ABSTRACT Inferior vena cava filter embolization is not uncommon and can reach 11.8%. However, device migration to the heart is not frequent and occurs in cases after inferior vena cava filter fracture. We present the case of a young woman who was submitted to a routine inferior vena cava filter placement three days before and presented with hemodynamic instability. Since the device was not retrievable, the surgical team opted for an open cardiac surgery under cardiopulmonary bypass to remove the inferior vena cava filter and avoid other further complications.

Abstract Heyde syndrome manifests as aortic stenosis associated with gastrointestinal bleeding. We describe the case of a 64-year-old man who came to the emergency room due to acute heart failure and intermittent gastrointestinal bleeding. Treatment involves initial correction of anemia and heart failure followed by aortic valve replacement. The prosthesis used depends on the characteristics of each patient and valve replacement allows the resolution of bleeding in most cases. Gastrointestinal bleeding in patients with aortic stenosis is associated with severity of the valve obstruction. A mechanical prosthesis was used with no recurrent bleeding even with the need for lifelong anticoagulation therapy.

Resumo A partir da importância do setor sucroenergético para a economia brasileira, e da importância do processo de corte/carregamento/transporte (CCT) de cana no suprimento e no custo da produção do açúcar, álcool e derivados, este artigo tem como estudo de caso uma usina de cana-de-açúcar paulista. O objetivo deste trabalho foi construir propostas de redução do lead time do processo CCT de cana-de-açúcar, através da análise do Manufacturing Critical-Path Time (MCT) da usina estudada. Com os dados coletados nas visitas à campo, foi feito o MCT de uma tonelada de cana ao longo de seu processo logístico. Essa ferramenta apontou para o grande tempo de espera da cana aguardando para ser moída no pátio da usina. Por intermédio da teoria da dinâmica dos sistemas, concluiu-se que esse tempo era alto decorrente da variabilidade no campo, consequência da ineficiência do uso da colhedora e do sistema logístico de alocação dinâmica dos recursos de transporte nas frentes de colheita. Para melhorar a eficiência da colhedora, foi proposto uma redução de 40% de seu tempo de parada por “aguardando transbordo”, que resultaria em uma economia de R$1.172.784,00/ano. Para realizar essa redução, foram identificadas cinco propostas a serem implementadas em paralelo: integração dos sistemas de computador de bordo e o sistema do controlador de tráfego; integração dos computadores de bordo da colhedora e dos transbordos; implementação de sensores de volume de preenchimento da julieta; foco na gestão dos líderes de frente; e, implementação do trabalhador multifuncional.

Abstract This article parts from the importance of sugarcane based energy to the Brazilian economy and, consequently, the important role of the process that involves cutting/loading/transportation (CLT) of cane in the supplies and production cost of sugar, alcohol and its derivates. For this purpose, a study case was carried in a sugarcane factory from São Paulo state, aiming to build a proposal that could reduce the lead time in the CLT process through the analysis of the Manufacturing Critical-Path Time (MCT) of the studied industry. With the data collected in the field visits, the MCT of one tone of sugarcane during its logistic process was done. This tool revealed a long waiting time of the cane waiting to be milled in the patio of the factory. Through the theory of dynamic of the systems it was concluded that this time was high due to the variability of the field as a consequence of an inefficient use of the harvester and the logistic system of dynamic allocation of the transportation resources ahead in the harvest. To make the harvester more efficient, a reduction of 40% of the laying time “awaiting transshipment” was proposed, which would result in a save of R$ 1.172.784,00/year. For this reduction five proposals were identified to be implemented in parallel: integration of the systems of the board computer and the traffic control system; integration of the board computers of the harvester and the transports; implementation of truck filling volume sensors; focus in the management of basis leaders; and, implementation of a multitask worker.

O acúmulo de quilo no espaço pericárdico ou quilopericárdio é uma condição que, com maior frequência, ocorre após trauma, cirurgia cardíaca e torácica ou associado a tumores, tuberculose ou linfoangiomatose. Quando não é possível a identificação precisa da etiologia, o quilopericárdio é denominado primário ou idiopático. Essa é uma situação clínica rara. Descrevemos um caso em paciente do sexo feminino, com 20 anos de idade, tratada cirurgicamente. A propósito do caso, apresentamos breve revisão da literatura e comentários sobre quadro clínico, etiopatogenia, exames diagnósticos complementares e opções de tratamento.

The accumulation of chyle in the pericardial space, or chylopericardium, is a condition occurring most frequently after trauma, cardiac and thoracic surgery, or in association with tumors, tuberculosis or lymphangiomatosis. When its precise cause cannot be identified, it is called primary or idiopathic chylopericardium. This is a rare clinical entity. We report the case of a surgically treated 20-year-old female patient. A brief review of the literature and comments on the clinical presentation, etiopathogenesis, ancillary diagnostic tests and treatment options are also presented.

La acumulación de quilo en el espacio pericárdico o quilopericardio es una condición que con mayor frecuencia ocurre después de trauma, cirugía cardíaca y torácica o asociado a tumores, tuberculosis o linfoangiomatosis. Cuando no es posible la identificación precisa de la etiología, el quilopericardio se denomina primario o idiopático. Esta es una situación clínica rara. Describimos un caso en paciente del sexo femenino, con 20 años de edad, tratada quirúrgicamente. A propósito del caso, presentamos una breve revisión bibliográfica y comentarios sobre el cuadro clínico, la etiopatogenia, exámenes diagnósticos complementarios y opciones de tratamiento.

A proteção miocárdica permitiu enorme avanço na moderna cirurgia cardíaca, reduzindo a mortalidade e permitindo que operações cada vez mais complexas pudessem ser realizadas. A alteração na população eleita para procedimentos cirúrgicos cardiológicos mudou significativamente nas últimas décadas, com o aumento de pacientes mais idosos, com função ventricular deprimida e miocárdio hipertrofiado. Essa última condição, desde os primórdios da cirurgia cardíaca, constituiu-se em grande desafio. Diversas técnicas de proteção ao miocárdio hipertrofiado foram descritas, porém com resultados não alentadores. As características da hipertrofia miocárdica no adulto com cardiopatia cirúrgica apresentam particularidades desafiadoras. Nesse artigo, procuramos atualizar o estado da arte sobre a proteção miocárdica ao coração hipertrofiado.

The myocardial protection allowed great advance in cardiac surgery, decreasing the mortality and making more feasible complex surgeries. Latterly, the patient population elected for cardiac procedures has been changing towards elderly patients with ventricular function depressed and myocardial hypertrophy. The myocardial hypertrophy condition represents a great challenge since the beginning of the cardiac surgery. Several techniques have been described to protect the myocardial hypertrophy, however with no satisfactory results. In this manuscript we present the state of the art technique of myocardial protection.

Este estudo teve como objetivo avaliar a composição centesimal dos camarões alimentados com dieta complementada com semente de linhaça. Foram utilizados 800 camarões pós-larvas, divididos em 2 tratamentos: 1) grupo teste à base de caseína complementada com linhaça; e 2) grupo caseína. Os valores médios de umidade (79,8 ± 0,82; 78,9 ± 1,68), cinzas (0,62 ± 0,07; 0,68 ± 0,2), carboidratos (2,63 ± 0,13; 1,58 ± 0,7) e proteínas (15,6 ± 1,02; 16,0 ± 0,58) dos grupos controle e teste, respectivamente, não apresentaram diferença significativa ao final do experimento. Entretanto, a concentração de lipídio total sofreu uma redução significativa (p < 0,05) ao longo do estudo, em ambos os grupos, sendo mais pronunciada no grupo controle (74%), estando ao final do experimento o teor de lipídio mais elevado ( < 0,05) no grupo teste (1,87 ± 0,03) do que no grupo controle (1,12 ± 0,021). Contudo, o teor de colesterol no grupo teste (0,115 ± 0,004) apresentou-se inferior ao do grupo controle (0,1306 ± 0,001), embora sem diferença estatística significativa. Conclui-se que a semente de linhaça na ração parece aumentar o lipídio dos camarões, sugerindo que esse aumento não implica em elevação do colesterol. Havendo, ainda, a possibilidade de ser devido à maior incorporação dos ácidos graxos poliinsaturados.

The aim of this study was to analyze the chemical composition of shrimp fed with a diet complemented with linseed. 800 post larvae shrimp were divided into two types of treatment: 1) a test group based on casein complemented with linseed, and 2) a casein group. At the end of the experiment, the two treatments showed no significant differences in the mean values of humidity (79.8 ± 0.82; 78.9 ± 1.68), ashes (0.62 ± 0.07; 0.68 ± 0.2), carbohydrates (2.63 ± 0.13; 1.58 ± 0.7) and proteins (15.6 ± 1.02; 16.0 ± 0.58). However, the concentration of total lipids in both groups declined significantly during this study. This decline was more marked in the control group (74%). At the end of the experiment, the lipid content was higher (p < 0.05) in the test group (1.87 ± 0.03) than in the control group (1.12 ± 0.021). However, the cholesterol content in the test group (0.115 ± 0.004) was lower than in the control group (0.1306 ± 0.001), albeit without a statistically significant difference. It was concluded that the addition of linseed to the food seems to increase the shrimps' lipids, which suggests that this increase does not imply heightened cholesterol levels. It is also possible that the increase in lipids is due to the incorporation of a large proportion polyunsaturated fatty acids.

OBJETIVO: A insuficiência renal aguda (IRA) no pósoperatório (PO) de cirurgia cardíaca é complicação grave. O objetivo deste trabalho é avaliar o tempo de circulação extracorpórea (CEC) como fator de risco para IRA. MÉTODO: Foram avaliados 116 pacientes de um único centro, submetidos a cirurgia cardíaca com CEC. Foram avaliados os dados demográficos, características clínicas, variáveis intra e pós-operatórias. A creatinina sérica e o clearance de creatinina foram avaliados até o 5ºPO. IRA foi definida como necessidade de diálise. Os pacientes foram estratificados em dois grupos: grupo CEC< 70 min e grupo CEC> 90min. RESULTADOS: O aumento médio da creatinina sérica no PO foi 0,18+0,41 no grupo CEC<70min e 0,42+0,44 no grupo CEC>90min (p=0,005). Diálise foi necessária em 1,3% dos pacientes do grupo CEC<70min, e em 12,5% do grupo CEC> 90min (p=0,018). O risco relativo para diálise foi 1,12 (IC 95%, 1,00-1,20) para CEC>90min. Não houve diferença para mortalidade (5,2 versus 7,5%, p=0,631). CONSLUSÃO: O desenvolvimento de IRA no pós-operatório de cirurgia cardíaca foi observado em pacientes com tempo de CEC superior a 90 minutos, embora o clearance de creatinina não tenha demonstrado alteração entre os grupos.

OBJECTIVE: Acute renal failure following heart surgery is a major complication. The aim of this study was to evaluate if duration of cardiopulmonary bypass (CPB) is an important factor that influences kidney dysfunction. METHODS: One hundred and sixteen patients from a single center referred for heart surgery were evaluated. Demographic factors, clinical data, operative and postoperative variables were evaluated. Serum creatinine and creatinine clearance were calculated until the fifth postoperative day. Acute renal failure was defined as the requirement of dialysis therapy. Patients were divided into two groups, the CPB<70min Group, patients with CPB duration equal to or less than 70 minutes and the CPB<90min Group, patients with CPB duration equal to or more than 90 minutes. RESULTS: The median increases in serum creatinine were 0.18 + 0.41(CPB<70min) and 0.42 + 0.44 (CPB>90min p=0.005). Dialysis was indicated in 1.3% (CPB<70min) and 12.5% 90min - p = 0.018). The odds ratio for dialysis was 1.12 (95% CI; 1.00-1.20) for CPB>90min. There was no significant difference in mortality (5.2 versus 7.5%, p = 0.631). CONCLUSION: The greatest likelihood of developing kidney failure after heart surgery is observed when CPB is at least 90 minutes, although creatinine clearance was not significantly altered between the groups studied.

FUNDAMENTO: O tratamento intervencionista da insuficiência coronariana é subempregado nos pacientes em diálise, pois há poucos estudos comprovando sua eficácia. OBJETIVO: Avaliar os resultados do tratamento intervencionista da doença arterial coronariana em pacientes tratados por diálise no HC da FMB. MÉTODOS: Foram avaliados 34 pacientes em diálise submetidos à angiografia coronariana entre set/95 e out/04, divididos de acordo com a presença ou ausência de lesão coronariana, tipo de tratamento e presença ou ausência de diabetes. Os grupos foram comparados de acordo com suas características clínicas e sobrevida. A sobrevida dos pacientes submetidos a tratamento intervencionista foi comparada à sobrevida geral dos 146 pacientes em diálise na instituição no mesmo período. O tratamento intervencionista foi indicado nas mesmas situações clínicas que na população geral. RESULTADOS: Os 13 pacientes que realizaram coronariografia e não exibiam lesões coronarianas apresentaram sobrevida de 100% em 48 meses, contra 35% dos 21 coronarianos no mesmo período. Os diabéticos coronarianos apresentaram sobrevida inferior aos não-diabéticos. A angioplastia exibiu pior prognóstico quando comparada à cirurgia; entretanto 80% dos submetidos a angioplastia eram diabéticos. Os 17 pacientes submetidos a procedimentos de revascularização coronariana apresentaram sobrevida semelhante aos 146 pacientes gerais do serviço. CONCLUSÃO: Esta pequena casuística mostra que a revascularização miocárdica, quando indicada, pode ser realizada em pacientes em diálise. Esta conclusão é corroborada pelo índice de mortalidade semelhante nos dois extratos de pacientes: coronarianos revascularizados e pacientes em diálise de maneira geral.

BACKGROUND: Interventional treatment of coronary insufficiency is underemployed among dialysis patients. Studies confirming its efficacy in this set of patients are scarce. OBJECTIVE: To assess the results of interventional treatment of coronary artery disease in patients undergoing dialysis. METHODS: A total of 34 dialysis patients submitted to coronary angiography between September 1995 and October 2004 were divided according to presence or absence of coronary lesion, type of treatment and presence or absence of diabetes mellitus. The groups were compared according to their clinical and survival characteristics. Survival of patients undergoing interventional treatment was compared to overall survival of 146 dialysis patients at the institution in the same period. Interventional treatment was indicated to the same clinical conditions in the general population. RESULTS: Thirteen patients with no angiography coronary lesions presented a survival rate of 100% in 48 months as compared to 35% of 21 patients with coronary artery disease. Diabetic patients had a lower survival rate compared with non-diabetics. Angioplasty had a worse prognosis compared to surgery; however, 80% of patients undergoing angioplasty were diabetic. Seventeen patients submitted to interventional procedures presented a survival rate similar to that of the others 146 hemodialysis patients without clinical evidence of coronary disease. CONCLUSION: This small series shows that myocardial revascularization, whenever indicated, can be performed in dialysis patients. This conclusion is corroborated by similar mortality rates in two groups of patients: coronary patients submitted to revascularization and overall dialysis patients.

OBJETIVO:Avaliar os fatores de risco clínicos pré-cirurgicos para o desenvolvimento de Insuficiência Renal Aguda (IRA) em pacientes submetidos à cirurgia cardíaca. MÉTODO: Foram estudados, de modo prospectivo, 150 pacientes submetidos à cirurgia cardíaca, durante 21 meses consecutivos, havendo um leve predomínio de homens (57%), idade média de 56 ± 15 anos, sendo que 66% apresentavam insuficiência coronariana como principal diagnóstico e 34% valvulopatias. A mediana da creatinina sérica no período pré-operatório foi de 1,1 mg/dl. IRA foi definida como elevação de 30% da creatinina sérica basal. O protocolo de variáveis clínicas teve seu preenchimento iniciado 48 horas antes do procedimento cirúrgico e encerrado 48 horas após o mesmo, incluindo variáveis cardiológicas e não-cardiológicas, além de resultados laboratoriais. RESULTADOS: A IRA esteve presente em 34% dos casos. Após análise multivariada, presença de doença vascular periférica foi fator pré-operatório identificado. CONCLUSÃO: Os resultados obtidos nesse estudo permitiram sinalizar alguns fatores contributivos para o desenvolvimento de IRA em cirurgia cardíaca, o que pode possibilitar condutas clínicas simples para evitar a disfunção renal nestas situações e, conseqüentemente, redução da taxa de mortalidade. No presente trabalho, o tamanho da amostra talvez tenha impedido a identificação de outros fatores de risco significativos.

OBJECTIVE: To evaluate clinical risk factors for the development of Acute Renal Failure (ARF) in patients who undergo cardiac surgery. METHOD: Over a period of 21 consecutive months, one hundred and fifty patients who underwent cardiac surgery were studied. There was a slight prevalence of men (57%) and the average age was 56 ± 14.8 years. Sixty-six percent presented with coronary artery disease as the main diagnosis and 34% valvular heart disease. The median preoperative serum creatinine was 1.1 mg/dL. ARF was defined as a 30% increase in serum creatinine above baseline. The protocol of clinical variables initiated 48 hours before the surgical procedure and finished 48 hours after it and included cardiological and non-cardiological variables and laboratory data. RESULTS: ARF was present in 34% of the cases. After multivariate analysis, being a man and the presence of peripheral vascular disease were identified as the preoperative factors. CONCLUSION: The results obtained in this study identified some risk factors for the development of ARF in cardiac surgery, suggesting simple clinical procedures that may prevent renal dysfunction in these situations and, consequently, reduce the mortality rate. In the present study, the sample size has possibly impeded the identification of other significant risk factors.

OBJETIVO: Comparar e avaliar experimentalmente as alterações estruturais e ultra-estruturais em corações hipertrofiados isolados de coelhos submetidos à parada protegida pela solução de cardioplegia sangüínea e cardioplegia cristalóide. MÉTODO: O estudo compreendeu um grupo controle e dois grupos experimentais. No grupo I, a parada cardíaca foi obtida pela infusão da solução de cardioplegia sangüínea contínua e tépida. No grupo II, a parada cardíaca foi conseguida pela infusão da solução de cardioplegia cristalóide intermitente e fria. No grupo controle, os corações foram submetidos à parada anóxia normotérmica por 45 minutos. Após experimentos, oito amostras da parede lateral do ventrículo esquerdo foram coletadas e fixadas em formaldeído 10% e glutaraldeído 2,5% para análises estrutural e ultra-estrutural. RESULTADOS: Os resultados estruturais e as descrições ultra-estruturais mostraram que os corações submetidos à parada protegida pela cardioplegia sangüínea contínua e tépida (grupo I) estavam mais preservados com alterações celulares menos acentuadas se comparados aos submetidos à parada protegida pela cardioplegia cristalóide intermitente e fria (grupo II) e ao grupo controle. CONCLUSÃO: A cardioplegia sangüínea contínua e tépida (Grupo I) foi mais eficiente na preservação da integridade estrutural e ultra-estrutural do miocárdio, quando comparada à cardioplegia cristalóide intermitente e fria (Grupo II).

OBJECTIVE: To experimentally compare the structural and ultrastructural changes in isolated hypertrophied rabbits' hearts submitted to cardiac arrest protected using sanguineous and crystalloid cardioplegia solutions. METHOD: The study comprised two experimental groups and one control group. In Experimental Group I, cardiac arrest was achieved by the continuous infusion of tepid sanguineous cardioplegia solution. In Experimental Group II, cardiac arrest was obtained by an intermittent infusion of a cold crystalloid cardioplegia solution. In the Control Group the hearts were submitted to normothermic anoxic arrest for 45 minutes. After the procedures, eight samples of the left ventricle lateral wall were collected and fixed in 10% formaldehyde and 2.5% glutaraldehyde for structural and ultrastructural analysis. RESULTS: The structural and ultrastructural results demonstrated that the hearts submitted to cardiac arrest protected by continuous tepid sanguineous cardioplegia, Group I, were better preserved and with less accentuated cellular alterations compared to those submitted to cardiac arrest protected using intermittent cold crystalloid cardioplegia and the Control Group. CONCLUSION: Continuous tepid sanguineous cardioplegia was more efficient in the preservation of the structural and ultrastructural integrity of the myocardium when compared to intermittent cold crystalloid cardioplegia.

OBJETIVO: Avaliar a intensidade de dor e o nível de funcionalidade em pacientes submetidos à cirurgia cardíaca nos períodos pré-operatório, 7º pós-operatório e alta hospitalar, relacionando-os entre si. Relacionar funcionalidade com: sexo, faixa etária, primeira cirurgia cardíaca ou reoperação, uso de circulação extracorpórea (CEC), tipo de cirurgia e acompanhamento fisioterapêutico. MÉTODO: Foram estudados 41 pacientes que realizaram cirurgia cardíaca eletiva por toracotomia médio-esternal (TME) no HC da Faculdade de Medicina de Botucatu/UNESP. A intensidade de dor foi avaliada pela escala de VAS e a funcionalidade, pela escala MIF (medida de independência funcional) no domínio físico. RESULTADOS: A intensidade de dor mais elevada foi no 7º pós-operatório comparado com os momentos pré-operatório e alta. No pré-operatório, não houve índice de dor; na alta, a intensidade mediana foi 3 (dor moderada). Os níveis mais elevados de perda funcional ocorreram no 7º pós-operatório, quando comparados com os escores totais do pré-operatório e da alta. Verificou-se correlação significativa entre dor e funcionalidade, demonstrando que o decréscimo do nível de dor entre o 7º pós-operatório e a alta contribuiu para a elevação dos níveis funcionais. CONCLUSÃO: As avaliações realizadas no pré-operatório proporcionaram resultados preditivos a serem alcançados. As avaliações realizadas no 7º pós-operatório e na alta possibilitaram a classificação dos pacientes de acordo com perdas e ganhos, indicando aqueles que necessitavam de maior cuidado e treinamento em suas capacidades.

OBJECTIVE: To evaluate, in patients submitted to heart surgery, the intensity of pain and the level of functionality during the preoperative period, on the 7th postoperative day and at hospital discharge. A secondary objective was to evaluate any possible relationship between pain and functionality taking into account the following variables: gender, age, first heart surgery or re-interventions, use of cardiopulmonary bypass (CPB), type of surgery and physiotherapeutic follow-up. METHOD: Forty-one patients who had undergone elective heart surgery at the Teaching Hospital of Botucatu/UNESP were studied. Pain intensity was measured using the VAS scale and functionality by the FIM scale (Functional Independence Measure) in the physical domain. RESULTS: It was observed that the intensity of pain was higher on the 7th postoperative day when compared with the preoperative period and at hospital discharge. No pain rating score was shown in the preoperative period, while a median pain intensity of 3 (moderate pain), was noted at hospital discharge. The highest levels of functional loss occurred on the 7th postoperative day compared to the total scores obtained in the preoperative period and at hospital discharge. A significant correlation between pain and functionality was observed; a decrease in level of pain between the 7th postoperative day and hospital discharge contributed to an increase in the functional levels. CONCLUSION: The evaluations performed in the preoperative period provided predictable results. The evaluations carried out on both the 7th postoperative day and at hospital discharge enabled a classification of patients according to their functional gain or loss, which contributed to identify those who require more care and training of their abilities.

O presente estudo teve por objetivo padronizar modelo experimental de estimulação cardíaca artificial atrial em coelhos. Foram utilizados 20 coelhos raça Norfolk-2000 de ambos os sexos com peso mínimo de 2200g. Após anestesia endovenosa com pentobarbital(30mg/Kg), foi dissecada a veia jugular externa esquerda e introduzido eletrodo Ethicon<FONT FACE="Symbol">Ò</font> especialmente preparado, até átrio direito via veia cava esquerda. Foram realizadas leituras dos parâmetros elétricos(ondaP, resistência e limiar de comando) no ato cirúrgico e com 7, 14 e 21 dias denominados M1,M2, M3 e M4 respectivamente. Observamos diferença significativa em todos as variáveis analisadas entre M1 e demais momentos. Os resultados são compatíveis com o que se conhece de estimulação cardíaca artificial em outras espécies animais e no homem. O presente modelo mostrou ser viável para estudos de estimulação cardíaca artificial.

The aim of the present study was to standardize an experimental model for artificial atrial heart pacing in rabbits. Twenty rabbits(Norfolk-2000, both sexes with minimum weigth of 2200g) were studied. After endovenous anesthesia with pentobarbital sodium(30mg/Kg) the external jugular vein was dissected and a pacemaker lead(Ethicon<FONT FACE="Symbol">â</font> ) was introduced through left cava vein until the rigth atriun. The eletric parameters(P wave, resistance and threshold) were accomplished at surgical time and at 7, 14 and 21 post-operative day. These experimental moments were denominated M1, M2, M3 and M4 respectively. The statistical analisys showed significant difference between M1 and all others experimental moments. The results are compatible with the actual knowlodge of artificial heart pacing in others animal species and in human beings. The present model showed to be viable for artificial pacing studies.
