Na presente investigação, nosso objetivo principal foi relatar diferentes interações in vitro de um isolado bacteriano ambiental com espécies fúngicas. Através da identificação clássica, nós verificamos que o bacilo ambiental apresentava endósporos terminais e subterminais, características metabólicas de mesofilia, acidofilia, halotolerância, redução de nitrato e produção de enzimas, como caseinase e catalase. Análise de seqüências parciais do gene 16S RNAr contendo de 400 a 700 bases revelou identidade com gênero Bacillus. No entanto, a espécie Bacillus subtilis foi confirmada somente depois da análise de seqüências dos genes rpoB, gyrA, and 16S RNAr. Intensa atividade inibitória aos fungos ambientais, como Penicillium sp, Aspergillus flavus, A. niger, e fitopatogênicos, como Colletotrichum sp, Alternaria alternata, Fusarium solani e F. oxysporum f.sp vasinfectum, foi observada em coculturas com a cepa bacteriana (B. subtilis), particularmente em ágar Sabouraud dextrose e ágar DNase. Pigmentos de cor avermelhada e vermelho-amarronzado, provavelmente feomelaninas, foram secretados respectivamente por colônias de A. alternata e A. niger depois de sete dias de co-cultivo.

In this work, different reactions in vitro between an environmental bacterial isolate and fungal species were related. The Gram-positive bacteria had terminal and subterminal endospores, presented metabolic characteristics of mesophilic and acidophilic growth, halotolerance, positive to nitrate reduction and enzyme production, as caseinase and catalase. The analysis of partial sequences containing 400 to 700 bases of the 16S ribosomal RNA gene showed identity with the genus Bacillus. However, its identity as B. subtilis was confirmed after analyses of the rpoB, gyrA, and 16S rRNA near-full-length sequences. Strong inhibitory activity of environmental microorganisms, such as Penicillium sp, Aspergillus flavus, A. niger, and phytopathogens, such as Colletotrichum sp, Alternaria alternata, Fusarium solani and F. oxysporum f.sp vasinfectum, was shown on co-cultures with B. subtilis strain, particularly on Sabouraud dextrose agar (SDA) and DNase media. Red and red-ochre color pigments, probably phaeomelanins, were secreted by A. alternata and A. niger respectively after seven days of co-culture.

OBJETIVO: relacionar as leveduras identificadas aos sinais e sintomas clínicos das pacientes com candidíase vulvovaginal e investigar a importância dos parceiros sexuais na reincidência da infecção. MÉTODOS: foi desenvolvido estudo prospectivo de julho de 2001 a julho de 2003 com uma amostra de mulheres residentes na Grande São Paulo. Foram avaliadas 179 pacientes com suspeita clínica de vaginite fúngica, com idade entre 18 e 65 anos. Os critérios para exclusão foram: gravidez, comprometimento imunológico intrínseco e extrínseco, incluindo AIDS, diabetes, imunossupressão, pacientes em terapia com corticosteróides, antibióticos ou hormônios, em pós-menopausa, em uso de dispositivo intra-uterino e duchas vaginais ou espermicidas. Amostras de secreções vaginais ou da glande dos parceiros sexuais de pacientes com vaginite de repetição foram coletadas para microscopia e cultura de fungos. Colônias fúngicas isoladas em CHROMagar Candida foram identificadas por provas clássicas. O teste exato de Fisher foi usado para correlacionar o quadro clínico com as leveduras isoladas das pacientes. RESULTADOS: os sinais e sintomas clínicos mais relevantes na candidíase vulvovaginal foram prurido e corrimento, seguidos por eritema e edema, estatisticamente independente do agente etiológico. Leveduras foram diagnosticadas por microscopia direta em 77 pacientes com vulvovaginites, sendo obtidos 40 cultivos de Candida spp. Candida albicans (70%), C. glabrata (20%), C. tropicalis (7,5%) e C. guilliermondii (2,5%) foram identificadas. As leveduras prevalentes nos parceiros foram C. albicans e C. glabrata. As mesmas espécies foram detectadas nas companheiras e parceiros em 87% dos casos. CONCLUSÕES: as vulvovaginites fúngicas foram mais freqüentes em mulheres entre 18 e 34 anos de idade. Não foi observada correlação entre as espécies de leveduras detectadas e a sintomatologia clínica. Os parceiros sexuais podem ser importantes reservatórios de Candida spp e estar relacionados à manutenção da candidíase vulvovaginal.

PURPOSE: to relate yeasts identified by laboratory tests to clinical signs and symptoms in patients with vulvovaginal candidiasis, and to investigate the importance of the sexual partners in the recurrence of the infection. METHODS: from July 2001 to July 2003, a sample of 179 patients aged from 18 to 65 years old, with clinical suspicion of fungal vaginitis were analyzed in a prospective study in Great São Paulo. Exclusion criteria included: pregnancy, impaired intrinsic or extrinsic immune response (including Aids), diabetes or immunosuppression; patients undergoing corticosteroid, antibiotic or hormone therapy, in post menopause, with intrauterine device (IUD) or making use of vaginal douches or spemicides. Samples of vaginal and penis secretions from partners of patients with relapse of vaginitis episodes were collected for microscopy and fungal culture. Fungal colonies isolated in CHROMagar Candida were identified by classical methods. Fisher's exact t-test was used to correlate the clinical picture with the yeasts isolated from patients. RESULTS: the most relevant clinical signs and symptoms were pruritus and vaginal discharge followed by erythema and edema, statistically independent from the etiological agent. Direct microscopy revealed yeasts in 77 patients with vulvovaginitis, and 40 Candida spp cultures were obtained. Candida albicans (70%), C. glabrata (20%), C. tropicalis (7,5%) and C. guilliermondii (2,5%) were identified. The yeasts prevalent in partners were C. albicans and C. glabrata. The same species were detected in female and male sex partners in 87% of the cases. CONCLUSIONS: fungal vulvovaginitis was more frequent in women between 18 and 34 years old. No correlation was observed between the species of yeast detected and clinical symptomatology. Sexual partners are important Candida spp reservoirs and may be related to the maintenance of the vulvovaginal candidiasis.

RACIONAL: Endoscópios são rotineiramente utilizados em hospitais e clínicas e podem ser fontes potenciais de infecção cruzada quando a descontaminação é inadequada OBJETIVO: As rotinas de descontaminação dos endoscópios flexíveis (broncoscópios, gastrocópios e colonoscópios) realizadas em dois hospitais universitários do Brasil foram avaliadas durante 3 anos MATERIAL E MÉTODOS: Amostras aleatórias foram coletadas dos canais internos dos endoscópios, depois que o aparelho era utilizado nos pacientes e após o processo de desinfecção RESULTADOS: Contaminação superior a 103 foi verificada em amostras coletadas após o exame endoscópico, sendo isolado bacilos gram-negativos (n = 142: 56%), cocos gram-positivos (n = 43: 17%), leveduras (n = 43: 17%) e bacilos gram-positivos (n = 26: 10%). Em 72 das 149 amostras coletadas após procedimentos de limpeza e desinfecção, detectou-se bacilos gram-negativos (n = 55: 61%), cocos gram-positivos (n = 21: 23%), bacilos gram-positivos (n = 8: 9%) e leveduras (n = 6: 7%). Gastroscópios e colonoscópios eram os aparelhos com maior freqüência e taxa de contaminação. Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, Enterobacter spp, Serratia marcescens, Proteus mirabilis, Citrobacter freundii, Staphylococcus aureus, Staphylococcus coagulase negative, Micrococcus luteus, Candida albicans, C. tropicalis, C. glabrata, C. guilliermondii, Bacillus spp and Corynebacterium spp foram os mais identificados CONCLUSÃO: A limpeza inapropriada e curto período de tempo de desinfecção eram, respectivamente, os maiores fatores associados com a presença de microrganismos em gastroscópios e colonoscópios. De acordo com os organismos isolados, considera-se que a desinfecção nos hospitais era de nível baixo a intermediário. Após a investigação, os centros de endoscopia adequaram seus protocolos, sanando os problemas verificados nos procedimentos de descontaminação dos endoscópios.

BACKGROUD: Endoscopes are routinely used in hospitals and clinics of the world and they can be potential sources of cross-infection when the decontamination process is unsuitable AIM: The routines of flexible endoscope (bronchoscopes, esophagogastroduodenoscopes and colonoscopes) disinfection procedures used in two Brazilian university hospitals were evaluated during a 3-year period METHODS: Aleatory samples from internal channels of endoscopes were collected after patient examination and after cleaning/disinfection procedures RESULTS: A contamination >3 log10 was achieved in samples recovered from endoscopes after patient examination. These samples yielded gram-negative bacilli (n = 142: 56%), gram-positive cocci (n = 43: 17%), yeast cells (n = 43: 17%), and gram-positive bacilli (n = 26: 10%). Approximately, 72 out of 149 samples (48.32%) collected after undergoing the cleaning and disinfection procedures disclosed gram-negative bacilli (n = 55: 61%), gram-positive cocci (n = 21: 23%), gram-positive bacilli (n = 8: 9%) and yeast cells (n = 6: 7%). Esophagogastroduodenoscopes and colonoscopes were the most frequently contaminated devices. Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, Enterobacter spp, Serratia marcescens, Proteus mirabilis, Citrobacter freundii, Staphylococcus aureus, Staphylococcus coagulase negative, Micrococcus luteus, Candida albicans, C. tropicalis, C. glabrata, C. guilliermondii, Bacillus spp and Corynebacterium spp were predominantly identified CONCLUSION: Inappropriate cleaning and low times of disinfection were respectively the major factors associated with the presence of microorganisms in colonoscopes and esophagogastroduodenoscopes. By analyzing the identified germs, hospital disinfection was considered of either intermediate or poor level. After this investigation, both university centers improved their previous protocols for disinfection and conditions for reprocessing endoscopes.

RACIONAL: O método com utilização manual de glutaraldeído é amplamente empregado para desinfecção de endoscópios. A elevada rotina nos serviços de gastroscopia, pequena quantidade de equipamentos e a falta de conhecimento técnico sobre os processos de descontaminação contribuem para desinfecção inadequada dos endoscópios, intensificando o risco de transmissão de microrganismos. A água eletrolítica ácida tem apresentado eficácia na inativação e destruição de microrganismos e vem sendo usada na descontaminação de endoscópios. OBJETIVO: Verificar a eficiência microbicida da água eletrolítica ácida, produzida pelo aparelho Cleantop WM-1, em 20 gastroscópios contaminados após uso em pacientes. MATERIAL E MÉTODOS: Amostras coletadas do canal de biopsia dos endoscópios, após uso em pacientes (n = 20) e depois da desinfecção (n = 20), foram cultivadas em ágar tripticaseína de soja, MacConkey e Sabouraud dextrose. RESULTADOS: Dezessete das 20 amostras coletadas após o uso do aparelho em pacientes revelaram a presença de bacilos gram-negativos, cocos gram-positivos e leveduras em taxas de 10³ a 10(5) ufc/mL. Nenhuma amostra, das 20 coletadas após a descontaminação, apresentou contaminação microbiana. CONCLUSÃO: Nesse estudo preliminar, a desinfecção mecânica realizada pelo aparelho Cleantop com água eletrolítica ácida revelou resultados satisfatórios pela eliminação de microrganismos e otimização no tempo de processamento dos gastroscópios

BACKGROUND: The manual disinfection of endoscopes with glutharaldeyde is widely employed. The great routine in gastroenteroscopy services, low number of equipment and the lack of technical knowledge about the decontamination processes are factors that stimulate the inadequate endoscope disinfection, intensifying the risk of transmission of microorganisms. The electrolysed acid water has been effective in the inactivation and destruction of microorganisms. AIM: The purpose of this investigation was to verify the microbicidal efficiency of electrolyzed acid water (Cleantop WM-1) to decontaminate gastroscopes after their using in patients. MATERIAL AND METHODS: Samples from biopsy channel of flexible endoscopes collected after patient use (n = 20) and after disinfection (n = 20) were cultivated in tryptic soy agar, MacConkey agar and Sabouraud dextrose agar. RESULTS: Seventeen of the 20 samples collected after patients examination yielded gram-negative bacilli, gram-positive coccus and yeast cells in contamination of 3 to 5 log10 ufc/mL. Microbial growth was not verified in samples collected after the decontamination process. Conclusion - In this preliminary study, the mechanical disinfection carried through the Cleantop device with electrolyzed acid water showed satisfactory results for the elimination of microorganisms and time optimization in the reprocessing of gastroscopes.

In this work 20 clinical and 3 environmental yeast isolates were characterized by classical morphological and physiological methods, as well as molecular methods based on PCR of the ITS1-5.8S rDNA-ITS2 region. The characteristic morphology and biochemical profiles observed in these samples correspond to those described for the Pichia genera, more specifically to P. anomala. The profiles of susceptibility to five antifungal drugs were determined by two broth dilution methods. The results obtained by both methods were comparable and showed that clinical isolates presented more resistance to azoles, amphotericin B, and 5-fluorocytosine, than environmental ones did. By amplification and sequencing of internal transcribed spacers (ITS1 and ITS2) and the ribosomal 5.8S DNA, the yeast samples were divided into four groups, where the strains within each group had the same sequence. Of the analyzed yeast isolates, 78% were identified as Pichia anomala. Using RAPD analysis with seven different Operon primers, polymorphism was observed within the four groups. Our study highlights the growing importance of P. anomala in fungemic episodes in premature neonates. Furthermore, the methodologies used provide a powerful tool to identify and determine differences in similar strains of this yeast

Foi pesquisada a presença de fungos na conjuntiva ocular de 23 pacientes com AIDS, 24 pacientes infectados pelo HIV, ambos de controle ambulatorial e 48 indivíduos HIV negativos, durante um ano, nos meses de fevereiro, abril, julho e outubro. O fungo mais isolado foi Penicillium sp, seguido de Aspergillus sp, Candida sp e Rhodotorula sp. Embora sem diferença significante, em todas as coletas foi verificado um crescimento maior de fungos na conjuntiva dos portadores de AIDS, seguido pelo grupo infectado pelo HIV e em menor número nos indivíduos HIV-negativos.

Samples from the ocular conjunctiva of 23 patients with AIDS, 24 patients infected with human immunodeficiency virus (HIV) and 48 immunocompetent individuals were taken during the months of February, April, July and October. During the year an increase in fungi of the conjunctiva of the AIDS patients, followed by patients infected with HIV and a smaller percentage in healtly people was observed. The most frequently isolated fungus was Penicillium sp followed by Aspergillus sp, Candida sp and Rhodotorula sp.

Paracoccidioidomicose é considerada a micose sistêmica endêmica mais prevalente na América Latina. Apesar da maior parte da casuística de paracoccidioidomicose ocorrer entre trabalhadores rurais, há poucos casos documentados de ocorrência dessa micose entre índios brasileiros. São apresentados 2 casos de paracoccidioidomicose em índios Suruí, família linguística Tupi-Mondé, procedentes de Cacoal, Rondônia. Ambos apresentaram sorologia positiva à imunodifusão apenas com antígenos da fase miceliana do P. brasiliensis. Os autores apresentam revisão de literatura sobre a ocorrência dessa micose entre índios brasileiros e discutem a necessidade de futuras investigações buscando caracterizar as diferenças regionais de cepas de P. brasiliensis e seu impacto no diagnóstico sorológico dessa micose.

Paracoccidioidomycosis has been considered the most frequent endemic systemic mycosis in Latin America. Although most cases of paracoccidioidomycosis involve rural workers, this systemic fungal disease has been scarcely reported among Amerindian populations from Brazil. We report two cases of paracoccidioidomycosis in Tupi-Mondé Amerindians from Cacoal, state of Rondônia, Brazil. Both cases exhibited positive serological results by a specific immunodiffusion test only when the assay was performed with antigens obtained from the mycelial form of P. brasiliensis. The authors present a literature review of paracoccidioidomycosis in Brazilian Amerindians and discuss the need for further investigations about the impact of the antigenic diversity of P. brasiliensis from different geographic areas on the serological diagnosis of PCM.

Os mecanismos de virulência em Paracoccidioides brasiliensis não estão totalmente esclarecidos. Há um consenso que subcultivos sucessivos de P. brasiliensis acarretam a perda de sua patogenicidade que pode ser revertida pelo reisolamento do agente após passagem animal. As propriedades morfológicas e bioquímicas que são recuperadas ou demonstradas após passagem animal podem fornecer novas informações quanto a fatores relacionados à patogenicidade e virulência de P. brasiliensis. Nós avaliamos características morfológicas: porcentagem de células brotantes, número de brotamentos por célula e determinação do diâmetro de 100 células-mãe em 30 isolados de P. brasiliensis, antes e após passagem animal. Os isolados foram obtidos de pacientes com diferentes formas clínicas de paracoccidioidomicose (PCM): forma aguda (grupo A n=15) e forma crônica (grupo C n=15). A mensuração do tamanho das células foi feita em microscópio óptico Olympus CBB acoplado com régua micrométrica. Nós medimos o maior eixo transversal e longitudinal de 100 células viáveis de cada preparação. A porcentagem de células brotantes bem como o número de brotamentos por célula não foram influenciados pela passagem animal independente da origem da amostra (grupo agudo ou crônico). Os valores dos tamanhos dos isolados de P. brasiliensis dos grupos A e C medidos antes e após passagem animal exibiram o mesmo comportamento. Após passagem animal, houve diferenças estatisticamente significativas entre o tamanho das células de P. brasiliensis isolados a partir de testículos inoculados com amostras dos grupos A e C. O diâmetro máximo das células mãe dos isolados do grupo A exibiram tamanho de 42.1<FONT FACE="Symbol">m</font>m, em contraste com 32.9<FONT FACE="Symbol">m</font>m exibidos pelas células mãe do grupo C (p<0.05). O diâmetro de 1500 células do grupo A exibiram tamanho médio de 16.0<FONT FACE="Symbol">m</font>m (SD ± 4.0), valor significativamente maior que 14.1<FONT FACE="Symbol">m</font>m (SD ± 3.3) exibidos pelas 1500 células do grupo C (p<0.05). Nossos resultados confirmam o polimorfismo exibido por P. brasiliensis em amostras biológicas e reforça a necessidade de mais investigações com o objetivo de elucidar o papel que parâmetros morfológicos do fungo possam assumir na história natural da doença.

The basis for virulence in Paracoccidioides brasiliensis is not completely understood. There is a consensus that the sequencial in vitro subcultivation of P. brasiliensis leads to loss of its pathogenicity, which can be reverted by reisolation from animal passage. Attention to morphological and biochemical properties that are regained or demonstrated after animal passage may provide new insights into factors related to the pathogenicity and virulence of P. brasiliensis. We evaluated morphological characters: the percentage of budding cells, number of buds by cell and the diameter of 100 mother cells of yeast-like cells of 30 P. brasiliensis isolates, before and after animal passage. The isolates were obtained from patients with different clinical forms of paracoccidioidomycosis (PCM): acute form (group A, n=15) and chronic form (group C, n=15). The measurement of the yeast cell sizes was carried out with the aid of an Olympus CBB microscope coupled with a micrometer disc. We measured the major transverse and longitudinal axes of 100 viable cells of each preparation. The percentage of budding cells as also the number of buds by cell was not influenced by animal passage, regardless of the source of the strain (acute or chronic groups). The size values of P. brasiliensis isolates from groups A and C, measured before the animal passage exhibited the same behavior. After animal passage, there was a statistically significant difference between the cell sizes of P. brasiliensis isolates recovered from testicles inoculated with strains from groups A and C. The maximum diameter of mother cells from group A isolates exhibited a size of 42.1<FONT FACE="Symbol">m</font>m in contrast with 32.9<FONT FACE="Symbol">m</font>m exhibited by mother cells from group C (p<0.05). The diameter of 1500 mother cells from group A isolates exhibited a medium size of 16.0<FONT FACE="Symbol">m</font>m (SD ± 4.0), a value significantly higher than the 14.1<FONT FACE="Symbol">m</font>m (SD = ± 3.3) exhibited by 1500 mother cells from group C isolates (p<0.05). Our results reinforce the polymorphism exhibited by P. brasiliensis in biological material and the need for further investigations to elucidate the role of morphological parameters of the fungus in the natural history of the disease.

Diferente de histoplasmose e criptococose, doenças fúngicas associadas a depressão da imunidade celular, fungemia causada por Candida spp. tem sido raramente relatada em pacientes com AIDS. O presente relato ilustra um caso de candidemia em paciente não neutropênico portador de AIDS, cuja origem parece ter sido via translocação através do trato gastrointestinal.

Apart from cryptococcosis and histoplasmosis, which are mycoses contained by T cell-mediated mechanisms of host defense, fungemia is rarely found in AIDS patients. The frequency of fungemia due to Candida spp. has been reported to be as low as 1 %. We report a non-neutropenic AIDS patient who presented a candidemia which probably arose from her gastrointestinal tract.

Leveduras do gênero Candida têm sido reconhecidas como importantes causadoras de fungemias hospitalares. Foram estudados os DNA cromossômicos de oito cepas de C. albicans, obtidas de oito pacientes com fungemia hospitalar, por cariotipagem eletroforética através de "pulsed-field gel electrophoresis". As cepas foram obtidas pelo isolamento da levedura em seis hemoculturas e duas infecções relacionadas ao uso de cateter intra-venoso central. Foram identificados sete perfis de DNA cromossômico. Dois pacientes mostraram cepas com o mesmo perfil de DNA sugerindo transmissão nosocomial. A cariotipagem eletroforética revelou excelente capacidade discriminatória entre os isolados sendo útil no estudo das candidemias hospitalares.

Yeasts of the genus Candida have been recognized as important microorganisms responsible for nosocomial fungemia. Six blood-stream and two intravenous central catheter C. albicans strains were isolated from eight patients and studied by electrophoretic karyotyping of chromosomal DNA by pulsed-field gel electrophoresis. Seven chromosomal DNA profiles were identified. Two patients showed isolates with the same profile, suggesting nosocomial transmission. Karyotyping of C. albicans revealed an excellent discriminatory power among the isolates and may therefore be useful in the study of nosocomial candidemia.

Foi estudado o efeito killer de 9 cepas padrão de leveduras sobre 146 amostras de Candida albicans isoladas dos seguintes espécimes clínicos: mucosa bucal, fezes, lavado brônquico, escarro, secreção vaginal, urina, lesão de pele, lesão de unha e sangue. Usando este sistema foi possível diferenciar 23 biotipos de C. albicans. Os biotipos 211, 111 e 811 foram os mais freqüentemente isolados. A maioria das amostras de C. albicans (98,6%) foi sensível a pelo menos uma ou mais das 9 cepas killer. Empregando- se este sistema foi possível demonstrar que 2 pacientes albergavam mesmo biotipo killer, respectivamente, 111 e 211, em diferentes espécimes clínicos, e em outro paciente, o mesmo biotipo (211) foi isolado de hemoculturas realizadas em ocasiões distintas. O uso do sistema killer para diferenciar os tipos entre as espécies de leveduras patogênicas, pode ser um método útil para estabelecer a eventual fonte de infecção, constituindo uma ajuda valiosa para o controle e vigilância de infecções nosocomiais causadas por leveduras.

The authors studied the killer effect of nine standard strains of yeasts on 146 samples of Candida albicans isolated from the following clinical specimens: oral mucosa, feces, bronchial wash, sputum, vaginal secretion, urine, skin lesion, nail lesion and blood. Using this system it was possible to differentiate 23 biotypes of Candida albicans. The biotypes 211, 111 and 811 were most frequently isolated. Most of the samples of C. albicans (98.6%) were sensitive to at least one or more of the nine killer strains. Using the killer system it was possible to show that two patients harbored the same killer biotypes, 111 and 211, respectively, in different clinical specimens and another patient harbored the same biotype (211) in blood cultures effected in different ocasions. The utilization of the killer system to differentiate types among species of pathogenic yeasts can be a useful method to stablish the eventual source of infection, and it is a valuable tool to control and watch for nosocomial infections caused by yeasts.

Até a década de 80, antes do surgimento da AIDS, a criptococose era relativamente rara. Verificou-se que esta doença afeta muito frequentemente os pacientes com AIDS e pode ser a primeira manifestação desta síndrome. O objetivo deste trabalho é o de comparar o resultado dos exames das amostras do LCR inicial na meningencefalite por criptococo entre pacientes com e sem AIDS. Este trabalho é baseado no estudo de 41 pacientes com neurocriptococose sem AIDS e de 23 pacientes com neurocriptococose e AIDS. Este estudo permitiu verificar que a reação inflamatória, no sistema nervoso, quando presente, foi de menor intensidade nos pacientes com AIDS; a reação inflamatória esteve presente em todos os pacientes sem AIDS e ausente em 21,7% dos pacientes com AIDS; o quadro inflamatório crônico, semelhante ao da meningencefalite tuberculosa, clássico nas meningencefalites por criptococo, ocorreu raramente nos pacientes com AIDS; em média, o número de criptococos no LCR foi maior nos pacientes com AIDS; pode ser feito diagnóstico presuntivo da AIDS quando o criptococo estiver presente no LCR que não apresenta alterações citológicas e bioquímicas.

Neurocryptococcosis was a rare nervous system infection. With the rising number of patients with AIDS it became a very frequent disease. This infection is supposed to infect patients with some kind of immunodeficiency and the CSF alterations often simulate tuberculous meningitis. The purpose of this research was to compare the CSF changes in AIDS and non-AIDS patients with meningoencephalitis caused by Cr. neoformans. There were analysed 41 CSF samples from non-AIDS patients with neurocryptococcosis and 23 CSF samples from AIDS patients with neurocryptococcosis. The results of this research allowed to conclude that the inflammatory changes in the CSF from AIDS patients showed a lower intensity compared to those non-AIDS patients. These results showed as well, that the CSF samples from non-AIDS patients always revealed some changes besides the yeast cells. In some samples of AIDS patients, however the unique change was the presence of the yeast. It was demonstrated also, that the presence of Cr. neoformans in CSF, not accompanied by any other change, may suggest that is a patient with AIDS. In non-AIDS patients CSF alterations often simulates tuberculous meningitis. However these alterations were rare in AIDS patients. The yeast cells were more numerous in CSF samples from AIDS patients than in those from non-AIDS patients

São descritos os dois primeiros casos de neurocriptococose durante a gravidez diagnosticados em São Paulo (Brasil). Uma paciente foi tratada com anfotericina B no segundo trimestre, a outra, com anfotericina B no primeiro trimestre e no 2º trimestre com anfotericina B e 5 flucitosina. Ambas tiveram boa evolução materna e fetal. É apresentada revisão da literatura da neurocriptococose em gestantes.

Two cases of neurocryptococcosis were diagnosed during pregnancy in São Paulo (Brazil). Amphotericin B was used in the second trimester in one patient. The other, received amphotericin B during the first trimester of pregnancy and 5-fluorocytosine was added in the second trimester. In both, the pregnancy was uneventful and the fetus suffered no damage. The therapy used to treat pregnant women with cryptococcal meningitis is commented. The newborn follow-up is discussed. A review of the literature concerning neurocryptococcosis during pregnancy is presented.

Foram estudados retrospectivamente os prontuários de 15 pacientes portadores de leishmaniose e 28 pacientes de paracoccidioidomicose, nas formas de acometimento de mucosa oronasofaringolaringeana, atendidos na Escola Paulista de Medicina de 1986 a 1990. Estes pacientes foram comparados com relação às seguintes variáveis: sexo, idade, tempo de evolução da doença, topografia da lesão e queixas clínicas. Sexo, tempo de evolução da doença, topografia de lesão em septo nasal, palatos, língua e lábios, e queixa clínica com relação a dor em cavidade oral, disfagia/odinofagia e obstrução nasal apresentaram diferença estatisticamente significante em sua ocorrência nos dois grupos de pacientes. Os autores consideram estes achados relevantes na diferenciação clínica entre as duas moléstias.

We retrospectively evaluated the medical records of 15 patients diagnosed with leishmaniasis and of 28 patients diagnosed with paracoccidiodorrcycosis presenting with involvement of the oral-nasal-pharyngeal mucosa seen at Escola Paulista de Medicina from 1986 through 1990. These patients were compared in regard to the following variables: sex, age, time since disease onset, location of lesion, and clinical complaints. Sex, time since disease onset, lesion at the nasal septum, palate, tongue, and lips, and history of oral pain, dysphagia/ odinophagia, and nasal obstruction were significantly different in the two groups of patients. The authors point out to the relevance of these findings in the differential clinical diagnosis of leishmaniasis and paracoccidiodomycosis.

Com o objetivo de verificar o comportamento clínico-epidemiológico da cromoblastomicose no Estado do Maranhão, foi feito um estudo retrospectivo e prospectivo de 13 casos, no serviço de Doenças Infecciosas e Parasitárias do Hospital dos Servidores do Estado do Maranhão no período de nov/88 a julho/91. Para a investigação, foi utilizada uma ficha protocolo com todos os dados necessários para uma análise posterior. Nos casos analisados observou-se maior prevalência na faixa etária entre 50 e 60 anos (46,1%) e do sexo masculino (84,6%). Doze pacientes eram procedentes do Estado do Maranhão, dentre os quais 10 da microrregião da baixada ocidental maranhense. Quanto à profissão, 12 (92,3%) eram lavradores. Na sua maioria apresentavam as lesões nos membros inferiores deforma verrugo- confluentes, cor acastanhada, com prurido. O tempo de evolução variou de 0 a 15 anos em 12 casos (92,3%). Quanto aos aspectos laboratoriais, o exame histológico feito em 12 pacientes, diagnosticando cromoblastomicose em 100% deles e a cultura isolou Fonsecaea pedrosoi em 9 casos (70%). O tratamento realizado em todos os pacientes, com algumas variações foi feito com 5 - fluorocitosina apresentando bons resultados evolutivamente. Constatou-se neste trabalho uma provável zona endêmica de cromoblastomicose na microrregião da baixada ocidental maranhense, até agora desconhecida.

The aim of this study was to observe the clinical and of epidemiological behavior of chromoblastomycosis in the State of Maranhão. A retrospective and prospective study ofl 3 cases was performed at the infectious diseases section of the Hospital dos Servidores do Estado do Maranhão, from November, 1988 to July, 1991. In the investigation a protocol record was used with a view to further analysis. A higher prevalence betweem 50 and 60 years of age (46.1%) and in male patients (92.3%) was noted. Twelve patients (92.3%) were from Maranhão State, and 10 of them (76.9%) came from the Western Microregion of the State of Maranhão. Regarding occupation, 92.3 % were landworkers, and most of them presented the lesions on the lower limbs (84.6%) of a vegetant warty aspect, brown in color with itching. Histopathological exammination diagnosed chromomycosis in 100% of the cases. Culture was performed in 11 cases, and Fonsecaea pedrosoi isolated in 9 of them. Treatment with 5-fluorocytosine resulted in a good evolutive response. This study indicates the existence of a probable endemic area of chromomycosis in hinterland of Maranhão (Western Microregion) that hither to unknown.
