RESUMO JUSTIFICATIVA E OBJETIVOS: A pandemia de COVID-19 exacerbou os sintomas de pacientes com dor crônica. Porém, é necessário considerar o contexto sociocultural de enfrentamento da pandemia nos diferentes países. O objetivo deste estudo foi avaliar a dor, os sintomas psicoemocionais, a qualidade do sono e o nível de atividade física em pacientes com dor crônica durante a pandemia de COVID-19 no Brasil. MÉTODOS: Foram incluídos indivíduos com fibromialgia, enxaqueca e dor lombar. O levantamento foi realizado por meio de questionário elaborado pelos pesquisadores no aplicativo Forms, do Google Drive, entre outubro de 2020 e março de 2021, com perguntas diretas e claras sobre dor, aspectos psicoemocionais, qualidade do sono e nível de atividade física. Análise de regressão logística binária ou multinominal foi realizada para identificar possíveis preditores. RESULTADOS: Foram avaliadas 973 pessoas, 63,5% apresentavam fibromialgia, sendo 98,3% do sexo feminino. A ansiedade aumentou a razão de chances em 395% dos pacientes com dor crônica sentirem dor (ß: 1,375; OR: 3,956; p= 0,001) e a dor aumentou em 62,3% a chance desses indivíduos não realizarem atividade física (ß: -0,474; OR: 0,623; p=0,001). A intensidade da dor aumentou a razão de chances de ter insônia em 186,9% (ß: 0,625; OR: 1,869; p= 0,001) e a chance de tomar fármacos para dormir em 160,4% (ß: 0,472; OR: 1,604; p=0,001). CONCLUSÃO: O isolamento social da pandemia de COVID-19 maximizou o ciclo vicioso entre sintomas dolorosos, ansiedade e distúrbios do sono em pacientes com dor crônica no Brasil. A intensificação desses fatores está associada à redução de atividade física. ABSTRACT BACKGROUND AND OBJECTIVES: The social distancing of COVID-19 pandemic exacerbated symptoms of chronic pain patients. Furthermore, it is necessary to consider the sociocultural context of coping with the pandemic in different countries. The objective of this study was to evaluate pain, psychoemotional symptoms, sleep quality, and level of physical activity in chronic pain patients during the COVID-19 pandemic in Brazil. METHODS: Individuals with fibromyalgia, migraine and chronic low back pain were included. The survey was performed through a questionnaire elaborated by researchers on the Forms application, from Google Drive, in the period between October 2020 and March 2021, with straight and clear questions about pain, psychoemotional aspects, sleep quality, and level of physical activity. Binary or multinominal logistic regression analysis was performed to identify possible predictors. RESULTS: 973 people were evaluated, 63.5% had fibromyalgia, and 98.3% were female. Anxiety increased the odds ratio in 395% of chronic pain patients to feel pain (ß: 1.375; OR: 3.956; p=0.001) and a greater intensity of pain increases in 62.3% the chance of these individuals not performing physical activity (ß: -0.474; OR: 0.623; p=0.001). Pain intensity increased the odds ratio of having insomnia by 186.9% (ß: 0.625; OR: 1.869; p=0.001) and the chance of taking sleep drugs by 160.4% (ß: 0.472; OR: 1.604; p=0.001). CONCLUSION: The social isolation of COVID-19 pandemic maximized the vicious cycle between painful symptoms, anxiety, and sleep disorders in chronic pain patients in Brazil. The intensification of these factors is associated with the reduction of physical activity levels. RESUMO JUSTIFICATIVA E OBJETIVOS Identificar o nível de conhecimento sobre a neurofisiologia da dor na migrânea crônica por meio da aplicação do Questionário de Neurofisiologia da Dor. MÉTODOS Foi realizado um estudo transversal com 30 pacientes com migrânea crônica. O nível de conhecimento neurofisiológico da dor foi avaliado pela versão brasileira do Questionário de Neurofisiologia da Dor. RESULTADOS: Em geral, pacientes com migrânea crônica apresentaram baixo nível de conhecimento sobre a neurofisiologia da dor com 4,03±1,52 acertos em 12 questões (34,1%). Destes 30 pacientes, houve predomínio do sexo feminino (72,7%), idade 33±8,60 anos, maior escolaridade (73,4%), intensidade de dor leve em repouso (2,83±2,39), subclassificação predominante de migrânea crônica sem aura (56,7%) e com história de migrânea há mais de 10 anos (60,1%). Este artigo identificou o nível de conhecimento sobre a neurofisiologia da dor na migrânea crônica por meio da aplicação do Questionário de Neurofisiologia da Dor. CONCLUSÃO Pacientes com migrânea crônica apresentam baixo nível de conhecimento neurofisiológico.

ABSTRACT BACKGROUND AND OBJECTIVES Identify the level of knowledge about the neurophysiology of pain in patients with chronic migraine through the application of the Neurophysiology of Pain Questionnaire. METHODS: A cross-sectional study was carried out with 30 patients with chronic migraine. The level of neurophysiological knowledge of pain was assessed by the Brazilian version of the Neurophysiology of Pain Questionnaire. RESULTS: In general, patients with chronic migraine had a low level of knowledge about pain neurophysiology with 4.03±1.52 correct answers in 12 questions (34.1%). From those 30 patients, there was a predominance of females (72.7%), aged 33±8.60 years, higher schooling level (73.4%), mild intensity of pain at rest (2.83±2.39), predominant subclassification of chronic migraine without aura (56.7%) and with a history of migraine for more than 10 years (60.1%). This article identified the level of knowledge about neurophysiology of pain in chronic migraines through the application of the Neurophysiology of Pain Questionnaire. CONCLUSION: Patients with chronic migraine have a low level of neurophysiological knowledge.

RESUMO JUSTIFICATIVA E OBJETIVOS: Mobilização articular é uma técnica não farmacológica usada no tratamento da dor musculoesquelética crônica. No entanto, é controverso devido à falta de estudos que comparem seus efeitos sobre essa condição de dor. O objetivo deste estudo foi avaliar o risco de viés em ensaios clínicos que investigam o efeito da mobilização articular na dor musculoesquelética crônica. CONTEÚDO: Foi realizada uma busca sistematizada no Pu-bmed, Cochrane Library, ScienceDirect, Scielo, PEDro, CINAHL, SPORTDiscus, LILACS, BVS, PsycINFO, Web of Science e Scopus em setembro de 2019 com a combinação de três palavras-chave: Musculoskeletal Manipulations AND Chronic Pain AND Musculoskeletal Pain. Ensaios clínicos controlados e aleatorizados que avaliaram o uso de mobilização articular associada ou não a outras terapias no tratamento da dor muscu-loesquelética crônica foram incluídos. Foram encontrados 5587 artigos e analisados 14 estudos, incluindo 812 participantes, com idade média de 54 anos, sendo o sexo feminino o mais afetado. Nestes, a mobilização articular promoveu redução da dor em curto e longo prazo, amplitude de movimento, força e melhora da função quando utilizado isoladamente ou em associação à fisioterapia convencional. Em relação à qualidade metodológica, a maioria dos estudos foi classificada com baixo risco para seleção, desempenho, detecção e viés de relato. No item “other bias”, que considerou terapeutas com tempo de experiência e tipos de tratamento aplicados, apenas um estudo apresentou baixo risco e outro estudo apresentou risco incerto. CONCLUSÃO: Mobilizaç ão articular parece ser uma técnica eficaz para o tratamento da dor musculoesquelética crônica. No entanto, é necessário realizar estudos com maior qualidade metodológica, promovendo maior apoio à prática baseada em evidências.

ABSTRACT BACKGROUND AND OBJECTIVES: Joint mobilization is a non-pharmacological technique used to treat chronic musculoskeletal pain. However, it is controversial due to a lack of studies comparing its effects on this painful condition. The objective of this study was to assess the risk of bias in clinical trials investigating the effect of joint mobilization on chronic musculoskeletal pain. CONTENTS: A systematic search on Pubmed, Cochrane Library, ScienceDirect, Scielo, PEDro, CINAHL, SPORTDiscus, LILACS, BVS, PsycINFO, Web of Science, and Scopus was performed on September 2019 from the combination of three keywords: Musculoskeletal Manipulations AND Chronic Pain AND Musculoskeletal Pain. Randomized controlled clinical trials that evaluated the use of joint mobilization associated or not to other therapies in chronic musculoskeletal pain treatment were included. Five thousand five hundred eighty-seven articles were screened, and 14 studies were analyzed, including 812 participants, with a mean age of 54 years, and female being the most affected. According to these articles, joint mobilization promoted the reduction of pain intensity in short and long terms, increase in range of motion, strength and function when used alone or in association with conventional physiotherapy. Regarding methodological quality, most of the studies were classified with low risk for selection, performance, detection and reporting bias. In the “other bias” item, which considered therapists experience time and types of treatment applied, only one study presented low risk and other study presented an unclear risk. CONCLUSION: Joint mobilization seems to be an effective technique for the treatment of chronic musculoskeletal pain. However, it is still necessary to investigate and compile studies with greater methodological quality, thus promoting greater support to evidence-based practice.
