RESUMO A doença renal crônica (DRC) representa um dos principais problemas de saúde pública da atualidade. A dosagem da creatinina sérica e a estimativa da taxa de filtração glomerular (TFG) são as principais ferramentas para avaliação da função renal. Para a estimativa da TFG, existem diversas equações, sendo a mais recomendada a CKD-EPI (Chronic Kidney Disease – Epidemiology). Existem ainda algumas controvérsias com relação à dosagem da creatinina sérica e da estimativa da TFG, uma vez que vários fatores podem interferir nesse processo. Uma importante mudança recente foi a retirada da correção por raça das equações para estimativa da TFG, que superestimavam a função renal, e consequentemente retardavam a implementação de tratamentos como diálise e transplante renal. Neste documento de consenso da Sociedade Brasileira de Nefrologia e Sociedade Brasileira de Patologia Clínica e Medicina Laboratorial são revisados os principais conceitos relacionados à avaliação da função renal, possíveis controvérsias existentes e recomendações para a estimativa da TFG na prática clínica. ABSTRACT Chronic kidney disease (CKD) represents one of today’s main public health problems. Serum creatinine measurement and estimation of the glomerular filtration rate (GFR) are the main tools for evaluating renal function. There are several equations to estimate GFR, and CKD-EPI equation (Chronic Kidney Disease – Epidemiology) is the most recommended one. There are still some controversies regarding serum creatinine measurement and GFR estimation, since several factors can interfere in this process. An important recent change was the removal of the correction for race from the equations for estimating GFR, which overestimated kidney function, and consequently delayed the implementation of treatments such as dialysis and kidney transplantation. In this consensus document from the Brazilian Societies of Nephrology and Clinical Pathology and Laboratory Medicine, the main concepts related to the assessment of renal function are reviewed, as well as possible existing controversies and recommendations for estimating GFR in clinical practice. Resumo Todas as células do corpo humano apresentam acúmulo de globotriaosilceramida (Gb3) na doença de Fabry devido à mutação que ocorre no gene da enzima α-galactosidase A. Trata-se de uma doença ligada ao sexo. Os achados clínicos são: angioqueratomas cutâneos; acroparestesias e acidentes vasculares encefálicos precoces; sudorese diminuída e intolerância ao calor; alterações oculares; hipertrofia miocárdica, arritmias; alterações gastrointestinais e renais. O envolvimento renal ocorre devido ao acúmulo do Gb3 em todos os tipos de células renais. Portanto, os pacientes podem apresentar distúrbios das funções glomerulares e tubulares. Os podócitos são particularmente acometidos, com apagamento dos pedicélios e desenvolvimento de proteinúria. O diagnóstico é feito por meio da detecção de reduzida atividade plasmática ou leucocitária da α-galactosidase e pela detecção da mutação do gene da α-galactosidase. O tratamento com reposição enzimática contribui para o retardo da progressão da doença renal, principalmente se instituído precocemente.

Abstract Every cell in the human body has globotriaosylceramide accumulation (Gb3) in Fabry disease due to the mutation in gene of the enzyme α-galactosidase A. It is a disease linked to sex. The main clinical features are: cutaneous angiokeratomas; acroparestesias and early strokes; decreased sweating and heat intolerance; ocular changes; myocardial hypertrophy, arrhythmias; gastrointestinal disorders and renal involvement. Renal involvement occurs due to Gb3 accumulation in all types of renal cells. Therefore, patients may present glomerular and tubular function disorders. Podocytes are particularly affected, with pedicels effacement and development of proteinuria. The diagnosis is made by detection of reduced plasma or leukocyte α-galactosidase activity and genetic study for detecting the α-galactosidase gene mutation. Treatment with enzyme replacement contributes to delay the progression of kidney disease, especially if initiated early.

Nesta revisão, são explicados os fenômenos envolvidos nas trocas de fluidos e solutos através da membrana peritoneal, tanto na situação fisiológica quanto no contexto da diálise peritoneal. Para tanto, são utilizados os modelos matemáticos desenvolvidos para estudo do transporte pela membrana, tais como o "Modelo de Poros" e o "Modelo Distributivo". Os ganhos científicos com as pesquisas nesse campo são contemplados e as áreas que merecem pesquisas adicionais também são citadas. Assim, o estado atual do conhecimento fisiológico a respeito dessa modalidade de terapia renal substitutiva, no que se refere aos eventos relacionados à membrana peritoneal, encontra-se sintetizado nesse manuscrito.

In this review, phenomena involved in fluid and solute exchange through the peritoneal membrane, both in the physiologic and in the peritoneal dialysis settings, are explained. For that purpose, mathematical models developed for the study of molecule transport through the membrane, such as the "Pore Model" and the "Distributive Model" are used. Scientific accomplishments in the field are described and areas that require additional research are also cited. Knowledge about the physiologic mechanisms involved in this renal replacement therapy modality, concerning events directly related to the peritoneal membrane itself, is synthesized in this manuscript.

Pacientes com insuficiência renal crônica terminal em uso de diálise peritoneal (DP) estão sujeitos a diversas complicações da própria terapia de substituição renal. Relatamos uma complicação rara da DP na qual o peritôneo, após anos de contato com a substância hipertônica dialisante, é gradualmente substituído por tecido fibroso. O paciente em questão teve diversas intercorrências após o início da DP, incluindo uma peritonite bacteriana, hiperparatireoidismo terciário (sendo tratado com duas paratireoidectomias) e colelitíase (sendo tratado com colecistectomia videolaparoscópica). Após 8 anos de diálise peritoneal, foi transferido para hemodiálise por diminuição da ultrafiltração e episódios de suboclusão intestinal, sendo diagnosticado como peritonite esclerosante encapsulante (PEE). Atualmente, está em corticoterapia e com diminuição significativa dos sintomas e provável estabilização da PEE.

Patients with chronic renal failure in use of peritoneal dialysis (PD) are subject to various complications of the renal replacement therapy. We report a rare complication of PD in which the peritoneum, after years of contact with hypertonic dialysate, is gradually replaced by fibrous tissue. This patient had several complications after initiation of PD including a bacterial peritonitis, tertiary hyperparathyroidism (being treated with parathyroidectomy 2) and cholelithiasis (being treated with laparoscopic cholecystectomy). After 8 years of peritoneal dialysis was transferred to hemodialysis by decreasing ultrafiltration and episodes of intestinal sub-occlusion, being diagnosed as sclerosing encapsulating peritonitis (SEP). He is currently on corticotherapy with a significant reduction of symptoms and likely stabilization of the SEP.

Ultrafiltration failure in patients undergoing peritoneal dialysis is a condition with an incidence that increases over time. It is related to increased cardiovascular morbidity and mortality and is a major cause of the abandonment of the treatment technique. Because the number of patients undergoing renal replacement therapy is increasing with society aging and because approximately 10% of this population is treated with peritoneal dialysis, this matter is becoming more common in everyday practice for clinicians involved in the care of patients with chronic renal failure. In this review, we summarize the available measures used to prevent and treat ultrafiltration failure and the current state of research in the field, both in the experimental and clinical settings, focusing on the possible clinical applications of recent findings.

Anemia é uma complicação quase universal nos pacientes em estágios avançados de doença renal crônica (DRC). Ela está associada com maior número de internações hospitalares, maior mortalidade e pior qualidade de vida dos pacientes. Ela tem várias causas, sendo deficiência de eritropoetina e ferro as duas principais causas. A condição inflamatória presente na DRC interfere com a ação da eritropoetina e com a absorção intestinal de ferro e mobilização de ferro dos estoques, devido ao aumento de hepcidina. A correção parcial (não completa) da anemia promove melhores resultados nos pacientes com DRC.

Anemia is an almost universal complication of patients in advanced stages of chronic kidney disease (CKD). It is associated with more hospitalizations, increased mortality and worse quality of life. Although there are several causes, erythropoietin and iron deficiency are the most common. The inflammatory condition present in CKD interferes with the action of erythropoietin, intestinal iron absorption and iron mobilization from deposits, due to increased hepcidin concentrations. Partial but incomplete correction of anemia promotes better outcomes in patients with CKD.

A doença cardiovascular (DCV) permanece sendo uma das maiores causas de morte em pacientes com doença renal crônica (DRC). A hipertrofia ventricular esquerda (HVE) está presente em 75% dos pacientes ao iniciarem diálise, sugerindo que esta deve estar presente precocemente no curso da DRC. Poucos estudos avaliaram a prevalência de HVE na pré-diálise. Foram avaliados 309 pacientes clinicamente estáveis em acompanhamento por pelo menos três meses em cinco Centros no Brasil. Perfil bioquímico e marcadores inflamatórios foram avaliados. Dados são apresentados como media ± DP. Observamos que a HVE esteve presente em 53% dos pacientes, idade = 60 ± 13 anos, e 55 ± 14 anos para aqueles sem HVE. Diabetes mellitus como doença de base esteve presente em 35% dos pacientes em ambos os grupos. Filtração glomerular estimada foi 30 ± 11 e 32 ± 12 mL/min para pacientes com HVE e sem, respectivamente (p = 0,19). A distribuição de pacientes mostrou que 60% com HVE se encontravam no estágio 4. Análise logística multivariada mostrou que eram determinantes independentes para HVE: idade (p < 0,001), cálcio (p < 0,001), hemoglobina (p < 0,048) e pressão arterial diastólica (p < 0,001). Pressão arterial sistólica, lipídeos e marcadores inflamatórios não se correlacionaram com a HVE. Em conclusão, a incidência de HVE foi alta mesmo entre pacientes sob tratamento especializado e com exceção da idade, a HVE se correlacionou com fatores reversíveis. Alertamos para a necessidade do diagnóstico da DRC e prevenção da HVE na pré-diálise de forma rigorosa para diminuir a mortalidade decorrente de DCV nesta população.

Cardiovascular disease (CVD) remains the major cause of death in patients with chronic kidney disease (CKD). Left ventricular hypertrophy (LVH) is present in 75% of patients starting dialysis, suggesting that LVH might be present from an early stage of CKD. Few studies have addressed the predialysis prevalence of LVH. This study evaluated 309 clinically stable patients under treatment for at least three months at five Brazilian centers. Biochemical profile and inflammatory markers were assessed. Data were shown as mean ± SD. Left ventricular hypertrophy was present in 53% of the patients, whose mean age was 60 ± 13years. The mean age of those without LVH was 55 ± 14 years. Diabetes mellitus was the underlying disease in 35% of the patients in both groups. Estimated glomerular filtration rate was 30 ± 11 and 32 ± 12 mL/min for patients with and without LVH, respectively (p = 0.19). The distribution of patients showed that 60% of those with LVH were in stage 4. Multivariate logistic regression analysis indicated the following independent determinants for LVH: age (p < 0.001); calcium (p < 0.001); hemoglobin (p < 0.048); and diastolic blood pressure (p < 0.001). Systolic blood pressure, lipids, and inflammatory markers showed no correlation with LVH. In conclusion, the incidence of LVH was high even among patients under conservative treatment, and, except for age, LVH correlated with reversible factors. The need for strictly diagnosing CKD and preventing LVH in the predialysis phase is emphasized to decrease mortality due to CVD in that population.

OBJETIVO: Avaliar a variabilidade dos níveis de hemoglobina (Hb) em pacientes em hemodiálise (HD) tratados com eritropoetina. MÉTODOS: Foram coletados dados retrospectivos de 249 pacientes que estavam em HD e apresentavam, nos três meses anteriores, média de Hb entre 10,5 g/dL e 12,5 g/dL. O período de observação total foi de 36 meses. A cada mês de coleta, classificaram-se os valores de Hb em: < 10,5g/dL, 10,5g/dL< Hb< 12,5g/dL (intervalo alvo), ou Hb >12,5g/dL. Além disto, os pacientes foram divididos em seis categorias de variabilidade da Hb: baixo persistente (<10,5g/dL), alvo persistente (10,5 a 12,5 g/dL), alto persistente (>12,5g/dL), flutuação de baixa amplitude com Hb baixo, flutuações de baixa amplitude com Hb alto e flutuações de alta amplitude. RESULTADOS: Mês a mês, a média da proporção de pacientes com Hb dentro da faixa alvo foi de 50% (variação, 42% a 61%). A proporção de valores de Hb médios acima da faixa alvo (30%) foi mais frequente que a proporção abaixo do alvo (20%). Durante os períodos de seis, 12, e 36 meses, a proporção de pacientes com Hb baixa persistente se reduziu de 3,6% para 0%; de 31,7% para 2,8% naqueles com Hb alta persistente; de 7,6% para 0% naqueles com baixa amplitude com Hb baixo; e de 41,3% para 8,3% nos pacientes com baixa amplitude com Hb alto. Entretanto, houve aumento na proporção de pacientes (de 21,5% a 88,9%) com alta amplitude de Hb. Portanto, à medida que o tempo de observação se alongou observou-se maior variabilidade dos valores de hemoglobina. Nenhum paciente manteve os níveis de Hb dentro do alvo durante todo o período do estudo. CONCLUSÃO: A manutenção da Hb dentro da faixa alvo é difícil, especialmente em períodos longos e a variabilidade ocorre mais frequentemente para valores mais elevados de Hb.

OBJECTIVE: Correction of anemia using epoetin decreases morbidity and increases survival and quality of life in end-stage renal disease. Maintaining hemoglobin levels within the range proposed by guidelines has become a major challenge, with hemoglobin cycling affecting more than 90% of patients undergoing hemodialysis. The variability of hemoglobin levels over time was assessed in our patients. METHODS: Data were retrospectively collected on 249 patients undergoing hemodialysis over a 3-year period at seven centers in Brazil. Hemoglobin was measured at least monthly, and target levels were those between 10.5 g/dL and 12.5 g/dL. Patients were grouped into six categories of variability consistently low (<10.5g/dL), consistently target range (10.5 to 12.5 g/dL), consistently high (>12.5g/dL), low amplitude fluctuation with low hemoglobin levels, low amplitude fluctuation with high hemoglobin levels and high amplitude fluctuation. None of the patients maintained stable hemoglobin levels for the entire 36-month period. RESULTS: The mean monthly proportion of patients that had hemoglobin levels within the target range was 50% (range, 42% to 61%). Mean levels above the target (30%) were more frequent than those below it (20%). During 6, 12, and 36 months, proportions of patients with consistently low levels of hemoglobin decreased from 3.6% to 0%, from 31.7% to 2.8% for those with consistently high, from 7.6% to 0% for those with low amplitude fluctuation with low hemoglobin levels and from 41.3% to 8.3% for those with low amplitude fluctuation with high hemoglobin levels. However, the proportions of patients with high amplitude fluctuation increased from 21.5% to 88.9%. CONCLUSION: Maintaining hemoglobin levels within the target range is difficult, especially for longer periods of time. Missing the target seems more often due to levels above it, but high-amplitude fluctuations eventually occur in the majority of patients.

Fibrose sistêmica nefrogênica (FSN), também conhecida como dermopatia fibrosante nefrogênica (DFN), é uma condição que ocorre apenas em pacientes com disfunção renal. Além das lesões cutâneas, esta síndrome inclui fibrose de músculo esquelético, articulações, fígado, pulmão e coração e pode ser fatal. Esta doença foi primeiramente descrita em 1997 e vários estudos descrevem a relação etiológica da FSN com a exposição a agentes de contraste contendo gadolínio, usado em exames de ressonância magnética. Esta revisão tem como objetivo alertar médicos clínicos e nefrologistas sobre essa nova patologia que acomete pacientes com alteração da função renal, demonstrando aspectos demográficos e epidemiológicos, apresentação clínica, diagnóstico e prognóstico além das opções de prevenção e terapêuticas atuais. Concluímos que todo paciente apresentando creatinina sérica elevada deve ter sua função renal (clearance de creatinina) estimada, visando a segurança na realização da ressonância magnética.

Nephrogenic systemic fibrosis (NSF), also known as nephrogenic fibrosing dermopathy (NFD), is a condition that has occurred only in patients with renal insufficiency. Besides lesions of the skin, this syndrome include fibrosis of skeletal muscle, joints, liver, lung, and heart, with possible fatal outcomes. This disease was first described in 1997 and several reports described the development of NSF after the exposure to gadolinium-based magnetic resonance imaging contrast agents. This review aims to alert physicians and nephrologists about this new pathology that affects patients with renal dysfunction, describing its demographic and epidemiologics aspects, clinic presentation, diagnosis and prognosis, beyond options to prevent and current treatment. We concluded that in all patient with elevated serum creatinine physicians should estimade his kidney function (creatinine clearence) in order to safety of magnetic resonance.

Criou-se um espaço na internet para a discussão de casos clínicos no website da Sociedade Brasileira de Nefrologia (http://www.sbn.org.br). O objetivo deste trabalho é apresentar o funcionamento deste espaço virtual e os aprimoramentos gerados após sua implantação, em setembro de 2001. Atualmente, 62 casos já foram divulgados e diagnosticados. Todos estão arquivados neste espaço para consulta sem cadastro prévio. A internet mostrou-se também um ambiente propício à discussão anatomoclínica, uma vez que permite a participação de profissionais de centros distantes e sem condição local para este tipo de atividade, tão importante para o contínuo aprendizado da Medicina.

An environment was built on the Internet for discussing clinical cases on the website of the Brazilian Society of Nephrology (http://www.sbn.org.br). The aim of this work is to present the operation of this system and the improvements made since its introduction in September 2001. Up to the present moment, 62 cases have been published and diagnosed. All of them are stored in the website for consultation without any previous registration. The Internet also proved to be a promising environment for clinical-pathological discussion for allowing the participation of professionals from distant centers and without local conditions for this kind of activity of great importance for continuing education in Medicine.
