Abstract Background Patients with epilepsy (PWE) frequently have comorbid psychiatric disorders, the most common of which are depression and anxiety. Attention deficit disorder with hyperactivity (ADHD) is also more frequent among PWE, though that condition has been scarcely studied among the adult PWE population. Objective This study aimed to compare the presence of ADHD symptoms between adult PWE and the general population. Methods This was an observational case-control study. Ninety-five adult PWE from a tertiary center in southern Brazil were compared with 100 healthy controls. All subjects were submitted to three structured scales: 1) the World Health Organization Adult ADHD Self-Report Scale version 1.1 (ASRS); 2) the Hospital Anxiety and Depression Scale (HADS); and 3) the Adverse Events Profile (AEP). Dichotomic variables were analyzed through chi-square test and Fisher's exact test, as appropriate, and non-parametric variables were analyzed through the Mann-Whitney U test. Results Medians and interquartile ranges (IR) were: 1) ASRS: 26.00 (IR: 18 to 38) among PWE versus 17.00 (IR: 11 to 24) among controls, p< 0.001; 2) HADS: 14.00 (IR: 8 to 21) among PWE versus 11.00 (IR: 8 to 16) among controls, p= 0.007; 3) AEP: 3800 (IR: 31 to 49) among PWE versus 33.00 (IR: 23 to 43) among controls, p= 0.001. Conclusion PWE showed a higher burden of symptoms of ADHD, depression, and anxiety when compared with controls, which replicates in the Brazilian population the findings of current literature that point toward a higher prevalence of such disorders among PWE. Resumo Antecedentes Pacientes com epilepsia (PCE) frequentemente apresentam comorbidades psiquiátricas, principalmente depressão e ansiedade. O transtorno do déficit de atenção e hiperatividade (TDAH) também é mais frequente nos PCE, porém foi pouco estudado na população adulta de PCE. Objetivo Comparar a presença de sintomas de TDAH entre PCE adultos e a população geral. Métodos Noventa e cinco PCE adultos de um centro terciário no Sul do Brasil foram comparados a 100 controles saudáveis. Todos os sujeitos foram submetidos a três escalas estruturadas: 1) a Escala Autorrelatada de TDAH em Adultos da Organização Mundial da Saúde, versão 1.1 (ASRS); 2) a Escala Hospitalar de Ansiedade e Depressão (HADS); e 3) o Perfil de Eventos Adversos (AEP). Variáveis dicotômicas foram analisadas através dos testes chi-quadrado e exato de Fisher, conforme apropriado, e as variáveis não paramétricas foram analisadas através do teste U de Mann-Whitney. Abstract Introduction Sensory deficits of the upper airways are common in people with Parkinson disease. Compounded by considerable deterioration of the swallowing function, these deficits may contribute to the elevated rates of morbidity and mortality among this population. Objective To examine the sensitivity of the vocal tracts of people with Parkinson disease using nasal videoendoscopy and to compare the results with paired controls. Methods The present was a prospective, observational, case-control study. Sensitivity assessments were conducted in a sample of 24 people divided into 2 groups: one group of 12 patients with Parkinson’s disease, and a control group with 12 healthy subjects. The study group also underwent a fiberoptic endoscopic evaluation of swallowing and answered the Swallowing Disturbance Questionnaire (SQD) to detect dysphagia. Results There was a significant difference (p < 0.05) in the region of the arytenoid cartilages, showing that sensitivity was better preserved among the control group, and that sensitivity deficits were present in the study group. The qualitative results showed sensorial impairment in the study group than in the control group regarding the base of the tongue, the vestibular folds, and the vocal folds. The study group showed self-awareness regarding the deficits in the swallowing function, but there was no statistically significant association between swallowing function deficits and deterioration of sensorial function among them. Resumo Introdução: A infeção do trato urinário (ITU) é a segunda infeção mais frequente da comunidade. Objetivos: Determinar os micro-organismos responsáveis pelas ITU nos cuidados de saúde primários e o seu perfil de sensibilidade aos antibióticos, bem como avaliar se a abordagem das ITU está a ser feita de acordo com as recomendações da DGS. Métodos: Estudo observacional e retrospetivo em quatro USF de Almada. Foram incluídos os utentes de idade igual ou superior a 18 anos, a quem foram codificados os problemas do ICPC-2 (U71-Cistite/Infeção Urinária Outra e U70-Pielonefrite/Pielite), de julho a dezembro de 2019. A recolha da informação foi feita através do programa informático SClínico e da plataforma MIM@UF. As variáveis estudadas foram: sexo, idade, tipo de ITU, pedido de urocultura, antibioterapia empírica, resultado da urocultura e teste de sensibilidade aos antimicrobianos. Resultados: Das 963 ITU, 88,3% ocorreram em mulheres, principalmente entre os 68 e 77 anos (18,9%). O tipo de ITU mais prevalente foi a cistite não complicada na mulher não grávida (56,3%). A antibioterapia empírica foi prescrita em 86,7% dos casos, sendo a fosfomicina o antibiótico mais utilizado (58,6%). Foram feitos 405 pedidos de urocultura, na sua maioria com resultado positivo. O micro-organismo mais frequentemente identificado foi a E. coli (66,4%), sendo o mais prevalente em todos os grupos etários e nos diferentes tipos de ITU, e sensível em 63,7% dos casos à fosfomicina. Na análise global das diferentes unidades verificou-se que a urocultura foi pedida em concordância com a norma da DGS em 70,4% dos casos e que a antibioterapia empírica esteve de acordo com a DGS em 63% dos casos. Conclusões: Neste estudo conclui-se que as ITU são mais frequentes nas mulheres entre os 68 e 77 anos. O micro-organismo mais prevalente é a E.coli, apresentando uma sensibilidade significativa à fosfomicina e à nitrofurantoína.

Abstract Introduction: Urinary tract infection (UTI) is the second most prevalent infection in the community. Aims: To determine the microorganisms responsible for UTI in Primary Health Care and their profile of sensitivity to antibiotics, as well as to evaluate if the approach of the UTI is concordant with the National Health Authority’s recommendations. Methods: Observational and retrospective study in four primary health care centers in Almada. Patients aged 18 years or older, whose appointments were coded with ICPC-2’s problems (U71-Cystitis/Urinary Infection Other and U70-Pyelonephritis/Pyelitis) between July and December 2019 were included. The SClínico software and the MIM@UF platform were chosen to collect data. The variables studied were: sex, age, type of UTI, urine culture request, empirical antibiotics, the result of urine culture, and antimicrobial sensitivity test. Results: Of the 963 UTIs, 88.3% occurred in women, mainly between 68 and 77 years old (18.9%). The most prevalent type of UTI was uncomplicated cystitis in non-pregnant women (56.3%). Empirical antibiotic therapy was prescribed in 86.7% of the cases, with fosfomycin being the most used one (58.6%). Four hundred and five urine culture requests were made, most of them with a positive result. E. coli was the most frequently identified microorganism (66.4%), being the most prevalent in all age groups and in the different types of UTI. It was sensitive to fosfomycin in 63.7% of cases. In the global analysis of the different units, it was found that urine culture was in accordance with the national recommendation in 70.4% of the cases and that empirical antibiotics were in accordance in 63% of the UTI cases. Conclusions: UTIs in the community are more frequent in women between 68 and 77 years old. The most prevalent bacterial microorganism is E.coli, with significant sensitivity to fosfomycin and nitrofurantoin.

RESUMO Objetivo Analisar os gastos públicos em saúde destinados aos serviços ambulatoriais em Fonoaudiologia nas cinco Regiões Federativas do Brasil. Métodos Trata-se de um estudo ecológico realizado por meio da busca de dados secundários disponíveis em uma plataforma virtual de domínio público, DATASUS (Departamento de Informática do Sistema Único de Saúde). Os dados coletados referiram-se aos valores aprovados para os procedimentos da Fonoaudiologia no período de 2009 a 2018 nas cinco Regiões Federativas. Adotou-se análise descritiva dos dados e exposição dos resultados em valores absolutos, relativos e taxas de crescimento. Resultados Verificou-se que o investimento médio em reais (R$), por ano, nos serviços de Fonoaudiologia no Brasil foi de, aproximadamente, R$ 223.952.639.232,00; sendo 47,2% dos recursos destinados à Região Sudeste. Dentre as grandes áreas de especialização, a Audiologia foi responsável por 95,4% do investimento, seguida pelas áreas da Linguagem (4,0%), Motricidade Orofacial (0,5%) e Voz (0,1%). Conclusão Os serviços fonoaudiológicos demandam considerável parcela dos recursos públicos, sendo a área da Audiologia responsável por quase metade desses gastos, seguida pelas grandes áreas da Linguagem, Motricidade Orofacial e Voz.

ABSTRACT Purpose To analyze public spending on health services to outpatient services in Speech Therapy in the five federative regions of Brazil. Methods This is an ecological study carried out through the search for secondary data available on a public domain virtual platform, DATASUS. The collected data refer to the values approved for the Speech Therapy procedures in the period from 2009 to 2018 in the five federative regions. Descriptive analysis of data and exposure of results in absolute and relative values and growth rates were adopted. Results It was found that the average investment in reais, per year, in speech therapy services in Brazil was approximately R$ 223,952,639,232.00, with 47.2% of the resources destined for the Southeast region. Among the major areas of specialization, Audiology accounted for 95.4% of the investment, followed by Language (4.0%), Orofacial Motricity (0.5%) and Voice (0.1%). Conclusion The findings indicate that speech therapy services demand a considerable portion of public resources, with the area of Audiology being responsible for almost half of these expenses, followed by the large area of Language, Orofacial Motricity and Voice.

RESUMO Objetivo Descrever e caracterizar um achado, o escape posterior tardio de resíduo alimentar na deglutição, segundo idade, gênero e consistência do alimento, que ocorreu no evento. Método A coleta de dados ocorreu por meio da análise de cada exame de videonasoendoscopia funcional da deglutição anteriormente gravado em um ambulatório especializado. A população do estudo contemplou 200 pacientes de ambos os gêneros, na faixa etária entre 46 e 87 anos, com e sem patologia de base para disfagia. As imagens foram estudadas individualmente pelo pesquisador e analisadas por juízes, com o objetivo de identificar e selecionar imagens que constatassem a presença ou ausência do evento em estudo. Resultados Verificou-se o escape tardio em 45 exames do total de 200 analisados. Os exames selecionados para o estudo apresentaram o escape residual posterior tardio em pelo menos uma consistência. A maior frequência do escape posterior tardio ocorreu com o líquido. A análise mostrou significância do evento em estudo com a população que apresentava idades mais avançadas da nossa amostra. Conclusão O escape posterior tardio ocorre predominantemente na consistência líquida, em população mais idosa e sem predomínio de gênero.

ABSTRACT Purpose To describe and characterize a finding, i.e., delayed posterior leakage of food residue during swallowing, according to age, gender and food consistency, which occurred in the event. Methods Data were collected through the analysis of each functional videonasoendoscopy test of swallowing previously recorded in a specialist outpatient clinic. The study population included 200 patients, both males and females, aged between 46 and 87 years, with and without an underlying pathology for dysphagia. The images were studied individually by the researcher and analyzed by judges in order to identify and select images that would confirm the presence or absence of the study event. Results Delayed escape was found in 45 out of the 200 analyzed tests. The tests selected for the study showed delayed posterior bolus leakage in at least one consistency. The highest frequency of delayed posterior leakage occurred with fluids. The analysis showed the significance of the study event with the population related to older ages in the sample analyzed. Conclusion Delayed posterior leakage occurs predominantly in the liquid consistency in older populations with no gender predominance.

RESUMO Esta pesquisa busca entender o processo de participação na organização dos Jogos Goianos da Saúde Mental. A finalidade é contribuir com a reflexão sobre as possibilidades e limites de construção de redes de cuidado em saúde mental. Trata-se de pesquisa com análise documental, de abordagem descritiva e qualitativa, que seguiu procedimentos de análise de conteúdo. Identificaram-se duas categorias empíricas, a saber: da participação presencial à construção de rede; e decisões que impactaram a participação dos usuários. Advoga-se aqui uma participação profunda que mobiliza sonhos coletivos que as práticas manicomiais não conseguem colocar em uma camisa de força.

ABSTRACT This research project seeks to understand the process of participation in the organization of the Mental Health Games of Goiás. It aims to contribute to the reflection on the possibilities and limits of building mental health care networks. This is a documentary analysis research project, with descriptive and qualitative approach, which followed content analysis procedures. Two empirical categories were identified, namely: from face-to-face participation to network construction; and decisions that impacted the participation of users. A profound participation is advocated, which mobilizes collective dreams that asylum practices cannot put on a straitjacket.

RESUMEN Esta investigación busca comprender el proceso de participación en la organización de los Juegos Goianos de Salud Mental. El objetivo es contribuir a la reflexión sobre las posibilidades y los límites de la construcción de redes de atención de salud mental. Se trata de una investigación con análisis documental, enfoque descriptivo y cualitativo, que siguió los procedimientos de análisis de contenido. Se identificaron dos categorías empíricas, a saber: desde la participación cara a cara hasta la construcción de redes; y decisiones que afectaron la participación de los usuarios. Aquí se aboga por una participación profunda que movilice sueños colectivos que las prácticas manicomiales no pueden poner en una camisa de fuerza.

Abstract Economic evaluations in Dentistry have been increasing in recent years. They are a relevant contribution if an economic issue exists. Knowing if a new intervention is an efficient way of allocating available (and scarce) resources (the concept of opportunity costs), a well-designed economic evaluation may be helpful. One option is to conduct a trial-based economic analysis, which extracts a considerable board of information from a trial. This approach produces a more controlled result since many sources of variations might be reduced. On the other hand, some aspects could not be predicted directly from the trial or even extrapolated. Thus, combining model-based analysis may be an idea. In this paper, we intended to discuss important aspects to be considered by researchers in further economic evaluations. This paper will be systematically divided into sessions related to the study design as time horizon and perspective, health effects, costs, and data analysis. In the end, we expect the reader could be able to plan a trial-based economic evaluation, which should be a careful, meticulous, quite laborious and especially transparent process.

Resumen: Este artículo analiza una investigación-intervención realizada con un grupo de apoyo a la adopción en la ciudad de Canoas, Rio Grande do Sul, Brasil. Con ese fin, articula los datos de la investigación con el concepto de grupo-dispositivo, para comprender cómo el concepto puede contribuir como una herramienta de intervención para el campo de la psicología en el apoyo de la adopción. Dieciocho miembros del Grupo de Apoyo a la Adopción, en diferentes fases del proyecto adoptivo, participaron en el estudio. Los datos se analizaron a través del Análisis del discurso desde una perspectiva foucaultiana, que comprende los discursos en una ubicación histórico-social singular. A través de los resultados fue posible mostrar cómo el grupo-dispositivo, en el campo de la adopción, desnaturaliza los significados establecidos sobre el perfil de adopción deseado, los obstáculos de la adaptación, los prejuicios involucrados en la adopción y los movimientos grupales. Al enfatizar los movimientos singulares, el concepto permite que otros sentidos sobre el cuidado, la protección y la constitución familiar sean producidos

Abstract: The research aimed to adapt the Social and Emotional Loneliness Scale for Adults (SELSA) to Brazil, gathering evidence of validity and accuracy. Two studies were carried out: in Study 1, 319 university students (Mage = 24.36) answered the SELSA and demographic questions. The exploratory factor analysis results indicated a three-factor structure (Family, Romantic and Social), explaining 70.38% of the total variance, with a Cronbach's alpha (α) ranging from .84 to .92; in Study 2, with 200 university students (Mage = 25.35) who answered the same instruments of the previous study. Confirmatory factorial analysis indicated adequate indicators (CFI = 0.98, TLI = .98, RMSEA = .07, Pclose = .06). Reliability was satisfactory (.77 to .93). It is concluded that the instrument presented good psychometric parameters, being an alternative tool for researchers investigating loneliness and its correlates, proposing new studies with a more representative sample

Resumo: A pesquisa objetivou adaptar a Social and Emotional Loneliness Scale for Adults (SELSA) para o Brasil, reunindo evidências de validade e precisão. Realizaram-se dois estudos: no Estudo 1, participaram 319 universitários (Midade = 24.36) que responderam a SELSA e a questões demográficas. Os resultados da AFE indicaram uma estrutura trifatorial (Familiar, Romântica e Social), explicando 70.38% da variância total, com um alfa de Cronbach (α) variando de 0.84 a 0.92; no Estudo 2, participaram 200 universitários (Midade = 25.35) que responderam aos mesmos instrumentos do estudo anterior. A análise fatorial confirmatória apontou indicadores adequados (CFI = 0.98, TLI = 0.98, RMSEA = 0.07, Pclose = 0.06). A confiabilidade foi satisfatória (0.77 a 0.93). Conclui-se que o instrumento apresentou bons parâmetros psicométricos, mostrando-se uma ferramenta alternativa para os pesquisadores que investigam a solidão e seus correlatos, propondo-se novos estudos com uma amostra mais representativa

Resumo Objetivos: verificar se existe associação entre fatores socioeconômicos e adesão ao tratamento no hipotireoidismo congênito e verificar se existe associação entre fatores socioeconômicos e sintomas vestibulococleares percebidos pelos pais / cuidadores de crianças diagnosticadas com hipotireoidismo congênito. Métodos: estudo transversal, exploratório e descritivo, com amostra de conveniência. A casuística foi composta por 108 crianças com diagnóstico clínico e laboratorial de hipotireoidismo congênito, de ambos os sexos com idade ≥ 5 anos. Foi aplicado um questionário estruturado para os pais/cuidadores, formado por questões fechadas e objetivas sobre a presença ou ausência de zumbido, hipoacusia e tontura/vertigem nas crianças com hipotireoidismo congênito. Resultados: não houve associação entre fatores socioeconômicos e adesão ao tratamento ou percepção dos sintomas cócleo-vestibulares. Conclusões: os fatores socioeconômicos não influenciaram na adesão ao tratamento nem na percepção de sintomas cócleo-vestibulares pelos cuidadores de crianças com hipotireoidismo congênito.

Abstract Objectives: to verify if there is an association between socioeconomic factors and adherence to treatment in congenital hypothyroidism and to verify if there is an association between socioeconomic factors and vestibulocochlear symptoms noticed by parents/caregivers of children diagnosed with congenital hypothyroidism. Methods: a cross-sectional, exploratory and descriptive study, with a convenience sample. The sample consisted of 108 children with clinical and laboratory diagnosis of congenital hypothyroidism, of both sexes, aged ≥ 5 years. The researchers applied a structured questionnaire to parents/caregivers, consisting of closed and objective questions about the presence or absence of tinnitus, hearing loss and dizziness/vertigo in children with congenital hypothyroidism. Results: There was no association between socioeconomic factors and adherence to treatment or perception of cochlear-vestibular symptoms. Conclusions: socioeconomic factors did not influence treatment adherence or perceived cochlear-vestibular symptoms by caregivers of children with congenital hypothyroidism.

RESUMO Objetivo: Investigar a manifestação de sintomas do transtorno do processamento auditivo central em crianças com hipotireoidismo congênito. Métodos: Estudo de caráter exploratório, descritivo e transversal com 112 pacientes com hipotireoidismo congênito com idade ≥5 anos. Realizou-se entrevista com os pais/cuidadores no momento da espera da consulta médica. Portadores de outras afecções médicas foram excluídos. Como instrumento de pesquisa utilizou-se o protocolo estruturado de anamnese para avaliação do processamento auditivo rotineiramente empregado por audiologistas. A análise estatística utilizou o teste Qui-quadrado. Resultados: A distribuição por sexo foi semelhante (meninas: 53,3%). Os casos não-disgenesia constituíram a forma fenotípica mais prevalente para o hipotireoidismo congênito (88,4%), e verificou-se que 65,3% das crianças apresentavam algum episódio de níveis séricos irregulares de hormônio tireoestimulante. Dentre as manifestações mais frequentes dos sintomas do transtorno do processamento auditivo central, as queixas relaciondas às funções cognitivas auditivas, como: figura-fundo (83,0%), atenção auditiva (75,9%) e memória auditiva (33,0%) foram as mais evidentes. Reclamações relacionadas ao rendimento escolar foram reportadas em 62,3%. Conclusões: Os dados obtidos evidenciaram altas frequências de sintomas de defasagem nas funções cognitivas relacionadas ao processamento auditivo central, em especial na atenção auditiva, figura-fundo e memória auditiva nos portadores do hipotireoidismo congênito.

ABSTRACT Objective: To investigate the presence of central auditory processing disorder symptoms in children with congenital hypothyroidism. Methods: An exploratory, descriptive, cross-sectional study of 112 patients with congenital hypothyroidism aged ≥5 years old. An interview was held with the parents/caregivers at the time of the medical consultation. Patients with other medical conditions were excluded. As a research instrument, the structured protocol of anamnesis was used to evaluate the auditory processing routinely used by audiologists. For statistical analysis, the chi-square test was used. Results: Sex distribution was similar in both boys and girls (girls: 53.3%). The most prevalent phenotypic form of congenital hypothyroidism was no dysgenesis (88.4%), and 65.3% of the children had an episode of irregular serum thyroid-stimulating hormone (TSH) levels. Among the manifestations of the most frequent central auditory processing disorder symptoms, problems were reported with regard to cognitive functions, as they related to hearing, such as figure-background ability (83.0%), auditory attention (75.9%) and auditory memory (33.0%). Complaints related to school performance were reported in 62.3% of the cases. Conclusions: The data obtained show a high frequency of lag symptoms in cognitive functions related to central auditory processing, particularly with regard to auditory attention, figure-background ability and auditory memory in patients with congenital hypothyroidism.

RESUMO Introdução: A qualidade da emissão vocal é dependente da integridade do mecanismo de retroalimentação auditiva. A presença de eventuais falhas nesse mecanismo está relacionada à indução do abuso e/ ou mau uso vocal e, por conseguinte, do surgimento de quadros de disfonia. Objetivo: Avaliar a influência da retroalimentação auditiva na intensidade e na frequência da voz, em indivíduos sem queixas vocais. Métodos: Participaram da pesquisa 40 sujeitos do gênero feminino, sem queixas vocais e com limiares auditivos dentro dos padrões de normalidade. As participantes foram submetidas a uma avaliação auditiva, composta por audiometria tonal liminar, imitanciometria e por uma avaliação acústica vocal dos parâmetros de intensidade e frequência, realizada em três momentos: antes, durante e após a exposição ao ruído branco. Resultados: Houve diferença significativa na relação das médias obtidas na intensidade e entre o limiar dos reflexos acústicos contralaterais e as frequências vocais, nos três momentos da avaliação acústica. Conclusão: Os achados sugerem que a retroalimentação auditiva interfere no controle da intensidade e frequência vocal.

ABSTRACT Introduction: The quality of the vocal emission is dependent on the integrity of the auditory feedback mechanism of the presence of eventual failures is related to the induction of abuse and / or vocal misuse, and therefore of the surgeon of dysphonia. Purpose: To evaluate the influence of auditory feedback in voice intensity and frequency in individuals with no vocal complaints. Methods: Participants were 40 female subjects without vocal and auditory thresholds within normal standards complaints. Participants underwent an auditory evaluation composed of pure tone audiometry, tympanometry and a vocal acoustic assessment of intensity and frequency parameters, carried out in three stages: before, during and after exposure to white noise. Results: There was significant difference in the average of the ratio obtained in intensity and between the threshold of contralateral acoustic reflexes and vocal frequencies in the three moments of acoustic evaluation. Conclusion: Thus, the findings suggest that auditory feedback interferes with the control of the intensity and vocal frequency.

ABSTRACT Endocochlear, retrocochlear and/or central origin hearing damage may be related to the absence of appropriate levels of thyroid hormone during morphogenesis and/or auditory system development. Hearing disorders related to the thyroid are not well studied, despite speculation on the pathophysiological mechanisms. The objective of this review was to characterize the main pathophysiological mechanisms of congenital hypothyroidism and to evaluate the relationship with central and peripheral hearing disorders. We conducted a literature review using the databases MedLine, LILACS, Cochrane Library, SciELO, Institute for Scientific Information (ISI), Embase, and Science Direct between July and September on 2016. We identified the studies that address hearing disorder mechanisms on the congenital hypothyroidism. Congenital hypothyroidism may have clinical and subclinical manifestations that affect the auditory system and may be a potential risk factor for hearing impairment. Hearing impairment can severely impact quality-of-life, which emphasizes the importance of monitoring and evaluating hearing during the clinical routine of these patients.

Resumo Objetivo Investigar a frequência de sintomas otoneurológicos em crianças com hipotireoidismo congênito e relacioná-los com os aspectos clínicos. Métodos Estudo exploratório de caráter descritivo seccional. A casuística foi composta pelos pais e/ou cuidadores de 105 crianças com diagnóstico de hipotireoidismo congênito e com 5 anos ou mais de idade. A coleta de dados utilizou um questionário estruturado elaborado e testado pelos pesquisadores. As informações clínicas e laboratoriais foram obtidas dos prontuários médicos. Resultados A maioria dos indivíduos (72,4%) demonstrou sintomas relacionados à disfunção vestibulococlear, na qual a tontura/vertigem correspondeu a 56,2% dos sintomas, seguida pela hipoacusia (43,8%) e pelo zumbido (12,4%). Houve correlação estatística entre os sintomas de hipoacusia (p=0,016) e a idade para o teste de rastreio neonatal, bem como os sintomas de tontura/vertigem com a etiologia do hipotireoidismo congênito (p=0,003). Na percepção da hipoacusia (RP=0,49; IC=0,31-0,77), a exposição ao fator demonstrou associação com a probabilidade de 95%. Conclusão Os achados sugerem significativa prevalência e associação entre hipotireoidismo congênito e sintomas otoneurológicos, especialmente tontura, hipoacusia e zumbido.

Abstract Purpose To investigate the frequency of otoneurological symptoms in children with congenital hypothyroidism and correlate them to the clinical aspects. Methods Exploratory study of sectional descriptive character. The sample was composed by parents and/or caregivers of 105 children diagnosed with congenital hypothyroidism and age of 5 years or more. Data collection used a structured questionnaire developed and tested by researchers. Clinical and laboratory data were obtained from medical records. Results Most subjects (72.4%) showed symptoms related to vestibulocochlear disorder, in which dizziness/vertigo corresponded to 56.2% of symptoms, followed by hearing loss (43.8%) and tinnitus (12.4%). We observed statistical correlation between hearing loss (p = 0.016) and age of neonatal screening test, as well as symptoms of dizziness/vertigo with congenital hypothyroidism (p = 0.003). For hearing loss (PR = 0.49; CI = 0.31 to 0.77), the exposure factor showed a correlation with the probability of 95%. Conclusion These findings suggest a significant prevalence and association between congenital hypothyroidism and otoneurological symptoms, especially dizziness, hearing loss and tinnitus.

We report a case of a 50-year-old woman who presented to the emergency department with large bowel obstruction and anemia. The initial imaging study suggested an inoperable rectal tumor with involvement of surrounding structures. In this paper, we discuss the diagnostic work-up of this patient with a diagnosis of pelvic/perirectal inflammatory myofibroblastic tumor (IMT). IMT is a rare tumor with intermediate malignant potential that frequently mimics clinical and imaging features of malignancy. Additionally, to the best of our knowledge, this is the first case of a pelvic IMT that regressed without surgical excision.

A doença renal isquêmica ou nefropatia isquêmica relacionada à doença renovascular pode evoluir de forma rápida e progressiva para a insuficiência renal crônica. É fundamental a identificação e o tratamento precoces desta condição clínica, prevenindo a ocorrência de doença renal em estágio terminal, com consequente necessidade de terapia de substituição renal. Há uma década, o controle da hipertensão renovascular era o objetivo primário no manejo de pacientes com doença renovascular. Atualmente, a meta está dirigida principalmente para a estabilização e a melhora da função renal, além do controle dos níveis pressóricos.

Ischemic renal disease or ischemic nephropathy related to renovascular disease can progress rapidly and gradually to chronic renal failure. Early diagnosis and treatment is crucial for this clinical condition, because they can prevent occurrence of end-stage renal disease, with consequent need for renal replacement therapy. A decade ago, renovascular hypertension control was the primary objective in the management of patients with renovascular disease. Currently, the goal is addressed primarily to the stabilization and improvement of renal function, in addition to blood pressure control.
