Abstract Introduction The Strengthening and Stretching for Rheumatoid Arthritis of the Hand (SARAH) program is a personalized, progressive 12-week exercise program for people with hand problems due to rheumatoid arthritis (RA). Patients are provided with two guidance documents, the ‘Patient Exercise Booklet’ and the ‘Personal Exercise Guide’, to continue the exercises independently at home. Objective This study aimed to translate and culturally adapt the SARAH protocol into Brazilian Portuguese and validate its content. Methods The guidance documents ‘Patient Exercise Booklet’ and ‘Personal Exercise Guide’ of the SARAH program were translated and culturally adapted to Brazilian Portuguese. The content validity was obtained by calculating the content validity index (CVI). Results The Brazilian version of the SARAH protocol reached semantic, idiomatic, conceptual, and cultural equivalences. The CVI was greater than 0.8, corresponding to a satisfactory index. The verbal comprehension was 4.9, showing good verbal comprehension ofthe target population. RESUMO Objetivo: Avaliar os efeitos da implementação de um programa de educação sobre higienização das mãos e a adesão a essa prática entre profissionais de uma UTI. Métodos: Estudo quase-experimental com séries temporais interrompidas conduzido ao longo de 12 meses: 5 meses antes do programa de educação (período basal), 2 meses durante a fase intensiva do programa (período de intervenção) e 5 meses durante a fase de manutenção do programa (período pós-intervenção). A avaliação da adesão às práticas de higienização das mãos foi feita por um dos pesquisadores sem conhecimento da equipe da UTI. O desfecho primário foi a variação da taxa de adesão à higienização das mãos. Também foram avaliadas a duração da ventilação mecânica (VM), a incidência de pneumonia associada à ventilação mecânica (PAVM) em 28 e 60 dias e mortalidade nos mesmos momentos. Resultados: Com base em 959 observações, houve um aumento nas taxas de adesão à higienização das mãos de 31,5% no período basal para 65,8% no período de intervenção e para 83,8% no período pós-intervenção, representando uma razão de prevalência 2,09 e 2,66 maior que o período basal, respectivamente (p < 0,001). Apesar desse aumento, não houve diferenças significativas em relação à duração da VM ou à incidência de PAVM e taxa de mortalidade em 28 e 60 dias. Conclusões: Neste estudo, o programa de educação em higienização das mãos aumentou a adesão a esse procedimento durante o período de monitorização, sem alterar as taxas de PAVM, a duração da VM e a mortalidade.

ABSTRACT Objective: To evaluate the effects that a hand hygiene education program has on the compliance of health professionals in an ICU. Methods: This was a quasi-experimental study with an interrupted time-series design, conducted over a 12-month period: the 5 months preceding the implementation of a hand hygiene education program (baseline period); the 2 months of the intensive (intervention) phase of the program; and the first 5 months thereafter (post-intervention phase). Hand hygiene compliance was monitored by one of the researchers, unbeknownst to the ICU team. The primary outcome measure was the variation in the rate of hand hygiene compliance. We also evaluated the duration of mechanical ventilation (MV), as well as the incidence of ventilator-associated pneumonia (VAP) at 28 days and 60 days, together with mortality at 28 days and 60 days. Results: On the basis of 959 observations, we found a significant increase in hand hygiene compliance rates-from 31.5% at baseline to 65.8% during the intervention phase and 83.8% during the post-intervention phase, corresponding to prevalence ratios of 2.09 and 2.66, respectively, in comparison with the baseline rate (p < 0.001). Despite that improvement, there were no significant changes in duration of MV, VAP incidence (at 28 or 60 days), or mortality (at 28 or 60 days). Conclusions: Our findings indicate that a hand hygiene education program can increase hand hygiene compliance among ICU professionals, although it appears to have no impact on VAP incidence, duration of MV, or mortality.

Introdução Para evitar a seleção de esforços submáximos durante as medidas das pressões inspiratórias e expiratórias máximas (PImáx e PEmáx), alguns critérios de reprodutibilidade têm sido sugeridos, destacando-se os propostos pela ATS/ERS e pela SBPT. Entretanto, não foram encontrados na literatura estudos que confrontem esses dois critérios, ou que investiguem a combinação de ambos. Objetivos Avaliar os valores pressóricos selecionados e o número de manobras necessárias para se alcançar a capacidade máxima por meio do emprego dos critérios de reprodutibilidade propostos pela ATS/ERS, pela SBPT e pelo presente estudo. Materiais e métodos Foram avaliados 113 indivíduos saudáveis (43,04 ± 16,94 anos), de ambos os sexos, considerando-se os critérios de reprodutibilidade propostos pela ATS/ERS, pela SBPT e pelo presente estudo. Análise estatística realizada com estatística descritiva, seguida do teste ANOVA para medidas repetidas (post hoc LSD) ou do teste de Friedman (post hoc Wilcoxon), segundo a normalidade dos dados. Resultados O critério proposto resultou em um número de manobras significativamente maior (PImáx e PEmáx – mediana e intervalo interquartílico 25%-75%: 5[5-6], 4[3-5] e 3[3-4] para este estudo, SBPT e ATS/ERS, respectivamente; p < 0,01) e na seleção de valores pressóricos mais elevados (PImáx – média e intervalo de confiança 95%: 103[91,43-103,72], 100[97,19-108,83] e 97,6[94,06-105,95]; PEmáx – mediana e intervalo interquartílico 25%-75%: 124,2[101,4-165,9], 123,3[95,4-153,8] e 118,4[95,5-152,7] para este estudo, SBPT e ATS/ERS, respectivamente; p < 0,05). Conclusão O critério proposto resultou em valores pressóricos mais próximos da real capacidade máxima dos sujeitos avaliados. Este novo critério deve ser considerado em estudos futuros relacionados às medidas de PImáx e PEmáx.

Introduction To avoid the selection of submaximal efforts during the assessment of maximal inspiratory and expiratory pressures (MIP and MEP), some reproducibility criteria have been suggested. Criteria that stand out are those proposed by the American Thoracic Society (ATS) and European Respiratory Society (ERS) and by the Brazilian Thoracic Association (BTA). However, no studies were found that compared these criteria or assessed the combination of both protocols. Objectives To assess the pressure values selected and the number of maneuvers required to achieve maximum performance using the reproducibility criteria proposed by the ATS/ERS, the BTA and the present study. Materials and method 113 healthy subjects (43.04 ± 16.94 years) from both genders were assessed according to the criteria proposed by the ATS/ERS, BTA and the present study. Descriptive statistics were used for analysis, followed by ANOVA for repeated measures and post hoc LSD or by Friedman test and post hoc Wilcoxon, according to the data distribution. Results The criterion proposed by the present study resulted in a significantly higher number of maneuvers (MIP and MEP – median and 25%-75% interquartile range: 5[5-6], 4[3-5] and 3[3-4] for the present study criterion, BTA and ATS/ERS, respectively; p < 0.01) and higher pressure values (MIP – mean and 95% confidence interval: 103[91.43-103.72], 100[97.19-108.83] and 97.6[94.06-105.95]; MEP: median and 25%-75% interquartile range: 124.2[101.4-165.9], 123.3[95.4-153.8] and 118.4[95.5-152.7]; p < 0.05). Conclusion The proposed criterion resulted in the selection of pressure values closer to the individual’s maximal capacity. This new criterion should be considered in future studies concerning MIP and MEP measurements.

MÉTODOS: Estudo transversal com 49 sujeitos saudáveis, eutróficos, de ambos os sexos, com média de idade de 23,08 ± 2,50 anos. As medidas foram realizadas utilizando-se um transdutor de pressão, e os parâmetros foram estimados a partir de um algoritmo matemático desenvolvido para a pesquisa: pressões de pico (Ppico), de platô (Pplatô), média máxima (Pmédia) e segundo a área (Párea). Foi empregada a estatística descritiva para caracterização da amostra, seguida por ANOVA para medidas repetidas e teste post hoc de Bonferroni ou teste de Friedman e teste post hoc de Wilcoxon, assim como correlações de Pearson ou Spearman, segundo a normalidade dos dados. A concordância entre as variáveis foi avaliada pelo método gráfico de Bland & Altman. RESULTADOS: Houve diferenças significativas entre todos os parâmetros, tanto para PImáx (Ppico = 95,69 ± 27,89 cmH2O; Párea = 88,53 ± 26,45 cmH2O; Pplatô = 82,48 ± 25,11 cmH2O; Pmédia = 89,01 ± 26,41 cmH2O; p < 0,05 entre todos) quanto para PEmáx (Ppico = 109,98 ± 40,67 cmH2O; Párea = 103,85 ± 36,63 cmH2O; Pplatô = 98,93 ± 32,10 cmH2O; Pmédia = 104,43 ± 36,74 cmH2O; p < 0,0083 entre todos). Houve baixa concordância entre a maior parte das medidas, sendo as diferenças entre os parâmetros maiores quanto mais elevados os valores pressóricos considerados. CONCLUSÕES: Os parâmetros avaliados não são intercambiáveis, sendo as diferenças entre eles maiores à medida que valores pressóricos mais elevados são atingidos.

OBJECTIVE: To investigate four parameters defining maximal respiratory pressures and to evaluate their correlations and agreements among those parameters for the determination of MIP and MEP. METHODS: This was a cross-sectional study involving 49 healthy, well-nourished males and females. The mean age was 23.08 ± 2.5 years. Measurements were carried out using a pressure transducer, and the estimated values for the parameters peak pressure (Ppeak), plateau pressure (Pplateau), mean maximal pressure (Pmean), and pressure according to the area (Parea) were determined with an algorithm developed for the study. To characterize the study sample, we used descriptive statistics, followed by repeated measures ANOVA and Bonferroni post hoc test or by the Friedman test and the Wilcoxon post hoc test, as well as by Pearson's or Spearman's correlation coefficients, depending on the normality of the data. The agreement between the variables was assessed with Bland & Altman plots. RESULTS: There were significant differences among all of the parameters studied for MIP (Ppeak = 95.69 ± 27.89 cmH2O; Parea = 88.53 ± 26.45 cmH2O; Pplateau = 82.48 ± 25.11 cmH2O; Pmean = 89.01 ± 26.41 cmH2O; p < 0.05 for all) and for MEP (Ppeak = 109.98 ± 40.67 cmH2O; Parea = 103.85 ± 36.63 cmH2O; Pplateau = 98.93 ± 32.10 cmH2O; Pmean = 104.43 ± 36.74 cmH2O; p < 0.0083 for all). Poor agreement was found among almost all of the parameters. Higher pressure values resulted in larger differences between the variables. CONCLUSIONS: The maximal respiratory pressure parameters evaluated do not seem to be interchangeable, and higher pressure values result in larger differences among the parameters.

A Ioga é um sistema filosófico milenar originário da Índia, cujo objetivo principal é o desenvolvimento da união entre corpo e mente, através de exercícios, respiração e meditação, visando o bem-estar físico e mental. O estudo teve como objetivo avaliar os efeitos da prática regular de Hatha Ioga sobre a função ventilatória de mulheres saudáveis. Estudo transversal controlado, no qual participaram 25 mulheres, sendo os grupos IOGA e controle, compostos por 13 e 12 voluntárias, respectivamente. As voluntárias do grupo IOGA praticavam Hatha Ioga há pelo menos seis meses. A frequência respiratória foi avaliada através da contagem em um minuto; as pressões inspiratórias e expiratórias máximas foram avaliadas através da manovacuometria; as mobilidades torácicas axilar e xifóidea através da cirtometria; a capacidade vital forçada e o pico de fluxo expiratório através da espirometria. Foram encontradas diferenças significativas entre os grupos apenas com relação à frequência respiratória e mobilidade xifóidea. Pode-se concluir que a prática regular de Hatha Ioga mostrou-se capaz de influenciar positivamente a frequência respiratória e a mobilidade xifóidea.

Yoga is an ancient philosophic system that originated in India and whose main objective is the development of the union of mind and body through exercise, respiration and meditation. The objective of this study was to assess the effects of regular practice of Hatha Yoga on the respiratory function of healthy women. A controlled cross-sectional study was conducted on 25 female volunteers divided into two groups: Yoga (n=13) and control (n=12). The volunteers of the Yoga group had practiced Hatha Yoga for at least 6 months. The respiratory rate was counted over one minute. Maximal inspiratory and expiratory pressures were assessed with a manovacuometer. Axillary and xiphoid mobilities were assessed by cyrtometry. Chest expansion at the axillary and xiphoid levels was measured by cyrtometry. Forced vital capacity and peak expiratory flow were assessed by spirometry. Significant differences between groups were only observed for respiratory rate and xiphoid mobility. In conclusion, regular practice of Hatha Yoga had a positive impact on respiratory rate and xiphoid mobility in the population studied.

O objetivo do presente estudo foi avaliar, em um grupo de mulheres saudáveis, os efeitos da prática regular de Ioga sobre a qualidade de vida. Tratou-se de um estudo transversal controlado, no qual participaram 25 mulheres saudáveis, com idades entre 20 e 59 anos, divididas nos grupos IOGA (praticantes há pelo menos seis meses) e CONTROLE (sedentárias), compostos respectivamente por 13 e 12 voluntárias. A qualidade de vida foi avaliada utilizando-se o questionário SF-36. Para análise estatística foram realizados o teste t independente e as provas não paramétricas de Mann-Whitney. O grupo IOGA apresentou pontuação significativamente mais elevada no domínio "dor" e no escore físico geral (p < 0,05) do questionário SF-36. Concluímos que, na população avaliada, a prática regular de Hatha Ioga demonstrou ser capaz de influenciar positivamente determinados aspectos da qualidade de vida.

The aim of the present study was to evaluate, in a group of healthy women, whether the regular practice of Hatha Yoga could have an impact on their quality of life. It was a transversal controlled study, composed by 25 women, aged between 20 and 59 years, divided in two groups: YOGA (at least six months of Hatha Yoga practice) and CONTROL (non-active women), composed by 13 and 12 volunteers, respectively. The quality of life was assessed through the SF-36 questionnaire. The statistical procedures included the independent t test and the Mann-Whitney tests. The YOGA group presented higher scores in the "body pain" domain and in the general physical score (p < 0,05). It can be concluded that, in the assessed population, the regular practice of Hatha Yoga has positively influenced some aspects of the quality of life.
