ABSTRACT Purpose: To evaluate the presence of congenital hypertrophy of the retinal pigment epithelium in a large family affected by familial adenomatous polyposis and identify the causative mutation in the adenomatous polyposis coli gene. Thus, we aimed to determine the significance of congenital hypertrophy of the retinal pigment epithelium as a phenotypic marker of the disease. Methods: A family consisting of 95 individuals was evaluated. Among these, 45 individuals were randomly selected by convenience sampling method to undergo ophthalmological evaluation. A funduscopic exam, including slit lamp and indirect ophthalmoscopy, were performed in the selected patients. In those with retinal lesions, a retinography was obtained. The adenomatous polyposis coli gene was sequenced in one affected family member to identify the pathogenic mutation. Once the variant was identified, six undiagnosed family members were tested for the mutation via capillary electrophoresis sequencing. Results: Congenital hypertrophy of the retinal pigment epithelium was observed in 13 (28.9%) of the 45 individuals evaluated. Of these, nine patients were confirmed to have familial adenomatous polyposis (via colonoscopy or molecular testing). However, four patients had not been investigated. Of the 32 (71.1%) family members without the lesion, 14 did not have familial adenomatous polyposis and 18 were yet to be evaluated. The lesions were bilaterally present and exhibited a peculiar fish-tail shape in all the evaluated individuals. Adenomatous polyposis coli gene sequencing revealed a pathogenic variant c.4031del. (Ser1344*), in heterozygosity (49.27%), in exon 16. Conclusions: The study findings confirmed the significance of congenital hypertrophy of the retinal pigment epithelium as a phenotypic marker for familial adenomatous polyposis. Furthermore, it is an effective first-line screening method for at risk family members of such patients. Resumen La salud mental y la calidad de vida de los estudiantes se vieron perjudicadas por los efectos de la pandemia de COVID-19. El objetivo de este estudio es evaluar la calidad de vida de los estudiantes universitarios durante la pandemia. Se trata de un estudio transversal realizado en das universidades públicas brasilehas en 2020. La información se recolectó, mediante la utilización de dos cuestionarios: cuestionario sociodemográfico y WHOQOL-bref. Participaron un total de 144 estudiantes, la mayoría menores de 23 ahos (57%), mujeres (78%), autodeclaradas morenas (46%) y sin pareja (87%). La mayoría declaró dormir menos de 8 horas diarias, consideró buena su calidad de vida y se mostró satisfecha con su salud. Abstract Several studies have shown COVID-19 has affected the mental health and quality of life of students. This study aimed to assess the quality of life of students in two Brazilian public universities in 2020, using a cross-sectional study. The profile of the participants was delineated from a sociodemographic questionnaire, and the quality of life was assessed by the WHOQOL-bref. A total of 144 students were assessed, in which most of them were under 23 (57%), female (78%), self-declared brown (46%) and without a partner (87%). Most of them declared to sleep less than 8 hours a day, considered their quality of life to be good, and were satisfied with their health. The social relationships domain had the highest average, followed by the environment, physical and psychological domains. The perception of the student's quality of life was considered good, and the factors associated were sex, family income, and number of sleeping hours. Resumo Diversos estudos têm demonstrado que a pandemia do COVID-19 tem prejudicado a saúde mental e a qualidade de vida dos estudantes. Assim, o objetivo deste estudo foi avaliar a qualidade de vida dos estudantes em duas universidades públicas brasileiras em 2020, utilizando um estudo transversal. O perfil dos participantes foi delineado a partir de um questionário sociodemográfico e a qualidade de vida foi avaliada pelo WHOQOL-bref. Um total de 144 estudantes participaram, sendo a maioria com idade inferior a 23 anos (57%), do sexo feminino (78%), autodeclarada pardos (46%) e sem companheiro(a) (87%). A maioria declarou dormir menos de 8 horas/dia, considerou sua qualidade de vida como sendo boa e estavam satisfeitos com a sua saúde. RESUMO Objetivo: Relatar o caso de uma jovem que apresentava um fenótipo grave de displasia mandibuloacral tipo A (MADA) caracterizado por alterações osteolíticas proeminentes e defeitos ectodérmicos, associados a uma rara mutação homozigótica missense no gene LMNA (c.1579C>T). Descrição do caso: Uma menina de seis anos foi avaliada durante hospitalização apresentando os seguintes sinais dismórficos: alopecia subtotal, fácies dismórfica com olhos proeminentes, micrognatia e retrognatia acentuada, nariz pequeno e adunco, dentes apinhados e lábios finos, lipodistrofia generalizada, ombros estreitos e inclinados, rigidez articular e reabsorção óssea nas falanges terminais. Ao exame dermatológico, observou-se pele atrófica, perda da elasticidade cutânea, hiperceratose, calcinose dérmica e manchas hiperpigmentadas e hipocrômicas. Exames radiológicos realizados mostraram ausência de côndilos mandibulares bilaterais, reabsorção da clavícula com massa óssea amorfa local confluindo com as escápulas, articulações do ombro com subluxação e displasia óssea severa, com displasia coxofemoral, osteopenia e calcificações subcutâneas. Comentários: MADA é uma doença autossômica recessiva rara causada por mutações no gene LMNA. Caracteriza-se por deformidades craniofaciais, anomalias esqueléticas, alterações cutâneas, lipodistrofia em determinadas regiões do corpo e envelhecimento precoce. ABSTRACT Objective: To report the case of a girl presenting a severe phenotype of mandibuloacral dysplasia type A (MADA) characterized by prominent osteolytic changes and ectodermal defects, associated with a rare homozygous LMNA missense mutation (c.1579C>T). Case description: A 6-year-old girl was evaluated during hospitalization exhibiting the following dysmorphic signs: subtotal alopecia, dysmorphic facies with prominent eyes, marked micrognathia and retrognathia, small beaked nose, teeth crowding and thin lips, generalized lipodystrophy, narrow and sloping shoulders, generalized joint stiffness and bone reabsorption in the terminal phalanges. In dermatological examination, atrophic skin, loss of cutaneous elasticity, hyperkeratosis, dermal calcinosis, and hyperpigmented and hypochromic patches were observed. Radiology exams performed showed bilateral absence of the mandibular condyles, clavicle resorption with local amorphous bone mass confluence with the scapulae, shoulder joints with subluxation and severe bone dysplasia, hip dysplasia, osteopenia and subcutaneous calcifications. Comments: MADA is a rare autosomal recessive disease caused by mutations in LMNA gene. Abstract Objective To evaluate hip and knee muscular function, knee patient-reported outcome measures and hop performance in patients with a clinical indication for combined ACL+ALL reconstruction surgery compared to patients with an isolated ACL reconstruction surgery indication (preoperative phase) and to a control group. Design Cross-sectional study. Methods The sample was composed of male individuals, aged between 18 and 59 years, divided into three groups (ACL, ACL+ALL and Control). Isokinetic dynamometry was performed for the flexor and extensor knee muscles and for the hip abductors and adductors. SLHT, COHT and the Lysholm score were performed. Pain, swelling, and thigh trophism were also measured. Results The study participants were 89 male individuals: 63 in the injury group and 26 in the control group. After applying the criteria for an ALL reconstruction indication, 33 patients were assigned to the ACL Group and 30 patients to the ACL+ALL Group. Regarding knee and hip muscle function, both groups presented worse results when compared to the control group, however, did not show significant differences compared to each other. Regarding the functional variables, the ACL+ALL group showed a significantly shorter distance achieved in the Crossover Hop Test than the other groups, as well as more pain during the tests. Resumo O Housing First (HF) tem se difundido no cenário internacional como uma intervenção baseada em evidência para superar a situação de rua entre pessoas com agravos de saúde mental. No Brasil, o HF tem sido adotado como referência no desenvolvimento de iniciativas voltadas para pessoas em situação de rua que fazem uso prejudicial de drogas. Estudos apontam a necessidade de maior compreensão da dinâmica implantação do modelo HF em diferentes cenários. De tal modo, utilizando o método de revisão de literatura, analisamos os fatores que facilitam e dificultam a implantação do HF. Foram selecionados 68 artigos, publicados entre 2003 e 2020, nas bases PubMed, Scopus, PsychoINFO, Embase, Lilacs e Scielo. Identificamos fatores em quatro dimensões: características da intervenção, contexto de implantação, aspectos institucionais e processo de implantação. A indisponibilidade de habitações, a falta de coordenação dos serviços necessários aos moradores e a resistência dos agentes implantadores aos princípios do HF são fatores que dificultam a implantação. Por sua vez, agentes com valores, atitudes e competências convergentes ao modelo e educação permanente aparecem como facilitadores. Apontamos a necessidade de se compreender e favorecer o processo de integração do HF aos sistemas de proteção social existentes.

In Brazil, the HF has been adopted as a reference in the development of initiatives geared to people living in the streets who make harmful use of drugs. Studies point to the need for greater understanding of the dynamics of implementation in different settings. Thus, using the literature review method, we analyzed the factors that facilitate and hinder the implementation of HF. Sixty-eight papers published from 2003 to 2020 were selected from the PubMed, Scopus, PsychoINFO, Embase, Lilacs, and Scielo databases. We identified factors in four dimensions: intervention characteristics, implementation context, institutional aspects, and implementation process. The unavailability of housing, the lack of coordination of the services needed by residents, and the resistance of implementing agents to the HF principles are factors that hinder the implementation. In turn, agents with values, attitudes, and skills converging with the model and continuing education appear as facilitators. We point out the need to understand and favor the HF integration process in the existing social protection systems.

SUMMARY OBJECTIVE: The objective of this study was to estimate the current prevalence of burnout syndrome among medical students at the Federal University of Sergipe and identify its associated factors. METHODS: This cross-sectional study was conducted with medical students randomly selected between April and June 2019. This study compared two medical schools from the same university with different teaching models: the Federal University of Sergipe, Aracaju campus, with a traditional teaching model, and the Federal University of Sergipe Lagarto campus, with a problem-based learning teaching model. An online questionnaire on the sociodemographic characteristics, personal aspects, and educational process of the participants, in addition to the Malash Burnout Inventory-Student Survey questionnaire for screening burnout syndrome, was distributed to the participants. The descriptive analysis of the data, calculation of the prevalence ratios, and multivariate analysis by logistic regression were performed. RESULTS: This study included 213 students with an average age of 23±3.77, and 50.2% of the students were male. Among the sample, 21.6% of the students met the three-dimensional criterion for burnout syndrome and 51.6% met the two-dimensional criterion. Burnout levels were higher in the students who rarely received the emotional support they needed in the program (OR 3.98), those who thought about dropping out of the undergraduate (OR 2.88), and those who considered their academic performance to be regular or weak (OR 12.1). The traditional teaching model was not a factor associated with burnout syndrome. CONCLUSIONS: The results suggest that a high prevalence of burnout syndrome is associated with psychosocial factors and the educational processes of medical students.

Resumo Iniciativas de moradia assistida para pessoas em situação de rua vêm sendo desenvolvidas no Brasil, na política de drogas brasileira, tendo como referência a abordagem Housing First (Moradia Primeiro). Nesse contexto, é relevante analisar as influências das características locais no grau de implantação dessa modalidade de intervenção no cenário nacional. Para isso, realizamos análise de implantação, por meio de estudo de caso de projeto piloto de moradia assistida desenvolvido em Brasília-DF. Por meio de documentos oficiais, observação participante nos ambientes reais e virtuais, sistematização das perspectivas de trabalhadores e gestores responsáveis pela intervenção e modelização da proposta, caracterizamos a iniciativa. Em seguida, esta foi confrontada com critérios extraídos da proposta Housing First. Os resultados indicam a adequação da intervenção ao princípio fundamental de colocar a moradia em primeiro lugar, desvinculada de tratamento e abstinência, utilizando para isso soluções e adaptações locais. Entretanto, carências no processo de gestão do cuidado integral e precariedade da rede de políticas sociais aparecem como desafios à transposição do componente de inserção social que constitui a proposta. A interface entre os setores Saúde e Assistência precisa ser mais bem analisada e a matriz inspiradora do projeto melhor conhecida em seu contexto de origem.

Abstract Supportive housing interventions based on the Housing First approach have been implemented in Brazil as part of drug policy. It is relevant to analyze the influence of local characteristics on the implementation degree of this kind of intervention in this national context. We conducted an implementation evaluation case study of a supportive housing Project, in Brasília/DF. To describe the initiative we used official documents, participant observation of real and virtual environments, systematization of the perception of project workers/managers and logic model. The project description was then compared with criteria based on the HF proposal parameters. The results indicate the appropriateness of the intervention to the fundamental principle of putting housing in the first place, using local solutions and adaptations. However, shortcomings in the management of comprehensive care and disintegration of the social policy network challenge the implementation of the social integration component of the proposal. Health and Social Care interface needs to be better analyzed and the original model better known in its context of origin.

Resumo Partindo do pressuposto de que haveria no plano teórico certa correspondência à estagnação numérica e qualitativa de opções e ofertas de moradia voltadas às pessoas com transtornos mentais graves no Brasil, analisamos, por meio de revisão bibliográfica e bibliometria, como o tema vem sendo abordado em publicações nacionais indexadas nas bases Lilacs e Scopus, entre os anos 2000 e 2017. Constatamos que a distribuição regional brasileira dos autores é desigual e a produção científica ainda é numericamente limitada, predominantemente descritiva e focada nos processos de trabalho e suporte. Os desenhos metodológicos variam pouco e os referenciais teóricos são oriundos de textos seminais que embasaram o início da reforma psiquiátrica brasileira. Entendemos que os achados deste estudo permitem confirmar o pressuposto inicial de que o debate acerca da moradia para pessoas com transtornos mentais graves carece de novas abordagens teóricas e metodológicas, ao contrário do que indicam normativas recém-divulgadas pelo Ministério da Saúde nesse sentido.

Abstract Based on the assumption that there would be a certain correspondence in the theoretical plan to the numerical and qualitative stagnation of housing options and offers aimed at people with severe mental disorders in Brazil, we analyzed, through bibliographic review and bibliometrics, how the topic has been addressed in publications national indexed in the Lilacs and Scopus databases, between the years 2000 and 2017. We found that the Brazilian regional distribution of the authors is uneven, and the scientific production is still numerically limited, predominantly descriptive and focused on work and support processes. The methodological designs vary little, and the theoretical references are derived from seminal texts that supported the beginning of the Brazilian psychiatric reform. We understand that the findings of this study allow us to confirm the initial assumption that the debate about housing for people with severe mental disorders lacks new theoretical and methodological approaches, contrary to what is indicated by recently released regulations by the Ministry of Health in this regard.

Resumo Iniciativas de moradia assistida para pessoas em situação de rua vêm sendo desenvolvidas no Brasil, na política de drogas brasileira, tendo como referência a abordagem Housing First (Moradia Primeiro). Abstract Based on the assumption that there would be a certain correspondence in the theoretical plan to the numerical and qualitative stagnation of housing options and offers aimed at people with severe mental disorders in Brazil, we analyzed, through bibliographic review and bibliometrics, how the topic has been addressed in publications national indexed in the Lilacs and Scopus databases, between the years 2000 and 2017. We found that the Brazilian regional distribution of the authors is uneven, and the scientific production is still numerically limited, predominantly descriptive and focused on work and support processes. The methodological designs vary little, and the theoretical references are derived from seminal texts that supported the beginning of the Brazilian psychiatric reform. We understand that the findings of this study allow us to confirm the initial assumption that the debate about housing for people with severe mental disorders lacks new theoretical and methodological approaches, contrary to what is indicated by recently released regulations by the Ministry of Health in this regard.

ABSTRACT Objective to characterize the knowledge about the use of potentially dangerous drugs among hospital health care nurses. Methods quantitative, descriptive-exploratory research, with the application of a validated questionnaire, translated and adapted in Brazil, on potentially dangerous medications, with 26 nursing assistants in a university hospital. Univariate and bivariate descriptive statistical analysis of the data was performed using the Statistical Package for Social Science, version 20. Scores ≥70% were considered as sufficient knowledge and insufficient knowledge scores <70%. Results in the first domain of the questionnaire, called Medication Administration, the average percentage of correct answers was 64.6%. In the second domain, Clinical procedures, an average of 53.7% of correct answers was found. The use of electrolytes, chemotherapy and insulin were the main weaknesses in the knowledge of professionals. Conclusion insufficient knowledge about the administration and clinical procedures of potentially dangerous drugs was identified among hospital care nurses in this scenario.

RESUMO Objetivo caracterizar o conhecimento sobre uso de medicamentos potencialmente perigosos entre enfermeiros da assistência hospitalar à saúde. Métodos pesquisa quantitativa, descritiva-exploratória, com aplicação de questionário validado, traduzido e adaptado no Brasil, sobre medicamentos potencialmente perigosos, com 26 enfermeiros assistenciais em hospital universitário. Realizou-se análise estatística descritiva univariada e bivariada dos dados, por meio do Statistical Package for Social Science, versão 20. Consideraram-se como conhecimento suficiente escores ≥70% ou conhecimento insuficiente escores <70%. Resultados no primeiro domínio do questionário, denominado Administração dos medicamentos, o percentual médio de acertos foi de 64,6%. No segundo domínio, Procedimentos clínicos, encontrou-se média de 53,7% de acertos. O uso dos eletrólitos, quimioterápicos e de insulina foram as principais fragilidades no saber dos profissionais. Conclusão identificou-se conhecimento insuficiente sobre a administração e os procedimentos clínicos de medicamentos potencialmente perigosos entre os enfermeiros assistenciais hospitalares neste cenário.

Marco contextual: El principio de las unidades de cuidados de larga duración integrados (unidades de cuidados integrados - UCCI) es la readaptación y reintegración familiar y social. Objetivo: Conocer las perspectivas de los cuidadores informales sobre el momento del alta hospitalaria en cuidados de larga duración de sus familiares con demencia. Metodología: Se entrevistó a 15 cuidadores (edad media de 60 años) y se analizaron los datos cualitativamente mediante NVivo. Resultados: A pesar de la preocupación y el temor al momento de recibir el alta, la mayoría de los participantes no tomaron ninguna medida. Cuando se les preguntó qué harían si su familiar recibiera el alta de la UCCI, respondieron que considerarían el domicilio (n = 5) o una residencia para personas mayores (n = 10). Esta decisión tiene en cuenta uno o más motivos, a saber, los factores ambientales, la falta de disponibilidad del cuidador, la relación de proximidad, las cuestiones económicas y las cuestiones familiares. Conclusión: Los motivos que influyen en la integración y la prestación de cuidados al paciente después del alta deben ser considerados por los profesionales, con el fin de apoyar, formar y capacitar mejor a los cuidadores informales, medidas que deberían formar parte de una atención rutinaria.

Enquadramento: As unidades de cuidados continuados integrados (UCCI) têm como princípio a readaptação e reinserção familiar e social. Objetivo: Conhecer as perspetivas de cuidadores informais sobre o momento da alta do internamento em cuidados continuados dos seus familiares com demência. Metodologia: Quinze cuidadores (média de idades 60 anos) foram entrevistados e os dados analisados qualitativamente com recurso ao NVivo. Resultados: Apesar da preocupação e medo relativamente ao momento de alta, a maioria dos participantes não tomou qualquer ação. Quando questionados sobre o que fariam se o seu familiar tivesse alta da UCCI, referem considerar o domicílio (n = 5) ou uma estrutura residencial para pessoas idosas (n = 10). Esta decisão tem em conta um ou mais motivos, nomeadamente fatores ambientais, indisponibilidade do cuidador, relação de proximidade, questões económicas e questões familiares. Conclusão: Os motivos que pesam na integração e prestação de cuidados ao paciente após a alta devem ser considerados pelos profissionais, no sentido de um melhor apoio, formação e capacitação dos cuidadores informais, medidas que devem ser partes constituintes de uma rotina de cuidados.

Background: The purpose of long-term integrated care units is the readaptation and reintegration into family and social life. Objective: To understand informal caregivers' perspectives on the discharge of their relatives with dementia from long-term care facilities. Methodology: Fifteen caregivers (mean age of 60 years) were interviewed, and data were qualitatively analyzed using NVivo. Results: Despite the concern and fear about discharge, most participants did not take any action. When questioned about what they would do if their family member was discharged from the unit, they mentioned that they would return home (n = 5) or go to a residential facility for older people (n = 10). This decision is justified by one or more reasons, namely environmental factors, caregiver unavailability, relationship of proximity, economic issues, and family issues. Conclusion: Healthcare professionals should consider the reasons influencing the integration and delivery of patient care after discharge to better support, train, and empower informal caregivers. These measures should be part of a care routine.

Abstract Objective To analyze the professional militancy of nurses in the face of the precariousness of the work, presented by the Bahian print media in the 1970s and 1980s. Methods Historical-documentary research, with a qualitative approach, whose source was the Bahia newspaper A Tarde. The time continuum includes historical landmarks, such as the: creation of the Federal Nursing Council and its regional sections (1973); creation of the Union of Nurses of the State of Bahia (1981); and the approval of the Professional Exercise Law (1986). The national context was the period of the military dictatorship, which adopted a model of social protection and moved towards a political opening. The search terms “Enfermeira” and “Enfermeiro” were used, resulting in 24 reports. The content analysis in the thematic modality supported the organization. Results The professional nurse militancy confronted by the precariousness of work was present in the media through the representative entities, evidenced by the following aspects: devaluation of the workforce, marked by low remuneration and replacement of nurses by personnel without adequate qualification; work overload, evidenced by understaffing and poor qualification; and the search for legal determination of the working day and minimum wage. Conclusion Nurses used the media to denounce and make society aware of the determinants of the precariousness of work, but the effective actions of combat were minimally approached. The militant activity was concentrated in the scope of the representative entities, but did not demonstrate a collective movement of the nurses.

Resumen Objetivo Analizar la militancia profesional de enfermeras frente a la precarización del trabajo presentada por los medios de comunicación impresos del estado de Bahía en la década de los setenta y ochenta. Métodos Investigación histórica y documental, con enfoque cualitativo, cuya fuente fue el periódico bahiano A Tarde. El recorte temporal abarca marcos históricos como la creación del Consejo Federal de Enfermería y sus secciones regionales (1973), la creación del Sindicato de Enfermeros del Estado de Bahía (1981) y la aprobación de la Ley de Ejercicio Profesional (1986). El contexto nacional era el período de la dictadura militar, que adoptaba un modelo de protección social y caminaba hacia la apertura política. Se utilizaron los términos de búsqueda “enfermera” y “enfermero”, que dio como resultado 24 reportajes. La organización de los datos se basó en el análisis de contenido en la modalidad temática. Resultados La militancia profesional de enfermeras frente a la precarización del trabajo se hizo presente en los medios de comunicación a través de las entidades representativas, evidenciada por los siguientes aspectos: desvalorización de la fuerza de trabajo, marcada por la baja remuneración y reemplazo de enfermeras por personal sin la cualificación adecuada; sobrecarga de trabajo, demostrada en el subdimensionamiento y cualificación deficiente; y la búsqueda de determinación legal de la jornada de trabajo y del piso salarial. Conclusión Las enfermeras utilizaron los medios de comunicación para denunciar y concientizar a la sociedad sobre los determinantes de la precarización del trabajo, pero las acciones efectivas del combate fueron poco abordadas. La actuación militante estuvo concentrada en el ámbito de las entidades representativas y no se observó un movimiento colectivo de enfermeras.

Resumo Objetivo Analisar a militância profissional de enfermeiras frente à precarização do trabalho apresentada pela mídia impressa baiana nas décadas de 1970 e 1980. Métodos Pesquisa histórico-documental, com abordagem qualitativa, cuja fonte foi o jornal baiano A Tarde. O recorte temporal abarca marcos históricos como a criação do Conselho Federal de Enfermagem e suas seções regionais (1973); a criação do Sindicato dos Enfermeiros do Estado da Bahia (1981); e a aprovação da Lei do Exercício Profissional (1986). O contexto nacional era o período da ditadura militar, que adotava um modelo de proteção social e caminhava para a abertura política Utilizou-se os termos de busca “Enfermeira” e “Enfermeiro”, resultando em 24 reportagens. A análise de conteúdo na modalidade temática embasou a organização dos dados. Resultados A militância profissional de enfermeiras frente à precarização do trabalho se fez presente na mídia por meio das entidades representativas, evidenciada pelos seguintes aspectos: Desvalorização da força de trabalho, marcada pela baixa remuneração e substituição de enfermeiras por pessoal sem qualificação adequada; Sobrecarga de trabalho, evidenciada pelo subdimensionamento e qualificação deficiente; e busca por determinação legal da jornada de trabalho e do piso salarial. Conclusão As enfermeiras utilizaram-se da mídia para denunciar e conscientizar a sociedade sobre os determinantes da precarização do trabalho, porém, as ações efetivas de combate foram pouco abordadas. A atuação militante esteve concentrada no âmbito das entidades representativas, não sendo evidenciado um movimento coletivo das enfermeiras.
