SUMMARY Objective: The objective of this study is to update scientific knowledge about OPMDs, emphasizing epidemiological data, clinical appearance, symptomatology, differential diagnosis and therapeutic management, as well as highlighting the role of the general dentist in the clinical examination. Material and methods: A literature search was carried out, making use of the keywords “Leukoplakia”, “Erythroplasia”, “Cheilitis”, “Nevus, Pigmented”, “Oral Submucous Fibrosis” and “Lichen Planus”. Prioritizing articles published in the last 5 years on the scientific databases “Medline”, “Pubmed” and “SciELO”. Results: As a result of the search, 165 articles were obtained, which were carefully reviewed to preserve only those that were dedicated to describe, compare or treat lesions with malignant potential, including relevant data such as epidemiology, clinical appearance, symptomatology, differential diagnosis and management. therapeutic. In this way, the study was limited to 41 articles. Conclusions: By taking preventive measures, as well as through a clinical examination of soft tissues, early detection of potentially malignant disorders can be facilitated. The removal of suspicious lesions or a control of their carcinogenic conditions can largely prevent the development of malignant tumors.

RESUMEN Objetivo: El objetivo de este estudio es actualizar los conocimientos sobre Desordenes Orales Potencialmente Malignos, dando énfasis a datos epidemiológicos, apariencia clínica, sintomatología, diagnóstico diferencial y manejo terapéutico, así como también resaltar el rol del odontólogo general en el examen clínico. Material y métodos: Fue realizada una búsqueda de la literatura en las bases de datos científicas “Medline”, “Pubmed” y “SciELO”, utilizando las palabras clave “Leukoplakia”, “Erythroplasia”, “Cheilitis”, “Nevus, Pigmented”, “Oral Submucous Fibrosis”, “Lichen Planus”. La búsqueda fue orientada hacia artículos de revisión, estudios clínicos y reportes de caso, priorizando los artículos publicados en los últimos 5 años y sin hacer distinciones por el idioma. Resultados: Como resultado de la búsqueda se obtuvieron 165 artículos, los cuales fueron revisados cuidadosamente para conservar solo aquellos que se dedicaran a describir, comparar o tratar desordenes con potencial de malignidad, incluyendo datos de relevancia como epidemiología, apariencia clínica, sintomatología, diagnóstico diferencial y manejo terapéutico. De esta manera, el estudio se limitó a 41 artículos. Conclusiones: Al adoptar medidas preventivas, así como un meticuloso examen clínico sobre tejidos blandos, se puede facilitar la detección temprana de Desordenes Orales Potencialmente Malignos. La eliminación de lesiones sospechosas o un control de las condiciones cancerígenas puede evitar en gran mayoría el desarrollo de tumores malignos.

Resumo Introdução O MTA é um importante material de uso na odontologia, com boas propriedades biológicas. As respostas experimentais aos cimentos endodônticos que utilizam este composto no selamento e preenchimento do sistema de canais radiculares, aliados a uma boa instrumentação e limpeza, podem ser importantes no processo de reparo periapical. Objetivo Avaliar in vitro a profundidade de penetração de dois diferentes cimentos endodônticos nos túbulos dentinários e a infiltração bacteriana em obturações com os cimentos AH Plus e MTA Fillapex. Material e método 34 pré-molares monorradiculados humanos foram selecionados, suas coroas removidas e suas raízes padronizadas em 16 mm, a seguir foram, então, divididos aleatoriamente em dois grupos de 15 espécimes e instrumentados com o sistema rotatório Pro Taper. Um grupo foi obturado com o cimento AH Plus (Grupo AH) e outro com MTA Fillapex (Grupo FI). Nestes grupos, avaliaram-se a infiltração coronária de bactérias para Enterococcus Faecalis e a profundidade de penetração nos túbulos dentinários. Para a aferição da infiltração bacteriana, usou-se o teste exato de Fisher; para a penetração intratubular, usou-se o teste-t. Resultado Para a infiltração bacteriana, considerando o número de amostras infiltradas para AH e MTA Fillapex, houve diferença estatística significativa (p=0,0341). Na avaliação da penetração intratubular, a diferença entre os dois grupos de cimentos foi considerada estatisticamente significativa (p=0,0006). Conclusão Ambos os cimentos apresentaram infiltração bacteriana, sendo que o MTA Fillapex teve pior desempenho. A capacidade de penetração nos túbulos dentinários do AH Plus foi significativamente maior que a do MTA Fillapex.

Abstract Introduction The MTA is an important use in dentistry material with good biological properties. The experimental responses to sealers using this compound in the sealing and filling of the root canal system, combined with a good instrumentation and cleaning, may be important in the periapical healing process. Objective To evaluate in vitro penetration depth of two different sealers in dentinal tubules and bacterial infiltration in fillings with AH Plus and MTA Fillapex cements. Material and method 34 premolars human monorradiculados were selected, their removed and their standardized root crowns in 16mm, after then randomly divided into two groups of 15 specimens, and instrumented with rotary system Pro Taper. One group was obturated with cement AH Plus (group HA) and another MTA Fillapex (FI Group). These groups are evaluated for coronary infiltration of bacteria Enterococcus faecalis and the depth of penetration in the dentinal tubules. For the measurement of bacterial infiltration used Fisher's exact test for Intratubular penetration we used the t-test. Result For bacterial infiltration, considering the number of infiltrated samples AH and MTA Fillapex, there was a statistically significant difference (p = 0.0341). In the evaluation of intratubular penetration, the difference between the two groups of cement was statistically significant (p = 0.0006). Conclusion Both cements showed bacterial infiltration and the MTA Fillapex had worse performance, capabilities to enter the dentinal tubules AH Plus was significantly higher than the MTA Fillapex.

Objective: To analyze possible correlations among tubular dentine cement penetration, adhesiveness and apical leakage in fillings performed with gutta percha and an endodontic cement based on epoxy amine resin. Material and Methods: Sixty similar, extracted human mandibular central incisors were irrigated, instrumented and filled following the same protocol. First, apical leakage was quantified by fluid filtration tests. Then, these same specimens were sectioned for analysis of tubular dentine cement penetration and the middle thirds were submitted to push-out tests to analyze the adhesiveness of the fillings. Results: In brief, the means and standard deviations with a confidence interval of 95% were as follows: tubular dentine cement penetration (8.875±4.540), adhesiveness (4.441±2.683) and apical leakage (0.318±0.215). The data were confronted using the Pearson's test (P>0.05), and it was possible to prove that there was no correlation between (1) tubular dentine cement penetration and apical leakage (r2: 0.08276), (2) tubular dentine cement penetration and adhesiveness (r2: -0.2412) and (3) adhesiveness and apical leakage (r2: 0.1340). Conclusion: After analysis of these data, it could be observed that there exists no correlation among the variables analyzed in this study.

O propósito deste estudo foi avaliar a eficiência dos instrumentos rotatórios ProFile, GT, ProTaper, Race e K3 comparados com limas K para a remoção do material obturador durante o retratamento. Sessenta pré-molares inferiores foram instrumentados com o sistema rotatório GT e obturados usando a técnica de compactação termomecânica de guta-percha com cimento AH Plus. Os dentes foram divididos aleatoriamente em seis grupos de dez espécimes cada. As raízes foram seccionadas longitudinalmente, imagens digitais foram criadas através de um scanner, e as áreas de remanescentes de material foram medidas usando o programa Image Tool 1.21. Os resultados indicaram que o GT deixou significantemente menos material remanescente (1,18 ± 1,47) do que os instrumentos manuais (3,70 ± 3,16) e os instrumentos Hero (2,99 ± 2,58) (p<0,05). Não houve diferença estatisticamente significante entre as outras técnicas ProFile (1,99 ± 2,66), ProTaper (2,00 ± 1,99) e K3 (2,71 ± 2,87) quando comparadas com o GT. Como conclusão, GT, ProFile, ProTaper e K3 foram revelados como mais efetivos em remover a guta-percha do que os instrumentos manuais e os instrumentos Hero.

The purpose of this study was to evaluate the efficiency of ProFile, GT, ProTaper, Race and K3 rotary instruments compared with hand K-files for removal of gutta-percha during retreatment. Sixty mandibular premolars were instrumented with GT rotary files and filled by thermomechanical compaction of gutta-percha and AH Plus sealer. The teeth are randomly divided into 6 groups of 10 specimens each. The roots were split longitudinally, digital images were created using a flatbed scanner, and the areas with remaining filling material were demarcated using Image Tool 1.21 software. The results indicate that GT left significantly less (p<0.05) remaining filling material (1.18 ± 1.47) than hand (3.70 ± 3.16) and Hero instruments (2.99 ± 2.58). There was no statistically significant difference (p<0.05) among the others techniques: ProFile (1.99 ± 2.66), ProTaper (2.00 ± 1.99) and K3 (2.71 ± 2.87) when compared with GT. In conclusion, GT, ProFile, ProTaper and K3 were more effective in removing gutta-percha than manual and Hero instruments.

OBJECTIVES: The aim of this study was to evaluate cases of dental trauma treated at the specialized center of Pontifical Catholic University of Paraná, Curitiba, Brazil, during a period of 2 years. MATERIAL AND METHODS: A total of 647 patients were evaluated and treated between 2003 and 2005. Data obtained from each patient were tabulated and analyzed as to gender, age, etiology, time elapsed after the injury, diagnosis (type of trauma), and affected teeth. RESULTS: The results revealed that male individuals aged 7 to 13 years presented the highest prevalence of injury, and falling was the main causal factor. In most cases, the time elapsed between the accident and the first care ranged from 4 to 24 h. A total of 1,747 teeth were affected, with higher incidence of concussion/subluxation and coronal fracture, followed by lateral luxation and avulsion. The permanent maxillary central incisors were the most commonly affected teeth. CONCLUSION: The frequency and causes of dentoalveolar trauma should be investigated for identification of risk groups, treatment demands and costs in order to allow for the establishment of effective preventive measures that can reduce the treatment duration and costs for both patients and oral health services.

O tecido ósseo possui grande potencial regenerativo com capacidade para restaurar completamente sua estrutura e função originais. Há situações em que os defeitos ósseos não conseguem por si só obter o reparo, casos em que se fazem necessários o uso de enxertos, para um correto tratamento e bom prognóstico. Este experimento teve o propósito de analisar microscopicamente o efeito do tamanho das partículas de matriz de osso medular bovino desmineralizado, nas formas micro e macrogranular, na reparação de defeito ósseo em fêmures de coelhos, tendo como controle o coágulo sanguíneo. Após 1,3 e 6 meses da implantação dos materiais, os animais foram mortos e as peças anatômicas removidas. Uma reação granulomatosa tipo corpo estranho, envolvendo as partículas implantadas, foi observada. Estes resultados sugerem falhas na desmineralização e/ou na retirada de potenciais antigênicos durante a produção do biomaterial. Podemos concluir que o tamanho das partículas não influenciou na evolução do processo reparativo dos defeitos ósseos, atuando apenas como substâncias osteopreenchedoras e que o material implantado deverá sofrer um aprimoramento no controle de qualidade na linha de produção, uma vez que poderá representar uma boa alternativa para enxertos ósseos.

The bone tissue has a great regenerative potential, with ability to completely restore its structure and original functions. In some situations, though, bone defects cannot be self-repaired, thus requiring the use of grafts for a correct treatment and good prognosis. This work aimed at microscopically analyzing the effect of the particle size of demineralized bovine cancellous bone matrix in micro and macrogranular forms on the repair of bone defects in femurs of rabbits, with blood clot used as control. At 1, 3 and 6 months after implantation of the materials, the animals were killed and the anatomic specimens were removed. A foreign body-type granulomatous reaction containing macrophages and multinucleated giant cells in contact with the implanted particles was observed. These results suggest a failure in demineralization and/or interruption of the antigenic potential during production of the biomaterial. It is concluded that the size of the particles did not influence the evolution of the repair process of bone defects, acting only as bone-filler substances, and that the material implanted should be improved by quality control during production, since it may represent a good alternative for bone graft.

Considerando estudos anteriores sobre a similaridade entre a composição química do agregado de trióxido mineral e o cimento Portland, o objetivo deste estudo foi investigar a resposta pulpar de dentes de cães após pulpotomia e proteção pulpar direta com MTA Angelus e cimento Portland branco. Trinta e oito remanescentes pulpares foram recobertos com esses materiais. Cento e vinte dias após o tratamento, os animais foram sacrificados e os espécimes removidos e preparados para análise histológica. Ambos os materiais demonstraram os mesmos resultados quando utilizados como materiais de capeamento pulpar, induzindo a formação de ponte de tecido mineralizado e mantendo a vitalidade pulpar em todos os espécimes. Ambos matérias se mostraram efetivos como protetores pulpares após pulpotomia em dentes de cães.

Considering previous studies on the similarity between the chemical composition of the mineral trioxide aggregate and the Portland cement, the purpose of this study was to investigate the pulp response of dog's teeth after pulpotomy and direct pulp protection with MTA Angelus and white Portland cement. Thirty eight pulp remnants were protected with these materials. One hundred and twenty days after treatment, the animals were sacrificed and the specimens removed and prepared for histological analysis. Both materials demonstrated the same results when used as pulp capping materials, inducing hard tissue bridge formation and maintaining pulp vitality in all specimens. The MTA Angelus and the white Portland cement showed to be effective as pulp protection materials following pulpotomy.

Este estudo teve como objetivo avaliar a ação do EDTA, do laser de Nd:YAG e a associação de ambos na obturação dos canais laterais artificiais. Foram utilizados 45 dentes pré-molares inferiores humanos, nos quais foram confeccionados três canais laterais com o auxílio de um alargador com diâmetro compatível de uma lima do tipo K número 15. Os dentes foram instrumentados pela técnica regressiva utilizando brocas de Gates Glidden nos terços cervical e médio e limas manuais no terço apical e irrigação com hipoclorito de sódio a 1 %. Os dentes foram divididos em três grupos: Grupo I EDTA por 5 minutos, Grupo II aplicação de laser de Nd:YAG com 15 Hz, 100mJ e 1,5 Watts e Grupo III a associação de ambos. As raízes foram obturadas pela técnica Híbrida de Tagger, radiografadas e as radiografias escaneadas. Para o preenchimento dos canais laterais foram dados escores. A análise estatística mostrou não haver diferença significante entre os grupos analisados globalmente e nem quando analisados por terços.

This study aimed at evaluating the influence of EDTA, Nd:YAG laser and the combination of both for filling of artificial lateral root canals. Forty-five human mandibular premolars were employed, on which three artificial lateral root canals were prepared by means of a reamer with a similar diameter to a K file #15. The teeth were instrumented through the stepback technique employing Gates Glidden burs at the middle and cervical thirds and manual files at the apical portion, and irrigation with 1% sodium hypochloride. The teeth were divided in three groups: Group 1 -EDTA for 5 minutes; Group 2 -application of Nd:YAG laser at 15 Hz, 100 mJ and 1.5 Watts; and Group 3 - association of both. Roots were filled through the Tagger's hybrid technique, radiographed and the radiographs were digitized. Scores were assigned to the filling of the lateral root canals. Statistical analysis revealed no significant differences between the entire groups and also on the analysis of each third.
