Abstract Purpose To assess the action of pentoxifylline, administered by subcutaneous route, on skin flap tissue repair in rats, and to verify the histological aspects and biomarkers. Methods Thirty-two male Wistar rats were divided into four groups: control (CT) and treated with pentoxifylline (P1, P3 and P5). Modified McFarlane technique flap was used. Ten days later, the animals were euthanized and the areas of viable and necrotic tissue were evaluated. Hematoxylin/eosin staining was used to assess the morphometric characteristics of the number of vessels and epithelial thickness. Picrosirius red was used to assess collagen density. VEGF and TGF-?1 levels on the skin flap and serum of the animals were measured by the ELISA method. Results The macroscopic evaluation of the skin flap dimensions showed reduced necrotic tissue in the pentoxifylline (p < 0.05) treated groups. There was an increase in angiogenesis and reepithelization, demonstrated by analyses with an increased number of vessels (p < 0.05), VEGF and epithelial thickness. Fibrogenic effect showed decreased collagen density and TGF-β1 in the skin flap and serum. Conclusion The benefits of pentoxifylline administered by subcutaneous route, at dose 100 mg/kg, which was effective to improve the survival of skin flap by acting on tissue repair components, stimulating angiogenesis and reepithelization, in addition to reducing fibrogenesis

RESUMO Objetivos: verificar a influência do dimetilsulfóxido e da pentoxifilina na vitalidade e no processo de reparo tecidual de retalhos cutâneos em ratos. Método: foram estudados 30 ratos Wistar, nos quais foi confeccionado retalho cutâneo dorsal de 2cm de largura por 8cm de comprimento, de base caudal, e distribuídos em três grupos: Grupo Controle (n=10) com aplicação de gaze umedecida com solução salina a 0,9%, no leito do retalho, por 30 segundos; Grupo dimetilsulfóxido (n=10) com injeção de 1ml de dimetilsulfóxido a 5% divididos em cinco injeções de 0,2ml na transição dos segmentos do retalho; Grupo pentoxifilina (n=10) com injeção de 1ml pentoxifilina 20mg/kg, divididos em cinco injeções de 0,2ml na transição dos segmentos do retalho. Os fármacos foram administrados no transoperatório, em dose única e por via subcutânea. Os retalhos cutâneos foram observados quanto às alterações de cor e textura. No décimo dia de pós-operatório aferiu-se a dimensão do tecido viável e de necrose, seguido da exérese da peça para análise histológica. Resultados: a medida da dimensão de tecido viável e de necrose dos grupos não apresentou diferenças. A análise histológica mostrou que o grupo dimetilsulfóxido apresentou neovascularização, infiltrado inflamatório com leucócitos e estroma conjuntivo mais estruturado. O grupo pentoxifilina, mostrou neovascularização e infiltrado inflamatório com granulação moderada e intensa. O grupo controle evoluiu com maior índice de necrose no segmento distal. Conclusão: dimetilsulfóxido e pentoxifilina influenciaram na vitalidade do retalho e no processo de reparo tecidual. Entretanto, não evitaram a necrose macroscopicamente.

ABSTRACT Objectives: to verify the influence of dimethylsulfoxide and pentoxifylline on the vitality of cutaneous flaps in rats and the tissue repair process. Methods: were studied 30 Wistar rats, submitting them to a 2cm wide by 8cm long dorsal cutaneous flap, of caudal base. We distributed the animals in three groups: Control Group (n=10) with application gauze moistened with 0.9% Saline in the flap bed for 30 seconds; Dimethylsulfoxide group (n=10), with administration of 1ml of 5% dimethylsulfoxide divided into five injections of 0.2ml in the transition of the flap segments; Pentoxifylline group (n=10), with administration of pentoxifylline 20mg/kg, diluted to 1ml and divided into five injections of 0.2ml in the transition of the flap segments. Drugs were administered intraoperatively, in a single dose and subcutaneously. We observed the skin flaps for changes in color and texture. On the 10th postoperative day, we checked the dimensions of viable and necrotic tissues, followed by excision of the specimen for histological analysis. Results: the measurements of length of the viable and necrotic tissues between groups showed no differences. Histological analysis showed that the Dimethylsulfoxide group presented neovascularization, inflammatory infiltrate with leukocytes and more structured conjunctival stroma. The Pentoxifylline group showed neovascularization and inflammatory infiltrate, with moderate to intense granulation. The control group evolved with a higher rate of necrosis in the distal segment. Conclusion: dimethylsulfoxide and pentoxifylline influenced the vitality of the flap and the tissue repair process. However, they did not prevent necrosis macroscopically.

OBJETIVO: analisar através de biologia molecular a diversidade da microbiota da junção ileocecal antes e após a ressecção da válvula ileocecal e reconstrução do trânsito com e sem a criação de "neoesfíncter". MÉTODOS: Os animais foram distribuídos em dois grupos: Grupo A (n=7) com ressecção da válvula ileocecal e anastomose ileocólica término-terminal em plano único, e Grupo B (n=7) com ressecção da válvula ileocecal e anastomose ileocólica término-terminal em plano único e confecção do esfíncter artificial. Reoperados com 20 dias coletou-se novamente conteúdo intraluminar do íleo e do cólon. Das amostras coletadas, extraiu-se DNA para reação de PCR-DGGE. Os padrões de bandas eletroforéticas , gerados na reação, foram submetidos ao programa Bionumerics para análise da similaridade e da diversidade da microbiota. RESULTADOS: a diversidade da microbiota foi maior e em mais amostras do íleo do que as do cólon. O grupo com a válvula apresentou os maiores valores e variações no cólon de 2,11 a 2,93. Em três animais de cada grupo estabeleceu-se comparação da similaridade e não se assemelharam ao controle. CONCLUSÃO: a ressecção da válvula ileocecal levou à mudanças da microbiota ileal e, com a criação de novo esfíncter, as variações foram maiores.

OBJECTIVE: To analyze, through molecular biology, the diversity of the intestinal microbiota before and after resection of the ileocecal junction and reconstruction of intestinal transit with and without the creation of a neosphincter. METHODS: Fourteen Wistar rats were divided into two groups: Group A (n = 7), submitted to resection of the ileocecal valve and end-to-end, single-layer ileocolic anastomosis; and Group B (n = 7) with resection of the ileocecal valve and end-to-end, single-layer ileocolic anastomosis followed by construction of an artificial sphincter. Intraluminal contents were collected from both groups. The animals were reoperated 20 days after the first procedure, with new collection of intraluminal contents of the ileum and colon. From the samples collected, DNA was extracted for PCR-DGGE. The electrophoretic banding patterns generated in the reaction were analyzed for similarities and diversities of the microbiota. RESULTS: The diversity of microorganisms was larger and in more samples when collected from the ileum than from the colon. The group with the neosphincter showed the highest variation in the colon, from 2.11 to 2.93. In three animals from each group was established comparing the similarity and not resembled the control. CONCLUSION: ileocecal resection led to changes in ileal microbiota and, with the creation of new sphincter, the changes were even greater.

OBJETIVO: Avaliar o papel de válvulas artificiais constituídas por seromiotomias circunferenciais duplas no comprimento dos vilos, no diâmetro do intestino delgado e no peso de ratos. MÉTODO: Foram utilizados 40 ratos, distribuídos em quatro grupos. Os animais do Grupo R foram submetidos à ressecção de 50% do intestino delgado, sem criação de válvulas. No Grupo RV associaram duas válvulas à ressecção intestinal. No Grupo V foram criadas duas válvulas, sem ressecção intestinal. O Grupo C forneceu a altura normal dos vilos. A eutanásia deu-se entre o décimo e o 14º dia pós-operatórios. RESULTADOS: Houve aumento no comprimento dos vilos nos grupos R, RV e V. Comparado ao Grupo R, o comprimento dos vilos nos grupos RV e V foi semelhante nos segmentos proximal e distal. No Grupo RV, os vilos do segmento proximal tiveram comprimento superior ao do distal. No Grupo V, o comprimento dos vilos do segmento proximal foi menor que do distal. A alça intestinal teve diâmetro maior que no pré-operatório no Grupo R e nos segmentos proximal à primeira válvula e distal à segunda, dos grupos RV e V. A ressecção intestinal levou à perda ponderal nos grupos R e RV, sem diferença entre os grupos. No Grupo V houve ganho de peso, significativo quando comparado aos grupos R e RV. Apesar de não impedirem a perda ponderal em animais submetidos à ressecção, as válvulas não determinaram perda superior à da ressecção isolada. CONCLUSÃO: Essas válvulas parecem influenciar positivamente a adaptação intestinal e podem ser incluídas entre as técnicas de reabilitação intestinal cirúrgica, isoladamente ou precedendo intervenções de alongamento do intestino.

BACKGROUND: to evaluate the role of artificial valves created by double circumferential seromyotomies in the villi length, in the small intestine diameter and in the weight of rats. METHOD: 40 rats have been distributed into four groups. The R Group animals were submitted to the resection of 50% of the small intestine, without the creation of valves. In the RV Group, two valves were associated to intestinal resection. In the V Group, two valves were created, without intestinal resection. The C Group provided the villi average height. Euthanasia was performed between 10 and 14 days after surgery. RESULTS: there has been an increase in villi length in Groups R, RV and V. Compared to the R Group, the proximal segment villi length was greater than the distal one. In V Group, the proximal segment villi length was smaller than the distal one. The intestine had a larger diameter than in the presurgical in R Group, and in the segments proximal to the first valve and distal to the second one, in Groups RV and V. Intestinal resection led to weigh loss in Groups R and RV, without differences among the groups. In V Group there has been significant weight gain compared to groups R and RV. Although they did not prevent weight loss in the animals submitted to resection, the valves did not cause greater loss than that of the resection itself, isolated. CONCLUSION: these valves seem to positively interfere in intestinal adaptation and might be included among the techniques of surgical intestinal rehabilitation, isolated or preceding interventions of intestine lengthening.

OBJETIVO: Avaliar as alterações morfológicas ocorridas após à confecção de esfíncteres no intestino delgado de ratos. MÉTODO: Foram estudados 30 ratos, Wistar, distribuídos em três grupos de 10 animais. Grupo A, controle, foi retirado um segmento de intestino delgado de 20mm de extensão. Grupo B, foram realizados dois esfíncteres, um a 100mm e outro a 150mm da junção íleo-cecal. Grupo C, um esfíncter à 100mm da junção íleo-ceco-cólica. Entre 10º e 14º dia os animais do Grupo B e C foram re-operados para a ressecção do segmento intestinal envolvendo os esfíncteres com margem de 10mm proximal e distal. As aferições de peso foram feitas no pré-operatório das duas intervenções. O diâmetro das alças foi computado antes e depois da confecção dos esfíncteres. Nas peças ressecadas dos três grupos foram medidas as alturas das vilosidades dos segmentos pré e pós-esfíncter dos Grupos B e C, segmento intermediário do Grupo B e segmento intestinal do Grupo A. Os dados obtidos foram submetidos à análise estatística do programa SPSS. RESULTADO: O peso dos animais aumentou nos dois grupos com significância no grupo B. Aumento do diâmetro das alças e a média das alturas das vilosidades foram significante. CONCLUSÃO: A confecção dos esfíncteres leva a alterações da camada de vilos e dos diâmetros das alças, semelhantes àquelas que ocorrem na fase de adaptação intestinal após ressecções.

BACKGROUND: To evaluate variations in weight and diameter of the bowel rats submitted to small bowel surgical sphincter construction comparing values before and after surgery and to study modifications in the mucosal villus. METHOD: Thirty Wistar rats were submitted to the study divided in three groups of ten. Group A (control) - a segment of small bowel of 20mm of extension was removed. Group B - two sphincters, one 100mm and another 150mm from the ileocecal junction were done. Group C - a sphincter from 100mm of the ileocecal junction was done. Bowel samples were collected and villosity studies were studied 10 to 14 days after first surgery. Measurements of bowel diameters and weights were compared before and after surgery modifications at the end of the study. Height villus measurements of the segments on pre and post-sphincters on both Groups B and C and between sphincters in Group B and also intestinal segment of the Group A were done. Data were submitted to statistics analysis using SPSS program. RESULT: Animals weight increased in both groups and in Group B the most. Also, intestine diameter increased and the average of the villus heights was larger. CONCLUSION: Making intestine sphincters results in alterations of the villus layer and increased bowel diameter, similarly to patients that were submitted to wide bowel resections and are adapted. The animals' weight gain can be understood as improvement of nutrients absorption.

OBJETIVO: Realizar um estudo morfométrico comparativo da musculatura longitudinal e circular do intestino delgado de ratos submetidos à construção cirúrgica de um e de dois piloros no intestino delgado utilizando-se da técnica de criação de piloros proposta por Rena et al. MÉTODO: Foram utilizados 52 ratos Wistar machos divididos em três grupos. O Grupo A, destacado como controle, composto de 10 animas, cada um forneceu 20mm de segmento de intestino para o estudo. Os animais do Grupo B, composto de 32 animais, foram submetidos à construção de dois piloros, um a 100mm e outro a 150mm da válvula ileocecal e os animais do grupo C, composto de 10 animais, foram submetidos à construção de um piloro a 100mm da mesma. A eutanásia ocorreu no décimo dia. A morfometria das camadas musculares longitudinal e circular acima e abaixo dos piloros foi estudada com utilização do microscópico modelo "Axiostar plus" conectado à câmera "Axioncam Version 5.05.10" com objetiva X5 / 0,12 no o programa "AxioVision 3.1.2.1". RESULTADOS: As alças apresentaram aumento da espessura da camada muscular acima e abaixo do ponto da operação. O estudo morfométrico comparativo das camadas musculares longitudinal e circular mostrou aumento significativo em comparação ao controle, não demonstrando significância entre os grupos B e C. CONCLUSÃO: Os resultados deste estudo revelaram importante aumento de espessura das camadas musculares, porém, menos acentuados que aqueles descritos na literatura quando foram realizados em animais submetidos à estenose fixa. Esses dados indicam que a construção operatória de piloros pela técnica utilizada promove alterações musculares de menor monta, possivelmente, pela função valvular do piloro.

BACKGROUND AND PURPOSE: To undertake a morphometric comparative study of the longitudinal and circular layers of the small bowel muscularis propria in rats, whom underwent surgical construction of one and two pylorus in the small bowel, utilizing the technique of the creation of two pylorus described by Rena et al. METHODS: 52 male Wistar rats were divided in three groups. Group A, 10 animals, was defined as the control group, and 20 mm of the bowel segment were removed for the study. Group B animals underwent construction of two pylorus, with one at 100 mm and the second at 150 mm from the ileo-cecal valve, 10 animals. Animals from group C, 32 animals, underwent construction of one pylorus at 100 mm from the ileo-cecal valve. The rats were sacrificed at ten days after the procedure. The morphometric analysis of the longitudinal and circular layers of the muscularis propria above and below the pylorus were studied with the help of a microscope Axiostar Plus model connected to the Axioncam camera version 5.05.10 with 5x/0,12 objective utilizing the software Axion Vision 3.1.2.1. RESULTS: The bowel loops demonstrated thickening of the muscular layer in segments above and below the point where the surgical procedure took place. The morphometric comparative study of the longitudinal and circular muscular layers demonstrated significant increase when compared to the control group. There were no significant differences among groups B and C. CONCLUSION: The results demonstrated significant increase in muscle layers, however not to the point of the increase reported in the literature for animals that underwent fixed stenosis. These data indicates that surgical construction of the pylorus by the technic used by us promotes muscular changes of minor significance, possibly, by the valvular function of the pylorus.
