455) CONCLUSÃO: as mães com nível superior de escolaridade oferecem significantemente mais mamadeira aos seus filhos, o que não ocorreu quanto ao uso de chupeta. Não foi observada diferença significante quanto ao uso de chupeta e mamadeira em relação à faixa etária e profissão de mães.

PURPOSE: to evaluate the relationships among schooling level, age group and mother profession with offer of pacifier and baby's bottles to your children. METHOD: 190 mothers with ages varying from 20 to 45 having children from 0 to 10 took part in this study. Mothers answered a questionnaire with questions regarding profession, schooling level and use of nipples and baby's bottles by their children. RESULTS: no significant differences were found between mothers having primary or secondary education and mothers with higher education regarding the use of nipples (p= 0.915). On the other hand, baby's bottle use was significantly higher among children of mothers with higher education (p= 0.0124). No significant difference regarding age group of mothers and the presence of these habits in children or between mothers who work in the house and those who work outside as regards the use of nipples (p=0.427; p=0.061) and baby's bottles (p=0.427; p=0.798). We also observed no difference among mothers who work in the health area and those working in other fields (nipples: p=0.339; baby's bottles: p=0.455). CONCLUSION: mothers with higher education offer significantly more baby's bottles to their children, but not nipples. No significant difference was observed as for the use of nipples and baby's bottles as related to age group and mothers' profession.

OBJETIVO: verificar o efeito imediato das técnicas vocais vibração, som nasal e sobrearticulação na voz e na laringe de mulheres sem queixas vocais. MÉTODO: participaram da pesquisa 32 sujeitos do sexo feminino, com idades entre 20 e 45 anos, sem queixas vocais, com qualidade vocal avaliada entre normal e alteração de grau leve Os sujeitos foram submetidos à análise perceptivo-auditiva pela escala visual analógica da vogal /ε/ e fala espontânea, análise acústica e laringoestroboscopia antes e após a realização das técnicas. RESULTADOS: a análise perceptivo-auditiva revelou melhora significante dos parâmetros impressão global da voz, rouquidão e estabilidade na vogal /ε/ e articulação na fala espontânea. A análise acústica evidenciou melhora significante do jitter e shimmer. A laringoestroboscopia evidenciou significante melhora no fechamento glótico e melhora na movimentação muco-ondulatória das pregas vocais. CONCLUSÃO: as técnicas vocais estudadas são capazes de proporcionar melhora imediata significante da qualidade vocal e da configuração laríngea.

PURPOSE: to check the immediate effect of vocal techniques: vibration, nasal sound and overarticulation. METHOD: 32 female subjects with normal to mild dysphonia took part in the study, with ages from 20 to 45 years. Subjects were submitted to perceptual analysis and laryngostroboscopic exams before and after the use of vocal techniques. RESULTS: subjects' vocal classification in perceptual analysis after accomplishing the vocal techniques showed significant improvement on parameters voice global impression, hoarseness and stability; and, in spontaneous speech, one showed a significant improvement on the parameter articulation. The acoustic analysis evidenced significant improvement of the jitter and shimmer. Laryngostroboscopic examination evidenced a significant increase in the glottic closing and an increase in the mucondulatory movement of the vocal folds. The results found in perceptual evaluation are compatible with some studies in the literature on the effect concerning the application of vocal techniques. However, laryngological findings identified significant improvements that are rarely described in the literature. CONCLUSION: the studied vocal techniques are capable to provide significant immediate improvement of vocal quality and laryngeal configuration.

OBJETIVO: Este estudo transversal e prospectivo objetivou avaliar qualidade vocal, respiração e região cervicoescapular em instrumentistas de sopro integrantes da Banda Musical do Exército. MÉTODOS: Foram estudados 42 sujeitos do gênero masculino, militares, com idades entre 20 e 45 anos, divididos em: Grupo Experimental, com 30 instrumentistas de sopro, e Grupo Controle, com 12 instrumentistas de percussão. Todos passaram por entrevista; avaliação vocal - registro da voz e análise perceptivo-auditiva com uso da escala GRBASI; avaliação do tipo respiratório; avaliação fisioterapêutica da região cervicoescapular - inspeção visual dos perfis anterior, lateral e posterior, palpação dos músculos trapézio fibras-superiores, esternocleidomastoídeos, escalenos e base de occipital; e testes de mobilidade cervical e de encurtamentos musculares. RESULTADOS: Não houve diferença entre Grupo Experimental e Controle quanto à presença de queixas vocais; os sujeitos do Grupo Experimental apresentaram mais sintomas laríngeos e relataram mais dor em músculo trapézio e região cervical quando comparados aos sujeitos do Controle. Os sujeitos do Grupo Experimental apresentaram maior alteração nos parâmetros vocais tensão e instabilidade do que os sujeitos do Grupo Controle. Constatou-se maior incidência da respiração costodiafragmático-abdominal no Grupo Experimental. Não houve diferença entre os grupos estudados quanto à postura, encurtamentos musculares e mobilidade cervical; porém o Grupo Experimental apresentou mais dor à palpação em músculo trapézio - fibras superiores. CONCLUSÃO: Os militares instrumentistas de sopro apresentaram alterações vocais e da musculatura cervicoescapular e necessitam de ações de promoção à saúde vocal e postural.

PURPOSE: This cross-sectional prospective study had the aim to evaluate vocal quality, breathing and the cervicoscapular region in wind instrumentalists of the Army Band. METHODS: Forty-two male military subjects with ages ranging from 20 to 45 years were divided into: Experimental Group, with 30 wind instrumentalists, and Control Group, with 12 percussionists. All subjects underwent an interview, vocal evaluation - voice record and auditory-perceptual analysis using the GRBASI scale, respiratory type evaluation, physical therapy evaluation of the cervicoscapular region - visual examination of anterior, lateral and posterior profiles, palpation of the trapezius, sternocleidomastoid, scalene and base of occipital muscles, and cervical mobility and muscle shortenings tests. RESULTS: No difference was found between Experimental and Control groups regarding the presence of vocal complaints; subjects from the Experimental Group had more laryngeal symptoms and reported more pain in the trapezius muscle and the cervical region, when compared to control subjects. They also had greater alterations in vocal tension and instability parameters than subjects from the Control Group. There was a higher incidence of costodiaphragmatic-abdominal breathing among the Experimental Group subjects. There was no difference between the groups regarding posture, muscle shortenings, and cervical mobility; however, the Experimental Group presented more pain during the palpation of the trapezius muscle. CONCLUSION: The military wind instrumentalists presented vocal and cervicoscapular alterations, and require actions to promote vocal and postural health.

OBJETIVO: Analisar a postura e a função da região crânio-cervical em sujeitos disfônicos. MÉTODOS: Participaram do estudo 28 mulheres (31,25±8,14 anos), divididas em dois grupos: experimental (N=16 portadoras de disfonia) e controle (N=12 clinicamente normais). As voluntárias foram submetidas à avaliação do Índice de Disfunção Crânio-Cervical (IDCC) e fotogrametria, sendo determinado o ângulo anterior formado entre a sétima vértebra cervical e o tragus, o qual corresponde à posição da cabeça no plano sagital. A análise das fotos foi realizada por três examinadores, duas vezes cada um, com intervalo de uma semana entre elas. A análise dos dados constou do teste de Shapiro-Wilk, seguido do teste t de Student, (p<0,05). RESULTADOS: Em relação à fotogrametria, não houve diferença (p=0,2565) entre os valores médios do ângulo anterior do grupo controle (50,92±5,18 graus) e do grupo experimental (49,63±5,46 graus). O IDCC mostrou que o grupo experimental apresentou disfunção crânio-cervical, sendo 37,5 % leve, 37,5% moderada e 25% severa. Já no grupo controle 100% das voluntárias apresentaram disfunção crânio-cervical leve. CONCLUSÃO: Não houve diferença na posição da cabeça entre os grupos avaliados. Porém, as mulheres disfônicas apresentaram disfunção crânio-cervical mais acentuada que as do grupo controle.

Purpose: To analyze the posture and the function of the craniocervical region in women with dysphonia. METHODS: Twenty eight women participated in the study (31.25±8.14 years), divided into two groups: experimental (N=16, patients with dyphonia) and control (N=12, clinically normal). The volunteers were submitted to evaluation of the Craniocervical Dysfunction Index (CDI) and photogrammetry, determining the anterior angle formed between the seventh cervical vertebra and the tragus, which corresponds to the head position in the sagittal plane. The analysis of the photos was carried out by three examiners, twice each, with an interval of one week between them. Data analysis used the Shapiro-Wilk test, followed by the Student's t-test, (p<0,05). RESULTS: No difference was found between the mean values of the anterior angle of the control (50.92±5.18 degrees) and the experimental (49.63±5.46 degrees) groups in the photogrammetry (p=0.2565). The CDI showed that the experimental group had craniocervical dysfunction, which was mild in 37.5% of the cases, moderate, in 37.5%, and severe in 25%. In the control group, 100% of the volunteers had presented mild craniocervical dysfunction. CONCLUSION: There was no difference between the evaluated groups regarding head position. However, dysphonic women presented more severe craniocervical dysfunction than the group control.

TEMA: estudos mostram correlação entre disfonia e tensão muscular. OBJETIVO: avaliar a atividade elétrica dos músculos supra-hióideos (SH), esternocleidomastóideo (ECM) e trapézio (T) bilateralmente, a dor e a voz, após aplicação da estimulação elétrica nervosa transcutânea (TENS). MÉTODO: participaram dez mulheres com nódulos ou espessamento mucoso bilateral e fenda à fonação. As voluntárias receberam dez sessões de TENS (200µs e 10Hz) por 30 minutos. A dor foi avaliada pela escala visual analógica, a voz por meio de laringoscopia, análise perceptivo-auditiva e acústica e o sinal mioelétrico pela raiz quadrada da média (RMS). A coleta dos dados de voz e EMG deu-se por emissão da vogal /E/ e fala espontânea. A análise estatística constou do teste de Shapiro-Wilk, seguido do teste de Wilcoxon ou t Student ou de Friedman (p < 0,05). RESULTADOS: observou-se que a TENS diminuiu o RMS, pré e pós-tratamento, para TD (2,80 ± 1,36 para 1,77 ± 0,93), TE (3,62 ± 2,10 para 2,10 ± 1,06), ECME (2,64 ± 0,69 para 1,94 ± 0,95) e SH (11,59 ± 7,72 para 7,82 ± 5,95) durante a emissão da vogal /E/, e TD (3,56 ± 2,77 para 1,93 ± 1,13), TE (4,68 ± 2,56 para 3,09 ± 2,31), ECMD (3,94 ± 2,04 para 2,51 ± 1,87) e ECME (3,54 ± 1,04 para 3,12 ± 3,00) durante a fala espontânea (FE), além da diminuição da dor. Quanto à voz, ocorreu diminuição do grau das lesões laríngeas e, na análise perceptivo-auditiva, não houve diferença durante a emissão da vogal /E/, porém durante a FE ocorreu diminuição do grau de disfonia e rouquidão. CONCLUSÃO: a TENS é eficaz na melhora do quadro clínico e funcional de mulheres disfônicas.

BACKGROUND: studies indicate correlation between dysphonia and muscle tension. AIM: to evaluate bilaterally the electrical activity of the suprahyoid muscles (SH), sternocleidomastoid (SCM), and trapezius (T), the presence of pain and the voice, after applying transcutaneous electrical nerve stimulation (TENS). METHOD: ten (10) women with nodules or bilateral mucus thickening, and phonation fissure. Volunteers were submitted to 10 TENS sessions (200µs and 10Hz) for 30 minutes. Pain was evaluated using an analogical visual scale; the voice was evaluated through laryngoscopy and through a perceptive-auditory and acoustic analysis; and the myoelectric signal was converted using the Root Media Square (RMS). Voice and EMG data gathering was performed during the production of the E/vowel and during spontaneous speech (SS). STATISTICAL ANALYSIS: Shapiro-Wilk Test followed by the Wilcoxon Test, or t Student, or Friedman Test (p < 0.05). RESULTS: It was observed that the TENS decreased the RMS readings, pre and pos treatment, for the Right T (RT) (2.80 ± 1.36 to 1.77 ± 0.93), the Left T (LT) (3.62 ± 2.10 to 2.10 ± 1.06), the Left SCM (LSCM) (2.64 ± 0.69 to 1.94 ± 0.95), and the SH (11.59 ± 7.72 to 7.82 ± 5.95) during the production of the E/vowel; and for the RT (3.56 ± 2.77 to 1.93 ± 1.13), the LT (4.68 ± 2.56 to 3.09 ± 2.31), the Right SCM (RSCM) (3.94 ± 2.04 to 2.51 ± 1.87), and the LSCM (3.54 ± 1.04 to 3.12 ± 3.00) during SS. A relieve in pain was also observed. Regarding the voice analysis, there was a decrease in level of laryngeal injuries; no difference was observed during the production of the E/vowel in the perceptive-auditory analysis; there was a decrease in the level of dysphonia and hoarseness during SS. CONCLUSION: TENS is effective in improving the clinical and functional signs of dysphonic women.
