O aprendizado por meio da interação com pacientes é vital para a educação médica. É sabido que a participação do aluno na prática médica é bem aceita pelos pacientes. Entretanto, na especialidade de Ginecologia, a natureza íntima da consulta torna a discussão do tema mais delicada. Este trabalho avaliou como as mulheres atendidas em ambulatórios do SUS percebem o atendimento ginecológico prestado por estudantes de Medicina e objetivou identificar pontos positivos e negativos da consulta relatados por elas. Foi feito um estudo de corte transversal com 360 mulheres através da realização de entrevistas. Os resultados apontaram que, embora a maioria das mulheres considerasse boa a presença do estudante, 52,7% relataram receio quanto à presença deste. A principal justificativa para o sentimento positivo foi o interesse do aluno. Dentre as que julgaram ruim a presença do estudante, a vergonha foi o principal motivo citado. A maioria avaliou que o sexo do aluno não influencia sua opinião sobre a consulta. Concluiu-se que a avaliação de como as mulheres vivenciam esse atendimento é essencial para aprimorar a relação estudante-paciente e aumentar a aceitação desse tipo de atendimento.

Learning through interaction with patients is vital for medical education. It is well established that the participation of the student in medical practice is well accepted by patients. However, in the specialty of gynecology, the intimate nature of the consultation makes the discussion of the topic more delicate. This study evaluated how women attending public clinics view gynecological care provided by medical students and intended to identify the strengths and weaknesses of the consultations they reported. This cross-sectional study was conducted through questionnaires answered by 360 women. The results showed that although most women consider student presence as a good thing, 52.7% reported fear of their presence. The main reason for the positive feelings was the student's interest. Among those who viewed it as a negative thing, feeling embarrassed was the main reason cited. The majority found that student gender does not influence in their opinion about the consultation. In conclusion, the assessment of how women experience this care is essential to enhance the student-patient relationship and increase acceptance of this type of care.

O objetivo deste estudo foi analisar a tendência de menções a causas não relacionadas ao HIV/ AIDS em adultos que viviam com esta doença no Brasil. Foram analisados os dados de óbitos de pessoas de 15 a 69 anos, ocorridos entre 1999 e 2010. Utilizaram-se as razões de chance de mortalidade padronizadas para comparar a mortalidade de causas não associadas ao HIV/AIDS entre o grupo em que havia menção ao HIV/ AIDS na declaração de óbito e o grupo em que não havia. De 1999 a 2010, ocorreram 6.120.670 óbitos entre adultos no país. Os óbitos do grupo HIV representaram 2,2% do total. Causas não associadas ao HIV, mais especificamente, doenças cardiovasculares, do aparelho genito-urinário e neoplasias malignas não associadas ao HIV apresentaram crescimento maior no grupo em que também havia menção ao HIV/AIDS. Os resultados apontam para um aumento maior da mortalidade por doenças não associadas ao HIV entre pessoas que viviam com HIV/AIDS. Esse resultado sugere que o longo convívio com o HIV e a terapia antirretroviral exercem importante papel na ocorrência desses agravos.

The study analyzed mortality data for persons 15 to 69 years of age from 1999 to 2010. Standardized mortality odds ratios were used to compare mortality from non-AIDS-related causes in the group with HIV/AIDS listed on the death certificate and in the group without HIV/ AIDS. From 1999 to 2010, there were 6,120,670 deaths among adults in Brazil. Deaths in the HIV group represented 2.2% of the total. Non- AIDS-related causes, and more specifically cardiovascular diseases, diseases of the genitourinary system, and non-AIDS-related malignant neoplasms increased at higher rates in the group with HIV/AIDS listed on the death certificate. The results point to a greater increase in mortality from non-AIDS-related diseases among persons with HIV/AIDS. This finding suggests that long contact with HIV and antiretroviral therapy play an important role in the occurrence of these diseases.

Este estudio tiene como objetivo analizar la tendencia en la mención de las causas de muerte no relacionadas con el VIH/ SIDA, en los certificados de defunción de los adultos que vivían con el VIH/SIDA en Brasil. Se estudiaron con detenimiento los datos de las muertes de los adultos brasileños entre 1999 y 2010, además, se utilizaron las odds ratios estandarizadas de mortalidad para comparar las causas de muerte entre las personas que vivían con el VIH/SIDA y las personas que no vivían con la enfermedad. De 1999 a 2010, se produjeron 6.120.670 muertes de adultos en Brasil. En un 2,2% del total de muertes en adultos, el VIH/SIDA figuraba mencionado en el certificado de defunción. Las causas no asociadas al VIH/SIDA, más específicamente, las enfermedades cardiovasculares, las enfermedades genitourinarias y neoplasias malignas, presentan un aumento de mayor intensidad en la frecuencia respecto a las que se enumeran como causas de muerte para quienes también vivían con el VIH.

OBJETIVO: verificar a viabilidade do transplante autógeno de testículos na parede abdominal e omento, em ratos, sem anastomose vascular, analisando a estrutura histológica das células testiculares após o implante. MÉTODOS: foram utilizados 60 ratos Wistar, machos, de 10-12 semanas de idade, distribuídos em três grupos: grupo controle: 20 ratos sem orquiectomia, com operação simulada; grupo 2: 20 ratos com orquiectomia bilateral sendo um testículo implantado no omento maior; grupo 3: 20 ratos com orquiectomia bilateral, sendo um testículo implantado na parede abdominal. Após dois meses eles foram mortos e os testículos avaliados pelo exame anatomopatológico. RESULTADO: o peso dos implantes teve perda de 0,62g no grupo 2, de 0,73g no grupo 3 e no grupo controle houve aumento de 0,1g. Ao estudo anatomopatológico, no grupo controle a estrutura testicular foi preservada; no grupo 2 encontrou-se 80% de inflamação e necrose, não foram visualizadas células de Sertoli ou de Leydig, em dois animais encontraram-se túbulos seminíferos; no grupo 3 encontrou-se 75% de inflamação e 60% de necrose, somente em um conseguiu-se visualizar células de Sertoli e em três células de Leydig. CONCLUSÃO: não é viável o transplante autógeno de testículo sem anastomose vascular em ratos no omento maior e na parede abdominal.

OBJECTIVE: To verify the feasibility of autologous transplantation of testes to the abdominal wall and omentum of rats without vascular anastomosis, analyzing the histological structure of the testicular cells after implantation. METHODS: We used 60 male Wistar rats, 10-12 weeks of age, which were divided into three groups: control group: 20 rats without orchiectomy with sham operation; group 2: 20 rats undergoing bilateral orchiectomy, with one of the testicles being implanted into the greater omentum; and group 3: 20 rats submitted to bilateral orchiectomy, with one testicle implanted in the abdominal wall. After two months they were euthanized and the testes evaluated by histopathology. RESULTS: the weight of the implants had a loss of 0.62 g in group 2, 0.73 g in group 3, whilst in the control group testes increased by 0.1 g. In pathological studies, the testicular structure was preserved in the control group; in group 2 there was 80% of inflammation and necrosis, Sertoli and Leydig cells were not visualized, and seminiferous tubules were found in two animals; in group 3 we found 75% of inflammation and a 60% necrosis, Sertoli cells could be visualized in only one specimen, while Leydig cells were seen in three. CONCLUSION: autologous transplantation of testis to the greater omentum and abdominal wall without vascular anastomosis is not viable in rats.

INTRODUÇÃO: O objetivo deste estudo foi avaliar a concordância entre os kits de fator antinúcleo (FAN) HEp-2 em relação aos padrões de fluorescência observados. Objetivou-se, ainda, avaliar o desempenho dos kits em relação à intensidade de fluorescência apresentada pelo uso dos conjugados fluorescentes, o número de mitoses por campo presentes no poço de reação e a titulação necessária para se obter o limiar de reatividade mínima (1+) dos soros controle reagentes presentes nos kits. MATERIAIS E MÉTODOS: Setenta e quatro amostras com resultados conhecidos foram testadas em quatro kits comerciais para a pesquisa de FAN HEp-2. O processamento estatístico foi feito pelo programa STATA® 9.2 e a concordância entre os resultados foi aferida pelo teste Kappa. RESULTADOS: O índice de concordância entre os kits, obtido pelo teste Kappa, foi superior a 0,94. Houve grande variabilidade quanto ao número de mitoses por campo presentes por poço de reação e quanto à intensidade de fluorescência dos conjugados, além de ter havido significativa diferença em dois kits em relação à diluição necessária para obtenção do limiar de reatividade mínima (1+). CONCLUSÃO: A taxa de concordância geral obtida pelo teste Kappa foi elevada, atingindo índice superior a 0,94. As maiores diferenças foram devido à presença de amostras com padrão variando entre nuclear pontilhado fino (PF), nuclear pontilhado fino denso (PFD) e nuclear homogêneo (HOMO). É necessária maior padronização dos kits quanto ao soro controle para a obtenção do limiar de reatividade mínima e em relação à intensidade de fluorescência dos conjugados.

INTRODUCTION: The objective of this study was to assess the concordance among antinuclear antibody (ANA) HEp-2 kits as to the observed fluorescent patterns. The objective was also to evaluate their performance concerning the following items: fluorescence intensity due to the use of fluorescent conjugates, the number of mitosis per field on the reaction wells, and the required dilution to obtain minimum reactivity (1+) of the reagent control-serum present in the kits. MATERIAL AND METHODS: Seventy-four samples previously known results were tested using four commercial kits ANA HEp-2 kits. The statistical evaluation was carried out with STATA® 9.2 software and the concordance among the results was assessed with Kappa test. RESULTS: The concordance coefficient among the kits obtained with Kappa test was higher than 0.94. There was a great variability as to the number of mitosis per field on the reaction wells and the intensity of fluorescence conjugates. Concerning the required dilution to obtain a minimum reactivity (1+), there was a significant difference in two kits. CONCLUSION: The general concordance level obtained with Kappa test was higher than 0.94. The greatest differences were observed in samples with nuclear fine speckled (FS), nuclear dense fine speckled (DFS) and nuclear homogeneous (HOMO) patterns. As far as serum for minimum reactivity level and intensity of fluorescent conjugates are concerned, a better standardization is yet to be pursued.

A vigilância entomológica da doença de Chagas em Mambaí e Buritinópolis, no Estado de Goiás, Brasil, tem sido mantida com participação da população, notificando a presença de vetores nas habitações. Passado longo tempo após instituídas as ações de controle e tendo-se já certificado a interrupção da transmissão vetorial, buscou-se avaliar o conhecimento e as práticas da população nessa situação. Os resultados apontam progressivo desinteresse pelo tema doença de Chagas, atribuível à redução da magnitude do problema representado pela enfermidade, a pouca participação das escolas na vigilância, à pequena importância dos vetores secundários e nativos e, em conseqüência, às limitadas intervenções dos serviços de controle em resposta às notificações. Propõe-se, que atividades de busca direta por amostragem sejam periodicamente realizadas e maior envolvimento das instituições de ensino.

Entomological surveillance of Chagas disease in Mambaí and Buritinópolis, in the State of Goiás, Brazil, has been kept up through the local population's participation, consisting of reporting the presence of vectors inside their homes. A long time has elapsed since instituting these control measures and it has now been certified that vector transmission has been halted. Thus, this study sought to evaluate the population's knowledge and practices in this situation. The results show that there has been progressive indifference towards the topic of "Chagas disease", which can be attributed to the reduction in the magnitude of the problem that this disease represented, little participation in surveillance among schools, low epidemiological importance of secondary and native vectors and, consequently, limited control interventions from health services in response to notifications. It is proposed that direct search activities by means of sampling should be carried out periodically, and that there should be greater involvement among teaching institutions.

OBJETIVO: Verificar os níveis de PSA total no Hamster Sírio, Mesocricetus auratus, jovem e adulto e demonstrar possíveis correlações entre esses níveis com as alterações histológicas dos anexos sexuais: próstata, vesículas seminais e testículos. MÉTODO: Foram examinados dez (n=10) Hamsters jovens, com idade inferior a sete semanas de vida e vinte (n=20) Hamsters com idade superior a um ano. Fez-se a dosagem do PSA e estudo histológico dos anexos sexuais em ambos os grupos e procurou-se a correlação entre o PSA encontrado e as alterações histológicas. RESULTADOS: A média de idade para os animais jovens, (grupo controle), foi de 46,7 dias (Desvio-Padrão-DP=1,16). Nos animais adultos, (grupo experimental), a média de idade não foi determinada, embora todos apresentassem idade acima de um ano no momento da morte. A média do peso dos animais jovens quando foram mortos foi de 57kg e dos animais adultos 126,5g. O PSA foi dosado no plasma de todos os animais adultos e em sete dos animais jovens. Em três animais do grupo jovem o PSA não foi detectado. A média do PSA nos animais jovens foi de 0,252ng/mL (nanogramo por mililitro) e nos animais adultos de 0,325ng/mL. Os animais jovens não apresentaram alterações histológicas nos anexos sexuais examinados. Entre os Hamsters adultos, quatorze (70%) animais apresentaram alguma alteração nos anexos sexuais: dez (50%) apresentaram Hiperplasia Benigna da Próstata (HBP); um (5%) apresentou HBP, prostatite e inflamação das vesículas seminais; um (5%) inflamação supurativa das vesículas seminais; um (5%) apresentou infarto testicular e prostatite; um (5%) apresentou inflamação das vesículas seminais, sem HBP e prostatite. Não se detectou relação estatística entre os níveis de PSA e a ocorrência de HBP, embora os portadores da hiperplasia prostática exibissem médias de PSA bastante superiores às apresentadas pelos não portadores de hiperplasia. Não foram também determinadas relações estatísticas entre os níveis de PSA e as alterações das vesículas seminais e testículo. CONCLUSÕES: 1- O Hamster Sírio, Mesocricetus auratus, apresenta PSA sérico dosável e seu valor médio para o Hamster jovem é de 0,252ng/mL e no Hamster adulto é de 0,325ng/mL.2. Não foi possível correlacionar os níveis de PSA com as alterações histológicas encontradas nos anexos sexuais do Mesocricetus auratus.

BACKGROUND: The objective of this work is to verify total PSA in young and adult Syrian Hamster and demonstrate a possible correlation among those levels and sexual anexes histological changes: prostate, seminal vesicles and testicles. METHODS: On this experimental study ten young Hamsters were used, with ages below seven weeks, and twenty animals with age above one year. PSA dosage and a histological analysis of sexual anexes were performed in both groups in order to get the correlation among the PSA level and histological changes. RESULTS: The average age for young animals (control group) was 46.7 days (Std Dev=1.16). In adult animals (experimental group) the average age was undetermined, although all groups were above one year old, at the death moment. Average weigh of those young animals, when dead, was 57.0g (Std Dev= 3.5) and adult animals 126.5g (Std Dev=19.3) being acceptable. PSA was dosed in old adult animals plasma and in seven of the youngest ones. PSA was not detected in three animals of young group. Young animals PSA average was 0.252ng/mL (nanogran per mililiter) and in adults some presented 0.325ng/mL. Young animals did not present histologic changes. In adult hamsters, fourteen (70.0%) of them presented change in their sexual anexes: ten (50.0%) PBH (prostate benign hiperplasia), one (5.0%) HBP, prostatis and seminal vesicle inflammation, one (5.0%) seminal vesicle suppurative inflammation, one (5.0%) testicle infarct and prostatitis. There was no statistic relationship between PSA levels and PBH, although those with PBH evidenced PSA higher than those without PBH. The relationship between PSA levels and seminal vesicle and testicle changes were not yet determined. CONCLUSION: The Syrian Hamster, Mesocricetus auratus, present dosed PSA in blood, considering that, its average value for young Hamster is 0.252ng/mL and 0.328ng/mL for adults. To get a correlation between PSA levels and sexual anex histological changes was not possible in the present work.

Objetivo: Expor a técnica utilizada na colheita de sangue para dosagem do PSA ( Prostate Specific Antigen ) e retirada do aparelho geniturinário no hamster sírio, Mesocricetus auratus, e correlacionar os achados do PSA com as alterações histológicas dos anexos sexuais desse roedor. Métodos: Foram usados no experimento trinta (n= 30) Hamsters: dez (n=10) animais considerados jovens com idade média no momento da morte de 47,5 dias e vinte (n=20) animais considerados adultos com idade superior à um ano. Após serem anestesiados com cloridrato de quetamina e diazepam, foi colhido diretamente da veia cava, em nível de abdome superior, cerca de 1,5mL a 2,0mL de sangue para dosagem do PSA totalpelo método ELISA, com antígeno humano. Morriam após colheita do sangue. Constatado a morte do animal, fazia-se a laparotomia retirando-se em monobloco todo aparelho geniturinário para estudo histológico dos anexos sexuais. Correlacionou-se o PSA com as alterações histológicas encontradas. Resultados: Os animais após serem anestesiados com cloridrato de quetamina e diazepam intraperitonealmente, obteve-se um excelente plano anestésico, que possibilitou colher via trans-dérmica da veia cava inferior em abdome superior sangue para dosagem do PSA. O animal morria após colheita do sangue. Fazia-se a laparotomia, com retirada em monobloco do aparelho geniturinário para estudo histológico e comparação das alterações encontradas nos anexos sexuais com o PSA dosado. Entre os Hamsters Jovens a média do PSA encontrado foi de 0,252ng/mL. Desvio Padrão (DP) = 0,36. Entre os Hamsters adultos esta média foi de 0,325 ng/mL, DP= 0,12 . Quando comparou-se as médias do PSA entre os dois grupos de jovens e adultos obteve-se p= 0,0427. Dentre os Hamsters jovens, três apresentaram PSA não detectado. A análise histológica mostrou que, entre os animais jovens, não foi identificada qualquer alteração das estruturas microscópicas da próstata, vesículas seminais e testículos. Observou-se nos Hamsters adultos (n=20), setenta por cento de alterações nos anexos sexuais a saber: um caso (5%) de processo supurativo das vesículas seminais, um caso (5%) de processo inflamatório isolado das vesículas seminais, um caso (5%) de Processo inflamatório das vesículas seminais, HBP e prostatite concomitante e um caso (5%) de infarto testicular e prostatite. Onze animais (55%), dos Hamsters adultos, apresentaram HBP (Hiperplasia Benigna da Próstata). Conclusão: A técnica cirúrgica descrita mostrou-se eficiente para a obtenção dos orgãos e estruturas do aparelho geniturinário, estudo histológico desses orgãos e dosagem do PSA no Hamster Sírio, Mesocricetus auratus.

Purpose: To explain the available technique in collecting blood for PSA (Prostate Specific Antigen) dosing and the genitourinary system extracting from Syrian Hamster, Mesocricetus auratus, and correspond PSA findings to anexed sexual histologic changes. Methods: Thirty (n=30) Hamsters were used in the experiment: ten (n=10) animals were on the average age of 47,5 days on the decease mourent, being qualified as young and twenty ( n=20) animals over the age of one year were qualified as adults. After being under chetamina cloridate and diazepam anesthesia, on the superior abdomen level, c. 1,5mL to 2,0mL of blood were collected for total PSA dosing though ELISA method, with human antigen. They died after blood extracting. After verifying the animal death, laparatomy was carried out, extracting in complete monoblock the genitourinary system for a histologic study of sexual anexes. A PSA correlation was set with the found histologic changes. Results: An excellent anesthesian plan was obtained after shooting chetamina cloridate and diazepam intraperitoneum on the animals, that made possible to collect blood through the cava vein trans-derm via to the superior abdomen for the PSA dosing. The animal died after blood collecting. Laparatomy was carried out with the complete monoblock extracting of the genitourinary system for a histologic study of sexual anexes and found changes to dosed PSA. Among young Hamsters the PSA found level was 0,252ng/mL, Pattern change (PC) = 0,36. Among adult Hamsters, the average was 0,325ng/mL, (PC)=0,12. When comparing these PSA averages between the young and adult groups, the obtained result was p= 0.0427. Among young Hamsters, three of them presented PSA no detectable and young animals did not present histologic changes in their checked sexual annexes, among adult Hamsters, fourteen (70) of them presented change in their sexual annexes such as: ten (50%) present only PBH (Prostate Benign Hiperplasia), one (5%) prostatitis and seminal vesicle inflammation and PBH, one (5%) seminal vesicle suppurative inflammation, one (5%) seminal vesicle inflammation and one (5%) presented testicle infarct and prostatitis. Conclusion: The depicted surgical technique has shown to be efficient in obtaining the genitourinary system with its organs and structures, even for a histologic study of those organs and PSA dosing in Syrian Hamster, Mesocricetus auratus.

OBJECTIVE: To detect the prevalence of systemic hypertension in children and to establish the relation between blood pressure levels and sex, age, ethnicity, weight, and height. METHODS: The prevalence of systemic hypertension and its relation to sex, age, ethnicity, weight, and height were studied in 611 students aged 7 to 14 years out of 19.928 students classified according to age, ethnicity, and sex, who underwent anthropometric evaluation and blood pressure measurement. Hypertensive individuals were considered those whose blood pressure level was > the 95th percentile for age and sex, confirmed on 3 examinations. RESULTS: The prevalence of hypertension was 16.6% in the first evaluation, and 4.6% and 2.5% in the subsequent evaluations. The mean blood pressure levels increased with age. Weight was important, not only to determine blood pressure in healthy children, but also to determine systemic hypertension in children, which was not observed with height despite the different studies. The prevalence of systemic hypertension in the different ethnic groups and the mean blood pressure levels according to sex were similar. CONCLUSION: In addition to routine physical examinations, age, weight, and appropriate cuff size should be considered when assessing blood pressure in children to prevent hypertension, morbidity and mortality, and to avoid placing a financial burden on health care providers.
