Resumo Introdução: Os acidentes por transporte terrestre estão entre as dez principais causas de morte no mundo, principalmente entre os jovens. Essas mortes refletem um importante impacto socioeconômico e precisam ser mensurados. Um dos recursos é o indicador Anos Potenciais de Vida Perdidos (APVP). Objetivo: Descrever aspectos sociodemográficos e estimar os APVPs devido a acidentes por motocicletas no Brasil. Método: Estudo descritivo de dados secundários oficiais, incluindo óbitos por acidentes com motocicletas em 2019. O indicador APVP e a taxa APVP foram calculados por sexo, faixa etária e regiões geográficas. Resultados: Em 2019, ocorreram 11.182 óbitos por acidentes com motocicleta, envolvendo principalmente homens, solteiros, pardos, de 20 a 39 anos e ensino médio completo, os quais sucederam-se principalmente no Nordeste. A taxa de APVP foi de 194,6 anos perdidos/100.000 habitantes e as taxas de mortalidade variaram de 3,4 a 8,4 mortes/100.000 habitantes. A proporção de fatalidades foi oito vezes maior em homens que em mulheres. Conclusões: Os achados deste estudo reforçam que esses acidentes são um problema considerável, com impacto socioeconômico prevenível e, com ações intersetoriais e medidas de fácil execução e baixo custo. Abstract Introduction: Road traffic accidents are among the ten leading causes of death worldwide, especially among young people. These deaths have an important socio-economic impact and need to be measured. One resource is the Potential Years of Life Lost (PYLL) indicator. Objective: To describe sociodemographic aspects and estimate the PYLL due to motorcycle accidents in Brazil. Method: Descriptive study of official secondary data, including deaths from motorcycle accidents in 2019. PYLL indicator and PYLL rate were calculated by sex, age group and regions. Results: In 2019, there were 11,182 deaths from motorcycle accidents, involving mainly men, singles, browns, aged 20 to 39 and having completed high school, mainly in the Northeast. The PYLL rate was 194.6 years lost/100,000 inhabitants, and the mortality rates ranged from 3.4 to 8.4 deaths/100,000 inhabitants. The proportion of fatalities was eight times higher in men than in women. Conclusions: The findings of this study reinforce that these accidents are an important problem with a preventable socioeconomic impact by promoting intersectoral actions and measures that are easy to perform and low cost. Resumo O método aqui apresentado consiste em técnica cirúrgica minimamente invasiva para osteossíntese de fraturas transtrocantéricas com Dynamic Hip Screw (DHS) 135º. Esta técnica é indicada no tratamento de fraturas 31-A1 e 31-A2 (Classificação Arbeitsgemeinschaft für Osteosynthesefragen - AO) que cumpram os pré-requisitos exigidos para o uso do DHS. A cirurgia é realizada, preferencialmente, antes de 48 horas após o acometimento da fratura. Com a utilização do mesmo instrumental da técnica cirúrgica tradicional e auxílio do arco-C, realiza-se redução incruenta da fratura e implantação do DHS por incisão cirúrgica com 2 cm, através de dissecção dos tecidos subjacentes, com mínimo sangramento e agressão às partes moles. No pós-operatório imediato, o paciente é estimulado ao ortostatismo e à deambulação com carga total, o que antecipa a alta hospitalar e favorece a reabilitação funcional precoce. O retorno ambulatorial é agendado com 2, 6, 12 e 24 semanas de pós-operatório, com avaliação radiográfica, a fim de avaliar a consolidação da fratura.

Abstract The method presented here consists of a minimally invasive surgical technique for osteosynthesis of transtrochanteric fractures with Dynamic Hip Screw (DHS) 135º. It is indicated in the treatment of 31-A1 and 31-A2 fractures (Arbeitsgemeinschaft für Osteosynthesefragen Classification - AO) that meet the prerequisites required for using DHS. The surgery is performed, preferably, before 48 hours after the fracture. With the use of the same instruments as the traditional surgical technique and the aid of the C-arm, a closed reduction of the fracture and implantation of the DHS is performed by a 2-cm surgical incision, through dissection of the underlying tissues, with minimal bleeding and damage to the soft parts. In the immediate postoperative period, the patient is encouraged to orthostatism and walk with full load, which anticipates hospital discharge and favors early functional rehabilitation. Outpatient return is scheduled at 2, 6, 12 and 24 weeks postoperatively, with radiographic evaluation to assess fracture healing.

Investigou-se, através da análise digital de imagens, as repercussões do tratamento cirúrgico para controle da hipertensão porta e seus efeitos na vasculatura gástrica de pacientes jovens portadores de esquistossomose mansônica. Foram incluídos no estudo pacientes no pré-operatório (n=5) e grupos de pacientes submetidos à intervenção cirúrgica, em diferentes períodos (0-2 anos, n=04; 2-6 anos, n=13; acima de 6 anos, n=10). Foram obtidas biópsias endoscópicas da mucosa do antro e corpo gástrico, submetidas à rotina histológica e montadas em blocos de parafina. Confeccionaram-se lâminas histológicas que foram usadas para a análise histomorfométrica dos seguintes parâmetros: número médio de vasos por campo, diâmetro médio e espessura da parede dos vasos. Os resultados obtidos evidenciaram uma diminuição significativa da densidade e do diâmetro dos vasos a partir dos dois anos de pós-operatório até o período superior a 6 anos. Os dados dão suporte ao conceito de que a técnica cirúrgica ministrada ameniza, em longo prazo, as alterações histopatológicas específicas, como a hemorragia e a ectasia.

The repercussions from surgical treatment for controlling portal hypertension and its effects on the gastric vasculature of young patients with mansonic schistosomiasis were investigated by digital image analysis. The study included five patients at the preoperative stage and 27 patients who had undergone surgical intervention at different times in the past: 0-2 years ago, n=4; 2-6 years ago, n=13, and more than 6 years ago, n=10. Endoscopic biopsies were taken from the mucosa of the gastric antrum and body endoscopic mucosa and the samples underwent routine histological tests after embedding in paraffin blocks. Histological thin sections were used for histomorphometric analysis of the following parameters: mean number of vessels per field, and mean diameter and thickness of the vessel walls. The results showed that, between the patients whose operation was not more than two years ago and those whose operation was more than six years ago, there was a significant decrease in the density and diameter of the vessels. These findings give support to the concept that the surgical treatment administered decreases specific histological alterations like hemorrhage and ectasia, over the long term.
