Abstract Objective To analyze the effect of combined training (CT) in postural control and gait parameters in postmenopausal women. Methods A parallel-group, randomized, control study was conducted with 16 weeks of combined training (n = 16) versus a non-training control group (n = 12) in postmenopausal women (aged 59.3 ± 8.0). Pre and postintervention assessments included postural control (using an AMTI force platform – Advanced Mechanical Technology, Inc., Watertown, MA, USA) and gait impairments (using baropodometry). In addition, the upper limb strength and abdominal tests, as well as aerobic capacity, assessed functional indicators. Results The CT intervention in postmenopausal women resulted in improved gait (stride length (p = 0.006); speed (p = 0.013); double support time (p = 0.045); and improved postural control (displacement area of postural sway in a normal base of support with eyes open (p = 0.006). Combined training increased functional indicators (abdominal - p = 0.031; aerobic capacity - p = 0.002). Conclusion In conclusion, combined aerobic plus strength training effectively improved gait and balance control in older women. The postmenopausal women from the CT group walked faster and with bigger steps after the intervention than the control group. In addition, they presented decreased postural sway in standing and decreased the percentage of double support time while walking, which means improved static and dynamic balance control and functional indicators. Resumo Objetivo Analisar o efeito do treinamento combinado (TC) no controle postural e nos parâmetros da marcha em mulheres na pós-menopausa. Métodos Foi realizado um estudo controlado randomizado de grupos paralelos com 16 semanas de treinamento combinado (n = 16) versus um grupo controle sem treinamento (n = 12) em mulheres na pós-menopausa (59,3 ± 8,0 anos). As avaliações pré e pós-intervenção incluíram controle postural (usando a plataforma de força AMTI) e deficiências da marcha (usando baropodometria). Além disso, os testes de força de membros superiors e abdominal, bem como a capacidade aeróbica, avaliaram indicadores funcionais. RESUMEN Introducción: En la enfermedad cerebro vascular uno de los principales problemas es la espasticidad, que compromete el control postural, la funcionalidad y la calidad de vida. Para lograr la mayor recuperación funcional, se han utilizado los enfoques neuromusculares del ejercicio terapéutico (Ej: Rood, Bobath, Brunnstrom y la Facilitación Neuromuscular Propioceptiva), sin embargo, existe controversia sobre la eficacia de estas intervenciones. Objetivo: Realizar un análisis crítico de la evidencia científica disponible sobre el efecto de los enfoques tradicionales en la espasticidad de los miembros inferiores en personas post-Enfermedad Cerebro Vascular. Métodos: Se incluyeron ensayos clínicos controlados de las bases de datos PUBMED, MEDLINE, PEDro, CINAHL, SCIENCEDIRECT y SCOPUS. La calidad metodológica de los artículos incluidos se evaluó por medio de la escala PeDro. Resultados: Once estudios cumplieron con los criterios de inclusión, obteniendo una calidad metodológica de intermedia a alta. Entre las limitaciones de los estudios revisados se destacan principalmente la ausencia del cálculo del tamaño de muestra, de enmascaramiento de los evaluadores, pérdida de participantes, muestras heterogéneas, carencia de protocolos estandarizados y diversidad de variables evaluadas. Conclusión: El tratamiento fisioterapéutico para disminuir la espasticidad puede ser efectivo aplicado entre tres y cinco días por semana, durante 40 minutos. Sin embargo, estos resultados están soportados en estudios con limitaciones metodológicas y dificultades en su comparación. Por ello, se requieren nuevos estudios para evaluar la eficacia de dichos enfoques, con el fin implementar intervenciones fisioterapéuticas basadas en la mejor evidencia, que contribuyan en una mejor calidad de vida de esta población.

ABSTRACT Introduction: Spasticity due to a stroke is one of the major changes that leads to sensorimotor deficits, causing postural control and functional capability deficits. In order to achieve the best possible functional recovery, several neuromuscular therapeutic exercise approaches have been employed (eg Rood, Bobath, Brunnstrom and proprioceptive neuromuscular facilitation), but still there are many doubts about the effectiveness of these interventions. Purpose: To perform a critical analysis of the available scientific evidence on the effect of the traditional approaches in spasticity recovery of the lower limbs in post-stroke individuals. Methods: Randomized controlled Trials available in PubMed, MEDLINE, PEDro, CINAHL, and Scopus data SCIENCEDIRECT bases were included in this study. The methodological quality of included articles was evaluated by the PEDro scale. Results: Eleven studies were included, obtaining a methodological quality of intermediate to high. The main observed limitations of the analyzed studies were: the absence of sample size estimation, the lack of blind evaluators, loss of participants, heterogeneous samples, lack of standardized protocols and diversity of employed variables. Conclusions: Based upon the results, it can be suggested that spasticity reduction can be effective applying from three to five days interventions per week, lasting 40 minutes. However, such suggestions are supported by studies with methodological limitations and employing different procedures that prevent a direct comparison among the studies. Therefore new studies are required to evaluate the effectiveness of such approaches, to implement physiotherapy interventions based on the scientific evidences that would further contribute to a better quality of life of post-stroke individuals.

Background: Partial body weight support (BWS) systems have been broadly used with treadmills as a strategy for gait training of individuals with gait impairments. Considering that we usually walk on level ground and that BWS is achieved by altering the load on the plantar surface of the foot, it would be important to investigate some ground reaction force (GRF) parameters in healthy individuals walking on level ground with BWS to better implement rehabilitation protocols for individuals with gait impairments. Objective: To describe the effects of body weight unloading on GRF parameters as healthy young adults walked with BWS on level ground. Method: Eighteen healthy young adults (27±4 years old) walked on a walkway, with two force plates embedded in the middle of it, wearing a harness connected to a BWS system, with 0%, 15%, and 30% BWS. Vertical and horizontal peaks and vertical valley of GRF, weight acceptance and push-off rates, and impulse were calculated and compared across the three experimental conditions. Results: Overall, participants walked more slowly with the BWS system on level ground compared to their normal walking speed. As body weight unloading increased, the magnitude of the GRF forces decreased. Conversely, weight acceptance rate was similar among conditions. Conclusions: Different amounts of body weight unloading promote different outputs of GRF parameters, even with the same mean walk speed. The only parameter that was similar among the three experimental conditions was the weight acceptance rate.

CONTEXTUALIZAÇÃO: A esteira motorizada desencadeia passadas em bebês similares ao andar. Entretanto, os efeitos da velocidade da esteira nas características das passadas desencadeadas por ela ainda são desconhecidos. OBJETIVO: Examinar o efeito da velocidade da esteira em passadas desencadeadas em bebês. MÉTODOS: Seis bebês com desenvolvimento típico, com idades entre 11 e 13 meses, iniciando o andar independente, foram filmados andando na esteira em quatro velocidades (0,10; 0,16; 0,22 e 0,28 m/s), com marcas passivas posicionadas em suas principais articulações (ombro, quadril, joelho e tornozelo). As imagens foram digitalizadas e analisadas com os softwares Ariel Performance Analysis System, Matlab e SPSS (Statistical Package for Social Sciences, v.10). As variáveis analisadas foram as características descritivas, temporais, coordenativas e angulares das passadas desencadeadas pela esteira nas quatro velocidades. RESULTADOS: Em 0,22 e 0,28 m/s, ocorre aumento da velocidade e duração da passada, além de uma menor duração da fase de apoio (FA) na velocidade de 0,22 m/s. Nas velocidades de 0,22 e 0,28 m/s,observa-se aumento da amplitude articular do joelho. CONCLUSÕES: As velocidades moderadas e rápidas parecem ser as mais indicadas para desencadear passadas em bebês que estão iniciando o andar independente, sendo ainda que a velocidade de 0,22 m/s parece ser a mais apropriada. BACKGROUND: The motorized treadmill elicits stepping in infants, however the effects of belt speed on treadmill-elicited stepping patterns are still unknown. OBJECTIVE: To examine the effect of belt speed on treadmill-elicited infant stepping. METHODS: Six normally developing infants, aged between 11 and 13 months and acquiring independent walking, were videotaped while stepping on a treadmill at four belt speeds (0.1, 0.16, 0.22, and 0.28 m/s), with passive markers affixed to the major joints (shoulder, hip, knee, and ankle). The images were digitized and analyzed using the software programs Ariel Performance Analysis System, Matlab, and SPSS (Statistical Package for Social Sciences, v.10). The analyzed variables were the descriptive, temporal, coordinative, and angular characteristics of the treadmill-elicited steps at the four speeds. RESULTS: At 0.22 and 0.28 m/s, stride velocity and duration increased, and the duration of the stance phase decreased at 0.22 m/s. Moreover, at 0.22 and 0.28 m/s, the knee joint range of motion increased. CONCLUSIONS: The moderate and high speeds seem to be more appropriate to elicit stepping in infants who are acquiring independent walking, with 0.22 m/s being the speed that seemed the most appropriate. OBJETIVOS: Comparar o desempenho dos sistemas sensoriais e motor entre jovens e idosos e identificar as contribuições das possíveis diferenças para o controle postural. MÉTODOS: Vinte idosos (68,9±3,7 anos de idade) e 20 jovens (21,9±2,1anos de idade) realizaram testes visuais; somatossensoriais (sensibilidade cutânea e cinestésica); motores (torque articular e latência de ativação muscular) e de controle postural (postura ereta em semi-tandem). RESULTADOS: As análises de variância (ANOVAs) e as análises de multivariância (MANOVAs) indicaram desempenho inferior dos idosos nos testes sensoriais: acuidade visual (p=0,001); sensibilidade ao contraste visual (p=0,009); sensibilidade cutânea (p<0,001); sensibilidade cinestésica de joelho (p<0,001) e tornozelo (<0,001), e motores: torque em flexão de joelho feminino (p=0,010) e masculino (p<0,001); extensão de joelho feminino (p=0,002) e masculino (p<0,001); dorsiflexão de tornozelo feminino (p=0,029) e masculino (p=0,006), flexão plantar de tornozelo feminino (p=0,004) e masculino (p=0,004) e latência de ativação muscular (p<0,001). Os idosos também apresentaram maior oscilação corporal na direção ântero-posterior (p=0,035). Análise de regressão múltipla revelou que a percepção ao movimento passivo foi a única variável que contribuiu para a maior oscilação corporal na direção ântero-posterior em idosos, R²=0,142, p<0,05. CONCLUSÕES: Diferenças no desempenho sensorial e motor ocorrem entre jovens e idosos e atenção deveria ser direcionada para a contribuição do sistema proprioceptivo para o controle postural de idosos.

OBJECTIVES: The aim of this study was to compare the performance of the sensory and motor systems and the contributions of possible differences to postural control. METHODS: Twenty older adults (68.9±3.7 years of age) and twenty young adults (21.9±2.1 years of age) underwent visual, somatosensory (tactile and kinesthetic sensitivity), motor tests (joint torque and muscle activation latency) and postural control assessments (upright semi-tandem posture). RESULTS: MANOVA and ANOVA indicated that older adults had a poorer performance in the sensory tests: visual acuity (p=0.001), visual contrast sensitivity (p=0.009), tactile sensitivity (p<0.001) and kinesthetic sensitivity of the knee (p<0.001) and ankle (<0.001); and in the motor tests: female (p=0.010) and male (p<0.001) knee flexion torque; female (p=0.002) and male (p<0.001) knee extension torque; female (p=0.029) and male (p=0.006) ankle dorsiflexion torque; female (p=0.004) and male (p=0.004) ankle plantar flexion torque; and muscle activation latency (p<0.001). The older adults also had greater body sway amplitude on the anterior-posterior direction (p=0.035). Multiple regression analysis revealed that perception of passive motion was the only variable that contributed to greater body sway on the anterior-posterior direction among older adults (R²=0.142; p<0.05). CONCLUSIONS: There are differences in sensory and motor performance between young adults and older adults, and attention should be directed toward the contribution of the proprioceptive system to postural control among older adults.

Com o avanço da idade, os seres humanos apresentam alterações no controle postural. Tais alterações têm sido consideradas ocorrer em função da diminuição da capacidade dos sistemas sensoriais em fornecer informações e do sistema motor em produzir ações motoras adequadas para manter o corpo equilibrado e em uma posição desejada. Estas alterações levariam a uma diminuição no desempenho do sistema de controle postural e estariam associadas ao aumento da incidência de quedas em idosos. Todavia, essa relação entre alterações sensoriais e motoras e a diminuição no desempenho do sistema de controle postural ainda não é bem compreendida. E, mais importante, os maiores problemas para o funcionamento do sistema de controle postural poderiam não estar associados a alterações em cada um desses sistemas, mas sim, poderiam estar relacionados a alterações no relacionamento entre informação sensorial e ação motora, que ocorrem em idosos. Assim, este trabalho de revisão visa apresentar e discutir os principais aspectos do controle postural, com ênfase no relacionamento entre informação visual e ação motora, e como estes aspectos podem auxiliar o entendimento das alterações observadas no controle postural em idosos.

Aging process has led human beings to exhibit changes in postural control. Such changes have been considered to occur due to problems in the sensory systems to provide information about body position on space, and also to problems in the motor system to produce adequate and coordinate muscle activation to maintain the body in a desirable position. These changes would lead elderly people to decline their performance in postural control and would be associated to the increase of incidence in falls of this population. However, this relationship between sensory and motor changes, and the decrease of the performance in postural control is poorly understood; more important, functioning problems of the postural control system would not be related to changes on each one of these systems. It could be related with changes in the relationship between sensory information and motor action in elderly people during the maintenance of the upright stance. Thus, the purposes of this paper are to present and to argue main aspects related to postural control, with emphasis in the relationship between visual information and motor action, and how these aspects can help us to understand the changes observed in the postural control in elderly people.
