Resumo Objetivo: Este estudo avalia os efeitos da sarcopenia e da caquexia na qualidade de vida de pacientes com câncer gastrointestinal durante o ciclo inicial de quimioterapia, enfatizando a importância da tomografia computadorizada (TC) na avaliação da massa muscular. Materiais e Métodos: Estudo prospectivo com 60 pacientes adultos com câncer gastrointestinal que iniciaram quimioterapia de janeiro a dezembro de 2017. A TC foi utilizada para o diagnóstico de sarcopenia e o Quality of Life Questionnaire Core 30 da European Organization for Research and Treatment of Cancer foi utilizado para avaliar a qualidade de vida. Resultados: A média de idade dos pacientes foi 60,9 anos e 33 (55%) eram homens. Entre os pacientes, 33 (55%) eram caquéticos e 14 (24%) eram sarcopênicos. A quimioterapia reduziu significativamente a qualidade de vida, especialmente nos domínios físico, de desempenho de papéis e social, sem diferenças entre os grupos caquéticos e sarcopênicos. Conclusão: A diminuição da qualidade de vida não difere significativamente entre pacientes caquéticos/sarcopênicos e não caquéticos/não sarcopênicos com câncer gastrointestinal submetidos a quimioterapia, conforme classificado pela massa muscular medida por TC. No entanto, a avaliação da massa muscular por TC continua crucial para orientar estratégias de intervenção personalizadas. A integração dessa avaliação nos laudos radiológicos pode fornecer informações valiosas para o planejamento de cuidados específicos, melhorando a qualidade de vida dos pacientes durante o tratamento. Abstract Objective: This study evaluates the effects of sarcopenia and cachexia on the quality of life (QoL) of patients with gastrointestinal cancer during their initial cycle of chemotherapy, emphasizing the significance of computed tomography (CT) in assessing muscle mass. Materials and Methods: In this prospective study, we evaluated 60 adult patients with gastrointestinal cancer who started chemotherapy between January and December of 2017. Sarcopenia was diagnosed on the basis of CT findings, and QoL was assessed with the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30. Results: The mean age was 60.9 years, and 33 (55.0%) of the patients were men. Of the 60 patients, 33 (55.0%) had cachexia and 14 (23.3%) had sarcopenia. Chemotherapy significantly reduced QoL, particularly in the physical, role functioning, and social domains, with no differences between the cachexia and sarcopenia groups. Conclusion: Among patients with gastrointestinal cancer submitted to chemotherapy, the chemotherapy-induced decline in QoL does not seem to differ significantly between those with cachexia or sarcopenia, as classified by CT-measured muscle mass, and those without. However, CT-based muscle mass evaluation remains crucial for guiding customized intervention strategies. Integrating this evaluation in radiological reports can provide valuable insights for planning specific care, thus improving patient QoL during treatment. ABSTRACT Objective: This study investigated the acute effects of high-intensity interval (HIIE) and moderate-intensity continuous (MICE) exercise on ghrelin levels in obese men. Subjects and methods: A total of 10 obese men (age 27.6 ± 1.8 years, body mass index 35.4 ± 4.5 kg/m², body fat 39.9 ± 2.1%) performed two exercise sessions in a randomized order: HIIE (10 × 1 min intervals at 90% of the maximal heart rate [HRmax] interspersed by 1 min of active recovery) and MICE (20 min at 70% of the HRmax). Ghrelin levels were assessed pre-, post- and 1h post-exercise, and energy intake was assessed 1h post-exercise through an ad libitum meal. Results: HIIE and MICE showed a trend to decrease ghrelin levels immediately post-exercise (-14.1 ± 21.6% and −9.6 ± 23.8%, respectively, p = 0.07) and decreased 1h post-exercise (-12.7 ± 31.8% and −13.8 ± 21.7%, respectively, p < 0.05). No changes were observed for post-exercise energy intake (p > 0.05). There was a positive correlation between the change in ghrelin levels and post-exercise energy intake only for HIIE (r = 0.63, p = 0.05). Conclusion: In summary, a single session of HIIE and MICE elicits a reduction on ghrelin levels without changing post-exercise energy intake in obese men.

ABSTRACT BACKGROUND: During a surgical procedure, patients are often subjected to fasting for times that are more prolonged than the ideal, which may lead to complications. OBJECTIVE: To evaluate the duration of perioperative fasting and its association with postoperative complications, length of hospital stay (LOS) and mortality among gastric and colorectal cancer patients. DESIGN AND SETTING: Cohort study developed in a surgical oncology hospital in the city of Natal (Rio Grande do Norte, Brazil). METHODS: Patients aged over 18 years were included. The Clavien-Dindo surgical complication scale was used to evaluate occurrences of postoperative complications. LOS was defined as the number of days for which patients stayed in the hospital after surgery, or until the day of death. RESULTS: Seventy-seven patients participated (59.8 ± 11.8 years; 54.5% females; 70.1% with bowel tumor). The incidences of postoperative complications and death were 59.7% and 3.9%, respectively. The duration of perioperative fasting was 59.0 ± 21.4 hours, and it was higher among non-survivors and among patients with prolonged hospital stay (≥ 6 days). For each one-hour increase in the durations of perioperative and postoperative fasting, the odds of prolonged hospitalization increased by 12% (odds ratio, OR = 1.12; 95% confidence interval, CI 1.04-1.20) and 5% (OR = 1.05; 95% CI 1.02-1.08), respectively. CONCLUSION: Prolonged perioperative fasting, especially in the postoperative period, was observed in a sample of patients with gastric and colorectal cancer, and this was an independent predictor of LOS.

ABSTRACT BACKGROUND: Children with acute lymphoblastic leukemia are at risk of malnutrition, but few studies have described the changes in nutritional status during the different phases of chemotherapy. OBJECTIVE: To evaluate changes in nutritional status, food intake and appetite-regulating hormones among children and adolescents with acute lymphoblastic leukemia in the first phase of chemotherapy. DESIGN AND SETTING: Cohort study developed in the pediatric oncology departments of two hospitals in the city of Natal, Rio Grande do Norte, Brazil. METHODS: Fourteen children/adolescents (mean age of 7 years; 50% female) with acute lymphoblastic leukemia were monitored over the 28 days of an induction chemotherapy cycle. Anthropometric measurements, 24-hours food weight records and appetite-regulating hormone levels (ghrelin, leptin, insulin and cortisol) were obtained at three different times (before, in the middle and at the end of the induction phase). RESULTS: Most of the patients (85.7%) had normal weight at the beginning of the treatment, and this did not change significantly during the 28 days. Energy and nutrient intakes improved from the start of the treatment to the midpoint, according to the ghrelin levels (from 511.1 ± 8.3 to 519.3 ± 6.6 pg/ml; P = 0.027). Other appetite-regulating hormones did not present changes. CONCLUSION: Food consumption improves during the first phase of treatment, without alterations in anthropometric nutritional status.

ABSTRACT Objective A large number of studies have used abdominal computed tomography (CT) to quantify body composition, and different software programmes have been used to perform these analyses. Thus, this comparison is important to enable researchers to know the performance of more accessible software. Subjects and methods Fifty-four abdominal CT scans of obese (BMI 30 to 39.9 kg/m2), sedentary adults (24-41 years) patients from a Brazilian single center were selected. Two software programs were compared: Slice-O-Matic (Tomovision, Canada) version 5.0 and OsiriX version 5.8.5. The body composition analysis were segmented using standard Hounsfield unit (HU) (adipose tissue: -190 to +30 and skeletal muscle: -29 to +150) and measured at the mid third lumbar vertebra (L3) level on a slice showing both transversal processes. Bland-Altman limits of agreement analyses were used to assess the level of agreement between Slice-O-Matic and OsiriX. Results A total of fifty-four participants were evaluated, with majority women (69%), mean of age 31.3 (SD 6.5) years and obesity grade I most prevalent (74.1%). The agreement, in Bland-Altman analysis, between Slice-O-Matic and OsiriX analisys for the muscle mass tissue, visceral adipose tissue and subcutaneous adipose tissue were excellent (≥ 0.954) with P-values < 0.001. Conclusion These findings show that Slice-O-Matic and OsiriX softwares agreement in measurements of skeletal muscle and adipose tissue and sarcopenia diagnosis in obese patients, suggesting good applicability in studies with body composition in this population and clinical practice.

ABSTRACT BACKGROUND: The lipid accumulation product (LAP) index is an abdominal adiposity marker. OBJECTIVE: The aim of this study was to describe the cardiovascular risk of primary healthcare users through the LAP index and correlate it with anthropometric and biochemical indicators. DESIGN AND SETTING: Cross-sectional study in primary care units in a city in northeastern Brazil. METHODS: The subjects responded to a structured questionnaire that contained questions about their sociodemographic condition, and then underwent an anthropometric nutritional assessment. The LAP index values were expressed as three degrees of cardiovascular risk intensity: high risk (above the 75th percentile), moderate risk (between the 25th and 75th percentiles) and low risk (below the 25th percentile). RESULTS: The median LAP index was 52.5 cm.mmol/l (range: 28.2-86.6), and there was no statistically significant difference between the sexes: 57.7 cm.mmol/l (24.5-91.1) and 49.5 cm.mmol/l (29.8-85.2) for females and males, respectively (P = 0.576). Among all the subjects, 67.2% were overweight and there was a statistically significant difference in mean LAP index between those who were and those who were not overweight. Statistically significant differences in anthropometric and biochemical markers for cardiovascular risk were observed among individuals who had higher LAP index values. There were significant correlations between the LAP index and all of the biochemical variables. CONCLUSIONS: These significant correlations between the LAP index and the traditional biochemical risk markers may be useful within conventional clinical practice, for cardiovascular risk screening in primary healthcare.

RESUMO Introdução: Sabe-se que ingestão de bebidas que contêm carboidrato pode aprimorar o desempenho esportivo. No entanto, muitos times não têm recursos financeiros suficientes para investir em produtos comerciais e precisam preparar suas próprias bebidas de hidratação. Jacuba é usada como bebida esportiva (fonte de carboidrato) em clubes de futebol no Nordeste do Brasil. Objetivo: Avaliar a influência de duas bebidas com carboidrato sobre glicose sanguínea, desidratação e fadiga em um time de futebol do Nordeste do Brasil. Métodos: Foram avaliados 22 jogadores de futebol em dois dias de treinamento físico, com ingestão de água à vontade e duas bebidas: 1) bebida à base de maltodextrina, ou 2) Jacuba. A glicemia capilar foi medida antes e depois do exercício. A fadiga foi avaliada usando a Escala de Borg após o exercício. Para verificar a desidratação dos atletas no final do treino, realizou-se o teste de urinálise e a verificação da alteração da massa corporal durante o treinamento. Resultados: A glicose sanguínea mudou para 13,1±29,5 mg/dl e 7,6±22,2 mg/dl nos dias de treinamento com o consumo de maltodextrina e Jacuba, respectivamente, sem significância estatística. Não houve mudanças na massa corporal e no consumo de água e de líquidos totais nos dois dias de treinamento. A pontuação da Escala de Borg após o treinamento foi 15,2±2,6 e 14,9±3,0 para maltodextrina e Jacuba, respectivamente (p = 0,12). Conclusão: A Jacuba pode ser usada para melhorar a hidratação e manter a glicose sanguínea em atletas de futebol, porque seus resultados não diferem da bebida com maltodextrina.

ABSTRACT Introduction: It is known that ingestion of carbohydrate-containing beverages can enhance sports perfor-mance. However, many teams do not have sufficient financial resources to invest in commercial products and need to prepare their own hydration drinks. Jacuba is a beverage used as sport drink (source of carbohydrate) in soccer clubs in Northeastern Brazil. Objective: To evaluate the influence of two beverages with carbohydrate on blood glucose, dehydration and fatigue in a soccer team of Northeastern Brazil. Methods: Twenty-two soccer players were evaluated in two days of physical training, with ingestion of water ad libitum and two beverages: 1) maltodextrin-based drink, or 2) Jacuba. Capillary blood glucose was measured before and after training. Fatigue was assessed using Borg Scale after exercise. To evaluate the dehydration of the athletes at the end of exercise, the urinalysis test and the verification of the body mass change during the training were performed. Results: Blood glucose changed to 13.1±29.5 mg/dL and 7.6±22.2 mg/dL on training days with consumption of maltodextrin and Jacuba, respectively, without statistical significance. There were no changes in body mass and in water and total fluids intake in the two days of training. The Borg Scale score after training was 15.2±2.6 and 14.9±3.0 for maltodextrin and Jacuba, respectively (p=0.12). Conclusion: Jacuba can be used to improve hydration and maintain blood glucose in soccer players because their results do not differ with those of mal-todextrin-based drink.

RESUMEN Introducción: Se sabe que la ingestión de bebidas que contienen carbohidratos puede mejorar el rendimiento deportivo. Sin embargo, muchos equipos no tienen suficientes recursos financieros para invertir en productos comerciales y tienen que preparar sus propias bebidas de hidratación. Jacuba es utilizada como una bebida deportiva (fuente de carbohi-dratos) en los clubes de fútbol del Nordeste de Brasil. Objetivo: Evaluar la influencia de dos bebidas con carbohidratos en glucosa en la sangre, deshidratación y fatiga en un equipo de fútbol en el Nordeste de Brasil. Métodos: Se evaluó un total de 22 jugadores de fútbol en dos días de entrenamiento físico, con la ingesta de agua sin restricción y dos bebidas: 1) la bebida a base de maltodextrina, o 2) Jacuba. La glucosa en la sangre capilar se midió antes y después del ejercicio. La fatiga se evaluó mediante la escala de Borg después del ejercicio. Para evaluar la deshidratación de los atletas en el final del entrenamiento se llevó a cabo un análisis de orina y la verificación del cambio en la masa corporal durante el entrenamiento. Resultados: La glucosa en sangre ha cambiado para 13,1±29,5 mg/dl y 22,2±7,6 mg/dl en los días de entrenamiento con el consumo de maltodextrina y Jacuba, respectivamente, sin significación estadística. No hubo cambios en el peso corporal y el consumo de agua y líquidos totales en los dos días de entrenamiento. La puntuación de la escala de Borg después del entrenamiento fue de 15,2±2,6 y 14,9±3,0 para maltodextrina y Jacuba, respectivamente (p = 0,12). Conclusión: Se puede utilizar Jacuba para mejorar la hidratación y mantener la glucosa en sangre en jugadores de fútbol, debido a que sus resultados no fueron diferentes de la bebida con maltodextrina.

O objetivo do estudo foi verificar a prevalência do uso de suplementos alimentares nas academias da cidade de Porto Alegre. Foram entrevistados 316 indivíduos com idade entre 18-59 anos. A abordagem foi por voluntariedade, em 22 academias da cidade. Observou-se um expressivo uso de suplementos nutricionais pelos participantes (28,8%), e os homens utilizam mais suplementos do que as mulheres (p<0,05). Os suplementos mais utilizados foram aqueles à base de proteínas, que são ingeridos principalmente por indicação de educadores físicos. O alto consumo de suplementos nutricionais aponta a necessidade de alertar à população quanto aos riscos relacionados ao uso indevido de suplementos nutricionais.

The aim of this study was to verify the prevalence of the use of food supplements at fitness centers in the city of Porto Alegre. Were interviewed 316 individuals at an age range between 18 and 59. Individuals participated voluntarily in 22 fitness centers in the city. The study has revealed a significant use of nutritional supplements by the participants (28.8%) and men intake more supplements than women (p<0.05). The most common supplements reported in the study were those mainly based on proteins, which are mainly chosen because of suggestions made by physical educators. The high intake of nutritional supplements indicates the necessity of alerting the population about the risks related to the inappropriate use of nutritional supplements.

El objetivo del estúdio fue verificar la prevalencia de la utilizacíon de suplementos dietéticos en las academias de la ciudad de Porto Alegre. Fueron entrevistadas 316 personas con edad entre 18 y 59 años. La abordaje fue por voluntariedad en 22 academias de la ciudad. Se ha observado un expresivo uso de suplementos nutricionales por los participantes (28,8%), y los hombres utilizan más los suplementos que las mujeres (p<0,05). Los suplementos con mayor utilizacíon fueron los a base de proteínas que son ingeridos principalmente por la indicación de educadores físicos. Lo elevado consumo de suplementos nutricionales demuestra la necesidad de advertir la populación sobre los riesgos relacionados al uso inadecuado de suplementos nutricionales.

OBJETIVO: Tendo em vista que pacientes internados em unidade de terapia intensiva estão em risco nutricional, e que a terapia nutricional nem sempre é iniciada no momento adequado, o objetivo deste estudo foi associar o estado nutricional, a nutrição precoce e a hiperglicemia com a mortalidade de pacientes internados em unidade de terapia intensiva. MÉTODOS: Trata-se de um estudo de coorte histórica com utilização de banco de dados secundários de 453 pacientes que, após permanecerem durante um período mínimo de 48 horas na unidade de terapia intensiva, foram acompanhados até o 8º dia de internação. O estado nutricional foi classificado de acordo com índice de massa corporal. Considerou-se nutrição precoce a oferta de energia nas primeiras 48 horas de internação, independentemente da via. A glicemia foi monitorada com glicosímetro. RESULTADOS: A maioria dos pacientes era do gênero masculino (54,2%) e quase a metade apresentava excesso de peso (48,4%). Ao final das primeiras 48 horas, 69,4% dos pacientes já estavam sendo alimentados, e apenas 13,5% ainda apresentavam hiperglicemia. Os pacientes que receberam terapia nutricional precoce apresentaram menor risco de mortalidade (p=0,002), independentemente de possuir outros fatores associados com a mortalidade. CONCLUSÕES: A associação significativa entre a terapia nutricional precoce e a sobrevivência ressalta a importância da nutrição para pacientes graves. A baixa frequência de hiperglicemia pode ser um indicador da adequada prescrição da terapia nutricional e aplicação do protocolo de insulina na unidade de terapia intensiva da instituição.

OBJECTIVE: Because patients hospitalized in intensive care units are at risk for poor nutrition, and nutritional therapy is not always started at an appropriate time, the present study aimed to correlate nutritional status, early nutrition, and hyperglycemia with patient mortality in an intensive care unit. METHODS: This archival cohort study used the secondary database of 453 patients who stayed at least 48 hours in an intensive care unit and were assessed for 8 days of hospitalization. Patient nutritional status was defined according to the body mass index. Early nutrition was defined as an feeding energy within the first 48 hours of hospitalization, regardless of the administration route. Blood glucose levels were monitored using a glucometer. RESULTS: A majority of patients were male (54.2%), and approximately half of patients were overweight (48.4%). At the end of the first 48 hours, 69.4% of patients had received nutrition, and only 13.5% of patients still exhibited hyperglycemia. The patients who received early nutritional therapy exhibited lower a mortality risk (p = 0.002), regardless of the presence of other factors associated with mortality. CONCLUSIONS: The significant correlation between early nutritional therapy and survival emphasizes the importance of nutrition in severely ill patients. The low frequency of hyperglycemia found in this study might indicate that the prescription of nutritional therapy and the application of an insulin protocol are appropriate at institutional intensive care units.

A alimentação adequada é a chave para a melhora do desempenho esportivo. Com isso, um conhecimento nutricional satisfatório torna-se importante para as práticas alimentares saudáveis e, consequentemente, mais eficazes para o rendimento. O objetivo deste estudo foi avaliar o conhecimento nutricional de atletas profissionais de atletismo e triathlon. Foram avaliados 50 atletas com idade média de 32,4 ± 11,1 anos, sendo 37 (74%) do sexo masculino. Os atletas preencheram um questionário que continha informações sobre treinamento, consultas nutricionais e a escala de conhecimento nutricional validada. Para associar o conhecimento nutricional com as variáveis de escolaridade e consulta com nutricionista, foi utilizado o teste do Qui-quadrado, e para correlacionar as variáveis ordinais foram utilizadas a correlação de Pearson. Os atletas treinavam a 9,4 + oito anos, 82% possuíam nível superior incompleto ou completo e 70% já haviam consultado um profissional nutricionista. A média de pontos da escala foi de 9,6 ± 2,2 pontos, que corresponde à classificação de "moderado conhecimento nutricional", e não foi verificada diferença estatisticamente significativa entre o conhecimento nutricional e o sexo (p = 0,067). Ter frequentado ou não um curso superior não modificou o conhecimento nutricional destes atletas (p = 0,352), assim como consultar ou não o nutricionista (p = 0,362). Porém, quanto maior a escolaridade, maior foi o número de atletas que relataram o acompanhamento com o nutricionista. Estes resultados apontam para a necessidade de educação continuada em nutrição para atletas profissionais, bem como a necessidade de outros estudos avaliando o conhecimento nutricional neste público.

Proper nutrition is the key to better sports performance. Therefore, satisfactory nutrition knowledge becomes important to the practice of healthy nutrition habits and, consequently, more efficient to performance. This study aimed to evaluate the nutrition knowledge of professional track and field athletes and triathletes with application of a scale previously validated in Brazil. The sample was composed of 50 athletes with mean age of 32.4 ± 11.1 years, being 37 of them (74%) male athletes. The athletes filled up a questionnaire containing questions on training, nutritionist appointments and the scale. Chi-square was used to associate the nutrition knowledge with the variables of educational background and nutritionist appointment and the Pearson Correlation Coefficients were used to correlate the ordinal variables, with significance rates of p< 0. 05. The athletes had trained for 9. 4 ± 8 years; 82% had high incomplete or complete educational level and 70% had already had a nutritionist appointment. The average of points in the scale was of 9.6 ± 2.2 points, which corresponds to the classification of "moderate nutrition knowledge", and the males showed higher score in the scale of nutrition knowledge than the females (p = 0.047) The educational level (p = 0.352) and nutritionist appointment (p = 0.285) variables had no correlation with nutrition knowledge results in the present study. However, the higher the educational level, the higher the number of athletes who had nutrition counseling. These results indicate the need of continuous education on nutrition for athletes, as well as further investigation which evaluates the nutrition knowledge in this sample.

O objetivo do estudo foi analisar a perda de peso (PP) e o consumo de líquidos durante sessões de treinamento de ballet clássico. A amostra foi composta por vinte e uma bailarinas, que foram avaliadas em três sessões de ballet: uma sessão sem oferta de líquidos, uma sessão com oferta de água e outra com oferta de bebida esportiva. Na sessão em que as bailarinas receberam água, a mediana de consumo foi 376,19 mL e a mediana de PP foi -190g. Na sessão com bebida esportiva, a mediana do consumo foi de 226,19 mL e a mediana da PP foi -150g. Observou-se diferença na ingestão de líquidos, mas não na PP, durante as sessões com consumo de líquidos (p=0,01 e p=0,79, respectivamente). As bailarinas, voluntariamente, beberam mais água do que bebida esportiva durante os treinos de ballet clássico.

The purpose of this study was to analyze the weight loss(WL) and consumption fluids during training sessions in classical ballet. The sample was composed of 21 ballet dancers, who were evaluated in three sessions of ballet: session without a supply of liquid, a session with water supply and other with provision of sports drink. At the session in which the dancers received water, the median consumption was 376,19 mL and the median was WL-190g. In the session with sports drink, the median consumption was 226,19 mL and the median was WL-150g. Observed difference in fluid intake, but not in WL, during the sessions with fluid intake (p = 0,01 and p = 0,79, respectively). The dancers, voluntarily, drank more water than the sports drink during training of classical ballet.

El objetivo de este estudio fue analizar la pérdida de peso (PP) y la ingesta de líquidos durante las sesiones de formación de ballet clásico. La muestra estuvo conformada por 21 bailarines, que fueron evaluados en tres sesiones de ballet: sesión sin un suministro de líquido, una sesión com La prestación de agua y otro con bebida deportiva. En la sesión en la que los bailarines recibieron água, la mediana del consumo era de 376,19 ml y la mediana de PP-190g. En la sesión con bebida deportiva, la mediana del consumo era de 226,19 ml y la mediana de PP-150g. Observado diferencia en la ingesta de líquidos, pero no en PP, durante las sesiones con la ingesta de líquidos (p = 0,01 y p = 0,79, respectivamente). Los bailarines, voluntariamente, bebió más agua que bebida deportiva durante el entrenamiento del ballet clásico.

FUNDAMENTO: A circunferência abdominal (CA) é a medida que mais se correlaciona com os fatores de risco e morte por doença cardiovascular. Entretanto, o impacto da obesidade no prognóstico de pacientes com doenças cardiovasculares permanece controverso e requer maiores esclarecimentos. OBJETIVO: Avaliar a CA como preditor de evolução em 30 dias em pacientes que internaram com síndrome coronariana aguda (SCA), em hospital de referência no tratamento de doenças cardiovasculares. MÉTODOS: Coorte contemporânea com 267 pacientes que internaram por SCA e que foram seguidos por 30 dias após a alta levando em consideração os eventos cardiovasculares maiores - MACE - (óbito, reinfarto, reinternação para procedimentos de revascularização). Nas primeiras 24 horas da admissão, os pacientes responderam a um questionário e posteriormente tiveram a CA mensurada. A análise estatística foi realizada com SPSS 17.0, utilizando o teste do Qui-quadrado para variáveis categóricas e o teste t de Student para as variáveis numéricas, com o nível de significância de p < 0,05. As variáveis que apresentaram valores de p < 0,10, na análise bivariada, foram incluídas em um modelo de regressão logística para avaliar o papel da CA como preditor independente de MACE. RESULTADOS: Após análise multivariável, apenas o gênero feminino (RC = 8,86; 95% IC:4,55-17,10; p < 0,00), hipertensão arterial sistêmica (RC = 2,06; 95% IC:1,10-3,87; p = 0,02) e história familiar de cardiopatia isquêmica (RC = 2,10; 95% IC:1,17-3,74; p = 0,01) permaneceram associados com os MACE. CONCLUSÃO: Em nosso estudo, a CA alterada não se associou à maior incidência de MACE em 30 dias de seguimento.

BACKGROUND: Abdominal circumference (WC) is the measure that correlates most closely with the risk factors and death from cardiovascular disease. However, the impact of obesity on the prognosis of patients with cardiovascular disease remains controversial and requires further clarification. OBJECTIVE: To evaluate WA as a predictor of 30-day outcome in patients who were hospitalized with acute coronary syndrome (ACS), in a referral hospital for the treatment of cardiovascular diseases. METHODS: Contemporary cohort 267 patients who were hospitalized for ACS and who were followed for 30 days after discharge, taking into account the major cardiovascular events - MACE - (death, reinfarction, rehospitalization for coronary artery bypass grafting procedures). In the first 24 hours of admission, patients answered a questionnaire and were subsequently measured for WC. The statistical analysis was performed with SPSS 17.0, using the chi-square test for categorical variables and Student t test for numerical variables, with significance level of p < 0.05. The variables that had p < 0.10 in the bivariate analysis were included in a logistic regression model to evaluate the WC role as an independent predictor of MACE. RESULTS: After multivariate analysis, only the female gender (OR = 8.86; 95% CI: 4.55-17. 10, p < 0.00), hypertension (OR = 2.06; 95% CI: 1.10-3.87; p = 0.02) and family history of ischemic heart disease (OR = 2.10; 95% CI: 1.17-3.74; p = 0.01) remained associated with the MACE. CONCLUSION: In our study, the modified AC was not associated with increased incidence of MACE over the 30 days of follow-up.

FUNDAMENTO: La circunferencia abdominal (CA) es la medición que se correlaciona con los factores de riesgo y la muerte por enfermedad cardiovascular. Sin embargo, el impacto de la obesidad en el pronóstico de los pacientes con enfermedades cardiovasculares sigue siendo controvertido y requiere una mayor clarificación. OBJETIVO: Evaluar la CA como un predictor de evolución en 30 días en pacientes que fueron hospitalizados con síndrome coronario agudo (SCA), en un hospital de referencia para el tratamiento de las enfermedades cardiovasculares. MÉTODOS: Cohorte contemporánea con 267 pacientes que fueron hospitalizados por SCA y que fueron seguidos durante 30 días después del alta, teniendo en cuenta los eventos cardiovasculares mayores - MACE - (muerte, reinfarto, rehospitalización por procedimientos de revascularización). En las primeras 24 horas del ingreso, los pacientes respondieron a un cuestionario y se midió posteriormente la CA. El análisis estadístico se realizó con SPSS 17.0, mediante la prueba de Chi-cuadrado para variables categóricas y prueba t de Student para las variables numéricas, con un nivel de significación de p < 0,05. Las variables que presentaron valores de p < 0,10, en el análisis bivariado, se incluyeron en un modelo de regresión logística para evaluar el papel de la CA como un predictor independiente de MACE. RESULTADOS: Tras el análisis multivariante, solamente el sexo femenino (RC = 8,86; 95% IC:4,55-17,10; p < 0,00), hipertensión arterial sistémica (RC = 2,06; 95% IC:1,10-3,87; p = 0,02) e historia familiar de cardiopatía isquémica (RC = 2,10; 95% IC:1,17-3,74; p = 0,01) permanecieron asociados con los MACE. CONTEXTO: As doenças inflamatórias intestinais são doenças crônicas e, com isso, podem exercer impacto sobre a qualidade de vida do indivíduo. OBJETIVO: Avaliar a qualidade de vida de adultos com doenças inflamatórias intestinais, através do Inflammatory Bowel Disease Questionnaire -validado para a língua portuguesa, e comparar esta resposta entre pacientes com a doença em atividade ou não. MÉTODOS: Foram entrevistados 50 indivíduos de ambos os sexos, com idade entre 18 e 60 anos. RESULTADOS: A média de idade da amostra foi de 42,2 + 13,6 anos, o tempo da doença foi de 98,8 + 74,3 meses e 72% destes indivíduos tinham doença de Crohn. Sete indivíduos relataram estar no momento da atividade da doença, mas não houve diferença significativa entre o tempo de doença e o estado nutricional quando comparados os pacientes com atividade e em remissão da doença. Não foi verificada diferença significativa na pontuação do questionário dos pacientes com as diferentes doenças inflamatórias intestinais quando estes estão na fase remissiva (172 + 42,4 e 173,6 + 28,2 para retocolite ulcerativa e doença de Crohn, respectivamente, P = 0,886). Quando comparada a pontuação dos pacientes que estavam em crise, verificou-se que esses apresentam menor qualidade de vida do que os pacientes em remissão (123,8 + 44,5 e 173,3 + 31,5 para os pacientes em crise e em remissão, respectivamente, P = 0,001). CONCLUSÃO: Foi possível verificar que a qualidade de vida dos pacientes com doenças inflamatórias intestinais não difere quando diagnosticadas nas suas duas classificações, quando os pacientes estão na remissão. O aspecto principal para determinação de um prejuízo na qualidade de vida seria estar na fase da atividade da doença.

CONTEXT: Chronic diseases have an impact on the quality of life of the individuals. OBJECTIVE: To evaluate the quality of life of adults with inflammatory bowel disease by the inflammatory bowel disease questionnaire certified for Portuguese language. METHODS: We interviewed 50 individuals from both genders aged from 18 to 60 years old, with Crohn's disease and ulcerative colitis, regardless of disease activity. RESULTS: The average age of the sample was 42.2 ± 13.6 years old, the disease length was 98.8 ± 74.3 months, and 72% of patients had Crohn's disease and 14% presented disease activity. There was no significant difference in questionnaire scores of patients with different inflammatory bowel disease when they are in the remission phase (172.0 ± 42.4 and 173.6 ± 28.2 for ulcerative colitis and Crohn's disease, respectively, P = 0.886). When compared to scores of patients who were in crisis, it was found that they have a lower quality of life that patients in remission (123.8 ± 44.5 and 173.3 ± 31.5 for patients in crisis and remission, respectively, P = 0.001). CONCLUSION: It was noticed that the quality of life of patients with inflammatory bowel disease did not differ among patients with Crohn's disease or ulcerative colitis, when patients are in remission. The main aspect which determines the loss of quality of life would be being at the stage of disease activity.

O objetivo deste estudo foi avaliar o efeito do consumo agudo de cafeína sobre a oxidação de lipídeos e desempenho durante o exercício aeróbico. Foram avaliados 15 indivíduos do sexo masculino, com idade média de 22,3 ± 2,7 anos, que realizaram teste de cargas progressivas em esteira rolante para determinação do consumo máximo de oxigênio (VO2máx) e limiares ventilatórios (LV). Cada voluntário realizou três testes submáximos na intensidade de 10% abaixo do segundo LV, sendo orientados a permanecer em exercício até a exaustão. Trinta minutos antes de cada teste submáximo, foram ingeridos 250ml de uma das bebidas compostas por: café com adoçante (CAD), café com açúcar (CA) e café descafeinado com adoçante (CD). Durante o exercício, os indivíduos foram monitorados pelo ergoespirômetro e frequencímetro. A oxidação de lipídeos foi predita pelo quociente respiratório (QR) durante o teste, e o desempenho foi verificado pelo tempo de exercício. Para comparar os resultados de QR e tempo de exercício entre os grupos, foi utilizado Anova fatorial, e considerou-se significância estatística um valor de p < 0,05. A média de VO2máx foi de 50,18 ± 9,9ml/kg/min. Com a ingestão de CAD, a média do QR foi de 0,98 ± 0,18 e o tempo médio em exercício foi de 24,1 ± 17,04 minutos. Com a ingestão de CA, a média do QR foi de 0,96 ± 0,2 e o tempo médio em exercício foi de 24,4 ± 17,8min. No teste com ingestão de CD, a média do QR foi de 1,01 ± 0,24 e a média do tempo em exercício foi de 20,6 ± 9,7min. Não houve diferença significativa entre os testes nos valores do QR e nem tempo de exercício (p = 0,697 e p = 0,598, respectivamente). A cafeína não aumentou a oxidação de lipídeos nem o desempenho de indivíduos jovens do sexo masculino.

The purpose of this study was to evaluate the effect of acute caffeine consumption on lipid oxidation and performance during aerobic exercise. Fifteen healthy male individuals, 22.3 ± 2.7 years old, performed a progressive test on treadmill for determination of maximal oxygen uptake (VO2max) and ventilatory thresholds. Each volunteer performed three submaximal tests at the intensity of 10% below the second ventilatory threshold, being guided to remain on exercise until exhaustion. Thirty minutes before each submaximal test, the subjects ingested 250ml of one of following drinks: coffee with sweetener (CSW), coffee with sugar (CS) or decaffeinated coffee with sweetener (CD). During the exercise, the individuals's heart rate was monitored and respiratory gases analyses were done. The lipid oxidation was predicted by the respiratory quotient (RQ) during the test and performance was verified by exercise duration. In order to compare the RQ results and time of exercise among the three groups, factorial Anova was used, and a value of p < 0.05 was considered as statistically significant. The individuals had VO2max of 50.18 ± 9.9 ml/kg/min. CAD ingestion caused RQ average of 0.98 ± 0.18, and the average exercise duration was of 24.1 ± 17.04 min; CA ingestion caused RQ average of 0.96 ± 0.2 and the average exercise duration was 24.4 ± 17.8 min. Finally, CD ingestion caused the RQ average of 1.01 ± 0.24, and the average exercise duration was of 20.6 ± 9.7 min. There were no significant differences in the RQ values or exercise duration among the three interventions (p = 0.697 and p = 0.598, respectively). Caffeine did not increase lipid oxidation or performance of young male individuals.
