Abstract Purpose To compare the operative outcomes of laparoscopic surgical treatment for bowel endometriosis in a public teaching hospital versus in a private referral hospital. Methods The indications for surgery, type and time of operation, length of hospital stay, need for a temporary stoma, rate of conversion to open surgery, and postoperative complications were evaluated. Results One hundred eighty-one patients were included (150 patients, 82.9%, in a private hospital). In the private hospital, there were more patients with infertility [56% vs. 29%; P=0.01] as an indication for surgery) and segmental resection was more common in the private hospital (48% vs. 29%, p=0.05). The average operative time (211.9±83.4 minutes vs. 128 ± 55 minutes, p<0.001) as well as the length of hospital stay (3.97±1.7 days vs. 1.56±0.85 days, p<0.001) was higher in the public hospital; the rate of conversion to open surgery was significantly lower in the private hospital (2% vs. 32.3%, p<0.001). Operations performed at the public hospital were associated with higher rates of postoperative complications (Clavien-Dindo II and II) (38.7% x 11.3%, p=0.021; OR 3.2, CI 95% 1.2-8.0). Conclusion Laparoscopic surgery in private centers was associated with reductions in major complications, surgical times, lengths of stay and rates of conversion to open surgery compared to that in public teaching hospitals.

Resumo Objetivo O objetivo do presente estudo foi analisar a expressão dos genes CD63, S100A6 e GNB2L1, que participam em mecanismos relacionados à complexa fisiopatologia da endometriose. Métodos Um estudo caso-controle foi realizado com 40 mulheres diagnosticadas com endometriose e 15 mulheres férteis e saudáveis. Amostras pareadas de endométrio eutópico e de lesões endometrióticas (implantes endometrióticos peritoneais e ovarianos) foram obtidas de mulheres com endometriose nas fases proliferativa (n = 20) ou secretora (n = 20) do ciclo menstrual. Como controle, amostras pareadas de biópsia endometrial foram coletadas de mulheres saudáveis nas fases proliferativa (n = 15) e secretora (n = 15) nomesmo ciclomenstrual. Foram analisados os níveis de expressão dos genes CD63, S100A6 e GNB2L1 por reação em cadeia da polimerase em tempo real. Resultados Foi observado um aumento nos níveis de transcritos dos genes CD63, S100A6 e GNB2L1 em implantes ectópicos quando comparado ao endométrio eutópico de mulheres com e sem endometriose, independente da fase do ciclo menstrual. Conclusão Estes achados sugerem que os genes CD63, S100A6 e GNB2L1 podem estar envolvidos na patogênese da endometriose, pois participam de mecanismos como inibição de apoptose, angiogênese e proliferação celular, os quais levam à perda da homeostase celular no endométrio ectópico e, portanto, contribuem para o implante e a sobrevivência do tecido no ambiente extrauterino.

Abstract Objective The aim of the present study was to analyze the expression of the CD63, S100A6, and GNB2L1genes, which participate in mechanisms related to the complex pathophysiology of endometriosis. Methods A case-control study was conducted with 40 women who were diagnosed with endometriosis, and 15 fertile and healthy women. Paired samples of eutopic endometrium and endometriotic lesions (peritoneal and ovarian endometriotic implants) were obtained from the women with endometriosis in the proliferative (n = 20) or secretory phases (n = 20) of the menstrual cycle. As controls, paired endometrial biopsy samples were collected from the healthy women in the proliferative (n = 15) and secretory (n = 15) phases of the samemenstrual cycle.We analyzed the expression levels of the CD63, S100A6, and GNB2L1 genes by real-time polymerase chain reaction. Results An increase in CD63, S100A6, and GNB2L1 gene transcript levels was observed in the ectopic implants compared with the eutopic endometrium of the women with and without endometriosis, regardless of the phase of the menstrual cycle. Conclusion These findings suggest that the CD63, S100A6, and GNB2L1 genesmay be involved in the pathogenesis of endometriosis, since they participate in mechanisms such as inhibition of apoptosis, angiogenesis and cell proliferation, which lead to the loss of cell homeostasis in the ectopic endometrium, thus contributing to the implantation and survival of the tissue in the extrauterine environment.

CONTEXTO E OBJETIVO: O câncer endometrial é o tipo mais prevalente de neoplasia maligna do trato genital. Os objetivos deste estudo foram: calcular a sensibilidade, especifi cidade e acurácia, bem como valor preditivo positivo e negativo das histeroscopias diagnósticas em comparação com a análise histopatológica de todas as lesões da cavidade endometrial. DESENHO E LOCAL: Estudo retrospectivo e descritivo no setor de endoscopia ginecológica do hospital universitário, terciário e público da Faculdade de Medicina de Ribeirão Preto, Universidade de São Paulo. MÉTODOS: A histeroscopia diagnóstica foi indicada nas seguintes situações: espessura endometrial > 4 mm em pacientes assintomáticas, sangramento na pós-menopausa, endométrio irregular ou endométrio de difícil avaliação pelo ultra-som, com ou sem sangramento vaginal. A avaliação ultra-sonográfi ca foi realizada não mais que três meses antes da histeroscopia. RESULTADOS: A idade média das pacientes foi 61,1 ± 2,0 anos, e a duração média do período pós-menopausa foi de 12,7 ± 2,5 anos. Das 510 pacientes, 293 (57,5%) foram submetidas à biópsia endometrial, no estudo histopatológico, 18 pacientes apresentavam carcinoma endometrial, hiperplasia típica ou atípica e nenhuma delas apresentava espessura endometrial maior que 8 mm. Nenhuma diferença signifi cativa foi encontrada entre as espessuras medianas das várias lesões benignas (p > 0,05). A sensibilidade (94,4%), especifi cidade (97%), acurácia (96,8%) e valores preditivos positivo e negativo (68% e 99,6% respectivamente) foram altos em nosso estudo. CONCLUSÃO: Nossos resultados sugerem que a histeroscopia diagnóstica apresenta boa validade como ferramenta diagnóstica para lesões malignas e hiperplasias, bem como para lesões benignas, com exceção dos leiomiomas submucosos, para o qual a sensibilidade foi de somente 52,6%.

CONTEXT AND OBJECTIVES: Endometrial cancer is the most prevalent type of malignant neoplasia of the genital tract. The objective of this study was to calculate the sensitivity, specificity, accuracy and positive and negative predictive values for diagnostic hysteroscopy, in comparison with histopathological tests, for all lesions of the endometrial cavity. DESIGN AND SETTING: Retrospective descriptive study at the public tertiary-level university hospital of Faculdade de Medicina de Ribeirão Preto, Universidade de São Paulo. METHODS: Diagnostic hysteroscopy was indicated in the following instances: endometrial thickness > 4 mm in asymptomatic patients; postmenopausal bleeding; and irregular endometrium or endometrium difficult to assess from ultrasound, with or without vaginal bleeding. Ultrasound evaluations were carried out no more than three months prior to hysteroscopy. RESULTS: There were 510 patients, with a mean age of 61.1 ± 2.0 years and mean time elapsed since the menopause of 12.7 ± 2.5 years. Endometrial biopsies were performed on 293 patients (57.5%). Histopathological analysis showed that 18 patients presented endometrial carcinoma or typical or atypical hyperplasia, and none of them presented endometrial thickness of less than 8 mm. No significant differences were found between the median thicknesses of the various benign lesions (p > 0.05). In our data, the sensitivity, specificity, accuracy and positive and negative predictive values for cancer or hyperplasia were 94.4%, 97.0%, 96.8%, 68% and 99.6%, respectively. CONCLUSIONS: Our results suggest that hysteroscopy is valuable as a diagnostic tool for malignant/hyperplastic and benign lesions, except for submucous myomas, for which the sensitivity was only 52.6%.

OBJETIVO: determinar a eficácia do spray de lidocaína a 10% aplicado no colo uterino antes da execução da histeroscopia diagnóstica para reduzir a dor e o desconforto causados pelo exame. MÉTODOS: Participaram do estudo 261 pacientes atendidas consecutivamente, entre março de 2004 e março de 2005. As pacientes foram randomizadas para dois grupos: um grupo recebendo aplicação tópica do spray de lidocaína (grupo lidocaína) e outro não recebendo nenhuma medicação antes do procedimento (grupo controle). Trinta miligramas de lidocaína spray a 10% foram aplicados na superfície do colo uterino, sendo a histeroscopia iniciada cinco minutos depois. Imediatamente após o final do procedimento a paciente respondeu um questionário sobre dor e quantificou a intensidade da dor, em centímetros, usando uma escala analógica visual não graduada. Os testes t não pareado, Mann-Whitney e chi2 foram aplicados na análise estatística, com nível de significância de 0,05. RESULTADOS: não houve diferença entre os grupos quanto à idade, paridade ou porcentagem de pacientes na menacme ou menopausa, ou quanto à indicação do procedimento e os achados histeroscópicos. Uma biópsia foi realizada em 57 das 132 pacientes no grupo lidocaína, e em 48 das 129 do grupo controle (p=0,96). O escore médio de dor foi 4,3±2,9 no grupo lidocaína, e 3,9±2,5 no grupo controle (p=0,2). Diferença no escore médio de dor foi observada somente na comparação entre pacientes na menacme e menopausa recebendo ou não a lidocaína spray, com p=0,01 e 0,04, respectivamente. CONCLUSÕES: o uso da lidocaína spray durante a histeroscopia diagnóstica não minimiza o desconforto e a dor das pacientes, e não deve ser utilizada.

PURPOSE: to determine the efficacy of 10% lidocaine spray applied to the cervix before the procedure of diagnostic hysteroscopy, in order to reduce the painful process and the discomfort caused by the exam. METHODS: a total of 261 consecutive patients participated in the study, which was conducted from March 2004 to March 2005. The patients were randomly assigned to one of two groups: one group receiving topical lidocaine spray (lidocaine group - LdG) and the other, receiving no medication before the procedure (control group - CG). In the LdG patients, thirty milligrams of 10% lidocaine spray were applied to the surface of the cervix five minutes before hysteroscopy started. Immediately, after the end of the procedure, the patients from both groups were asked to respond to a questionnaire about pain and to quantify the pain, in centimeters, using a 10-cm non-graduated visual analog scale. The unpaired t test, the Mann-Whitney test and the chi2 test were used for statistical analyses, considering p significant if lower than 0.05. RESULTS: there was no statistically significant difference between groups regarding age, parity or percentage of patients in menacme or menopause, or regarding the indications for the procedure and the hysteroscopic findings. A biopsy was necessary in 57 of the 132 LdG patients and in 48 of the 129 CG patients (p=0.96). The mean pain score was 4.3±2.9 in LdG and 3.9±2.5 in CG (p=0.2). A difference in the mean pain score was observed only among patients in menacme and menopause receiving or not the lidocaine spray, with p=0.01 and p=0.04 respectively. CONCLUSIONS: the use of lidocaine spray during diagnostic hysteroscopy does not minimize the discomfort and pain of the patients and therefore should not be applied.

Apesar da grande heterogeneidade das manifestações clínicas relacionadas à endometriose, observa-se a sua elevada prevalência em mulheres inférteis e portadoras de dor pélvica crônica. O impacto sócio-econômico desta afecção enigmática é elevado e os dados relacionados à eficácia das diferentes abordagens terapêuticas descritas são bastante conflitantes. Assim, os objetivos do presente artigo são apresentar as evidências científicas disponíveis acerca das diferentes modalidades terapêuticas aplicáveis e estabelecer recomendações para o tratamento da infertilidade e da dor pélvica crônica relacionadas à endometriose. Em pacientes com endometriose mínima ou leve a supressão da função ovariana não é efetiva para melhorar a fertilidade, mas a ablação das lesões associadas à adesiólise parece ser mais efetiva do que a realização exclusiva da laparoscopia diagnóstica. Não há evidências suficientes para determinar se a excisão cirúrgica em casos de doença moderada ou severa melhoraria as taxas de gestação. A fertilização in vitro parece ser uma abordagem adequada, especialmente nos casos de coexistência de fatores de infertilidade e/ou falha de outras abordagens terapêuticas. Deve-se avaliar a possibilidade de usar agonistas do GnRH por 3 a 6 meses, previamente à realização de fertilização in vitro. Em relação ao alívio da dor, verifica-se que a supressão da função ovariana por 3 a 6 meses em pacientes com doença confirmada laparoscopicamente reduz a dor associada à endometriose. Todas as medicações estudadas parecem apresentar eficácia similar, embora os efeitos adversos e de os custos sejam diferentes. A ablação das lesões endometrióticas reduz a dor associada à endometriose, sendo menos efetiva nos casos de doença mínima. A exérese de endometriomas com diâmetro > 4 cm parece melhorar a taxa de fecundidade natural e após procedimentos de reprodução assistida, além de reduzir a dor e os riscos de recidiva. Por fim, é importante ressaltar que há muita controvérsia e que as recomendações acima descritas deverão ser revistas à medida que estudos clínicos randomizados, controlados e com casuística adequada gerarem evidências mais concretas e confiáveis.

Despite a wide heterogeneity of clinical manifestations related to endometriosis, a high prevalence of the disease is observed in infertile women and in those with chronic pelvic pain. This enigmatic condition has a high socioeconomic impact, and the described data regarding efficacy of the therapeutic approaches are quite conflicting. Thus, the purpose of the present study was to describe the available scientific evidence about the applicable therapeutic modalities and to provide recommendations for the treatment of infertility and the chronic pelvic pain related to endometriosis. Although suppression of ovarian function in patients with minimal or mild endometriosis is not effective in improving fertility, ablation of the lesions associated with adhesiolysis seems to be more effective than exclusive diagnostic laparoscopy. There is no sufficient evidence to determine whether surgical excision in cases of moderate or severe disease would improve the pregnancy rates. In vitro fertilization seems to be an adequate approach, especially in cases of coexistence of infertility factors and/or failure of other treatments. The possibility of using GnRH for 3 to 6 months before in vitro fertilization should be considered. Regarding pain relief, suppression of ovarian function for 3 to 6 months in patients with laparoscopically-confirmed disease reduces the pain associated with endometriosis. All studied medication seem to have similar efficacy, differing only in terms of adverse effects and costs. Ablation of endometriotic lesion reduces the pain associated with endometriosis, being less effective in cases of minimal disease. Exeresis of endometriomas with diameter > 4 cm seems to improve the rate of natural fecundity and the rate for ??? obtained after assisted reproduction procedures, in addition to reducing both pain and recurrence risk. Finally, it is important to emphasize that this subject is much controversial and the recommendations herein described should be revised as randomized controlled clinical trials with adequate casuistic generate more concrete and reliable evidence.

OBJETIVO: avaliar a concordância entre os métodos não invasivos - queixa de dor pélvica, ultra-sonografia transvaginal (US-TV) e histerossalpingografia (HSG) e a endoscopia ginecológica no diagnóstico de fatores tubo-peritoneais responsáveis por infertilidade conjugal. MÉTODOS: foi realizado estudo do tipo corte transversal incluindo 149 pacientes inférteis submetidas à avaliação clínica, ultra-sonografia transvaginal, histerossonografia, histeroscopia e laparoscopia. Na avaliação de dor pélvica foram consideradas anormais a queixa de dor pélvica tipo dispareunia, dismenorréia ou dor acíclica e dor à mobilização do colo uterino e palpação de anexos. O exame ultra-sonográfico foi considerado alterado com os achados de alterações morfológicas anexiais ou uterinas (hidrossalpinge, miomas ou malformações uterinas). A histerossalpingografia foi considerada anormal na presença de alteração anatômica tubária e obstrução unilateral ou bilateral. Avaliou-se a concordância diagnóstica individual entre os diversos métodos não invasivos e a endoscopia por meio da análise kappa. RESULTADOS: a concordância entre dor pélvica, US-TV e HSG e avaliação endoscópica foi, respectivamente, de 46,3% (kapa=0,092; IC 95%: -0,043 a 0,228), 24% (kapa=-0,052; IC 95%: -0,148 a 0,043) e 46% (kapa=0,092; IC 95%: -0,043 a 0,228). Quando se considerou pelo menos um método não invasivo positivo, a concordância com a avaliação endoscópica foi de 63% (kapa=-0,014; IC 95%: -0,227 a 0,199). A sensibilidade e especificidade em predizer achados na endoscopia foram de 39,5 e 80% na presença de dor pélvica, de 14,5 e 72% na presença de alteração na US-TV, de 39,5 e 80% na presença de alteração na HSG e de 70,2 e 28% na presença de ao menos uma alteração na avaliação não invasiva. CONCLUSÃO: há fraca concordância diagnóstica entre os diversos métodos não invasivos e a endoscopia na investigação de infertilidade conjugal secundária a fatores tubo-peritoneais.

PURPOSE: to evaluate the agreement between noninvasive methods - pelvic pain, transvaginal ultrasound and hysterosalpingography - and the gynecologic endoscopy approach for the diagnosis of tuboperitoneal factors responsible for conjugal infertility. METHODS: this is a cross-sectional study including 149 infertile patients who were submitted to clinical evaluation, transvaginal ultrasound, hysterosalpingography, hysteroscopy, and laparoscopy. In the evaluation of pelvic pain, the following complaints were considered to be abnormal: pelvic pain of the dyspareunia type, dysmenorrhea or acyclic pain, and pain upon mobilization of the cervix and palpation of the adnexa. Ultrasonographic examination was considered to be altered when adnexal or uterine morphological changes (hydrosalpinx, myomas or uterine malformations) were detected. Hysterosalpingography was considered to be abnormal in the presence of anatomical tubal changes and unilateral or bilateral obstruction. The agreement between noninvasive methods and endoscopy was evaluated by kappa statistics. RESULTS: the agreements between pelvic pain, transvaginal ultrasound, and hysterosalpingography and the endoscopic approach were 46.3% (kappa=0.092; CI 95%: -0.043 to 0.228), 24% (kappa=-0.052; CI 95%: -0.148 to 0.043), and 46% (kappa=0.092; CI 95%: -0.043 to 0.228), respectively. When at least one alteration detected by noninvasive methods was considered, the agreement with endoscopic approach was 63% (kappa=-0.014; CI 95%: -0.227 to 0.199). Sensitivity and specificity in predicting alterations on endoscopic approach were 39.5 and 80% in the presence of pelvic pain, 14.5 and 72% in the presence of alteration on transvaginal ultrasound, 39.5 and 80% in the presence of alteration on hysterosalpingography, and 70.2 and 28% in the presence of at least one alteration by noninvasive methods. CONCLUSION: there is a poor diagnostic agreement between the several noninvasive methods and endoscopy in the investigation of conjugal infertility secondary to tuboperitoneal factors.
