RESUMO Objetivo verificar o risco de distúrbio alimentar pediátrico em pré-escolares que nasceram prematuros e relacionar com os aspectos neonatais, de aleitamento materno e de hábitos orais. Métodos estudo descritivo e longitudinal. A amostra foi composta por pré-escolares que nasceram prematuros e que foram acompanhados até os 24 meses de idade corrigida. A coleta de dados consistiu em análise de prontuários, entrevista e aplicação da Escala Brasileira de Alimentação Infantil. Resultados participaram 19 pré-escolares, com média de idade de 5 anos e 1 mês, nascidos prematuros (idade gestacional de 32 semanas), saudáveis e com função motora oral normal aos 2 anos de idade corrigida. A aplicação da escala permitiu identificar risco de distúrbio alimentar pediátrico em 31,57% da amostra. Não houve diferença entre os grupos com e sem risco do distúrbio quanto às variáveis neonatais, de aleitamento materno e de hábitos orais. Apenas a variável tempo de transição alimentar no período de internação neonatal apresentou diferença entre os grupos, sendo maior em crianças que não apresentaram risco para o distúrbio. Conclusão O risco de distúrbio alimentar pediátrico ocorreu em quase um terço dos prematuros em idade pré-escolar. O tempo de transição alimentar no período de internação neonatal foi a única variável que apresentou diferença entre os grupos com e sem risco para o distúrbio. ABSTRACT Purpose To verify the risk of pediatric eating disorders in preschoolers who were born prematurely and relate it to neonatal aspects, breastfeeding and oral habits. Methods Descriptive and longitudinal study. The sample was made up of preschoolers who were born prematurely and who were followed up until 24 months of corrected age. Data collection consisted of analysis of medical records, interviews and application of the Brazilian Infant Feeding Scale (EBAI). Results 19 preschoolers participated, with a mean age of 5.11±0.51, born prematurely (gestational age of 32.05 ± 3.26 weeks), healthy and with normal oral motor function at 2 years of corrected age. The application of the EBAI made it possible to identify the risk of pediatric eating disorders in 31.57% of the sample. There was no difference between the groups with and without risk of pediatric eating disorders regarding neonatal variables, breastfeeding and oral habits. Only the variable feeding time during the neonatal hospitalization period showed a difference between the groups, being higher in children who were not at risk for pediatric eating disorders. Conclusion The risk of pediatric eating disorders occurred in almost a third of pre-school-age premature infants. RESUMO Objetivo verificar os efeitos da internação em Unidade de Cuidado Intermediário Neonatal Canguru (UCINCa), segunda etapa do Método Canguru, no desenvolvimento das habilidades de alimentação oral em recém nascidos pré-termo (RNPT). Método estudo observacional analítico do tipo longitudinal prospectivo, realizado em um hospital público no Sul do Brasil, com acompanhamento até a alta hospitalar. A amostra foi constituída por 20 RNPT internados na UCINCa e 26 RNPT na Unidade de Cuidado Intermediário Neonatal Convencional (UCINCo), que foram avaliados periodicamente por meio dos níveis de habilidade oral, em mamadeira, conforme os critérios de proficiência e taxa de transferência de leite. Os desfechos considerados foram progressão do nível de habilidade oral, dias de transição para obtenção da via oral plena, dias de internação hospitalar. ABSTRACT Purpose analyze the effects of hospitalization in the Kangaroo Neonatal Intermediate Care Unit (UCINCa), the second stage of the Kangaroo Care (KC), on the development of oral feeding skills in preterm neonates. Methods an analytical observational study of the prospective longitudinal type, carried out in a public hospital in Southern Brazil, where infants were accompanied until hospital discharge. The sample consisted of 20 preterm neonates hospitalized at the UCINCa and 26 preterm neonates at the Conventional Neonatal Intermediate Care Unit (UCINCo), that were periodically evaluated through the levels of oral skill, in a bottle, according to the criteria of proficiency and milk transfer rate. The outcomes considered were a progression of the oral skill level, days of transition to obtain the full oral route, and days of hospital stay. Results the duration of transition to exclusive oral feeding was shorter for preterm neonates at the UCINCa (4.5 vs. 10 days) relative to those at the UCINCo (p = 0.041). RESUMEN Objetivo: evaluar lo adecuado de utilizar una tecnología de atención-educativa del tipo videoclip para aprender la fisiología de la lactancia con la red de apoyo familiar para mujeres en lactancia. Método: estudio metodológico guiado por el Modelo de Traducción del Conocimiento en Acción. Los datos se recolectaron entre julio y septiembre de 2022 en la ciudad de Santa Cruz do Sul, estado do Río Grande do Sul, Brasil. Después de la consulta de puericultura, cada puérpera indicó al menos un integrante de su red de apoyo para que participara de la investigación. En sendas entrevistas presenciales, 52 participantes respondieron los siguientes instrumentos: Evaluación de Tecnología de Asistencia y Validación de la Apariencia de Tecnologías Educativas en Salud. La normalidad de las variables se verificó con la prueba de Kolmogorov-Smirnov. Las variables cuantitativas con distribución normal se describieron con valores medios y desviaciones estándar. ABSTRACT Objective: to evaluate the adequacy of using a care-educational technology with video clips for learning the physiology of lactation by the family support network for breastfeeding women. Method: a methodological study guided by the Knowledge Translation in Action Model. The collection procedure was carried out from July to September 2022 in the city of Santa Cruz do Sul, state of Rio Grande do Sul, Brazil. After the childcare consultation, each puerperal woman indicated one or more members of her support network to participate in the research. In face-to-face interviews, 52 participants answered the following instruments: Assistive Technology Assessment and Face Validation of Educational Technologies in Health. Normality of the variables was verified using the Kolmogorov-Smirnov test. The quantitative variables with normal distribution were described by mean and standard deviation. Face Validity Index and Cronbach's Alpha were calculated. Results: the video clip was evaluated as adequate (1.75) in all attributes: Interactivity (1.75), Objectivity (2.00), Relevance and efficacy (2.00) and Clarity (2.00). RESUMO Objetivo Verificar a influência do Método Canguru sobre as taxas de aleitamento materno em recém-nascidos pré-termo, tanto na alta hospitalar, como ao longo do primeiro ano de vida, assim como sua relação com o momento de introdução da alimentação complementar e a qualidade da dieta. Métodos Estudo longitudinal do tipo observacional, prospectivo e analítico em um hospital público no Sul do Brasil. Participaram do estudo 46 recém-nascidos pré-termo, que foram internados em Unidade de Cuidado Intermediário Neonatal Canguru ou Unidade de Cuidado Intermediário Neonatal Convencional. Resultados A frequência de aleitamento materno exclusivo na alta hospitalar foi mais elevada nos recém-nascidos pré-termo da Unidade de Cuidado Intermediário Neonatal Canguru (p<0,001), sendo certo que, aos quatro meses de idade corrigida, 35% deles continuavam em aleitamento materno exclusivo (p=0,029), em comparação com os bebês da Unidade de Cuidado Intermediário Neonatal Convencional. Já aos seis e 12 meses, não foram encontradas diferenças nas taxas de aleitamento materno entre os grupos participantes. Aos quatro meses de idade corrigida, aproximadamente 45% dos lactentes de ambos os grupos já haviam iniciado a alimentação complementar. Aos 12 meses de idade corrigida, o consumo de alimentos ultraprocessados foi de 38,9% nos lactentes da Unidade de Cuidado Intermediário Neonatal Canguru e de 70% nos da Unidade de Cuidado Intermediário Neonatal Convencional (p=0,054), não sendo encontradas diferenças significativas. Conclusão O Método Canguru favoreceu o aleitamento materno exclusivo na alta hospitalar e aos quatro meses de idade corrigida. Já para a introdução precoce da alimentação complementar, o Método Canguru não se mostrou protetor, sendo encontrado um elevado percentual de consumo de alimentos processados pelos lactentes.

ABSTRACT Objective To assess the influence of Kangaroo Mother Care on breastfeeding rates in preterm neonates, both at hospital discharge and throughout the first year of life, as well as its relation with the time of introduction of early complementary feeding and diet quality. Methods Observational, prospective and analytical longitudinal study conducted in a public hospital in southern Brazil. The study included 46 preterm neonates, who were admitted to a Kangaroo Neonatal Intermediate Care Unit or Conventional Neonatal Intermediate Care Unit. Results The frequency of exclusive breastfeeding at hospital discharge was higher in preterm neonates of Kangaroo Neonatal Intermediate Care Unit (p<0.001), and at four months of corrected age, 35% of them continued on exclusive breastfeeding (p=0.029), as compared to infants in Conventional Neonatal Intermediate Care Unit. However, at six and 12 months, no difference was found in breastfeeding rates between the participating groups. At four months of corrected age, approximately 45% of the breastfeeding infants in both groups were already on complementary feeding. At 12 months of corrected age, consumption of ultra-processed foods was 38.9% in infants from the Kangaroo Neonatal Intermediate Care Unit and 70% in infants from the Conventional Neonatal Intermediate Care Unit (p=0.054), and no significant differences were found. Conclusion The Kangaroo Mother Care fostered exclusive breastfeeding at hospital discharge and at 4 months of corrected age. On the other hand, with regard to the early introduction of complementary feeding, Kangaroo Mother Care was not protective, and a high rate of consumption of processed foods by infants was found.

RESUMO Objetivo Verificar a influência de um estímulo gustativo na pressão de sucção, durante a sucção não nutritiva (SNN), em recém-nascidos a termo, saudáveis e com peso adequado à idade gestacional. Método Estudo quase experimental do tipo ensaio clínico não randomizado com uma amostra de conveniência de 60 recém-nascidos (RN), 30 alocados no grupo estudo (GE) e 30 no grupo controle (GC). Os RN foram avaliados quanto à pressão de sucção, durante a SNN em chupeta. Para o GE foi adicionado estímulo gustativo à chupeta, umedecida com o colostro. O GC não recebeu nenhum estímulo, além da própria chupeta. As pressões média, mínima e máxima foram medidas com o equipamento S-Flex®. Resultados O GE apresentou pressão média e máxima de sucção significativamente maiores do que o GC. Ainda, houve diferença estatisticamente significativa, entre os grupos, para a 2ª medida da pressão média de sucção. Conclusão Os resultados demostraram que os RN do GE apresentaram pressões de sucção, média e máxima, significativamente maiores, quando comparados ao GC. A utilização de um estímulo gustativo associado à SNN modificou a pressão de sucção e parece potencializar as habilidades orais.

Abstract Purpose To verify the influence of a taste stimulus on the suction pressure, during the non-nutritive sucking (SNN), in newborns, healthy and with weight appropriate to the gestational age. Methods Quasi-experimental study of the non-randomized clinical trial type with a convenience sample of 60 newborns (NB), 30 allocated in the study group (EG) and 30 in the control group (CG). The NB were evaluated for sucking pressure during the SNN in a pacifier. For the EG, a gustatory stimulus was added to the pacifier, moistened with colostrum. The CG did not receive any stimulus, other than the pacifier itself. The average, minimum and maximum pressures were measured with the equipment S-Flex®. Results The SG presented mean and maximum sucking pressure significantly higher than the CG. In addition, there was a statistically significant difference between the groups for the second measurement of mean sucking pressure. Conclusion The results showed that the NB of the SG presented sucking pressures, average and maximum, significantly higher, when compared to the CG. The use of a taste stimulus associated with SNN modified the sucking pressure and seems to enhance oral skills.

RESUMO Objetivo compreender como é a formação, o conhecimento, a demanda e a atuação clínica dos fonoaudiólogos do estado do Rio Grande do Sul na temática de distúrbios alimentares pediátricos. Métodos estudo transversal, descritivo, de abordagem quantitativa. A população foi composta por profissionais de Fonoaudiologia atuantes nas instituições de saúde e educação dos 42 municípios de abrangência da Macrorregião Centro-Oeste do Rio Grande do Sul. O contato com os participantes ocorreu a partir de um cadastro disponibilizado pelas Coordenadorias Regionais de Saúde dessa macrorregião. A coleta de dados ocorreu por meio de um questionário online disponível na plataforma Formulários Google. Resultados o conhecimento para a atuação junto à população pediátrica com distúrbio alimentar mostrou-se insuficiente para a adequada condução dos casos. As maiores dúvidas das fonoaudiólogas foram a respeito da etiologia e tratamento da doença, além de 74,1% das participantes referirem insegurança para avaliar e tratar os pacientes com esse quadro. Conclusão a maioria das fonoaudiólogas da Macrorregião Centro-Oeste do Rio Grande do Sul sente insegurança para atuar com o distúrbio alimentar pediátrico e relata pouco conhecimento sobre o tema. Portanto, considera-se necessário o investimento em formação continuada para as profissionais, a fim de favorecer a criação de saberes comuns, fornecendo subsídios científicos e elementos técnicos para a replicação nos seus locais de trabalho.

ABSTRACT Purpose To understand the training, knowledge, demand and clinical performance of speech-language pathologists on the subject of pediatric feeding disorders in the interior of Rio Grande do Sul. Methods cross-sectional study, descriptive of quantitative approach. The population of this study was composed of Speech-Language Pathology professionals working on health and education institutions in the 42 cities of Midwest macro-region of Rio Grande do Sul. The contact with the participants was obtained from a register made available by the Regional Health Coordinations of this macro-region. The data collection occurred through an online quiz made available on the Google Forms platform. Results the knowledge for working with the pediatric population with feeding disorder proved to be insufficient for proper handling of cases. The biggest doubts of speech-language pathologists are about the etiology and treatment of the disease, in addition to 74.1% of the participants reporting lack of confidence to assess and treat patients with this condition. Conclusion most of the speech-language pathologists of the Midwest macro-region of Rio Grande do Sul do not feel up to handle cases with pediatric feeding disorder and report little knowledge about the subject. Thus, the investment in continuing training is considered necessary, in order to improve the creation of common knowledge, providing scientific subsidies and technical elements for replication in their workplaces.

RESUMO Objetivo Descrever os instrumentos utilizados para captação e análise acústica dos sinais de ausculta cervical e identificar aqueles com maior potencial para aplicação na clínica fonoaudiológica. Estratégia de pesquisa Trata-se de uma revisão integrativa de literatura. As buscas foram realizadas nas bases de dados MEDLINE/PubMed, Scopus e Web of Science, a partir da combinação de termos de relevância e operadores booleanos, durante o mês de novembro de 2020. Critérios de seleção Artigos científicos publicados nos idiomas português, espanhol ou inglês, nos últimos cinco anos (2016-2020) e que apresentassem estudo da ausculta cervical. Resultados Foram encontrados 98 artigos. Após a aplicação dos critérios de seleção, 26 artigos foram selecionados para esta revisão. Para captação dos sinais de ausculta cervical, o microfone foi o instrumento mais utilizado, seguido pela técnica de ausculta cervical de alta resolução, que combina sinais acústicos e vibratórios registrados por um microfone e um acelerômetro, respectivamente. Softwares e/ou algoritmos foram selecionados para análise acústica dos sinais, de acordo com o objetivo de cada estudo. Conclusão O método de ausculta cervical de alta resolução e a análise acústica por meio de algoritmos de aprendizado de máquina apresentaram grande potencial para utilização na prática clínica fonoaudiológica para avaliação e monitoramento da deglutição.

ABSTRACT Purpose Describe the instruments used to capture and analyze the acoustic signals obtained from cervical auscultation, and identify those with the greatest potential for application in the speech pathology clinic. Research strategy This is an integrative literature review. Searches were performed in the MEDLINE/PubMed, Scopus and Web of Science databases in November 2020, using relevant keywords combined with Boolean operators. Selection criteria Scientific articles published in Portuguese, Spanish or English in the last five years (2016-2020) and that presented a study of cervical auscultation. Results Ninety-eight articles were found. After the application of selection criteria, 26 articles were selected for this review. Microphones were the most common instruments used to perform cervical auscultation, followed by high-resolution cervical auscultation techniques, which combine acoustic and vibrational signals recorded by a microphone and an accelerometer, respectively. Acoustic analysis was performed using different software packages and/or algorithms depending on the goals of each study. Conclusion The combination of high-resolution cervical auscultation and machine learning for acoustic analysis has great potential for utilization in the clinical assessment and monitoring of swallowing in speech pathology.

RESUMO Objetivo determinar a confiabilidade do equipamento S-FLEX® para verificar pressão de sucção não nutritiva em recém-nascidos a termo. Métodos estudo transversal e quantitativo, com 30 recém-nascidos a termo, saudáveis, entre o primeiro e terceiro dia de vida, internados em um hospital terciário. Foram realizadas duas avaliações em cada recém-nascido, por dois examinadores distintos, com intervalo de quatro horas. Foram realizadas três determinações sucessivas da pressão máxima e a média de sucção não nutritiva, por meio do equipamento S-FLEX®. Para determinar a confiabilidade, avaliou-se a repetibilidade e a reprodutibilidade das medidas. Análise de variância e coeficiente de correlação intraclasse foram utilizados para análise dos resultados. Resultados a pressão máxima de sucção, na 1ª avaliação, foi de 10,44 (± 2,25), 10,12 (± 2,25) e 9,73 (± 2,61) mmHg, (p = 0,120), respectivamente para a 1ª, 2ª e 3ª medidas. Na 2ª avaliação, foi de 10,10 (± 2,35), 9,98 (± 2,02) e 10,37 (± 2,29), (p = 0,404). A pressão média de sucção na 1ª avaliação foi de 3,64 (± 1,04), 3,59 (± 1,04) e 3,56 (± 1,03), (p = 0,852) e, na 2ª avaliação, foi 3,59 (± 1,04), 3,51 (± 0,92) e 3,63 (± 1,04), (p = 0,721). Para a pressão máxima de sucção, o coeficiente de correlação foi de 0,708 (p = 0,001); para a pressão média foi de 0,828 (p<0,001), evidenciando boa e excelente correlação, respectivamente. Conclusão o S-FLEX® mostrou-se um equipamento confiável para determinar a pressão de sucção não nutritiva em recém-nascidos a termo, saudáveis.

ABSTRACT Purpose to establish the reliability of the S-FLEX® to measure non-nutritive sucking pressure in full-term newborns. Methods a cross-sectional and quantitative study was conducted in a tertiary hospital, on 30 one- to three-days-old infants born healthy and at term. Each newborn was evaluated twice, each time by a different examiner, at an interval of 4 hours. In each assessment, maximum and mean non-nutritive sucking pressure were measured three times consecutively. The reliability of the instrument was investigated by calculating the repeatability and reproducibility of these measurements. Repeatability was determined by analyzing the consecutive measures taken by each examiner using repeated-measures analysis of variance (ANOVA). Results the maximum sucking pressure was found to be 10.44 (± 2.25), 10.12 (± 2.25) and 9.73 (± 2.61) mmHg, (p = 0.120), in the first, second and third consecutive measurements, respectively. In the second evaluation, the values were 10.10 (± 2.35), 9.98 (± 2.02) and 10.37 (± 2.29) mmHg, (p = 0.404). The measurements of mean sucking pressure taken by examiner A in the first evaluation were 3.64 (± 1.04), 3.59 (± 1.04) and 3.56 (± 1.03) mmHg, (p = 0.852), respectively, and, in the second evaluation, 3.59 (± 1.04), 3.51 (± 0.92) and 3.63 (± 1.04) mmHg, (p = 0.721). The correlation coefficients between measurements of maximum and mean sucking pressure were 0.708 (p = 0.001) and 0.828 (p<0.001), which are indicative of good and excellent correlations, respectively. Conclusion The S-FLEX® can provide reliable measurements of maximum and mean non-nutritive sucking pressure in healthy full-term newborns.

RESUMO Objetivo: investigar o Schedule Oral Motor Assessment (SOMA) para utilização com recém-nascidos pré-termo, e comparar seus resultados com o Preterm Oral Feeding Readiness Scale (POFRAS) na liberação da alimentação por via oral. Métodos: trabalho de caráter transversal e quantitativo. Estudo composto por uma amostra de 45 recém-nascidos pré-termo, saudáveis e clinicamente estáveis, avaliados no momento da liberação da alimentação por via oral. Neste momento foi realizada a avaliação do POFRAS e do SOMA. Para a análise dos dados foi utilizado o software Stata 10.0. Resultados: todos 10 recém-nascidos pré-termo que apresentaram prontidão para alimentação por via oral tinham função motora oral normal, e, os 16 que apresentaram disfunção motora oral, esses não apresentavam prontidão para a mamada (p<0,05). O tempo de transição de alimentação por via oral foi de 13,5 (±8,1) dias nos recém-nascidos pré-termo, com melhores resultados nas duas avaliações, e de 17,7 (±10,9) dias aos que não apresentaram prontidão para a alimentação oral e tiveram disfunção motora oral no momento da liberação da via oral, resultando em um dado com relevância clínica mesmo não apresentando significância (p>0,05). Conclusão: estes resultados sugerem que o SOMA pode ser um método complementar para avaliação da função oral-motora no momento da liberação da via oral de recém-nascido pré-termo.

ABSTRACT Objective: this study aimed at investigating the Schedule Oral Motor Assessment (SOMA) tool to be used with preterm infants and to compare its results with the Preterm Oral Feeding Readiness Assessment Scale (POFRAS) to start oral feeding. Methods: a cross-sectional and quantitative study, consisting in a sample of 45 healthy and clinically stable preterm infants, assessed at their first oral feeding with two tools: the Schedule Oral Motor Assessment and Preterm Oral Feeding Readiness Assessment Scale. Stata 10.0 software was used for data analysis. Results: 10 preterm infants with readiness for oral feeding showed normal oral motor function, and 16, presented with oral motor dysfunction, did not show readiness for feeding (p <0.05). The time of transition for full oral feeding was 13.5 (± 8.1) days for preterm infants with better results in both assessment tools, and 17.7 (± 10.9) days for those who did not show readiness for oral feeding and had oral motor dysfunction to initiate oral feeding, resulting in a given clinical relevance, even showing no significance (p> 0.05). Conclusion: these results suggest that the Schedule Oral Motor Assessment can be an adjunctive method for evaluation of the oral motor function at the first oral feeding in preterm infants.

RESUMO Objetivo: avaliar as medidas antropométricas orofaciais segundo o tipo facial de crianças do sexo masculino e feminino. Métodos: participaram do estudo 126 crianças, 64 do sexo feminino e 62 do sexo masculino, com idade entre sete anos e 11 anos. As medidas antropométricas orofaciais foram comparadas entre os tipos faciais obtidos por meio da análise cefalométrica. Inicialmente foi realizada análise descritiva dos tipos faciais de acordo com os sexos. Por meio da Análise de Variância, foi realizada a comparação das medidas antropométricas no sexo masculino e feminino isoladamente e, posteriormente, foi realizada a análise independe do sexo. Os dados foram analisados considerando nível de significância de 5%. Resultados: encontrou-se um número maior de braquifaciais, seguidos de mesofaciais e dolicofaciais na totalidade da amostra. Na comparação das medidas antropométricas entre os três tipos faciais, evidenciou-se no sexo masculino diferença significante na medida de terço facial inferior e no sexo feminino, nas medidas da altura facial, terço inferior e lábio inferior. Quando comparadas as medidas antropométricas de forma independente do sexo, evidenciou-se diferença significante no terço inferior, lados direito e esquerdo da face, lábios superior e inferior. Conclusão: como esperado, nas análises de todos os resultados, considerando ou não os sexos, as medidas antropométricas orofaciais foram menores nos braquifaciais, com maior evidência encontrada nas medidas relacionadas ao plano vertical inferior.

ABSTRACT Purpose: assess anthropometric orofacial measurements of boys and girls according to facial type. Methods: participants were 126 children, 64 girls and 62 boys, aged between seven and 11 years. Anthropometric orofacial measurements were compared between the facial types obtained by cephalometric analysis. Initially, a descriptive analysis of facial types was conducted based on sex. Anthropometric measurements in boys and girls were compared separately by analysis of variance, followed by sex-independent analysis. Data were analyzed considering a significance level of 5%. Results: a larger number of brachyfacial individuals were found in the sample, followed by mesofacial and dolichofacial subjects. Comparison of facial measurements for the three types showed a significant difference in the lower third of the face among boys, and in face height, lower third and lower lip among girls. When anthropometric measurements were compared independently of sex, a significant difference was observed in the lower third, right and left sides of the face, as well as the upper and lower lips. Conclusion: as expected, analyses of all the results, considering sex or not, found lower anthropometric orofacial measurements in brachyfacial individuals, which was most evident in measurements related to the lower vertical plane.

RESUMO Objetivo Descrever e comparar as variáveis cinemáticas temporoespaciais do movimento mandibular durante a mastigação unilateral deliberada e habitual de indivíduos saudáveis. Método Participaram do estudo 8 voluntários saudáveis, do gênero masculino, com faixa etária entre 19 e 24 anos. Os dados cinemáticos foram obtidos através do sistema de análise de movimento Qualysis (QTM - Qualisys Track Manager). Foram realizados registros de mastigação unilateral direita (MU) e habitual (MH) de bala de goma de gelatina de consistência firme. Foram analisadas variáveis relacionadas à (1) sequência mastigatória (duração, número de ciclos e frequência mastigatória); (2) ciclo mastigatório: duração do ciclo mastigatório, amplitude de movimento mandibular vertical e médio-lateral durante o ciclo mastigatório, velocidade máxima durante as fases de abertura e fechamento. A comparação das variáveis durante a MU e MH foi realizada por meio do teste t pareado (p<0,05) e os tamanhos de efeito (‘d’ de Cohen) foram calculados. Resultados Em relação à sequência mastigatória, observou-se menor frequência mastigatória durante MU comparada à MH (1,19±0,21 e 1,29±0,16Hz, respectivamente, p=0,004, d=0,53) e menores velocidades máximas de abertura (MU=67,4 mm/s e MH=80,02; p=0,053; d=0,80) e fechamento (MU=71,77±9,35mm/s e MH=83,51±17 mm/s, p=0,014, d=0,79) do ciclo mastigatório. Conclusão As variáveis cinemáticas relacionadas à sequência e ao ciclo mastigatório foram influenciadas pelo padrão mastigatório adotado – unilateral ou habitual.

ABSTRACT Purpose To describe and compare the temporal-spatial kinematic variables of mandibular movement during deliberate unilateral and habitual mastication in healthy young-adult individuals. Methods The study sample was composed of eight male healthy volunteers aged 19 to 24 years. The kinematic data were obtained using a motion analysis system - Qualisys Track Manager (QTM) ProReflex MCU. Recordings were performed during deliberate unilateral mastication (UM) and habitual mastication (HM) of firm-consistency gummy candy. The following variables were analyzed: (1) masticatory sequence: duration, number of masticatory cycles, and chewing rate; (2) masticatory cycle: duration, vertical and medial-lateral mandibular range of motion in relation to the skull, and maximum velocity during the opening and closing phases. Data of the variables were compared during UM and HM by the paired t test, and the effect sizes ('d' Cohen) were calculated. Results Regarding the variables of the masticatory sequence, smaller chewing rate was observed for UM compared with that for HM (1.19±0.21Hz and 1.29±0.16Hz, respectively, p=0.004, d=0.53). Smaller values of maximum velocity during the opening (MU=67.4 mm/s and MH=80.02, p=0.053, d=0.80) and closing (MU=71.77±9.35mm/s and MH=3.51±7mm/s, p=0.014, d=0.79) phases of the masticatory cycle were observed in deliberate unilateral mastication compared with those in habitual mastication. Conclusion Kinematic variables associated with the sequence and cycle of mastication are influenced by the chewing pattern adopted - deliberate unilateral or habitual.

RESUMO Objetivo Correlacionar a saturação periférica de oxigênio com a idade gestacional e com o nível de habilidade de alimentação oral, na introdução da alimentação oral, de recém-nascidos pré-termo. Método Estudo transversal e quantitativo no qual participaram 169 recém-nascidos pré-termo, clinicamente estáveis. A verificação da saturação de oxigênio foi realizada antes e após a introdução da primeira alimentação por via oral. Os recém-nascidos pré-termo foram estratificados em três grupos baseados na idade gestacional ao nascimento: 26-29, 30-33 e 34-36 semanas. A habilidade para alimentação oral foi classificada em quatro níveis com base na habilidade oral e resistência apresentada pelo recém-nascido pré-termo. Resultados Não houve diferença na saturação periférica de oxigênio entre os estratos da idade gestacional e entre os níveis de habilidade oral. Foram observadas diferenças nos grupos com idade gestacional de 30-33 semanas (p=0,04) e 34-36 semanas (p=0,02) e no nível I de habilidade de alimentação oral (p=0,04) quando comparada com a saturação inicial e final. Correlações significativas (p<0,001) foram encontradas entre a idade gestacional e níveis de habilidade oral (r=0,38); no Grupo A, entre a idade gestacional e a saturação inicial (r=0,83); e, no Grupo B, entre o nível de habilidade oral com a saturação inicial, e com a idade gestacional. Conclusão A saturação de oxigênio apresentou correlação quando comparada com a idade gestacional e com o nível de habilidade oral.

ABSTRACT Purpose To correlate the peripheral oxygen saturation with gestational age and the level of oral feeding skills in the introduction of oral feeding in preterm infants. Methods This is a cross-sectional, quantitative study whose sample was composed of 169 clinically stable preterm infants. Peripheral oxygen saturation was assessed before and after introduction of oral feeding. The preterm infants were stratified into three groups based on their gestational age at birth: 26-29, 30-33, and 34-36 weeks. The preterm infants were classified into four levels according to their oral feeding skill and resistance. Results No differences in oxygen saturation were observed between the strata of gestational age and between the levels of oral feeding skill. Differences were observed in the groups of preterm infants aged 30-33 weeks (p=0.04) and 34-36 weeks (p=0.02) and on the level I of oral feeding skills (p=0.04) when oxygen saturation was compared at pre- and post-first oral feeding. Significant correlations (p<0.001) were found between gestational age and the levels of oral feeding skills (r=0.38); in Group A, between gestational age and oxygen saturation before the first oral feeding (r=0.83); in Group B, between the level of oral feeding skill and oxygen saturation before the first oral feeding (r=0.26) and between level of oral feeding skill and gestational age (r=0.26). Conclusion Correlation was found for peripheral oxygen saturation when compared with gestational age and with the level of oral feeding skills.

RESUMO Objetivo avaliar a acurácia do Preterm Oral Feeding Readiness Scale - POFRAS para iniciar a alimentação oral de recém-nascidos pré-termo e verificar a concordância entre este instrumento e o instrumento de avaliação do Nível de Habilidade Oral. Métodos Foram avaliados 82 recém-nascidos pré-termo quanto à prontidão para o início da alimentação oral através do POFRAS e da avaliação do Nível de Habilidade Oral, durante a primeira alimentação oral. A acurácia do POFRAS foi estimada em relação à variável proficiência, por meio da Curva ROC (Receiver Operating Characteristic Curve). Para a análise da concordância entre os instrumentos, foi utilizado o coeficiente Kappa. Resultados A acurácia global do POFRAS foi de 71,29%. O ponto de corte 29 foi o que apresentou melhor equilíbrio entre sensibilidade e especificidade. O coeficiente Kappa mostrou fraca concordância entre os instrumentos na identificação dos RN aptos e inaptos a mamar por via oral (k=0.281). Conclusão a acurácia do POFRAS para o início da alimentação oral, estimada por meio da variável proficiência, foi semelhante à obtida com a técnica de translactação. Observou-se fraca concordância entre os instrumentos avaliados. Sugere-se, portanto, que estes instrumentos de avaliação sejam usados de forma complementar na prática clínica, uma vez que ambos apresentam aspectos importantes do comportamento alimentar do prematuro, que ao serem analisados conjuntamente permitirão orientar a conduta necessária para propiciar uma transição alimentar mais breve e eficaz para essa população.

ABSTRACT Purpose To assess the accuracy of the Preterm Oral Feeding Readiness Scale (POFRAS) on the beginning of oral feeding in preterm infants and to verify the concordance between this tool and the Oral Feeding Skill Level. Methods 82 preterm infants were assessed by POFRAS regarding their readiness to initiate oral feeding and by the oral feeding skill level evaluation during the first oral feeding. POFRAS's accuracy was estimated regarding proficiency by a Receiver Operating Characteristics (ROC) curve. The concordance between the tools was obtained by analysis of the Kappa coefficient. Results POFRAS's global accuracy was of 71.29%. The cut-off value of 29 was the one that presented most optimization of the sensitivity based on specificity. The Kappa coefficient has shown a weak concordance between the instruments to identify infants able and unable to oral feeding (k=0.281). Conclusion POFRAS's accuracy to initiate oral feeding considering the proficiency was similar to that obtained with the technique of translactation. We observed a weak concordance between the instruments. We suggest that, in clinical practice, both instruments should be used in a complementary manner, since both present important aspects of the preterm feeding behavior that together will better guide the necessary conduct to provide an effective and quick transition to full oral feeding in this population.

RESUMO Objetivo Verificar se a presença de prontidão para iniciar a alimentação oral, obtida por meio do Preterm Oral Feeding Readiness Scale (POFRAS) se relaciona com o desempenho alimentar na primeira oferta oral de leite, com o tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. Métodos Estudo longitudinal, que envolveu 65 recém-nascidos pré-termo. Foi realizada a avaliação da prontidão para início da alimentação oral, por meio do POFRAS, constituindo-se dois grupos: com e sem prontidão para iniciar a alimentação por via oral, em dois estratos de idade gestacional ao nascer: de 28 a 33 semanas e de 34 a 36 semanas. Em cada estrato, os grupos foram comparados entre si, quanto ao desempenho alimentar na primeira oferta oral de leite (por meio das variáveis proficiência, taxa de transferência e desempenho alimentar oral) e quanto ao tempo de transição da sonda para a via oral plena e o tempo de internação hospitalar. O teste t-Student ou o teste de Wilcoxon foram utilizados para comparar as variáveis contínuas e o teste exato de Fisher, para as variáveis categóricas. Resultados A prontidão para iniciar a alimentação por via oral se relacionou com melhores resultados de proficiência e taxa de transferência na primeira alimentação, contribuindo para a transição mais rápida da sonda para a via oral plena, nas crianças nascidas com idade gestacional entre 28 a 33 semanas. Conclusão a avaliação por meio do POFRAS permite estabelecer um prognóstico da alimentação oral em recém-nascidos pré-termo, menores de 34 semanas. No entanto, não exclui a necessidade de avaliação da biomecânica da deglutição.

ABSTRACT Purpose To verify whether readiness for oral feeding as evaluated by the Preterm Oral Feeding Readiness Scale (POFRAS) is associated with performance on the first oral feeding; length of transition from tube to full oral feeding, and duration of hospitalization. Methods This was a longitudinal study involving 65 preterm infants. Participants were divided into groups according to readiness for oral feeding as determined by the POFRAS, and gestational age (29-33 weeks and 34-36 weeks). Performance on the first oral feeding (proficiency, transfer rate and overall transfer), number of days from introduction to full independent oral feeding, and length of hospital stay were compared between groups. Continuous variables were compared using Student’s t-test or Wilcoxon’s signed-rank test. Categorical variables were compared using Fisher’s exact test. Results Readiness for oral feeding was associated with higher proficiency and transfer rates on the first oral feeding, resulting in a shorter transition from tube to independent oral feeding in preterm infants with 29 to 33 weeks of gestational age at birth. Conclusion The POFRAS contributed to the prognosis of oral feeding outcomes in preterm neonates with less than 34 weeks of gestational age at birth. However, its use does not eliminate the need for an assessment of swallowing mechanisms.

OBJETIVO: Avaliar a influência da habilidade motora oral do prematuro sobre seu desempenho alimentar oral e crescimento, durante o período de internação neonatal.MÉTODOS: Foram avaliados 51 recém-nascidos (RNs) internados na Unidade de Tratamento Intensivo (UTI) Neonatal de um hospital do Sul do Brasil, entre julho 2012 e março 2013. A avaliação da habilidade de alimentação oral, segundo Lau e Smith, foi realizada após prescrição para início da alimentação por via oral (VO). O desempenho alimentar oral foi avaliado por meio das variáveis dias de transição da sonda para VO plena e tempo de internação. O crescimento foi avaliado por peso, comprimento e perímetro cefálico (PC), utilizando as curvas de Fenton, no nascimento, liberação VO, VO plena e alta hospitalar.RESULTADOS: Ao nascer, 71% dos prematuros eram adequados para idade gestacional, a maioria era do sexo masculino (53%), tendo média de 33,6 (±1,5) semanas de idade gestacional. A idade gestacional nos momentos avaliados não influenciou na habilidade oral do pré-termo, não diferindo entre os níveis. O tempo de transição da sonda para VO plena e o período de internação hospitalar foram menores quanto maior o nível de habilidade oral. Ao nascer, houve uma tendência a baixo peso e baixa habilidade oral. Na liberação da VO, as crianças do nível IV apresentaram maior peso.CONCLUSÃO: O nível de habilidade oral do prematuro interferiu positivamente no tempo de transição alimentar da sonda para VO plena e permanência hospitalar. O crescimento, representado pelo ganho de peso, não sofreu influência do nível de habilidade oral.

OBJECTIVE: To evaluate the influence of oral motor skills of premature infants on their oral feeding performance and growth, during neonatal hospitalization.METHODS: Fifty-one newborns hospitalized in the neonatal intensive care unit of a hospital in Southern Brazil, between July 2012 and March 2013, were evaluated. The evaluation of oral feeding skills, according to Lau and Smith, was applied after prescription for starting oral feeding. The oral feeding performance was analyzed using the following variables: days taken to start independent oral feeding and hospital discharge. Growth was measured by weight, length, and head circumference, using the curves of Fenton, at birth, first and independent oral feeding, and hospital discharge.RESULTS: At birth, 71% preterm infants were proper for gestational age, most of them were males (53%), with average of 33.6 (±1.5) weeks of gestational age. The gestational age in the assessment did not influence the oral feeding performance of the premature infant and did not differ between levels. Time of transition from tube feeding to oral feeding and hospital stay was shorter when the oral skills were higher. At birth, there was a tendency of low weight and low oral feeding performance. Level IV premature infants in the release of oral feeding presented higher weights.CONCLUSION: The level of oral skills of the premature infant interfered positively on time of feeding transition from tube to independent oral feeding and hospital stay. Growth, represented by weight gain, was not affected by the level of oral skill.
