RESUMO Introdução Poucos estudos realizaram, concomitantemente, o potencial evocado auditivo de tronco encefálico (PEATE) e o P300 na Síndrome de Down (SD), em indivíduos audiologicamente normais, para a avaliação da via auditiva central, principalmente pela dificuldade de realizar estes procedimentos nesta população. Estudos anteriores sugeriram que indivíduos com SD podem apresentar padrões de respostas diferentes das encontradas em indivíduos com desenvolvimento típico, sendo que a identificação destes seria fundamental para o estabelecimento de um diagnóstico audiológico preciso. Objetivo Caracterizar o PEATE e o P300 em indivíduos com SD audiologicamente normais. Método Foram analisados o PEATE e o P300 de 17 indivíduos com SD e 21 com desenvolvimento típico de sete a 15 anos. A análise foi quantitativa e qualitativa, utilizando medidas descritivas e os testes de hipótese. Resultados Os valores de latência foram menores no PEATE para o grupo SD, com diferença estatisticamente significante para a onda V, interpicos III-V e I-V; não foram encontradas diferenças significantes nos valores de latência do P300. Observou-se maior número de indivíduos com valores precoces para as latências do PEATE e com latências atrasadas para o P300 no grupo SD; ambas as comparações mostraram diferenças significantes. Conclusão Crianças e adolescentes com SD podem apresentar respostas precoces para os componentes do PEATE, sugerindo que a via auditiva destes necessita de menor tempo para a transmissão neural do estímulo acústico até o tronco encefálico. Quanto ao P300, indivíduos com SD podem apresentar latências aumentadas, sugerindo comprometimento na via auditiva central quanto ao processamento cortical da informação auditiva.

ABSTRACT Introduction Few studies have performed Brainstem (BAEP) and P300 Auditory Evoked Potentials simultaneously to assess central auditory pathways in normal hearing individuals with Down syndrome (DS), mainly because of the difficulty in applying these procedures to this population. Previous studies have suggested that individuals with DS might present different patterns of response compared with those of individuals with typical development; nevertheless, the identification of these potentials would be crucial for the establishment of an accurate audiological diagnosis. Purpose To characterize BAEP and P300 in normal-hearing individuals with DS. Methods BAEP and P300 were analyzed in 17 individuals with DS and in 21 individuals with typical development aged 7 to 15 years. The results were quantitatively and qualitatively analyzed using descriptive measures and hypothesis tests. Results In the quantitative analysis, latency values were lower in the BAEP for the DS group, with statistically significant difference for wave V and interpeaks III-V and I-V; there were no significant differences in the P300 latency values. In the qualitative analysis, there were a larger number of individuals with early values for BAEP latencies and late latencies for P300 in the DS group; both comparisons showed statistically significant differences. Conclusion Children and adolescents with DS can present early responses to the components of BAEP, suggesting that their auditory pathway requires less time for the neural transmission of acoustic stimuli to the brainstem. Concerning P300, individuals with DS may present increased latencies, suggesting impairment in the central auditory pathway for the cortical processing of auditory information.

RESUMO OBJETIVO: Analisar a literatura científica sobre alterações alimentares em crianças prematuras durante os primeiros anos de vida. FONTE DOS DADOS: A base de dados do Pubmed foi a precursora para a seleção dos artigos. Os textos foram analisados quanto aos seus objetivos, desenhos da pesquisa e características do grupo pesquisa e foram selecionados os seguintes temas para compor os critérios: (1) publicações de 1996 a 2014; (2) participação de bebês e crianças, do nascimento até os 10 anos; (3) desenvolvimento das habilidades motoras orais necessárias para a alimentação; (4) desenvolvimento do processo de alimentação e (5) dificuldades alimentares durante a infância. SÍNTESE DOS DADOS: Foram identificados 282 estudos, 17 foram usados e cinco artigos foram identificados por meio da lista de referência bibliográfica dos artigos selecionados, totalizando 22 referências bibliográficas. CONCLUSÃO: A revisão permitiu concluir que recém-nascidos pré-termo muito baixo peso (RNPTMBP) são mais propensos a apresentar problemas de alimentação nos estágios iniciais pós-natal e durante a infância quando comparados com crianças a termo. O monitoramento da alimentação após a alta hospitalar é estritamente recomendado em um programa de intervenção precoce com vistas ao melhor desenvolvimento das habilidades alimentares.

ABSTRACT OBJECTIVE: To analyze the scientific literature on dietary changes in preterm children during the first years of life. DATA SOURCE: The PubMed database was used for article selection. The texts were analyzed according to their objectives, research design, and research group characteristics. The following were selected to comprise the criteria: (1) publications in the period from 1996 to 2014; (2) participation of infants and children from birth to 10 years of age; (3) development of oral motor skills necessary for feeding; (4) development of the feeding process; and (5) feeding difficulties during childhood. SUMMARY OF THE FINDINGS: There were 282 studies identified, of which 17 were used in the review, and five more articles were identified through the reference list of selected articles, totaling 22 references. CONCLUSION: Very low birth weight preterm newborns are more likely to have feeding problems in early postnatal stages and during childhood when compared with full-term infants. Monitoring the feeding of these infants after hospital discharge is strictly recommended in an early intervention program aiming at better development of feeding skills.

Objetivo Identificar como a função da deglutição tem sido avaliada por meio da ultrassonografia (USG). Estratégia de pesquisa Esta revisão da literatura levantou publicações científicas internacionais sobre a USG e seu uso na avaliação da deglutição, por meio da base de dados PubMed. Foi realizada a localização e seleção dos estudos através de levantamento de textos publicados sobre o assunto, no período de janeiro de 2002 a agosto de 2013, limitando-se a estudos em seres humanos, no idioma inglês. Critérios de seleção Foram excluídos aqueles repetidos por sobreposição das palavras-chave, estudos de caso, revisões de literatura, cartas ao editor e os não relacionados diretamente à temática. Resultados Foram identificados 17 estudos que corresponderam aos critérios de inclusão. Observou-se que mais da metade dos estudos avaliou a deglutição de indivíduos adultos saudáveis, sem preferência por nenhum dos gêneros. Os parâmetros adotados para a análise das imagens não foram unânimes, havendo variação considerável entre os estudos. Conclusão A USG da deglutição demonstrou ser um método rápido, não invasivo, de baixo custo, que fornece parâmetros objetivos sobre a deglutição e que pode ser realizado em beira de leito, uma vez que o equipamento costuma ser de fácil manuseio e transporte.

Purpose To identify how the deglutition function has been evaluated using ultrasound (US). Research strategy This literature review used the PubMed database to survey international scientific publications about US and its use to evaluate deglutition. Studies were located and selected by surveying relevant articles published between January 2002 and August 2013. The survey was limited to studies on humans published in English. Selection criteria Repeated studies (determined by overlapping keywords), case studies, literature reviews, letters to the editor and studies not directly related to the topic were excluded. Results A total of 17 studies matching the inclusion criteria were identified. More than half of the studies evaluated the deglutition of healthy adults with no preference for gender. The parameters adopted for image analysis were not unanimous, and there was considerable variation among studies. Conclusion US proved to be a fast, non-invasive, low-cost method for evaluating objective parameters of deglutition. As a further advantage, US may be performed at bedside because the equipment is typically easy to handle and transport.

Objetivo Caracterizar o sistema auditivo periférico de indivíduos com síndrome de Down, por meio da audiometria convencional e de altas frequências. Métodos Estudo do tipo transversal e observacional. Participaram 15 indivíduos com síndrome de Down, de ambos os gêneros, entre 7 e 15 anos de idade. Foram realizados os seguintes procedimentos: Meatoscopia, Timpanometria com pesquisa do reflexo acústico ipsilateral e contralateral, Audiometria Tonal, Audiometria Vocal e Audiometria de Altas Frequências. Resultados Houve predomínio de perda auditiva condutiva de grau leve, em uma ou ambas as orelhas. As médias dos limiares auditivos para a audiometria convencional ficaram abaixo de 20 dBNA e, para a audiometria de altas frequências, ficaram entre 20 e 40 dBNA. O coeficiente de correlação de Pearson revelou correlação moderada positiva, entre os limiares de 9 a 14 kHz e a idade. Conclusão De forma geral, não foram observadas diferenças significativas, quando comparadas as orelhas direita e esquerda de indivíduos com síndrome de Down, na audiometria tonal, imitanciometria e logoaudiometria. A maioria das crianças apresentou alteração de orelha média e perda auditiva condutiva. A audiometria de altas frequências sugere o início de prejuízo da função coclear, que pode estar associado às otites médias frequentes e/ou à degeneração coclear precoce.

Purpose This study sought to characterize the peripheral auditory system of individuals with Down syndrome (DS) using conventional and high-frequency audiometry. Methods We performed a cross-sectional and observational study. Fifteen individuals with DS, who were of both genders and between 7 and 15 years of age, participated in this study. The following procedures were performed: otoscopy, tympanometry with ipsilateral and contralateral acoustic reflex, pure-tone audiometry, vocal audiometry and high-frequency audiometry. Results There was a predominance of mild conductive hearing loss in one or both ears. The mean hearing thresholds for conventional audiometry were below 20 dB HL and between 20 and 40 dB HL for high-frequency audiometry. The Pearson correlation coefficient indicated a moderate positive correlation between the 9-14 kHz thresholds and age. Conclusion Overall, no significant differences were observed when comparing the right and left ears of individuals with DS, in regards to pure-tone audiometry, immittance testing and speech audiometry. Most children showed middle ear abnormalities and conductive hearing loss. Moreover, high-frequency audiometry suggested the onset of impaired cochlear function, which may be associated with frequent otitis media episodes and/or early cochlear degeneration.

OBJETIVO: Descrever o desempenho linguístico de crianças com síndrome de Down falantes do Português Brasileiro por meio da análise da Extensão Média do Enunciado, comparar esse desempenho ao de crianças com Distúrbio Específico de Linguagem e com Desenvolvimento Típico e verificar se as crianças com síndrome de Down apresentam atrasos ou desvios do desenvolvimento linguístico. MÉTODO: Participaram do estudo 25 crianças com síndrome de Down (grupo pesquisa), pareadas pela idade mental ao grupo controle de crianças com desenvolvimento típico e ao grupo controle de crianças com Distúrbio Específico de Linguagem. Os participantes foram divididos em subgrupos de acordo com a faixa etária (três, quatro e cinco anos). Foram colhidas amostras de fala do grupo pesquisa e realizadas análises por meio da Extensão Média do Enunciado medida em morfemas e em palavras. RESULTADOS: Observaram-se diferenças de desempenhos entre o grupo pesquisa e os grupos controle, sendo que o primeiro apresentou valores de Extensão Média do Enunciado inferiores em todas as faixas etárias, o que caracteriza o atraso do desenvolvimento gramatical e linguístico geral. CONCLUSÃO: A descrição das habilidades linguísticas de crianças com síndrome de Down falantes do Português Brasileiro apontou para déficits gramaticais importantes, principalmente no que se refere ao uso de palavras funcionais.

PURPOSE: To describe the linguistic performance of Brazilian Portuguese-speaking children with Down syndrome by analyzing their Mean Length Utterance; to compare their performance to that of children with Specific Language Impairment and Typical Development; and to verify whether children with Down syndrome present developmental language delay or disorder. METHOD: Participants were 25 children with Down syndrome (Research Group), matched by mental age to a Control Group of typically developing children, and to a Control Group of children with Specific Language Impairment. Participants were divided into subgroups, according to age range (three, four and five years). Speech samples were collected for the Research Group, and the Mean Length Utterance was analyzed for morphemes and words. RESULTS: Differences were observed between the performance of the Research Group and both Control Groups, and the former presented inferior Mean Length Utterance values for all age ranges, characterizing a delay in grammar and general language development. CONCLUSION: The description of the linguistic abilities of Brazilian Portuguese-speaking children with Down syndrome indicated important grammatical deficits, especially regarding the use of functional words.

O objetivo da presente revisão de literatura foi analisar artigos científicos internacionais publicados sobre a fisiologia da deglutição de alimentos líquidos nas fases oral e faríngea. A metodologia empregada envolveu a formulação da pergunta; localização e seleção dos estudos; avaliação crítica dos artigos; conforme os preceitos do Cochrane Handbook. Foram identificados 185 artigos, dos quais se excluiu 141 por não relacionarem-se diretamente ao tema e analisou-se 29 estudos. As pesquisas estão fortemente relacionadas às formas de identificação de disfagia e não as características proporcionadas pela deglutição de diferentes consistências. Quanto à metodologia empregada nos artigos analisados observa-se que na maioria dos estudos não há grupo-controle. Os grupos estudados são heterogêneos, principalmente quando considerando indivíduos com alterações neurológicas, além disso, não há pareamento de idade na maioria dos estudos. Dessa forma, os achados desta revisão demonstram que há dificuldade na aplicabilidade clínica dos achados científicos, dificultando a prática baseada evidências.

The purpose of the present literature review was to analyze international scientific papers published on the physiology of swallowing fluids in the oral and pharyngeal phases. The employed methodology involved the formulation of the question, location and selection of studies, critical assessment of the manuscripts, according to the concepts of the Cochrane Handbook. We identified 185 articles, out of which 141 were excluded for not being directly related to the theme. Twenty-nine studies were analyzed. The researchers are strongly related to ways of identifying dysphagia and not the features offered by swallowing various consistencies. Regarding the methodology employed in the reviewed articles it is observed that in most studies there are no control groups. The studied groups are heterogeneous, especially when considering individuals with neurological disorders. In addition, subjects are not paired by age. Thus, the findings of this review indicate that there is great difficulty for clinicians to apply in their daily practice scientific findings, which in turn makes evidence-based practice very limited.

OBJETIVO: Levantar artigos científicos internacionais sobre a fisiologia da deglutição de alimentos nas consistências néctar, mel, pudim, pastosa heterogênea, semissólida e sólida, nas fases oral e faríngea. MÉTODOS: Trata-se de estudo de revisão qualitativa da literatura. Para a seleção dos artigos, foi utilizada a base de dados PubMed com emprego dos descritores "Swallowing and consistency", "Swallowing and solid" e "Swallowing and pasty", limitando-se a pesquisas publicadas no idioma inglês, entre os anos de 2005 e 2010, realizadas com seres humanos maiores de 18 anos. A metodologia empregada envolveu formulação da pergunta, localização e seleção dos estudos, e avaliação crítica dos artigos, conforme os preceitos do Cochrane Handbook. RESULTADOS: Foram identificados 211 estudos, dos quais 18 foram analisados, pois permitiam acesso ao texto completo e eram diretamente relacionados ao tema. CONCLUSÃO: Os estudos apresentam metodologia pouco uniforme, não havendo padronização, principalmente quanto aos métodos de avaliação. Em geral, as pesquisas foram realizadas com sujeitos saudáveis ou remeteram a um tipo de patologia, sem utilização de casos-controle. A heterogeneidade dos estudos possibilita que diferentes grupos de patologias sejam avaliados, porém, a variabilidade metodológica dificulta a definição e generalização dos padrões encontrados. Sendo assim, não é possível evidenciar dados que embasem a prática clínica fonoaudiológica no que diz respeito à fisiologia normal ou alterada da deglutição de diferentes consistências alimentares, tanto para sujeitos normais quanto para os acometidos por alguma desordem.

PURPOSE: To analyze published international scientific papers on the physiology of deglutition in oral and pharyngeal phases, considering different food consistencies: nectar, honey, pudding, pasty heterogeneous, semi-solid, and solid. METHODS: This is a qualitative literature reviews. The studies considered were selected on PubMed, using the keywords "Swallowing and consistency", "Swallowing and solid", and "Swallowing and pasty", limiting the search to manuscripts published in English in the period between 2005 and 2010, and conducted with human beings over 18 years old. The methodology involved question formulation, location and selection of studies, and critical analyses of the manuscripts, according to the concepts of the Cochrane Handbook. RESULTS: Two hundred and eleven studies were identified, out of which only 18 allowed access to the full text and were directly related to the theme. CONCLUSION: The studies presented very few similarities between the applied methodologies, especially when considering assessment methods. Overall, the studies were conducted with healthy individuals or with a specific pathology, without presenting the comparison with control cases. The heterogeneity of studies allows the investigation of different swallowing disorders. However, methodological variability makes it difficult to define and generalize the identified swallowing patterns. For this reason, it is not possible to identify parameters on which to base the clinical practice of speech-language therapists, especially when considering the normal or altered physiology of swallowing different food consistencies.

OBJETIVO: Analisar literatura científica sobre fisiologia e efeitos dos exercícios fonoaudiológicos utilizados nos tratamentos de alterações da Motricidade Orofacial. ESTRATÉGIA DE PESQUISA: Foram seguidos os preceitos do Cochrane Handbook que envolveu a formulação da questão a ser investigada, a localização e seleção dos estudos e a avaliação crítica dos artigos. CRITÉRIOS DE SELEÇÃO: Os artigos foram selecionados por meio da base de dados PubMed, utilizando-se os seguintes descritores: "physiology exercise AND speech, language and hearing science", "exercise physiology AND speech therapy", "exercise physiology AND myofunctional therapy", e "physiology exercise AND swallowing therapy". Foram incluídos somente artigos de língua inglesa publicados no período de 2000 a 2010. ANÁLISE DOS DADOS: Os textos foram analisados quanto aos seus objetivos; desenho da pesquisa; características do grupo pesquisa; existência de grupo controle; critérios de avaliação; terapêutica proposta; resultados encontrados; e verificação da existência de discussão sobre a fisiologia muscular dos exercícios. RESULTADOS: Foram identificados 108 estudos e destes, 38 foram analisados, pois permitiam acesso ao texto completo e relacionavam-se ao tema proposto. Os artigos foram classificados em: ensaios clínicos e pesquisas experimentais, estudos de casos, revisões de literatura e textos teóricos, cartas ao editor e críticas aos artigos. CONCLUSÃO: A revisão permite concluir que o conhecimento sobre os efeitos musculares dos exercícios empregados pelos clínicos é pouco aprofundado. Ainda não há evidência cientifica suficiente para determinar a frequência em que estes devem ser realizados. Em geral, tem-se pesquisado a eficácia de programas terapêuticos, sem investigar se os exercícios propostos são efetivos individualmente.

PURPOSE: To analyze the scientific literature about the physiology and effects of exercises used in the treatment of oral myofunctional disorders. RESEARCH STRATEGY: The methodology used followed the concepts of the Cochrane Handbook and involved question formulation related to the topic of investigation, identification and selection of the studies, and a critical evaluation of the selected articles. SELECTION CRITERIA: Papers were selected on PubMed database using the following keyword combinations: "physiology exercise AND speech, language and hearing science", "exercise physiology AND speech therapy", "exercise physiology AND myofunctional therapy", and "physiology exercise AND swallowing therapy". Only papers written in English and published between the years of 2000 and 2010 were included in the analysis. DATA ANALYSIS: Manuscripts were analyzed according to their objectives, research design, participants, inclusion of a control group, assessment criteria, therapeutic proposal, results and existence about physiology of the chosen exercises. RESULTS: One hundred and eight studies were identified, out of which 38 had access to the full text and were directly related to the investigated topic. The articles were classified as clinical trials and experimental research, case studies, literature reviews and theoretical articles, letters to the editor and critical analyses. CONCLUSION: This review concluded that there is a lack of knowledge about the effects of the myofunctional exercises used by clinicians. Also there is a lack of scientific evidence to determine the frequency at which they should be performed. Generally, the articles investigate the efficacy of treatment programs without inquiring whether the included exercises are individually effective.

OBJETIVO: Caracterizar o desenvolvimento das noções de classificação e seriação em crianças com síndrome de Down. MÉTODOS: A amostra foi constituída por 15 crianças, com idade cronológica entre cinco e 13 anos, localizadas no período pré-operatório do desenvolvimento cognitivo, divididas em três grupos igualitários, de acordo com sua idade mental, cujas médias foram de 3,4 anos para G1, de 4,3 anos para G2 e de 5,4 anos para G3, obtidas por meio da aplicação do Primary Test of Nonverbal Intelligence (PTONI) em sessão diferente das demais provas. As crianças foram submetidas a provas de classificação, com materiais não-figurativos e figurativos, e de seriação, com conjuntos de canecas e de bastonetes de tamanhos diferentes e escalonados. As crianças foram avaliadas individualmente em sessões diferentes para classificação e seriação, sem ordem previamente estabelecida. Todas as sessões foram filmadas e transcritas em protocolos específicos. RESULTADOS: Diferenças estatisticamente significantes foram observadas para a classificação figural entre G1 e G3, mais utilizada em G1 e não mais presente em G3, e para a classificação não-figural por características semânticas, com menor uso em G1 e maior em G3. Para a seriação, os resultados estatisticamente significantes foram obtidos entre G1 e G3, com relação à empírica e entre G3 e os outros grupos, para a intermediária. A seriação operativa somente apareceu em G3. CONCLUSÃO: As análises intra e intergrupos apontaram o caráter evolutivo e cumulativo das noções de classificação e seriação. Foi verificado que as crianças estudadas seguiram a ordem de aquisição como observada no desenvolvimento típico. Registro no Clinical Trials nº NCT00952354.

PURPOSE: To characterize the development of classification and seriation notions in children with Down syndrome. METHODS: The sample was composed of 15 children with chronological ages between five and 13 years old. Subjects were placed in the pre-operational period of cognitive development and arranged in three egalitarian groups, according to their mental age. Mean mental ages were 3.4 years for G1, 4.3 years for G2, and 5.4 years for G3, as measured by the Primary Test of Nonverbal Intelligence (PTONI), administered in a different session than the others tasks. Children were submitted to classification tasks using non-figurative and figurative materials, and seriation tasks using bowls and sticks of different sizes. Subjects were individually assessed in different sessions for classification and seriation tasks, with no previously established order. All sessions were registered in video and transcribed in specific protocols. RESULTS: Statistically significant differences between G1 and G3 were observed for figural classification, with a higher use by G1 and no use by G3, and for non-figural classification with semantic characteristics with a lower use by G1 and a higher use by G3. For seriation, statistically significant results were obtained for comparisons between G1 and G3 regarding the empirical kind, and for comparisons between G3 and the other groups regarding the intermediate kind. Operational seriation was observed only for G3. CONCLUSION: Within- and between-group analysis showed the progressive and cumulative character of classification and seriation notions. Moreover, the studied children followed the same acquisition order observed in typical development. Clinical Trials registration # NCT00952354.

TEMA: descrever os aspectos fonoaudiológicos de um caso de Síndrome de Crouzon, com idade de 6:4 anos, submetendo-o às avaliações das áreas de fala, linguagem, cognição, sistema estomatognático e audição. PROCEDIMENTOS: o sujeito foi submetido às seguintes avaliações: ABFW, Prova de Vocabulário Receptivo, Avaliação do Desenvolvimento de Linguagem-Cognição, Avaliação de Estruturas e Funções do Sistema Estomatognático, Audiometria Tonal Limiar, Imitanciometria e Logoaudiometria. RESULTADOS: a Audiometria Tonal identificou perda auditiva condutiva de grau moderado bilateralmente, apresentando compatibilidade com a Logoaudiometria e a Imitanciometria. A Avaliação de Sistema Estomatognático indicou tônus de estruturas diminuído, com postura e mobilidade alteradas. As funções de sucção, mastigação, deglutição e respiração mostraram-se alteradas. Na avaliação da Fonologia, identificaram-se simplificação do encontro consonantal, plosivação de fricativa e outros. Em Fluência, os dados estavam abaixo do esperado para a mesma faixa etária e sexo. Na prova de Pragmática o número de atos por minuto foi 14,4, apresentando como meio comunicativo predominante o gestual. Na prova de Vocabulário Receptivo, o valor total de designações usuais estava 7,1% abaixo do valor de referência. No Vocabulário Expressivo, os dados apontaram para referência entre 4 e 5 anos, abaixo dos parâmetros esperados para a idade do sujeito do estudo. Em Linguagem e Cognição, as análises indicaram defasagem em relação à fase de desenvolvimento. CONCLUSÃO: os comprometimentos causados pela síndrome são difusos e interligados, tendo a importância desse estudo apresentar os aspectos fonoaudiológicos de um caso de Síndrome de Crouzon e fornecer dados para aprofundar a investigação fonoaudiológica e o processo terapêutico.

BACKGROUND: to describe the results of speech, language, cognition, stomatognathic system and hearing assessments of a case of Crouzon syndrome at the age 6:4 years. PROCEDURE: the subject carried out the following evaluations: ABFW, Test of Receptive Vocabulary, Language-Cognition Development Evaluation, Evaluation of Structures and Functions of the Stomatognathic System, pure-tone audiometry threshold, immitance measures and vocal audiometry. RESULTS: the pure-tone audiometry identified bilateral moderate conductive hearing loss, compatible with vocal audiometry's and immitance measures' results. The stomatognathic system evaluation showed that the structures had reduced tonus and altered posture and mobility. Suction, chewing, deglutition and breathing functions were also altered. Phonologically, the following processes were identified: Cluster Simplification, Stopping of Fricatives and Others. In the Fluency evaluation, subject's performance was below the expected scores for matched age and gender. In the Pragmatics test, the child had 14.4 acts per minute and, predominantly, gestural communication. The Receptive Vocabulary Test showed scores 7.1% below reference. In the Expressive Vocabulary Test, data indicated a performance compatible to the reference values of 4 and 5 year-old children, below the expected scores for the subject's age. Regarding language and cognition, the analysis indicated a gap between the child's performance and the developmental level. CONCLUSION: the deficits caused by the syndrome are diffuse and interconnected. The present study had the aim to present the Speech-Language and Hearing Pathology associated aspects of a Crouzon syndrome case and to provide initial data to further investigate these aspects and the intervention process.

TEMA: a comunicação expressiva na criança com síndrome de Down (SD). OBJETIVO: este trabalho teve por objetivo o estudo qualitativo e quantitativo das diferentes formas de expressões comunicativas em crianças com SD; a emergência da sua expressão oral e sua relação com os gestos; a evolução dos gestos e a sua qualificação. Também se pesquisou a efetividade da terapia fonoaudiológica na criança com SD segundo o método dialético-didático, fundamentado no método clínico de Piaget. MÉTODO: participaram deste estudo oito crianças com SD (faixa etária entre trinta e três e cinqüenta e dois meses, no início da pesquisa), quatro constituindo o grupo pesquisa (GP) e quatro o grupo controle 1 (GC1); e quatro com desenvolvimento típico (DT) (faixa etárias entre quatorze e dezesseis meses, no início da pesquisa), ou grupo controle 2 (GC2). Todas apresentavam desenvolvimento cognitivo entre o final do período sensório-motor e início do pré-operatório, e foram avaliadas três vezes: inicial, após seis meses e após doze meses. As avaliações foram filmadas e transcritas. Os materiais utilizados foram brinquedos apropriados para a fase de desenvolvimento cognitivo apresentado pelas crianças. O processo terapêutico, apenas para o GP, constou de quarenta sessões terapêuticas, com materiais semelhantes aos das avaliações. RESULTADOS: verificou-se que GP teve melhor evolução que GC1. Os sujeitos que melhor conseguiram se expressar foram os que apresentaram melhor evolução no desenvolvimento cognitivo. CONCLUSÃO: foi possível confirmar a eficácia do método dialético-didático como processo terapêutico, demonstrada na evolução do desenvolvimento da linguagem do GP em relação ao GC1.

BACKGROUND: expressive communication in the child with Down syndrome (DS). AIM: this study had as a purpose the qualitative and quantitative analyses of the different forms of communication in children with DS; the emergence of oral expression and its relationship with the use of gestures; the development of gestures and their qualification; the effectiveness of the dialectic-didactic method, based on the clinical method proposed by Piaget, as a form of speech-language intervention. METHOD: participants of this study were eight children with DS (ages between 33 and 52 months at the beginning of the research) - four composing the research group (RG) and four composing the control group one (CG1); and four children with normal development ND (ages between 14 and 16 months at the beginning of the research) - control group number two (CG2). All children presented cognitive development classified between the final sensory motor stage and the beginning of the pre-operational stage, and were assessed three times during a period of 12 months: initial, after six months and after twelve months. All assessments were recorded and transcribed. Toys, appropriate to the cognitive stage of the children, were used as materials during the assessments. The therapeutic process, exclusively for the RG, consisted of 40 therapy sessions, using similar materials to those used at during the assessments. RESULTS: children in the RG developed better than children in CG1. Children who expressed themselves better were those who presented a better cognitive development. CONCLUSION: it was possible to confirm the effectiveness of the dialectic-didactic method as a therapy method, shown through the language development of the RG when compared to GC1.

OBJETIVO: Verificar a eficácia do trabalho fonoaudiológico na função da sucção em bebês com síndrome de Down por meio da eletromiografia de superfície. MÉTODOS: Em cada grupo (pesquisa e controle) participaram cinco bebês do gênero masculino e três do feminino, com idades entre seis e dez meses, diferenciando-se pelo diagnóstico da síndrome de Down. A pesquisa foi realizada em três fases: a) avaliação inicial (avaliação clínica do sistema estomatognático e de eletromiografia de superfície - provas de repouso habitual, sucção de mamadeira e retirada de papinha da colher); b) processo terapêutico; c) reavaliação. O grupo controle participou apenas da primeira e terceira fases. RESULTADOS: Os dados eletromiográficos mostraram diferença estatisticamente significante entre as provas de repouso habitual e mamadeira/papinha. No repouso habitual, recrutam-se poucas unidades motoras, em comparação aos demais movimentos realizados pelos músculos. CONCLUSÃO: A eletromiografia de superfície não permitiu verificar a diferença da contração do músculo orbicular da boca, antes e após a terapia, nem entre os grupos pesquisados, pois os movimentos não recrutaram unidades motoras suficientes para detectar as diferenças esperadas. Entretanto observou-se melhora dos aspectos observados através da avaliação fonoaudiológica clínica.

PURPOSE: To verify the efficacy of myofunctional intervention on the sucking function of infants with Down syndrome using the surface electromyography. METHODS: Five male and three female infants with ages from six to ten months were paired in a study and a control groups, which were differentiated only by the diagnosis of Down syndrome. The research was carried out in three phases: a) initial assessment (clinical evaluation of the stomatognathic system and surface electromyography - tasks of habitual rest, bottle sucking and taking puree from the spoon); b) therapeutic process; c) reassessment. The control group participated only in the first and third stages. RESULTS: The electromyographic data showed a significant difference between habitual rest and bottle/puree tasks. In the habitual rest, fewer motor units are recruited, in comparison to the other muscle movements. CONCLUSION: The surface electromyography didn't allow the identification of differences between the contractions of the mouth orbicular muscle before and after the intervention, neither between the studied groups, once the movements didn't recruit enough motor units to detect the expected differences. However, the clinical evaluation did show an improvement of the assessed aspects.

TEMA: a literatura aponta a postura corporal como um aspecto importante no tratamento de crianças com alterações sensório-motoras. No caso do paralítico cerebral, os reflexos apresentam-se mais intensos do que as reações de retificação e de equilíbrio, inibindo-as, provocando assim um atraso ou impedimento do controle cervical, de tronco e de quadril, que se reflete no Sistema Estomatognático; OBJETIVO: verificar a relação entre a postura corporal e a adequação do Sistema Estomatognático nessa população, quanto à postura e funcionalidade e sua efetividade no processo terapêutico fonoaudiológico; MÉTODO: foram realizadas avaliação inicial, intervenção fonoaudiológica e reavaliação em dezessete crianças com alterações sensório-motoras, com idades entre um ano e seis anos e três meses. A intervenção terapêutica foi realizada durante dez meses, com sessões semanais individuais, sempre com a presença do cuidador. Todas as sessões foram transcritas em protocolo específico e a avaliação e a reavaliação foram gravadas em videoteipe; RESULTADOS: observamos melhora estatisticamente significante dos aspectos do sistema estomatognático em 100% das crianças, tanto nas estruturas isoladamente, quanto em conjunto. O mesmo foi observado com relação às funções; CONCLUSÃO: a adequação da postura corporal das crianças estudadas favoreceu de forma significativa o desenvolvimento e adequação do sistema estomatognático quanto à postura e a funcionalidade.

BACKGROUND: literature points that body posture is an important aspect in the treatment of children with sensorimotor deficits. Considering individuals with cerebral palsy, reflexes are often more intense than reactions of rectification and equilibrium, causing, therefore, a delay or obstacle in cervical, torso and hip control. This delay has as a consequence an impact on the Stomatognathic System. AIM: to verify the relation between body posture and the Stomatognathic System in this population, regarding posture and function, and its effectiveness in the process of speech-language intervention. METHOD: 17 children with sensorimotor deficits, aged between 1 and 6:3 years, were submitted to an initial assessment, followed by speech-language intervention and re-assessment. Speech-language intervention occurred for a period of 10 months, with weekly individual sessions, always in the presence of the caretaker. All sessions were transcribed in a specific protocol and the assessment and re-assessment sessions were videotaped. RESULTS: a statistically significant improvement of stomatognathic system in 100% of the children was observed, not only of the isolated structures, but also of the whole system. The same was observed for the assessed functions. CONCLUSION: the improvement of body posture of the studied children favored significantly the development and improvement of the stomatognathic system regarding the aspects of posture and function.
