Resumen Objetivo: analizar las tendencias en las tasas de mortalidad por enfermedad de Alzheimer en Brasil y sus macrorregiones por grupo de edad y sexo, de 2000 a 2019. Métodos: estudio de series temporales de mortalidad por enfermedad de Alzheimer en Brasil y sus macrorregiones por grupo de edad y sexo; los datos se obtuvieron del Sistema de Información sobre Mortalidad del Ministerio de Salud de Brasil; se utilizó el modelo Prais-Winsten para analizar tendencias. Resultados: hubo 211.658 óbitos, con tendencia creciente en la mortalidad por enfermedad de Alzheimer en el país, en adultos mayores de 60-69 años (VPA = 4,3; IC95% 2,9;5,9), 70-79 años (VPA = 8,1; IC95%: 4,8;11,5) y ≥ 80 años (VPA = 11,3; IC95% 8,1;14,6) y en todas las macrorregiones, grupos de edad y sexo. Conclusión: Brasil y todas sus macrorregiones mostraron una tendencia creciente en las tasas de mortalidad por enfermedad de Alzheimer siguiendo la tendencia mundial.

AN ANALYSIS OF THE PROCESS OF DEVELOPING QUESTIONS PRODUCED BY CHEMISTRY STUDENTS IN A TEACHING PROGRAM BASED ON SCIENTIFIC COMMUNICATION LITERATURE. The objective of the present investigation was to evaluate the use of scientific dissemination literature (SDL) in the process of formulating questions developed by chemistry students in a Teaching Program in Chemistry. To this end, the book entitled The Disappearing Spoon was analyzed in terms of categories referring to format and content, while the 18 questions produced by these students about the chapters read were analyzed in terms of presupposition and request. The investigation was carried out with 16 undergraduates who were taking a theoretical course on “Introduction to Chemistry”, as part of the Teaching Program in Chemistry. According to the analysis carried out, some particularities of format, such as the scarcity of visual resources, the autonomous structure of the chapters and the language, may have influenced the elaborated presentations. It is interesting to note that almost half of the questions produced were of an investigative nature, demonstrating the potential of SDL to stimulate high cognitive skills. In addition, the questions with investigative requests were mostly opinion questions, which may have been motivated by characteristics of the text read, such as the content with a controversial approach used by the author, when reporting on some aspects of scientific activity.

Abstract Background: The Val66Met polymorphism of the brain-derived neurotrophic factor (BDNF) gene is a potential biomarker of vulnerability to pain. Thus, the present study aimed to investigate the association of this polymorphism with clinical and biopsychosocial factors in patients with chronic low back pain (CLBP). Methods: A total of 107 individuals with CLBP answered questionnaires that were validated and adapted for the Brazilian population, including the Brief Inventory of Pain, the Central Sensitization Inventory, the Roland Morris Disability Questionnaire, the Tampa Scale for Kinesiophobia, the Pain Catastrophizing Scale, the Survey of Pain Attitude-Brief, and the Hospital Anxiety and Depression Scale. All of the subjects were genotyped for the BDNF Val66Met polymorphism. Results: The sample showed moderate scores of disability, central sensitization, and kinesiophobia, in addition to mild anxiety, hopelessness, and ruminant thoughts. No significant association was observed between the Val66Met polymorphism and the variables analyzed. Besides, there was no relationship between the BDNF Val66Met polymorphism with CSI, catastrophization, or disabilities that were generated by CLBP. Conclusions: The results showed that the Val66Met polymorphism of the BDNF gene was not associated with clinical and biopsychosocial characteristics of CLBP in the sample studied.

Abstract Purpose: To evaluate the quality of referrals for a first Rheumatology consultation at a tertiary care center in a southern Brazilian capital (Porto Alegre, RS), having as background findings from a similar survey performed in 2007/2008. Since then, our state has implemented referral protocols and a triage system with teleconsulting support exclusively for referrals from locations outside the capital, permitting a comparison between patients screened and not screened by the new system. Methods: Physicians of the Rheumatology Service at Hospital Nossa Senhora da Conceição prospectively collected information regarding first visits over a 6-month period (Oct 2017 to March 2018). We recorded demographic characteristics, diagnostic hypotheses, date of referral, and the municipality of origin (within the state of Rio Grande do Sul). We considered adequate referrals from primary health care when a systemic autoimmune inflammatory disease (SIRD) was suspected at first evaluation by the attending rheumatologist. Results: Three hundred fifty-seven patients/appointments were eligible for analysis (193 from the capital and 164 from small and medium towns). In 2007/2008, suspected SIRD occurred in 76/260 (29.2%) and 73/222 (32.9%) among patients from the capital and outside counties, respectively (P = 0.387). In 2017/2018, suspected SIRD occurred in 75/193 (38.9%) and 111/164 (67.7%) in patients from the capital and outside counties, respectively (difference: 28.8, 95% CI: 19.0 to 38.9, P < 0.001), indicating a marked improvement in referrals submitted to the new triage system. Conclusion: The quality of Rheumatology referrals in our state improved over the 10-year interval under study, particularly among patients from locations submitted to referral protocols and teleconsulting support.

Calorie and protein delivery in critically ill surgical and non-surgical patients receiving enteral nutrition therapy

ABSTRACT Providing enteral nutritional therapy to critically ill patients is a challenge, especially during the first days after starting intensive care. These challenges appear to be greater in critically ill surgical patients. Objectives: Describe and compare enteral nutrition practices in critically ill surgical and non-surgical patients. Methods: We conducted a prospective cohort study of surgical and non-surgical patients receiving exclusive enteral nutritional therapy. The values for calorie and protein delivery during the first week after admission to the intensive care unit were recorded. Results: 103 patients were enrolled (54.4% male, mean age: 63.9 years, 26.2% surgical). The median time of initiation of enteral nutritional therapy was the third day in non-surgical and the fourth day in surgical patients. Surgical patients had a lower calorie and protein delivery than non-surgical patients on the second to fourth days after admission. At the end of the first week, 20.2% of the patients had not received any diet, and there was no significant difference in nutrient delivery between the groups. Of the surgical and non-surgical patients, 42.9% and 39.3% were receiving ≥20 kcal/kg/day, and 28.6% and 34.4% were receiving ≥1.2 g/kg of protein per day, respectively. Conclusion: There was a delay in starting patients on enteral nutritional therapy and calorie and protein delivery was low, especially among surgical patients. By the end of the first week, calorie and protein delivery was similar in both groups.
RESUMEN Proporcionar terapia nutricional enteral a pacientes críticos es un desafío, especialmente durante los primeros días después de comenzar los cuidados intensivos. Estos desafíos parecen ser mayores en pacientes quirúrgicos críticos. El propósito del estudio fue describir y comparar las prácticas de nutrición enteral en pacientes críticos quirúrgicos y no quirúrgicos. Se realizó un estudio de cohorte prospectivo de pacientes quirúrgicos y no quirúrgicos que recibieron terapia nutricional enteral exclusiva. Se registraron los datos de entrega de calorías y proteínas durante la primera semana después de la admisión a la unidad de cuidados intensivos. Se estudiaron 103 pacientes (54,4% hombres, edad media: 63,9 años, 26,2% quirúrgicos). La mediana de tiempo de inicio de la terapia nutricional enteral fue el tercer día en pacientes no quirúrgicos y el cuarto día en pacientes quirúrgicos. Los pacientes quirúrgicos tuvieron una entrega de calorías y proteínas más baja que los pacientes no quirúrgicos en el segundo a cuarto día después del ingreso. Al final de la primera semana, el 20,2% de los pacientes no habían recibido ningún aporte nutricional, y no hubo diferencias significativas en el suministro de nutrientes entre los grupos. De los pacientes quirúrgicos y no quirúrgicos, 42,9% y 39,3% estaban recibiendo ≥20 kcal/kg/día, y 28,6% y 34,4% estaban recibiendo ≥1,2 g/kg de proteína por día, respectivamente. Conclusiones: Hubo un retraso en el inicio de los pacientes en terapia nutricional enteral y la entrega de calorías y proteínas fue baja, especialmente entre los pacientes quirúrgicos. Al final de la primera semana, la entrega de calorías y proteínas fue similar en ambos grupos.

RESUMO A disposição espacial em que plantas se encontram na área de cultivo é fator que influencia diretamente na produtividade de grãos, pois o melhor arranjo entre essas pode resultar em maior aproveitamento de água, luz e nutrientes. O objetivo do trabalho foi avaliar o desempenho agronômico de sorgo semeado em espaçamento de fileiras duplas no Cerrado. Para tanto, foi conduzido nas safrinhas de 2018 e 2019, experimentos de campo em delineamento de blocos completos em esquema fatorial 2 x 5, com quatro repetições, em que se utilizou dois espaçamentos de entrelinhas de semeadura: tradicional (0,50 m) e fileiras duplas (0,25 m/0,75 m) e cinco populações de plantas: 120, 180 (recomendada), 240, 300 e 360 mil plantas ha-1. Foram avaliadas características morfológicas, componentes do rendimento e produtividade de grãos utilizando análise conjunta dos dados para os dois anos de estudo. Em 2018, devido ao menor volume de chuvas durante o ciclo da cultura, a utilização do arranjo em fileiras duplas apresentou produtividade de grãos inferior ao espaçamento tradicional. Por outro lado, em 2019, ano com melhor distribuição hídrica, a produtividade foi superior a 2018. O aumento da população de plantas por hectare incrementou a produtividade de grãos de sorgo, com ganhos de até 24% em relação à população recomendada de 180 mil plantas ha-1, demonstrando que o adensamento nessa cultura pode ser uma técnica promissora desde que levado em consideração seu custo/benefício.
ABSTRACT The spatial arrangement of plants in the cultivation area is a factor that directly influences grain yield, since the best arrangement can result in the greater use of water, light and nutrients. The aim of the study was to evaluate the agronomic performance of sorghum sown in double row spacing in the Brazilian Cerrado. For this purpose, field experiments were conducted in the 2018 and 2019 seasons in a complete block design with a factorial scheme of 2 x 5. There were four replications, using two row spacing: traditional (0.50 m) and double rows (0.25 m/0.75 m) and five plant populations: 120, 180 (recommended), 240, 300 and 360 thousand, plants ha-1. Morphological characteristics, yield components and grain yield were evaluated using joint data analysis for the two years of study. In 2018, due to lower rainfall during the crop season, the use of a double row arrangement presented grain yield lower than traditional spacing. On the other hand, in 2019, the year with the best water distribution, the yield was superior to that of 2018. The increase in plant population per hectare increased the yield of sorghum grains, with gains of up to 24% in relation to the recommended population of 180 thousand plants ha-1, demonstrated that the densification in this crop can be a promising technique as long as its cost/benefit is considered.

RESUMO O consórcio de sorgo granífero com braquiária proporciona a produção de grãos e massa seca na entressafra; contudo, há carência de informações relacionadas à implantação dessas espécies. Sendo assim, este trabalho teve por objetivo identificar a melhor associação de sorgo granífero e braquiária na segunda safra para produção de grãos e massa seca na região do Cerrado. O experimento foi conduzido em Rio Verde, Goiás, no delineamento experimental de blocos randomizados, em esquema fatorial 2×3×4+5, com quatro repetições. Foram avaliados dois híbridos de sorgo granífero (BRS 330 e DKB 551) associados a três espécies de Brachiaria (Brachiaria brizantha cultivar Marandu, Brachiaria decumbens e Brachiaria ruziziensis) em quatro sistemas de implantação da braquiária (linha, entrelinha, linha + entrelinha do sorgo e a lanço). Os tratamentos adicionais referiam-se aos dois monocultivos de sorgo e às três espécies de braquiária. O consórcio de sorgo com braquiária ocasionou redução no rendimento de grãos, porém os sistemas de implantação da braquiária na linha, entrelinha e a lanço foram os mais promissores. O híbrido de sorgo DKB 551 foi o mais sensível à competição com a braquiária, sem, contudo, haver diferenças de rendimento de grãos em consórcio. Os consórcios das braquiárias na entrelinha e na linha + entrelinha, principalmente com a B. ruziziensis, foram os mais promissores para produção de massa seca e proteína bruta total. O cultivo de braquiária com o sorgo proporcionou maior aporte de massa seca na entressafra, fato importante para o sistema de plantio direto nos sistemas de produção agrícola dos Cerrados.
ABSTRACT The grain sorghum intercropped with Brachiaria provides the production of grains and dry mass in the off-season; however, there is a lack of information related to the implantation of these species. The objective of this study was to identify the best association of sorghum and Brachiaria in the production of grain and dry matter in the Brazilian Cerrado region. The experiment was conducted in Rio Verde, State of Goiás, in the experimental design of randomized blocks, in a 2×3×4+5 factorial scheme, with four replications. Two sorghum hybrids (BRS 330 and DKB 551) associated to three Brachiaria species (Brachiaria brizantha cultivar Marandu, Brachiaria decumbens and Brachiaria ruziziensis) were evaluated in four systems of Brachiaria implantation (row, interrow, row + interrow of sorghum and spread). Additional treatments referred to the two sorghum monocrops and the three Brachiaria species. The sorghum and Brachiaria consortium resulted in a reduction in grain yield, but the Brachiaria implantation systems in the row, interrow and spread were the most promising. The sorghum hybrid DKB 551 was the most sensitive to the competition with Brachiaria, although there were no grain yield differences in the intercropping. The Brachiaria intercropped in the interrow and in the row + interrow, mainly with B. ruziziensis, were the most promising for the production of dry mass and total crude protein. The cultivation of Brachiaria intercropped with sorghum provided a greater contribution of dry mass in the off-season, an important fact for the no-tillage system in the agricultural production systems of the Brazilian Cerrado.

Abstract Background: Fibromyalgia (FM) is a musculoskeletal chronic pain syndrome that impacts negatively patient's daily lives. Its pathogenesis is characterized by a complex relationship between biological and psychosocial factors not fully understood yet. Pain catastrophizing is associated with FM and is an important predictor of outcomes. This study aimed to answer two questions: (i) whether the allele and genotype frequencies of BDNF Val66Met (rs6265) polymorphism differs between FM patients and healthy controls (HC); and (ii) if the BDNF Val66Met polymorphism is a factor that predicts pain catastrophizing in FM. Methods: In a cross-sectional design, 108 FM patients and 108 HC were included. FM patients responded to the Brazilian Portuguese version of the Pain Catastrophizing Scale (BP-PCS) to assess pain catastrophizing, as well as other validated tools for anxiety (The State-Trait Anxiety Inventory - STAI), depression (Beck Depression Inventory II -BDI-II) and functional aspects (Fibromyalgia Impact Questionnaire - FIQ; Central Sensitization Inventory validated and adapted for Brazilian population - CSI-BP; Pittsburgh Sleep Quality Index - PSQI; and Resilience Scale). All subjects were genotyped for the BDNF Val66Met polymorphism. Results: Val allele was significantly more frequent in FM patients compared to the control group (p < 0.05). Also, FM patients with Val/Val genotype showed more pain catastrophizing thoughts, and this genotype was significantly associated with magnification and rumination dimensions of BP-PCS (p < 0.05). Furthermore, there were significant differences in levels of anxiety and symptoms of depression, years of education, and the functional situation between the FM and control groups. Conclusions: The findings show an association of BDNF Val66Met polymorphism with pain catastrophizing in FM, which opens new avenues to comprehend the interplay between molecular genetic characteristics and neuroplasticity mechanisms underpinning FM.

Abstract Functional dyspepsia and lactose intolerance (adult-type hypolactasia, ATH) are common conditions that may coexist or even be confounded. Their clinical presentation can be similar, however, lactose intolerance does not form part of the diagnostic investigation of functional dyspepsia. Studies on the association between functional dyspepsia and ATH are scarce. This study aimed to evaluate whether ATH is associated with symptoms of functional dyspepsia. Patients fulfilling the Rome III diagnostic criteria for functional dyspepsia underwent genetic testing for ATH. Dyspeptic symptoms were evaluated and scored according to a validated questionnaire. The diagnostic criteria for ATH was a CC genotype for the -13910C/T polymorphism, located upstream of the lactase gene. The mean scores for dyspeptic symptoms were compared between patients with ATH and those with lactase persistence. A total of 197 functional dyspeptic patients were included in the study. Mean age was 47.7 years and 82.7% patients were women. Eighty-eight patients (44.7%) had a diagnosis of ATH. Abdominal bloating scores were higher in ATH patients compared to the lactase persistent patients (P=0.014). The remaining dyspeptic symptom scores were not significantly different between the two groups. The study results demonstrate an association between ATH and bloating in patients with functional dyspepsia.

BACKGROUND Hepatitis B virus (HBV) infection is a major public health problem in Brazil. Several risk factors are involved in HBV infection and their identification by a rational and essential approach is required to prevent the transmission of this infection in Brazil. OBJECTIVES To evaluate risk factors associated with HBV infection in South Brazil. METHODS A total of 260 patients with HBV and 260 controls from Caxias do Sul (state of Rio Grande do Sul, Brazil) participated in this study. All participants were given a standard questionnaire to yield the sociodemographic information and to identify HBV risk factors. HBV infection was detected by HBsAg test in all participants. FINDINGS HBV infection in these cases was strongly associated with history of a family member HBV-infected, mainly mother [odds ratio (OR) = 4.86; 95% confidence intervals (CI): 1.69–13.91], father (OR = 5.28; 95% CI: 1.58–17.71), and/or siblings (OR = 22.16; 95% CI: 9.39–52.25); sharing personal objects (OR = 1.40; 95% CI: 1.37–2.38); and having history of blood transfusion (OR = 2.05; 95% CI: 1.10–2.84). CONCLUSIONS HBV infection was strongly associated with having a family member infected with hepatitis B, sharing personal objects, and having history of blood transfusion.

Interferon lambda 4 (IFNL4) gene polymorphism is associated with spontaneous clearance of HCV in HIV-1 positive patients

Abstract Approximately one-third of the individuals infected with human immunodeficiency virus type 1 (HIV-1) are co-infected with hepatitis C virus (HCV). Co-infected patients have an increased risk for developing end-stage liver diseases. Variants upstream of the IFNL3 gene have been associated with spontaneous and treatment-induced clearance of HCV infection. Recently, a novel polymorphism was discovered, denoted IFNL4 ΔG > TT (rs368234815), which seems to be a better predictor of spontaneous clearance than the IFNL4 rs12979860 polymorphism. We aimed to determine the prevalence of the IFNL4 ΔG > TT variants and to evaluate the association with spontaneous clearance of HCV infection in Brazilian HIV-1 patients. The IFNL4 ΔG > TT genotypes were analyzed by polymerase chain reaction followed by restriction digestion in 138 HIV-1 positive patients who had an anti-HCV positive result. Spontaneous clearance of HCV was observed in 34 individuals (24.6%). IFNL4 genotype distribution was significantly different between individuals who had spontaneous clearance and chronic HCV patients (p=0.002). The probability of spontaneous clearance of HCV infection for patients with the IFNL4 TT/TT genotype was 3.6 times higher than for patients carrying the IFNL4 ΔG allele (OR=3.63, 95% CI:1.51-8.89, p=0.001). The IFNL4 ΔG > TT polymorphism seems to be better than IFNL4 rs12979860 to predict spontaneous clearance of the HCV in Brazilian HIV-1 positive patients.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.
