OBJETIVO: Avaliar as mudanças terapêuticas, dadas por decisão médica, relativas ao tratamento do glaucoma primário de ângulo aberto em um centro de referência e a possível relação entre a persistência do tratamento e progressão da doença em casos controlados com medicação. MÉTODOS: Uma revisão de prontuários foi realizada em 65 pacientes com glaucoma primário de ângulo aberto encaminhados a um hospital terciário Os seguintes dados clínicos foram analisados: primeira medicação instituída, persistência com o tratamento inicial, melhor acuidade visual corrigida, índice ''desvio médio'' do campo visual, relação escavação/disco óptico e pressão intraocular. Os pacientes foram classificados em quatro categorias, a fim de se verificar a evolução clínica. RESULTADOS: A média do número de visitas/ano foi de 4,4 ± 3,5 e o período de seguimento foi de 40,7 ± 22,8 meses. A média de tempo persistência foi de 12,9 ± 13,9 meses. Em seis e doze meses, respectivamente, 39,1% e 62,5% dos pacientes tinham interrompido o regime terapêutico inicialmente previsto, principalmente pela adição (42%) ou mudança (26%) do esquema terapêutico. Treze pacientes (21%) evoluíram para uma pior categoria de glaucoma primário de ângulo aberto, no entanto, apesar desta tendência, nenhuma correlação significativa foi encontrada entre baixa persistência e agravamento do glaucoma primário de ângulo aberto. CONCLUSÕES: As taxas de persistência com o esquema terapêutico inicial foram baixas, quando medidas através das mudanças observadas por decisões médicas, durante o curso do tratamento. Uma melhor decisão terapêutica inicial é crítica, a fim de poder se oferecer um tratamento mais estável e eficaz para o glaucoma primário de ângulo aberto.

PURPOSE: To evaluate the initial medical decision regarding primary open-angle glaucoma (POAG) treatment in a referral center, and to assess the relationship between the persistence of treatment and the disease progression in cases managed exclusively with medication. METHODS: A retrospective chart review was performed for 65 patients with primary open-angle glaucoma referred to a tertiary hospital. The following clinical data were analyzed: initial medication, persistence of treatment, best corrected visual acuity, visual field mean deviation index, cup/disc ratio, and intraocular pressure. Patients were classified into four categories in order to verify the clinical evolution. RESULTS: The mean number of visits/ year was 4.4 ± 3.5, and the follow-up period was 40.7 ± 22.8 months. Mean persistence time was 12.9 ± 13.9 months. By six and twelve months, respectively, 39.1% and 62.5% of patients had discontinued the initially prescribed regimen, mainly by adding to (42%) or changing (26%) the course of treatment. Thirteen patients (21%) were reclassified to a worse category of primary open-angle glaucoma, however, despite this trend, no significant correlation was found between shorter persistence and primary open-angle glaucoma worsening. CONCLUSIONS: Persistence rates with initial therapy schemes were low, as measured by medical decisions to change the course of treatment. Therefore, improvement in the initial medical decision is crucial in order to offer a more stable and effective treatment for primary open-angle glaucoma.

OBJETIVOS: Avaliar aspectos epidemiológicos dos traumas oculares num pronto-socorro de referência. MÉTODOS: O estudo teve dois grupos distintos de indivíduos: no primeiro (A) composto por vítimas de trauma ocular, em que 1.483 pacientes atendidos consecutivamente ao longo de 2003 tiveram os dados epidemiológicos analisados, e o segundo (B), em que 133 pacientes tiveram aspectos clínicos e fatores de risco e de proteção analisados consecutivamente por meio de questionário padrão, de 17 de maio a 28 de junho de 2004. RESULTADOS: No levantamento A, a maioria dos pacientes era do sexo masculino (1.314 ou 89%). Os traumas por corpos estranhos (CE) da superfície ocular foram os mais comuns, respondendo por 863 (58%) casos. O levantamento B mostrou que a proteção ocular foi usada em apenas 17% (22) dos pacientes. Os acidentes geralmente ocorreram no local de trabalho 70% (93), e o domicílio foi o segundo local de maior freqüência (22%). Ainda no levantamento B, 34% dos entrevistados tinham acidentes oculares prévios. CONCLUSÕES: O perfil dos pacientes vítimas de trauma ocular no presente estudo observou a maior ocorrência entre homens e de acidentes por corpos estranhos. O uso de proteção ocular ainda é incipiente e, por outro lado, a recorrência de trauma é considerável. Uma estratégia contínua junto à população, de forma preventiva e educativa com especial atenção ao ambiente de trabalho e doméstico, é necessária para reduzir a ocorrência de trauma ocular.

PURPOSE: To evaluate epidemiological aspects of eye injuries in a reference emergency center. METHODS: The study had two surveys: in the first (A) victims of ocular trauma where 1,483 patients admitted along 2003 had their epidemiological data analyzed and the second (B), where 133 patients had clinical aspects and risk and protective factors analyzed by a standard questionnaire between May and June 2004. RESULTS: In survey A, most patients were males (1,314 or 89%). Ocular surface foreign body was the most frequent occurrence with 863 (58%) cases. In survey B it was shown that ocular protection was used only by 17% (22) patients. The accidents occurred more frequently at the workplace 70% (93), followed by the home (22%). In survey B, 34% of the patients had previous accidents. CONCLUSIONS: The profile of patients victims of ocular trauma shows that there is a higher frequency of males and accidents involving cornea foreign body. The use of protective glasses is incipient and recurrence of this lesion is considerable. A preventive and educational strategy among the population with special focus on workplaces and homes is necessary to reduce ocular trauma occurrence.

OBJETIVO: Calcular a prevalência do aleitamento materno entre crianças menores de dois anos de idade, residentes na região Noroeste de Campinas, São Paulo. MÉTODOS: Estudo transversal foi conduzido, em 2001, junto a 4 103 crianças, questionando sobre data de nascimento, sexo, alimentação e serviço de saúde utilizado. O questionário foi aplicado em 42 postos de vacinação durante a Campanha Nacional de Vacinação contra Poliomielite (Campólio). A dieta foi classificada em amamentação exclusiva, predominante, continuada e aleitamento artificial. RESULTADOS: No primeiro semestre, a prevalência de aleitamento materno exclusivo foi de 31,6% e a de aleitamento total 74,5%. Das crianças com idade entre 6 e 12 meses, 38,0% recebiam leite materno. No segundo ano, a prevalência de aleitamento materno foi reduzida para 22,1%. O aleitamento materno exclusivo passou de 72,2% aos 7 dias de idade para 53,8% aos 15 dias, 33,3% aos 3 meses, 10,0% aos 4 meses e 5,7% aos 6 meses. A prevalência do aleitamento materno total foi de 100,0% aos 7 dias; 79,1% aos 3 meses; 54,3% aos 6; 34,4% aos 12 meses; 26,1% aos 18 e zero aos 24 meses. A mediana de amamentação exclusiva foi de 67 dias e a de amamentação total foi de 6,6 meses. Das crianças vinculadas ao Sistema Único de Saúde, 42,2% receberam aleitamento materno, em contraste com 34,4% das usuárias de serviços privados (p<0,00001). Após controle pela idade, o efeito protetor do Sistema Único de Saúde se manteve, só desaparecendo quando se incluiam as variáveis renda familiar e escolaridade materna. CONCLUSÃO: A prevalência de aleitamento materno está abaixo das recomendações oficiais.

OBJECTIVE: To calculate the prevalence of breast-feeding among children less than two years old, in the northwest region of the city of Campinas, São Paulo, Brazil. METHODS: A survey was conducted in 2001, with 4 103 children, querying about birth date, gender, feeding practices and health services routinely used. The questionnaire was applied at the 42 public immunization centers during the 2001 National Campaign against Polyomielites. The child's diet was categorized as exclusive breast-feeding, predominant breast-feeding, continued breast-feeding and bottle-feeding. RESULTS: The prevalences of the exclusive and the total breast-feeding in the first semester were, respectively, 31.6%, and 74.5%. Among children aged from six to twelve months, 38.0% were breast-fed. In the second year, the prevalence of breast-feeding was reduced to 22.1%. Exclusive breast-feeding changed from 72.2% at the 7th day of life, to 53.8% at the 15th day, to 33.3% at the 3rd month, to 10.0% at the 4th, and to 5.7% at the 6th month. The total breast-feeding prevalence was 100.0% at the 7th day, 79.1% at the 3rd month, 54.3% at the 6th, 34.4% at the 12th, 26.1% at the 18th, and none at the 24th month. Exclusive and the total breast-feeding medians were, respectively, 67 days and 6.6 months. Among children who attended the public health system, 42.2% were breast-fed, against 34.4% of those who attended a private service (p<0.00001).The protecting effect of the Public Health Service remained, when the statistic model was adjusted for age; conversely, it vanished, when variables like income and maternal education were included in the model. CONCLUSION: The prevalence of breast-feeding is below official recommendations.
