Abstract Introduction Cisplatin damages the auditory system and is related to the generation of free radicals. Glutathione peroxidase is an endogenous free radicals remover. Objective To investigate the mechanisms involved in otoprotection by N-acetylcys- teine through the expression of glutathione peroxidase in outer hair cells from rats treated with cisplatin. Methods Male Wistar rats were intraperitoneally injected with cisplatin (8 mg/Kg) and/or received oral administration by gavage of N-acetylcysteine (300 mg/Kg) for 3 consecutive days. On the 4th day, the animals were euthanized and beheaded. The tympanic bullae were removed and prepared for scanning electron microscopy and Results Among the groups exposed to ototoxic doses of cisplatin, there was an increase in glutathione peroxidase immunostaining in two groups, the one exposed to cisplatin alone, and the group exposed to both cisplatin and N-acetylcysteine. Conclusion The expression of glutathione peroxidase in the outer hair cells of rats exposed to cisplatin showed the synthesis of this enzyme under cellular toxicity conditions.

Resumo: A paca (Cuniculus paca), um dos maiores roedores da fauna brasileira, possui características inerentes à sua espécie que podem contribuir como uma nova opção de animal experimental; assim, considerando-se que há crescente busca por modelos experimentais apropriados para pesquisas audiológicas e otológica cirúrgicas foram analisados e descritos em detalhes a anatomia macroscópica e ultraestrutural da orelha desse roedor. Para o estudo, utilizaram-se 15 animais adultos provenientes do plantel do Setor de Animais Silvestres da Faculdade de Ciências Agrárias e Veterinárias, Unesp-Jaboticabal, Jaboticabal/SP. Após anestesia e eutanásia, avaliou-se toda a composição da orelha externa, registrando-se e descrevendo-se os detalhes, também se dissecou a região temporal para melhor visibilização e detalhamento da bula timpânica e estas foram removidas e abertas a fim de expor as estruturas da orelha, as quais foram analisadas, macroscopicamente e ultraestruturalmente. O pavilhão auricular apresenta forma triangular e côncava com cristas irregulares e ápice pontiagudo; o conduto auditivo externo é sinuoso em seu trajeto até a membrana timpânica; a bula timpânica encontra-se na parte posterior-inferior do crânio; a orelha média é formada por uma região cavitária preenchida por estruturas ósseas e membranosas. É delimitada pela membrana timpânica e as janelas redonda e oval, sendo a membrana timpânica de forma plana e que veda todo o conduto auditivo. A anatomia da orelha da paca é semelhante à da cobaia e do ponto de vista de modelo experimental apresenta grandes vantagens em comparação com a orelha do rato.

Abstract: Paca (Cuniculus paca), one of the largest rodents of the Brazilian fauna, has inherent characteristics of its species which can conribute as a new option for animal experimantation. As there is a growing demand for suitable experimental models in audiologic and otologic surgical research, the gross anatomy and ultrastructural ear of this rodent have been analyzed and described in detail. Fifteen adult pacas from the Wild Animals Sector herd of Faculdade de Ciências Agrárias e Veterinárias, Unesp-Jaboticabal, were used in this study. After anesthesia and euthanasia, we evaluated the entire composition of the external ear, registering and ddescribing the details; the temporal region was often dissected for a better view and detailing of the tympanic bulla which was removed and opened to expose the ear structures analyzed mascroscopically and ultrastructurally. The ear pinna has a triangular and concave shape with irregular ridges and sharp apex. The external auditory canal is winding in its path to the tympanic mebrane. The tympanic bulla is is on the back-bottom of the skull. The middle ear is formed by a cavity region filled with bone and membranous structures bounded by the tympanic membrane and the oval and round windows. The tympanic membrane is flat and seals the ear canal. The anatomy of the paca ear is similar to the guinea pig and from the viewpoint of experimental model has major advantages compared with the mouse ear.

INTRODUÇÃO: O condicionamento auditivo consiste da pré-exposição de um agente lesivo em baixos níveis para proteger contra uma posterior apresentação lesiva. OBJETIVO: Confirmar se o condicionamento com um agente diferente do utilizado para causar o trauma pode ser efetivo. MÉTODO: Estudo experimental com 17 cobaias albinas divididas como a seguir- grupo Som: exposto a um ruído branco de 85 dB centrado em 4 kHz, 30 minutos por dia, por 10 dias consecutivos; grupo Cont: administração intramuscular de gentamicina 160 mg/kg por dia, por 10 dias consecutivos; grupo Expt: condicionado com ruído como o grupo Som. Após cada exposição ao ruído, recebeu gentamicina similarmente ao grupo Cont. Os animais foram avaliados por emissões otoacústicas produto de distorção (EOAPDs), potencial evocado auditivo de tronco encefálico (PEATE) e microscopia eletrônica de varredura (MEV). RESULTADOS: Os animais que foram condicionados com ruído não mostraram qualquer efeito protetor quando comparados com os que receberam apenas a gentamicina em doses ototóxicas. Esta ausência de proteção foi observada tanto funcionalmente quanto morfologicamente. CONCLUSÃO: Os autores concluíram que o condicionamento com ruído branco a 85 dB por 30 minutos, por dia por 10 dias consecutivos, não protege contra uma administração de gentamicina 160 mg/kg/dia, por 10 dias consecutivos.

INTRODUCTION: Auditory conditioning consists of the pre-exposure to low levels of a potential harmful agent to protect against a subsequent harmful presentation. OBJECTIVE: To confirm if conditioning with an agent different from the used to cause the trauma can also be effective. METHOD: Experimental study with 17 guinea pigs divided as follows: group Som: exposed to 85 dB broadband noise centered at 4 kHz, 30 minutes a day for 10 consecutive days; group Cont: intramuscular administration of gentamicin 160 mg/kg a day for 10 consecutive days; group Expt: conditioned with noise similarly to group Som and, after each noise presentation, received gentamicin similarly to group Cont. The animals were evaluated by distortion product otoacoustic emissions (DPOAEs), brainstem auditory evoked potentials (BAEPs) and scanning electron microscopy. RESULTS: The animals that were conditioned with noise did not show any protective effect compared to the ones that received only the ototoxic gentamicin administration. This lack of protection was observed functionally and morphologically. CONCLUSION: Conditioning with 85 dB broadband noise, 30 min a day for 10 consecutive days does not protect against an ototoxic gentamicin administration of 160 mg/kg a day for 10 consecutive days in the guinea pig.

A oxigenação hiperbárica têm favorecido a prevenção e o tratamento de afecções auditivas como a ototoxicidade. OBJETIVO: Estudar os efeitos da oxigenação hiperbárica em lesão ototóxica promovida pela amicacina. Forma de estudo: Experimental. MÉTODO: Avaliados aspectos funcionais de 12 cobaias albinas por meio das emissões otoacústicas produtos de distorção e do potencial evocado auditivo de tronco encefálico, antes e após o uso de amicacina (600 mg/kg/dia) e das sessões com oxigenação hiperbárica (2 ATA, 60 minutos). Aspectos morfológicos foram avaliados por meio de microscopia eletrônica de varredura. Grupos de estudo com três animais: grupo 1 - solução salina + oxigenação hiperbárica; grupo 2 - amicacina 8 dias; grupo 3 - amicacina + 7 dias de repouso e grupo 4 - amicacina + oxigenação hiperbárica. RESULTADOS: Grupo 1 apresentou preservação da funcionalidade e da morfologia durante todo experimento. Grupo 2 demonstrou, ao final do experimento, lesões estatisticamente significantes das células ciliadas com alterações funcionais. Grupos 3 e 4 apresentaram alterações estatisticamente significantes dos aspectos funcionais e morfológicos após o uso da amicacina, mantendo estas alterações após os procedimentos propostos. CONCLUSÃO: A oxigenação hiperbárica não promoveu alterações na morfologia das células ciliadas da cóclea e aos limiares eletrofisiológicos das cobaias submetidas à amicacina.

Hyperbaric oxygen therapy (HBOT) has enhanced the prevention and treatment of auditory ailments such as ototoxicity. OBJECTIVE: To study the effects of HBOT upon ototoxic injuries produced by amikacin. METHOD: This experimental study included 12 albino guinea pigs, whose auditory function was assessed through distortion product otoacoustic emissions (DPOAEs) and brainstem auditory evoked potentials (BAEPs) before and after the administration of amikacin (600 mg/kg/day) and HBOT sessions (2 ATA, 60 minutes). Morphological features were analyzed through scanning electron microscopy. Subjects were divided into four groups, as follows: group 1 - saline solution + HBOT; group 2 - amikacin for 8 days; group 3 - amikacin + seven days of rest; and group 4 - amikacin + HBOT. RESULTS: Group 1 subjects had preserved function and morphology throughout the experiment; Group 2 subjects had statistically significant levels of hair cell injury and functional impairment; Subjects on groups 3 and 4 had statistically significant functional and morphological impairment after the administration of amikacin, which were still present after the proposed procedures had been carried out. CONCLUSION: Hyperbaric oxygen therapy did not change the cochlear hair cell morphology or the electro-physiological thresholds of the guinea pigs given amikacin.

Há comprovação de que o fenômeno de resistência ocorre quando a dose não lesiva da amicacina protege as células ciliadas contra a ototoxicidade da própria amicacina. OBJETIVO: O objetivo deste trabalho é verificar se o fenômeno de resistência é temporalmente persistente. MÉTODO: Estudo experimental com 14 cobaias albinas (Cavia porcellus) divididas em três grupos. Avaliação da função auditiva por emissões otoacústicas por produto de distorção (EOAPD): na pré-exposição à amicacina, no 15º dia de aplicação da dose não lesiva, no final da aplicação da dose lesiva e antes da decapitação. RESULTADOS: O Grupo A (controle) apresentou função auditiva e padrão histológico normais. No Grupo B (amicacina 20mg/kg/dia intramuscular por 30 dias e dose lesiva (400 mg/kg/dia) por 12 dias) e no Grupo C (mesmo esquema do grupo B, porém mantidos por 60 dias e sacrificados), as OEA-PD confirmaram função auditiva normal no período pré-exposição e manutenção do padrão após dose não lesiva, porém, houve perda importante da função auditiva após término do período de aplicação da dose lesiva. CONCLUSÃO: Não houve manutenção do fenômeno da autodefesa estendida por um período de 30 a 60 dias após a aplicação de doses lesivas de amicacina.

There is evidence that a "resistance phenomenon" occurs when a none-damaging dose of amikacin protects the hair cells from ototoxicity. Our goal is to prove that this resistance is persistent. METHOD: Experimental study - 14 albino guinea pigs (Cavia porcellus) divided into three groups. The auditory function was assessed by distortion product otoacoustic emissions (DPOAE): before exposure to amikacin, on the 15th day after the non-damaging dose was injected, at the end of the damage dose injection and prior to decapitation. RESULTS: Group A (control) presented normal hearing and histological pattern. Group B (amikacin 20mg/kg/day (IM) for 30 days and affecting dose (400 mg / kg / day) for 12 days and Group C (same protocol of Group B, but kept for 60 days and slaughtered), the DPOAE confirmed normal auditory function in the pre-exposure and maintenance of the standard-dose; however, significant loss of auditory function after the end of the damaging dose injection. CONCLUSION: The protection phenomenon did not extended for a period of 30 to 60 days after the application of damaging doses of amykacin.

Pensando em melhorar a qualidade de vida dos pacientes com doenças do humor vítreo, os oftalmologistas começaram a utilizar recentemente implantes biodegradáveis com corticoide. Estes mesmos implantes podem ser uma alternativa no tratamento da RSC e, para isso, realizamos um estudo experimental em seios maxilares de coelhos. OBJETIVO: Avaliar histologicamente a mucosa de seio maxilar de coelhos após a colocação de implante biodegradável de prednisolona. MÉTODO: Dezoito coelhos foram divididos aleatoriamente em dois grupos: Grupo 1: no seio maxilar esquerdo foi inserido um implante biodegradável com prednisolona; Grupo 2: No seio maxilar esquerdo foi inserido um implante biodegradável sem medicação. Os seios maxilares do lado direito serviram como controle. Após 7, 14 e 28 dias foram escolhidos aleatoriamente três coelhos de cada grupo e a resposta tecidual inflamatória foi avaliada. RESULTADOS: Foi encontrada diferença não significativa de inflamação na mucosa, quando comparamos o grupo de coelhos que receberam implantes com e sem medicação com o grupo controle; ou quando comparamos o grupo que recebeu implante com prednisolona com o grupo que recebeu implante sem medicação. CONCLUSÃO: Não foram observados sinais de toxicidade ou inflamação na mucosa do seio maxilar do coelho à presença do implante com ou sem prednisolona.

In an attempt to improve the quality of life of patients with vitreous humor disease, ophthalmologists began offering steroid-eluting biodegradable implants to their patients. These implants can be used as an alternative treatment for CRS and this is why this experimental study was carried out on rabbit maxillary sinuses. OBJECTIVE: This study aims to assess the histology of the mucosa of the maxillary sinuses of rabbits after the placement of a prednisolone-eluting biodegradable implant. METHOD: Eighteen rabbits were randomly divided into two groups: group 1 - subjects had drug-eluting implants placed on their left maxillary sinuses; group 2 - subjects had non-drug-eluting implants placed on their left maxillary sinuses. The right maxillary sinuses served as the controls. After seven, 14, and 28 days three rabbits in each group were randomly picked to have their tissue inflammatory response assessed. RESULTS: Levels of mucosal inflammation were not significantly different between the groups with and without drug-eluting implants and the control group, or when the groups with drug-eluting implants and non-drug-eluting implants were compared. CONCLUSION: Signs of toxicity or mucosal inflammation were not observed in the maxillary sinuses of rabbits given prednisolone-eluting implants or non-drug-eluting implants.

A identificação precoce das alterações auditivas possibilita a intervenção ainda no "período crítico" e ideal de estimulação da linguagem e da audição. A ototoxicidade infantil é um tema bastante controverso. Têm sido relatadas percentagens variáveis de casos de ototoxicidade em crianças com vários antibióticos aminoglicosídeos. Os principais grupos pediátricos que recebem antibióticos aminoglicosídeos são recém-nascidos com infecções graves na UTI neonatal. OBJETIVOS: Verificar o aspecto funcional das células ciliadas externas da cóclea a esquemas terapêuticos utilizados para o tratamento de infecções no período neonatal. FORMA DE ESTUDO: Experimental. MATERIAL E MÉTODO: Foram estudadas 26 cobaias albinas, através das emissões otoacústicas por produto de distorção, prévia e posteriormente a aplicação de gentamicina. RESULTADOS: Em todas as avaliações, o estado funcional das células ciliadas externas, estudadas pelas emissões otoacústicas por produto de distorção, mostraram-se preservadas. CONCLUSÃO: Neste experimento não foram observadas alterações no funcionamento das células ciliadas externas de cobaias albinas sob tratamento com gentamicina nas doses de 4 mg/Kg/dia e 2,5 mg/Kg/dia a cada 12 horas, por 10 e 14 dias.

The early identification of hearing impairment allows for an intervention still in the "critical" and ideal period of hearing and language stimulation. Pediatric ototoxicity is a very controversial topic. There have been variable percentages of ototoxicity cases in children with different aminoglycosides antibiotics. The main pediatric groups whom receive aminoglycosides are newborns with severe infections on the neonatal ICU. AIM: to check the functional aspect of the cochlear external hair cells and treatment regimens used to treat infections during the neonatal period. STUDY DESIGN: Experimental. Materials and Methods: we studied 26 albino guinea pigs, through distortion product otoacoustic emissions, before and after the use of gentamicin. RESULTS: in all the assessments, the external hair cells functional status, studied by means of the distortion product otoacoustic emissions, proved preserved. CONCLUSION: In the present study, we did not notice changes in outer hair cell function in the albino guinea pigs treated with gentamicin in the doses of 4 mg/Kg/day and 2.5 mg/Kg/day every 12 hours for 10 and 14 days.

A radiação pode causar lesão na orelha interna podendo provocar surdez sensório-neural e inclusive levar à anacusia. A amifostina é uma substância citoprotetora seletiva de tecidos sadios, amplamente utilizada durante a radio e quimioterapia de tumores malignos. OBJETIVO: O objetivo deste estudo experimental foi verificar se existe efeito antioxidante e radioprotetor da amifostina no órgão de Corti de cobaias albinas irradiadas em região de cabeça e pescoço. MATERIAL E MÉTODO: O estudo realizado envolveu quatro grupos de animais: um grupo foi submetido à irradiação em dose única de 350cGy. Dois grupos receberam a mesma dose de radiação, porém receberam doses de 100 e 200mg/kg de amifostina, 30 minutos antes da irradiação. Um grupo recebeu apenas amifostina, na dose de 200mg/Kg. Todas as cobaias foram sacrificadas 30 dias após o experimento e suas bulas retiradas para estudo em microscópio de varredura. RESULTADO: O grau de lesão das células ciliadas externas foi menor nos dois grupos que receberam a amifostina que no grupo apenas irradiado. Não foi encontrada diferença de proteção entre os grupos que receberam doses de 100 e 200mg/kg de amifostina. Não houve lesão no grupo que recebeu apenas amifostina. CONCLUSÃO: Amifostina mostrou ser um radioprotetor do órgão de Corti de cobaias albinas irradiadas.

Radiation can cause damage to the inner ear, from a simple hearing loss all the way to profound deafness. Amifostine is a cytoprotective substance extensively used during radio-chemotherapy for malignant tumors. AIM: the objective of the present investigation was to establish the antioxidant and radioprotective effects of amifostine on the organ of Corti of albino guinea pigs irradiated in the head and neck region. MATERIALS AND METHODS: An experimental study conducted on four groups of guinea pigs were used; One group received only amifostine, one group was submitted to a single dose of 350 cGy and the other two were similarly irradiated but received amifostine doses of 100 or 200 mg/kg. All animals were slaughtered 30 days after the experiment, their bullae were removed and the damaged outer hair cells were counted. RESULT: The extent of injury was lower in the outer hair cells of the two groups treated with amifostine compared to the group that was only irradiated. There was no difference between the group treated with 100 and 200 mg/kg of amifostine. The group that received only amifostine had no cochlear damage. CONCLUSION: Amifostine is an effective cytoprotective substance in the Organ of Corti of irradiated guinea pigs.

O uso de mostras animais é importante na pesquisa otológica e o conhecimento da anatomia de sua orelha permite sua utilização adequada. OBJETIVO: Estudar a anatomia da orelha da cobaia e do rato por microscopia óptica de luz (MOL) e microscopia eletrônica de varredura (MEV) e suas vantagens anatômicas na pesquisa otológica básica. MATERIAL E MÉTODO: Os ossos temporais, as bulas timpânicas e cócleas de três cobaias e ratos albinos foram fotografados e analisados ao MOL e MEV. RESULTADOS: O rato não é tão simples de manipular como a cobaia, e freqüentemente apresenta otite média. O rato apresenta uma junção frágil da bula timpânica, duas e meia espiras na cóclea e a membrana timpânica não veda todo o conduto auditivo externo. A cobaia possui uma bula inteiriça, martelo e bigorna fundidos e três e meia espiras na cóclea. Pela MEV a cobaia e o rato possuem Membrana Tectória, Membrana de Raissner e o Órgão de Corti. As Células de Hensen estão presentes somente na cobaia. CONCLUSÃO: A cobaia foi considerada de fácil manipulação para a microdissecção, pelo tamanho e rigidez do osso temporal, e para experimentos cirúrgicos envolvendo o estribo, janela oval e a membrana timpânica. Pela MEV nota-se semelhança entre cobaia e rato, podendo ambos serem utilizados em estudos da orelha interna.

The use of animal samples is important in otologic research and understanding the anatomy of their ears help make proper use of them in research projects. AIM: to study guinea pig's and rat's ears under light microscopy(LM) and scanning electron microscopy(SEM) and understand their anatomical advantages in basic otologic research. MATERIALS AND METHODS: The temporal bones, tympanic bullas and cochleas from three albino guinea pigs and rats were photographed and analyzed under LM and SEM. RESULTS: Rats aren't as simple to handle as guinea pigs, and often present with otitis media. Rats have a fragile junction of the tympanic bulla, two and half turns in the cochlea, and their tympanic membranes do not seal off the entire external auditory canal. Guinea pigs have full bullas, their incus and malleus are fused and they have three and half cochlear turns. Under SEM, guinea pigs and rats have Tectori Membrane, Raissner's Membrane and the Organ of Corti. Only guinea pigs have Hensen's Cells. CONCLUSION: Guinea pigs were considered easy to handle for microdissection purposes because of the size and robustness of their temporal bones, and for surgical experiments involving the stapes, the oval window and the tympanic membrane. Under SEM there are similarities guinea pigs and rats, and both can be used in inner ear studies.

There is evidence that the auditory evoked potential (AEP) is altered by malnutrition both in laboratory animals and in humans. The objective of the present study was to determine whether changes in the AEP caused by malnutrition could be reversed by nutritional rehabilitation and sensorymotor and environmental stimulation during hospitalization. Six children aged 5-33 months with severe malnutrition (kwashiorkor, marasmus and marasmic-kwashiorkor) were admitted to the Pediatric Ward of a University Hospital. Normal age and sex-matched children from the hospital day-care center were enrolled as a control group. The AEP was tested in an electrically and acoustically isolated room using a Nicolet CA 2000 microcomputer. Clicks of 90; 80; 70 and 60 dBn HL were presented through earphones. The results suggest that malnutrition leads to an increase in wave I latencies in patients with marasmus, and in waves I, III and V in those with kwashiorkor or marasmic-kwashiorkor type at 90 dB HL. At discharge, all but one patient with kwashiorkor showed reduced latencies of waves I, III and V compared to the values on admission. Despite the small sample, these preliminary results pointed out that the process of sensory stimulation used in our study in a properly directed, systematic and individualized manner showed encouraging results in terms of AEP recovery in these children.

A ototoxicidade ainda é um desafio para medicina. A descoberta dos mecanismos endógenos autoprotetores das células ciliadas externas associados a métodos de avaliação funcional e ultra-estrutural das mesmas abriu nova perspectiva no entendimento e controle destes mecanismos. OBJETIVO: O trabalho objetivou determinar se subdoses de gentamicina protegia contra ototoxicidade da amicacina baseado nestes mecanismos e determinar se a amplitude das emissões otoacústicas teria correlação com grau de integridade das células ciliadas. MATERIAL E MÉTODO: Estudo experimental. Utilizando 31 cobaias, administrou-se soro fisiológico, gentamicina e amicacina, isoladamente e associadas, via intramuscular, por 12, 30 e 42 dias. Pesquisa de emissões otoacústicas foi realizada no início e final do experimento, comparado com estudo da integridade coclear, por microscopia eletrônica. RESULTADOS: Subdoses de gentamicina não protegeram a orelha interna contra toxicidade da amicacina; diminuições da amplitude das emissões otoacústicas apresentaram forte correlação com aumento de lesões das células ciliadas. CONCLUSÃO: Os achados contribuem para o entendimento dos mecanismos de ototoxicidade e otoproteção da orelha interna. A determinação da correlação entre amplitude de emissões e integridade celular tem grande importância no acompanhamento das lesões de células ciliadas, com possível aplicação no monitoramento de ototoxicidade por drogas em humanos.

Ototoxicity is still a challenge to medicine. The discovery of self-protecting endogenous mechanisms of the outer hair cells associated with their functional and ultra-structural assessment methods has opened new horizons in the understanding and controlling of these mechanisms. AIM: this paper aimed at establishing whether or not underdoses of gentamicin could protect the inner ear against the harmful effects of amikacin, based on these protection mechanisms and determine if the otoacoustic emission amplitudes could be associated with the level of hair cell integrity. MATERIALS AND METHODS: Experimental study. We used 31 guinea pigs. They were injected with saline solution, gentamicin and amikacin, alone and in combinations -intramuscular injections - during 12, 30 and 42 days. The otoacoustic emissions were recorded in the beginning and at the end of the experiment, comparing it with the cochlear integrity study carried out by electron microscopy. RESULTS: gentamicin underdoses did not protect the inner ear against amikacin toxicity; the reduction in otoacoustic emissions was strongly associated with an increase in hair cell lesions. CONCLUSION: these findings help understand inner ear otoprotection and ototoxicity. Establishing the correlation between the emissions amplitude an cell integrity plays an important role in the follow up of hair cell damage, with possible monitoring of ototoxicity caused by drugs in humans.

O presente trabalho refere-se ao estudo dos resultados clínicos e histológicos obtidos após a turbinectomia inferior parcial (TIP), cirurgia indicada no tratamento da obstrução nasal crônica causada pela hipertrofia das conchas nasais inferiores. MATERIAL E MÉTODOS: Foram estudados vinte pacientes, divididos em dois grupos de dez cada (grupos A e B), submetidos à TIP, associada à septoplastia ou não. Os pacientes foram reavaliados clinicamente e histologicamente (com biópsia das áreas regeneradas das conchas inferiores), em dois períodos pós-operatórios diferentes: um grupo após oito a doze meses (grupo A) e outro após dois anos de TIP (grupo B). RESULTADOS: Os resultados clínicos mostraram-se satisfatórios para o alívio da obstrução nasal no grupo A, e insatisfatórios no grupo B. Entretanto, melhores resultados histológicos, com maior recuperação e diferenciação epitelial da mucosa regenerada das conchas inferiores após a TIP foram observados no grupo B, com sua ultraestrutura ciliar normal. CONCLUSÕES: A cirurgia revelou ser eficaz a curto, mas não em médio prazo, apesar da recuperação histológica ter sido importante.

We report clinical and histological results obtained after partial inferior turbinectomy (PIT), surgery indicated for the treatment of chronic nasal obstruction. METHODS: Twenty patients were divided into two groups submitted to PIT plus septoplasty and PIT alone. The patients were reassessed clinically and histologically by means of a biopsy of the regenerated areas in the inferior turbinates at two different times after PIT, i.e., after 8 to 12 months (group A) and after two years (group B). RESULTS: The clinical results proved to be satisfactory for the relief of nasal obstruction in group A and unsatisfactory in group B. However, better histological results with better recovery and epithelial differentiation of the regenerated mucosa of the inferior turbinates after PIT were observed in group B. CONCLUSION: Surgery proved to be effective on a short-term but not on a long-term basis, and histological recovery did not accompany improvement of clinical signs and symptoms.

A Cisplatina é uma potente droga antineoplásica, largamente utilizada para o tratamento do câncer, tanto em adultos quanto em crianças. Dentre seus efeitos colaterais, a ototoxicidade se apresenta como um dos mais importantes e leva à perda auditiva irreversível, bilateral, para as altas freqüências (4KHz -8KHz). Estudos têm tentado identificar drogas que, associadas à cisplatina, possam atuar como otoprotetores. Sabe-se que o mecanismo da ototoxicidade pela cisplatina está relacionado a alterações nos mecanismos antioxidantes das células ciliadas, principalmente as células ciliadas externas da cóclea. A amifostina tem conhecida ação antioxidante, com conhecido efeito otoprotetor aos efeitos lesivos da radioterapia. OBJETIVO: Nossa proposta foi avaliar através de emissões otoacústicas, por produtos de distorção (EOAPD) e por microscopia eletrônica de varredura (MEV), a existência de possível efeito otoprotetor da amifostina no tratamento com cisplatina. FORMA DE ESTUDO: Experimental. MATERIAL E MÉTODO: O estudo foi realizado em cobaias albinas, que foram divididas em três grupos: Grupo 1: 6 animais -12 orelhas - cisplatina 8,0 mg/Kg/dia (via intraperitoneal) por três dias; Grupo 2: 6 animais - 12 orelhas - amifostina 100 mg/Kg/ dia (via intraperitoneal) e 90 minutos após, cisplatina 8,0 mg/Kg/dia (via intraperitoneal) por três dias; Grupo 3: 03 animais - 06 orelhas - amifostina 100 mg/Kg/dia (via intraperitoneal) por três dias. RESULTADO: Encontramos EOAPD presentes e células ciliadas externas presentes, sem lesão anatômica a MEV, nos grupos 2 e 3. Concluímos que a amifostina, por sua ação antioxidante, atua como otoprotetor a ototoxicidade pela cisplatina. No entanto, seu uso não é recomendável nos casos de tumores potencialmente curáveis, por não se saber exatamente a influência da cisplatina na eficácia da quimioterapia.

Cisplatin is an antineoplastic drug for cancer treatment in children and adults. The side effects of cisplatin ototoxicity are significant: irreversible bilateral hearing damage to high frequencies (4 kHz - 8 kHz). Reports recognize some drugs that are associated with cisplatin to obtain an otoprotector effect. The ototoxicity mechanisms of cisplatin are related to injury of hair cell oxidation mechanism, especially of outer hair cells. AIM: Using otoacoustic emissions distortion products (DPOEA) and scanning electron microscopy we intended to verify the action of amifostine, a radioprotective drug that has well known antioxidant characteristics and otoprotector effects to cisplatin injury. STUDY DESIGN: Experimental. MATERIAL AND METHODS: We used an experimental guinea pig model. The study was performed as follows: group 1: 6 animals, 12 ears, cisplatin 8.0 mg/Kg/day (IP), 3 days. Group 2: 6 animals, 12 ears, amifostine 100 mg/Kg/day (IP) and after 90 minutes, cisplatin 8.0 mg/Kg/day (IP), 3 days and group 3: 3 animals, 6 ears, amifostine 100 mg/Kg/day (IP), 3 days. RESULTS: DPOEA were present before and after treatment in groups 2 and 3. The normal cilium architecture of outer hair cells was supported in all cochlear turns in groups 2 and 3. We concluded that amifostine has a potential otoprotector effect against cisplatin ototoxicity and could be used in clinical trials.

OBJETIVO: Nossa proposta foi de avaliar através de emissões otoacústicas, por produtos de distorção (EOAPD) e por microscopia eletrônica de superfície (ME), a ação do extrato de ginkgo biloba (EGB 761), que tem conhecida ação antioxidante, como possível otoprotetor, utilizando como modelo experimental cobaias albinas. FORMA DE ESTUDO: Experimental. MATERIAL E MÉTODO: Observamos EOAPD presentes pré e pós tratamento no grupo EGB (100 mg/Kg/dia via oral) e 90 minutos após cisplatina (80 mg/Kg/dia via intraperitoneal) por 8 dias. RESULTADO: Houve também manutenção da arquitetura ciliar nas células ciliadas externas em todas as espiras da cóclea, enquanto que no grupo tratado somente com cisplatina (80 mg/Kg/dia via intraperitoneal) por 8 dias, houve desaparecimento das EOAPD pós tratamento, com desaparecimento dos cilios das células ciliadas externas e distorção na arquitetura dos cílios remanescentes à ME. CONCLUSÃO: Concluímos que a EGB, por sua ação antioxidante, atua como fator otoprotetor à ototoxicidade pela cisplatina, devendo ser testada tal ação na prática clínica em pacientes que utilizam a cisplatina, pois o uso do EGB está extremamente difundido no tratamento de diferentes doenças.

AIM: We intend to studies using otoacoustic emissions - distortion products (DPOEA) and scanning microscopy to verify the action of ginkgo biloba (GBE-761) that has well known antoxidizing characteristics, that can function like otoprotector effects. STUDY DESIGN: Experimental. MATERIAL AND METHOD: We use an experimental guinea pig model. We found DPOEA positive before and after treatment in the GBE group (100 mg/ Kg/ day - oral) and after 90 minutes cisplatin (8,0 mg/ Kg/ day - intraperitoneal - 8 consecutive days). RESULTS: The normal cilium architecture of outer hair cells was supported in all cochlear spirals and in the group treated only with cisplatin (8,0 mg/ Kg/ day - intraperitoneal - 8 consecutive days), the DPOEA was not present and strong injury to cilium of outer hair cells showed cilium disorders upon scanning microscopy. CONCLUSION: We conclude that GBE has a potential otoprotector effect against cisplatin ototoxycity and could be used in clinical trials.

Objetivo: Este estudo teve por objetivo verificar a ocorrência de otoproteção contra a ototoxicidade da amicacina em cobaias. Forma de estudo: experimental. Material e método: Foram estudadas as células ciliadas externas do órgão de Corti de 31 animais por meio de microscopia eletrônica de varredura (JEOL-JMS 5200). Foram estudados 4 grupos: A, controle; B, amicacina 20 mg/kg/dia por 30 dias; C, amicacina 400 mg/Kg/ dia 12 dias; grupo D, amicacina 20 mg/kg/dia por 30 dias seguido de 400 mg/kg/dia por 12 dias. Resultados: Nos grupos A e B, as células estavam normais em todas as cócleas; no grupo C houve lesões extensas nas duas espiras mais basais; no grupo D houve redução significante das lesões nas duas mais basais em comparação com o grupo C que recebeu a dose ototóxica de amicacina isoladamente. Conclusão: Conclui-se que a dose não ototóxica de amicacina administrada previamente à dose ototóxica do mesmo antibiótico tem efeito protetor, estatisticamente significante, nas duas espiras mais basais das cócleas de cobaias.

Aim: The objective of the present study was to determine the occurrence of self-protection of hair cells against the ototoxicity of amikacin in guinea pigs. Study design: experimental. Material and method: The outer hair cells of the organ of Corti of 31 animals were studied by scanning electron microscopy. The animals were divided into four groups: A, control; B, amikacin, 20 mg/kg/day for 30 days; C, amikacin, 400 mg/kg/day for 12 days; group D, amikacin, 20 mg/kg/day for 30 days followed by 400 mg/kg/day for 12 days. Results: In groups A and B, the cells were normal in all cochleae, in group C there were extensive lesions of the two more basal turns, in group D there was a significant reduction of lesions in the two more basal turns compared to group C, which had received the ototoxic dose of amikacin alone. Conclusion: We conclude that the non-ototoxic dose of amikacin administered before the ototoxic dose of the same antibiotic had a statistically significant protective effect on the two more basal turns of the guinea pig cochlea.
