RESUMO O leite integral e a bebida láctea UAT são submetidos ao mesmo tratamento térmico, mas são produtos diferentes, já que na bebida láctea é permitida a adição de até 50% de soro lácteo. Neste estudo, objetivou-se avaliar as características microbiológicas destes produtos com relação à legislação vigente do Ministério da Agricultura, Pecuária e Abastecimento (MAPA) e, ainda, da população psicrotrófica, ao longo do período de validade. Foram analisadas 150 amostras, 75 de cada produto, a partir de cinco diferentes marcas comerciais nacionais. As 15 amostras de cada marca foram subdivididas em três lotes: início, meio e final do período de validade, cada qual com cinco amostras do mesmo lote. Verificou-se para o leite que todas as amostras de leite UAT estavam de acordo com a legislação vigente. A população de psicrotróficos aumentou em 26,6% das amostras depois que foram abertas e mantidas sob refrigeração por 48 horas. Os resultados para a bebida láctea evidenciaram que 12% das amostras estavam em desacordo com o estabelecido pela legislação vigente. A população de psicrotróficos aumentou em 36% das amostras depois que foram abertas e mantidas sob refrigeração por 48 horas. Tais resultados devem servir de alerta para a necessidade de fiscalização mais rigorosa da bebida láctea UAT, pois parte das amostras não atenderam aos padrões microbiológicos estabelecidos pela legislação.

ABSTRACT UHT milk and dairy drinks are submitted to the same heat treatment, but they are different products, since the addition of up to 50% of whey is allowed in dairy drinks. The aim of this study was to evaluate the microbiological characteristics of these products in relation to current legislation, during the course of their shelf life. In the present study, 150 samples, 75 of each type of product, from 5 different national brands were analyzed. The 15 samples of each brand were separated into 3 lots containing 5 samples each, analyzed at the beginning, middle and end of the validity period. Upon microbiological analysis all UHT milk samples were in conformance with current legislation. After the samples were opened and chilled for 48 hours, 26.6% of them presented an increased psychrotrophic population. The dairy drink results showed that 12% of the samples were out of conformance with the current legislation. After the samples were opened and chilled for 48 hours, 36% of them presented an increased psychrotrophic population. These results should be considered as an alert in regard to the need for rigorous inspection of UHT milk and dairy drink products.

RESUMO O presente estudo teve como objetivo avaliar o potencial de atividade antimicrobiana in vitro dos extratos de algumas plantas endêmicas do Cerrado tais como Baccharis dracunculifolia. Cochlospermum regium. Croton antisyphiliticus. Eugenia dysenterica e Lippia sidoides, frente ao agente Staphylococcus aureus isolado de leite mastítico, ósteo do teto da vaca, equipamento de ordenhadeira, fossas nasais e garganta do ordenhador. Os extratos foram preparados a partir das partes aéreas e sistema radicular das plantas utilizando os solventes metanol, hexano e clorofórmio na concentração de 10%. Para avaliação da atividade antimicrobiana foi aplicada a técnica de microdiluição em caldo para determinação da Concentração Inibitória Mínima (CIM) seguida da determinação da Concentração Bactericida Mínima (MBC). Os extratos de Baccharis dracunculifolia. Croton antisyphiliticus, seguido do extrato de Lippia sidoides, apresentaram, respectivamente, melhor atividade inibitória sobre a multiplicação da bactéria Staphylococcus aureus. Os resultados obtidos demonstraram, ainda, que as estirpes isoladas do leite e das fossas nasais do ordenhador foram resistentes a gentamicina, princípio ativo muito utilizado no combate à mastite bovina, porém sensíveis aos extratos das plantas referidas, reforçando a importância das plantas medicinais como recurso terapêutico e sua aplicabilidade.

ABSTRACT The objective of this study is to evaluate the potencial microbial activity in-vitro from the extract of some endemic plants from Cerrado such as Baccharis dracunculifolia. Cochlospermum regium. Croton antisyphiliticus. Eugenia dysenterica and Lippia sidoides, against the agent Staphylococcus aureus isolated from bovine mastitic milk, osteo from cow’s teat, milker equipament, nasal cavitites and milker’s gullet. The extracts were prepared from aerial parts as well as the reticular systems of plants using the solvents methanol, hexane and chloroform at a concentration of 10%. To evaluate the antimicrobial activity, the technique of microdilution in broth was used for determining the Minimal Inibitory Concentration (MIC) followed by the determination of Minimal Bactericidal Concentration (MBC). The extracts from Baccharis dracunculifolia and Croton antisyphiliticus, followed by extracts from Lippia sidoides, reported respectively, presented better inhibitory activity against the multiplication of the bacteria Staphylococcus aureus. Furthermore, the results demonstrate that the isolated strains from the milk and nasal cavities of the milker showed strong resistance against gentamicin, active agent commonly applied to combat mastitis bovine. However, there was sensitivity against extracts from the reported plants, reinforcing the importance of the medicinal plants as a therapeutic resource and its aplicability.

A total of 72 strains of Staphylococcus aureus were examined for the production of staphylococcal enterotoxins (SE) A, B, C, D and toxic shock syndrome toxin (TSST-1). The strains were isolated from milk samples from cows with mastitis in dairy herds of São Paulo State, Brazil. Off 72 isolates, 38 (52.8%) produced SEA, 38 (52.8%) SEB, 32 (44.4%) SED, 28 (38.9%) SEC and 27 (37.5%) TSST-1. From the 72 strains, 66 (91.7%) produced, at least, one or more toxin, including TSST-1.

RESUMO Este trabalho teve por objetivo conhecer a sensibilidade antimicrobiana “in vitro” de 72 cepas de Staphylococcus aureus isoladas em amostras de leite de vacas com mastite clínica e subclínica, em 10 propriedades rurais do Estado de São Paulo. Os testes foram realizados pela da técnica de difusão em discos padronizada por Kirby-Bauer. Os principios ativos que apresentaram maior sensibilidade foram a gentamicina (98,6%) e a eritromicina (98,6%), seguidos pela estreptomicina (94,4%), oxacilina (84,7%), novobiocina (73,4%), vancomicina (72,2%), ampicilina (4,2%) e a penicilina (2,8%). Os resultados obtidos evidenciaram, ainda, que as 72 (100%) cepas estudadas apresentaram resistência a pelo menos 2 antibióticos ou quimioterápicos e que nenhum destes princípios ativos, agindo isoladamente, pode ser ativo contra qualquer das cepas experimentadas.

ABSTRACT The purpose of this study was to determine the in-vitro susceptibility pattern of 72 Staphylococcus aureus strains, isolated from bovine mastitic milk. The calves were reared on 10 dairy farms in the state of São Paulo, Brazil. These strains were tested by the standardized disc diffusion method. The highest susceptibility was to gentamicin (98.6%) and erytromycin (98.6%), followed by streptomycin (94.4%), oxacillin (84.7%), novobiocin (73.4%), vancomycin (72.2%), ampicillin (4.2%) and penicillin (2.8%). Results showed that 72 (100%) strains were resistant to at least 2 antibiotics and chemotherapeutics. None of the active principles, acting alone, was effective against any of the strains tested.

Sentinel node (SN) status is the most important prognostic factor for localized melanoma. Usually, patients with Breslow thickness of less than 1.0 mm are not included in SN protocols. However, the literature presents a rate ranging from 3 to 7% of nodal recurrence in thin melanoma. Ulceration, regression and high mitotic rate have been considered to be indications for an SN biopsy. The metastatic potential of the vertical growth phase is uncertain. To correlate pathological features in thin melanoma with SN metastasis, we reviewed 358 patients submitted to SN biopsy. Seventy-seven patients with lesions of 1 mm or smaller were included in the study group. Histological evaluation of the primary tumor included thickness, Clark level, mitotic rate, ulceration, regression, and growth phase. Lymphoscintigraphy was performed on all patients. Lymphatic mapping and gamma probe detection were both used for SN biopsy. Histological examination of SN consisted of hematoxylin-eosin and immunohistochemical staining. Median follow-up was 37 months. Six patients had micrometastases. Statistical analysis by the Fisher test showed that ulceration (P = 0.019), high mitotic rate (P = 0.008) and vertical growth phase (P = 0.002) were positively correlated with micrometastases. If other studies confirm these results, more melanoma patients must be submitted to SN biopsy.

Os autores descrevem um caso clínico de ptose palpebral causada por uma patologia rara, o linfocitoma cútis. A paciente apresentava o quadro clínico do linfocitoma cútis em sua forma disseminada, com sintomatologia visual causada por nódulos palpebrais. A conduta cirúrgica realizada foi a ressecção dos nódulos palpebrais por meio de incisão de blefaroplastia, e o exame histológico confirmou o diagnóstico de linfocitoma cútis. O tratamento cirúrgico realizado foi eficiente para a melhora funcional das pálpebras.

A clinical case of acquired palpebral ptosis caused by a rare pathology, lymphocitoma cutis, is reported. An eyelid ptosis was characterized with the patient unable to lift the eyelid. The resection of the eyelid nodules was performed using blefaroplasty incision as access and the result of the anatomopathological examination confirmed the diagnosis: lymphocitoma cutis. The surgical result was satisfactory not only esthetically but also functionally (2-year follow-up).

Os autores descrevem as características anatômicas do músculo levantador do lábio superior por meio de dissecações em cadáveres. OBJETIVO. Contribuir ao estudo dos músculos da mímica, estudando os seus detalhes anatômicos e suas relações, ajudando na realização de retalhos da face. MÉTODOS. Foram dissecadas 20 peças de cadáveres, verificando-se suas inserções, comprimento, largura, espessura, relações, inervação e irrigação. RESULTADOS. Em todos os casos, o músculo levantador do lábio superior apresentou inserção proximal na margem inferior da órbita e inserção distal de dois tipos: por meio de fibras laterais sobrepondo-se superficialmente ao músculo orbicular do lábio e de fibras profundas interdigitando-se, formando parte do embricamento do canto da boca (70%); e mediante fibras sobrepondo-se superficialmente ao músculo orbicular do lábio (30%). A média do comprimento foi de 24,66mm e a média da espessura, de 3,57mm. A largura em nível da inserção distal foi, em média, de 11,2mm, enquanto que em nível da inserção proximal foi, em média, de 15,96mm. Relaciona-se anteriormente ao músculo levantador do ângulo da boca, posteriormente à porção distal do músculo zigomático menor (90%) e posteriormente à porção média do músculo zigomático menor (10%). A inervação é feita pelo ramo inferior do nervo zigomático (n. facial) e nervo infra-orbital (n. trigêmeo). Ramos da artéria angular irrigam a porção inferior do músculo e a artéria infra-orbital nutre a porção superior deste músculo.

The authors describe the anatomical characteristics of the levator labii superioris muscle by dissection in cadavers. PURPOSE. We describe the characteristics of these muscle, the details and relations, hopefully contributing to the study of muscle of the face. METHODS. Twenty faces of cadavers were dissected. The following features were studied: origin, insertion, length, width, thickness, relations, innervation and blood supply. RESULTS. In all cases the muscle originated from the inferior orbital margin. Two insertions were observed: via lateral fibers, superficial to the orbicularis oris muscle and via deep fibers thar form part of the raphe at the corner of the mouth (70%); via superficial fibers to the orbicularis oris muscle (30%). The average of the length was 24.66mm and the average of the thickness was 3.57mm. The width at its insertion was 11,2 mm, and at the origin was 15.96mm. The levator labii superioris muscle was found to be anterior to the levator anguli oris; it was posterior to the distal portion of the zygomaticus minor (90%) and posterior to the mid portion of the zygomaticus minor (10%). The innervation was from the inferior branch of the zygomatic nerve (facial nerve) and from the infraorbital nerve (trigeminal nerve). The inferior portion of the muscle is supplied by branches of the angular artery and the superior part from branches of the infraorbital artery.

Os autores relatam caso de paciente que, após várias ressecções prévias de carcinomas de face, evoluiu com deformidade do lábio superior, com comprometimento estético e funcional. Associadamente, apresentava perda total da columela. A reparação total do lábio superior foi realizada por meio do retalho musculocutâneo, em ilha do depressor do ângulo da boca.

Case report of a man that had been previously submitted to many resections of facial skin carcinomas. A serious deformity on the upper lip and columella remains. A total upper lip and columella reconstruction through the depressor anguli oris myocutaneous island flap is described.
