ABSTRACT BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare and heterogeneous disease that is difficult to diagnose and requires complex and expensive diagnostic tools. The saccharin transit time test is a simple and inexpensive tool that may assist in screening patients with PCD. OBJECTIVES: This study aimed to compare changes in the electron microscopy findings with clinical variables and saccharin tests in individuals diagnosed with clinical PCD (cPCD) and a control group. DESIGN AND SETTING: An observational cross-sectional study was conducted in an otorhinolaryngology outpatient clinic from August 2012 to April 2021. METHOD: Patients with cPCD underwent clinical screening questionnaires, nasal endoscopy, the saccharin transit time test, and nasal biopsy for transmission electron microscopy. RESULTS: Thirty-four patients with cPCD were evaluated. The most prevalent clinical comorbidities in the cPCD group were recurrent pneumonia, bronchiectasis, and chronic rhinosinusitis. Electron microscopy confirmed the clinical diagnosis of PCD in 16 of the 34 (47.1%) patients. CONCLUSION: The saccharin test could assist in screening patients with PCD due to its association with clinical alterations related to PCD. Resumo Introdução: Avaliar a percepção olfativa em distúrbios olfativos é de extrema importância para a correta conduta terapêutica. No entanto, apenas o teste University of Pennsylvania smell identification test e o teste sniffin’sticks são validados no Brasil. Objetivos: Avaliar a correlação e concordância entre os testes Connecticut chemosensory clinical research center e do brief-smell identification test e University of Pennsylvania smell identification test em participantes saudáveis e em participantes com distúrbios olfativos de acordo com os resultados e aspectos técnicos dos dois testes. Método: Cinquenta participantes sem queixas olfativas e 50 participantes com distúrbios olfativos submetidos ao teste Connecticut chemosensory clinical research center e ao brief-smell identification test foram incluídos. Os seguintes testes foram usados para análise estatística: teste U de Mann-Whitney, correlação de Spearman, coeficiente de correlação intraclasse e plotagem de Bland-Altman. Um erro alfa (nível de significância) de 0,05 foi considerado nas análises estatísticas feitas no estudo. Resultados: Ambos os testes foram eficazes para diferenciar os grupos sem a presença de valores sobrepostos para os marcadores medidos. Além disso, houve uma forte correlação entre a correlação de Spearman e o coeficiente de correlação intraclasse entre os testes e para as duas narinas. Entretanto, as correlações foram menores quando os grupos foram avaliados individualmente. O gráfico de Bland-Altman não mostrou viés quando todos os participantes foram avaliados simultaneamente. Conclusões: Os testes para avaliar a percepção olfativa apresentaram um elevado nível de concordância. Em nossa amostra, podemos inferir que o Connecticut chemosensory clinical research center é equivalente ao brief-smell identification test e pode ser usado no diagnóstico de rotina de pacientes com queixas de distúrbios olfativos, considerando a vantagem de seu baixo custo.

Abstract Introduction: Assessing olfactory perception in olfactory disorders is of utmost importance in therapy management. However, the University of Pennsylvania Smell Identification Test and the Sniffin’ Sticks are the only tests validated in Brazil. Objectives: To evaluate the correlation and agreement between the Chemosensory Clinical Research Center olfactory test and the Brief-Smell Identification Test – University of Pennsylvania Smell Identification Test – in healthy participants and in participants with olfactory disorders based on the results and technical aspects of both tests. Methods: Fifty participants without olfactory complaints and 50 participants with olfactory disorders who underwent the Chemosensory Clinical Research Center olfactory test and the Brief-Smell Identification Test were included. The following tests were used for statistical analysis: Mann-Whitney U test, Spearman’s correlation, intraclass correlation coefficient and Bland-Altman plot. An alpha error (significance level) of 0.05 was considered in the statistical analysis. Results: Both tests were effective in distinguishing the groups without the presence of overlapping values for the measured markers. Additionally, there was a strong correlation between Spearman’s correlation and intraclass correlation coefficient between the tests and for both nostrils. However, the correlations were lower when the groups were individually evaluated. The Bland-Altman plot showed no bias when all participants were simultaneously evaluated. Conclusions: The tests to assess olfactory perception presented a high level of agreement. In our sample, we could infer that the Connecticut Chemosensory Clinical Research Center olfactory test is similar to the Brief-Smell Identification Test and can be used in the routine diagnosis of patients with complaints of olfactory disorders, considering the advantage of its low cost.

Resumo Introdução Alterações olfativas são bastante comuns na população, causam significativo impacto na qualidade de vida. A documentação da função olfatória é fundamental para o diagnóstico, tratamento e seguimento de pacientes que cursam com doenças inflamatórias das vias aéreas superiores, neurodegenerativas ou infecções virais. Entre os diferentes testes de olfato existentes, o teste do Connecticut Chemosensory Clinical Research Center (CCCRC) é barato, de fácil aplicação, mas que ainda não foi avaliado em grande escala para a população brasileira. Objetivo Validar o teste de olfato CCCRC com adaptação para a população brasileira, avaliar o desempenho de voluntários saudáveis e a estabilidade do teste em retestes. Método Neste estudo fizemos adaptação cultural do teste CCCRC para o Brasil. Para validação e determinação dos escores de normalidade, aplicamos o teste em 334 voluntários saudáveis, com mais de 18 anos. O reteste foi ainda feito em até quatro semanas em 34 voluntários adicionais para avaliar concordância dos resultados. Resultados Avaliando o desempenho dos participantes, valores de normosmia e hiposmia leve foram obtidos em mais de 95% deles. Mulheres (58,4%) apresentaram melhor acurácia em relação aos homens (41,6%), p< 0,02; e indivíduos acima dos 60 anos apresentaram pior desempenho (mediana: 6; percentil 75: 6,5; percentil 25: 5). O teste e reteste dos 34 voluntários demonstrou que houve concordância (coeficiente de correlação intraclasse, CCI) considerada boa em narina esquerda (CCI = 0,65) e excelente em narina direita (CCI = 0,77) no escore combinado. Conclusão O teste CCCRC adaptado para o Brasil apresentou valores de normalidade semelhantes ao teste originalmente descrito e a validações em outros países, com alta taxa de reprodutibilidade. Considerando a relação custo-benefício altamente favorável, o CCCRC adaptado é uma ferramenta muito útil para mensuração da função olfatória na população brasileira.

Abstract Introduction Olfactory changes are quite common in the population, causing a significant impact on the quality of life. Documentation of the olfactory function is essential for the diagnosis, treatment and follow-up of patients with inflammatory diseases of the upper airways, neurodegenerative diseases or viral infections. Among the different existing smell tests, the CCCRC is an inexpensive test, easy to apply, but it has not yet been evaluated on a large scale in the Brazilian population. Objective To validate the CCCRC smell test, after adaptation for the Brazilian population, evaluating the performance of healthy volunteers and the stability of the test in retests. Methods In this study, we carried out a cultural adaptation of the CCCRC test to Brazil. To validate and determine the normality scores, we applied the test to 334 healthy volunteers, aged >18 years of age. The retest was also carried out in up to four weeks on 34 additional volunteers to assess validity of the results. Results When evaluating the participants’ performance, normosmia and mild hyposmia values were obtained in more than 95% of them. Women (58.4%) showed better accuracy than men (41.6%): p < 0.02, and individuals over 60 years of age showed worse performance (median: 6; 75th percentile: 6.5; 25th percentile). The test and retest of the 34 volunteers demonstrated that there was agreement (ICC, intraclass correlation coefficient) considered good in the left nostril (ICC = 0.65) and excellent in the right nostril (ICC = 0.77) in the combined score. Conclusion The CCCRC test adapted to Brazil showed normal values, similar to the originally-described test and validations in other countries, with a high reproducibility rate. Considering the highly favorable cost-benefit ratio, the adapted CCCRC is a very useful tool for measuring olfactory function in the Brazilian population.

Resumo Introdução Numerosos exames podem ser realizados para fornecer informações clínicas e diagnósticas em respiradores bucais com atresia maxilar e mordida cruzada posterior, entretanto a correlação entre esses exames ainda é incerta. Objetivo Avaliar a correlação entre rinometria acústica, rinomanometria computadorizada e tomografia computadorizada por feixe cônico em respiradores bucais com atresia maxilar. Método Um estudo de corte transversal foi realizado em 30 respiradores bucais com atresia maxilar (7-13 anos) com mordida cruzada posterior. Os exames avaliados foram: (i) rinomanometria acústica: volumes nasais (0-5 cm e 2-5 cm) e áreas mínimas de corte transversal 1 e 2 da cavidade nasal; (ii) rinomanometria computadorizada: fluxo nasal e resistências médias inspiratórias e expiratórias; (iii) tomografia computadorizada por feixe cônico: corte coronal na cabeça da concha inferior (larguras 1 e 2), concha média (larguras 3 e 4) e na maxila (Largura 5). Rinomanometria acústica e rinomanometria computadorizada foram avaliadas antes e depois da administração de vasoconstritor. Os resultados foram comparados pelo teste de correlação de Spearman e pelo teste de Mann-Whitney (α = 0,05). Resultados Foram encontradas correlações positivas entre: (i) fluxo antes da administração de vasoconstritor e largura 4 (Rho = 0,380) e largura 5 (Rho = 0,371); (ii) largura 2 e área mínima de corte transversal 1 antes da administração de vasoconstritor (Rho = 0,380); (iii) fluxo antes da administração de vasoconstritor e volumes nasais de 0-5 cm (Rho = 0,421), 2-5 cm (Rho = 0,393) e área mínima de corte transversal 1 (Rho = 0,375); (iv) largura 4 e volume nasal de 0-5 cm antes da administração do vasoconstritor (Rho = 0,376), volume nasal de 2-5 cm antes do uso de vasoconstritor (Rho = 0,376), áreas mínimas de corte transversal 1 antes da administração de vasoconstritor (Rho = 0,410) e áreas mínimas de corte transversal 1 após o uso do vasoconstritor (Rho = 0,426); (v) largura 5 e largura 1 (Rho = 0,542), largura 2 (Rho = 0,411) e largura 4 (Rho = 0,429). Foram encontradas correlações negativas: (i) largura 4 e resistência inspiratória média (Rho = −0,385); (ii) resistência inspiratória média antes da administração de vasoconstritor e volume de 0-5 cm (Rho = −0,382) e resistência expiratória média antes da administração de vasoconstritor e área mínima de corte transversal 1 (Rho = −0,362). Conclusão Correlações foram encontradas entre a rinometria acústica, a rinomanometria computadorizada e a tomografia computadorizada de feixe cônico em respiradores bucais com atresia maxilar.

Abstract Introduction To provide clinical information and diagnosis in mouth breathers with transverse maxillary deficiency with posterior crossbite, numerous exams can be performed; however, the correlation among these exams remains unclear. Objective To evaluate the correlation between acoustic rhinometry, computed rhinomanometry, and cone-beam computed tomography in mouth breathers with transverse maxillary deficiency. Methods A cross-sectional study was conducted in 30 mouth breathers with transverse maxillary deficiency (7-13 y.o.) patients with posterior crossbite. The examinations assessed: (i) acoustic rhinometry: nasal volumes (0-5 cm and 2-5 cm) and minimum cross-sectional areas 1 and 2 of nasal cavity; (ii) computed rhinomanometry: flow and average inspiratory and expiratory resistance; (iii) cone-beam computed tomography: coronal section on the head of inferior turbinate (Widths 1 and 2), middle turbinate (Widths 3 and 4) and maxilla levels (Width 5). Acoustic rhinometry and computed rhinomanometry were evaluated before and after administration of vasoconstrictor. Results were compared by Spearman's correlation and Mann-Whitney tests (α = 0.05). Results Positive correlations were observed between: (i) flow evaluated before administration of vasoconstrictor and Width 4 (Rho = 0.380) and Width 5 (Rho = 0.371); (ii) Width 2 and minimum cross-sectional areas 1 evaluated before administration of vasoconstrictor (Rho = 0.380); (iii) flow evaluated before administration of vasoconstrictor and nasal volumes of 0-5 cm (Rho = 0.421), nasal volumes of 2-5 cm (Rho = 0.393) and minimum cross-sectional areas 1 (Rho = 0.375); (iv) Width 4 and nasal volumes of 0-5 cm evaluated before administration of vasoconstrictor (Rho = 0.376), nasal volumes of 2-5 cm evaluated before administration of vasoconstrictor (Rho = 0.376), minimum cross-sectional areas 1 evaluated before administration of vasoconstrictor (Rho = 0.410) and minimum cross-sectional areas 1 after administration of vasoconstrictor (Rho = 0.426); (v) Width 5 and Width 1 (Rho = 0.542), Width 2 (Rho = 0.411), and Width 4 (Rho = 0.429). Negative correlations were observed between: (i) Width 4 and average inspiratory resistance (Rho = −0.385); (ii) average inspiratory resistance evaluated before administration of vasoconstrictor and nasal volumes of 0-5 cm (Rho = −0.382), and average expiratory resistance evaluated before administration of vasoconstrictor and minimum cross-sectional areas 1 (Rho = −0.362). Conclusion There were correlations between acoustic rhinometry, computed rhinomanometry, and cone-beam computed tomography in mouth breathers with transverse maxillary deficiency.

Nas últimas décadas, vários trabalhos sobre respiração bucal (RB) têm surgido na literatura; em contrapartida, pouco se conhece sobre vários aspectos desta síndrome, incluindo gravidade, repercussões sobre o rendimento físico e escolar. OBJETIVO: Comparar o rendimento físico pelo teste de caminhada de seis minutos (TC6') e rendimento escolar de crianças e adolescentes com RB e respiradores nasais (RN). MÉTODO: Estudo de corte transversal descritivo e prospectivo em crianças RB e RN que foram submetidas ao TC6' e avaliação do rendimento escolar. RESULTADOS: Foram incluídos 156 escolares, 87 meninas (60 RN e 27 RB) e 69 meninos (44 RN e 25 RB). Foram analisadas variáveis durante o TC6': frequência cardíaca (FC), frequência respiratória, saturação periférica de oxigênio, distância percorrida em seis minutos e escala de Borg modificada. Todos os valores das variáveis estudadas foram estatisticamente diferentes entre os grupos RB e RN, com exceção do rendimento escolar e FC no TC6'. CONCLUSÃO: A RB afeta o rendimento físico e não o rendimento escolar, sendo observado padrão alterado no TC6' no grupo RB. Uma vez que os RB desse estudo foram classificados como não graves outros estudos comparando as variáveis de rendimento escolar e TC6' são necessários para o melhor entendimento do processo dos desempenhos físico e escolar em crianças com RB.

In recent decades, many studies on mouth breathing (MB) have been published; however, little is known about many aspects of this syndrome, including severity, impact on physical and academic performances. OBJECTIVE: Compare the physical performance in a six minutes walk test (6MWT) and the academic performance of MB and nasal-breathing (NB) children and adolescents. METHOD: This is a descriptive, cross-sectional, and prospective study with MB and NB children submitted to the 6MWT and scholar performance assessment. RESULTS: We included 156 children, 87 girls (60 NB and 27 MB) and 69 boys (44 NB and 25 MB). Variables were analyzed during the 6MWT: heart rate (HR), respiratory rate, oxygen saturation, distance walked in six minutes and modified Borg scale. All the variables studied were statistically different between groups NB and MB, with the exception of school performance and HR in 6MWT. CONCLUSION: MB affects physical performance and not the academic performance, we noticed a changed pattern in the 6MWT in the MB group. Since the MBs in our study were classified as non-severe, other studies comparing the academic performance variables and 6MWT are needed to better understand the process of physical and academic performances in MB children.

OBJETIVOS: verificar o efeito da intervenção fonoaudiológica em um grupo de respiradores orais e propor terapia fonoaudiológica mínima no tratamento da respiração oral. MÉTODO: estudo prospectivo longitudinal, casuística de 40 sujeitos respiradores orais tratados no Hospital das Clínicas - setor de Reabilitação Orofacial do Ambulatório de Respirador Oral da Disciplina de Otorrinolaringologia da Faculdade de Ciências Médicas da Universidade Estadual de Campinas - UNICAMP. Realizada documentação fotográfica, avaliação clínica, aplicação de protocolo para categorizar modo respiratório, postura (lábios e bochechas); força e praxias (lábios, bochechas e língua) nas semanas 0, 12 e 24. A proposta terapêutica constou de treino e conscientização da respiração nasal; manobras para aquecimento e vascularização da musculatura orofacial; aplicação de pontos e zonas motoras na face; manobras passivas; uso do impulso distal; exercícios miofuncionais e registro da percepção dos pacientes sobre suas condições olfativas e obstruções nasais.Teste estatístico: não paramétrico de Igualdade de Duas Proporções, p < 0,05. RESULTADOS: houve adequação da função respiração nasal. Aumento da força de lábios, língua e bochechas. Melhora nas praxias: bico direita, bico esquerda, estalo e vibração de lábios. Vibração e estalo de língua. Inflar simultaneamente as bochechas, inflar bochecha direita e esquerda. O tempo com maior ganho terapêutico foi de 12 semanas. A partir deste dado foi esquematizado protocolo com 12 sessões estruturadas abordando estratégias utilizadas nesta pesquisa. CONCLUSÃO: a pesquisa demonstrou que o uso da reabilitação miofuncional para pacientes respiradores orais foi eficiente com maior evolução terapêutica observada na semana 12.

PURPOSE: to check the effect of speech and language therapy on a group of mouth breathers, as well proposing a minimum therapy for mouth breathing treatment. METHOD: longitudinal prospective study, casuistic on 40 mouth breather subjects who were patients from the Otorhinolaryngology Ambulatory of UNICAMP Hospital. On weeks 0, 12 and 24, we proceeded photographical documentation, clinical assessment, and protocol application in order to categorize the breathing mode, posture (lips and cheeks), strength and movement (lips, cheeks and tongue). The applied therapeutic proposal consisted of practicing and awareness on nasal breathing; maneuvers to warm and vascularize the orofacial muscles; application of points and motor zones on the face; passive maneuvers; use of distal impulse; myofunctional exercises and register of patients’ perception on their olfactive conditions and nasal obstructions. A non-parametric two proportion equality test was used, p < 0.05. RESULTS: nasal breathing function was adjusted. Increase on lip, tongue and cheek strength. Improvement on the movement: lips pouted to the right and left, snap and lip vibration. Vibration and snap of tongue. Simultaneous inflation of the cheeks, inflation of left and right cheeks alternately. The highest therapeutic gain occurred on 12 weeks. From such data it was possible to come up a protocol made up of 12 structured sessions using the strategies from this study. CONCLUSION: the study has demonstrated the effectiveness of using myofunctional rehabilitation for mouth breathers, and the highest therapeutic evolution took place during the 12th week.

OBJETIVO: Avaliar as funções orofaciais de mastigação, deglutição e fala em crianças asmáticas e crianças saudáveis. MÉTODOS: Estudo de corte transversal com 54 crianças, com idades entre 7 e 10 anos completos, de ambos os gêneros. Do total, 27 compuseram o grupo experimental e foram subdivididas em dois níveis de gravidade de asma: Grupo I - asma intermitente ou persistente leve; Grupo II - asma persistente moderada ou grave. Fizeram parte do grupo controle (Grupo III) 27 crianças saudáveis. Para a avaliação fonoaudiológica foi utilizado o Protocolo de Avaliação Miofuncional Orofacial adaptado. A adaptação constituiu na exclusão da parte estrutural, já que esta não fazia parte do objetivo do estudo. RESULTADOS: Alterações nas funções orofaciais foram encontradas, com diferenças entre os três grupos estudados. Essas alterações não tiveram relação com a gravidade da asma, visto que o maior índice de alterações foi encontrado no Grupo I (asma leve). CONCLUSÃO: Independentemente do nível de gravidade da doença, crianças asmáticas apresentam padrões alterados de mastigação, deglutição e fala.

PURPOSE: To compare the orofacial functions (chewing, swallowing and speech) in children with asthma and healthy children. METHODS: A cross sectional study including 54 children of both genders with ages between 7 and 10 years was conducted. Twenty-seven of these subjects composed the experimental group, and were subdivided into two severity levels of asthma: Group I - mild intermittent and persistent asthma; Group II - persistent moderate to severe asthma. Twenty-seven healthy children were included in the control group (Group III). Speech-language pathology evaluation used the adapted Orofacial Myofunctional Assessment Protocol. Adaptation consisted in the exclusion of the structural part of the test, since this was not the aim of the study. The structural part was excluded because it was not the aim of this study. RESULTS: It was found alterations in oral functions, with significant differences between the three groups. These alterations showed no correlation with asthma severity, since the highest rate of alterations was found in Group I (mild asthma). CONCLUSION: Regardless of the severity level, children with asthma have altered patterns of chewing, swallowing and speech.

A respiração bucal ou oral (RO) crônica e persistente tem sido associada a alterações posturais. Embora alterações de postura, na RO, causem diminuição da força muscular respiratória, menor expansibilidade torácica, prejuízo na ventilação pulmonar com consequências na capacidade de exercício, poucos estudos têm comprovado todas estas situações. OBJETIVO: Avaliar a tolerância ao exercício, a força muscular respiratória e a postura corporal em crianças respiradoras orais (RO), comparadas com respiradoras nasais (RN). MATERIAL E MÉTODO: Estudo de coorte contemporânea com corte transversal, no qual foram incluídas crianças RO e RN de 8 a 11 anos. Foram excluídas crianças obesas, asmáticas, com doenças respiratórias crônicas, distúrbios neurológicos, ortopédicos e cardiopatas. Todos os participantes foram submetidos à avaliação postural, pressão inspiratória máxima (PImáx), pressão expiratória máxima (PEmáx), teste de caminhada de seis minutos (TC6) e avaliação otorrinolaringológica. RESULTADOS: Participaram 107 crianças (45 RO e 62 RN). Houve associação entre alteração da postura cervical e padrão respiratório: 36(80,0%) RO e 30(48,4%) RN apresentaram alteração da postura da cabeça, respectivamente (RCP=4,27[IC95%:1,63 - 11,42], p<0,001). Médias de PImáx e PEmáx foram menores nos RO (p=0,003 e p=0,004). CONCLUSÃO: Crianças RO apresentaram alteração postural da coluna cervical e diminuição da força muscular respiratória quando comparados com RN.

Chronic and persistent mouth or oral breathing (OB) has been associated with postural changes. Although posture changes in OB causes decreased respiratory muscle strength, reduced chest expansion and impaired pulmonary ventilation with consequences in the exercise capacity, few studies have verified all these assumptions. OBJECTIVE: To evaluate exercise tolerance, respiratory muscle strength and body posture in oral breathing (OB) compared with nasal breathing (NB) children. MATERIAL AND METHOD: A cross-sectional contemporary cohort study that included OB and NB children aged 8-11 years old. Children with obesity, asthma, chronic respiratory diseases, neurological and orthopedic disorders, and cardiac conditions were excluded. All participants underwent a postural assessment, maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), the six-minute walk test (6MWT), and otorhinolaryngologic evaluation. RESULTS: There were 107 children (45 OB and 62 NB). There was an association between abnormal cervical posture and breathing pattern: 36 (80.0%) OB and 30 (48.4%) NB presented abnormal head posture (OR=4.27 [95% CI: 1.63-11,42], p<0.001). The mean MIP and MEP were lower in OB (p=0.003 and p=0.004). CONCLUSION: OB children had cervical spine postural changes and decreased respiratory muscle strength compared with NB.

OBJETIVO: Avaliar a tolerância ao exercício submáximo e a força muscular respiratória em relação à anteriorização da cabeça (AC) e ao tipo respiratório em crianças com respiração bucal (RB) ou nasal (RN). MÉTODOS: Estudo analítico transversal com um grupo controle no qual foram incluídas crianças de 8 a 12 anos com diagnóstico clínico otorrinolaringológico de RB, recrutadas do Ambulatório do Respirador Bucal do Hospital de Clínicas da Universidade Estadual de Campinas, Campinas (SP), entre outubro de 2010 e janeiro de 2011. Os critérios de exclusão foram obesidade, asma, doenças respiratórias crônicas, cardiopatias e distúrbios neurológicos ou ortopédicos. Todos os participantes foram submetidos a avaliação postural, teste de caminhada de seis minutos (TC6) e determinação de PImáx e PEmáx. RESULTADOS: Das 92 crianças do estudo, 30 tinham RB e 62 tinham RN. No grupo RB, não houve diferenças nas médias de PImáx, PEmáx e distância percorrida pelo TC6 (DTC6) entre o grupo com AC classificada como grave ou moderada e aquele com AC normal (p = 0,622; p = 0,957; e p = 0,079, respectivamente). No grupo RN, as médias de PImáx e PEmáx foram maiores no grupo com AC moderada do que naquele com AC normal (p = 0,003 e p = 0,004, respectivamente). Os valores de PImáx, PEmáx e DTC6 foram menores no grupo RB do que no grupo RN. A presença de AC moderada determinou maiores valores de PImáx e PEmáx. CONCLUSÕES: A RB afetou negativamente a biomecânica respiratória e a capacidade de exercício. A presença de AC moderada atuou como um mecanismo de compensação para uma melhor função da musculatura respiratória.

OBJECTIVE: To evaluate submaximal exercise tolerance and respiratory muscle strength in relation to forward head posture (FHP) and respiratory mode in children, comparing mouth-breathing (MB) children with nasal-breathing (NB) children. METHODS: This was a controlled, analytical cross-sectional study involving children in the 8-12 year age bracket with a clinical otorhinolaryngology diagnosis of MB, recruited between October of 2010 and January of 2011 from the Mouth Breather Clinic at the State University of Campinas Hospital de Clínicas, located in the city of Campinas, Brazil. The exclusion criteria were obesity, asthma, chronic respiratory diseases, heart disease, and neurological or orthopedic disorders. All of the participants underwent postural assessment and the six-minute walk test (6MWT), together with determination of MIP and MEP. RESULTS: Of the 92 children in the study, 30 presented with MB and 62 presented with NB. In the MB group, the differences between those with moderate or severe FHP and those with normal head posture, in terms of the mean MIP, MEP and six-minute walk distance (6MWD), were not significant (p = 0.079, p = 0.622, and p = 0.957, respectively). In the NB group, the mean values of MIP and MEP were higher in the children with moderate FHP than in those with normal head posture (p = 0.003 and p = 0.004, respectively). The mean MIP, MEP, and 6MWD were lower in the MB group than in the NB group. Values of MIP and MEP were highest in the children with moderate FHP. CONCLUSIONS: Respiratory biomechanics and exercise capacity were negatively affected by MB. The presence of moderate FHP acted as a compensatory mechanism in order to improve respiratory muscle function.

OBJETIVO: Verificar a associação do tipo respiratório oral (RO) e nasal (RN) e da classificação da postura corporal em variáveis clínicas de crianças e adolescentes com a síndrome do respirador oral, em relação a um grupo-controle de mesma faixa etária. MÉTODOS: Estudo analítico do tipo observacional e transversal, com grupo-controle, realizado em hospital universitário. Foram incluídas crianças maiores de 5 anos, distribuídas em dois grupos: controle saudável (RN) e grupo RO. O grupo RO incluiu pacientes com diagnóstico de síndrome do respirador oral confirmado por exame clínico médico e nasofibroscopia. Participaram do grupo-controle voluntários saudáveis da mesma faixa etária, cujo tipo RN foi confirmado por avaliação médica. Todos os participantes foram submetidos à avaliação postural. Para análise dos dados, foram utilizados os testes: não paramétrico de Mann-Whitney, qui-quadrado e exato de Fisher, considerando-se nível de significância de 0,05%. RESULTADOS: Foram incluídos 306 RO e 124 RN. O tipo RO conferiu maior prevalência no gênero masculino (p = 0,0002), maior grau e frequência de obstrução nasal e tamanho das amígdalas (p = 0,0001) em comparação ao RN. Também apresentou maior incidência de rinite alérgica (p = 0,0001), padrão respiratório torácico (p = 0,0001), palato ogival (p = 0,0001) e classificação postural desfavorável (p = 0,0001) em relação ao grupo-controle. Os índices de classificação postural foram diretamente proporcionais à obstrução nasal (p = 0,0001) e ao gênero masculino (p = 0,0008). CONCLUSÕES: Alterações posturais foram significativamente mais frequentes nas crianças do grupo com síndrome do respirador oral, o que reforça a necessidade de precocidade no tratamento interdisciplinar dessa síndrome.

OBJECTIVE: To investigate associations between mouth breathing (MBr), nose breathing (NBr) and body posture classification and clinical variables in children and adolescents, by comparing patients with mouth breathing syndrome with a control group of similar age. METHODS: This was an observational, analytical, controlled, cross-sectional study conducted at a university hospital. Children aged 5 years or more were recruited to one of two groups: healthy controls (NBr) or an MBr group. The MBr group comprised patients with a diagnosis of mouth breathing syndrome confirmed by clinical examination by a physician plus nasal endoscopy. The control group comprised healthy volunteers of the same age, with NBr confirmed by medical examination. All participants underwent postural assessment. Data were analyzed using the Mann-Whitney nonparametric test, the chi-square test and Fisher’s exact test, to a significance level of 0.05%. RESULTS: A total of 306 MBr and 124 NBr were enrolled. Mouth breathers were more likely to be male (p = 0.0002), have more frequent and more severe nasal obstruction and larger tonsils (p = 0.0001) than NBr. Mouth breathers also exhibited higher incidence rates of allergic rhinitis (p = 0.0001), of thoracic respiratory pattern (p = 0.0001), high-arched palate (p = 0.0001) and unfavorable postural classifications (p = 0.0001) with relation to the control group. Postural classification scores were directly proportional to nasal obstruction (p = 0.0001) and male sex (p = 0.0008). CONCLUSIONS: Postural problems were significantly more common among children in the group with mouth breathing syndrome, highlighting the need for early interdisciplinary treatment of this syndrome.

OBJETIVO: quantificar a ocorrência de movimentos primários de língua em 130 crianças respiradores oronasais e em 89 respiradoras nasais, com idades de cinco a 12 anos, avaliadas no Setor de Respiração Oral do Ambulatório de Otorrinolaringologia do Hospital de Clínicas da Universidade Estadual de Campinas. MÉTODOS: no momento da avaliação fonoaudiológica, foi solicitado à criança deglutir saliva, fechar os olhos, colocar a língua para fora da boca e permanecer assim por, no mínimo, um minuto, tempo necessário para os movimentos primários serem observados. Duas fonoaudiólogas observaram juntas cada criança, a fim de constatar a presença dos movimentos ou não. RESULTADOS: sobre a ocorrência de movimentos primários de língua, foi encontrada em 44,75% da amostra (n=98), enquanto 55,25% (n=121) não apresentaram tais movimentos. Foi observado que as categorias, nasal e oronasal, apresentaram diferenças estatisticamente para a ocorrência de movimentos primários de língua. CONCLUSÃO: em respiração oronasal há uma maior freqüência de manutenção dos movimentos primários de língua e há diferença estatisticamente significante entre o grupo de respiradores oronasais e respiradores nasais para a variável movimento primário de língua.

PURPOSE: to quantify the occurrence of primary tongue movements in 130 children with oronasal breathing and in 89 with nasal breathing with ages between 5 and 12 years old, evaluated in the Section of Mouth Breathing, of the Clinical Hospital of Unicamp, within the Otorrinolaringology Clinic. METHODS: during the evaluation, the children were requested to swallow saliva, close their eyes and put their tongue outside the mouth. They were supposed to stay like this for, at least, one minute, which is enough time to monitor their primary movements. Two language speech pathologists monitored each child, in order to verify the presence of movements. RESULTS: the occurrence of the primary tongue movements was found in 44.75% of the sample (n=98), while 55.25% (n=121) did not show any such movements. It was observed that both nasal and oronasal categories showed statistically significant difference as for the occurrences of primary tongue movements. CONCLUSION: oronasal breathing has higher frequency for maintenance of primary tongue movements, and there is statistically significant difference between oronasal and nasal breathing groups as for the primary tongue movement's variable.

OBJETIVO: propor um protocolo específico para documentação fotográfica do paciente na área de Motricidade Oral. MÉTODO: foi utilizada uma câmera digital fixada em um tripé e foram realizadas fotografias padronizadas corporais e de face. Realizou-se ainda, a teleradiografia lateral contrastada com bário sobre a língua. RESULTADOS: as fotografias facilitaram a visualização dos nossos resultados terapêuticos. CONCLUSÃO: concluiu-se que a documentação proposta auxilia no diagnóstico e no estudo do prognóstico do paciente, bem como, pode ser utilizado como material auxiliar nas orientações aos responsáveis e ao paciente.

PURPOSE: to propose a specific protocol for photographic register of Oral Motricity patients. METHODS: a digital camera mounted on a tripod was used. Standardized photos of body and face were taken. A tele-radiography modified by using contrast with barium which was spread over the patient's tongue was taken. RESULT: the pictures made easy the visualization of our therapeutic results. CONCLUSION: so far it is possible to conclude that the proposed documentation helps in the diagnosis process and prognosis analysis, and it may be used as supplementary material for guiding patients and parents on the best treatment to follow.
