Resumo O artigo 206.º da Lei do Orçamento de Estado de 2022 prevê a contratação de médicos sem especialidade para o exercício de funções destinadas a médicos especialistas em Medicina Geral e Familiar (MGF), sendo previsível um impacto nefasto desta medida no sistema de saúde português e na saúde da população portuguesa. Os médicos de família criam uma aliança terapêutica na relação médico-doente e cuidam da saúde dos seus utentes de uma forma abrangente ao longo de toda a vida. Com a presente Lei, o governo desvaloriza o papel e a formação especializada do médico de família, descredibilizando a especialidade. Acreditamos que nesta Lei figura mais um retrocesso no caminho da especialidade de Medicina Geral e Familiar, praticada por profissionais altamente diferenciados. Pela lente dos médicos de família, ao invés do referido no artigo 206.º, a aposta do governo deverá ser na contratação de médicos de família em número suficiente para os centros de saúde, na redução drástica da burocracia a que estão sujeitos, na simplificação dos indicadores de desempenho orientados para verdadeiros benefícios em saúde dos utentes e na contratação de outros profissionais para as equipas, também em número suficiente para responder às necessidades.

Abstract Article 206 of the 2022 State Budget Law provides for hiring doctors without a specialty to perform functions intended for specialized doctors in General and Family Medicine, with a harmful impact of this measure being foreseeable on the Portuguese health system and the health of the Portuguese population. Family doctors create a therapeutic alliance in the doctor-patient relationship and take care of the health of their users in a comprehensive way throughout their lives. With this law, the government devalues the family doctor’s role and specialized training, discrediting the specialty. We believe that this law represents yet another setback in the path of the specialty of General and Family Medicine, practiced by highly differentiated professionals. Through the lens of family doctors, instead of what is referred to in Article 206, the government’s focus should be on hiring enough family doctors for health centers, drastically reducing the bureaucracy to which they are subject, simplifying indicators performance-oriented towards true benefits in the health of users and hiring other professionals for the teams in sufficient numbers to respond to needs.

ABSTRACT The canonical position of the adjectives in Romance languages is post-nominal. However, it is observed a triparted distribution: although the majority of the romance adjectives is exclusively post-nominal, a very few adjectives can be only pre-nominal, and a relatively numerous group of adjectives can come after and also before the nucleus, with a change in meaning. Despite those distributional patterns, for the cartography approach there are only two syntactic sources for adjectives: a reduced relative (always post-nominal, leading to extensional interpretation) and functional projections, which include all pre-nominal adjectives and some post-nominal adjectives, responsible for the intensional interpretation. We propose that those adjectives that are licensed in Italian both before and after the nominal head are in a functional projection, DegP, and are implicit comparatives. RESUMO Em línguas românicas, a posição canônica do adjetivo é a pós-nominal. Abstract This article proposes an analysis of the creation process of the National Museum of the Republic in Brasília, a public museum managed by the Department of Culture and Creative Economy of the Federal District, and aspects of the formation of its collection. Abstract The objective of the present study was to investigate the frequency of oral leukoplakia and oral erythroplakia among young patients from three Brazilian reference centers in Oral and Maxillofacial Pathology. A retrospective study was carried out from 2011 to 2021 on 861 patients diagnosed with oral leukoplakia and oral erythroplakia. Demographic and clinicopathological data were evaluated. Fisher’s exact test was used to evaluate the association among sex, age, anatomical location, and histopathological diagnosis. A total of 83 (9.64%) cases involved young patients (aged <40 years). Among these, biopsy records were included in 31 (37.34%) cases, all of which received a clinical diagnosis of oral leukoplakia. Seventeen (54.84%) patients were female, mostly in their fourth decade of life (n = 22/70.97%), and their mean age at diagnosis was 32.61(± 5.21) years. Among informed cases, seven (22.58%) patients were smokers. The lateral border of the tongue (n = 9/29.03%) was the most affected site. In 13 (41.94%) cases, oral leukoplakias showed a homogeneous appearance. The mean size of the lesions was 1.47 cm (0.2–3.0 cm) and the mean time of disease progression was 64.37 (± 65.90) months. The histopathological analysis showed that 11 cases (35.48%) exhibited some degree of epithelial dysplasia. Acanthosis and/or hyperkeratosis were observed in 20 cases (64.52%). No significant associations were observed between sex and anatomical location, age and anatomical location, nor between sex and histological diagnosis (p > 0.05). Abstract Background Stress is the feeling of discomfort or oppression caused by an individual's reaction to stressors, which can be internal or external, varying according to the person's perception and coping ability. Objective To evaluate the prevalence and associated factors related to the intensity of stressful events in college students. Methods A cross-sectional study with 1,842 new college students of Pelotas, RS. The prevalence of stressful events was evaluated using a score. Exposure variables were sex, gender identity, sexual orientation, age, skin color, marital status, maternal schooling, socioeconomic class, occupation, place of origin, cohabitation, place of residence, tobacco, alcohol, and drug use. Results The overall prevalence of stressful events in the sample was high (95.3%). The most prevalent stressful events were feeling very anxious about graduation overload (83,8%) and pressure to perform well (65,0%). The associated factors were: (a) women: age 18 to 22, be homosexual/bisexual, and be from other cities of the state or other state/country; (b) men: being homosexual/bisexual, coming from another state/country, and alcohol consumption at least two to three times a week. Abstract There is an interaction between dendrocytes and mast cells in the skin. However, in elastosis-related diseases such as actinic cheilitis (AC) and lower lip squamous cell carcinoma (LLSCC), this interaction remains unknown. We investigated the presence of intact and degranulated mast cells in AC and LLSCC. Associations of mast cells with factor XIIIa+ dendrocytes and inflammatory infiltrate were assessed. Forty cases of AC (20 with low-grade and 20 with high-grade epithelial dysplasia), 50 cases of LLSCC, and 10 cases of normal oral mucosa were evaluated. Toluidine blue staining was performed to identify mast cells, and mast cell densities were calculated in the inflammatory infiltrate. Factor XIIIa+ dendrocytes were immunohistochemically quantified. The highest ratio of intact/degranulated mast cells density was detected in LLSCC (5.9 cells/mm2), followed by AC with high-grade epithelial dysplasia (4.8 cells/mm2). Statistically significant differences were found in the density of intact mast cells compared to degranulated mast cells in AC with low-grade epithelial dysplasia (p<0.001), AC with high-grade epithelial dysplasia (p=0.005), and LLSCC (p<0.001). A positive correlation between degranulated mast cells and total inflammatory infiltrate (p=0.03) was observed in the LLSCC group. The expression of factor XIIIa+ dendrocytes was highest in AC with low-grade epithelial dysplasia (16.5 cells/mm2). ABSTRACT Introduction: Vision is one of the senses that most integrates the individual with the environment, since it captures most of the sensory stimuli to which the human being is exposed, and it is precisely this integration that allows the creation of memories and the formation of the knowledge. Low vision in childhood impairs learning, socialization, and also determines visual quality in adulthood. Objective: to identify the visual acuity of school children using the Snellen Test and refer and carry out actions with those with low vision. Methods: Cross-sectional study with a quantitative approach. The Snellen Table was used to determine the visual acuity of students in the 1st year of elementary school in the state education network. Results: Participants are aged 5-7 years, 40 females and 38 males. Of these, 23 had low visual acuity and were referred to an ophthalmological consultation. Conclusion: Based on the results of the present study, the application of the Snellen Test is suggested, as it is an easy-to-train, low-cost screening test, so that it become useful in promoting eye health. RESUMO Introdução: A visão é um dos sentidos que mais integra o indivíduo ao meio ambiente, pois capta a maior parte dos estímulos sensoriais aos quais o ser humano está exposto, e é justamente essa integração que permite a criação de memórias e a formação do conhecimento. A baixa visão na infância prejudica o aprendizado, a socialização e também a qualidade visual na idade adulta. Objetivo: Identificar a acuidade visual de crianças escolares, utilizando a tabela de Snellen e encaminhar e realizar intervenções necessárias naquelas com baixa visão. Métodos: Estudo transversal, com abordagem quantitativa. Utilizou-se a tabela de Snellen para determinar a acuidade visual de alunos do 1º ano do ensino fundamental da rede estadual de ensino. Resultados: Os participantes tem idade de 5 a 7 anos, sendo 40 do sexo feminino e 38 do sexo masculino. Destes, 23 apresentavam baixa acuidade visual e foram encaminhados para consulta oftalmológica. Conclusão: Com base nos resultados do presente estudo, sugere-se que a aplicação do teste de Snellen, por ser um teste de triagem de fácil treinamento e baixo custo, torne-se útil na promoção de saúde ocular. Abstract: In recent times, there has been an environmental pressure to reduce the amount of pesticides applied to crops and, consequently, the crop production costs. Therefore, investments have been made in technologies that could potentially reduce the usage of herbicides on weeds. Among such technologies, Machine Learning approaches are rising in number of applications and potential impact. Therefore, this article aims to identify the main machine learning algorithms used in integrated crop-livestock systems for weed management. Based on a systematic literature review, it was possible to determine where the selected studies were performed and which crop types were mostly used. The main research terms in this study were: "machine learning algorithms" + "weed management" + "integrated crop-livestock system". Although no results were found for the three terms altogether, the combinations involving "weed management" + "integrated crop-livestock system" and "machine learning algorithms" + "weed management" returned a significant number of studies which were subjected to a second layer of refinement by applying an eligibility criteria. The achieved results show that most of the studies were from the United States and from nations in Asia. Machine vision and deep learning were the most used machine learning models, representing 28% and 19% of all cases, respectively. Abstract Background: Promoting physical contact, especially skin-to-skin contact, immediately or as soon as possible in preterm newborns is still a challenge. Objective: To determine the prevalence of physical contact between mothers and preterm newborns at birth and to analyze the association between the type of physical contact between them in the delivery room and the prevalence of exclusive breastfeeding at hospital discharge. Methodology: Cross-sectional study with a nonprobability sample of 78 mothers and 79 preterm newborns. Data were collected from July to November 2017, through interviews and medical records, and analyzed through descriptive statistics and Poisson Regression with robust variance. Results: The prevalence of any type of physical contact in the delivery room was 51.9%, without statistical difference between the type of physical contact between mothers and infants and exclusive breastfeeding at discharge. Conclusion: Physical contact by touching and kissing after birth did not influence the prevalence of exclusive breastfeeding at hospital discharge.

Objetivo: Comprobar la prevalencia del contacto físico entre la madre y el recién nacido prematuro en el momento del nacimiento y asociar el tipo de contacto físico realizado por el binomio en la sala de partos con la prevalencia de la lactancia materna exclusiva en el momento del alta hospitalaria. Metodología: Estudio transversal con una muestra no probabilística de 78 madres y 79 recién nacidos prematuros. La recogida de datos se realizó de julio a noviembre de 2017, mediante entrevista y consulta en antecedentes clínicos, y los datos se analizaron según la estadística descriptiva y la regresión de Poisson con varianza robusta. Resultados: La prevalencia de cualquier tipo de contacto físico realizado en la sala de partos fue del 51,9%, sin diferencias estadísticas entre el tipo de contacto físico realizado entre la madre y el bebé, y la lactancia materna exclusiva en el momento del alta. Conclusión: El contacto físico mediante caricias y besos durante el parto no influyó en la prevalencia de la lactancia materna exclusiva en el momento del alta hospitalaria.

ABSTRACT Introduction: Peritoneal dialysis patients are at increased risk of developing abdominal wall complications, including hernias. This study aimed to determine the incidence of hernias in peritoneal dialysis patients, identify risk factors, and evaluate their impact on technique survival. Methods: A retrospective cohort study was conducted, including prevalent peritoneal dialysis patients in a single center, from January 2010 to October 2020. Data on baseline characteristics, previous abdominal surgeries, and hernia history were collected. Patients with diagnosed hernias during peritoneal dialysis were compared to a control group. Results: Among 155 peritoneal dialysis patients, hernias occurred in 29% (n=45) of the population, with an incidence rate of 0.09 hernias/patient/year. Umbilical hernias were the most common subtype. Autosomal dominant polycystic kidney disease (OR (odds ratio) 3.925; 95% confidence interval (CI), 1.208 - 12.75) history of surgical repaired hernias (OR 5.586; 95% CI 1.540 - 20.255) and a higher number of previous abdominal surgeries (OR 1.680; 95% CI 1.025 - 2.752) were independent predictors of hernia development. Surgical treatment was performed in 59.3% of cases, with a 18.8% recurrence rate. Higher daytime infusion volumes were associated with hernia recurrence (p=0.045). Peritoneal dialysis technique survival did not differ significantly between the hernia and control groups (p=0.700). Conclusion: Hernias are frequent in peritoneal dialysis patients, with autosomal dominant polycystic kidney disease, previous abdominal surgeries, and history of surgically repaired hernias as identified risk factors. Hernias do not significantly interfere with peritoneal dialysis technique survival, but comprehensive management strategies, including early surgical repair and peritoneal dialysis prescription adaptations, can optimize outcomes.

The Xingu River crosses several indigenous territories along its course, among which is one of the most extensive and populous in Brazil. In order to admit and forward the demands of the Mebêngôkre regarding their linguistic rights, the Extension Project Actions to Combat Linguistic Prejudice (UFRJ), in partnership with the municipality's Department of Indigenous School Education, created the action "Me kuni umari — in network for the rights language”. That is the experience reported here.

O Rio Xingu atravessa diversos territórios indígenas ao longo de seu curso, dentre os quais, está um dos mais extensos e populosos no Brasil. Ali vive o povo Mebêngôkre (Kayapó), que fala a língua de mesmo nome, da família Jê, tronco Macro-Jê. Suas reservas ocupam áreas que abragem os estados do Mato Grosso e Pará. Entre elas está a TI Kayapó com mais de 30 escolas na região, mas os Mebêngôkre sofrem as pressões típicas de línguas minorizadas. Em 2019, a língua foi cooficializada em São Félix do Xingu (PA), mas esse fato ainda não modificou a contento a realidade linguística local. Para recolher e encaminhar as demandas dos Mebêngôkre quanto a seus direitos linguísticos, o Projeto de Extensão Ações de Combate ao Preconceito Linguístico (UFRJ), em parceria com o Departamento de Educação Escolar Indígena do município, criaram a ação “Me kuni umari — em rede pelos direitos linguísticos”, da qual trata este relato de experiência.

ABSTRACT The objective of this work was to characterize reference bacteria strains, belonging to the genus Bacillus and species of correlated genera, by simplified morphological and biochemical methods. The morphological characterization is based on the aspects of the colonies, as well as cytomorphology of the species, by optical and scanning microscopy. For biochemical characterization, the sensitivity test to antimicrobials by disk-diffusion is performed. Moreover, the strains were characterized by extracting intracellular proteins. Characteristics such as shape, color, and consistency of the colonies, in addition to the type of spore and production of protein crystals were determinants for the morphological characterization of these species. The antibiogram revealed high resistance to β-lactam group antibiotics, in species of Bacillus cereus s.l group. In Bacillus subtilis s.l. group there was high susceptibility to antibiograms, mainly for species of B. subtilis. The protein profile provided specific protein patterns for some species, mainly bands of 130 e 65 kDa for B. thuringiensis, 140 e 130 kDa for Lysinibacillus sphaericus, and 115 kDa for Brevibacillus laterosporus. Abstract Clinical case description: This report presents the case of a 32-year-old man with palpable solid nodule in the left testicle with a diagnostic hypothesis of testicular cancer, which turned out to be splenic tissue through pathological anatomy analysis. Relevance: The splenogonadal fusion is a rare and benign congenital anomaly related to the presence of ectopic spleen tissue in the gonadal parenchyma, caused by fusion between these two tissues during the fifth or sixth week of embryonic development. This condition is of great importance because its diagnosis is difficult and often confused with other gonadal disorders such as testicular neoplasm. Clinical implications: The most common consequence of this testicular disorder is the unnecessary orchiectomy, in which a variety of cases are submitted. To avoid testicle removal, intraoperative pathological examination can be used in these cases. Conclusions: The ectopic spleen in testicular parenchyma is a rare condition and its diagnosis is difficult. It is important to consider this condition as a differential diagnosis in testicular disorders, especially in the case of diagnostic hypothesis of testicular cancer, so that testicle removal can be prevented by the use of intraoperative pathological exam.

Resumen Descripción del caso clínico: En este reporte se presenta el caso de un hombre de 32 años con nódulo sólido palpable en testículo izquierdo con hipótesis diagnóstica de cáncer de testículo, que en realidad la anatomía patológica descubrió que se trataba de tejido esplénico. Relevancia: La fusión esplenogonadal es una anomalía congénita rara y benigna relacionada con la presencia de tejido esplénico ectópico en el parénquima gonadal, causada por la fusión entre estos dos tejidos durante la quinta o sexta semana de desarrollo embrionario. Esta condición es de gran importancia porque su diagnóstico es difícil y muchas veces se confunde con otros trastornos gonadales como la neoplasia testicular. Implicaciones clínicas: La consecuencia más frecuente de este trastorno testicular es la orquiectomía innecesaria, a la que se someten diversos casos. Para evitar la extirpación del testículo, en estos casos se puede utilizar el examen patológico intraoperatorio. Conclusiones: El bazo ectópico en parénquima testicular es una entidad rara y de difícil diagnóstico. Es importante considerar esta condición como un diagnóstico diferencial en los trastornos testiculares, especialmente en el caso de la hipótesis diagnóstica de cáncer testicular, para que la extracción del testículo pueda ser evitada mediante el examen patológico intraoperatorio.
