Abstract: Introduction: Acute myocardial infarction has a high prevalence and possibility of death if timely care is not given. It is possible that treatment could be delayed during a health crisis such as the COVID-19 pandemic. Objective: The objective of this study was to evaluate the impact of the COVID-19 pandemic on the function of the infarction code in the High Specialty Medical Unit of our institution. The medical indicators of acute coronary ischemic syndrome with ST-segment elevation were compared at two times: before the COVID-19 pandemic and during it. Material and methods: An ambispective, analytical, comparative cohort study was conducted, comparing the periods of February-September 2019 and February-September 2020, in the High Specialty Medical Unit, Centro Médico del Bajío number 1, Instituto Mexicano del Seguro Social, Leon, Guanajuato, Mexico. All patients older than 18 years with clinical, electrocardiographic, and enzymatic data of acute coronary ischemic syndrome, accepted in the medical network as infarction code registered in the National Registry of Acute Coronary Ischemic Syndromes (RENASCA) electronic case report form were included in the study. Results: The independent samples t-test was used to determine the impact of the infarction code initiative based on the reperfusion therapies performed, the door-to-needle, the door-to-balloon time, and the ischemia time. There was a significant difference in the reperfusion strategy, with greater thrombolysis during the COVID-19 pandemic (57.4% versus 72.6%, odds ratio [OR] 1.97, 95% confidence interval [CI] 0.49-0.82, p < 0.001), as well as reperfusion criteria (21.03% versus 35.37%, OR 2.05, 95% CI 0.53-0.80, p < 0.0001). Mortality was not different between the time periods. Conclusions: The COVID-19 pandemic has not impacted operation of the infarction code in the High Specialty Medical Unit Centro Médico del Bajío number 1 because the medical indicators of acute coronary ischemic syndrome with ST-segment elevation did not change.

Abstract Background: The Sierra Madre Oriental is a Physiographic Province located in northeastern Mexico and has distinctive geological, climatic, and edaphic characteristics. The flora in this region has not been inventoried as a whole. Question: What is the floristic diversity of the Physiographic Province of the Sierra Madre Oriental? What is the geographic affinity of its genera? Site and years of study: The Physiographic province of the Sierra Madre Oriental. The work was conducted from August 2012 to December 2020. Methods: An exhaustive search for information was carried out using various sources such as herbaria, databases, floristic works, and monographs. To each species was registered the type of vegetation, geographic affinity and endemism. Results: A total of 6,981 species are registered; 1,542 are endemic to the Sierra Madre Oriental. The most diversity families were Asteraceae (1,000 species), Fabaceae (495), Cactaceae (365) and Poaceae (328). The temperate forests housed the greatest number of species (2,906). Querétaro was the richest state (2,803), followed by Coahuila with (2,710) and Nuevo León (2,406). The greatest number of genera of the species in the Sierra Madre Oriental have geographical boreal affinity (2,742), followed by species with tropical affinity (2,020), endemics to Mexico (1,227), with a cosmopolitan distribution (803), and from the deserts of the world (189). Conclusions: The Sierra Madre Oriental is home to more than a quarter of the vascular flora and 13 % of the endemic plants from Mexico. The area combines genera of different geographic affinities, mostly boreal, followed by tropical and endemic.
Resumen Antecedentes: La Sierra Madre Oriental es una Provincia Fisiográfica ubicada en el noreste de México con características geológicas, climáticas y edáficas distintivas. La flora de esta región aún no ha sido inventariada en conjunto. Preguntas: ¿Cuál es la diversidad florística de la Provincia Fisiográfica de la Sierra Madre Oriental? ¿Cuál es la afinidad geográfica de sus géneros? Sitio y años de estudio: Provincia Fisiográfica de la Sierra Madre Oriental. El trabajo se desarrolló desde agosto del 2012 a diciembre del 2020. Métodos: Se hizo una búsqueda exhaustiva de información en diversas fuentes como herbarios, bases de datos, trabajos florísticos y monografías. Para cada especie, se registró la vegetación, afinidad geográfica del género y endemismo. Resultados: Se registran 6,981 especies de las cuales 1,542 son endémicas. Las familias más ricas en especies son Asteraceae (1,000 especies), Fabaceae (495), Cactaceae (365) y Poaceae (328). Los bosques templados registran el mayor número de especies (2,906). Querétaro fue el estado con más especies (2,803), seguido de Coahuila (2,710) y Nuevo León (2,406). La Sierra Madre Oriental comprende una mayor cantidad de especies con géneros de afinidad geográfica boreal (2,742), seguidas por especies de afinidad tropical (2,020), especies endémicas de México (1,227), cosmopolitas (803) y de los desiertos del mundo (189). Conclusiones: La Sierra Madre Oriental alberga más de la cuarta parte de la flora vascular y un 13 % de las plantas endémicas del país. Es un área que reúne géneros de diferentes afinidades geográficas en su mayoría boreales, seguidos de tropicales y endémicos.

Características epidemiológicas de la cirrosis hepática en el Hospital Civil de Guadalajara

Objective. A prospective study of the demographic and epidemiologic characteristics of patients with liver cirrhosis at the Hospital Civil de Guadalajara within a period of one year. Material and methods. A total of 157 patients (48 women, and 109 men) from the Departments of Internal Medicine, Gastroenterology and Liver Clinic were studied. Diagnosis of liver cirrhosis had been previously established based on clinical, biochemical or histopathological information. Additionally, a questionnaire specialized on liver diseases was applied to all patients. Results. Alcoholism was the main cause for cirrhosis (38% in women and 95% in men) followed by viral etiology. The alcoholic beverages consumed with greater frequency were tequila and 96° G.L. alcohol. Women were in the Child-Pugh’s "B" class while men were in class "C". Complications were mostly gastrointestinal bleeding, ascitis and hepatic encephalopathy. Differences between sexes were observed for various characteristics. Conclusions. Alcoholism was by far the most frequent cause of liver cirrhosis. The second cause, in women, was of viral origin (16.7%). A national committee of vigilance of liver diseases is proposed to generate more complete and detailed information on the epidemiology of liver cirrhosis.
Objetivo. Estudiar prospectivamente las características demográficas y epidemiológicas de los pacientes con cirrosis hepática en el Hospital Civil de Guadalajara en un periodo de un año. Material y métodos. Se estudiaron 157 pacientes (48 mujeres y 109 hombres), de los servicios de Medicina Interna, Gastroenterología y Clínica de Hígado con diagnóstico de cirrosis hepática el cual se hizo con base en la información clínica, bioquímica o histopatológica; asimismo, se les aplicó un cuestionario especializado en enfermedades hepáticas. Resultados. La principal causa de la cirrosis fue el alcoholismo (38% en mujeres y 95% en hombres), seguida de la etiología viral. Las bebidas más frecuentes fueron el tequila y el alcohol de 96° G.L. El grado de insuficiencia hepática más frecuentemente observado fue el de Child-Pugh "B" en mujeres y "C" en los hombres. Las complicaciones más frecuentes fueron hemorragia de tubo digestivo, ascitis y encefalopatía hepática. Se observaron diferencias en varias características relacionadas con el sexo de los pacientes. Conclusiones. El alcoholismo fue la primera causa de cirrosis hepática. En mujeres la segunda causa fue la viral (16.7%). Se propone un comité nacional de vigilancia de enfermedades del hígado, para generar una información más completa y detallada acerca de la epidemiología de la cirrosis hepática.
