RESUMEN Introducción: Se ha tratado de identificar los factores genéticos relacionados con susceptibilidad para enfermedad inflamatoria intestinal (EII), y los hallazgos actuales se inclinan por un modelo de patología complejo, sin un patrón hereditario claro. Objetivo: Realizar caracterización fenotípica y genotípica de pacientes con EII en población colombiana y describir su posible asociación con predisposición. Materiales y métodos: Serie de casos, 16 pacientes con EII por criterios clínicos y anatomopatológicos, inicio de síntomas gastrointestinales después de los 18 años. Todos tuvieron asesoramiento genético pre-test y se realizaron árboles genealógicos de mínimo tres generaciones. También, genotipificación, por medio de un panel de genes múltiples que incluía genes relacionados con EII y algunos trastornos autoinmunitarios. Finalmente, se realizó análisis genómico de variantes. Resultados: 9 mujeres y 7 hombres, con edad media de diagnóstico de EII 35 años, y 32 años para aparición de síntomas gastrointestinales. 11/16(68,75%) requirieron terapia biológica. 10/16 (62,5%) presentaron refractariedad a terapia estándar. 3/16 (18,75%) tenían antecedentes familiares positivos de EII. 100% casos presentaron al menos un single nucleotide polymorphism relacionado con riesgo de EII en más de un gen. Los genes más relacionados con colitis ulcerosa (CU), fueron CD48, CD6, y TYK2 para CU, y CD6 e ITGAM para la enfermedad de Crohn. El gen más frecuente fue CD6. Se observó en 3/16 (18,75%) presencia de hasta 5 genes, 4 en 3/16 (18,75%), y tres en 5/16 (31,25%). Conclusión: En EII hay presencia de variantes genéticas con predisposición asociada, pero sin patogenicidad confirmada, y cuya sumatoria parece contribuir en su fisiopatología.

ABSTRACT Introduction: Attempts have been made to identify the genetic factors related to susceptibility to inflammatory bowel disease (IBD), and the current conclusions are in favor of a complex pathology model, without a clear hereditary pattern. Objective: To perform phenotypic and genotypic characterization of patients with IBD in Colombian population and to describe its possible association with predisposition. Materials and methods: case series, 16 patients with IBD according to clinical and pathological criteria, onset of gastrointestinal symptoms after 18 years of age. All had pre-test genetic counseling and family trees of at least three generations were made. Also, genotyping, using a multigene panel that included genes related to IBD and some autoimmune disorders. Finally, a genomic analysis of variants was performed. Results: 9 women and 7 men, with mean age of diagnosis of IBD of 35 years, and gastrointestinal symptoms appearance of 32 years. 11/16 (68.75%) required biological therapy. 10/16 (62.5%) were refractory to standard therapy. 3/16 (18.75%) had positive family history of IBD. 100% cases presented at least one single nucleotide polymorphism related to IBD risk in more than one gene. The genes most related to ulcerative colitis (UC) were CD48, CD6, and TYK2 for UC, and CD6 and ITGAM for Crohn's disease. The most frequent gene was CD6. It was found presence of up to 5 genes in 3/16 (18.75%), 4 in 3/16 (18.75%), and three in 5/16 (31.25%). Conclusion: In IBD there is the presence of genetic variants with associated predisposition, but without confirmed pathogenicity, and whose sum seems to contribute to its pathophysiology.

ABSTRACT Introduction: Colombia has been subject to intense genetic and cultural currents due to its geographical location. Hemoglobinopathies are the most common recessive diseases found worldwide and represent an important public health problem, according to the region and ancestry of each country. Objectives: To evaluate the frequency of hemoglobin variants according to the geographical region in a population group adjusted to sex and age in Colombia. Methods: This was a descriptive retrospective study of hemoglobin variants performed by electrophoresis in patients treated at and/or referred to specialized care institutions in Bogota, Colombia between January 2009 and December 2020. Results: A total of 2,224 results were analyzed, 48.4% male and 51.5% female; 63.3% of patients were without alterations, 14.3% presented with thalassemia, 17.3%, HbS, 2.3%, HbS/C, 1.8%, HbC, 0.5%, HbE and 0.5% persistent HbF, with HbS being more prevalent in males (p = 0.005). When assessing the geographical regions of Colombia, a higher prevalence of HbS was found in the Pacific (p = 0.005) and Caribbean regions, while Thalassemia and HbS were more prevalent in the Andean and Orinoquia regions, and it was rare to find any hemoglobinopathies (p = 0.0001) in the Amazonian region. Conclusions: The main hemoglobinopathies found in Colombia are HbS, predominantly in males, and Thalassemia. The distribution of hemoglobinopathies in different geographical regions of Colombia is influenced by ancestry. ABSTRACT Background: Peripheral spondylarthritis is a chronic Inflammatory disease whose clinical presentation is related to the presence of arthritis, enthesitis and/or dactylitis. This term is used interchangeably with some of its subtypes such as psoriatic arthritis, reactive arthritis, and undifferentiated spondyloarthritis. Objective: To develop and formulate a set of specific recommendations based on the best available evidence for the diagnosis, treatment, and monitoring of adult patients with peripheral spondyloarthritis. Methods: A working group was established, clinical questions were formulated, outcomes were graded, and a systematic search for evidence was conducted. The guideline panel was multidisciplinary (including patient representatives) and balanced. Following the for mal expert consensus method, the GRADE methodology "Grading of Recommendations Assessment, Development and Evaluation" was used to assess the quality of the evidence and generate the recommendations. The clinical practice guideline includes ten recommendations related to monitoring of disease activity (n = 1) and treatment (n = 9). Results: In patients with peripheral spondyloarthritis, the use of methotrexate or sulfasalazine as the first line of treatment is suggested, and local injections of glucocorticoids are conditionally recommended. In patients with failure to cDMARDs, an anti TNFα or an anti IL17A is recommended. In case of failure to bDMARDs, it is suggested to use another bDMARD or JAK inhibitor. In patients with peripheral spondylarthritis associated with inflammatory bowel disease, it is recommended to start treatment with cDMARDs; in the absence of response, the use of an anti TNFα over an anti-IL-17 or an anti-IL-12-23 is recom mended as a second line of treatment. In patients with psoriatic arthritis, the combined use of methotrexate with a bDMARD is conditionally recommended for optimization of dosing. To assess disease activity in Psoriatic Arthritis, the use of DAPSA or MDA is suggested for patient monitoring. Conclusions: This set of recommendations provides an updated guideline on the diagnosis and treatment of peripheral spondyloarthritis.

RESUMEN Antecedentes: La espondiloartritis periférica es una patología Inflamatoria crónica cuya presentación clínica está determinada por la presencia de artritis, entesitis y/o dactilitis. Este término se utiliza indistintamente con algunos de sus subtipos como artritis psoriásica, artritis reactiva y espondiloartritis indiferenciada. Objetivo: Desarrollar y formular un conjunto de recomendaciones específicas basadas en la mejor evidencia disponible para el diagnóstico, el tratamiento y el seguimiento de pacientes adultos con espondiloartritis periférica. Métodos: Se constituyó un grupo desarrollador, se formularon preguntas clínicas, se graduaron los desenlaces y se realizó la búsqueda sistemática de la evidencia. El panel de la guía fue multidisciplinario (incluyendo representantes de los pacientes) y balanceado. Siguiendo el método de consenso formal de expertos, se utilizó la metodología GRADE (Grading of Recommendations Assessment, Development and Evaluation) para para evaluar la calidad de la evidencia y generar las recomendaciones. La guía de práctica clínica incluye 10 recomendaciones: una sobre seguimiento de la actividad de la enfermedad y nueve sobre tratamiento. Resultados: En pacientes con espondiloartritis periférica se sugiere usar metotrexato o sulfasalazina como primera línea de tratamiento y se recomienda en forma condicional la inyección local de glucocorticoides. En los pacientes que fallan a cDMARDs, se recomienda iniciar un anti TNFα o un anti IL17A. Ante falla terapéutica a la primera línea con bDMARDs, se sugiere usar otro bDMARD o un inhibidor JAK. En pacientes con espondiloartritis periférica y enfermedad inflamatoria intestinal asociada, se recomienda iniciar tratamiento con cDMARDs; en ausencia de respuesta, se recomienda el uso de un anti TNFα sobre un anti IL-17 o un anti IL-12-23 como segunda línea de tratamiento. En pacientes con artritis psoriásica se recomienda, de forma condicional, el uso combinado de metotrexato con bDMARD para favorecer la optimización de la dosis de estos. Para evaluar la actividad de la enfermedad en artritis psoriásica, se sugiere el uso del DAPSA o MDA para el seguimiento de los pacientes. Conclusiones: Este conjunto de recomendaciones proporcionan una guía actualizada sobre el diagnóstico y el tratamiento de la espondiloartritis periférica.

Resumen Actualmente, la pandemia por la enfermedad por coronavirus de 2019 (COVID-19) ha sido uno de los grandes retos para el personal de la salud y se ha convertido en un mayor desafío cuando hay pacientes con enfermedad inflamatoria intestinal (EII); debido a la fisiopatología de la enfermedad y los tratamientos utilizados, se pueden generar interrogantes difíciles de contestar ante el corto tiempo de evidencia científica existente sobre la vacunación para coronavirus del síndrome respiratorio agudo grave de tipo 2 (SARS-CoV-2). Es claro que la vacunación es una de las intervenciones de salud pública más efectivas para lograr una inmunidad colectiva en esta pandemia; por tal motivo, se realiza una revisión enfocada en los aspectos claves frente a la vacunación para la infección por SARS-CoV-2 en los pacientes con EII según la evidencia disponible.

Abstract Currently, the coronavirus disease 2019 (COVID-19) pandemic has been one of the great challenges for health personnel and has become a greater challenge when we have patients with inflammatory bowel disease (IBD); because of the pathophysiology of the disease and the treatments used, it can generate questions that are difficult to answer given the short time of scientific evidence that we have on vaccination for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The vaccination is one of the most effective public health interventions to get immunity in this pandemic; for this reason, we made a review with the key aspects about vaccination for infection by SARS-CoV-2 in patients with IBD according to the available evidence.

Resumen Los coronavirus son una extensa familia de virus; algunos pueden ser causa de diversas enfermedades humanas, que van desde el resfriado común hasta el síndrome respiratorio agudo severo. La enfermedad por coronavirus (COVID-19) es causada por el coronavirus zoonótico SARS-CoV-2, y comenzó en China en diciembre de 2019. En marzo de 2020, la Organización Mundial de la Salud (OMS) lo declaró una pandemia mundial. Esta enfermedad tiene un gran espectro de signos y síntomas, predominantemente se presenta con tos y fiebre, lo que resulta en una proporción de pacientes que desarrollan síndrome de dificultad respiratoria aguda (SDRA). Se ha planteado que la afectación pulmonar grave por el virus puede ser causada también por hiperinflamación y un cuadro similar a la linfohistiocitosis hemofagocítica (HLH), el cual es un síndrome clínico provocado por una respuesta inadecuada del sistema inmunológico a un desencadenante, sea infeccioso, neoplásico, metabólico o reumatológico, que resulta en una reacción inflamatoria desproporcionada. Esta respuesta inmune altamente activada causa una liberación exagerada o tormenta de citocinas responsable del cuadro clínico. De allí la importancia de revisar el concepto actual y su desenlace en COVID-19. Recientemente se han publicado artículos en los que se sugiere que la presencia de síntomas gastrointestinales se ha asociado a COVID 19 con mayores indicadores de gravedad, lo que motiva al gastroenterólogo a explorar cómo el sistema digestivo podría impactar en esta hiperinflamación.

Abstract Coronaviruses are an extensive family of viruses, some of them can produce different kind of human diseases, ranging from the common cold to severe acute respiratory syndrome. Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 zoonotic coronavirus and started in China in December 2019. In March 2020, the World Health Organization (WHO) declared it a global pandemic. This disease has a wide spectrum of signs and symptoms, predominantly presenting with cough and fever, resulting in a proportion of patients who develop acute respiratory distress syndrome (ARDS). It has been argued that severe lung involvement by the virus may also be caused by hyperinflammation and a condition similar to hemophagocytic lymphohistiocytosis (HLH), which is a clinical syndrome caused by an inadequate response of the immune system to a trigger, whether infectious, neoplastic, metabolic, or rheumatologic, leading to a disproportionate inflammatory reaction. This activated immune response producing an exaggerated release or storm of cytokines responsible for this disease. There are recently publications suggesting that the presence of gastrointestinal symptoms is associated with COVID 19 disease with higher indicators of severity, which motivates the gastroenterologist to explore how the digestive system could impact this hyperinflammation.

Resumen La pandemia por SARS-Cov-2, ha tomado gran relevancia por su impacto en los diversos sistemas de salud en el mundo. Inicialmente solo se contemplaba la importancia de los síntomas respiratorios y la fiebre; sin embargo, a diario tenemos más reportes y publicaciones sobre la relevancia que está teniendo en el sistema digestivo, ya que algunos pacientes informan síntomas gastrointestinales como diarrea, vómito y dolor abdominal. Los estudios han identificado el ARN del SARS-CoV-2 en muestras de heces de pacientes infectados, en las que se encontró que su receptor que es el de la enzima convertidora de angiotensina 2 (ECA2) se expresaba altamente en las células epiteliales gastrointestinales. Esto sugiere que el SARS-CoV-2 puede infectar activamente y replicarse en el tracto gastrointestinal. Esto tiene implicaciones importantes para el tratamiento de la enfermedad, la transmisión y el control de infecciones. En pacientes con patologías crónicas como la enfermedad inflamatoria intestinal (EII), se han generado gran cantidad de preguntas e incertidumbres ante la presencia de COVID-19 junto con las implicaciones que pueden tener tanto en la severidad de los síntomas, como en la descompensación de la patología de base y en la continuidad del tratamiento inmunosupresor.

Abstract The SARS-Cov-2 pandemic has taken great relevance due to its diverse impact in the worldwide health systems. Initially, only the importance or respiratory symptoms and fever was considered, however daily we have more reports and publications about the relevance that gastrointestinal symptoms, like diarrhea, vomit, and abdominal pain, are having in COVID-19. Several studies have identified SARS-CoV-2 RNA in stool samples of infected patients, and it was also found that the viral receptor, the angiotensin-converting enzyme 2 (ACE2) receptor is highly express in the gastrointestinal cells. These findings suggest that SARS-Cov-2 can actively infect and replicate in the gastrointestinal tract. All the above have relevant implications in the disease treatment, transmission, and infection control. In patients with chronic pathologies, such as inflammatory bowel disease (IBD), many questions and uncertainties, about symptoms severity, disease decompensation and use of immunosuppressive drugs, have been generated in the presence of COVID-19.

RESUMEN Introducción: La psoriasis es una enfermedad crónica asociada con múltiples comorbilidades, incluida la enfermedad inflamatoria intestinal. Sin embargo, no existen estudios previos en Colombia sobre la relación de los síntomas gastrointestinales en pacientes con psoriasis. Objetivos: Se determinó la frecuencia de síntomas sugestivos de enfermedad gastrointestinal y autoanticuerpos asociados en pacientes con psoriasis, y su relación con la actividad de la misma. Además, se comparó con controles sanos. Materiales y métodos: Estudio transversal con componente analítico que incluyó una encuesta de síntomas y el análisis sérico de autoanticuerpos sugestivos de enfermedad gastrointestinal en casos y controles. Resultados: Ochenta y cuatro individuos analizados, distribuidos en 44 casos y 40 controles. El 64% de los casos tenían psoriasis leve y el 36% presentó psoriasis moderada a severa. El 71, el 16 y el 14% fueron tratados con tratamientos tópicos, orales y biológicos, respectivamente. Los principales síntomas gastrointestinales fueron distensión abdominal (45%), estreñimiento (39%) y fatiga (34%). La comparación arrojó una mayor prevalencia de síntomas gastrointestinales en los casos, y la variable fatiga fue estadísticamente significativa (p: 0,04). Solo el perfil de autoanticuerpos antinucleares fue estadísticamente significativo (0 vs. 43%; p: 0,001). No se encontró asociación con actividad de la enfermedad. Conclusiones: La prevalencia de síntomas y autoanticuerpos sugestivos de enfermedad gastrointestinal en pacientes se considera importante, a pesar de no mostrar diferencias significativas con los controles. Algunos pacientes con síntomas gastrointestinales mostraron autoanticuerpos positivos. Esto sugiere la necesidad de un seguimiento clínico adecuado. El dermatólogo tiene un papel decisivo en el manejo integral de los pacientes.

ABSTRACT Introduction: Psoriasis is a chronic disease associated with multiple comorbidities including inflammatory intestinal disease. There are no previous studies in Colombia regarding the relationship of gastrointestinal symptoms in patients with psoriasis. Objective: To determine the frequency of symptoms and autoantibodies suggestive of gastrointestinal disease in patients with psoriasis, and to compare the results with healthy controls, as well as their relationship with disease activity. Materials and methods: Cross-sectional study with an analytical component, including a questionnaire on symptoms and serum analysis of antibodies suggestive of gastrointestinal disease in cases and controls. Results: The analysis included 84 individuals, separated into groups of 44 cases and 40 controls. Mild psoriasis was observed in 64% of cases, and the remaining 36% had moderate to severe psoriasis. Topical treatments were given to 71% of cases, with another 16% using oral treatments, and 14% received biological treatment. The main symptoms of gastrointestinal disease were, abdominal distention (45%), constipation (39%), and fatigue (34%). A comparison showed a higher prevalence of gastrointestinal symptoms in cases vs. controls, and fatigue was statistically significant (p = 0.04). Only the antinuclear autoantibodies in the serum analysis were statistically significant (0 vs. 43%; p = 0.01). Conclusions: The prevalence of symptoms and autoantibodies suggestive of gastrointestinal disease in patients with psoriasis is considered important, even although there are no significant differences when compared with healthy controls. Some patients with gastrointestinal symptoms had positive autoantibodies. This suggests that an adequate clinical follow-up should be carried out. Dermatologists have a decisive role in the integral management of patients.

RESUMEN Antecedentes: La artritis reumatoide (AR) es una enfermedad autoinmune caracterizada prin cipalmente por deterioro articular y disminución de la densidad mineral ósea. La proteína Dickkopf 1 (DKK1) ejerce una función reguladora negativa de la vía Wnt comprometida con la diferenciación de osteoblastos y se ha observado que puede estar sobreexpresada en pacientes con AR. Objetivo: Proveer información actualizada sobre el conocimiento de la asociación entre los niveles séricos de DKK1 y la presencia de dafño óseo y articular en pacientes con AR. Método: Se realizó una revisión sistemática cualitativa en las bases de datos Pubmed, Embase, Cochrane y Scielo utilizando los términos Dickkopf 1, DKK1, Dickkopf related pro tein 1, rheumatoid artrhitis, biomarcador, resorción ósea. Resultados: Se escogieron 12 estudios que llenaban los requisitos de la búsqueda; 7 fueron cohortes prospectivas, 4 estudios de corte transversal y uno ensayo clínico. De los 12 estudios revisados, 10 analizaron la asociación entre niveles séricos de DKK1 y presencia de dann o óseo como desenlace primario. Uno de ellos analizó esta asociación como desenlace secundario y otro la relación RSP01/DKK1. Los resultados hasta la fecha parecen indicar que la DKK1 tendría un papel activo en estadios avanzados de AR y no en la fase inicial. Conclusiones: La proteína DKK1 desempeña un papel fisiopatológico esencial en la disminu ción de la masa ósea y la remodelación articular, dependiendo de la fase de la enfermedad, en pacientes con AR. Su papel como biomarcador o estrategia terapéutica sería una interesante alternativa aún en estudio.

ABSTRACT Background: Rheumatoid arthritis (RA) is an autoimmune disease that is mainly characterized by joint deterioration and decreased bone mineral density. The Dickkopf 1 protein (DKK1) exerts a negative regulatory function of the Wnt pathway involved in the differentiation of osteoblasts, and has been observed to be overexpressed in patients with RA. Objective: To provide updated information on current knowledge about the relationship between DKK1 serum levels and the presence of bone and joint damage in RA patients. Method: A qualitative systematic review was carried out in the PubMed, Embase, Cochrane and Scielo databases using the terms Dickkopf 1, DKK1, Dickkopf related protein 1, Rheumatoid Arthritis, and Bone biomarker. Results: A total of 12 studies were chosen that met the requirements of the search. These included 7 prospective cohorts, 4 cross-sectional studies, and 1 clinical trial. Of the 12 studies reviewed, 10 analyzed the relationship between serum DKK1 levels and the presence of bone damage as the primary outcome. One of them analyzed this relationship as a secondary outcome and another one the RSP01/DKK1 ratio. The results to date seem to indicate that DKK1 could have an active role in advanced stages of RA, but not in the initial phase. Conclusions: The DKK1 protein plays an essential pathophysiological role in the decrease of bone mass and joint remodelling, depending on the stage of the disease in patients with RA. Its role as a biomarker or therapeutic strategy would be an interesting alternative still under study.

Background: Oxygen transport is altered in hemoglobinopathies. Aim: To study the distribution of hemoglobinopathies in Andean subjects without African ancestry. Material and Methods: We analyzed blood samples of 1,407 subjects aged 18 to 59 years (58% females), living in the central Andean region of Colombia, referred to discard hemoglobinopathies. The frequency and type of hemoglobinopathy was established by capillary and agarose gel electrophoresis. Results: The frequency of hemoglobinopathies was 34.5% and higher among females. The structural variants found were: AS-heterozygous hemoglobin (8.1%), homozygous SS (3.7%), heterozygous SC (2.2%), AC heterozygotes (0.5%) and heterozygous AE (0.3%). Quantitative variants found were Hb A-Beta thalassemia (13.91%) and Hb H (0.06%), Beta-thalassemia heterozygotes C (0.88%), S-Beta thalassemia heterozygotes (6.07%) and compound heterozygous SC/Beta thalassemia (0.25%), with a persistence of fetal hemoglobin 0. Composite thalassemia was also found in 31%. All techniques showed good correlation and capillary electrophoresis demonstrated a greater detection of hemoglobin variants. Conclusions: The frequency of hemoglobin variants in the analyzed population was high, which is an important public health indicator. The most common hemoglobin variant was HbA/Increased structural Hb A2 and the mos frequent structural hemoglobinopathy was sickle cell trait. Capillary electrophoresis can discern any Hb variants present in the population.

Introducción: Las alteraciones de la enzima metilen-tetrahidrofolato reductasa (MTHFR) se asocian con riesgo cardiovascular y cerebrovascular y con presencia de concentraciones altas de homocisteína. Se relacionan también con defectos congénitos, complicaciones en embarazo, cáncer y toxicidad del Metotrexato (MTX). Los polimorfismos del gen MTHFR afectan la actividad de la enzima. Se han descrito asociaciones genéticas con la eficacia del tratamiento con MTX. Objetivo: Establecer la frecuencia del polimorfismo C>T en el nucleótido 677 del gen MTHFR en un grupo de individuos Colombianos. Métodos: Estudio descriptivo de corte transversal. Se recolectaron retrospectivamente resultados de microarreglos farmacogenéticos que incluyen polimorfismos asociados con la sensibilidad al MTX (PathwayGenomics©). Se analizó la frecuencia del polimorfismo C>T del polimorfismo rs1801133 del gen MTHFR. Resultados: Se analizaron microarreglos de 68 hombres y 84 mujeres. Las comparaciones del genotipo C/C frente a C/T y a T/T fueron estadísticamente significativas (p= 0.001 y p= 0.026 respectivamente) tanto como la comparación entre C/T y T/T (p= 0.0001). Conclusiones: Los genotipos C/C y C/T en Colombia son tan variables como en otros grupos sanos en otras poblaciones. Los sujetos de nuestra población podrían tener riesgo para el desarrollo de enfermedades asociadas al polimorfismo del gen MTHFR y con genotipos de riesgo de presentar toxicidad y efectos adversos del MTX, lo cual sugiere la necesidad de evaluar alternativas terapéuticas con estudios farmacogenéticos.

Introduction: Abnormal levels of the enzyme methylenetetrahydrofolate reductase (MTHFR) are associated with an increased risk of both cardiovascular and cerebrovascular disease and higher concentrations of homocysteine. Abnormal levels are also related to birth defects, pregnancy complications, cancer and toxicity to methotrexate (MTX). Polymorphisms of MTHFR affect the activity of the enzyme. Genetic associations have been related to treatment efficacy. Objective: To establish the frequency of the C>T polymorphism at nucleotide 677 of the MTHFR gene in a group of Colombian individuals. Methods: Data from pharmacogenetic microarrays that include MTX sensibility-associated polymorphisms were retrospectively collected (Pathway Genomics©). The frequency of the C>T MTHFR rs1801133 marker polymorphism was analyzed. Results: Microarray data from 68 men and 84 women were analyzed. Comparisons of genotype C/C vs. C/T and T/T were statistically significantly different (p= 0.00, p= 0.026, respectively), as were C/T and T/T (p= 0.0001). Conclusions: Results for the C/C and C/T genotypes in a Colombian population are similar to other previously studied groups of healthy subjects. Subjects from our population might be at risk of developing diseases associated with MTHFR polymorphisms and might present toxicity and adverse effects if treated with MTX, which suggests the need to evaluate therapeutic alternatives based on individual pharmacogenetic studies.

Actualmente se percibe una necesidad apremiante en la identificación y validación de biomarcadores que reflejen tempranamente el inicio de actividad lúpica o que se conviertan en predictores de la misma. La actividad clínica del lupus eritematoso sistémico (LES) es ondulante a lo largo del tiempo y la actividad subyacente persistente lleva a daño tisular. Este daño es reflejo de cambios irreversibles en la función y estructura orgánica, por lo que la prevención, más que el tratamiento, debería ser la meta de cualquier terapia en LES y así lograr disminuir la morbimortalidad y los costos directos e indirectos causados por la enfermedad. Es necesario encontrar biomarcadores no invasivos de actividad lúpica que no solo permitan tomar de forma oportuna decisiones terapéuticas, sino que también se correlacionen con los desenlaces clínicos y sean útiles en los ensayos clínicos, permitiendo acortar el tiempo del desarrollo de estos estudios. Este artículo pretende buscar la evidencia que se tiene con respecto a los nuevos biomarcadores existentes para actividad de la enfermedad en LES y su utilidad actual y futura, enfatizando en la necesidad de buscar nuevas moléculas que permitan un diagnóstico más precoz de la actividad de la enfermedad.

There is a need for the identification and validation of biomarkers that reflect the early onset of lupus activity or may be predictors of this. The clinical activity of systemic lupus erythematosus (SLE) fluctuates over time and the underlying activity leads to persistent tissue damage. This damage is a reflection of irreversible changes in the function and organic structure, so prevention, rather than treatment, should be the goal of any therapy in SLE.This will reduce morbidity, mortality, direct and indirect costs caused by the disease. It is necessary to find biomarkers of lupus activity that not only allow making treatment decisions in the short term, but also to correlate with clinical outcomes. These could also be useful in clinical trials and may shorten the duration of these studies. This article aims to find evidence on new biomarkers for SLE disease activity, and their current and future use. Emphasis will be made on the need to find new molecules for an early diagnosis of disease activity.

Spondyloarthritis is a group of several related but phenotypically distinct chronic inflammatory diseases, characterized by progressive new bone formation which leads to ankylosis and functional disability. Radiographic images evidence not only erosive changes but also overgrowth of bony structures called syndesmophytes. These inflammation, bone destruction and new bone formation are located in the entheses, which constitutes the primary organ of the disease. As a consequence, the inflammatory process results in excess of bone formation and the impact depends on the location, cell type, cytokines and local microenvironment factors. Several molecules playing a role as immune modulators or regulators of bone homeostasis, mediate the imbalance between bone resorption and formation. In the same way, animal models suggest that joint ankylosis may be independent from the effects of tumor necrosis factor alpha. Therefore, the process of new tissue (bone) formation can be considered as an additional therapeutic target. The Wnt signaling pathway, which is considered the primary regulator of osteoblastogenesis, constitutes a new research field of great interest in the last decade.

INTRODUCCIÓN: la esclerosis múltiple es la enfermedad neurológica más común y discapacitante en adultos jóvenes. Hasta el momento no existe ninguna prueba certera para su diagnóstico. El laboratorio clínico ha tomado gran importancia al incluir la valoración de la producción intratecal de anticuerpos en el proceso diagnóstico de esta patología. OBJETIVO: describir el comportamiento de las bandas oligoclonales del líquido cefaloraquídeo en un grupo de pacientes. MATERIALES Y METODOS: se realizó un estudio observacional descriptivo tipo serie de casos para describir el comportamiento del resultado de las bandas oligoclonales (BO) en el líquido cefalorraquídeo, en pacientes con esclerosis múltiple (EM) del Hospital Militar Central (HMC) de Bogotá que estaban en tratamiento. Se calcularon las frecuencias absolutas y medidas de tendencia central de las variables estudiadas. RESULTADOS: entre la población de usuarios del Hospital Militar Central hubo 26 pacientes con esclerosis multiple, con edad media de 43 años (DS = 8,54). El 75,1 % (19 pacientes) mujeres y el 26.9% (7 pacientes) hombres. El 50 % (13 pacientes) presentaron bandas oligoclonales positivas.. CONCLUSIÓN: al utilizar la electroforesis de proteínas de alta resolución como técnica para la determinación de las bandas oligoclonales en el líquido cefalorraquídeo se obtuvieron resultados similares a los reportados en la literatura. Sin embargo, se deben hacer todos los esfuerzos para introducir la detección de bandas oligoclonales por la técnica recomendada internacionalmente: el isoelectroenfoque comparando simultáneamente el suero y el líquido cefalorraquídeo, con lo cual se puede obtener una gran sensibilidad; allí radica su importancia y utilidad como valor predictivo negativo.

INTRODUCTION: multiple sclerosis is the most common disabling neurological disease in youth adults. So far there is no accurate test for diagnosis. The laboratory tests have become very important because include assessment of intrathecal antibody production in the diagnosis of this disease. OBJETIVE: describe the behavior of cerebrospinal fluid oligoclonal bands in a group of patients MATERIAL AND METHODS: a descriptive case series, with the population of MS patients currently receiving specific treatment in the Hospital Militar Central in Bogota. The absolute frequencies and measures of central tendency of the variables studied were calculated. RESULTS: among the population described there are 26 patients with MS, with a mean age of 43 years old (SD +/- 8.542). 75.1% (19) were females and 26.9% (7) male. 50% (13) of patients had positive oligoclonal bands.. CONCLUSION: with a standardized technique for determining oligoclonal bands in cerebrospinal fluid, as the high-resolution agarose gel electrophoresis the authors obtained similar results as reported in the international literature. It should make all efforts to introduce the detection of BO with the internationally recommended technique named "isoelectric focusing", who simultaneously compared serum and cerebrospinal fluid, that can get a great sensitivity and a high negative predictive value.

Las espondiloartritis corresponden a un grupo heterogéneo de enfermedades crónicas caracterizadas por entesitis tanto axial como periférica, artritis y menos comúnmente manifestaciones extra articulares. Se encuentran fuertemente ligadas a factores genéticos y en algunos pacientes a infecciones por bacterias artritogénicas. Su presentación y curso clínico se encuentran influenciados por la etnia, el género y la edad de inicio de la enfermedad. La Espondilitis Anquilosante (EA) como prototipo de espondiloartritis es una enfermedad hereditaria con un 90% de susceptibilidad atribuible a factores genéticos. Desde el descubrimiento de la asociación del alelo HLA-B*27 en los años setenta, las bases moleculares de esta asociación, una de las más fuertes entre una molécula del Complejo Mayor de Histocompatibilidad y una enfermedad, permanecen sin esclarecer. La fuerte asociación del HLA-B*27 confiere a este alelo un papel significativo de susceptibilidad para el desarrollo de la enfermedad. Varios estudios han informado la asociación de otros genes dentro del Complejo Mayor de Histocompatibilidad con la susceptibilidad para el desarrollo de la enfermedad en varios grupos poblacionales.

Spondylarthritis refers to a heterogeneous group of chronic diseases characterized by both axial and peripheral enthesitis, arthritis and extra articular manifestations. There is strongly linked to genetic factors and in some patients is related to clinical infections by arthritogenic bacteria. The clinical presentation and evolution are influenced by ethnicity, gender and age of onset. Ankylosing Spondylitis as a prototype of Spondylarthritis is an inherited disease with 90% of susceptibility related to genetic factors. Since the publication of the association of HLA-B*27 in the 70's, the molecular component of this association, one of the strongest between a molecule of Major Histocompatibility Complex and disease, remains unclear. The strong association of HLA-B*27 gives to this allele a significant role in susceptibility related to disease development. Several studies have reported the association of other genes within the Major Histocompatibility Complex to susceptibility for development of the disease in others population groups.

Among rheumatic diseases and specifically spondyloarthropathies (SpA), the study of biomarkers, defined as molecules that reflect either biologic or specific pathological process, is an important and necessary area in basic and clinical research, being a consequence or the response of an intervention. Other markers provide information about the pathogenesis of this disease. Recently, HLA-B27 has been used as diagnostic criteria to detect SpA. Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Bath Ankylosing Spondylitis Functional Index (BASFI) are clinical scores used to assess disease activity. A new activity index, Ankylosing Spondylitis Disease Activity Score (ASDAS) considers erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) as biomarkers. This review describes the state of the art of research on SpA biomarkers. There are promising new candidates as biomarkers such as metallopro-teinase 3, Type II collagen neoepitopes (C2C and C1-2C), C-propeptide of Type II collagen (CPII), aggrecan 846 epitope, macrophage colony stimulating factor, serum amyloid A protein and interleukin-6, among others.
