Resumo Introdução: O Teste AVD-Glittre (TGlittre) foi desenvolvido para avaliar a capacidade funcional por meio de um grupo de atividades de vida diária, porém ainda pouco se sabe sobre as respostas fisiológicas esperadas durante a sua execução. Objetivo: Avaliar as respostas fisiológicas induzidas pelo TGlittre em pacientes idosos com DPOC em comparação as respostas induzidas pelo Teste de Caminhada de 6 Minutos (TC6m). Método: Estudo observacional transversal. Participaram 15 pacientes idosos com DPOC (70±6 anos, VEF1: 47±16% do previsto). Em dois dias diferentes, realizou-se o TGlittre e o TC6m, registrando-se a frequência cardíaca, saturação periférica de oxigênio e a percepção de esforço no 1º, 4º e 6º minutos do TC6m e no início, após cada volta e ao final do TGlittre. Após a análise de normalidade (Shapiro-Wilk), utilizou-se o teste de Wilcoxon para comparações entre as variáveis fisiológicas em resposta aos testes funcionais e o coeficiente de correlação de Spearman para verificar a associação entre as variáveis. Resultados: Ao final do TGlittre, a frequência cardíaca foi superior à do TC6m (106,7±21,9 vs 96,4±16,2 bpm, p = 0,02). As demais variáveis fisiológicas foram similares ao final em ambos os testes. A frequência cardíaca final do TGlittre correlacionou-se com a frequência cardíaca final do TC6m (r = 0,69; p = 0,002). Conclusão: O TGlittre foi capaz de induzir a uma frequência cardíaca superior em relação ao TC6m, com maior demanda metabólica, porém com respostas ventilatórias similares no grupo estudado.

Abstract Introduction: The Glittre-ADL (TGlittre) test was developed to assess functional capacity in a group of activities of daily living, but little is known about the physiological responses expected during its implementation. Objective: To evaluate the physiological responses induced by TGlittre in COPD patients and compare them with those induced by the 6-minute walk test (6MWT). Method: This is a cross-sectional study involving 15 elderly patients with COPD (70±6 years and predicted FEV1 of 47±16%). The TGlittre and 6MWT were performed on two different days, evaluating heart rate, peripheral oxygen saturation and perceived exertion in the 1st, 4th and 6th minutes of the 6MWT and at the start, after each lap and the end of TGlittre. After the normality test (Shapiro-Wilk), the Wilcoxon test was applied to compare the functional tests, and Spearman’s correlation coefficient to assess the association between variables. Results: At the end of TGlittre, heart rate was faster than in the 6MWT (106.7±21.9 vs 96.4±16.2bpm, p = 0.02). The other physiological variables were similar at the end of both tests. Heart rate at the end of TGlittre correlated with the final heart rate in the 6MWT (r = 0.69; p = 0.002). Conclusion: TGlittre induced a faster heart rate than in the 6MWT, with increased metabolic demand, but with similar ventilatory responses.

Objetivo Investigar a ocorrência de fragilidade e analisar a capacidade funcional e qualidade de vida nos idosos atendidos em um serviço de geriatria e gerontologia em Belém-PA. Método Estudo transversal, descritivo e analítico. Foram avaliados 103 idosos quanto a aspectos sociodemográficos e clínicos, fragilidade de acordo com o fenótipo de Fried, histórico de quedas, autopercepção de saúde, capacidade funcional (CF) e qualidade de vida (QV). Os idosos foram classificados em frágeis (FR), pré-frágeis (PF) e não frágeis (NF), os grupos foram comparados por meio de teste binomial, de Kruskal-Wallis e ANOVA, e a relação entre CF e QV por meio da correlação de Pearson. Resultados A média de idade foi 73,39(±6,42) anos; 23,0% dos idosos eram FR, 57,0% PF e 20,0% NF. Os critérios do fenótipo mais pontuados foram fraqueza muscular e inatividade física. Houve diferença na CF entre FR e PF (p<0,01) e FR e NF (p<0,01). Os idosos FR apresentaram menor QV e as facetas mais pontuadas foram intimidade (15,33±2,26) e morte e morrer (14,88±3,26). Houve correlação entre CF e QV nos grupos PF (p=0,0273) e NF (p=0,0017); 62,1% dos idosos apontaram saúde regular e 34,0% tinham histórico de queda. Conclusão Fraqueza muscular e inatividade física foram mais marcantes no desenvolvimento da fragilidade, que esteve associada a pior CF e QV, apesar de a maioria dos idosos ser independente. Esses dados são importantes para a detecção precoce de fatores determinantes da fragilidade, já que os critérios abordados são passíveis de reversão.

Objective To investigate the occurrence of frailty and analyze the functional capacity and quality of life in the elderly in a department of geriatrics and gerontology in Belém-PA. Method Cross-sectional, descriptive and analytical study.103 elderly people were assessed as the socio-demographic and clinical aspects, frailty, according to the phenotype of Fried, history of falls, self-perceived health, functional capacity (FC) and quality of life (QOL). The elderly were classified as frail (FR), pre-frail (PF) and non-frail (NF). The groups were compared using the binomial test, Kruskal-Wallis test and ANOVA test, and the relationship between FC and QOL through Pearson correlation. Results The mean age was 73.39(±6.42) years; 23.0% of the elderly were FR, 57.0%were PF and 20.0% were NF. The highest criteria of phenotype were muscle weakness and physical inactivity. Was no difference in FC between FR and PF (p<0.01) and FR and NF (p<0.01). The FR elderly had lower QOL and the highest scores were intimacy (15.33±2.26) and death and dying (14.88±3.26). There were correlation between FC and QOL in groups PF (p=0.0273) and NF (p=0.0017). 62.1% of seniors pointed to health as regular and 34.0% had a history of falling. Conclusion Muscle weakness and physical inactivity were most striking in the development of frailty, which was associated with worse QOL and FC, despite most seniors be independent. These data are important for early detection of determinants of frailty, since the criteria discussed here are reversible.

OBJETIVO Analisar a prevalência e fatores associados à abstinência tabágica entre pacientes atendidos em unidade de referência para cessação do tabagismo.MÉTODOS Estudo transversal que analisou 532 prontuários de pacientes atendidos em unidade de referência para cessação do tabagismo em Belém, PA, entre janeiro de 2010 e junho de 2012. Foram analisadas variáveis sociodemográficas e aquelas relacionadas à história tabágica e ao tratamento realizado. Considerou-se para significância estatística p < 0,05.RESULTADOS A média de idade dos participantes foi 50 anos e cerca de 57,0% dos pacientes eram do sexo feminino. A carga tabágica média foi 30 maços/ano e o tempo médio de tabagismo foi aproximadamente 32 anos. A maioria dos pacientes permaneceram em tratamento por quatro meses. A taxa de abstinência tabágica foi 75,0%. Na análise de regressão, terapia de manutenção, ausência de gatilhos de recaída e dependência química mais baixa foram significativamente associados à cessação.CONCLUSÕES A taxa de abstinência tabágica observada foi 75,0%. O processo de cessação associou-se a diversos aspectos, como grau de dependência química, sintomas de abstinência e tempo de acompanhamento do paciente em um programa de tratamento multiprofissional. Estudos desta natureza, portanto, auxiliam na reunião de informações epidemiológicas consistentes, fundamentais à elaboração de ações de prevenção e combate ao tabagismo mais efetivas.

Sociodemographic variables and those related to smoking history and treatment were analyzed. Statistical significance was set at p < 0.05.RESULTS The mean age of the participants was 50 years; 57.0% of the patients were women. The mean tobacco load was 30 packs/year, and the mean smoking duration was approximately 32 years. Most patients remained in treatment for four months. The rate of smoking abstinence was 75.0%. Regression analysis indicated that maintenance therapy, absence of relapse triggers, and lower chemical dependence were significantly associated with smoking cessation.CONCLUSIONS The smoking abstinence rate observed was 75.0%. The cessation process was associated with several aspects, including the degree of chemical dependence, symptoms of withdrawal, and period of patient follow-up in a multidisciplinary treatment program. Studies of this nature contribute to the collection of consistent epidemiological data and are essential for the implementation of effective smoking prevention and cessation strategies.

Objetivo: Identificar a funcionalidade global de idosos submetidos a internação, correlacionando o desempenho para as atividades de vida diária básicas (ABVD) e instrumentais (AIVD) com os principais sistemas funcionais (cognição, humor, mobilidade e comunicação). Métodos: Trata-se de estudo observacional transversal e analítico com a participação de 94 idosos internados em uma clínica médica. Os instrumentos utilizados foram: Escala de Katz, Escala de Lawton & Brody, Miniexame do Estado Mental (MEEM), Escala de Depressão Geriátrica 15 (GDS-15), Teste Timed Up and Go (TUG) e a Avaliação Funcional das Habilidades de Comunicação da Associação Americana de Fonoaudiologia (ASHA FACS). Resultados: Foram avaliados 94 idosos, cuja maioria apresentava algum grau de dependência para ABVD (61,71%) e para AIVD (52,13%). Nos sistemas funcionais, a autonomia mostrou-se preservada, com desempenho médio de 18,14 pontos no MEEM, e de 4,43 pontos na GDS-15; e a independência, alterada, por desempenho médio de 21,82 segundos no TUG, classificando-os com uma mobilidade regular, e de 5,27 pontos na ASHA FACS, considerando-os com a necessidade de auxílio moderado nessa função. Evidenciou-se moderada e significativa associação entre o desempenho para as ABVDs e AIVDs com quase todos os sistemas funcionais. Conclusão: A população estudada apresentou funcionalidade global alterada, devido a algum grau de dependência para as ABVDs e AIVDs, com autonomia preservada e independência prejudicada. As correlações evidenciaram que com o decréscimo dos principais sistemas funcionais, ocorreu declínio da funcionalidade global.

Objective: Identify the global functionality of hospitalized elderly, correlating the performance to basic (BADL) and instrumental (IADL) activities of daily life, with the main functional systems (cognition, mood, mobility and communication). Methods: Analytical, observational, cross-sectional study, with the participation of 94 elderly patients admitted to a medical clinic. The tools used in the assessments were: the Katz Scale, Lawton and Brody Scale, Mini-Mental State Examination (MMSE), Geriatric Depression Scale 15 (GDS-15), Timed Up and Go Test (TUG) and the Functional Assessment of Communication Skills from the American Speech-Language-Hearing Association (ASHA FACS). Results: 94 elderly people were evaluated, where most had some degree of dependence for BADL (61.71%) and IADL (52.13%). In functional systems, autonomy proved to be preserved with an average performance of 18.14 points on the MMSE, and 4.43 points in the GDS-15; independence was altered, for average performance of 21.82 seconds on the TUG, classifying them with regular mobility, and 5.27 points in ASHA FACS, considering them as in need of moderate assistance in this task. It was evidenced moderate and significant association between performance for IADL and BADL in nearly all functional systems. Conclusion: The study population presented changed overall functionality, due to some degree of dependence in BADL and IADL, with preserved autonomy and impaired independence. The correlations showed that with the decline of the major functional systems occurred decline in overall functionality.
