Abstract Background Strategies aimed at improving the diagnosis and treatment of cardiovascular diseases (CVD) are of utmost importance. Clinical decision support systems (CDSS) are guided by updated guidelines and may be capable of benefiting screening and early diagnosis initiatives in remote regions. Objective To develop a CDSS for screening hypertension and diabetes mellitus (DM), as well as to assess its feasibility and usability in the context of a primary care setting in a resource-constrained region. Methods This research focused on the strategy of screening hypertension and DM based on CDSS. A software was developed and implemented in 10 municipalities in the Northeast of the state of Minas Gerais, Brazil, from April 2017 to October 2018. Patients of 30 to 69 years of age were attended by primary care practitioners using the software. A Likert-scale questionnaire with 15 questions to assess usability (System Usability Scale) and satisfaction was applied to the professionals at the end of study. Groups were compared by applying the chi-squared test for categorical variables. Statistical significance was considered as p < 0.05. Results In the field study, 13,775 individuals were assessed; 185 patients were diagnosed with hypertension; 35 were diagnosed with DM, and 5 were diagnosed with both diseases. For the usability and satisfaction assessment, 258 healthcare professionals participated. Fifty-one percent considered their prior knowledge for the use of technologies to be good; 53.7% reported a desire to use the application frequently, and 78.4% would recommend the platform. Conclusion The implementation of a CDSS developed to assist in the screening of hypertension and DM was feasible in the context of a primary care setting in a resource-constrained region, with good user satisfaction. ABSTRACT OBJECTIVE To estimate seroprevalence of SARS-CoV-2 antibodies in schoolchildren aged 4 to 14 years living in the city of São Paulo, according to clinical, demographic, epidemiological, and social variables, during the school closure period as a measure against covid-19 spread. METHODS A serological survey was made in September 2020 with a random sample stratified by school system (municipal public, state public and private) type. A venous blood sample was collected using the Wondfo SARS-CoV-2 Antibody Test (lateral flow method) for detection of total SARS-CoV-2 virus antibodies. Semi-structured questionnaires were applied to collect clinical, demographic, social, and epidemiological data. RESULTS Seroprevalence of SARS-CoV-2 antibodies in schoolchildren was of 16.6% (95%CI 15.4–17.8). The study found higher seroprevalence in the municipal (18.5%; 95%CI 16.6–20.6) and state (16.2%; 95%CI 14.4–18.2) public school systems compared to the private school system (11.7; 95%CI 10.0–13.7), among black and brown students (18.4%; 95%CI 16.8–20.2) and in the most vulnerable social stratum (18.5 %;95%CI 16.9–20.2). Lower seroprevalence was identified in schoolchildren who reported following the recommended protective measures against covid-19. CONCLUSION Seroprevalence of SARS-CoV-2 antibodies is found mainly in the most socially vulnerable schoolchildren. This study can contribute to support public policies that reinforce the importance of suspending face-to-face classes and developing strategies aimed at protective measures and monitoring of the serological status of those who have not yet been included in the vaccination schedule. RESUMO OBJETIVO Estimar a soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares de quatro a 14 anos de idade residentes no município de São Paulo, segundo variáveis clínicas, demográficas, epidemiológicas e sociais, durante o período de fechamento das escolas como medida de controle da covid-19. MÉTODOS Realizou-se um inquérito sorológico em setembro de 2020 com amostra aleatória estratificada por tipo de rede de ensino (pública municipal, pública estadual e privada). Foi coletada amostra de sangue venoso utilizando-se o teste de imunoensaio de fluxo lateral da fabricante Wondfo para detecção de anticorpos totais contra o vírus SARS-CoV-2. Aplicaram-se questionários semiestruturados para o levantamento de dados clínicos, demográficos, sociais e epidemiológicos. RESULTADOS A soroprevalência de anticorpos do vírus SARS-CoV-2 em escolares foi de 16,6% (IC95% 15,4–17,8). O estudo encontrou soroprevalências mais elevadas na rede pública municipal (18,5%; IC95% 16,6–20,6) e estadual (16,2%; IC95% 14,4–18,2) em relação à rede privada (11,7; IC95% 10,0–13,7) e entre escolares da raça/cor preta e parda (18,4%; IC95% 16,8–20,2) e no estrato social mais vulnerável (18,5%; IC95% 16,9–20,2). A pesquisa identificou menores soroprevalências nos escolares que relataram seguir as medidas recomendadas de proteção contra a covid-19. CONCLUSÃO A soroprevalência de anticorpos contra o vírus SARS-CoV-2 atinge principalmente os escolares socialmente mais vulneráveis. Este estudo pode contribuir para embasar políticas públicas que reforcem a importância da suspensão das aulas presenciais e da necessidade de estratégias de medidas de proteção e acompanhamento do status sorológico daqueles que ainda não foram contemplados no calendário vacinal. Abstract Based on the follow-up of a young woman, we sought to understand how and what experiences contribute to the formation of more socially active young people - from a Gramscian perspective, organic intellectuals. The question is, therefore, which experiences have or have had an influence on the lives of young people that contribute or have contributed to constructing processes towards social emancipation? To this end, an individual territorial follow-up—a social technology developed by social occupational therapy—was used, resulting in descriptions and analyses of situations so that this young woman could engender movements of catharsis, highlighting her participation and protagonism in university extension actions, virtual environments, student guild events, and activities linked to a high school research program. Resumo A partir do acompanhamento de uma trajetória de vida juvenil, buscou-se apreender de que forma e quais experiências colaboram para constituir jovens mais ativos socialmente - em uma perspectiva gramsciniana: intelectuais orgânicos/as. Questiona-se, portanto: quais seriam as experiências que incidem/incidiram sobre as vidas juvenis que contribuem/contribuíram para a construção de processos na direção da emancipação social? Assim, utilizou-se do acompanhamento singular e territorial — tecnologia social descrita e elaborada pela terapia ocupacional social — resultando nas descrições e análises de situações para que uma jovem engendrasse movimentos de catarse, destacando sua participação em ações de extensão universitária, ambientes virtuais, grêmio estudantil e atividades vinculadas ao programa de iniciação científica do ensino médio (PIBIC-EM). In this study, alkaline treatment was used to change the surface of the new beta titanium alloy Ti25Ta25Nb3Sn and enhance its bioactivity. Ti25Ta25Nb3Sn, a new experimental alloy was processed in the laboratory and the surfaces were modified using alkaline treatment. Samples were soaked at 60 0C for 24 hours in different NaOH solutions to obtain surfaces with different morphologies. The surfaces were characterized by different techniques such as X-ray diffraction (XRD), scanning electron microscopy (SEM), and Atomic Force Microscopy (AFM). The wettability of the surfaces was measured using the sessile drop method. In vitro studies were carried out to evaluate cell adhesion, proliferation, and bacteria adhesion. As a result of the alkaline treatment, a nanoporous and amorphous layer was formed on the Ti25Ta25Nb3Sn alloy surface with super hydrophilic behavior for all conditions evaluated. However, different nano topography was observed which affected biological response. The morphology obtained after immersion in 1.5 NaOH solution led to bacteria adhesion decreased and osteoblasts cells differentiation. Results of this study show that is possible to obtain a new titanium experimental alloy with excellent bulk and surface properties that may decrease the risk of infections and increase osseointegration. RESUMO Objetivos: Identificar os principais desfechos hospitalares dos pacientes submetidos à correção cirúrgica de escoliose neuromuscular, avaliar as taxas de complicação e o cumprimento de metas de mobilidade depois do uso de um protocolo gerenciado. Métodos: Trata-se de um estudo longitudinal e retrospectivo, com dados obtidos seis meses depois da aplicação do protocolo em 103 pacientes de ambos os sexos submetidos à correção cirúrgica de escoliose neuromuscular, em um hospital de nível terciário em São Paulo, nos períodos de junho a dezembro de 2018 (pré-protocolo) e entre maio e setembro de 2019 (pós-protocolo). Foram excluídos os dados dos pacientes submetidos a outras cirurgias ortopédicas de coluna previamente. Além dos dados para caracterização epidemiológica das doenças de base, foram analisadas as características clínicas e complicações. Resultados: Dos 103 pacientes avaliados, 53,4% eram do sexo feminino, média de idade de 14,9 anos, o diagnóstico mais frequente paralisia cerebral, ângulo médio da curvatura de 75° e as comorbidades mais observadas foram doenças pulmonares (25%). O protocolo atingiu adesão parcial dos profissionais, e depois da implementação, observou-se diminuição significativa da dor e da síndrome da resposta inflamatória sistêmica (SIRS), prevenção da imobilidade e baixa taxa de infecção. Conclusões: O uso de um protocolo com foco em pacientes submetidos à correção de escoliose neuromuscular, reduziu as complicações por SIRS e dor, manteve a taxa de infecção de sítio cirúrgico baixa e preveniu a imobilidade a curto prazo. ABSTRACT Objectives: To identify the main hospital outcomes of patients undergoing surgical correction of neuromuscular scoliosis and to assess complication rates and achievement of mobility goals after the use of a managed protocol. Methods: This is a longitudinal, retrospective study, with data obtained six months after the application of a protocol in 103 patients of both sexes submitted to surgical correction of neuromuscular scoliosis, at a tertiary level hospital in São Paulo, between June and December 2018 (pre-protocol) and between May and September 2019 (post-protocol). Data from patients who had previously undergone other orthopedic spine surgeries were excluded. In addition to the data for epidemiological characterization of the underlying diseases, the clinical characteristics and complications were analyzed. Results: Of the 103 patients evaluated, there was a predominance of females (53.4%) and a mean age of 14.9 years. The most frequent diagnosis was cerebral palsy, the mean angle of curvature was 75°, and the most frequently observed comorbidities were lung diseases (25%). The protocol was partially adhered to by professionals and after its implementation, there was a significant decrease in pain and the systemic inflammatory response syndrome (SIRS), prevention of immobility and a low rate of infection. Conclusions: The use of a protocol focused on patients undergoing correction of neuromuscular scoliosis led to reduced complications of SIRS and reduced pain; kept the surgical site infection rate low, and prevented short-term immobility. RESUMEN Objetivos: Identificar a los principales resultados hospitalarios de los pacientes sometidos a corrección quirúrgico de escoliosis neuromuscular, evaluar las tasas de complicación y el cumplimiento de metas de movilidad tras la implementación de un protocolo gestionado. Métodos: Se trata de un estudio longitudinal y retrospectivo, con dados obtenidos después de 6 meses de la aplicación de un protocolo en 103 pacientes de ambos sexos, sometidos a corrección quirúrgica de escoliosis neuromuscular, en un hospital de nivel terciario en São Paulo, en los periodos de junio a diciembre de 2018 (pre-protocolo) y mayo a septiembre de 2019 (postprotocolo). Se excluyeron los datos de los pacientes que se habían sometido previamente a otras cirugías ortopédicas de la columna vertebral Además de los dados para la caracterización epidemiológica de las enfermedades de base, se analizaron las características clínicas y las complicaciones. Resultados: De los 103 pacientes evaluados, el 53,4% eran mujeres, con una media de edad de 14,9 años, el diagnóstico más frecuente fue parálisis cerebral, el ángulo de curvatura promedio de 75° y las comorbilidades más observadas fueron las enfermedades pulmonares (25%). El protocolo logró una adhesión parcial de los profesionales, y tras su implementación, se observó una disminución significativa del dolor y del síndrome de respuesta inflamatoria sistémica (SRIS), prevención de la inmovilidad y una baja tasa de infección. Conclusiones: El uso de un protocolo centrado en pacientes sometidos a corrección de escoliosis neuromuscular redujo las complicaciones por SIRS y dolor, mantuvo baja la tasa de infección del sitio quirúrgico y previno la inmovilidad a corto plazo. ABSTRACT OBJECTIVE To estimate the evolution of the prevalence of SARS-CoV-2 virus infection among residents aged 18 years or over in the municipality of São Paulo. METHODS This is a population-based household survey conducted every 15 days, between June and September 2020, and January and February 2021. In total, the study comprised 11 phases. The presence of antibodies against SARS-CoV-2 was identified in venous blood using a lateral flow test, Wondfo Biotech. In the last phase, the researchers combined it with an immunoenzymatic test, Euroimmun. The participants also answered a semi-structured questionnaire on sociodemographic and economic factors, and on social distancing measures. Prevalence estimates and the 95% confidence interval were estimated according to regions, Human Development Index, sex, age group, ethnicity, education, income, and variables associated with risk or prevention of infection. To compare the frequencies among the categories of each variable, the chi-square test with Rao-Scott correction was used, considering a significance level of 5%. RESULTS In total, 23,397 individuals were interviewed and had their samples collected. The estimated prevalence of antibodies against SARS-CoV-2 ranged from 9.7% (95%CI: 7.9–11.8%) to 25.0% (95%CI: 21.7–28.7). The prevalence of individuals with antibodies against the virus was higher among black and brown people, people with lower schooling and income, and among residents of regions with lower Human Development Index. The lowest prevalences were associated with recommended measures of disease protection. The proportion of asymptomatic infection was 45.1%. CONCLUSION The estimated prevalence of the infection was lower than the cumulative incidence variation, except for the last phase of the study. The differences in prevalence estimates observed among subpopulations showed social inequality as a risk of infection. The lower prevalence observed among those who could follow prevention measures reinforce the need to maintain social distancing measures as a way to prevent SARS-CoV-2 infection.

RESUMO: Otite externa é queixa frequente em cães. Bactérias e leveduras estão comumente envolvidas e podem perpetuar as reações inflamatórias dentro do canal auditivo. Dentre as formas de diagnóstico, encontram-se a otoscopia, o exame citológico da secreção e a cultura microbiológica. Citologia também tem grande utilidade no acesso à evolução do tratamento. A terapia consiste de limpeza dos canais auditivos e posterior utilização de antibióticos ou produtos antifúngicos. Como alguns antimicrobianos utilizados no tratamento podem causar hipersensibilidade e até mesmo ototoxicidade, a busca por novas bases farmacológicas justifica a existência deste estudo, que teve como objetivo avaliar in vitro e in vivo a eficiência do óleo de Melaleuca alternifolia em otites bacterianas e fúngicas de cães. Vinte e oito cães, de um abrigo particular, apresentando sinais clínicos de otite externa, foram incluídos neste estudo clínico. Todos passaram por avaliação clínica, citologia e cultura de material das orelhas afetadas. De cada animal, uma orelha foi tratada com óleo de Melaleuca 5% e a outra com formulação ótica padrão, de acordo com a afecção (bacteriana, fúngica ou mista). As culturas também foram submetidas a testes de susceptibilidade in vitro aos mesmos agentes utilizados no tratamento in vivo. Os resultados da cultura mostraram 62,5% de infecção mista (bacteriana e fúngica), 33,9% de infecção bacteriana e 3,6%, de infecção fúngica a partir das 56 orelhas. Os micro-organismos mais isolados foram Staphylococcus intermedius, Staphylococcus aureus, Proteus mirabilis e Malassezia pachydermatis. As bactérias GRAM-positivas foram sensíveis à gentamicina em 60,5% e resistentes em 16,3% das amostras. A formulação com 5% de óleo essencial de Melaleuca produziu uma zona de inibição de 5mm em torno dos discos em uma das 63 amostras avaliadas. A formulação pura (100%) do mesmo produto produziu uma zona de 10mm de inibição em quatro das 63 amostras analisadas, uma zona de 9 mm em três amostras, uma zona de 8mm em 16 amostras, uma zona de 7mm em sete amostras, uma zona de 6mm em duas amostras e não havia nenhuma zona clara em 31 amostras. Zonas de inibição foram produzidas por estirpes de Staphylococcus intermedius, Staphylococcus hyicus, Corynebacterium sp., Proteus mirabilis e Enterobacter sp. Clinicamente, a formulação com o óleo essencial de melaleuca induziu significativamente uma melhora nas manifestações, tanto nas otites bacterianas quanto nas causadas por leveduras, sendo estatisticamente semelhante ao tratamento com nistatina (nas otites por levedura), mas menos eficaz que a solução de gentamicina nas otites bacterianas. Mais estudos devem ser realizados para avaliar as propriedades de difusão in vitro do óleo essencial de melaleuca. O bom espectro antimicrobiano, a boa resposta clínica e a ausência de reações adversas confirmam a possibilidade de desenvolvimento de formulação ótica com o óleo essencial de melaleuca, como uma alternativa para a terapia de infecções do ouvido em cães.

ABSTRACT: Otitis externa is a common complaint in dogs. Bacteria and yeasts are commonly involved and may perpetuate inflammatory reactions inside the ear canal. Otoscopy, cytological examination of secretion and microbiological culture embody forms of diagnosis. Cytology also has great use in accessing treatment evolution. Therapy usually consists of cleaning ear canals and subsequent use of antibiotics or antifungal products. As some of them may cause hypersensitivity and even ototoxicity, searching for new pharmacological bases is currently necessary and justifies this study, which aimed to evaluate in vitro and in vivo efficacy of tea tree essential oil for bacterial and yeast ear infections in dogs. Twenty-eight dogs from a particular shelter in Cuiabá (Mato Grosso, Brazil), presenting clinical signs of otitis externa, were enrolled in this clinical trial. In all of them, clinical and cytological evaluations, as well as culture and susceptibility testing of the affected ears were carried out. From each dog, one ear was treated with 5% tea tree essential oil lotion and the other with standard otic formulation, according to the type of infection (bacterial, yeast or both). In vitro susceptibility testings of all ear cultures, to the same drugs used in treatment, were also carried out. Culture results showed 62.5% bacterial and fungal infection, 33.9% bacterial infection and 3.6% fungal infection, from the 56 ear samples collected. The most common microorganisms isolated were Staphylococcus intermedius, Staphylococcus aureus, Proteus mirabilis and Malassezia pachydermatis. Gram-positive bacteria were susceptible to gentamycin in 60.5% and resistant in 16.3% of the samples. Five percent tea tree essential oil formulation produced a 5mm clear zone of inhibition around the disks in one of the 63 samples evaluated. Pure (100%) tea tree essential oil formulation produced a 10mm clear zone of inhibition around the disks in four of the 63 samples evaluated, a 9mm zone in three samples, an 8mm zone in 16 samples, a 7mm zone in seven samples, a 6mm zone in two samples and there was no clear zone in 31 samples. Inhibition zones were produced by strains of Staphylococcus intermedius, Staphylococcus hyicus, Corynebacterium sp., Proteus mirabilis and Enterobacter sp. tea tree essential oil ear solution significantly induced remission of clinical signs both in bacterial and yeast ear infections. It also reduced as much Malassezia pachydermatis ear infection as the nystatin solution used in this study, while gentamycin solution showed better antibacterial effect. More studies should be conducted to evaluate in vitro diffusion properties of tea tree essential oil. Good antimicrobial spectrum and the absence of adverse reactions confirm the importance of developing a tea tree formulation as an alternative therapy for ear infections in dogs.

BACKGROUND Serum brain-derived neurotrophic factor (BDNF) levels have been shown to be lower in patients with Chagas cardiomyopathy (ChC) than in patients with non-dilated chagasic cardiomyopathy. However, its prognostic value was not established in patients with ChC. METHODS Forty-nine patients with ChC (50 ± 7 years, New York Heart Association “NYHA” I-III); were evaluated by echocardiography, exercise testing, and blood analysis. Serum BDNF levels were determined using enzyme-linked immunosorbent assay sandwich. Patients were followed-up, and cardiac death was considered the end-point. The survival analyses were performed using Kaplan-Meier and Cox regression. RESULTS After 39 ± 14 months of follow-up, 12 patients (25%) died. The concentration of 2.5 ng/mL was the optimal cut-off value to predict survival with significant difference between the groups with low (≤ 2.5 ng/mL) and high (> 2.5 ng/mL) BDNF levels (p = 0.006). Lower serum BDNF levels (hazards ratio (HR) 1.1, 95% confidence interval (CI) 1.1-1.4; p = 0.001), peak oxygen uptake (HR 1.2, 95% CI 1.0-1.3; p = 0.009), and left ventricular ejection fraction (HR 0.8, 95% CI 0.7-0.9; p = 0.001) were the independent predictors of survival. The combination of low serum BDNF levels and reduced left ventricular ejection fraction were highly predictive of death (HR 5.6, 95% CI: 1.2-9.7; p = 0.026). CONCLUSION In patients with ChC, reduced serum BDNF levels, especially if associated with systolic function, may provide useful prognostic information.

Resumo Introdução: O tumor sólido pseudopapilífero do pâncreas é uma neoplasia rara de baixo potencial de malignidade e com comportamento incerto, diagnosticado principalmente em mulheres jovens. Método: Uma série de casos dessa patologia é revisada retrospectivamente, focalizando os aspectos clínicos, tomográficos e imuno-histoquímicos, o tratamento realizado e o desfecho. Resultados: Foram avaliadas 13 pacientes com massas pancreáticas, por meio de tomografia computadorizada, com diâmetro médio de 8,8 cm. Todas as pacientes submeteram-se a ressecção cirúrgica completa. A imuno-histoquímica confirmou o diagnóstico em todos os casos. Conclusão: Esses tumores ocorrem frequentemente em mulheres jovens. Suspeita-se do diagnóstico ao encontrar uma volumosa tumoração sólido-cística no pâncreas. As imagens podem fornecer informações diagnósticas antes da ressecção. Podem-se empregar abordagens conservadoras em pacientes selecionados, e a sobrevida geralmente é excelente com a ressecção completa.

Summary Introduction: Solid pseudopapillary tumor of the pancreas (SPTP) is a rare neoplasm of low malignant potential with uncertain behavior, diagnosed mainly in young women. Method: Our report comprises a series of cases of SPTP reviewed retrospectively, highlighting clinical, tomographic and immunohistochemical features, treatment performed and outcomes. Results: Thirteen patients were found to have pancreatic [solid] masses on computed tomography scan measuring a mean diameter of 8.8 cm. All patients underwent complete surgical excision. Immunohistochemistry confirmed diagnosis in all cases. Conclusion: SPTP occurs more frequently in young women. Diagnostic suspicion lies on the finding of a bulky, solid and cystic pancreatic mass. Imaging findings might provide diagnostic information before resection. Conservative approaches can be used in selected cases and survival rates are usually excellent following complete resection.

Investigação analítico‐quali/quantitativa detectando o domínio de Organização do Conhecimento frente à produção brasileira de artigos de periódicos de Ciência da Informação a partir de levantamentos realizados na Base de Dados Referencial de Artigos de Periódico em Ciência da Informação. O objetivo geral da pesquisa foi analisar comparativamente a ocorrência numérica absoluta e relativa de artigos de periódicos brasileiros da área de CI sobre a temática Organização do Conhecimento cobertos na base BRAPCI de 1972 a 2012 através de recortes temático‐conceituais. Apresenta conceituações para a compreensão da Organização do Conhecimento, a partir da abordagem da Análise de Domínio de Hjørland e Albrechtsen (1995), a qual permitiu que fossem identificados os conceitos pertencentes da OC. Para atender aos objetivos de pesquisa, utilizaram‐se técnicas quantitativas de avaliação da produção científica como método de análise dos dados recuperados na BRAPCI e análise qualitativa dos resultados. Estas sinalizaram que o Domínio de OC está em expansão no Brasil ao contrário do cenário internacional.

Investigation qualitative and quantitative analysis detecting the domain of Knowledge Organization front of the whole area of information science from surveys conducted in Referential Database Journal Articles in Information Science. The objective of the research is to analyze the occurrence numerical absolute and relative articles of Brazilian journals of information science on the topic covered in the Knowledge Organization based BRAPCI 1972‐2012 through conceptual‐themed cutouts. Presents concepts for understanding the Organization of Knowledge, from the approach of Domain Analysis of Hjørland and Albrechtsen (1995), which allowed them to be identified concepts of belonging OC. To meet the research objectives, we used quantitative techniques for assessing the scientific literature as a method of analyzing the data retrieved in BRAPCI and qualitative analysis. This signaled that the domain OC is expanding in Brazil unlike the international scene.

Esta investigación analiza el estatuto epistemológico actual del derecho a la información, diferenciándolo del campo disciplinar que se dedica a su estudio. Considera la inserción de este derecho en una matriz de análisis en la que se cruzan: su objeto de estudio, las generaciones de los derechos en las que los autores lo clasifican; su carácter conexo o subsidiario para la efectuación de otros derechos, en parte explicado por su elaboración en el siglo XIX, cuando fue enunciado junto al derecho de expresión en contrapunto con su autonomía adquirida en el siglo XX; su carácter o clasificación como derecho individual, difuso, colectivo o social; su limitación por la necesidad de respeto a la vida privada; su negación o contrario por la mentira y por el secreto. Como conclusión presenta un cuadro resumen en el que se consolida la síntesis de las propuestas de los diversos autores analizados acerca de este derecho.

This article analyzes the current epistemological status of the right to information, distinguishing it from earlier rights to information expression and access. It considers the inclusion of this right in an array of historical and social analyses: its object of study; the generations of the rights that come out it; its relationship to other rights (partly explained by its development in the 19th century together with the right of expression, but later gaining autonomy from the right to expression during the 20th century); its character as an individual right (and conversely, as a collective or social right); its character in respect to private life; and its relationship to denial, lying, and secrecy. In conclusion, the article presents a chart that summarizes the arguments of the paper and some of the authors discussed.

INTRODUCTION : Exercise-induced ventricular arrhythmia (EIVA) and autonomic imbalance are considered as early markers of heart disease in Chagas disease (ChD) patients. The objective of the present study was to verify the differences in the occurrence of EIVA and autonomic maneuver indexes between healthy individuals and ChD patients with no apparent cardiac involvement. METHODS : A total of 75 ChD patients with no apparent cardiac involvement, aged 44.7 (8.5) years, and 38 healthy individuals, aged 44.0 (9.2) years, were evaluated using echocardiography, symptom-limited treadmill exercise testing and autonomic function tests. RESULTS : The occurrence of EIVA was higher in the chagasic group (48%) than in the control group (23.7%) during both the effort and the recovery phases. Frequent ventricular contractions occurred only in the patient group. Additionally, the respiratory sinus arrhythmia index was significantly lower in the chagasic individuals compared with the control group. CONCLUSIONS : ChD patients with no apparent cardiac involvement had a higher frequency of EIVA as well as more vagal dysfunction by respiratory sinus arrhythmia. These results suggest that even when asymptomatic, ChD patients possess important arrhythmogenic substrates and subclinical disease.
