Resumo No âmbito das intervenções comunitárias que são promovidas conjuntamente por entidades da área da saúde e do social, as intervenções grupais desenvolvidas para cuidadores informais de pessoas com demência têm se instituído como um importante tópico de investigação e objeto de reflexão profissional. O presente estudo tem como objetivo expor o funcionamento de um conjunto de Grupos de Ajuda Mútua (GAM) enquanto respostas de continuidade no apoio a cuidadores informais de pessoas com demência que participam em Programas Psicoeducativos. Partindo de um Projeto de Intervenção Comunitária realizado na zona Norte de Portugal (Projeto Cuidar de Quem Cuida), o estudo descreve cinco GAMs ocorridos em municípios diferentes e implementados/dinamizados por agentes de desenvolvimento local. São apresentados os seus objetivos e principais características de funcionamento enquanto resposta de continuidade. Os resultados revelam a sua importância para os cuidadores, nomeadamente para a dinamização/ manutenção de redes de apoio social informal no âmbito das escassas respostas especificamente destinadas para esta população, bem como para o papel fundamental dos seus agentes dinamizadores (técnicos de saúde e/ou da ação social). Conclui-se pela relevância dos GAMs enquanto iniciativa comunitária de entreajuda e de suporte ativo e continuado para cuidadores informais de idosos com demência.

Abstract Within community interventions that involve entities from both the health and the social sectors, group interventions for informal caregivers of individuals with dementia have received increased attention and are both a contemporary research topic and a subject of professional reflection. This study reports a set of distinct Support Groups for Informal Caregivers that were implemented after the caregivers’ participation in a psychoeducational program. Based upon a community intervention project implemented in North Portugal (Caring for the Caregiver Project), the study describes five support groups established in five different municipalities and assumed by local agents from the health and/or social sectors. It focuses on each group’s main objectives and functioning characteristics as a continuity response that was organized at a broad community level. Main findings stress the importance of these support groups on maintaining the caregivers’ informal social network and mutual support system within a context of reduced services explicitly targeted for this population, and to the need of having a professional who assumes the role of organizing the sessions. Authors conclude on the relevance of support groups as a continuous and lively community intervention targeted at helping family carers of people with dementia.

BACKGROUND: Improved gait efficiency is one of the goals of therapy for children with cerebral palsy (CP). Postural insoles can allow more efficient gait by improving biomechanical alignment. OBJECTIVE: The aim of the present study was to determine the effect of the combination of postural insoles and ankle-foot orthoses on static and functional balance in children with CP. METHOD: A randomized, controlled, double-blind, clinical trial. After meeting legal requirements and the eligibility criteria, 20 children between four and 12 years of age were randomly allocated either to the control group (CG) (n=10) or the experimental group (EG) (n=10). The CG used placebo insoles and the EG used postural insoles. The Berg Balance Scale, Timed Up-and-Go Test, Six-Minute Walk Test, and Gross Motor Function Measure-88 were used to assess balance as well as the determination of oscillations from the center of pressure in the anteroposterior and mediolateral directions with eyes open and closed. Three evaluations were carried out: 1) immediately following placement of the insoles; 2) after three months of insole use; and 3) one month after suspending insole use. RESULTS: The EG achieved significantly better results in comparison to the CG on the Timed Up-and-Go Test as well as body sway in the anteroposterior and mediolateral directions. CONCLUSION: Postural insoles led to an improvement in static balance among children with cerebral palsy, as demonstrated by the reduction in body sway in the anteroposterior and mediolateral directions. Postural insole use also led to a better performance on the Timed Up-and-Go Test.

Background: Transcranial direct-current stimulation (tDCS) has been widely studied with the aim of enhancing local synaptic efficacy and modulating the electrical activity of the cortex in patients with neurological disorders. Objective: The purpose of the present study was to determine the effect of a single session of tDCS regarding immediate changes in spatiotemporal gait and oscillations of the center of pressure (30 seconds) in children with cerebral palsy (CP). Method: A randomized controlled trial with a blinded evaluator was conducted involving 20 children with CP between six and ten years of age. Gait and balance were evaluated three times: Evaluation 1 (before the stimulation), Evaluation 2 (immediately after stimulation), and Evaluation 3 (20 minutes after the stimulation). The protocol consisted of a 20-minute session of tDCS applied to the primary motor cortex at an intensity of 1 mA. The participants were randomly allocated to two groups: experimental group - anodal stimulation of the primary motor cortex; and control group - placebo transcranial stimulation. Results: Significant reductions were found in the experimental group regarding oscillations during standing in the anteroposterior and mediolateral directions with eyes open and eyes closed in comparison with the control group (p<0.05). In the intra-group analysis, the experimental group exhibited significant improvements in gait velocity, cadence, and oscillation in the center of pressure during standing (p<0.05). No significant differences were found in the control group among the different evaluations. Conclusion: A single session of tDCS applied to the primary motor cortex promotes positive changes in static balance and gait velocity in children with cerebral palsy.

O objetivo foi identificar os aspectos da dimensão prática do cuidado de enfermagem na perspectiva das representações sociais elaboradas por acadêmicos iniciantes e concluintes da graduação. O referencial teórico-metodológico foi o da Teoria das Representações Sociais. Tratou-se de uma pesquisa de abordagem qualitativa exploratória, com coleta de dados por entrevista individual com 28 acadêmicos, que sofreu análise temática de conteúdo. Os resultados demonstraram as peculiaridades dos elementos constituintes das representações sociais elaboradas pelos dois grupos. As representações sociais dos dois grupos se distinguem, já que os iniciantes descrevem uma prática voltada aos aspectos afetivo-expressivos, enquanto os concluintes trazem marcadamente a conjugação desses aspectos com o conhecimento científico. Conclusão: O curso de graduação, gradativamente, influencia nas representações sociais do cuidado e, consequentemente, na sua dimensão prática.

This study aims to identify aspects of the practical dimension in nursing care under the perspective from the social representations elaborated by nursing students beginning the graduation, and others about to conclude it. The theoretical-methodological reference was the Theory of Social Representations. This was an exploratory qualitative study, collecting data through individual interviews applied to 28 students which had a thematic content analysis. The results showed the peculiarities of the constituent elements of social representations made by both groups. Social representations of both groups are distinguished, as the beginners describe a practice focused on affective and expressive aspects markedly while the students about to be graduated bring, effectively, a combination of these aspects with scientific knowledge. Conclusion: The graduation program gradually influences the social representations in care, and consequently in its practical dimension.

Objetivo: identificar los aspectos de la dimensión práctica de la atención de enfermería desde la perspectiva de las representaciones sociales elaboradas por académicos juniores y concluyentes de la graduación. El referencial teórico y metodológico fue el de la Teoría de las Representaciones Sociales. Es una investigación de abordaje cualitativo exploratorio, con recolección de datos a través de entrevista individual con 28 académicos, y ha sido realizado el análisis temático de contenido. Los resultados demostraron las peculiaridades de los elementos constituyentes de las representaciones sociales elaboradas por dos grupos. Las representaciones sociales de los dos grupos se distinguen, ya que los juniores describen una práctica involucradas a los aspectos afectivos y expresivos, mientras los concluyentes trajeron marcadamente la conjugación de eses aspectos con el conocimiento científico. Conclusión: El curso de graduación, progresivamente, influencia en las representaciones sociales de la atención, y, consecuentemente, en su dimensión práctica.

O objetivo deste estudo foi comparar os fatores de risco para quedas em idosas com diferentes níveis de atividade física. A amostra foi composta por 50 idosas residentes na comunidade, pareadas pela idade (71,62 + 5,49 anos). Os desfechos avaliados foram número de quedas, fatores de risco avaliados pelo instrumento QuickScreen "test", tempo para realização do teste TUG, medo de cair e auto-percepção de saúde. Para avaliação do nível de atividade foi utilizado o questionário Perfil de Atividade Humana. As idosas foram alocadas para os grupos: inativo (GI) n=17, moderadamente ativo (GM) n=16 ou ativo (GA) n=17. O teste One-Way Anova foi utilizado para comparação dos fatores de risco para quedas entre os grupos GI, GM e GA. Para comparação das variáveis medo de cair e auto-percepção de saúde foi utilizado o teste Mann-Whitney U. O nível de significância foi estabelecido em α=0,05. Foram encontradas diferenças estatisticamente significativas entre os grupos GI x GM, GI x GA, sendo encontrada diferença significativa entre os grupos GM x GA apenas para o TUG. Os resultados sugerem que idosas com níveis de atividade física moderadamente ativo e ativo apresentam menor risco de quedas, comparado a idosas sedentárias.

The aim of this study was to compare the risk factors for falls in older women with different levels of physical activity. The sample was composed by 50 community-dwelling older women, matched by age (71,62 + 5,49 years old). The outcomes evaluated were number of falls, risk factors of QuickScreen Test, time to complete the TUG test, fear of falling and self-perception about health. The questionnaire Human Activity Profile was used to evaluate the level of physical activity. The older women were allocated to following groups: inactive (GI) n=17, moderate active (GM) n=16 or active (GA) n=17. One-Way Anova test was used to compare risk factors for falls between GI, GM and GA groups. Mann-Whitney U test was used to compare the variables fear of falling and self-perception about health. The level of significance was set at α=0,05. There were found significant differences between GI X GM, GI X GA. There were found significant difference between GM X GA groups only for TUG test. The results suggest that moderate active and active older women show less risk for fall, when compared with sedentary ones.

A intensidade dos cuidados prestados pelo Serviço de Apoio Domiciliário (SAD) é um tema importante em diversos países. Em alguns casos, é considerada um critério de qualidade e a base de cálculo do pagamento do Serviço de Apoio Domiciliário. Este estudo tem como objectivos analisar quantitativamente a intensidade das tarefas de cuidado prestadas no Serviço de Apoio Domiciliário em Portugal, em termos de periodicidade e tempo despendido (em minutos), e fazer o cálculo dos valores de referência do consumo de tempo para cada tarefa de cuidado nas Actividades de Vida Diária (Básicas e Instrumentais). No total foram analisadas oito instituições promotoras do Serviço de Apoio Domiciliário, as quais se agruparam segundo o contexto territorial a que pertencem (rural/ urbano) e a capacidade de utentes que possuem (pequena/ grande dimensão). De cada instituição foram aleatoriamente seleccionados seis utilizadores, resultando em amostra final de 48 sujeitos. A recolha de dados foi realizada com o recurso ao Protocolo de Serviço de Apoio Domiciliário (ProSAD). Os resultados indicam que a intensidade é superior nos cuidados prestados em termos de Actividades Básicas de Vida Diária, mas os valores variam em função da presença/ausência de cuidador informal. Concluiu-se que a intensidade dos cuidados em Portugal é reduzida, sendo que os poucos serviços prestados incidem nas Actividades Básicas de Vida Diária. O tempo a ser utilizado por cada utente do Serviço de Apoio Domiciliário deve ter por base critérios objectivos, como o estado funcional do utente e a rede de suporte social.

The intensity of care provided by Home Support Service (DSS) is a major issue in many countries. In some cases, it is considered a quality criterion and basis of calculation of payment of the Home Support Service. This study aims to quantitatively analyze the intensity of caregiving tasks provided in the Home Support Service in Portugal in terms of frequency and time spent (in minutes), and calculate reference values of the time consumption for each task care in Activities of Daily Living (Basic and Instrumental). Eight institutions promoters of Home Support Service were analyzed, grouped according to the local context to which they belong (rural / urban) and the ability of users that have (small / large). Each institution randomly selected six users, resulting in a final sample of 48 subjects. Data collection was performed with the use of the Protocol of Home Support Service (PROSAD). The results indicate that the intensity is higher than the care provided in terms of Basic Activities of Daily Living, but the values vary depending on the presence / absence of informal caregivers. The conclusion was that the intensity of care in Portugal is reduced, and the few services focus on the Basic Activities of Daily Living. The time to be used by each user of the Home Support Service should be based on objective criteria such as the functional state of the user and the social support network.the elderly about participation in groups. Interviews were conducted with 20 people of both genders and divided into two groups, of which ten attended the Grupo Flor de Maçã, and ten attend the Grupo Melhor Idade em Ação. Data were collected through semi-structured interviews and participant observation recorded in a field diary, which occurred in places and times scheduled with supervisors and participants. The data were analyzed by the participants' speech, processed and separated into pre-established categories. It was found that participation in living groups made dramatic improvements and changes in the lives of elderly of the study and improvements in several factors can be noticed, such as health, self-esteem and recovery. Many reported that activities in groups contributed significantly to the development of the functions of daily life, besides getting a recognition and appreciation, both by families and by society in general. We conclude that participation in groups is paramount in the quest to achieve better quality of life.
