Abstract Objective: Three-dimensional (3D) angular measurements between craniofacial planes pose challenges to quantify maxillary and mandibular skeletal discrepancies in surgical treatment planning. This study aims to compare the reproducibility and reliability of two modules to measure angles between planes or lines in 3D virtual surface models. Methodology: Twenty oriented 3D virtual surface models de-identified and constructed from CBCT scans were randomly selected. Three observers placed landmarks and oriented planes to determine angular measurements of pitch, roll and yaw using (1) 3D pre-existing planes, (2) 3D planes created from landmarks and (3) lines created from landmarks. Inter- and intra-observer reproducibility and repeatability were examined using the Intra-Class Correlation (ICC) test. One observer repeated the measurements with an interval of 15 days. ANOVA was applied to compare the 3 methods. Results: The three methods tested provided statistically similar, reproducible and reliable angular measurements of the facial structures. A strong ICC varying from 0.92 to 1.00 was found for the intra-observer agreement. The inter-observer ICC varied from 0.84 to 1.00. Conclusion: Measurements of 3D angles between facial planes in a common coordinate system are reproducible and repeatable either using 3D pre-existing planes, created based on landmarks or angles between lines created from landmarks.

RESUMO Introdução: o tratamento de danos bucomaxilofaciais é complexo e requer diagnóstico abrangente e preciso, além de um correto plano de tratamento. Objetivo: o objetivo deste relato de caso foi descrever o retratamento de uma paciente do sexo feminino, 27 anos de idade, envolvida em acidente automobilístico grave, que resultou na perda de cinco dentes, além de perda óssea alveolar na região anterior. Relato de caso: a paciente apresentava histórico de insucesso de tratamento ortodôntico e cirúrgico. No caso apresentado, o espaço referente aos molares inferiores ausentes foi reaberto, para possibilitar a reabilitação. Houve melhora no posicionamento dos incisivos inferiores. O canino inferior direito foi deslocado para mesial, possibilitando a correção da relação de Classe II intercaninos do mesmo lado. Implantes foram inseridos a fim de substituir os dentes superiores da região anterior. Conclusão: a reabilitação estética e funcional da região anterior, realizada por meio de uma abordagem multidisciplinar, foi fundamental para valorizar a estética facial da paciente, melhorar a função e promover estabilidade oclusal após dois anos de acompanhamento.

ABSTRACT Introduction: Treatment of maxillofacial injuries is complex and requires the establishment of a comprehensive and accurate diagnosis and correct treatment planning. Objective: The objective of this case report was to describe the re-treatment of a 27-year-old woman who was involved in a severe car accident that resulted in the loss of five anterior teeth and alveolar bone, and whose previous orthodontic and surgical treatments had been unsuccessful. Case report: In this case, the space for the missing mandibular molar was reopened to allow for rehabilitation. The positions of the mandibular incisors were improved. The right mandibular canine was moved to the mesial, allowing for correction of the Class II canine relationship on that side, and implants were placed to replace the maxillary anterior teeth. Conclusion: Anterior aesthetic and functional rehabilitation using a multidisciplinary approach was essential to improve the patient’s facial aesthetics, to obtain great improvement in function and to achieve occlusal stability after 2 years of follow-up.

OBJETIVOS: os objetivos foram determinar os torques de inserção, a resistência mecânica à fratura da ponta e do perfil transmucoso de mini-implantes (MI) de uma marca comercial (Conexão Sistemas de Prótese), e analisar sua morfologia de superfície. MÉTODOS: foram realizados testes mecânicos para medir o torque de inserção dos MI em corticais de diferentes espessuras, também foi avaliada a resistência à fratura da ponta e do perfil transmucoso. Antes e após os ensaios mecânicos, foi avaliada a superfície dos MI no microscópio eletrônico de varredura (MEV). RESULTADOS: os valores de resistência à fratura dos MI (22,14N.cm2 e 54,95N.cm2) foram maiores e estatisticamente diferentes (p < 0,05) dos torques de inserção nas corticais de 1mm (7,60N.cm2) e de 2mm (13,27N.cm2). Entretanto, o valor do torque de inserção na cortical de 3mm (16,11N.cm2) e no osso denso (23,95N.cm2) foi estatisticamente semelhante (p > 0,05) ao torque de fratura da ponta do MI (22,14N.cm2). O torque de fratura do perfil transmucoso (54,95N.cm2) foi maior e estatisticamente diferente (p < 0,05) dos torques de inserção em qualquer uma das situações testadas. A análise em MEV mostrou que os MI, como recebidos e após os testes mecânicos, possuem superfícies lisas e sem marcas significativas oriundas do processo de fabricação. CONCLUSÃO: os MI avaliados apresentaram adequada morfologia da superfície. A resistência do MI foi compatível com a instalação em corticais de 1 e 2mm, porém, em cortical de 3mm e em ossos densos recomenda-se prévia perfuração com broca.

OBJECTIVE: This study aimed to measure insertion torque, tip mechanical resistance to fracture and transmucosal neck of mini-implants (MI) (Conexão Sistemas de PróteseT), as well as to analyze surface morphology. METHODS: Mechanical tests were carried out to measure the insertion torque of MIs in different cortical thicknesses, and tip mechanical resistance to fracture as well as transmucosal neck of MIs. Surface morphology was assessed by scanning electron microscopy (SEM) before and after the mechanical tests. RESULTS: Values of mechanical resistance to fracture (22.14 N.cm and 54.95 N.cm) were higher and statistically different (P < 0.05) from values of insertion torque for 1-mm (7.60 N.cm) and 2-mm (13.27 N.cm) cortical thicknesses. Insertion torque was statistically similar (P > 0.05) to torsional fracture in the tip of MI (22.14 N.cm) when 3 mm cortical thickness (16.11 N.cm) and dense bone (23.95 N.cm) were used. Torsional fracture of the transmucosal neck (54.95 N.cm) was higher and statistically different (P < 0.05) from insertion torsional strength in all tested situations. SEM analysis showed that the MIs had the same smooth surface when received from the manufacturer and after the mechanical tests were performed. Additionally, no significant marks resulting from the manufacturing process were observed. CONCLUSION: All mini-implants tested presented adequate surface morphology. The resistance of mini-implants to fracture safely allows placement in 1 and 2-mm cortical thickness. However, in 3-mm cortical thickness and dense bones, pre-drilling with a bur is recommended before insertion.

INTRODUÇÃO: frequentemente, os pacientes ortodônticos apresentam restaurações de resina composta; no entanto, existem poucos estudos que avaliam a melhor forma de colagem ortodôntica nessa situação. OBJETIVO: o objetivo do presente trabalho foi avaliar a força adesiva de braquetes ortodônticos em restaurações resinosas com tratamento de superfície. MÉTODOS: foram utilizados 51 incisivos inferiores bovinos divididos aleatoriamente em três grupos. No grupo controle (GC), os braquetes foram colados em esmalte dentário; nos grupos experimentais com tratamento (GCT) e sem tratamento (GST), os braquetes foram colados em restauração de resina previamente realizada, diferenciando-se pelo tratamento de superfície com broca diamantada. Os dentes foram incluídos em tubos de PVC com resina acrílica autopolimerizável. O ensaio de cisalhamento foi executado em máquina universal de ensaios Emic. Os grupos foram submetidos à ANOVA com pós-teste de Tukey para verificação da diferença estatística entre os grupos (α = 0,05). RESULTADOS: GC (6,62MPa) e GCT (6,82MPa) apresentaram resultados semelhantes, enquanto o GST (5,07MPa) obteve resultados estatisticamente menores (p < 0,05). CONCLUSÃO: conclui-se que a melhor técnica de colagem de braquetes ortodônticos em restaurações de resina composta é a de realização de desgaste sobre a superfície.

INTRODUCTION: Orthodontic patients frequently present composite resin restorations, however there are few studies that evaluate the best way for orthodontic bonding in this situation. OBJECTIVE: The objective of this work was to evaluate the bond strength of orthodontic brackets in resin restorations with surface treatment. METHODS: Fifty one bovine lower incisors were randomly divided into three groups. On the control group (CG) the brackets were bonded to dental enamel; on experimental groups, brackets were bonded to resin restoration with diamond drill treatment (EGT) and with no treatment (EGN). The teeth were placed in PVC tubes with autopolymerized acrylic resin. The shear test was performed in EMIC universal testing machine. The groups were submitted to ANOVA analysis of variance with Tukey post test to verify the statistical difference between groups (α = 0.05). RESULTS: CG (6.62 MPa) and EGT (6.82 MPa) groups presented similar results, while EGN (5.07 MPa) obtained statistically lower results (p < 0.05). CONCLUSION: Therefore, it is concluded that the best technique for bonding of orthodontic brackets on composite resin restorations is the performance of surface detritions.

OBJETIVO: verificar se o mini-implante no palato é eficaz como ancoragem direta para distalização dos molares superiores. MÉTODOS: foi utilizado um modelo em acrílico da arcada superior. Após a confecção da canaleta na região correspondente aos alvéolos dentários, os dentes em acrílico foram fixados com cera #7, montado aparelho ortodôntico com a técnica Edgewise e inserido um mini-implante (SIN, São Paulo) no local correspondente à rafe palatina. Foram colocados arco 0,19" x 0,25" e barra transpalatina, soldados na barra dois ganchos para retenção de dois elásticos em cadeia de dois elos, a uma carga de 150g/f de cada lado (Unitek), que se estenderam dos ganchos até o mini-implante. O modelo da maxila foi mergulhado 40 vezes em banheira e fotografado após cada mergulho para observação da movimentação dentária. Os dados foram analisados pela análise da variânçia (ANOVA) e teste de Tukey. RESULTADOS: os molares deslocaram-se distalmente 3,1mm, em média, com inclinação distal entre 3 e 5mm. CONCLUÕES: a movimentação dos molares ocorreu pela inclinação distal, com leve rotação, mas sem efeito extrusivo.

OBJECTIVE: To assess whether palatal mini-implants are effective as direct anchorage for distal movement of the upper molars. METHODS: It was used an acrylic model of the upper dental arch. After making a groove in the region corresponding to dental alveolus, acrylic teeth were fixed in groove with #7 wax, with the roots being previously immersed in adhesive wax. The orthodontic appliance was placed according to the Edgewise technique and then a mini-implant (SIN, São Paulo, Brazil) was inserted at the site corresponding to the palatal raphe. A 0.019 x 0.025-in stainless steel archwire was made and attached to the upper arch with elastics. A transpalatal arch bar (0.019 x 0.025in) was mounted and two hooks were soldered to it in order to retain chain elastics (Unitek, Brazil) to be connected to the mini-implant under a force of 1.5 N on each side. The maxillary model was immersed in water 40 times and photographed after each immersion, for observation of dental movements. Analysis of variance (ANOVA) and Tukey's test were employed for analyzing the obtained data. RESULTS: Molars displaced distally 3.1 mm, in average, with distal inclination ranging from 3 to 5 mm. CONCLUSIONS: Molar movements occurred due to distal inclination, with a slight rotation and no extrusive effect.

OBJECTIVE: To assess and compare the number of Streptococcus mutans and Candida albicans colonies in patients with Haas and Hyrax appliances before and after insertion. METHODS: The sample consisted of 84 patients requiring orthodontic treatment. For all patients a midpalatal suture expansion was indicated. Patients were randomly divided into Group HA, who used the Haas appliance (n = 42) and Group HY, who used the Hyrax appliance (n = 42). Initially and thirty days after appliance insertion all patients were submitted to saliva collections. The saliva was diluted followed by seeding in Mitis Salivarius and CHROMagar media, for growth of S. mutans and C. albicans respectively. RESULTS: Results showed statistically significant difference between groups HA and HY for Streptococcus mutans and Candida albicans (p <0.05). Haas appliance promoted greater S. mutans and C. albicans proliferation when compared to Hyrax appliance. CONCLUSION: The Haas appliance favored greater proliferation of S. mutans and C. albicans when compared with the Hyrax appliance. Insertion of the appliances resulted in greater buildup of microorganisms.

OBJETIVO: o objetivo deste estudo foi avaliar as alterações causadas por cirurgia ortognática de recuo mandibular associada ou não à cirurgia maxilar combinada nas vias aéreas superiores (VAS). MÉTODOS: foram avaliadas radiografias cefalométricas de perfil pré-cirúrgicas e pós-cirúrgicas imediatas de 17 pacientes com Classe III. Foram realizadas medições do diâmetro do espaço aéreo (EA) no plano sagital, nas regiões correspondentes à hipofaringe e à orofaringe; também foram registradas as alterações na posição do osso hioide. Utilizou-se o teste t pareado e o coeficiente Pearson, buscando possíveis associações entre as alterações esqueléticas e as ocorridas no EA. RESULTADOS: observou-se redução significativa do EA na região da hipofaringe (média de 3,10mm, p=0,024). O osso hioide sofreu deslocamento inferior e posterior, além de diminuição da distância entre o mesmo e a região anterior da mandíbula. Não foi possível correlacionar, quantitativamente, a redução anteroposterior do EA com o recuo mandibular. Entretanto, observou-se correlação forte entre o diâmetro inicial do EA e a quantidade de redução observada ao nível da hipofaringe, e moderada em relação à orofaringe. CONCLUSÕES: o recuo mandibular pode causar estreitamento significativo das VAS, principalmente na porção mais inferior (hipofaringe). Portanto, deve-se atentar para sua avaliação durante o plano de tratamento ortocirúrgico, já que não foram descartados possíveis efeitos deletérios dessas alterações nas funções do indivíduo.

OBJECTIVE: The purpose of this study is to evaluate the alterations caused by mandibular setback surgery combined or not with maxillary surgery in the upper airways. METHODS: Preoperative and immediate postoperative lateral cephalometric radiographs of 17 Class III patients were evaluated. Measurements of the diameter of air space (AS) in the sagittal plane were performed in regions corresponding to the hypopharynx and oropharynx; changes in the position of the hyoid bone were also registered. The paired t test and Pearson coefficient were used to provide possible associations between skeletal changes and those occurred in the AS. RESULTS: There was a statistically significant reduction of AS in the hypopharynx region (average of 3.10 mm, p = 0.024). The hyoid bone moved backward and downward after the surgery, and the distance between this bone and the anterior region of mandible decreased. It was not possible to correlate, quantitatively, the anteroposterior reduction of the AS after the mandibular setback. However, there was a strong correlation between the initial diameter of AS and the amount of reduction seen at the hypopharynx; this correlation was found to be moderate at the oropharynx level. CONCLUSIONS: The mandibular setback surgery can cause significant narrowing of the upper airways, especially in hypopharynx region. Therefore, it is important to evaluate this structure during treatment planning of combined orthodontic and surgical cases. Possible deleterious effects of these changes on individuals' functions were not yet discarded.

OBJETIVO: analisar pelo método dos elementos finitos o deslocamento dos molares superiores frente a três diferentes inclinações do arco externo do aparelho extrabucal na tração do tipo cervical. MÉTODOS: maxila, dentes montados em má oclusão de Classe II e aparelho foram modelados através de formulação variacional e seus valores reproduzidos em coordenadas X, Y e Z. Foram realizadas simulações em microcomputador tipo PC, utilizando o programa ANSYS versão 8.1. Cada modelo de arco externo reproduziu linhas de força que passaram (1) acima (AcCR), (2) abaixo (AbCR) e (3) no centro de resistência (CR) do molar permanente superior de um mesmo modelo portador de má oclusão de Classe II. A avaliação restringiu-se ao movimento inicial dos molares frente à força extrabucal de 4 Newtons. RESULTADOS: o movimento distal inicial dos molares, tendo como ponto de referência a mesial do tubo, foi maior na coroa do modelo AbCR (0,47x10-6), e maior na raiz do modelo AcCR (0,32x10-6), provocando inclinações da coroa para distal e mesial, respectivamente. No modelo CR, os pontos na coroa (0,15x10-6) e raiz (0,12x10-6) deslocaram-se para distal equilibradamente, resultando em movimento de translação. Em todos os modelos, numa vista oclusal, houve tendência de rotação distal inicial da coroa, porém no modelo CR esse movimento foi muito pequeno. No sentido vertical (Z), todos os modelos revelaram movimento extrusivo (AbCR= 0,18x10-6; CR= 0,62x10-6; AcCR= 0,72x10-6). CONCLUSÃO: a simulação computacional do uso de aparelho extrabucal com tração cervical revelou a ocorrência de movimento extrusivo e distal, podendo ser por inclinação distal de coroa, de raiz ou movimento de translação.

OBJECTIVE: To analyze maxillary molar displacement by applying three different angulations to the outer bow of cervical-pull headgear, using the finite element method (FEM). METHODS: Maxilla, teeth set up in Class II malocclusion and equipment were modeled through variational formulation and their values represented in X, Y, Z coordinates. Simulations were performed using a PC computer and ANSYS software version 8.1. Each outer bow model reproduced force lines that ran above (ACR) (1), below (BCR) (2) and through the center of resistance (CR) (3) of the maxillary permanent molars of each Class II model. Evaluation was limited to the initial movement of molars submitted to an extraoral force of 4 Newtons. RESULTS: The initial distal movement of the molars, using as reference the mesial surface of the tube, was higher in the crown of the BCR model (0.47x10-6) as well as in the root of the ACR (0.32x10-6) model, causing the crown to tip distally and mesially, respectively. On the CR model, the points on the crown (0.15 x10-6) and root (0.12 x10-6) moved distally in a balanced manner, which resulted in bodily movement. In occlusal view, the crowns on all models showed a tendency towards initial distal rotation, but on the CR model this movement was very small. In the vertical direction (Z), all models displayed extrusive movement (BCR 0.18 x10-6; CR 0.62 x10-6; ACR 0.72x10-6). CONCLUSIONS: Computer simulations of cervical-pull headgear use disclosed the presence of extrusive and distal movement, distal crown and root tipping, or bodily movement.

The purpose of the present study was to evaluate the shear bond strength to enamel and the adhesive remnant index (ARI) of both metallic and polycarbonate brackets bonded under different conditions. Ninety bovine permanent mandibular incisors were embedded in acrylic resin using PVC rings as molds and assigned to 6 groups (n=15). In Groups 1 (control) and 3, metallic and polycarbonate orthodontic brackets were, respectively, bonded to the enamel surfaces using Transbond XT composite according to the manufacturer's recommendations. In Groups 2 and 4, both types of brackets were bonded to enamel with Transbond XT composite, but XT primer was replaced by the OrthoPrimer agent. In Groups 5 and 6, the polycarbonate bracket bases were sandblasted with 50-mm aluminum-oxide particle stream and bonded to the enamel surfaces prepared under the same conditions described in Groups 3 and 4, respectively. After bonding, the specimens were stored in distilled water at 37ºC for 24 hours and then submitted to shear bond strength test at a crosshead speed of 0.5 mm/min. The results (MPa) showed no statistically significant difference between Groups 4 and 6 (p>0.05). Likewise, no statistically significant differences (p>0.05) were found among Groups 1, 2, and 5, although their results were significantly lower than those of Groups 4 and 6 (p<0.05). Group 3 had statistically significant lower bond strength than Groups 2, 4, and 6, but no statistically significant differences were found on comparison to Groups 1 and 5. A larger number of fractures at the bracket/composite interface were evidenced by the ARI scores. OrthoPrimer bonding agent yielded higher bond strength in the groups using either conventional or sandblasted polycarbonate brackets, which was not observed in the groups using metallic brackets.

Este estudo avaliou alterações pulpares em molares de ratos movimentados com amplitude de força de 0,4 N. Vinte e cinco ratos machos adultos Wistar (Rattus norvegicus, albinus) foram aleatoriamente alocados em 5 grupos (n=5), sendo 1 grupo controle não submetido à aplicação de força, e 4 grupos teste submetidos a 6, 12, 24 e 72 h de aplicação de força. Os grupos teste receberam uma mola fechada tipo coil spring de níquel-titânio com 5 mm de comprimento, fixada do primeiro molar superior direito até os incisos superiores do animal. A mola tipo coil spring foi utilizada para mesialização do primeiro molar. Após o período específico de movimentação de cada grupo experimental, os animais foram sacrificados e as peças contendo o dente movimentado e o seu tecido pulpar foram processadas e coradas com hematoxilina e eosina para análise histológica em microscopia de luz. Os resultados mostraram alteração da camada de odontoblastos com hipertrofia dessas células, principalmente na região mesial da polpa coronária; edema no tecido conjuntivo pulpar, evidenciado na região central da polpa; alteração vascular, com acúmulo de eritrócitos e leucócitos dentro da luz dos vasos, principalmente na raiz mesial do dente movimentado. Essas alterações foram menos evidentes no período de 72 h. Dessa forma, conclui-se que o movimento dentário provoca alterações pulpares teciduais compatíveis com processo inflamatório, as quais são reversíveis se a agressão não ultrapassar o limite de tolerância do tecido.

This study evaluated pulp changes in molars of rats submitted to tooth movement by application of a 0.4 N force. Twenty-five adult male Wistar rats (Rattus norvegicus, albinus) were randomly assigned to 5 groups (n=5), being one control group not submitted to force application, and four study groups of 6, 12, 24 and 72 h of force application. The study groups received a 5-mm long nickel-titanium closed coil spring, placed from the right maxillary first molar to the maxillary incisors of each animal. The coil spring was used for mesial inclination of the first molar. After the specific period of tooth movement of each study group, the animals were sacrificed and specimens containing the teeth submitted to movement were processed and stained with hematoxylin and eosin for histological analysis under light microscopy. The results demonstrated alteration of the odontoblastic layer, with hypertrophy of odontoblasts especially at the mesial area of the coronal pulp, edema of the pulp connective tissue in the central area of the pulp, and vascular alteration with accumulation of erythrocytes and leukocytes inside the vessels, especially at the mesial root of the moved teeth. These changes were less remarkable for the 72-h period. Thus, it may be concluded that tooth movement yielded pulpal tissue alterations compatible with an inflammatory process, which are reversible if the aggression is not more intense than the physiological limit of tissue tolerance.

A relação entre respiração e morfologia dento-facial tem sido discutida e investigada sob vários aspectos. O objetivo deste estudo foi verificar a morfologia dentária e esquelética de crianças respiradoras nasais e bucais. Trinta e cinco crianças, com idade entre 7 e 10 anos, foram submetidas às avaliações ortodônticas e otorrinolaringológicas e foram, então, divididas em 2 grupos: um grupo contendo quinze crianças respiradoras nasais e outro, com vinte respiradores bucais. Cada uma das crianças foi submetida à análise cefalométrica. O tratamento estatístico indicou que a respiração alterada está associada com: 1) retrusão maxilo-mandibular em relação à base do crânio, nos pacientes respiradores bucais; 2) os ângulos SNGoGn e NSGn foram maiores no grupo de respiradores bucais; 3) a inclinação axial dos incisivos superiores e inferiores e o ângulo interincisal não foram diferentes entre os grupos. Não houve diferença estatisticamente significante nas alturas molares superiores e inferiores entre os dois grupos avaliados.

The relationship between dentofacial morphology and respiration has been debated and investigated from various approaches. The aim of this study was to verify the skeletal and dental relationship of mouth and nose breathing children. Thirty-five children, 7 to 10 years of age, were submitted to orthodontic and otorhinolaryngologic evaluations and were separated into 2 groups: 15 nose breathers and 20 mouth breathers. Each subject underwent a cephalometric radiograph analysis. Statistical analysis (Mann-Whitney U test) indicated that changed mode of breathing was associated with 1) maxillo-mandibular retrusion in relation to the cranial base in the mouth breathers; 2) the SNGoGn and NSGn angles were greater in the mouth breathing group; 3) incisor inclination in both jaws and the interincisal angle were not different between groups. There was no statistically significant difference in the maxillary and mandibular molar heights between the nose breathers and mouth breathers.
