This study assessed the effect of antiretroviral drugs administered to pregnant women on amylase and liver enzymes of the neonate. A prospective study was conducted on 52 neonates divided into three groups: infants born to HIV-infected mothers taking zidovudine (ZDV group, n = 18), infants born to mothers taking zidovudine + lamivudine + nelfinavir (TT group, n = 22) and infants born to normal women (control group, n = 12). Umbilical cord blood from the newborn infant was used to determine liver transaminases and amylase. Data were analyzed statistically by nonparametric tests, with the level of significance set at p<0.05. The median levels for TT group newborns were 33.3 U/L for oxaloacetic transaminase, 21.5 U/L for pyruvic transaminase, 1.9 mg/dL for total bilirubin, 153 mg/dL for alkaline phosphatase, and 9.6 U/L for amylase. These results did not differ from those obtained for Control newborns or newborns exposed to ZDV alone. No association was observed between the use of antiretroviral drugs during pregnancy and adverse effects on neonatal amylase and hepatic parameters at birth.

OBJETIVO: quantificar os valores do líquido amniótico no primeiro trimestre da gestação, em fetos normais, por meio de ultra-sonografia tridimensional e bidimensional. MÉTODOS: foram avaliados 25 fetos normais, da 8ª à 11ª semana de gestação. O estudo foi do tipo prospectivo longitudinal. As medidas do volume de líquido amniótico foram obtidas por meio de ultra-sonografia endovaginal, modos tridimensional e bidimensional. O estudo bidimensional consistiu em determinar o volume por cálculo matemático com base na forma de um elipsóide (constante 0,52), obtendo-se o volume do saco amniótico e do embrião. No estudo tridimensional, o volume do líquido amniótico foi feito pela técnica VOCAL, utilizando os graus de rotação 6, 9, 15 e 30°. Foi considerado como resultado final o volume do líquido amniótico obtido pelo grau de rotação6. Em ambos, o volume de líquido amniótico foi obtido pela subtração do volume do saco amniótico pela medida volumétrica do embrião. Para análise estatística utilizamos análise de variância (ANOVA), correlação e análise de regressão. O nível de significância adotado foi p < 0,05. RESULTADOS: a evolução no volume de líquido amniótico pela ultra-sonografia bidimensional foi de 5,4 para 39,5 cm³ da 8ª para a 11ª semana (ANOVA - p < 0,05). Observamos correlação entre idade gestacional e volume de líquido amniótico (p < 0,001, r² = 88,3%). No estudo tridimensional o volume de líquido amniótico aumentou de 5,7 para 42,9 cm³ da 8ª para a 11ª semana (ANOVA - p < 0,05). Também observamos correlação entre idade gestacional e volume de líquido amniótico (p < 0,001, r² = 98,1%). CONCLUSÃO: há aumento no volume de líquido amniótico no primeiro trimestre da gestação, quando avaliado nos modos bidimensional e tridimensional. Além disso, demonstramos que, quanto maior a idade gestacional, maior é o volume de líquido amniótico.

PURPOSE: To determine the values of amniotic fluid in normal fetuses during the first trimester of pregnancy by three- and bi-dimensional ultrasonography. METHODS: In a prospective longitudinal study, 25 normal fetuses were evaluated from the 8th to the 11th week of gestation. Amniotic fluid volume was measured by endovaginal ultrasonography with the three- and two-dimensional modes. The two-dimensional study consisted of volumetric determination by mathematical calculation based on an ellipsoidal shape (constant 0.52) to obtain the amniotic sac and embryo volumes. In the three-dimensional study, the amniotic fluid volume was determined by the VOCAL technique using 6, 9, 15, and 30 degrees of rotation. The amniotic fluid volume obtained by 6-degree rotations was considered to be the final result. In both modes, amniotic fluid volume was obtained by subtracting the volume of the embryo from the volume of the amniotic sac. Data were analyzed statistically for variance (ANOVA), correlation and regression analysis. The level of significance was set at p < 0.05. RESULTS: The amniotic fluid volume as measured by two-dimensional ultrasonography increased from 5.45 to 39.52 cm³ in the range from the 8th to the 11th week (ANOVA - p < 0.05). There was a correlation between gestational age and amniotic fluid volume (p < 0.001, r² = 88.3%). In the three-dimensional study, the amniotic fluid volume increased from 5.7 to 42.9 cm³ in the range from the 8th to the 11th week (ANOVA - p < 0.05), and again a correlation between gestational age and amniotic fluid volume (p < 0.001, r² = 98.1%) was observed. CONCLUSION: an increase in amniotic fluid volume occurs during the first trimester of pregnancy, as determined by the two- and three-dimensional modes. In addition, we have demonstrated that the higher the gestational age, the larger the amniotic fluid volume.

We reviewed colonization by group B Streptococcus beta-haemolyticus of Lancefield (SGB), or Streptococcus agalactiae, in pregnant women, and the consequences of infection for the mother and newborn infant, including factors that influence the risk for anogenital colonization by SGB. We also examined the methods for diagnosis and prophylaxis of SGB to prevent early-onset invasive neonatal bacterial disease. At present, it is justifiable to adopt anal and vaginal SGB culture as part of differentiated obstetrical care in order to reduce early neonatal infection. The rates, risk factors of maternal and neonatal SGB colonization, as well as the incidence of neonatal disease, may vary in different communities and need to be thoroughly evaluated in each country to allow the most appropriate preventive strategy to be selected.

OBJETIVO: avaliar a velocidade sistólica máxima (VSM) e o índice de resistência (IR) nas artérias fetais cerebral média (ACM), aorta supra-renal (ASR), aorta infra-renal (AIR) e artéria umbilical (AU), entre a 22ª e a 38ª semana de gestação. MÉTODOS: estudo prospectivo no qual foram avaliados os parâmetros de 33 fetos normais na 22ª, 26ª, 30ª, 34ª e 38ª semana de gestação. Foram incluídas gestações únicas, sem doenças e complicações e as que concordaram em participar do estudo. Os critérios de exclusão foram malformações fetais, descontinuidade do seguimento aos exames e mães usuárias de fumo, álcool e drogas ilícitas. Os exames ultra-sonográficos foram realizados por único observador. Para a aquisição do traçado dopplervelocimétrico na ACM, ASR, AIR e AU, o volume de amostra foi de 1 a 2 mm, colocado no centro das artérias. O ângulo de insonação foi de 5º a 19º, na ACM, inferior a 45º na ASR e AIR e inferior a 60ºna AU. Utilizamos filtro de parede de 50-100 Hz. O cálculo dos parâmetros foi realizado automaticamente, com a imagem congelada, tendo sido acionadas três medidas. O resultado final foi obtido pela média aritmética dos três valores. A análise estatística foi realizada pela análise de variância (ANOVA), teste post hoc de Bonferroni, coeficiente de correlação de Pearson e análise de regressão. O nível de significância foi p<0,05 em todas as análises. RESULTADOS: a VSM aumentou de 26,3 para 57,7 cm/s na ACM, entre a 22ª e a 38ª semana de gestação (p<0,05). Na ASR e na AIR, a VSM aumentou, entre a 22ª e a 34ª semana de gestação, de 74,6 e 59,0 cm/s para 106,0 e 86,6 cm/s, respectivamente (p<0,05). Na AU, a VSM aumentou entre a 22ª e a 34ª semana de gestação, porém diminuiu de 55,5 para 46,2 cm/s entre a 34ª e a 38ª semana de gestação. O IR, na ACM, foi menor na 22ª (0,81) e 38ª semana de gestação (0,75) e maior (0,85) na 26ª semana (p<0,05). Na ASR, os valores do IR foram estáveis em todas as semanas e na maioria delas na AIR (p>0,05). Na AU, o IR diminuiu de 0,69 para 0,56, entre a 22ª e a 38ªsemana gestacional (p<0,05). CONCLUSÃO: em fetos normais, na segunda metade da gestação, a VSM aumenta na ACM, ASR e AIR, diminuindo na AU entre a 34ª e a 38ª semana de gestação. O IR é menor na 22ª e 38ª semana de gestação na ACM, diminui entre a 22ª e a 38ª semana na AU e é constante na maioria das semanas gestacionais na ASR e AIR.

PURPOSE: to assess peak systolic velocity (PSV) and the resistance index (RI) in the middle cerebral artery (MCA), suprarenal aorta (SRA) and infrarenal aorta (IRA) of the fetus and in the umbilical artery (UA) between the 22nd and 38th week of gestation. METHODS: a prospective study which evaluated the parameters of 33 normal fetuses in the 22nd, 26th, 30th, and 38th week of gestation. Pregnant women with a singleton fetus with no diseases or complications and who agreed to participate were included in the study. Exclusion criteria were fetal malformations, discontinuation of prenatal care visits and mothers who smoked, used alcohol or illicit drugs. Ultrasound examinations were performed by a single observer. For the acquisition of the Doppler velocimetry tracing in the MCA, SRA, IRA and UA, the sample volume was 1 to 2 mm, placed in the center of the arteries. The insonation angle was 5º to 19º in the MCA, below 45º in the SRA and IRA, and less than 60º in the UA. We used a wall filter of 50-100 Hz. The parameters were calculated automatically with the frozen image, three measurements being made. The final result was obtained by the arithmetic mean of the three values. Data were analyzed by analysis of variance (ANOVA), post hoc Bonferroni test, Pearson's correlation, and regression analysis. The level of significance was set at p<0.05 in all analyses. RESULTS: PSV increased from 26.3 to 57.7 cm/s in the MCA between the 22nd and the 38th week of gestation (p<0.05). In the SRA and in the IRA, PSV increased between the 22nd and 34th week of gestation, from 74.6 and 59.0 cm/s to 106.0 and 86.6 cm/s, respectively (p<0.05). In the UA, PSV increased between the 22nd and the 34th week of gestation, but decreased from 55.5 to 46.2 cm/s between the 34th and the 38th week of gestation. In the MCA, the RI was lower in the 22nd (0.81) and 38th week of gestation (0.75) and higher (0.85) in the 26th week (p<0.05). In the SRA, the RI values were stable in all weeks and in the IRA they were stable in most weeks (p>0.05). In the UA, RI decreased from 0.69 to 0.56 between the 22nd and 38th week of gestation (p<0.05). CONCLUSION: in normal fetuses, in the second half of gestation PSV increased in the MCA, SRA and IRA, decreasing in the UA between the 34th and 38th week of gestation. RI was lower in the 22nd and 38th weeks of gestation in the MCA, decreased between the 22nd and the 38th week in the UA, and was constant in most of the gestational weeks in the SRA and IRA.

OBJETIVOS: Estudar o efeito das drogas anti-retrovirais sobre a quantificação dos linfócitos TCD4 e RNA do HIV-1 de gestantes portadoras do HIV-1 e parâmetros antropométricos de seus neonatos. MÉTODOS: Estudo prospectivo avaliando 57 gestantes e seus neonatos em três grupos: Grupo AZT, gestantes portadoras do HIV utilizando zidovudina (n=20); Grupo TT, mães utilizando zidovudina+lamivudina+nelfinavir (n=25), e Grupo Controle, mulheres saudáveis (n=12). A quantificação dos linfócitos TCD4 e RNA do HIV-1 de gestantes portadoras do HIV foi analisada em dois períodos durante a gestação. O prognóstico perinatal levou em consideração as taxas de pré-termos, restrição de crescimento intra-útero, mortalidade perinatal e transmissão vertical do HIV-1. Os dados foram analisados utilizando-se testes não paramétricos de qui-quadrado, Mann-Whitney, Friedman, Kruskal-Wallys e Wilcoxon para amostras pareadas, considerando-se significativos valores associados a p<0,05. RESULTADOS: Observou-se homogeneidade entre os dados demográficos e antropométricos de realce. A carga viral, inicialmente elevada (14.370 cópias/ml), reduziu-se significativamente no grupo com tratamento tríplice , chegando a 40 cópias/ml. Quanto à contagem de linfócitos CD4, observou-se recuperação significativa nas pacientes do grupo TT, no final da gestação, sendo esse valor significativamente diferente em comparação ao grupo AZT (p = 0,0052). Não se observou diferença entre os grupos quanto à duração da gestação, aos índices de Apgar, e à classificação antropométrica neonatal. Não houve nenhum caso de transmissão vertical do HIV-1. CONCLUSÕES: Os resultados obtidos na presente casuística demonstram eficiência e sugerem segurança no uso de anti-retrovirais na gestação sobre parâmetros antropométricos dos neonatos.

PURPOSE: To study the effect of antiretroviral drugs administered during pregnancy on CD4 lymphocyte counts and HIV-1 RNA levels of pregnant women and on the anthropometric parameters of their neonates. METHODS: A prospective study was conducted on 57 pregnant women and their neonates divided into 3 groups: ZDV Group, HIV-infected mothers taking zidovudine (n = 20); triple therapy (TT) Group, mothers taking zidovudine + lamivudine + nelfinavir (n = 25), and Control Group, normal women (n = 12). CD4 lymphocyte counts and HIV-1 RNA levels of pregnant women were analyzed during two periods of pregnancy. The perinatal prognosis took into account preterm rates, birth weight, intrauterine growth restriction, perinatal death, and vertical transmission of HIV-1. Data were analyzed statistically using the nonparametric chi-square, Mann-Whitney, Friedman, Kruskal-Wallis, and Wilcoxon matched pairs tests, with the level of significance set at P <.05. RESULTS: The major maternal demographic and anthropometric data were homogeneous for the various groups. HIV-1 viral burden, which was initially elevated, median of 14,370 copies/mL, was significantly reduced in the TT group, reaching 40 copies/mL. With respect to T-CD4+ lymphocyte counts, there was a significant recovery in Group TT at the end of pregnancy, this value being significantly different from that for the ZDV group (P =.0052). There was no difference between groups regarding gestation length, Apgar scores, or neonatal anthropometric classification. There was no case of vertical HIV-1 transmission. CONCLUSIONS: The results obtained for the present series demonstrate the efficiency and suggest safety of the use of antiretroviral drugs during pregnancy as revealed by anthropometric parameters of the neonate.

Leishmaniose visceral (LV) é doença endêmica no Estado de Mato Grosso do Sul, Brasil, principalmente nas áreas próximas dos rios Paraguai e Paraná. Nos últimos anos vem aumentando o número de casos, especialmente na Capital, Campo Grande, com conseqüente surgimento de casos em gestantes e conseqüentemente havendo risco de transmissão vertical do parasita. No presente trabalho é relatado um caso de LV em gestante, acompanhada por nosso grupo e tratada com anfotericina B lipossomal, não tendo ocorrido a transmissão vertical do parasita. O presente relato demonstra opção terapêutica em casos de calazar durante a gestação, tendo em vista a contra-indicação relativa do uso do antimoniato, droga de primeira escolha para tratamento em pacientes não-gestantes.

Visceral leishmaniasis (VL) is an endemic disease in the state of Mato Grosso do Sul, Brazil, mainly in the areas near the Paraguay and Paraná rivers. An increasing number of cases have been occurring especially in the state capital Campo Grande, with consequent occurrence of VL cases in pregnant women. This situation causes an elevated risk of vertical transmission of the parasite. In this report, we describe a case of VL in a pregnant woman followed up by our group, who was treated with liposomal amphotericin B, with no vertical transmission of the parasite. In our report, we demonstrate a therapeutic option for kala-azar during pregnancy, since antimoniate, the first-choice drug, is not prescribed during the gestational period.

Serum vitamin A during pregnancy and effects on obstetrics and perinatal outcomes in hiv infected pregnant women. To evaluate serum vitamin levels and its association with obstetrics and perinatal results in HIV infected pregnant women. Observational and prospective study carried out at Division of Infectious-Contagious Diseases in Gynecology and Obstetrics of the University Hospital, Medicine School of Ribeirão Preto, University of São Paulo, involving 57 pregnant women divided into 3 groups: Group 1, with 12 normal pregnant women, it was the control group; Group 2, with 20 HIV infected pregnant women, using ZDV; and Group 3, with 25 HIV infected pregnant women, using therapy I contend ZDV, 3TC and nelfinavir. The evaluation of the serum vitamin level was obtained three times during pregnancy at equidistant time intervals and in the immediate period after birth. We also evaluated the levels of this vitamin and the hemoglobin in the blood of the umbilical cord. We obtained maternal and newborn infant anthropometric data, as well as the counting of lymphocyte TCD4 and viral load of the HIV during the pregnancy. Reduced serum vitamin levels were observed in the Group 1(25%), the Group 2(29,4%) and the Group 3(28,6%). Association was not observed between serum levels of maternal retinol and the duration of the gestation in groups 2 and 3. In groups 1 and 3, an association was observed between the maternal concentration of retinol and the newborn hemoglobin (p=0,05). In distinct way to the Control group, association was not observed between the retinol levels of the umbilical cord and the weight of the newborn in gestations of Group 2, while a trend to this association was observed in gestations of Group 3 (p=0,06). We observed high prevalence of hipovitaminosis A in the population of this study, regardless of antiretroviral scheme used.

Avaliar os níveis plasmáticos de vitamina A e a sua associação com os resultados obstétricos e perinatais em gestantes portadoras do HIV. Estudo observacional e prospectivo realizado no Setor de Doenças Infecto-Contagiosas em Ginecologia e Obstetrícia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto, Universidade de São Paulo, envolvendo 57 gestantes divididas em 3 grupos: Grupo 1, com 12 gestantes normais, foi o grupo controle; Grupo 2, com 20 gestantes portadoras do HIV, utilizando AZT; e Grupo 3, com 25 gestantes portadoras do HIV, usando terapia combinada contendo AZT, 3TC e nelfinavir. A avaliação do nível plasmático de vitamina A foi realizada em três períodos equidistantes durante a gestação e no puerpério imediato. Avaliou-se também os níveis dessa vitamina e da hemoglobina no sangue do cordão umbilical. Foram aferidos dados antropométricos maternos, neonatais, assim como a contagem de linfócitos TCD4 e carga viral do HIV durante a gestação. Níveis plasmáticos reduzidos de vitamina A foi observada no grupo 1(25%), no grupo 2(29,4%) e no grupo 3(28,6%). Não se observou associação entre os níveis plasmáticos de retinol materno e a duração da gestação em gestantes do grupo 2 e 3. Nos grupos 1 e 3 observou-se associação entre a concentração materna do retinol e a hemoglobina do RN (p=0,05). De maneira distinta ao grupo controle, não se observou associação entre os níveis de retinol do cordão umbilical e o peso do recém-nascido em gestações do Grupo 2, enquanto uma tendência a essa associação foi observada em gestações do Grupo 3 (p=0,06). Verificou-se alta prevalência de hipovitaminose A na população deste estudo, independente do esquema anti-retroviral utilizado.

OBJETIVO: avaliar a ação de drogas anti-retrovirais sobre o metabolismo glicídico e sobre o pâncreas de ratas Wistar prenhes. MÉTODOS: estudo com ratas prenhes adultas da raça Wistar, pesando entre 200-230 g. Foram testadas a azidotimidina, lamivudina e o nelfinavir, em doses 10 vezes superiores à dose utilizada em gestantes. Foram avaliados sete grupos, contendo 10 ratas por grupo, incluindo o controle. O sacrifício foi realizado no 21º dia de prenhez. Procederam-se a dosagens de glicemia, insulina, glucagon, ácidos graxos livres (AGL) e glicogênio hepático. Para avaliação de lesão pancreática, optou-se pela contagem direta do número de células produtoras de insulina e glucagon marcadas por imuno-histoquímica. Os dados foram analisados pelo teste t de Student, sendo comparados os animais dos grupos controle e tratados. RESULTADOS: após 21 dias de prenhez houve elevação dos níveis séricos de glucagon (grupo controle: 88,2 pg/ml; grupos tratados: 99,7 a 120,7 pg/ml ) e redução dos níveis de insulina (grupo controle: 6,2 miUI/ml; grupos tratados: 2,1 a 2,7 miUI/ml) em todos os grupos tratados com anti-retrovirais. Não houve diferenças significativas nos valores plasmáticos de glicemia, AGL e valores de glicogênio hepático ao final dos 21 dias de prenhez. Não houve diferença quanto ao número de células pancreáticas produtoras de insulina e glucagon entre os grupos tratados e o grupo controle ao final dos 21 dias de prenhez. CONCLUSÕES: os fármacos anti-retrovirais utilizados durante a prenhez de ratas não infectadas alteram o metabolismo glicídico materno em grau leve causando queda de insulina e elevação do glucagon, com índices glicêmicos normais e número de células pancreáticas inalterado.

OBJECTIVE: to assess the action of antiretroviral drugs on glycid metabolism and on the pancreas of pregnant Wistar rats. METHODS: adult pregnant Wistar rats weighing 200-230g were used. Azidothymidine, lamivudine and nelfinavir were administered to the animals at doses 10 times higher than those administered to pregnant women. The animals were divided into seven groups of 10 animals, including a control group. The animals were sacrificed on the 21st day of pregnancy and glycemia, insulinemia, glucagonemia, free fatty acids (FFA) and hepatic glycogen were measured. Direct counts of the number of immunohistochemically labeled insulin- and glucagon-producing cells were used to determine pancreatic damage. Data were analyzed statistically by the Student's t-test comparing each treated group with the control group. RESULTS: increased serum glucagon (control group: 88.2 pg/ml; treated groups: 99.7-120.7 pg/ml) and reduced insulin (control group: 6.2 muIU/ml; treated groups: 2.1-2.7 muIU/ml) were observed in all groups treated with antiretroviral drugs after 21 days of pregnancy. There was no significant difference between the experimental groups and the control in glycemia, plasma FFA or hepatic glycogen. Also, there was no significant difference in number of insulin- and glucagon-producing cells between the treated groups and the control. CONCLUSION: treatment of noninfected rats with antiretroviral drugs during pregnancy altered maternal glycid metabolism causing insulin decrease and glucagon elevation, with normal glycemia and unchanged number of pancreatic cells.

Women have emerged as the fastest growing human immunodeficiency virus (HIV) infected population worldwide, mainly because of the increasing occurrence of heterosexual transmission. Most infected women are of reproductive age and one of the greatest concerns for both women and their physicians is that more than 1,600 infants become infected with HIV each day. Almost all infections are a result of mother-to-child transmission of HIV. With the advent of combination antiretroviral therapies, transmission rates lower than 2% have been achieved in clinical studies. Antiretroviral compounds differ from most other new pharmaceutical agents in that they have become widely prescribed in pregnancy in the absence of proof of safety. We reviewed antiretroviral agents used in pregnant women infected with human immunodeficiency virus, mother-to-child transmission, and their consequences for infants.

A redução da transmissão vertical (TV) do vírus da imunodeficiência humana tipo 1 (HIV-1) utilizando a profilaxia com a zidovudina (AZT) representa significativo avanço na assistência pré-natal e obstétrica destas pacientes. Condutas obstétricas invasivas são contra-indicadas em gestantes portadoras do HIV-1, em face do risco de aumento da taxa de TV deste vírus. Os autores relatam um caso de polidrâmnio recorrente em gestante portadora do HIV-1, que exigiu drenagem por amniocentese. Foram realizadas quatro punções ao longo da gestação, na 23ª, 26ª, 27ª e 29ª semanas, todas guiadas por ultra-sonografia, drenando, respectivamente, 1.800, 1.450, 1.700 e 1.960 mL de líquido amniótico claro em cada punção. Com 30 semanas e 5 dias de gestação a paciente apresentou trabalho de parto pré-termo, evoluindo para parto vaginal de recém-nato (RN) pesando 1.690 g e medindo 43 cm. O RN evoluiu com diagnóstico de nefropatia perdedora de sódio, tendo três aferições de reação em cadeia de polimerase para HIV-1 negativas. Os autores ilustram uma opção no manejo de situações que envolvam gestantes portadoras do HIV-1 que necessitem de procedimentos obstétricos invasivos, utilizando AZT endovenoso (2 mg/kg) previamente ao procedimento, medida que apresentou excelente resultado no caso descrito, evitando a infecção perinatal pelo HIV-1.

The reduction of mother-to-child transmission (MTCT) of the HIV-1 using zidovudine (ZDV) represents a cornerstone in the prenatal and obstetrical care to these patients. The invasive fetal and obstetric procedures are proscribed in HIV-1 infected pregnant patients, to avoid the increased risk of MTCT of this virus. The authors present a case of an HIV-1 infected woman with recurrent polyhydramnios. Four ultrasound-guided amniotic punctures were performed in the 23rd, 26th, 27th and 29th weeks of gestation, each one draining the respective volumes of 1,800, 1,450, 1,700 and 1,960 ml of clear amniotic fluid. The patient started preterm labor with 30 weeks and 5 days resulting in vaginal delivery of a male neonate weighing 1,690g and measuring 43cm. The baby presented a post natal diagnosis of a sodium-losing nephropathy and was submitted to three negative polymerase chain reaction tests for HIV-1. The authors point out that the option to manage cases of HIV-1 infected pregnancies that could need invasive obstetric procedures should be to give the patient 2 mg//kg of ZDV endovenously before the procedure, in order to avoid MTCT of HIV-1, as it has demonstrated good results in this case.

OBJETIVO: estudar a ação diabetogênica de drogas anti-retrovirais em ratas prenhes e o prognóstico perinatal das crias. MÉTODOS: estudo com ratas fêmeas prenhes adultas da raça Wistar, pesando entre 200-230 g. Foram testadas a azidotimidina (AZT), lamivudina (3TC) e nelfinavir (NFV), cujas dosagens foram padronizadas em 10 vezes a dose utilizada em gestantes, proporcionalmente ao peso dos animais. Foram avaliados sete grupos, incluindo o controle, contendo 10 ratas por grupo. O início do experimento foi o dia zero da prenhez e as cesarianas realizadas no 21º dia, após decapitação, sendo os fetos contados e pesados. Procedeu-se a dosagens de glicemia, insulina, glucagon e lactato no 21º dia. Avaliou-se também o peso do tecido adiposo retroperitoneal. Os dados foram analisados utilizando-se o teste t de Student para a análise estatística. RESULTADOS: os grupos tratados com 3TC, AZT + 3TC e AZT + 3TC + NFV demonstraram alterações com a redução das médias de ganho de peso materno diário, do peso da gordura retroperitoneal e peso das crias (grupo controle: 6,2 g; grupos contendo 3TC: 4,1 a 5,6 g), bem como dos valores de lactato (grupo controle: 5,8 mmol/mL; grupos contendo 3TC: 3,2 a 3,7 mmol/mL), quando comparados ao controle. Todos os grupos tratados com drogas anti-retrovirais apresentaram redução significativa do número de fetos por ninhada (grupo controle: 14,7; grupos medicamentos: 11,1 a 12,7) e dos valores séricos de insulinemia (grupo controle: 6,2 µUI/mL; grupos medicamentos: 2,1 a 2,7 µUI/mL) e elevação da glucagonemia (grupo controle: 88,2 pg/mL; grupos medicamentos: 99,7 a 120,7 pg/mL). Não houve diferenças estatisticamente significantes entre o grupo controle e tratados nos valores de glicemia. CONCLUSÕES: o uso de anti-retrovirais em ratas prenhes causa interferência no metabolismo glicídico dos animais durante o período de prenhez, provocando significativa redução do número das crias. Observou-se que o uso do 3TC resultou em menor ganho de peso materno e das crias, redução de insulina e lactato e elevação do glucagon.

PURPOSE: to experimentally evaluate the diabetogenic effects of antiretroviral drugs on pregnant Wistar rats and the perinatal effects on the offspring. METHODS: adult female pregnant Wistar rats weighing 200-230 g were used. The antiretroviral drugs zidovudine (ZDV), lamivudine (3TC) and nelfinavir (NFV) were used alone and in association at daily doses of ten times the dose normally used in pregnant women, proportionally to the animal's body weight. Seven groups were studied, including the control. The experiment started on day 0 of pregnancy and the pregnant animals were sacrificed on day 21. The fetuses were counted and weighed. Blood determinations of glucose, insulin, glucagon and lactate were performed on day 21. The retroperitoneal adipose tissue was weighed. Data were analyzed statistically by Student's t-test. RESULTS: the groups treated with 3TC, ZDV + 3TC and ZDV + 3TC + NFV showed decreasing values of maternal daily body weight gain, retroperitoneal adipose tissue weight and weight of fetuses (control group: 6.2 g; 3TC group variation: 4.1-5.6 g). The serum lactate levels were also decreased when compared to the control in these groups (control group: 5.8 mmol/mL; 3TC group variations: 3.2-3.7 mmol/mL). All antiretroviral-treated groups showed a decreasing number of fetuses when compared to the control (control group: 14.7; drug group variation: 11.1-12.7). All treated groups also showed decreasing serum values of insulin (control group: 6.2 µIU/mL; drug group variation: 2.1 to 2.7 µIU/mL) and increasing serum levels of glucagon when compared to the control (control group: 88.2 pg/mL; drug group variation: 99.7 to 120.7 pg/mL). There was no statistical significance of glucose levels when comparing treated groups to the control. CONCLUSIONS: the antiretroviral drugs interfered in carbohydrate metabolism of pregnant rats and reduced the number of fetuses. 3TC caused less maternal body weight gain, decreased fetus weight and lactate and insulin levels and increased serum glucagon.

Atualmente, dentro do contexto epidêmico da AIDS observam-se profundas modificações no perfil dos pacientes acometidos, especialmente traduzida pela pauperização e feminilização. A população mais afetada encontra-se em idade reprodutiva e entre adultos de 18 a 24 anos, a relação é de 1 homem para 1 mulher, o que ocorre indiscriminadamente como fenômeno global. Um dos maiores desafios para as gestantes portadoras do HIV-1 e seus médicos assistentes é a repercussão advinda da interação entre a infecção pelo HIV e a gestação. Esse artigo é uma revisão de literatura e tem por objetivos avaliar a influência da infecção pelo HIV-1 sob a perspectiva materna tecendo considerações sobre a função imunológica, o prognóstico materno e a interface HIV e abortamento. Até o presente, não se pode concluir que a gestação tenha, a curto prazo, um efeito norteador da evolução da infecção pelo HIV, porém a concomitância HIV e gestação pode adversamente afetar o prognóstico gestacional, especialmente face à freqüente associação com acréscimo nos índices de aborto e morbidade puerperal.

Profound modifications in the profile of patients are currently being observed within the epidemic context of AIDS, especially with respect to pauperization and feminization of the disease. The population most frequently affected is in the reproductive age, and among adults aged 18 to 24 years, the ratio is 1 man to 1 woman, a phenomenon occurring uniformly all over the world. One of the main challenges for HIV-1-infected pregnant women and their doctors is the effect of the interaction between HIV infection and pregnancy. The present article is a review of the literature; and its objective is to assess the influence of HIV-1 infection seen from the maternal perspective, with a discussion of immunologic function, maternal prognosis, and the HIV-abortion interface. At present, we cannot conclude that pregnancy has a short-term effect on the evolution of HIV infection, but the concomitance of HIV and pregnancy may adversely affect the prognosis of gestation, especially in view of its frequent association with increased abortion and puerperal morbidity rates.
