OBJETIVO: O objetivo deste estudo é avaliar, ao ecodopplercardiograma (ECO), o funcionamento do aparelho valvar mitral, em pacientes submetidos ao implante de cordas padronizadas de pericárdio bovino (CP de PB) para substituição de cordas tendíneas rotas ou alongadas com grau importante de afilamento. MÉTODO: Foram implantadas CP de PB em 23 pacientes portadores de insuficiência mitral por ruptura das cordas tendíneas ou cordas alongadas com afilamento importante. A idade variou de 23 a 85 anos (média de 62 anos). A causa mais freqüente foi a degeneração fibroelástica em 20 (87,0%) pacientes. As CP de PB foram confeccionadas em conjunto, unidas em suas extremidades por duas hastes reforçadas com poliéster formando um monobloco. As CP de PB medem dois mm de largura e distam entre si, paralelamente, por três mm. Cada conjunto CP de PB possui um medidor correspondente, variando seu comprimento entre 20 a 35 mm. Em 17 (73,9%) pacientes foram implantadas as CP de PB na cúspide posterior e, em seis (26,1%), na cúspide anterior. Todos os pacientes foram avaliados no pós-operatório pelo ECO, com tempo médio de seguimento de até seis meses. RESULTADOS: O ECO no pós-operatório demonstrou ausência de refluxo em 11 (47,8%) pacientes, refluxo discreto em oito (34,8%) e refluxo discreto/moderado em três (13,0%). A abertura e mobilidade da valva mitral eram normais nos 22 pacientes sobreviventes. CONCLUSÃO: O ECO demonstrou boa funcionalidade do aparelho valvar mitral nos pacientes submetidos ao implante das CP de PB para substituição de cordas tendíneas rotas ou alongadas e afiladas com adequada coaptação das cúspides.

OBJECTIVE: The objective of this study was to evaluate, using echocardiography, the functioning of the mitral valve apparatus in patients submitted to standardized bovine pericardium chordae implantation to substitute ruptured chordae tendineae or elongated chordae with a significant degree of thinning. METHOD: Standardized bovine pericardium chordae were implanted in 23 patients with mitral valve insufficiency due to ruptured or elongated chordae with significant thinning. The ages of the patients varied from 23 to 84 years old (mean 62 years old). The most common cause was fibroelastic degeneration affecting 20 (87.0%) patients. The standardized bovine pericardium chordae were manufactured in sets connected at both ends by two polyester-reinforced rods thereby forming a single block. The bovine pericardium chordae measure 2 mm wide with 3 mm between the chordae. The sets of bovine pericardium chordae are produced in lengths varying from 20 to 35 mm. In 17 (73.9%) patients bovine pericardium chordae were implanted in the posterior cusp and in 6 (26.1%) in the anterior cusp. All the patients were evaluated in the postoperative period by echocardiography after a mean follow-up of six months. RESULTS: The echocardiography in the postoperative period demonstrated an absence of reflux in 11 (47.8%) patients, slight reflux in 8 (34.8%) and slight to moderate reflux in 3 (13.0%). The opening and mobility of the mitral valve was normal in the 22 surviving patients. CONCLUSION: The echocardiography demonstrated good functioning of the mitral valve apparatus in patients submitted to the implantation of standardized bovine pericardium chordae to substitute ruptured chordae tendineae or elongated chordae with a significant degree of thinning.

A obstrução da via de saída do ventrículo direito (VSVD) tem gerado muita polêmica em torno da técnica da sua correção cirúrgica, sendo a reconstituição ainda motivo de controvérsias. Com essa finalidade, foram desenvolvidas duas próteses a partir do tronco pulmonar (TP) de porco: 1) a prótese bivalvular: poderia ser usada na correção da tétrade de Faliot associada a hipoplasía do anel pulmonar: 2) a prótese tubular valvada, possuindo a própria valva pulmonar: poderia ser empregada na correção de malformações com descontinuidade entre o VD e TP. Estes dois tipos de próteses foram testados em modelo experimental. Seis ovelhas foram submetidas a implante de prótese bivalvular, com o auxílio da circulação extracorpórea (CEC), após ampla ressecção do infundíbulo pulmonar, incluindo duas válvulas da valva pulmonar, procurando-se, com isto, imitar a reconstituição empregada no Fallot. O implante da prótese tubular valvada foi realizado em 12 ovelhas, sem o auxílio da CEC, mediante pinçamento tangencial do infundíbulo e TP, permitindo o desvio do fluxo sangüíneo através do conduto, após ligadura do TP. A prótese bivalvular implantada foi avaliada mediante parâmetros hemodinâmicos e ecocardiográficos na fase intra-operatória, conferindo desempenho satisfatório (insuficiência pulmonar discreta ou ausente e gradientes VD-TP menores de 10 mm Hg). A seguir, os animais foram sacrificados. O desempenho da prótese tubular valvada foi avaliada na fase intra-operatória com medidas hemodinâmicas, mostrando gradientes acima de 10 mmHg em apenas 3 casos. Sete ovelhas tiveram controle ecocardiográfíco com 99 a 135 dias de evolução, registrando gradientes de 9,85 mmHg a 49 mmHg (média 19,7). Quatro casos foram submetidos a estudo hemodinâmico no 6º mês de evolução, registrando discreto aumento do gradiente (média 22,3); a seguir os animais foram sacrificados e encaminhados para estudo anatomopatológico. A aplicação clínica da prótese bivalvular foi realizada em 3 pacientes portadores de t. de Fallot associada a hipoplasía do anel pulmonar (2 casos) e agenesia da valva pulmonar (1 caso), com idades de 16, 2 e 7 anos. Após evolução de 3 a 10 meses, os gradientes variaram entre 10 mmHg e 20 mmHg e discreta insuficiência pulmonar valvar ao estudo ecodopplercardiográfico. A prótese tubular valvar foi implantada em 2 pacientes portadores de atresia pulmonar associada a comunicação interventricular (CIV) e outro a transposição corrigida das grandes artérias (TCGA) associada a GIV e estenose subpulmonar, com idades de 10 e 6 anos, respectivamente. Após evolução de 5 a 12 meses, foram detectados suficiência da valva pulmonar, gradientes entre 15 mmHg e 18 mmHg, sem sinais de calcificação. Apesar de se considerar aceitáveis os resultados desta experiência, a ampliação das indicações deverá ser feita com cautela, até o melhor conhecimento da resistência da prótese a calcificação, infecção, obstrução e rotura.

Obstruction of the right ventricle outlet tract (RVOT) has been the object of arguments regarding its surgical correction, while there are different criteria for reconstruction. Thus two kinds of prostheses were developed from the pig pulmonary trunk (PT). 1) One with two valves of the pulmonary valve (PV), named bivalvular graft, and could be used for correction of Fallot with pulmonary ring hypoplasia. 2) The other, with a tubular form, containing the pig PV itself and named valved conduit, could be used in RVOT reconstruction for patients with pulmonary atresia (PA). These prostheses were tested in an experimental model: implant of the bivalvular graft was performed in 16 sheep with the aid of extracorporeal circulation (ECC). The surgical technique consisted of resection of the two valves of the PV and of the anterior wall of the infundibulum; this condition was similar to Fallot correction. Implant of the valvular conduit was carried out in 12 sheep, without ECC, by direct clamping of the RV infundibulum. The PT was then ligated, deviating blood flow through the conduit. Intraoperative hemodynamics and echodoppler evaluation of the bivalvular graft showed good PV competence and only 1 case of gradient higher than 10 mmHg. Because of frequent pulmonary hemorrhage followed by death due to ECC, this group was not evaluated in the postoperative period. Intraoperative hemodynamic measurements of the valvular conduit were carried out showing good valvular competence and gradient higher than 10 mmHg in 3 cases. There was no operative mortality. Seven sheeps were followed-up during the late postoperative period with a control echodoppler on days 99 and 135 of follow-up. Gradients ranged from 9.85 to 49 mmHg (mean = 19.7). Four sheep underwent hemodynamic studies at six months of follow-up. There was a slight increase in the gradient between RV and PT (mean = 22.3 mmHg), no gradient being observed inside the conduit. Anatomopathological evaluation was performed. Clinical applications of the bivalvular prostheses was performed in 3 patients with tetralogy of Fallot and hypoplasia of the pulmonary ring (2 cases) and PV absent (1 case); they were 16,2 and 7 years old. The postoperative echodopplercardiogram showed gradients between 10 to 20 mmHg and mild pulmonary valve insufficiency. Clinical application of the valvular conduit was performed in 2 patients: 1 with pulmonary atresia and ventricular septal defect (VSD), the other with corrected transposition of the great arteries, VSD and subpulmonary stenosis (10 and 6 years old, respectively). The postoperative echodopplercardiogram showed gradients between 15 to 18 mmHg. Although the results of the experiment may be considered acceptable, reconstruction of the RVOT with the newly developed prostheses, obviously requires to be tested over time to better evaluate their resistance to calcification, infection, obstruction and rupture.

Foram estudados 101 pacientes submetidos a plástica da valva mitral em seis anos, com seguimento de 100%. Entre eles, 36 eram do sexo masculino e 65 do sexo feminino, com idade variando de dois a 62 anos (M = 28 ± 16,4). Desses, 57 (56,4%) foram submetidos apenas a abordagem valvar mitral. Os demais foram submetidos a procedimentos associados, como plástica tricúspide (9,9%), revascularização do miocárdio (4,0%), entre outros. Não foi registrado óbito imediato. O índice de mortalidade tardia foi de 2% (AVC hemorrágico após cinco anos e septicemia), no primeiro ano. As complicações não fatais foram representadas pela endocardite evidenciada em dois pacientes (2%), sendo tratados e curados, e um paciente com reestenose mitral pós-plástica por reagudização da doença reumática. O estudo atuarial revelou um índice de 79,0 ± 17,7% de sobrevida, um total de 76,3 ± 17,8% de pacientes livres de complicações, 80,0 ± 17,9% de reoperações, 100,0% livres de tromboembolismo. Os resultados ecodoplercardiográficos registraram que 89% dos pacientes evoluíram com ausência de insuficiência. Dos 11% restantes, 7,4% apresentram insuficiência mitral discreta, 2,4% moderada e 2% importante. De acordo com a classificação da NYHA, os pacientes das classes III (83,8%) e IV (16,2%) passaram para as classes I (33,3%), II (60,6%), III (4,1%) e IV (2%). Os autores concluem que o anel de pericárdio flexível conforma-se perfeitamente com o anel valvar, não produz hemólise e se endoteliza completamente a médio prazo.

A hundred-and-one patients were studied in six years, with 100% of follow-up. Among them, 36 were male and 65 female, with an age range of two to 62 years (mean 28 ± 16.4%). Fifty seven of them (56.4%) underwent just a mitral surgery, the others and other associated procedures, as tricuspid plastic (9.9%), coronary artery revascularization (4.0%), among others. Hospital mortality was not registered. The late mortality rate was 2% for AVC hemorrhagic after five years, and septicemia during the first year. The non-fatal complications were represented by endocarditis in two patients (2%), treated and cured; and a mitral restenosis after plastic. The actuarial study revealed a survival rate of 79.0 ± 17.7% and rates of without complications, reoperation and thromboembolism of 76.3 ± 17.8, 80.0 ± 17.9 and 100%, respectively. The echocardiography results registered 89% of the patients with evolution to absence of insufficiency from the remaining 11%, 7.4% showed discrete mitral insufficiency, 2.4% moderate and 1.2% important. Under the NYHA classification, the patients functionally went from class III (83.3%) and IV (16.2%) to class I (33.3%), II (60.65), III (4.1%) and IV (2.0%). The authors conclude that the pericardium ring is flexible, it fits perfectly in the valvar ring, does not cause hemolysis, and shows completely endotelization after a certain time.

Doenças congênitas, como a atresia pulmonar e a tétrade de Fallot, entre outras, têm sido corrigidas com o uso de condutos extracardíacos. Existe uma variedade de condutos, valvulados ou não, que possibilita o acesso do fluxo sangüíneo do ventrículo direito à circulação pulmonar. Contudo, os tubos plásticos com válvulas biológicas ou metálicas apresentam problemas de degeneração e obstrução da prótese e peeling do tubo, além do alto custo. A experiência adquirida com o pericárdio bovino tratado, mostrando-se impermeável e de fácil sutura, possibilitou a confecção de tubos valvulados que, neste estudo, foram avaliados quanto a obstrução, calcificação e dilatação. A utilização de um modelo experimental animal permitiu a avaliação periódica detalhada por ecodopplercardiografia e cateterismo durante um, três e seis meses de evolução. As peças recuperadas foram estudadas por microscopia óptica e raios X, com a análise de diversas regiões da bioprótese. O exame macroscópico mostrou o pericárdio dos tubos preservados com sinais de pseudo-endotelizaçáo, e a maioria das válvulas com calcificação moderada, mas ainda com boa função. São apresentados e discutidos a técnica operatória empregada sem circulação extracorpórea e os resultados com o seguimento de cinco animais (carneiros), durante seis meses. Os autores concluem que o enxerto tubular valvulado de pericárdio bovino mostrou bons resultados na via de saída do ventrículo direito, permitindo prever um resultado satisfatório quando empregado em seres humanos, pela conhecida relação de degeneração acelerada das próteses biológicas no modelo experimental estudado, muitas vezes maior que no homem.

Congenital diseases as pulmonary atresia, tetralogy of Fallot, among others, have been corrected using the extracardiac conduits. There is a variety of conduits valved or not that make possible the access of sanguineous flux from right ventricle to pulmonary circulation. However, the synthetic conduits with biologic or metalic valves shows prosthesis degeneration and obstruction problems and peeling of the conduit, moreover, they are very expensive. The experience with treated bovine pericardium showing it is impermeable and easy to suture, led to the production of valved conduits that in this study were evaluated for obstruction, calcification and stretching. The utilization of an animal experimental model allowed the detailed periodic evaluation by echodopplercardiography and cateterism during one, three and six months. The recuperated conduits were studied by optical microscopy and X ray, with several parts of the bioprosthesis analysed. The microscopic examination showed the conduits pericardium preserved with signs of pseudo-endotelization, and the majority of valves with moderate calcification, but with good function yet. The surgery technique without extracorporeal circulation and the results with the follow-up of five animals during six months are presented and discussed. The authors conclude that the bovine pericardium valved conduit showed good results in the right ventricule outlet, allowing to expect a satisfatory result when applied in human beings by the known relation of acelerated degeneration of the biological prostheses on the studied experimental model, several times greater than in man.
