INTRODUÇÃO: A intervenção coronária percutânea (ICP) tem importante papel no tratamento da doença arterial coronária, em decorrência do desenvolvimento tecnológico e da ampliação de sua utilização nos cenários clínicos de maior complexidade. Apresentamos a experiência de serviço de hemodinâmica que iniciou sua atividade há cinco anos, localizado em hospital em Campinas (SP). MÉTODOS: De 2006 a 2011, 518 pacientes foram submetidos consecutivamente a ICP no Hospital Vera Cruz e incluídos neste estudo. As características clínicas, angiográficas e do procedimento e os resultados clínicos hospitalares foram analisados. RESULTADOS: A média de idade dos pacientes foi de 65,8 + 12,2 anos, 73,5% eram do sexo masculino, 29,9% eram diabéticos e metade dos procedimentos foi realizada na vigência de síndrome coronária aguda. No total, foram tratadas 890 lesões (74% delas do tipo B2/C), sendo implantado 1,6 stent/paciente, 37,1% dos quais farmacológicos. O diâmetro e o comprimento do stent foram, respectivamente, de 3,01 + 0,53 mm e 23,2 + 7 mm. O sucesso do procedimento foi de 93,6%, óbito ocorreu em 2,9% dos pacientes, acidente vascular cerebral em 0,8%, infarto do miocárdio em 3,5%, ICP de urgência em 0,8%, e trombose de stent em 0,97%. Hipertensão arterial [odds ratio (OR) 4,48, intervalo de confiança de 95% (IC 95%) 1,32-15,16; P = 0,016] e diabetes (OR 7,24, IC 95% 1,69-31,05; P = 0,008) foram preditores independentes de eventos adversos cardíacos e cerebrovasculares maiores (ECCAM) hospitalares. CONCLUSÕES: O sucesso do procedimento e a baixa ocorrência de ECCAM demonstram a efetividade e a segurança da ICP no atendimento de pacientes da prática clínica diária tratados em um novo serviço de cardiologia intervencionista, inserido em hospital localizado fora da capital do Estado de São Paulo.

BACKGROUND: Percutaneous coronary intervention (PCI) plays an important role in the treatment of coronary artery disease, as a result of technological development and its wide application in different clinical scenarios of greater complexity. We present data from a cath lab that started its activity five years ago at a private hospital in the city of Campinas (SP). METHODS: From 2006 to 2011, 518 consecutive patients were submitted to PCI at the Vera Cruz Hospital and were included in this study. Clinical, angiographic and procedural characteristics and the in-hospital clinical outcomes were analyzed. RESULTS: Patient average age was 65.8 + 12.2 years, 73.5% were male, 29.9% were diabetics and half of procedures were performed during an acute coronary syndrome. A total of 890 lesions (74% B2/C type) were treated with 1.6 stents/patient, 37.1% of them were drug eluting stents. Stent diameter and length were 3.01 + 0.53 mm and 23.2 + 7 mm, respectively. The procedure success was 93.6%, death occurred in 2.9% of the patients, stroke in 0.8%, myocardial infarction in 3.5%, reintervention in 0.8% and stent thrombosis in 0.97%. Hypertension [odds ratio (OR) 4.48, 95% confidence interval (95% CI) 1.32-15.16; P = 0.016] and diabetes (OR 7.24, 95% CI 1.69-31.05; P = 0.008) were independent predictors of in-hospital major adverse cardiac and cerebrovascular events (MACCE). CONCLUSIONS: The overall procedural success and low incidence of MACCE demonstrated the efficacy and safety of PCI in the daily clinical practice in a new interventional cardiology service, localized in a hospital outside the capital of the state of São Paulo.

FUNDAMENTO: Embora se reconheça que a cirurgia de reconstrução ventricular (CRV) promova remodelamento reverso, são necessários novos estudos para definir a influência da área de fibrose do ventrículo esquerdo (VE). OBJETIVO: Avaliar se a extensão da área de fibrose do VE é importante na recuperação funcional ventricular após CRV e correlacionar com fatores clínicos. MÉTODO: Análise prospectiva de 82 pacientes com disfunção ventricular submetidos à CRV. Analisou-se a importância das características clínicas e foram avaliadas as quantidades de fibrose, mensuradas por ressonância magnética em pequena, média e grande. RESULTADOS: Todos os pacientes foram acompanhados por 36 meses, com mortalidade de 6%. A quantidade de fibrose média foi de 25,8% ± 13,6%. Houve melhora da fração de ejeção do VE (FEVE), de 36,9% ± 6,8% para 48,2% ± 8,2% (p < 0,001). Houve relação inversa entre a quantidade de fibrose o incremento da FEVE (r = -0,83, p < 0,0001). Houve diminuição do volume sistólico final do VE de 43,3 ± 8,2ml/m² (p < 0,001). Houve melhora dos sintomas de insuficiência cardíaca, exceto nos pacientes com grande área de fibrose (p = 0,45). Os preditores independentes para eventos foram: área fibrótica (p = 0,01), idade (p = 0,01), volume sistólico final do VE (p = 0,03) e fração de ejeção (p = 0,02). O seguimento livre de evento foi diferente em relação à área de fibrose (p < 0,01). CONCLUSÃO: Em pacientes com disfunção ventricular, a extensão da área fibrótica foi um preditor independente da recuperação funcional do VE após CRV. A combinação de RMC e parâmetros clínicos podem auxiliar na indicação para CRV.

BACKGROUND: Although it is acknowledged that the ventricular reconstruction surgery (VRS) can promote reverse remodeling, new studies are necessary to define the influence of the left ventricular (LV) area of fibrosis. OBJECTIVE: To evaluate whether the extension of the area of fibrosis of the LV is important in the LV functional recovery after the surgery and correlate it with clinical factors. METHODS: Prospective analysis of 82 patients with ventricular dysfunction submitted to VRS. We analyzed the importance of the clinical characteristics and the amount of fibrosis was assessed, measured by cardiac magnetic resonance (CMR) as small, medium and large. RESULTS: All patients were followed for 36 months, with a mortality of 6%. The amount of medium fibrosis was 25.8% ± 13.6%. There was improvement in the left ventricular ejection fraction (LVEF), from 36.9% ± 6.8% to 48.2% ± 8.2% (p < 0.001). There was an inverse association between the amount of fibrosis and the increase in LVEF (r = -0.83, p < 0.0001). There was a decrease in the LV end-systolic volume of 43.3 ± 8.2ml/m² (p < 0.001). There was an improvement in heart failure symptoms, except in patients with large areas of fibrosis (p = 0.45). The independent predictors for events were: fibrotic area (p = 0.01), age (p = 0.01), LV end-systolic volume (p = 0.03) and LVEF (p = 0.02). The event-free follow-up was different in relation to the area of fibrosis (p < 0.01). CONCLUSION: In patients with ventricular dysfunction, the extension of the area of fibrosis was an independent predictor of the LV functional recovery after the VRS. The combination of cardiac MRI and clinical parameters can help in the indication for VRS.

FUNDAMENTO: Si bien se reconoce que la cirugía de reconstrucción ventricular (CRV) promueve remodelación reversa, son necesarios nuevos estudios para definir la influencia del área de fibrosis del ventrículo izquierdo (VE). OBJETIVO: Evaluar si la extensión del área de fibrosis del VI es importante en la recuperación funcional ventricular tras la CRV y correlacionarlo con factores clínicos. MÉTODO: Análisis prospectivo de 82 pacientes con disfunción ventricular sometidos a CRV. Se analizó la importancia de las características clínicas y se evaluaron las áreas de fibrosis, medidas por resonancia magnética y ponderadas como pequeña, mediana y grande. RESULTADOS: Se realizó un seguimiento de 36 meses a todos los pacientes, con mortalidad del 6%. La cantidad de fibrosis promedio fue del 25,8% ± 13,6%. Existió una mejora de la fracción de eyección del VI (FEVI), del 36,9% ± 6,8% al 48,2% ± 8,2% (p < 0,001). Existió relación inversa entre la cantidad de fibrosis y el incremento de la FEVI (r = -0,83, p < 0,0001). Hubo una disminución del volumen de fin de sístole del VI de 43,3 ± 8,2ml/m² (p < 0,001). Se produjo una mejoría en los síntomas de insuficiencia cardiaca, excepto en los pacientes con gran área de fibrosis (p = 0,45). Los predictores independientes para eventos fueron: área de fibrosis (p = 0,01), edad (p = 0,01), volumen de fin de sístole del VI (p = 0,03) y fracción de eyección (p = 0,02). El seguimiento libre de eventos fue diferente en relación con el área de fibrosis (p < 0,01). CONCLUSIÓN: En pacientes con disfunción ventricular, la extensión del área de fibrosis fue un predictor independiente de la recuperación funcional del VI luego de la CRV. La combinación de RMC y parámetros clínicos puede auxiliar en la indicación de CRV.

OBJETIVO: Analisar a importância da viabilidade do miocárdio e parâmetros clínicos na melhora da função ventricular, sintomas e prognóstico, como reversão do remodelamento. MÉTODO: Cento e quinze pacientes submetidos a revascularização do miocárdio com análise prévia da viabilidade do miocárdio com thallium-201. Fração de ejeção, volumes sistólicos e diastólicos do ventrículo esquerdo e classe funcional foram determinados, antes da cirurgia e a cada seis meses por 3 anos. RESULTADOS: Pacientes com > 4 segmentos viáveis demonstraram melhora da fração de ejeção de 34±6 para 44±4% (p<0,001), o volume diastólico foi de 113±31 para 91±22 ml/m² (p<0,001) e o volume sistólico foi 78,3±11 para 57±17 ml/m² (p<0,001). No grupo com < 4 segmentos viáveis, foi de 33,4±4 vs. 35,1±5% (p=0,19), o volume diastólico foi de 112±24 para 118±16 ml/m² (p=0,34), volume sistólico de 72±23 para 73±12 ml/m² (p= 0,81). Análise multivariada apontou fatores clínicos relacionados com evolução desfavorável (diabetes mellitus, mais que um infarto e longo intervalo de tempo entre infarto e operação). Melhora da classe funcional (NYHA) no grupo com mais segmentos viáveis e menor quantidade de eventos (log rank test, p = 0,0053). CONCLUSÃO: O benefício da revascularização, em pacientes com músculo viável, além da melhora função ventricular, proporciona remodelamento reverso, atenua a dilatação e reduz eventos.

OBJECTIVES: To evaluate the integration of both viability and clinical parameters on the improvement in systolic performance, symptoms and prognosis, with post-revascularization reverse remodeling. METHOD: One hundred and fifteen patients underwent thallium-201 imaging before myocardial revascularization. Left ventricular ejection fraction, left ventricular end-systolic volume index and left ventricular end-diastolic volume index were determined before and at each 6 months post-revascularization for 3 years. RESULTS: Patients with > 4 viable segments on thallium-201 imaging demonstrated an improvement in left ventricular ejection fraction from 34±6 to 44±4%, p<0.001), left ventricular end-systolic volume decreased from 78.3±11 to 57±17 mL/m², p<0.001; left end-diastolic volume decreased from 113±31 to 91±22 ml/m², p<0.001). Patients with < 4 viable segments failed to demonstrate an improvement of the left ventricle ejection fraction, 33.4±4 vs. 35.1±5% (p=0.19), and exhibited ongoing left ventricle end-systolic remodeling, 72±23 vs. 73±12 mL/m² (p= 0.81), and the left ventricle end-diastolic volume increased from 112±24 to 118±16 mL/m² (p=0.34), without improvement in NYHA class, and worse long-term prognosis (event; log rank test, p=0.0053). The multivariable analysis demonstrated clinical variables related to the unfavorable evolution showed diabetes, more than one myocardial infarction, and time interval between myocardial infarction and surgery were associated with worse prognosis. CONCLUSION: The benefits of myocardium revascularization in patients with viable muscle, as well as an improvement in the left ventricular ejection fraction, provide reverse remodeling, improvement in functional class and favorable long-term prognosis.

Estudou-se a germinação de uma espécie selvagem sob influência do ABA e GA3. Embriões isolados, incubados em soluções de sacarose contendo ABA e/ou GA3, foram analizados em SDS-PAGE para observar o perfil da mobilização de proteínas de reserva durante o início da germinação. Cotilédones de sementes incubadas em soluções aquosas de ABA e/ou GA3 foram analizados em sds-page e PAGE/Géis de Atividade para observar o perfil da mobilização de proteínas de reserva e atividade de proteases, respectivamente, após o início da germinação. Os resultados indicam que ABA bloqueia a mobilização protéica através da inibição da atividade enzimática em cotilédones, mas não impede totalmente a germinação, parecendo afetar germinação e atividade enzimática independentemente. Em embriões houve bloqueio da mobilização, mas a relação com a inibição da atividade enzimática não foi claramente demonstrada. ABA induziu a síntese de proteínas em cotilédones, mas não em embriões. Um polipeptídeo com 17 kD é degradado em 6 horas nos embriões, mas a degradação é bloqueada por ABA e/ou GA3. Para mesmas concentrações de ABA e GA3, GA3 não inibiu o efeito do ABA em embriões, mas inibiu em cotilédones. Os efeitos do ABA e GA3 não diferiram sensivelmente dos dados correntes, mas o comportamento do polipeptídeo de 17 kD é contraditório e sugere estudos específicos.

In this work, a woody species [A. peregrina (L.) Speg.] was studied in order to observe the effect of ABA and GA3 at the biochemical level during the process of seed germination. Embryos incubated in sucrose solution containing ABA and/or GA3 were analyzed through SDS-PAGE to observe the mobilization pattern of storage proteins during the beginning of germination. Cotyledons isolated from seeds incubated in aqueous solutions containing ABA and/or GA3, were also analyzed through SDS-PAGE and by PAGE/Activity Gels (polyacrylamide gels copolymerized with substrate for enzymes) to observe the mobilization pattern of storage proteins and protease activity after the beginning of the germination. Results of these experiments show that ABA blocks protein mobilization by inhibiting protease activity in cotyledons. This inhibition is not sufficient to prevent germination showing that the effect of ABA on germination is not dependent on protease activity. The blockage of storage protein mobilization was also observed in embryos, but no protease activity inhibition was clearly detected. ABA was able to induce the synthesis of proteins in cotyledons but not in embryos. A polypeptide with an approximate molecular weight of 17 kD, was degraded within 6 hours in control embryos, but this degradation was blocked by ABA and GA3. Using the same concentrations of ABA and GA3 on embryos and cotyledons, the effect of ABA was counteracted by GA3 in embryos, but not in cotyledons. Although the effects of ABA and GA3 were not so different from those shown in the literature, the behavior of 17 kD-polypeptide contradicts these reports suggesting that specific studies should be performed.
