Introduction: No study so far has tested a beverage containing glutamine 2 h before anesthesia in patients undergoing surgery. Objectives: The aim of the study was to investigate: 1) the safety of the abbreviation of preoperative fasting to 2 h with a carbohydrate-L-glutamine-rich drink; and 2) the residual gastric volume (RGV) measured after the induction of anesthesia for laparoscopic cholecystectomies. Methods: Randomized controlled trial with 56 women (42 (17-65) years-old) submitted to elective laparoscopic cholecystectomy. Patients were randomized to receive either conventional preoperative fasting of 8 hours (fasted group, n = 12) or one of three different beverages drunk in the evening before surgery (400 mL) and 2 hours before the initiation of anesthesia (200 mL). The beverages were water (placebo group, n = 12), 12.5% (240 mOsm/L) maltodextrine (carbohydrate group, n = 12) or the latter in addition to 50 g (40 g in the evening drink and 10g in the morning drink) of L-glutamine (glutamine group, n = 14). A 20 F nasogastric tube was inserted immediately after the induction of general anesthesia to aspirate and measure the RGV. Results: Fifty patients completed the study. None of the patients had either regurgitation during the induction of anesthesia or postoperative complications. The median (range) of RGV was 6 (0-80) mL. The RGV was similar (p = 0.29) between glutamine group (4.5 [0-15] mL), carbohydrate group (7.0 [0-80] mL), placebo group (8.5 [0-50] mL), and fasted group (5.0 [0-50] mL). Conclusion: The abbreviation of preoperative fasting to 2 h with carbohydrate and L-glutamine is safe and does not increase the RGV during induction of anesthesia.

Introducción: Ningún estudio hasta el momento ha investigado una bebida que contiene glutamina 2 h antes de la anestesia en pacientes sometidos a cirugía. Objetivos: El objetivo del estudio fue investigar: 1) la seguridad de la abreviación del ayuno preoperatorio para 2 h con una bebida conteniendo carbohidratos y L-glutamina, y 2) el volumen gástrico residual (RGV), medido después de la inducción de la anestesia en colecistectomías laparoscópicas. Métodos: Ensayo controlado aleatorizado con 56 mujeres (42 (17-65) años) sometidas a colecistectomía laparoscópica electiva. Las pacientes fueron aleatorizadas para recibir ayuno preoperatorio convencional de 8 horas (grupo ayuno, n = 12) o una de tres bebidas diferentes tomadas la noche antes de la cirugía (400 ml) y también 2 horas antes del inicio de la anestesia (200 ml). Las bebidas eran agua (grupo placebo n = 12), 12,5% (240 mOsm/l) maltodextrina (grupo carbohidrato, n = 12) o carbohidrato además de 50 g (40 g la noche anterior y 10 g por la mañana) de L-glutamina (grupo glutamina, n = 14). Una sonda nasogástrica 20 F fue insertada inmediatamente después de la inducción de la anestesia general para aspirar y medir el RGV. Resultados: Cincuenta pacientes completaron el estudio. Ninguno de los pacientes han presentado regurgitación durante la inducción de la anestesia ni complicaciones postoperatorias. La mediana (variación) del RGV fue de 6 (00-80) mL. El RGV fue similar (p = 0,29) entre el grupo glutamina (4,5 [0-15] mL), el grupo carbohidrato (7,0 [0,80] mL), grupo placebo (8,5 [0-50] mL), y grupo ayuno (5,0 [0-50] mL). Conclusión: La abreviación del ayuno preoperatorio para 2 h con carbohidratos y L-glutamina es seguro y no aumenta el RGV durante la inducción de la anestesia.

RESUMO Micoinseticidas e micoacaricidas podem ser definidos como produtos à base de propágulos vivos de fungos entomopatogênicos visando o controle de insetos e ácaros por meio de aplicações inundativas ou inoculativas. A partir de dados recentemente publicados sobre o uso de fungos entomopatogênicos em escala global e proposta para padronização dos diversos tipos de formulações, o presente trabalho tem dois objetivos principais: 1) analisar o “estado da arte” dos micopesticidas (micoinseticidas e micoacaricidas) brasileiros em relação aos produtos já desenvolvidos em escala global e, 2) enquadrar os produtos nacionais à base de fungos entomopatogênicos nesta nova classificação. No ano de 2007, 40 produtos encontravam-se disponíveis no mercado brasileiro e, aproximadamente, 19 empresas em funcionamento. A maioria dos micopesticidas brasileiros não tem registro; 2,5% dos produtos são comercializados na forma de material técnico (conídios puros), 72,5% correspondem aos concentrados técnicos (substratos líquidos ou sólidos colonizados por fungos) e apenas 25% são, de fato, formulações, todas elas do tipo dispersão oleosa. Situação oposta ocorre nos países industrializados, onde se sobressaem os produtos formulados e registrados. No Brasil, a eficiência de controle alcançada com micopesticidas nem sempre tem correspondido às expectativas dos usuários, e a inconsistência dos resultados de campo tem dificultado a maior adoção de tais produtos. No Brasil, embora os avanços tecnológicos após 40 anos de pesquisa tenham sido menores que o esperado, há expectativa de crescente adoção dos fungos entomopatogênicos em razão de nichos de mercado emergentes.

ABSTRACT Mycoinsecticides and mycoacaricides can be defined as biopesticide products based on living propagules of entomopathogenic fungi developed for inundative and inoculative biological control of insects and mites. Based on recently published data on global use of entomopathogenic fungi and a proposal for standardizing the definitions of mycopesticide formulations, the present work had two main objectives: 1) to analyze the state of the art of Brazilian mycopesticides (mycoinsecticides and mycoacaricides) compared to products already developed worldwide and, 2) fit Brazilian products based on entomopathogenic fungi into this new classification. In 2007, 40 products were available in the Brazilian market and, approximately, 19 companies under operation. Most Brazilian mycopesticides are not registered; 2.5% of the products are commercialized as technical material (conidial powders), 72.5% correspond to technical concentrates (liquid or solid fungus-colonized substrates) and only 25% are, indeed, formulations, all of them oil dispersions. The opposite situation takes place in industrialized countries, in which registered and formulated products are more common. In Brazil, usually the control efficiency delivered by mycopesticides has not been able to meet user’s expectations on a regular basis, and inconsistency in field trials from site to site and year to year has imposed limitations for a broader adoption of mycopesticides. In Brazil, although technological advances following 40 years of research were lower than expected, there is an expectation of increasing demand for entomopathogenic fungi due to emergent market niches.

Cytochrome P450 (CYP) is a superfamily of enzymes involved in the metabolism of endogenous compounds and xenobiotics. CYP2A6 catalyzes the oxidation of nicotine and the activation of carcinogens such as aflatoxin B1 and nitrosamines. CYP2E1 metabolizes ethanol and other low-molecular weight compounds and can also activate nitrosamines. The CYP2A6 and CYP2E1 genes are polymorphic, altering their catalytic activities and susceptibility to cancer and other diseases. A number of polymorphisms described are ethnic-dependent. In the present study, we determined the genotype and allele frequencies of the main CYP2A6 and CYP2E1 polymorphisms in a group of 289 volunteers recruited at the Central Laboratory of Hospital Universitário Pedro Ernesto. They had been residing in the city of Rio de Janeiro for at least 6 months and were divided into two groups according to skin color (white and non-white). The alleles were determined by allele specific PCR (CYP2A6) or by PCR-RFLP (CYP2E1). The frequencies of the CYP2A6*1B and CYP2A6*2 alleles were 0.29 and 0.02 for white individuals and 0.24 and 0.01 for non-white individuals, respectively. The CYP2A6*5 allele was not found in the population studied. Regarding the CYP2E1*5B allele, we found a frequency of 0.07 in white individuals, which was statistically different (P < 0.05) from that present in non-white individuals (0.03). CYP2E1*6 allele frequency was the same (0.08) in both groups. The frequencies of CYP2A6*1B, CYP2A6*2 and CYP2E1*6 alleles in Brazilians are similar to those found in Caucasians and African-Americans, but the frequency of the CYP2E1*5B allele is higher in Brazilians.

The aim of the present study was to demonstrate the presence of alpha-L-fucosidase in Trypanosoma cruzi. Immunocytochemical and biochemical techniques were used to localize and characterize a membrane-associated, neutral-pH-optimum, alpha-L-fucosidase from the parasite. Light and electron microscopy localized the alpha-L-fucosidase specifically on the surface of the parasite and on membranes in the posterior region of the epimastigote stage. Although much less intense, labeling was also detected on the surface of trypomastigotes. At least 50% of the alpha-L-fucosidase activity was associated with epimastigote membrane solubilized with 1 M NaCl or 1% Triton X-100, suggesting that alpha-L-fucosidase is peripherally associated with membranes. The enzyme from epimastigotes had a neutral pH optimum (near 7) but displayed low specific activity when p-nitrophenyl-alpha-L-fucoside was employed as substrate (0.028 U/mg protein for epimastigotes and 0.015 U/mg protein for tissue culture-derived trypomastigotes). Polyacrylamide gel electrophoresis and Western blotting analysis both showed an expected 50-kDa polypeptide which was immunoreactive with anti-alpha-L-fucosidase antibodies.
