In post disastrous situations, coordinated and integrated interventions aimed at relief and rehabilitation not only help facilitate reaching out to the affected communities in a timely fashion but also pave the way to channel scarce and valued resources towards end users in an efficient and effective manner. This article attempts to trace the origins and gradual development of 'inter-agency collaboration' and the implications thereof for disaster management strategies in Pakistan through an analysis of relief and rehabilitation interventions undertaken by the Government of Pakistan in collaboration with local and international Non-governmental Organisations (NGOs) and relief agencies in the ex post of the 2005 earthquake. Data for this study were collected through structured and semi-structured interviews from government officials, representatives of NGOs and relief agencies and ordinary women and men in the earthquake stricken localities of Balakot and Mansehra districts of Pakistan. On the heels of the 2005 earthquake, both local NGOs and faith-based organisations in concert with international NGOs and relief agencies from around the world rushed to assist Pakistan in it's rescue and relief operations at a time when the country was faced with the twin dilemma of both the non-existence of peculiar institutional arrangements for disaster management and a lack of the necessary technical and financial resources. The aftermath of the 2005 earthquake offered opportunity to the Government of Pakistan and the NGOs and relief agencies alike to transform their individual interventions into a robust and organised 'inter-agency collaboration', which was later on realised in the form of establishment of a national disaster management organisation called the 'Earthquake Reconstruction and Rehabilitation Authority (ERRA)'. The establishment of ERRA not only paved the way for avoiding duplication and wastage of resources but also ensued in reaching out to the affected communities in a timely fashion. The Pakistani case offers implications in terms of highlighting the salience of establishing 'inter-agency collaboration' in other settings.

Abstract Background: Panic disorder has long been associated with the changes in various neurotransmitters, such as Neuropeptide-S (NPS). Objective: In this study we aimed to determine whether there is a relationship between blood NPS levels and panic disorder. Methods: Twenty nine patients with panic disorder and thirty two healthy control subjects who were age and gender matched were enrolled to the study. Blood samples were taken from participants and plasma NPS levels were quantified by using an ELISA kit. Results: In the study group, median NPS blood level was 16.7 pg/mL and in the control group it was 32.5 pg/mL. There was a statistically significant difference (p = 0.021). Using receiver operating characteristics (ROC) curve, sensitivity and specificity of NPS blood level, for diagnosing panic disorder was calculated, and it was found 79.3% and 56.25% respectively (AUC:0.672, 95% CI: 0.540-0.787). Discussion: Malfunction at the NPS modulatory system in the cortical areas (which is causing excitations in brain areas, such as amygdala and hypothalamus) does not only increase anxiety symptoms and risk of panic disorder but also causes panic disorder patients to have lower plasma NPS levels than the control group. Abstract: Recently, there has been a remarkable increase in biomedical research being conducted in low and middle-income countries. This increase has brought attention to the need for high quality research ethics systems within these countries and a greater focus on research ethics training. Though most programs tend to concentrate on training individuals, less attention has focused on institutions as the target of such training. In this paper we demonstrate a rapid approach to evaluating institutional research capacity. The method adapts the Octagon Model, which evaluates institutional research ethics using eight domains: basic values and identity, organization of activities, implementation, relevance, proper skills, financing and administration, target groups, and working environment. The framework was applied to the University of Zambia in order to conduct a baseline assessment of university research ethics capacity. Internal and external assessments were conducted. The domains of working environment and proper skills scored highest, while relevance, target groups and identity scored lower. Consistent with previous work, a systems approach to evaluating institutional research development capacity can provide a rapid assessment of an institutional bioethics program. This case study reveals the strengths and weaknesses of the university’s research ethics program and provides a framework for future capacity growth.

Resumen: Recientemente, ha habido un notable aumento en la investigación biomédica en países de ingresos bajos y medianos. Este aumento ha llamado la atención sobre la necesidad de sistemas éticos de investigación de alta calidad en estos países y un mayor enfoque en la formación en ética de la investigación. Aunque la mayoría de los programas tienden a concentrarse en la formación de los individuos, menos atención se ha centrado en las instituciones como objetivo de dicha formación. En este trabajo se demuestra un enfoque rápido para evaluar la capacidad de investigación institucional. El método adapta el modelo Octagon, que evalúa la ética institucional de la investigación utilizando ocho dominios: valores básicos e identidad, organización de actividades, implementación, relevancia, habilidades adecuadas, financiamiento y administración, grupos objetivo y ambiente de trabajo. El marco se aplicó a la Universidad de Zambia, con el fin de realizar una evaluación inicial de la capacidad de ética de la investigación universitaria. Se realizaron evaluaciones internas y externas. Los ámbitos del entorno de trabajo y de las competencias apropiadas obtuvieron el puntaje más alto, mientras que la relevancia, los grupos objetivo y la identidad obtuvieron calificaciones más bajas. De acuerdo con trabajos previos, un enfoque sistémico para evaluar la capacidad de desarrollo institucional de la investigación puede proporcionar una evaluación rápida de un programa institucional de bioética. Este estudio de caso revela las fortalezas y debilidades del programa de ética de la investigación de la universidad y proporciona un marco para el futuro crecimiento de la capacidad.

Este aumento tem chamado a atenção para a necessidade de sistemas éticos de pesquisa de alta qualidade nesses países e um maior foco na formação em ética em pesquisa. Embora a maioria dos programas tende a se concentrar na formação dos indivíduos, menos atenção centrou-se em instituições como objetivo dessa formação. Este trabalho demonstra uma aproximação rápida para avaliar a capacidade de pesquisa institucional. O método adapta o modelo Octagon, que avalia a ética institucional de pesquisa usando oito domínios: valores básicos e identidade, organização das atividades, implementação, pertinência, competências adequadas, financiamento e administração, os grupos-alvo e ambiente de trabalho. O quadro foi aplicado para a Universidade da Zâmbia, a fim de fazer uma primeira avaliação da capacidade de ética em pesquisa universitária. Foram realizadas avaliações internas e externas. Os campos do ambiente trabalho e competências adequadas, obtiveram a maior pontuação, enquanto a relevância, grupos-alvo e identidade obtiveram qualificações inferiores. De acordo com trabalhos anteriores, uma abordagem sistêmica para avaliar a capacidade dedesenvolvimento institucional de pesquisa pode fornecer uma avaliação rápida de um programa institucional de bioética. Este estudo de caso revela os pontos fortes e pontos fracos do programa de ética em pesquisa da Universidade e fornece uma estrutura para o crescimento futuro da capacidade.

To change the recalcitrant nature of the lignocellulosic material for maximum hydrolysis yield, a comprehensive study was done by using sulphuric acid as an exclusive catalyst for the pretreatment process. The enzymatic digestibility of the biomass [Water Hyacinth: Eichhornia crassipes] after pretreatment was determined by measuring the hydrolysis yield of the pretreated material obtained from twenty four different pretreatment conditions. These included different concentrations of sulphuric acid (0.0, 1.0, 2.0 and 3.0%), at two different temperatures (108 and 121 ºC) for different residence times (1.0, 2.0 and 3.0h).The highest reducing sugar yield (36.65 g/L) from enzymatic hydrolysis was obtained when plant material was pretreated at 121 ºC for 1.0 h residence time using 3.0% (v/v) sulphuric acid and at 1:10 (w/v) solid to liquid ratio. The total reducing sugars obtained from the two-stage process (pretreatment + enzymatic hydrolysis) was 69.6g/L. The resulting sugars were fermented into ethanol by using Saccharomyces cerevisiae. The ethanol yield from the enzymatic hydrolyzate was 95.2% of the theoretical yield (0.51g/g glucose), as determined by GS-MS, and nearly 100% since no reducing sugars were detected in the fermenting media by TLC and DNS analysis.

Brazil, the Russian Federation, India, China and South Africa – the countries known as BRICS – are currently undergoing a deep epidemiological transition that is mainly driven by rapid economic growth and technological change. The changes being observed in the distribution of the burden of diseases and injuries – such as recent increases in the incidence of road traffic injuries – are matters of concern. BRICS may need stronger institutional capacity to address such changes in a timely way. In this paper, we present data on road traffic injuries in BRICS and illustrate the enormous challenge that these countries currently face in reducing the incidence of such injuries. There is an urgent need to improve road safety indicators in every country constituting BRICS. It is imperative for BRICS to invest in system-wide road safety interventions and reduce the mortality and morbidity from road traffic injuries.

Resumen Brasil, la Federación de Rusia, India, China y Sudáfrica, los países conocidos como BRICS, se encuentran en la actualidad en una transición epidemiológica profunda impulsada principalmente por el rápido crecimiento económico y el cambio tecnológico. Los cambios que se observan en la distribución de la carga de enfermedades y lesiones, como los aumentos recientes de la incidencia de los accidentes de tráfico, son motivo de preocupación. Es posible que los BRICS necesiten una capacidad institucional más fuerte para hacer frente a esos cambios de manera oportuna. En el presente artículo, presentamos datos sobre lesiones en accidentes de tráfico de los BRICS e ilustramos el enorme desafío al que dichos países se enfrentan actualmente para reducir la incidencia de las mismas. Hay una necesidad urgente de mejorar los indicadores de seguridad vial en todos los países BRICS. Es imperativo que estos países inviertan en intervenciones de seguridad vial en todo el sistema y reduzcan la mortalidad y morbilidad por accidentes de tráfico.

Résumé Le Brésil, la Fédération de Russie, l'Inde, la Chine et l'Afrique du Sud – les pays connus sous le nom de BRICS – connaissent actuellement une transition épidémiologique profonde qui s'explique principalement par la rapidité de la croissance économique et de l'évolution technologique. Les changements qui sont observés dans la distribution de la charge des maladies et des blessures, comme les hausses récentes de l’incidence des accidents de la route, suscitent des inquiétudes. Le groupe BRICS peut avoir besoin de capacités institutionnelles renforcées pour répondre rapidement à ces changements. Dans cet article, nous présentons les données sur les accidents de la route dans le groupe BRICS et nous illustrons l'énorme défi que doivent actuellement relever ces pays dans la diminution de l'incidence de ces accidents. Il est urgent d'améliorer les indicateurs de la sécurité routière dans chaque pays constituant le groupe BRICS. Il est impératif que le groupe BRICS investisse dans des interventions de sécurité routière dans l'ensemble du système et qu'il réduise la mortalité et la morbidité dues aux accidents de la route.

Road traffic injury (RTI) is the leading cause of death in persons aged 10-24 worldwide and accounts for about 15% of all male deaths. The burden of RTI is unevenly distributed amongst countries with over eighty-fold differences between the highest and lowest death rates. Thus the unequal risk of RTI occurring in the developing world, due to many reasons, including but not limited to rapid motorization and poor infrastructure, is a major global challenge. This editorial highlights a number of key issues that must inform programs designed to prevent RTI in the developing world, where the epidemic is all the more insidious. Firstly, road safety is a development issue; secondly, road traffic injury is a major health issue; thirdly, road traffic injuries can be prevented by the implementation of scientific measures; fourthly, pre-hospital and hospital emergency care is needed; and fifthly, research on RTI is neglected in low-income and middle-income countries. The repercussion of such progress to Peru is also discussed.

Los traumatismos causados por el tránsito son la principal causa de muerte en personas de 10-24 años a nivel mundial y representan alrededor del 15% de todas las muertes en varones. La carga de enfermedad de los traumatismos causados por el tránsito está distribuida de manera desigual entre los países pues la tasa de mortalidad más alta es ochenta veces superior a las más baja. Existe una clara desigualdad en el riesgo de ocurrencia de traumatismos causados por el tránsito, siendo notoriamente mayor en los países en desarrollo. Esta desigualdad se constituye como un reto mundial importante y se debe, aunque no sean los únicos factores, a muchas razones, incluyendo la rápida motorización y la pobre infraestructura. Este artículo hace énfasis en varios aspectos fundamentales cuya finalidad es informar a los programas diseñados para prevenir los traumatismos causados por el tránsito en los países en desarrollo, donde esta situación está más extendida. En primer lugar, la seguridad vial es un tema de desarrollo; en segundo lugar, los traumatismos causados por el tránsito constituyen un problema importante para la salud; en tercer lugar, los traumatismos causados por el tránsito pueden ser prevenidos mediante la implementación de medidas científicas adecuadas; en cuarto lugar, es necesaria la atención de emergencias hospitalarias y prehospitalarias; y, finalmente, la investigación en los traumatismos causados por el tránsito está relegada en los países de ingresos bajos y medios. OBJECTIVE: To determine the frequency and nature of childhood injuries and to explore the risk factors for such injuries in low-income countries by using emergency department (ED) surveillance data. METHODS: This pilot study represents the initial phase of a multi-country global childhood unintentional injury surveillance (GCUIS) project and was based on a sequential sample of children < 11 years of age of either gender who presented to selected EDs in Bangladesh, Colombia, Egypt and Pakistan over a 3-4 month period, which varied for each site, in 2007. FINDINGS: Of 1559 injured children across all sites, 1010 (65%) were male; 941 (60%) were aged > 5 years, 32 (2%) were < 1 year old. Injuries were especially frequent (34%) during the morning hours. They occurred in and around the home in 56% of the cases, outside while children played in 63% and during trips in 11%. Of all the injuries observed, 913 (56%) involved falls; 350 (22%), road traffic injuries; 210 (13%), burns; 66 (4%), poisoning; and 20 (1%), near drowning or drowning. Falls occurred most often from stairs or ladders; road traffic injuries most often involved pedestrians; the majority of burns were from hot liquids; poisonings typically involved medicines, and most drowning occurred in the home. The mean injury severity score was highest for near drowning or drowning (11), followed closely by road traffic injuries (10). There were 6 deaths, of which 2 resulted from drowning, 2 from falls and 2 from road traffic injuries. CONCLUSION: Hospitals in low-income countries bear a substantial burden of childhood injuries, and systematic surveillance is required to identify the epidemiological distribution of such injuries and understand their risk factors. Methodological standardization for surveillance across countries makes it possible to draw international comparisons and identify common issues.

MÉTODOS: Este estudio piloto representa la fase inicial de un proyecto plurinacional de vigilancia mundial de las lesiones infantiles no intencionales y se basó en una muestra secuencial de menores de 11 años, de ambos sexos, que se presentaron en 2007 en DU seleccionados de Bangladesh, Colombia, Egipto y Pakistán a lo largo de un periodo de 3 a 4 meses, variable según el centro. RESULTADOS: De 1559 niños lesionados observados en todos los centros, 1010 (65%) eran varones; 941 (60%) tenían³ 5 años, y 32 (2%) < 1 año. Las lesiones fueron especialmente frecuentes (34%) por la mañana. En el 56% de los casos se produjeron dentro de casa o en sus alrededores, en el 63% fuera, mientras jugaban, y en el 11% en viaje. De todas las lesiones observadas, 913 (56%) consistieron en caídas; 350 (22%) en lesiones causadas por el tráfico; 210 (13%) en quemaduras; 66 (4%) en intoxicaciones, y 20 (1%) en ahogamiento o casi ahogamiento. Las caídas más frecuentes se produjeron en escaleras; en el caso de las lesiones causadas por el tráfico, la mayoría de las víctimas fueron peatones; la mayoría de las quemaduras se produjeron con líquidos calientes; las intoxicaciones más frecuentes se debieron a medicamentos, y la mayoría de los ahogamientos se produjeron en casa. Las lesiones con mayores puntuaciones de gravedad fueron los ahogamientos o casi ahogamientos (11), seguidos de cerca por las lesiones causadas por el tráfico (10). Hubo 6 casos mortales: 2 por ahogamiento, 2 por caídas y 2 por lesiones causadas por el tráfico. CONCLUSIÓN: Los hospitales de los países de bajos ingresos tienen una carga considerable de lesiones infantiles. Es necesaria una vigilancia sistemática para identificar la distribución epidemiológica de esas lesiones y comprender sus factores de riesgo. La normalización metodológica de la vigilancia en los diferentes países permite establecer comparaciones internacionales e identificar problemas comunes.

Les chutes impliquaient le plus souvent des escaliers ou des échelles et les accidents de la circulation des piétons, la majorité des brûlures étaient dues à des liquides chauds, les empoisonnements faisaient intervenir habituellement des médicaments et la plupart des noyades se produisaient au domicile. Le score de gravité moyen des traumatismes était maximal pour les noyades et les presque-noyades (11), ce score étant suivi de près par celui des accidents de la circulation (10). L'étude a recensé 6 décès, dont 2 résultant d'une noyade, 2 d'une chute et 2 d'un accident de la circulation. CONCLUSION: Les hôpitaux des pays à faible revenu font face à une charge substantielle de traumatismes infantiles et une surveillance systématique est nécessaire pour déterminer la distribution épidémiologique de ces traumatismes et connaître les facteurs de risque associés. La standardisation méthodologique de la surveillance entre les pays permet de dresser des comparaisons internationales et d'identifier des problèmes communs.

Scientific progress is a significant basis for change in public-health policy and practice, but the field also invests in value-laden concepts and responds daily to sociopolitical, cultural and evaluative concerns. The concepts that drive much of public-health practice are shaped by the collective and individual mores that define social systems. This paper seeks to describe the ethics processes in play when public-health mechanisms are established in low- and middle-income countries, by focusing on two cases where ethics played a crucial role in producing positive institutional change in public-health policy. First, we introduce an overview of the relationship between ethics and public health; second, we provide a conceptual framework for the ethical analysis of health system events, noting how this approach might enhance the power of existing frameworks; and third, we demonstrate the interplay of these frameworks through the analysis of a programme to enhance road safety in Malaysia and an initiative to establish a national ethics committee in Pakistan. We conclude that, while ethics are gradually being integrated into public-health policy decisions in many developing health systems, ethical analysis is often implicit and undervalued. This paper highlights the need to analyse public-health decision-making from an ethical perspective.

El progreso científico es un motor importante de la evolución de las políticas y prácticas de salud pública, pero en ese terreno se aplican también diversos valores y hay que responder diariamente a problemas sociopolíticos, culturales y evaluativos. Las ideas que orientan gran parte de las prácticas de salud pública se ven conformadas por costumbres colectivas e individuales que definen los sistemas sociales. La finalidad de este artículo es describir los procedimientos éticos que entran en juego cuando se establecen mecanismos de salud pública en los países de ingresos bajos y medios, centrando la atención en dos casos en los que la ética contribuyó de forma decisiva a propiciar cambios institucionales positivos en las políticas de salud pública. En primer lugar presentamos una panorámica de las relaciones entre ética y salud pública; a continuación, ofrecemos un marco conceptual para el análisis ético de eventos en los sistemas sanitarios, indicando cómo podría este enfoque reforzar las posibilidades de los marcos existentes; y, en tercer lugar, demostramos la interrelación entre esos marcos analizando un programa de mejora de la seguridad vial en Malasia y una iniciativa de creación de un comité nacional de ética en el Pakistán. Llegamos a la conclusión de que, si bien las decisiones de política en materia de salud pública están incorporando gradualmente principios éticos en muchos sistemas de salud en desarrollo, con frecuencia los análisis éticos se realizan de forma implícita y están infravalorados. En este trabajo se subraya la necesidad de analizar la adopción de decisiones de salud pública desde una perspectiva ética.

Les progrès scientifiques sont un préalable important à l'évolution des politiques et des pratiques dans le domaine de la santé publique, mais sur le terrain, les individus investissent aussi dans des concepts chargés de valeur et répondent quotidiennement à des préoccupations sociopolitiques, culturelles et évaluatives. Les concepts qui influent le plus sur les pratiques de santé publique sont ceux façonnés par les mœurs collectives et individuelles définissant les systèmes sociaux. Le présent article s'efforce de décrire les processus éthiques en jeu lors de la mise en place de mécanismes de santé publique dans les pays à revenu faible et moyen, en se concentrant sur deux cas dans lesquels l'éthique a influé de manière cruciale sur l'apparition d'une évolution institutionnelle positive de la politique sanitaire. Nous commençons par donner une présentation générale de la relation entre éthique et santé publique ; en second lieu, nous apportons un cadre conceptuel pour l'analyse éthique des événements touchant les systèmes de santé, en notant dans quelle mesure cette approche peut renforcer le pouvoir des cadres existants, et enfin, nous démontrons les interactions entre ces cadres à travers l'analyse d'un programme d'amélioration de la sécurité routière en Malaisie et d'une initiative pour établir un comité national d'éthique au Pakistan. Nous parvenons à la conclusion que si l'éthique est intégrée progressivement aux décisions de santé publique dans nombre de systèmes de santé en développement, l'analyse éthique est souvent implicite et sous-évaluée Cet article attire l'attention sur la nécessité d'analyser les décisions de santé publique sous un angle éthique.
