Resumo: Os Grupos Funcionais de Alimentação (GFA) são utilizados para determinar a dinâmica da comunidade de macroinvertebrados bentônicos e como uma ferramenta de biomonitoramento. Os principais objetivos deste estudo foram: avaliar os efeitos de variáveis de riacho na abundância e riqueza de GFAs e os atributos do ecossistema (razão GFA) como uma ferramenta para avaliar as condições ecológicas dos córregos da Mata Atlântica. Foram amostrados 146 locais com diferentes condições de impacto no Rio de Janeiro, Brasil. A riqueza foi significativamente diferente com as condições de impacto entre todos os GFA. Os modelos de efeito misto mostraram que os GFA diferiam em suas respostas às variáveis abióticas quanto à abundância e riqueza. Além disso, eles diferem nas áreas impactadas quando comparados as áreas intermediária e de referência. A razão de GFA encontrou diferenças significativas ao longo do gradiente de impacto. A análise da razão de GFA evidenciou-se uma ferramenta rápida e barata, com potencial para ser utilizada no monitoramento de ecossistemas aquáticos no bioma Mata Atlântica. No entanto, mais estudos serão necessários para calibrar o método especificamente para a região da Mata Atlântica.

Abstract: Benthic macroinvertebrates Functional Feeding Group (FFG) have been used to determine aquatic assemblage dynamics and as a biomonitoring tool. The main goals of this study were to assess the effects of stream variables on the abundance and richness of FFGs and evaluate ecosystem attributes (FFG ratios) as a tool to assess ecological conditions of Atlantic Rainforest streams. We sampled 146 sites with different impairment conditions in Rio de Janeiro, Brazil. Richness was significantly different among impairment conditions for all FFGs. Mixed-effect models show that aquatic macroinvertebrate FFGs differed in their responses to abiotic variables for abundance and richness. Also, they were reduced in the impaired sites when compared to intermediate and reference sites. The FFG ratio indicated significant differences along the impairment gradient. The FFG ratio analysis was shown to be a fast and cheap tool that can be used for monitoring aquatic ecosystems in the Atlantic Forest biome. However, further studies are required to calibrate the method specifically for the Atlantic Forest region.

Abstract Background: There is a lack of information on the role of chronic use of hydroxychloroquine during the SARS-CoV-2 outbreak. Our aim was to compare the occurrence of COVID-19 between rheumatic disease patients on hydroxychloroquine with individuals from the same household not taking the drug during the first 8 weeks of community viral transmission in Brazil. Methods: This baseline cross-sectional analysis is part of a 24-week observational multi-center study involving 22 Brazilian academic outpatient centers. All information regarding COVID-19 symptoms, epidemiological, clinical, and demographic data were recorded on a specific web-based platform using telephone calls from physicians and medical students. COVID-19 was defined according to the Brazilian Ministry of Health (BMH) criteria. Mann-Whitney, Chi-square and Exact Fisher tests were used for statistical analysis and two binary Final Logistic Regression Model by Wald test were developed using a backward-stepwise method for the presence of COVID-19. Results: From March 29th to May 17st, 2020, a total of 10,443 participants were enrolled, including 5166 (53.9%) rheumatic disease patients, of whom 82.5% had systemic erythematosus lupus, 7.8% rheumatoid arthritis, 3.7% Sjögren’s syndrome and 0.8% systemic sclerosis. In total, 1822 (19.1%) participants reported flu symptoms within the 30 days prior to enrollment, of which 3.1% fulfilled the BMH criteria, but with no significant difference between rheumatic disease patients (4.03%) and controls (3.25%). After adjustments for multiple confounders, the main risk factor significantly associated with a COVID-19 diagnosis was lung disease (OR 1.63; 95% CI 1.03-2.58); and for rheumatic disease patients were diagnosis of systemic sclerosis (OR 2.8; 95% CI 1.19-6.63) and glucocorticoids above 10 mg/ day (OR 2.05; 95% CI 1.31-3.19). In addition, a recent influenza vaccination had a protective effect (OR 0.674; 95% CI 0.46-0.98). Conclusion: Patients with rheumatic disease on hydroxychloroquine presented a similar occurrence of COVID-19 to household cohabitants, suggesting a lack of any protective role against SARS-CoV-2 infection. ABSTRACT Background: Clinical improvements following highly active antiretroviral therapy (HAART) may increase high-risk behaviors resulting in sexually transmitted infections (STI). Optimism related to the success of HAART in slowing disease progression, reducing viral load, and improving health status might be important factors for increasing sexual risk behaviors such as less use of condoms. Objective: To determine the prevalence of Chlamydia trachomatis, Neisseria gonorrhoeae, syphilis, hepatitis B and C, high-risk HPV, and cervical cytological abnormalities among women living with HIV (WLHIV) who attended a Reference Center for STI/AIDS in Brazil. Methods: A cross-sectional study was conducted among 151 WLHIV attending an STI Clinic in Vitória city, Brazil. A structured questionnaire, including demographic, behavioral, and clinical information, was used for data collection. Serological tests for HIV, syphilis, hepatitis C and B, CD4 counts, and viral load determination were performed. Cervical samples were collected for cytology and real-time PCR for HPV, Chlamydia, and Neisseria gonorrhoeae. Results: In this study, 59% of women had at least one diagnosed STI at the time of the first clinic visit; 31% had clinical forms of anogenital HPV, 10% syphilis, 8%Neisseria gonorrhoeae, 5.0% trichomoniasis, 3% Chlamydia trachomatis, 1% hepatitis B, and 1% hepatitis C; 6.7% of the women presented with cervical cytological abnormalities. Furthermore, 46.3% of women had HR-HPV, and 17.6% had HPV 16/18. Only 5% of the women had a CD4 count <200 cells/mm3, 61.6% had undetectable HIV viral load, and 81.3% were currently on HAART. Conclusion: A high prevalence of STI and HR-HPV infections were observed among HIV-infected women in this investigation. Prevention programs need to focus on counseling WLHIV and their regular partners with focused interventions such as couples counseling and education programs.

ABSTRACT There are few data on the impact of COVID-19 in pregnancy, however, analyzing these data is important to guide the clinical practice, covering the early prevention, detection, patients’ isolation, epidemiological investigation, diagnosis and early treatment. This is a report of three cases of COVID-19 confirmed by real-time reverse transcription – polymerase chain reaction (RT-PCR) of nasopharyngeal secretions collected in swabs from pregnant women in the city of Vitoria, Espirito Santo State, Brazil. In the three cases, all the patients presented with fever, one had shortness of breath, one had diarrhea, two of them reported abdominal pain and two of them had cough. The three patients progressed with a severe clinical evolution of COVID-19. The permanence in the intensive care unit (ICU) was more than 10 days. Two of them recovered and one remained in the ICU with irreversible refractory shock, multiple organ failure and died. The mode of delivery was individualized and based on the obstetric indication and severity of the maternal infection, and the cesarean section was indicated in the two severe maternal COVID-19 cases that evolved favorably. These newborns were premature and tested negative for COVID-19 by RT-PCR.

Resumo Desde os tempos coloniais os quintais estão presentes como elemento tradicional nas casas urbanas amazônicas e brasileiras. Entretanto, atualmente, grande parte das novas moradias construídas, nas grandes cidades da região e do país, prescinde dos espaços contíguos às residências chamados de quintais. O propósito do estudo é analisar, historicamente, a importância e as funções dos quintais nas moradias brasileiras, e amazônicas em particular. O trabalho está dividido em três seções. Na primeira, discute os conceitos de quintais urbanos e de casa; na segunda analisa as transformações nas tipologias de moradias, destacando as permanências e as mudanças na importância e nas funções dos quintais. Por fim, conclui mostrando que, com o crescimento urbano e o desenvolvimento do capitalismo, a emergência de padrões modernos e pós-modernos de morar vem provocando o crescente esvaziamento das funções tradicionais de produção alimentar, de prática de atividades domésticas e de local privilegiado de lazer e convivência familiar. Como consequência, os quintais urbanos tem visto seu conceito e suas funções alterados ou, simplesmente, vêm sendo suprimidos das moradias. Por fim, alerta para as consequências desse processo para a qualidade de vida urbana, especialmente nas cidades amazônicas.

Abstract Ever since colonial times, backyards have been present as a traditional element in urban houses in the Amazon and in Brazil in general. However, currently, most new homes built in large cities in this region and throughout Brazil forego the spaces contiguous to homes called backyards. The purpose of this study is to conduct a historical analysis of the importance and functions of backyards in Brazilian homes, and in the Amazon in particular. The article is divided into three sections. The first discusses the concepts of urban backyards and houses; the second analyzes the transformations in housing typology, highlighting the continuities and the changes in the importance and functions of backyards. Finally, it concludes by showing that, with urban growth and the development of capitalism, the emergence of modern and post-modern patterns of living has increasingly been prompting the ebbing away of a backyard’s traditional functions of producing food, being a favored locus in which to engage on domestic activities, forms of leisure and of family life. As a result, backyards have seen their concept and functions or have simply disappeared. Consequently, an alert sounds to the consequences of this process for the quality of urban life, especially in Amazonian towns.

Abstract This essay analyzes of the construction of characters in three contemporary Brazilian novels written by Ana Luisa Escorel (Anel de vidro), Mariana Portella (O outro lado da sombra) and Elvira Vigna (O que deu para fazer em matéria de história de amor). These novels are examined from the perspective of the crisis of affects in globalized society.

Introdução: A infecção materna pelo HIV e comorbidades associadas podem ter duas consequências para a saúde fetal, a transmissão vertical e o desfecho perinatal adverso. Após o sucesso em reduzir a transmissão vertical, deve-se dar atenção ao risco potencial de nascimento pretermo (PRT) e de restrição de crescimento fetal (RCF). Objetivo: Determinar a prevalência de PRT e RCF em gestantes de baixa renda, infectadas pelo HIV, usuárias de terapia antirretroviral atendidas em hospital público terciário e verificar sua relação com o estágio da infecção viral. Casuística e métodos: Dentre os 250 partos de gestantes infectadas pelo HIV, ocorridos em um hospital universitário na cidade de Vitória, estado do Espírito Santo, Sudeste do Brasil, entre novembro de 2001 e maio de 2012, foram selecionadas 74 gestações não-gemelares, com idade gestacional confirmada por ultrassonografia e as dimensões neonatais: peso ao nascer (PN), comprimento (CN) e perímetros cefálico (PC) e abdominal (PA). Os dados foram extraídos dos prontuários clínicos e laboratoriais e o desfecho sumarizado como nascimento pretermo (PRT < 37 semanas), baixo peso ao nascer (BPN < 2500g) e como pequeno (PIG), adequado (AIG) e grande (GIG) para a IG, definido como tendo um menor valor, entre e maior que ± 1.28 z/IG escore, o critério clínico usual para demarcar os percentis 10 e 90. Resultados: PRT foi observado em 17,5%, BPN em 20,2% e PN, CN, PC e PA PIG em 16,2%, 19,1%, 13,8% e 17,4%, respectivamente. As respectivas proporções observadas nos casos de HIV e AIDS foram: PRT: 5,9 versus 27,5%, BPN: 14,7% versus 25,0%, PFN PIG: 17,6% versus 15,0%, CN: 6,0% versus 30,0%, PC: 9,0% versus 17,9% e PA: 13,3% versus 21,2%; somente a diferença de CN PIG foi estatisticamente significativa. Dentre 15 neonatos com BPN, oito (53,3%) eram somente PRT, quatro (26,7%) PIG somente e três (20,0%) PRT e PIG. Concomitância no mesmo caso de pelo menos duas dimensões PIG foi observada frequentemente. Conclusão: A proporção de baixo peso ao nascer foi maior que a prevalência local e brasileira e foi observada uma tendência para maior proporção de dimensões fetais PIG que a distribuição populacional esperada nesta pequena casuística de filhos de gestantes infectadas pelo HIV, usuárias de antirretrovirais, de baixa renda e assistidas em hospital público terciário. Observou-se também tendência para maior prevalência de PTR, BPN e dimensões fetais PIG em recém-nascidos de mães com AIDS comparados com os de mães infectadas por HIV sem AIDS.

Introduction: Maternal HIV infection and related co-morbidities may have two outstanding consequences to fetal health: mother-to-child transmission (MTCT) and adverse perinatal outcomes. After Brazilian success in reducing MTCT, the attention must now be diverted to the potentially increased risk for preterm birth (PTB) and intrauterine fetal growth restriction (IUGR). Objective: To determine the prevalence of PTB and IUGR in low income, antiretroviral users, publicly assisted, HIV-infected women and to verify its relation to the HIV infection stage. Patients and Methods: Out of 250 deliveries from HIV-infected mothers that delivered at a tertiary public university hospital in the city of Vitória, state of Espírito Santo, Southeastern Brazil, from November 2001 to May 2012, 74 single pregnancies were selected for study, with ultrasound validated gestational age (GA) and data on birth dimensions: fetal weight (FW), birth length (BL), head and abdominal circumferences (HC, AC). The data were extracted from clinical and pathological records, and the outcomes summarized as proportions of preterm birth (PTB, < 37 weeks), low birth weight (LBW, < 2500g) and small (SGA), adequate (AGA) and large (LGA) for GA, defined as having a value below, between or beyond the ±1.28 z/GA score, the usual clinical cut-off to demarcate the 10th and 90th percentiles. Results: PTB was observed in 17.5%, LBW in 20.2% and SGA FW, BL, HC and AC in 16.2%, 19.1%, 13.8%, and 17.4% respectively. The proportions in HIV-only and AIDS cases were: PTB: 5.9 versus 27.5%, LBW: 14.7% versus 25.0%, SGA BW: 17.6% versus 15.0%, BL: 6.0% versus 30.0%, HC: 9.0% versus 17.9%, and AC: 13.3% versus 21.2%; only SGA BL attained a significant difference. Out of 15 cases of LBW, eight (53.3%) were preterm only, four (26.7%) were SGA only, and three (20.0%) were both PTB and SGA cases. A concomitant presence of, at least, two SGA dimensions in the same fetus was frequent. Conclusions: The proportions of preterm birth and low birth weight were higher than the local and Brazilian prevalence and a trend was observed for higher proportions of SGA fetal dimensions than the expected population distribution in this small casuistry of newborn from the HIV-infected, low income, antiretroviral users, and publicly assisted pregnant women. A trend for higher prevalence of PTB, LBW and SGA fetal dimensions was also observed in infants born to mothers with AIDS compared to HIV-infected mothers without AIDS.

Os vírus do herpes simplex tipos 1 e 2 são os principais agentes infecciosos associados às ulcerações orais e vaginais. Estas infecções são amplamente reconhecidas como doenças sexualmente transmissíveis. Entre as opções de tratamento, o laser de baixa intensidade (LBI) mostrou resultados promissores como terapia de supressão de longa duração. Descrevemos dois casos clínicos com herpes labial recorrentes nos quais o LBI foi utilizado que permaneceram assintomáticos durante 17 meses de controle clínico.

Herpes simplex virus types 1 and 2 are the main infectious agents associated with oral and genital ulcerations. These infections are now widely recognized as sexually transmitted diseases. Among treatment options, low-level laser therapy (LLLT) has shown promising clinical results as a longer-lasting suppression therapy. Two clinical cases are described with recurrent labial herpes for which LLLT was used. Following treatment, both patients remained symptom free during the 17-month clinical follow-up period.

Atualmente, com o advento da terapia antirretroviral potente e o aumento da sobrevida dos pacientes pediátricos infectados pelo HIV, a busca da promoção de uma melhor qualidade de vida deve ser o foco principal na atenção a crianças vivendo com HIV/Aids. Torna-se necessária a utilização de um instrumento adequado a este grupo visando à investigação e à avaliação da sua qualidade de vida. Este estudo objetivou realizar validação da Escala de Qualidade de Vida da Criança em crianças infectadas pelo HIV atendidas em Serviços de Sida/Aids do Rio de Janeiro (RJ). Esta escala é composta de 26 perguntas e sua aplicação ocorreu em 100 crianças entre quatro e 12 anos e seus responsáveis. A análise estatística realizada utilizou correlações canônicas, intervalo de confiança e teste c². O ponto de corte obtido foi 49; a consistência interna expressa por um alfa de Cronbach da ordem de 0,73 para as crianças e 0,67 para os responsáveis. O perfil de resposta demonstrou satisfação elevada para itens como férias e aniversário, e os de menor satisfação foram internação e brincar sozinho. Conclui-se que a escala apresentou medidas psicométricas satisfatórias, tornando-se um instrumento confiável, consistente, válido e recomendável de mensuração da qualidade de vida de crianças infectadas pelo HIV.

With the advent of potent antiretroviral therapy and the increase in life expectancy of pediatric patients infected with HIV, the quest for the promotion of enhanced quality of life should currently be the main focus in care of children with HIV/Aids. The scope of this study was to validate the Scale of Children's Quality of Life in a group of children infected with HIV receiving clinical care in Aids Service Units in Rio de Janeiro, Brazil. This scale consists of 26 questions and was tested on 100 children, with ages varying between 4 and 12, and their respective parents or guardians. Statistical analysis was conducted using canonical correlation and confidence interval analysis and the c² test. The results showed that the cut-off point obtained was 49; the internal consistency with Cronbach's alpha was 0.73 for the children and 0.67 for parents or guardians. The response profile revealed marked satisfaction with aspects such as vacations and birthdays, though less satisfaction with items including hospitalization and playing alone. The conclusion was that the scale revealed satisfactory psychometric measurements, proving to be a reliable, consistent, valid and recommended instrument for measuring the quality of life of children infected with HIV.

A hanseníase apresenta acometimento cutâneo e neurológico característicos; entretanto, as manifestações reumáticas são relativamente comuns sendo, em alguns pacientes, a queixa inicial. O presente relato de caso descreve uma paciente do sexo feminino, com hanseníase borderline, cuja manifestação inicial foi poliartrite simétrica, lesões cutâneas ulceradas em membros inferiores e manifestações sistêmicas simulando doença reumática. Os autores enfatizam a importância do diagnóstico diferencial do comprometimento sistêmico, articular e cutâneo na hanseníase com as doenças reumáticas.

Leprosy is a systemic disease with predominant involvement of skin and nerves; articular manifestations are common and, in some cases, represent the initial complaint. We describe the case of a female patient with borderline leprosy, which manifested, initially, with symmetric polyarthritis, cutaneous ulcerative lesions on the lower limbs, and systemic manifestations mimicking rheumatic disease. The authors emphasize the importance of the differential diagnosis of the systemic, joint and, cutaneous involvement of leprosy with rheumatic diseases.
