ABSTRACT Introduction: Trauma primarily affects the economically active population, causing social and economic impact. The non-operative management of solid organ injuries aims to preserve organ function, reducing the morbidity and mortality associated with surgical interventions. The aim of study was to demonstrate the epidemiological profile of patients undergoing non-operative management in a trauma hospital and to evaluate factors associated with mortality in these patients. Methods: This is a historical cohort of patients undergoing non-operative management for solid organ injuries at a Brazilian trauma reference hospital between 2018 and 2022. Included were patients with blunt and penetrating trauma, analyzing epidemiological characteristics, blood transfusion, and association with the need for surgical intervention. Results: A total of 365 patients were included in the study. Three hundred and forty-three patients were discharged (93.97%), and the success rate of non-operative treatment was 84.6%. There was an association between mortality and the following associated injuries: hemothorax, sternal fracture, aortic dissection, and traumatic brain injury. There was an association between the need for transfusion and surgical intervention. Thirty-eight patients required some form of surgical intervention. Conclusion: The profile of patients undergoing non-operative treatment consists of young men who are victims of blunt trauma. Non-operative treatment is safe and has a high success rate. RESUMO Introdução: O trauma atinge principalmente a população economicamente ativa, causando impacto social e econômico. O tratamento não operatório das lesões de órgãos sólidos tem como objetivo preservar a função do órgão, diminuindo a morbimortalidade envolvida nos tratamentos cirúrgicos. O objetivo do estudo foi demonstrar o perfil epidemiológico dos pacientes submetidos ao tratamento não operatório em um hospital de trauma, bem como avaliar o fatores associados ao óbito nesses pacientes. Métodos: Trata-se de uma coorte histórica de pacientes submetidos à tratamento não operatório de lesão de órgãos sólidos, em um hospital referência de trauma brasileiro, entre 2018 e 2022. Foram incluídos pacientes vítimas de trauma contuso e penetrante, analisando as características epidemiológicas, hemotransfusão e associação ou não com necessidade de abordagem cirúrgica. Resultados: Foram incluídos 365 pacientes no estudo. Trezentos e quarenta e três pacientes receberam alta (93,97%) e a taxa de sucesso no tratamento não operatório foi de de 84,6%. Houve associação entre o desfecho óbito e as seguintes lesões associadas: hemopneumotorax, fratura de esterno, dissecção de aorta e traumatismo crânio encefálico. Houve associação entre necessidade de transfusão e abordagem cirúrgica. Trinta e oito pacientes necessitaram de alguma abordagem cirúrgica. Conclusão: O perfil dos pacientes submetidos a TNO são homens jovens, vítimas de trauma contuso. O tratamento não operatório é seguro e apresenta alta taxa de sucesso. ABSTRACT A hepatic abscess caused by a swallowed foreign body is a rare and challenging diagnosis. Most patients have nonspecific symptoms, and more than 90% of patients do not remember having swallowed it, which occurred accidentally. In this setting, fish bones, chicken bones, and toothpicks are the most found foreign bodies. We reported the case of a 54-year-old male patient admitted with abdominal pain and intermittent fever. He was diagnosed with liver abscess and treated successfully with antibiotics and a laparoscopic procedure; a rosemary twig was found during the abscess drainage procedure. Furthermore, a literature review of 22 cases of laparoscopic treated liver abscesses associated with a foreign body was made.

RESUMEN Durante un procedimiento quirúrgico, ante una masa gástrica, el cirujano debe tomar la decisión de resecarla o posponer la cirugía (menor morbilidad), así como discutir con el equipo médico el mejor tratamiento. Los tumores mesenquimales gástricos que se encuentran durante la cirugía bariátrica son raros, especialmente el schwannoma gástrico (GS). Presentamos el caso de una mujer de 57 años, hipertensa y diabética en tratamiento, con un índice de masa corporal (IMC) de 36 kg/m², refiriendo dificultad para adelgazar con dieta y actividad física. La paciente fue sometida a un bypass gástrico Y de Roux con resección en cuña de un nódulo ubicado en la pared anterior de la escotadura angular. El análisis inmunohistoquímico fue fuertemente positivo para la proteína S100, mientras que c-kit (CD117), CD45, actina de músculo liso (SMA) y AE1/AE3 fueron negativos. El GS debe considerarse como un diagnóstico diferencial de los tumores gástricos. Presentamos una opción de tratamiento que le permite tratar a pacientes obesos con nódulos gástricos en cualquier lugar en el mismo procedimiento que la cirugía bariátrica.

RESUMO O objetivo deste artigo é discutir a evolução da produtividade do trabalho brasileira nas décadas de 1990 e 2000, para lançar luz sobre a resiliência da economia brasileira em recuperar o crescimento. O crescimento da produtividade do trabalho no Brasil, após mostrar taxas anuais positivas entre 1950 e 1979, ficou estagnado após 1980. Seguindo a metodologia de McMillan e Rodrik (2011), este artigo inicialmente decompõe o crescimento da produtividade do trabalho no período 1950-2011, de acordo com o componente ‘structural change’ (potencializador de crescimento) e o ‘within effect’ (que reduz o crescimento, se não for acompanhado por uma mudança estrutural significativa enquanto o país ainda estiver em processo de catching-up). A seguir, é apresentado um exercício econométrico para explicar os determinantes do componente ‘structural change’ no período 1995-2011. Os resultados mostram que a estagnação da produtividade brasileira é explicada pela tendência de supervalorização da moeda brasileira, pela reprimarização da cesta de exportação, pelo baixo grau de abertura comercial do Brasil e pelas altas taxas de juros reais vigentes no período.

ABSTRACT The aim of this paper is to discuss the evolution of the Brazilian labour productivity in the 1990s and 2000s to shed some light on the resilience of the Brazilian economy to recover growth. Labor productivity growth in Brazil, after showing positive annual rates between 1950 and 1979, became stagnant after 1980. Following McMillan and Rodrik’s (2011) methodology, this paper at first decompose labor productivity growth in the period 1950-2011, according to “structural change” (which is considered growth-enhancing) and “within effect” (which is growth-reducing, if not accompanied by significant structural change while the country is still pursuing its catching-up process). Next, an econometric exercised is presented to explain the determinants of the structural change component of the labour productivity since economic opening in the 1990s. The results show that the stagnation of the Brazilian productivity is explained by the overvaluation trend of the Brazilian currency, the reprimarization of the export basket, the low degree of Brazil’s trade openness and the high real interest rates prevailing in the period.

RESUMO Racional: A melhor técnica de hernioplastia incisional ainda não foi definida. Uma das dificuldades na comparação é a heterogenicidade no perfil dos pacientes avaliados. Objetivo: Analisar os resultados de três técnicas de hernioplastia incisional após cirurgia bariátrica aberta. Método: Os pacientes que realizaram hernioplastias incisionais foram estudados e divididos em três grupos: técnica onlay, sutura simples e técnica retromuscular. Resultados e qualidade de vida após o reparo utilizando o Carolina’s Comfort Scale foram avaliados através de análise de prontuários, contato telefônico e consultas eletivas. Resultados: Foram analisados 363 relatos cirúrgicos e 263 foram incluídos: técnica onlay (n=89), sutura simples (n=100), técnica retromuscular (n=74). O perfil epidemiológico dos pacientes foi similar entre os grupos. A técnica onlay apresentou maiores taxas de seroma (28,89%) e usou dreno com maior frequência (55,56%). A sutura simples obteve maior tempo de internação (2,86 dias). O escore de qualidade de vida foi pior na técnica retromuscular (8,43) em relação à onlay (4,7) e à sutura simples (2,34), especialmente devido às queixas de dor crônica. Não houve diferença quanto a recidiva em curto prazo. Conclusão: A técnica retromuscular apresentou pior qualidade de vida do que as demais técnicas em um grupo homogêneo de pacientes. Os três grupos não mostraram diferenças em termos de recidiva de hérnia em curto prazo.

ABSTRACT Background: The best technique for incisional hernioplasty has not been established yet. One of the difficulties to compare these techniques is heterogeneity in the profile of the patients evaluated. Aim: To analyze the results of three techniques for incisional hernioplasty after open bariatric surgery. Method: Patients who underwent incisional hernioplasty were divided into three groups: onlay technique, simple suture and retromuscular technique. Results and quality of life after repair using Carolina’s Comfort Scale were evaluated through analysis of medical records, telephone contact and elective appointments. Results: 363 surgical reports were analyzed and 263 were included: onlay technique (n=89), simple suture (n=100), retromuscular technique (n=74). The epidemiological profile of patients was similar between groups. The onlay technique showed higher seroma rates (28.89%) and used a surgical drain more frequently (55.56%). The simple suture technique required longer hospital stay (2.86 days). The quality of life score was worse for the retromuscular technique (8.43) in relation to the onlay technique (4.7) and the simple suture (2.34), especially because of complaints of chronic pain. There was no difference in short-term recurrence. Conclusion: The retromuscular technique showed a worse quality of life than the other techniques in a homogeneous group of patients. The three groups showed no difference in terms of short-term hernia recurrence.

RESUMO Objetivo: analisar as características clínicas e epidemiológicas, as complicações pós-operatórias e a perda de peso em pacientes submetidos à gastrectomia vertical por via convencional, em uma instituição de saúde do Sistema Único de Saúde (SUS). Métodos: estudo transversal, longitudinal, retrospectivo realizado a partir da coleta de dados de prontuários para análise de variáveis em pacientes submetidos à gastrectomia vertical aberta, no SUS, no período de julho de 2013 a janeiro de 2017. Resultados: foram analisados 296 pacientes operados no período do estudo, dos quais 54% eram do sexo masculino; a média de idade foi de 39,9 anos ±11,4; o índice de massa corporal (IMC) médio no pré-operatório foi de 43,5kg/m² e no pós-operatório, de 30,3kg/m²; a perda de excesso de peso foi de 73,6%; 83,24% apresentaram uma perda de excesso de peso maior do que 50%; o IMC pré-operatório foi maior no grupo com perda de peso menor do que 50%. Observou-se uma taxa de complicações precoces com necessidade de internamento de 5,4% e um índice de mortalidade de 1%. Conclusão: a gastrectomia vertical aberta é uma técnica segura e eficaz para a perda de peso e que pode ser mais realizada no SUS. Dentre as variáveis avaliadas, o IMC prévio foi a única relacionada com o sucesso pós-operatório.

ABSTRACT Objective: to analyse clinical and epidemiological characteristics, postoperative complications, and weight loss in patients undergoing conventional vertical gastrectomy in a hospital under Brazil’s Public Health System (SUS). Methods: cross-sectional longitudinal retrospective study based on data collection from medical records for variable analysis in patients undergoing open vertical gastrectomy in SUS, from July 2013 to January 2017. Results: we analysed 296 patients operated on during the study period, of which 54% were male. The average age was of 39.9 years ±11.4; the average body mass index (BMI) was of 43.5kg/m² in the preoperative period and of 30.3kg/m² in the postoperative period; and the excess weight loss was of 73.6% (83.24% had an excess weight loss greater than 50%). Preoperative BMI was higher in the group with weight loss below 50%. We observed a 5.4% rate of early complications requiring hospitalization and a 1% mortality rate. Conclusion: open vertical gastrectomy is a safe and effective weight loss technique, which can be more performed in SUS. Among the variables evaluated in our work, the previous BMI was the only one related to the postoperative success.

Resumo No período 2000-2008, que cobre a análise deste trabalho, enquanto o crescimento médio do PIB real no Brasil foi de 3,7% ao ano, a produtividade do trabalho na indústria de transformação apresentou variação negativa de 1,0% ao ano. No Brasil, tem sido um “clichê” avaliar as relativamente baixas taxas de crescimento econômico no período como resultado do baixo crescimento da produtividade do trabalho nas últimas décadas. No entanto, de acordo com a lei Kaldor-Verdoorn, o recíproco também poderia ser verdade: as baixas taxas de crescimento da produtividade do trabalho no Brasil poderiam ser o resultado das baixas taxas de crescimento do PIB real. Com base nos pressupostos de Kaldor, regredimos a taxa de crescimento da produtividade do trabalho de 21 indústrias manufatureiras contra três variáveis principais: o PIB real (que captura a lei Kaldor-Verdoorn), a taxa de investimento e uma proxy para inovação tecnológica. Nossos resultados confirmaram a validade da lei de Kaldor-Verdoorn, pois o crescimento do PIB real foi a variável mais significativa para explicar o comportamento da produtividade do trabalho na indústria manufatureira no Brasil na década de 2000, seguido da taxa de investimento bruto.

Abstract In the 2000-2008 period, which covers the analysis of this paper, while the average real GDP growth in Brazil was 3.7 percent per year, labor productivity in the manufacturing industry had a negative variation of 1.0 percent per year. In Brazil, there has been a “cliché” evaluation ofthe relatively low economic growth rates in the period as being the result of low labor productivity growth in the last few decades. However, according to the so-called Kaldor-Verdoorn law, the reverse could also be true: low growth rates of labor productivity in Brazil could be an effect of low growth rates of the real GDP. Based on Kaldorian assumptions, we regressed the change in labor productivity of 21 Brazilian manufacturing industries, covering the 2000-2008 period, on three main variables: the real GDP (which captures the Kaldor-Verdoorn law), gross investment to value added ratio, and technological innovation. Our results confirmed the validity of the Kaldor-Verdoorn law, as the real GDP growth was the most significant variable to explain the behavior of labor productivity in the manufacturing industries in Brazil in the 2000s, followed by the gross investment rate.

RESUMO: Este artigo, de cunho eminentemente analítico, mostra que no âmbito da política macroeconômica é necessária consistência entre as políticas monetária, fiscal, cambial e salarial para viabilizar taxas de juros reais médias inferiores às taxas de retorno médias sobre o capital, taxas de câmbio reais competitivas (em torno da taxa de "equilíbrio industrial") e taxas de salários que evoluam de acordo com o crescimento da produtividade, condições para que se assegure o crescimento econômico sobre bases sustentáveis. Já com respeito à política industrial é preciso perseguir estratégias de diversificação produtiva, notadamente no setor manufatureiro e nos segmentos tradable do setor de serviços, mediante a identificação de prioridades estratégicas tanto nas cadeias produtivas, segmentos e setores próximos à base de vantagem comparativa preexistente, como naqueles mais próximos à fronteira tecnológica internacional. Embora os argumentos analíticos favoreçam a estratégia de diversificação produtiva, esta não deve ser confundida com semi-autarquia, o que significa que as cadeias e setores que não sejam foco da política industrial devem ter alíquotas de importação zero ou próximas de zero.

ABSTRACT: This paper is predominantly analytical. Concerning the macroeconomic regime, we propose more consistency between monetary, fiscal, exchange rate and wage policies. Such consistency is to provide sustainable long-term economic growth with structural change. Also, it is to make it feasible to not only maintain average real interest rates below the average real return rates on capital, but also competitive real exchange rates (i.e., marginal undervalued real exchange rates) and real wages that increase in step with productivity growth. This will help guarantee sustainable economic growth. As for industrial policy, theoretical and empirical evidence suggests the need to pursue strategies that diversify production, especially within the manufacturing sector as well as within tradable segments of the service sector. Although theoretical arguments are favorable to strategies that diversify production in countries that did not manage to catch up with developed countries, such strategies should avoid semi-autarky policies, which means that production chains, segments and sectors that are not the focus of industrial policy should have zero import tariffs, or close to zero.

RESUMO Racional: Entre as opções para o tratamento cirúrgico da obesidade, o mais utilizado é o bypass gástrico em Y-de-Roux. A anastomose gastrojejunal dele pode ser realizada de duas maneiras, manualmente ou utilizando grampeador linear e circular, e as complicações são dividas em precoces e tardias. Objetivo: Comparar a incidência de complicações precoces relacionadas com a confecção manual da anastomose gastrojejunal no bypass gástrico utilizando sonda de Fouchet com calibres diferentes. Métodos: Foi realizada análise retrospectiva transversal com 732 pacientes submetidos ao procedimento, divididos em dois grupos, grupo 1 com anastomose de 12 mm (n=374), e grupo 2 de 15 mm (n=358). Resultados: Os grupos apresentaram taxas de estenose de anastomose de 11% e 3,1% respectivamente, com p=0,05. Outras variáveis relacionadas à anastomose também foram analisadas, porém sem significância estatística (p>0,05). Conclusão: O diâmetro da anastomose de 15 mm esteve relacionado à menor ocorrência de estenoses. Verificou-se, contudo, que estes pacientes apresentaram maior sangramento no pós-operatório e menor infecção de sítio cirúrgico. Não ocorreram fístulas na presente casuística.

ABSTRACT Background: Among the options for surgical treatment of obesity, the most widely used has been the Roux-en-Y gastric bypass. The gastrojejunal anastomosis can be accomplished in two ways: handsewn or using circular and linear stapled. The complications can be divided in early and late. Aim: To compare the incidence of early complications related with the handsewn gastrojejunal anastomosis in gastric bypass using Fouchet catheter with different diameters. Method: The records of 732 consecutive patients who had undergone the bypass were retrospectively analyzed and divided in two groups, group 1 with 12 mm anastomosis (n=374), and group 2 with 15 mm (n=358). Results: The groups showed anastomotic stenosis with rates of 11% and 3.1% respectively, with p=0.05. Other variables related to the anastomosis were also analyzed, but without statistical significance (p>0.05). Conclusion: The diameter of the anastomosis of 15 mm was related with lower incidence of stenosis. It was found that these patients had major bleeding postoperatively and lower surgical site infection, and in none was observed presence of anastomotic leak.

RESUMO Racional: O bypass gástrico consta com taxa de distúrbios nutricionais e eletrolíticos de aproximadamente 17%. Os déficits mais frequentes são a desnutrição proteica, férrica e de zinco, além das vitamínicas. Apesar de raros, alguns quadros de desnutrição atingem tal gravidade que acaba sendo indicada internação e, por vezes, procedimentos cirúrgicos revisionais ou de reversão. Objetivo: Apresentar proposta de cirurgia revisional para tratamento de desnutrição severa após bypass gástrico. Métodos: O procedimento consiste em reconstituir o trânsito alimentar pelo duodeno e jejuno proximal, mantendo o componente restritivo do bypass gástrico. Como estratégia adicional, é realizada ressecção do fundo gástrico, visando intensificar a supressão da grelina e evitando reganho excessivo de peso. Resultado: Após estabilização inicial com suporte hidroeletrlítico e nutricional, o procedimento foi realizado em dois pacientes como tratamento definitivo do quadro de desnutrição. Bons resultados foram observados em seguimento de um ano. Conclusão: Como opção de melhora e/ou resolução da defasagem nutricional, a terapia cirúrgica é uma das alternativas. Ainda não há consenso quanto à técnica a ser utilizada. O procedimento aqui apresentado é baseado em fatores fisiopatológicos para o tratamento desta condição, com bons resultados iniciais, sem efeitos colaterais significativos. Seguimento de mais longo prazo é necessário para determinação de sua eficácia.

ABSTRACT Background The gastric bypass has nutritional and electrolyte disturbances rate of approximately 17%. The most common deficits are protein malnutrition, ferric and zinc, in addition to the vitamin. Although rare, some malnutrition stages reach such severity that ends up being necessary hospitalization and sometimes revisional or reversal surgical procedures. Aim: To present a proposal of surgical revision for treatment of severe malnutrition after bariatric surgery. Methods: The procedure is to reconstitute the food transit through the duodenum and proximal jejunum, keeping the gastric bypass restrictive component. As an additional strategy, the gastric fundus resection is performed, aiming to intensify the suppression of the greline and avoiding excessive weight regain. Results: After initial stabilization, nutritional and electrolytic support, the procedure was performed in two patients as definitive treatment of malnutrition status. Good results were observed at one year follow up. Conclusion: As improvement option and/or resolution of the nutritional alterations, surgical therapy is one of the alternatives. There is still no consensus on the surgical technique to be performed. This procedure is based on pathophysiological factors for the treatment of this condition, with good initial results, without significant clinical alterations. Longer follow-up will determine its effectiveness.

Racional: Hipoglicemia hiperinsulinêmica com sintomas neuroglicopênicos severos tem sido identificada como complicação tardia e rara em pacientes submetidos à gastroplastia com bypass em Y-de-Roux. Porém, a gravidade potencial de suas manifestações exige tratamento definitivo desta condição. A falta de tratamento efetivo gera a necessidade de desenvolver métodos clínicos ou cirúrgicos mais eficazes. Objetivo: Apresentar proposta de operação revisional para o tratamento da síndrome de hipoglicemia hiperinsulinêmica. Métodos: O procedimento consiste em reconstituir o trânsito alimentar pelo duodeno e jejuno proximal, mantendo o componente restritivo do by-pass gástrico. Como estratégia adicional de manutenção ponderal, é realizada ressecção do fundo gástrico, visando intensificar a supressão da grelina. Resultado: O procedimento foi realizado em três pacientes com bom resultado em seguimento de um ano. Conclusão: O procedimento de reconstituir o trânsito alimentar pelo duodeno e jejuno proximal, mantendo o componente restritivo do by-pass gástrico no tratamento da hipoglicemia hiperinsulinêmica apresentou bons resultados iniciais podendo validar sua indicação para outros casos.

Background: Hyperinsulinemic hypoglicemia with severe neuroglycopenic symptoms has been identified as a late and rare complication in patients submitted to Roux-en-Y gastric bypass. However, the potential gravity of its manifestations requires effective treatment of this condition. The absence of treatment makes it necessary to develop more effective clinical or surgical methods. Aim: To present one surgical option to revisional surgery in the treatment of hyperinsulinemic hypoglicemia Methods: The procedure consists in reconstituting alimentary transit through the duodenum and proximal jejunum, while keeping the restrictive part of the gastric bypass. As an additional strategy to maintain weight loss, is realized gastric fundus resection, aiming to suppress ghrelin production more effectively. Results: It was used in three patients with successful results in one year of follow-up. Conclusion: The procedure to reconstruct the food transit through the duodenum and proximal jejunum, keeping the restrictive component of gastric bypass in the treatment of hyperinsulinemic hypoglycemia showed good initial results and validated its application in other cases with this indication.

RESUMOEste artigo analisa o chamado tripé da política macroeconômica brasileira, que desde 1999 tem combinado um regime de metas de inflação, um regime de taxa de câmbio flutuante e metas de superávit fiscal primário. A menos que o seu modus operandi seja alterado, o tripé não será capaz de libertar a economia brasileira de outra "possível trindade": altas taxas de juros reais, a apreciação da taxa de câmbio real e crescimento econômico muito baixo. Depois de analisar brevemente a base teórica sob o tripé macroeconômico, o artigo mostra por que este regime de política macroeconômica, se avaliado numa perspectiva de longo ou médio prazo, não tem sido capaz de garantir a estabilidade dos preços nem o crescimento econômico. Além da sugestão de romper com a estratégia brasileira de crescer com poupança externa, o documento também sugere três principais formas de mudar o modus operandi do tripé brasileiro: i) aumentar o horizonte de tempo para atingir a meta de inflação, como tem sido o experiência da maioria dos países que adotam esse regime de política monetária; ii) restaurar o papel anticíclico da política fiscal brasileira; e iii) adotar uma combinação de mecanismos que visem prevenir que a moeda brasileira entre em uma nova tendência cíclica da apreciação em termos reais.

ABSTRACTThe traps of the tripad of the Brazilian macroeconomic policy. This paper analyses the so-called tripod of the Brazilian macroeconomic policy, which since 1999 has been combining an inflation target regime, a floating exchange rate regime and targets for primary fiscal surplus. I argue that, unless its modus operandi is changed, the tripod will not be able to free the Brazilian economy from another "possible trinity": high real interest rates, real exchange rate appreciation and very low economic growth. After briefly analysing the theoretical base under the macroeconomic tripod, I will show why this macroeconomic policy regime, if it is evaluated in a medium or long-term perspective, has not been able to assure neither price stability nor economic growth. In addition to the suggestion of breaking with the Brazilian strategy of growing with foreign savings, the paper also suggests three main ways of changing the modus operandi of the Brazilian tripod: i) increase the time horizon for reaching the inflation target, as has been the experience of most countries that adopt this monetary policy regime; ii) restore the countercyclical role of the Brazilian fiscal policy; and iii) adopt a mix of instruments aiming at preventing the Brazilian currency from entering into a new cyclical trend of appreciation in real terms.

O objetivo deste artigo é discutir a tendência à supervalorização da moeda brasileira na década de 2000, apresentando um modelo econométrico para estimar a taxa de câmbio real (TCR) e que deveria ser usada como nível de referência para orientar a política econômica de longo prazo . No modelo econométrico, consideramos componentes estruturais de curto prazo e longo prazo, sendo que ambos podem ser responsáveis para explicar a tendência à sobrevalorização da moeda brasileira. Nosso exercício econométrico confirma que a moeda brasileira havia sido persistentemente sobrevalorizado ao longo de quase todo o período em análise, e sugere que o nível da taxa de câmbio real de referência de longo prazo foi alcançado em 2004. Em julho de 2014, a taxa de câmbio nominal média deveria ter sido em torno de 2,90 reais por dólar (contra uma taxa nominal de 2,22 reais por dólar observada) para atingir o real nível de referência de 2004 (média do ano). Ou seja, de acordo com nossas estimativas, em julho de 2014, o real brasileiro estava sobrevalorizado em 30,6% em termos reais em relação ao nível de referência. Com base nestes resultados, concluímos o artigo sugerindo uma combinação de instrumentos políticos que deveriam ter sido utilizados, a fim de inverter essa tendência à sobrevalorização da taxa de câmbio, incluindo uma meta para se chegar, no médio e no longo prazo, à uma taxa de câmbio real que favoreceria a alocação de recursos para sectores tecnológicos mais intensivos.

The aim of this paper is to discuss the trend of overvaluation of the Brazilian currency in the 2000s, presenting an econometric model to estimate the real exchange rate (RER) and which should be a reference level of the RER to guide long-term economic policy. In the econometric model, we consider long-term structural and short-term components, both of which may be responsible for explaining overvaluation trend of the Brazilian currency. Our econometric exercise confirms that the Brazilian currency had been persistently overvalued throughout almost all of the period under analysis, and we suggest that the long-term reference level of the real exchange rate was reached in 2004. In July 2014, the average nominal exchange rate should have been around 2.90 Brazilian reais per dollar (against an observed nominal rate of 2.22 Brazilian reais per dollar) to achieve the 2004 real reference level (average of the year). That is, according to our estimates, in July 2014 the Brazilian real was overvalued at 30.6 per cent in real terms relative to the reference level. Based on these findings we conclude the paper suggesting a mix of policy instruments that should have been used in order to reverse the overvaluation trend of the Brazilian real exchange rate, including a target for reaching a real exchange rate in the medium and the long-run which would favor resource allocation toward more technological intensive sectors.

The main goal of our paper is to provide analytical arguments to explain why Brazil has not been able to restore its long-term capacity for economic growth, especially compared with its economy in the 1950-1979 period (7.3 per cent per year on average) or even with a select number of emerging economies in the 1980-2010 period(6.7 per cent per year on average, against 2.3 per cent per year on average in Brazil in the same period). We build our idea of convention to growth based on the Keynesian concept of convention. For our purposes, this concept could be briefly summarized as the way in which the set of public and private economic decisions related to different objectives, such as how much to produce and invest, how much to charge for products and services, how to finance public and private debt, how to finance research and development, and so on, are indefinitely - or at least until there is no change- carried out by the political, economic and social institutions. This analytical reference can be connected to the Neo-Schumpeterian National Innovation System (NIS) concept, which emphasizes not only institutions associated with science and technology per se, but also the complex interaction among them and other institutions. In this paper we identify two conventions to long-term growth in the last three decades in Brazil: the liberal and the neo-developmental. We show that the poor performance in the Brazilian economy in terms of real GDP growth from the 1980s on can be explained by a weak coordination between short-term macroeconomic policies and long-term industrial and technological policies. This weak coordination, in turn, can be associated with the prevalence of the liberal convention from the 1990s on, which has emphasized price stabilization to the detriment of a neo-developmental strategy whose primary goal is to sustain higher rates of growth and full employment in Brazil.
