Abstract Context Aging causes a progressive worsening in postural balance, affecting functional independence and increasing the risk of falls. Objective The aim of the study was to evaluate the effect of aging on the static balance in women from 50-years to 89-years of age. Design This was a cross-sectional study, with 400 irregularly active women were evaluated and grouped by age: Group 6thdecade (age 50 to 59) ‒ 58 participants; Group 7thdecade (age 60 to 69) ‒ 214 participants; Group 8thdecade (age 70 to 79) ‒ 92 participants; Group 9thdecade (age 80 to 89) ‒ 36 participants. Postural balance was evaluated using a portable force platform in a standard standing position, with Eyes Open (EO) and Eyes Closed (EC). Results In the two measurement conditions, the elderly women in Group 9thdecade presented mediolateral displacement and range, and mean velocity greater than the women's values in Groups 6thand 7thdecade. In the EO e EC situation, the displacement was higher in the elderly Group 9thdecade compared to younger groups. Group 8thhas a mean velocity greater than Group 6thdecade in the EO situation. Conclusions Posturography showed a decline in postural balance with advancing age, suggesting that the 9thdecade of life is a borderline age to this detriment due to an increase in postural instability.

RESUMO Introdução: A instabilidade postural afeta o controle postural desde os estágios iniciais da doença de Parkinson (DP). A literatura descreve benefícios do programa de fortalecimento muscular no equilíbrio, na capacidade funcional, porém poucos estudos investigaram seus efeitos no controle postural na DP. Objetivo: Investigar os efeitos de um programa de três meses de fortalecimento nas medidas da posturografia estática (PE) e nos testes clínicos na DP. Métodos: Participaram do ensaio aleatório controlado 74 pacientes, designados em um dos seguintes grupos: fortalecimento utilizando aparelhos de musculação (gmusc); fortalecimento com pesos livres e elásticos (gpeso); e grupo controle. As avaliações ocorreram antes da intervenção, 3 e 6 meses após intervenção. Foram avaliados alterações nas medidas da PE nas condições de olhos abertos, olhos fechados e dupla tarefa (desfecho primário), efeitos sobre sintomas motores, equilíbrio por meio de escalas clínicas, posturografia dinâmica e percepção da qualidade de vida (desfechos secundários). Resultados: Não houve interações significativas nas medidas da PE entre os grupos. As pontuações motoras da Escala Unificada de Avaliação da Doença de Parkinson (UPDRS-III) diminuíram em ambos os grupos de fortalecimento (p<0,05). Houve melhora da percepção da qualidade de vida para o domínio mobilidade no gmusc, assim como ganhos nas pontuações dos testes funcionais no gpeso, mantido no seguimento após seis meses (p<0,05). Conclusões: O programa de fortalecimento não altera o controle postural medida pela PE. Apesar disso, ambos os grupos de fortalecimento apresentaram melhor desempenho motor, efeitos positivos a médio prazo no equilíbrio no gpeso e melhor desempenho na qualidade de vida no gmusc.

ABSTRACT Background: Postural instability affects Parkinson’s disease (PD) patients’ postural control right from the early stages of the disease. The benefits of resistance training (RT) for balance and functional capacity have been described in the literature, but few studies have been conducted showing its effects on PD patients’ postural control. Objective: To investigate the effects of a three-month RT intervention on static posturography (SP) measurements and clinical functional balance assessment among PD patients. Methods: Seventy-four patients were randomly assigned to a three-month RT intervention consisting of using weightlifting machines at a gym (gym group) or RT consisting of using free weights and elastic bands (freew group), or to a control group. The participants were evaluated at baseline, three months and six months. We evaluated changes of SP measurements under eyes-open, eyes-closed and dual-task conditions (primary endpoint), along with motor performance and balance effects by means of clinical scales, dynamic posturography and perceptions of quality of life (secondary endpoints). Results: There were no significant interactions in SP measurements among the groups. Unified Parkinson Disease Rating Scale (UPDRS-III) motor scores decreased in both RT groups (p<0.05). Better perceived quality of life for the mobility domain was reported in the gym group while functional balance scores improved in the freew group, which were maintained at the six-month follow-up (p<0.05). Conclusions: This study was not able to detect changes in SP measurements following a three-month RT intervention. Both RT groups of PD patients showed improved motor performance, with positive balance effects in the freew group and better perceived quality of life in the gym group.

RESUMO Introdução: A marcha humana é um movimento complexo dependente de controle neural multinível, que permite um padrão periódico uniforme, regular e complexo, que a caracterizam como um sistema não linear. O déficit sensitivo e motor com diminuição das respostas proprioceptivas pode diminuir a capacidade de adaptação do sistema, como já demonstrado nas doenças de Parkinson, Alzheimer e Huntington. Contudo, pouco se conhece sobre o efeito da neuropatia diabética periférica nessas respostas. Objetivos: Analisar a influência da neuropatia diabética periférica na entropia em diferentes ambientes de marcha. Métodos: Dez idosos, sem e com diagnóstico de neuropatia diabética periférica, caminharam em esteira rolante (velocidade inicial 3 km/h e incremento de 0,5 km/h a cada 5 minutos até a velocidade 5 km/h) para o registro da aceleração do centro de massa nos componentes vertical, médio-lateral, e anteroposterior ao longo de todo teste. A entropia amostral dos três vetores foi calculada para cada velocidade de teste. Resultados: O componente vertical não apresentou nenhuma diferença com significância estatística. O componente médio-lateral mostrou diferenças com significância estatística para os fatores grupo, velocidade e interação entre os fatores (grupo e velocidade). O componente anteroposterior apresentou diferenças com significância estatística para o fator grupo, mas não para o fator velocidade e interação entre os fatores (grupo e velocidade). Em todas as comparações, foram encontrados tamanhos de efeito classificados como grandes. Conclusões: A neuropatia diabética periférica produziu alterações na capacidade de adaptação sobre as variações do ambiente durante a marcha, provavelmente, em decorrência de alterações da complexidade do sistema de controle neural multinível, que depende da retroalimentação sensitiva e motora, sabidamente afetadas pela neuropatia diabética periférica. ABSTRACT Introduction: Human gait is a complex movement dependent on multilevel neural control, which allows a consistent, regular and complex periodic pattern, properties that characterize it as a nonlinear system. Sensory and motor deficits, with diminished proprioceptive responses, may reduce the adaptive capacity of the system, as demonstrated in Parkinson's, Alzheimer's and Huntington's diseases. However, little is known about the effect of peripheral diabetic neuropathy on these responses. Objectives: To analyze the influence of peripheral diabetic neuropathy on entropy in different gait environments. Methods: Ten elderly patients, with and without a diagnosis of peripheral diabetic neuropathy, walked on a treadmill (initial speed of 3 km/h, with 0.5 km/h increments every 5 minutes up to the speed of 5 km/h) to record center of mass acceleration in the vertical, mediolateral and anteroposterior components throughout the test. The sample entropy of the three vectors was calculated for each test speed. Results: The vertical component did not show any statistically significant differences. The mediolateral component showed statistically significant difference for the factors group, speed, and interaction between factors (group and speed). The anteroposterior component showed statistically significant differences for the group factor, but not for speed and interaction between factors (group and speed). Effect sizes classified as large were found in all the comparisons. Conclusions: Peripheral diabetic neuropathy produced changes in the ability to adapt to changes in the environment during gait, probably due to changes in the complexity of the multilevel neural control system, which depends on motor and sensory feedback, known to be affected by peripheral diabetic neuropathy. No obstante, poco se conoce sobre el efecto de la neuropatía diabética periférica en esas respuestas. Objetivos: Analizar la influencia de la neuropatía diabética periférica en la entropía en diferentes ambientes de marcha. Métodos: Diez ancianos, sin y con diagnóstico de neuropatía diabética periférica, caminaron en cinta rodante (velocidad inicial 3 km/h e incremento de 0,5 km/h a cada 5 minutos hasta la velocidad de 5 km/h) para el registro de la aceleración del centro de masa en los componentes vertical, medio-lateral, e anteroposterior a lo largo de todo el test. La entropía de muestreo de los tres vectores fue calculada para cada velocidad de test. Resultados: El componente vertical no presentó ninguna diferencia con significancia estadística. El componente medio-lateral mostró diferencias con significancia estadística para los factores grupo, velocidad e interacción entre los factores (grupo y velocidad). El componente anteroposterior presentó diferencias con significancia estadística para el factor grupo, pero no para el factor velocidad e interacción entre los factores (grupo y velocidad). En todas las comparaciones, se encontraron tamaños de efecto clasificados como grandes. Conclusiones: La neuropatía diabética periférica produjo alteraciones en la capacidad de adaptación sobre las variaciones del ambiente durante la marcha, probablemente como consecuencia de alteraciones de la complejidad del sistema de control neural multinivel, que depende de la retroalimentación sensitiva y motora, sabidamente afectadas por la neuropatía diabética periférica. RESUMO Objetivo: Acompanhar a densidade mineral óssea (DMO) e a composição corporal, ao longo de seis anos, em idosos corredores de longa distância. Métodos: analisamos os prontuários médicos de um grupo de atletas idosos, corredores de longa distância, participantes do Grupo de Ortogeriatria do IOT-HC-FMUSP, e reunimos todos os atletas que tiveram a DMO avaliada no ano de 2001 e de 2007, sendo destes, 11 prontuários selecionados. Critérios de inclusão: ser corredor de longa distância; não parar de correr ao longo dos seis anos e ter os dois exames de DMO e composição corporal avaliados. A composição corporal foi avaliada por meio de densitometria óssea, com uma dupla energia de absorção de raios-X (DEXA), em um aparelho LUNAR-DPX. Resultados: Ao longo dos seis anos, a composição corpórea se manteve estável, havendo apenas um aumento significante na gordura expressa em (%) (p=0,003). Conclusão: A corrida de longa distância parece conservar a DMO de idosos corredores, porém com aumento de gordura. ABSTRACT Objective: To evaluate bone mineral density (BMD) and body composition over a six-year period in elderly long-distance runners. Methods: We analyzed the medical records of elderly athletes who were long-distance runners, were participants of the IOT-HCFMUSP Orthogeriatric Group, and had their BMD evaluated between 2001 and 2007; of these athletes, 11 were included in the study. Inclusion criteria: athletes should be long-distance runners, should not stop long-distance running during the six-year period, and should have undergone BMD and body composition evaluations. Body composition was evaluated using bone densitometry with dual-energy X-ray absorptiometry with a Lunar-DPX device. Results: Over the six-year period, body composition remained stable, but there was a significant increase only in the fat percentage (p = 0.003). Conclusion: Long-distance running may maintain BMD but may lead to an increase in the fat percentage in elderly runners. OBJECTIVE: To validate an instrument to determine the graduate students' knowledge level of health care on the basic support of life procedures, risk factors, damage and disorders in elderly patients with hip fractures. METHOD: A group of experts was asked to analyze the instrument in two steps. Firstly, the procedure was done subjectively and according to objective analysis using the Likert scale proposed by the Delphi method. After adjustment according to the suggestions, the version of the instrument was applied to 179 undergraduate students in the health area. RESULT: The instrument has achieved in its entirety and in parts (risk factors), the minimum criteria established for the Cronbach's alpha (i.e., ≥0.70). There was no change in the Cronbach's alpha (0.551) for the maintenance of initial items of the instrument, as well as the deletion of seven assessment items. CONCLUSION: The instrument developed has sufficient internal validity to determine the level of knowledge of undergraduate students in the health area on basic life support, damage, injuries and risk factors in elderly patients with hip fractures from falls. RESUMO OBJETIVO: O objetivo deste estudo foi determinar se a radiação eletromagnética de ondas curtas em modo pulsado com uma frequência de 45 Hz, promove mudanças teratogênicos, natimortos, mudanças no peso do órgão ao nascimento em fetos de ratas grávidas expostas a essa radiação. METODO: Foram estudados dez ratas grávidas da raça Marcole, cinco num grupo teste, submetidas a radiação eletromagnética com uma potência média de 4,5W, durante 15 minutos, diariamente, durante todo o período da gravideze cinco num grupo controle, não expostas a radiação. No 21º dia, os animais foram sacrificados para análise dos fetos. Para avaliar o efeito da radiação térmica, a temperatura abdominal foi medida antes e depois da aplicação, em conjunto com a temperatura ambiente prevalente. Os fetos foram submetidos ao reflexo de Galant para avaliara existênciade natimortos. Os fetos foram removidos e pesados; através de uma laparotomia mediana seus corações, estômagos, rins e fígados foram extirpados e pesados, frescose secos. RESULTADOS: Vinte e nove fetos foram colhidos no grupo controle e 59 no grupo de teste, sem casos de natimortos, teratogênese ou malformação dos órgãos internos. A análise não encontrou diferenças no peso ao nascer e peso de órgãos desidratados quando comparados com o grupo controle. CONCLUSÃO: Os resultados sugerem que a exposição a terapia de ondas curtas, sem o acúmulo significativo de calor, não levou a alterações teratogênicos e não afetou o peso dos fetos ou o peso dos órgãos internos desidratados.

BACKGROUND: The aim of this study was to determine whether shortwave electromagnetic radiation in pulsed mode with a frequency of 45 Hz, promotes teratogenic changes, stillbirths and changes in organ weight at birth in the fetuses of pregnant rats exposed to this radiation. METHOD: Ten black Macole pregnant female rats were studied, 5 in a test group, subjected to electromagnetic radiation with an average power of 4.5 W for 15 minutes, on a daily basis, during the entire period of pregnancy and 5 in a control group that was not exposed to radiation. At day 21, the animals were euthanized for analysis of fetuses. To assess the thermal effect of the radiation the abdominal temperature was measured before and after application, together with prevailing ambient temperature. The fetuses were evaluated through the Galant reflex to assess the existence of stillbirths. The fetuses were removed and weighed; through a midline laparotomy their hearts, stomachs, kidneys and livers were excised and weighed, wet and dry. Twenty-nine fetuses were harvested in the control group and 59 in the test group, with no cases of stillbirth, teratogenesis or malformation of internal organs. RESULTS: The analysis found no differences in birthweight and weight of dehydrated organs when compared to the control group. CONCLUSION: The results in our findings suggest that exposure to shortwave therapy without significant heat buildup, does not lead to teratogenic changes and did not affect the mass and weight of dehydrated internal organs.

CONTEXTO E OBJETIVO: A manutenção do equilíbrio postural exige a detecção dos movimentos do corpo, a integração das informações sensoriais no sistema nervoso central e uma resposta motora apropriada. O objetivo deste estudo foi avaliar se a dominância dos membros inferiores influencia o equilíbrio postural. TIPO DE ESTUDO E LOCAL: Este é um estudo transversal realizado na Faculdade de Medicina da Universidade de São Paulo (FMUSP) e no Hospital do Coração (HCor). MÉTODOS: Foram avaliados 40 indivíduos sedentários e saudáveis, do sexo masculino de 20 a 40 anos, sem lesões. O teste do equilíbrio unipodal foi realizado no equipamento Biodex Balance System, comparando membro dominante com não dominante do mesmo indivíduo. Os protocolos de instabilidade utilizados foram: nível 8 (mais estável) e nível 2 (menos estável) e três índices de instabilidade foram calculados: anteroposterior, medial/lateral e geral. RESULTADOS: A idade média foi de 26 ± 5 anos (20-38), massa corporal 72.3 ± 11 kg (46-107) e estatura 176 ± 6 cm (169-186). 34 voluntários (85%) tinham o membro direito como dominante (determinado pelo membro que chuta) e seis (15%), o membro esquerdo. Não houve diferenças significantes entre os membros dominantes e não dominantes nos dois níveis de estabilidade (oito e dois) em nenhum índice de instabilidade (geral, ântero-posterior e medial/lateral). CONCLUSÃO: A dominância dos membros inferiores não influencia o equilíbrio unipodal em indivíduos sedentários do sexo masculino.

CONTEXT AND OBJECTIVE: Maintainance of postural balance requires detection of body movements, integration of sensory information in the central nervous system and an appropriate motor response. The purpose of this study was to evaluate whether lower-limb dominance has an influence on postural balance. DESIGN AND SETTING: This was a cross-sectional study conducted at Faculdade de Medicina da Universidade de São Paulo (FMUSP) and at Hospital do Coração (HCor). METHODS: Forty healthy sedentary males aged 20 to 40 years, without any injuries, were evaluated. A single-foot balance test was carried out using the Biodex Balance System equipment, comparing the dominant leg with the nondominant leg of the same individual. The instability protocols used were level 8 (more stable) and level 2 (less stable), and three instability indices were calculated: anteroposterior, mediolateral and general. RESULTS: The volunteers' mean age was 26 ± 5 years (range: 20-38), mean weight 72.3 ± 11 kg (range: 46-107) and mean height 176 ± 6 cm (range: 169-186). Thirty-four of them (85%) presented right-leg dominance (defined according to which leg they used for kicking) and six (15%) had left-leg dominance. There were no significant differences between the dominant and nondominant legs at the two levels of stability (eight and two), for any of the instability indices (general, anteroposterior and mediolateral). CONCLUSION: The lower-limb dominance did not influence single-foot balance among sedentary males.
